    • AIM: To investigate inflammatory bowel disease (IBD) register-based subtype classifications over a patient's disease course and over time.METHODS: We examined International Classification of Diseases coding in patients with ≥2 IBD diagnostic listings in the National Patient Register 2002-2014 (n = 44,302).RESULTS: 18% of the patients changed diagnosis (17% of adults, 29% of children) during a median follow-up of 3.8 years. Of visits with diagnoses of Crohn's disease (CD) or ulcerative colitis (UC), 97% were followed by the same diagnosis, whereas 67% of visits with diagnosis IBD-unclassified (IBD-U) were followed by another IBD-U diagnosis. Patients with any diagnostic change changed mostly once (47%) or twice (31%), 39% from UC to CD, 33% from CD to UC and 30% to or from IBD-U. Using a classification algorithm based on the first two diagnoses ('incident classification'), suited for prospective cohort studies, the proportion adult patients with CD, UC, and IBD-U 2002-2014 were 29%, 62%, and 10% (43%, 45%, and 12% in children). A classification model incorporating additional information from surgeries and giving weight to the last 5 years of visits ('prevalent classification'), suited for description of a study population at end of follow-up, classified 31% of adult cases as CD, 58% as UC and 11% as IBD-U (44%, 38%, and 18% in children).CONCLUSIONS: IBD subtype changed in 18% during follow-up. The proportion with CD increased and UC decreased from definition at start to end of follow-up. IBD-U was more common in children.

    • Digital signatures are important in order to ensure the integrity and authenticity of information communicated over the Internet involving different stakeholders within and beyond the borders of different nations. The topic has gained increased interest in the European context and there is legislation and project initiatives aiming to facilitate use and standardisation of digital signatures. Open standards and open source implementations of open standards are important means for the interoperability and long-term maintenance of software systems implementing digital signatures. In this paper we report from a study aiming to establish the availability and effectiveness of software provided under an open source license for digital signing and validation of PDF documents. Specifically, we characterise the use of digital signatures in Swedish Governmental agencies, report on the interoperability of open source and proprietary licensed software for digital signatures in PDF documents, and establish the effectiveness of software provided under an open source license for validation of digital signatures in PDF documents.

  • Validating inflammatory bowel disease (IBD) in the Swedish National Patient Register and the Swedish Quality Register for IBD (SWIBREG)

    • Background: Both the Swedish National Patient Register (NPR) and the Swedish Quality Register for inflammatory bowel disease (IBD, SWIBREG) are important sources of research data and information. However, the validity of a diagnosis of IBD in these registers is unknown. Methods: Medical charts of 129 randomly selected patients from the NPR and 165 patients registered both in SWIBREG and the NPR were reviewed. Patients were classified according to standardized criteria for ulcerative colitis (UC), Crohn's disease (CD), or IBD unclassified (IBD-U). Positive predictive values (PPVs) for UC, CD, IBD-U (only SWIBREG), or having any form of IBD were then calculated. Results: For cases with >= 2 diagnoses of IBD in the NPR (hospitalizations or non-primary care outpatient visits), the PPV was 93% (95% CI: 87-97) for any IBD, 79% (66-88) for UC and 72% (60-82) for CD. In UC patients with >= 2 UC diagnoses but never a CD diagnosis, the PPV increased to 90% (77-97). The PPV for CD in patients with >= 2 CD diagnoses but never a UC diagnosis was 81% (67-91)). Combining data from SWIBREG (>= 1 record) and the NPR (>= 1 record), the PPV was 99% for any IBD (97-100), 96% (89-99) for UC, and 90% (82-96) for CD. Conclusion: The validity of the UC, CD, and IBD diagnoses is high in the NPR but even higher when cases were identified both in SWIBREG and the NPR. These results underline the need for a well-functioning Swedish Quality Register for IBD as a complement to the NPR.

  • Work stress, worries, and pain interact synergistically with modelled traffic noise on cross-sectional associations with self-reported sleep problems

    • To examine the risk of sleep problems associated with work stress (job strain, job demands, and decision authority), worries and pain and to investigate the synergistic interaction between these factors and traffic noise. Sleep problems and predictor variables were assessed in a cross-sectional public health survey with 12,093 respondents. Traffic noise levels were assessed using modelled A-weighted energy equivalent traffic sound levels at the residence. The risk of sleep problems was modelled using multiple logistic regression analysis. With regard to sleep problems not attributed to any external source (general sleep problems), independent main effects were found for traffic noise (women), decision authority (women), job strain, job demands, suffering from pain or other afflictions, worries about losing the job, experiencing bullying at work, having troubles paying the bills, and having a sick, disabled, or old relative to take care of (women). Significant synergistic effects were found for traffic noise and experiencing bullying at work in women. With regard to sleep problems attributed to traffic noise, strong synergistic interactions were found between traffic noise and, respectively, job demands (men), having pain or other afflictions, taking care of a sick, old, or disabled relative, and having troubles paying the bills. Main effects were found for worries about losing the job, experiencing bullying at work, job strain (men), and decision authority (men). Synergistic interactions could potentially contribute with 10-20% of the sleep problems attributed to traffic noise in the population. Work stress, pain, and different worries were independently associated with general sleep problems and showed in general no synergistic interaction with traffic noise. In contrast, synergistic effects between traffic noise and psychological factors were found with regard to sleep problems attributed to traffic noise. The synergy may contribute significantly to sleep problems attributed to traffic noise in the population.

    • Objectives We examined to what degree annoyance ratings to noise, air pollution and other common environmental factors in the home environment could be considered to mirror personality disposition in terms of habitual anxiety level and, when appropriate, objectively modeled noise and nitrogen emission (NOx). Methods A trait anxiety scale was introduced in a cross-sectional public health survey with 2,856 respondents. Of these, 705 had self-reported asthma and the rest constituted gender-matched referents. Annoyance to ten specific factors in the residential environment, mainly focusing on source-specific noise and air pollution, was assessed on a six-point likert scale. A-weighted energy equivalent continuous sound pressure level during a full day (24 h; L (Aeq,24)) as well as annual average NOx levels (mu g/m(3)) at the residential address were modeled with high resolution, using a road data base and a detailed emission data base for NOx. Results The two most prevalent complaints were annoyance to traffic noise and sounds from neighbors, which was reported by about 8% of the participants. Unadjusted logistic regression analyses using the continuous trait anxiety score as a predictor showed positive associations with ratings of annoyance from total traffic noise, sounds from neighbors, sound from ventilation, exhaust fumes from traffic, sounds from other installations, and vibrations from traffic (ORs between 1.37 and 2.14). Modeled noise and NOx exposure were positively related with annoyance to traffic noise and exhaust fumes, respectively. Adjustment of the trait anxiety scores for other individual characteristics and potential determinants did not change the overall pattern of results. Conclusion Trait anxiety scores were often mirrored in ratings of annoyance, which suggests caution when using annoyance reports either as a surrogate measure for environmental exposure on the individual-level in epidemiologic studies or when studying the moderating effects of annoyance on health outcomes.

    • Abstract in Undetermined Background: Measured or modeled levels of outdoor air pollution are being used as proxies for individual exposure in a growing number of epidemiological studies. We studied the accuracy of such approaches, in comparison with measured individual levels, and also combined modeled levels for each subject's workplace with the levels at their residence to investigate the influence of living and working in different places on individual exposure levels. Methods: A GIS-based dispersion model and an emissions database were used to model concentrations of NO2 at the subject's residence. Modeled levels were then compared with measured levels of NO2. Personal exposure was also modeled based on levels of NO2 at the subject's residence in combination with levels of NO2 at their workplace during working hours. Results: There was a good agreement between measured facade levels and modeled residential NO2 levels (r(s) = 0.8, p > 0.001); however, the agreement between measured and modeled outdoor levels and measured personal exposure was poor with overestimations at low levels and underestimation at high levels (r(s) = 0.5, p > 0.001 and r(s) = 0.4, p > 0.001) even when compensating for workplace location (r(s) = 0.4, p > 0.001). Conclusion: Modeling residential levels of NO2 proved to be a useful method of estimating facade concentrations. However, the agreement between outdoor levels (both modeled and measured) and personal exposure was, although significant, rather poor even when compensating for workplace location. These results indicate that personal exposure cannot be fully approximated by outdoor levels and that differences in personal activity patterns or household characteristics should be carefully considered when conducting exposure studies. This is an important finding that may help to correct substantial bias in epidemiological studies.

    • Airspace dimension assessment with nanoparticles (AiDA) is a novel method to measure distal airspace radius non-invasively. In this study, AiDA radii were measured in 618 individuals from the population-based Swedish CArdiopulmonary BioImaging Study, SCAPIS. Subjects with emphysema detected by computed tomography were compared to non-emphysematous subjects. The 47 individuals with mainly mild-to-moderate visually detected emphysema had significantly larger AiDA radii, compared with non-emphysematous subjects (326±48 μm vs 291±36 μm); OR for emphysema per 10 μm: 1.22 (1.13-1.30, p<0.0001). Emphysema according to CT densitometry was similarly associated with larger radii compared with non-emphysematous CT examinations (316±41 μm vs 291 μm±26 μm); OR per 10 μm: 1.16 (1.08-1.24, p<0.0001). The results are in line with comparable studies. The results show that AiDA is a potential biomarker for emphysema in individuals in the general population.

Different from other common approaches used for the estimation of periodicities of sequences of arbitrary, finite, symbol sets, that often map the symbolic sequence to a numerical representation, we here exploit a likelihood-based formulation in a sparse modeling framework to represent the periodic behavior of the sequence. The resulting criterion includes a restriction on the cardinality of the solution; two approximate solutions are suggested-one greedy and one using an iterative convex relaxation strategy to ease the cardinality restriction. The performance of the proposed methods are illustrated using both simulated and real DNA data, showing a notable performance gain as compared to other common estimators.

    • IntroductionPost-operative infections obtained from open-wound surgeries constitute an unnecessary load on both healthcare and affected patients. It is well established that increased air cleanliness reduces the number of post-operative infections. Therefore, the ventilation system is important in order to reduce the number of infectious particles in the air during surgery. Ventilation with high airflow, as in operating rooms, consumes a high amount of energy and it is thus desirable to find energy efficient solutions. ObjectivesThe purpose of this work was to evaluate air quality, energy efficiency and working environment comfort for three different ventilation techniques in operating rooms. MethodThe newly developed ventilation system temperature controlled airflow (TcAF) was compared with the conventionally used turbulent mixed airflow (TMA) and laminar airflow (LAF). In total, 750 air sample measurements were performed during 45 orthopaedic operations: 15 for each type of ventilation system [1]. The concentration of colony forming units (CFU)/m3 was measured at three locations in the rooms: close to the wound (<0.5 m), at the instrument table and peripherally in the room. The working environment comfort was evaluated in a questionnaire.ResultsOur study shows that both LAF and TcAF maintains CFU concentrations in the air during ongoing surgery significantly below 10 CFU/m3 at the wound and at the instrument table, and for TcAF also in the periphery of the room, see Figure 1. The median CFU concentration in TMA was at or above 10 CFU/m3 at all locations. TcAF used less than half the airflow to that of LAF, resulting in a 28% reduction in energy consumption. The working environment comfort was perceived less noisy and having less draft in the TcAF than the LAF ventilation.SummaryBoth the LAF and TcAF ventilation maintain high air cleanliness with low CFU concentrations throughout the operation. TMA is less efficient in removing bacteria from the air close to the patient.

    • Post-operative infections after surgery can be decreased by the use of efficient ventilation with clean air. In this study, we investigated three types of operating room ventilation: turbulent mixed airflow(TMA), laminar airflow (LAF) and a new type of ventilation named temperature controlled airflow(TcAF). Measurements of airborne bacteria were made during surgery and compared with values calculated by computational fluid dynamics (CFD). The results show that LAF and TcAF are most efficient in removing bacteria around the patient. With LAF, there are large differences in bacterial loads, depending on location in the room.

    • We have exposed sixteen test subjects to particle and gas phase phthalates in the controlled chamber exposure study. Deuterium labelled phthalates were used to generate particle D4-DEHP (di(2-ethylhexyl) phthalate) and gas phase D4-DEP (diethyl phthalate) for exposures scenarios allowed studying the dermal only and combined inhalational and dermal uptake. Metabolites were measured in urine samples before and after three hours of exposure. The inhalation was the dominant route of uptake for both DEHP and DEP in this study design and exposure settings. Larger uptake of DEP compared to DEHP both via inhalation and dermal uptake was observed. Dermal uptake of DEHP was not observed in this study. Inhalational urinary excretion factors of the metabolites were found to be 0.73 for DEHP and 0.53 for DEP. This study also highlights the importance of taking into consideration the deposited dose of inhaled particles in studies of uptake of particles.

    • Phthalates are ubiquitous in indoor environments, which raises concern about their endocrine disrupting properties. However, studies of human uptake from airborne exposure are limited. We studied the inhalation uptake and dermal uptake by air-to-skin transfer with clean clothing as a barrier of two deuterium-labelled airborne phthalates: particle-phase D4-DEHP (di-(2-ethylhexyl)phthalate) and gas-phase D4-DEP (diethyl phthalate). Sixteen participants, wearing trousers and long-sleeved shirts, were under controlled conditions exposed to airborne phthalates in four exposure scenarios: dermal uptake alone, and combined inhalation+dermal uptake of both phthalates. The results showed an average uptake of D4-DEHP by inhalation of 0.0014±0.00088 (µg kg-1 bw)/(µg m-3)/h. No dermal uptake of D4-DEHP was observed during the 3 hour exposure with clean clothing. The deposited dose of D4-DEHP accounted for 26% of the total inhaled D4-DEHP mass. For D4-DEP, the average uptake by inhalation+dermal was 0.0067±0.0045 and 0.00073±0.00051 (µg kg-1 bw)/(µg m-3)/h for dermal uptake. Urinary excretion factors of metabolites after inhalation were estimated to 0.69 for D4-DEHP and 0.50 for D4-DEP. Under the described settings, the main uptake of both phthalates was through inhalation. The results demonstrate the differences in uptake of gas and particles, and highlights the importance of considering the deposited dose in particle uptake studies.

  • Factors Influencing Choice of Wooden Frames for Construction of Multi-Story Buildings in Sweden

    • Construction of buildings with wooden frames higher than two stories has been permitted in Sweden since 1994. As construction of multi-story buildings with wooden frames is relatively new, people in the construction industry are more likely to construct these buildings with concrete frames. The current research evaluates the factors influencing the choice of wooden frames for construction of multi-story buildings in Sweden. The purpose of this study is to explain which advantages and disadvantages construction companies in Sweden consider with wooden construction and to highlight the factors for why multi-story buildings are built with wood to a lesser extent than with other materials. The main goal is to investigate what factors or assumptions construction companies base their decisions on, and whether experience and competence in wooden frames for construction of multi-story buildings are considered in short supply in Sweden today. The chosen method for this research is a descriptive survey study with a qualitative and quantitative approach. The survey is based on respondents from five leading building companies in Sweden with regard to the companies’ revenue. The respondents had either previous experience in constructing multistory buildings with wooden frames, experienced respondents (ERs), or no experience, unexperienced respondents (UERs). 63% of the respondents were ERs, while 37% of them were UERs. It is resulted that the respondents think there is a lack of competence and experience in wooden frames for construction of multi-story buildings in Sweden. Factors that have the greatest impact on decisions to construct with wooden frames are positive environmental and climatic aspects as well as production advantages. Factors that are considered as major obstacles to construct with wooden frames are cost, acoustics, and moisture problems.

    • Gradients of secreted signaling proteins guide growing blood vessels during both normal and pathological angiogenesis. However, the mechanisms by which endothelial cells integrate and respond to graded distributions of chemotactic factors are still poorly understood. We have in this study investigated endothelial cell migration in response to hill-shaped gradients of vascular endothelial growth factor A (VEGFA) and fibroblast growth factor 2 (FGF2) using a novel microfluidic chemotaxis chamber (MCC). Cell migration was scored at the level of individual cells using time-lapse microscopy. A stable gradient of VEGFA165 ranging from 0 to 50 ng/ml over a distance of 400 microm was shown to strongly induce chemotaxis of endothelial cells of different vascular origin. VEGFA121, unable to bind proteoglycan and neuropilin coreceptors, was also shown to induce chemotaxis in this setup. Furthermore, a gradient of FGF2 was able to attract venular but not arterial endothelial cells, albeit less efficiently than VEGFA165. Notably, constant levels of VEGFA165, but not of FGF2, were shown to efficiently reduce chemokinesis. Systematic exploration of different gradient shapes led to the identification of a minimal gradient steepness required for efficient cell guidance. Finally, analysis of cell migration in different regions of the applied gradients showed that chemotaxis is reduced when cells reach the high end of the gradient. Our findings suggest that chemotactic growth factor gradients may instruct endothelial cells to shift toward a nonmigratory phenotype when approaching the growth factor source.

    • The SARS-CoV-2 virus is currently causing a global pandemic. Infection may result in a systemic disease called COVID-19, affecting primarily the respiratory tract. Often the gastrointestinal tract and kidneys also become involved. Angiotensin converting enzyme 2 (ACE2) serves as the receptor for SARS-CoV-2. The membrane proteins, Transmembrane serine protease 2 (TMPRSS2) and Neuropilin 1 (NRP1) are accessory proteins facilitating the virus entry. In this study we show that the human proximal kidney tubules, express these factors. We hypothesized that cancers derived from proximal tubules as clear cell (CCRCC) and papillary renal cell carcinoma (PRCC), retain the expression of the SARS-CoV-2 entry factors making these cancers susceptible to SARS-CoV-2 infection. We used bioinformatics, western blotting, and assessment of tissue micro arrays (TMA) including 263 cases of CCRCC, 139 cases of PRCC and 18 cases of chromophobe RCC to demonstrate that the majority of CCRCC and PRCC cases retained the RNA and protein expression of the entry factors for SARS-CoV-2. We furthermore show that SARS-CoV-2 virus propagated robustly in primary cultures of CCRCC and PRCC cells with a visible virus cytopathogenic effect correlating with viral RNA expression levels. We also noted that the delta-variant of SARS-CoV-2 causes cancer cells to form syncytia in-vitro. This phenomenon was also identified histologically in CCRCC tissue from a patient that had been hospitalized for COVID-19, twelve months prior to nephrectomy. Our data provide insights into SARS-CoV-2 infectivity in renal cell carcinoma and that the virus causes a distinct cytopathogenic effect.

Therefore the contents of the report do not constitute an attempt at either questioning the use of existing regulatory data in the manner prescribed by international regulatory proceedures, or as a new risk assessment, based on the scientific information and concepts discussed. Here we report our findings, with particular emphasis on exisiting regulatory information, potential for lack of translation of results from regulatory animal testing to humans, lack of human exposure data and suggestions on plausible mode(s) of action of PPF in human neurodevelopmental adversities such as microcephaly.

    • The 2014–2015 Holuhraun fissure eruption provided a rare opportunity to study in detail the magmatic processes and magma plumbing system dynamics during a 6-month-long, moderate- to large-volume basaltic fissure eruption. In this contribution, we present a comprehensive dataset, including major and trace elements of whole-rock and glassy tephra samples, mineral chemistry, and radiogenic and oxygen isotope analyses from an extensive set of samples (n = 62) that were collected systematically in several field campaigns throughout the entire eruptive period. We also present the first detailed chemical and isotopic characterization of magmatic sulfides from Iceland. In conjunction with a unique set of geophysical data, our approach provides a detailed temporal and spatial resolution of magmatic processes before and during this eruption. The 2014–2015 Holuhraun magma is compositionally indistinguishable from recent basalts erupted from the Bárðarbunga volcanic system, consistent with seismic observations for magma ascent close to the Bárðarbunga central volcano, followed by dyke propagation to the Holuhraun eruption site. Whole-rock elemental and isotopic compositions are remarkably constant throughout the eruption. Moreover, the inferred depth of the magma reservoir tapped during the eruption is consistently 8 ± 5 km, in agreement with geodetic observations and melt inclusion entrapment pressures, but inconsistent with vertically extensive multi-tiered magma storage prior to eruption. The near constancy in the chemical and isotopic composition of the lava is consistent with the efficient homogenization of mantle-derived compositional variability. In contrast, occurrence of different mineral populations, including sulfide globules, which display significant compositional variability, requires a more complex earlier magmatic history. This may include sampling of heterogeneous mantle melts that mixed, crystallized and finally homogenized at mid- to lower-crustal conditions.

    • Background: The need for a mobile workforce inevitably means that the length of the total work day (working and traveling time) will increase, but the health effects of commuting have been surprisingly little studied apart from perceived stress and the benefits of physically active commuting. Methods: We used data from two cross-sectional population-based public health surveys performed in 2004 and 2008 in Scania, Sweden (56% response rate). The final study population was 21, 088 persons aged 18-65, working > 30 h/week. Duration (one-way) and mode of commuting were reported. The outcomes studied were perceived poor sleep quality, everyday stress, low vitality, mental health, self-reported health, and absence from work due to sickness during the past 12 months. Covariates indicating socioeconomic status and family situation, overtime, job strain and urban/rural residency were included in multivariate analyses. Subjects walking or cycling to work < 30 min were used as a reference category. Results: Monotonous relations were found between duration of public transport commuting and the health outcomes. For the category commuting > 60 min odds ratios (ORs) ranged from 1.2 - 1.6 for the different outcomes. For car commuting, the relationships were concave downward or flat, with increasing subjective health complaints up to 30-60 min (ORs ranging from 1.2 - 1.4), and lower ORs in the > 60 min category. A similar concave downward relationship was observed for sickness absence, regardless of mode of transport. Conclusions: The results of this study are concordant with the few earlier studies in the field, in that associations were found between commutation and negative health outcomes. This further demonstrates the need to consider the negative side-effects of commuting when discussing policies aimed at increasing the mobility of the workforce. Studies identifying population groups with increased susceptibility are warranted.

  • Patients' perspective on prostatic artery embolization : A qualitative study

    • Objectives: The aim was to describe the patients' experience of undergoing prostatic artery embolization.Methods: A retrospective qualitative interview study was undertaken with 15 patients of mean age 73 years who had undergone prostatic artery embolization with a median duration of 210 min at two medium sized hospitals in Sweden. The reasons for conducting prostatic artery embolization were clean intermittent catheterization (n = 4), lower urinary tract symptoms (n = 10) or haematuria (n = 1). Data were collected through individual, semi-structured telephone interviews 1-12 months after treatment and analysed using qualitative content analysis.Results: Four categories with sub-categories were formulated to describe the results: a diverse experience; ability to control the situation; resumption of everyday activities and range of opinions regarding efficacy of outcomes. Overall, the patients described the procedure as painless, easy and interesting and reported that while the procedure can be stressful, a calm atmosphere contributed to achieving a good experience. Limitations on access to reliable information before, during and after the procedure were highlighted as a major issue. Practical ideas for improving patient comfort during the procedure were suggested. Improved communications between treatment staff and patients were also highlighted. Most patients could resume everyday activities, some felt tired and bruising caused unnecessary worry for a few. Regarding functional outcome, some patients described substantial improvement in urine flow while others were satisfied with regaining undisturbed night sleep. Those with less effect were considering transurethral resection of the prostate as a future option. Self-enrolment to the treatment and long median operation time may have influenced the results.Conclusions: From the patients' perspective, prostatic artery embolization is a well-tolerated method for treating benign prostate hyperplacia.

  • Charting the human lung with inhaled nanoparticles : Development of AiDA - a new diagnostic technique for respiratory disease

    • Changes to the smallest structures of the distal lung are often the first sign of severe respiratory disease, but to date methods to detect and quantify these changes are limited. Chronic obstructive pulmonary disease (COPD) is currently estimated to be the third most common cause of death worldwide, but is to a large extent under-diagnosed, especially at early stages. This thesis explores a new method for examining the lung by measurements of lung deposition of airborne nanoparticles, with possible potential to be developed into a diagnostic method for detection of changes typical for early COPD. The technique is called Airspace Dimension Assessment with nanoparticles (AiDA).An instrument for lung deposition measurements was constructed and characterized on healthy subjects. The instrument produces a test aerosol containing monodisperse nanoparticles, which can be administered to subjects in inhalation experiments. The particle concentration in the inhaled test aerosol and in exhaled air from determined volumetric sample depths can be determined after controlled residence times in the lung.By comparing inhaled and exhaled concentrations, the deposition efficiency of the nanoparticles can be determined in a single breath. The instrument is, to our knowledge, currently the only instrument with this capability.Particle deposition was measured on healthy individuals and individuals with COPD. It was found that particle deposition was decreased for subjects with COPD, and this was interpreted as an effect of enlarged airspaces, pulmonary emphysema. This shows that particle deposition measured with the instrument reflects subject specific lung properties.A theory was developed for interpretation of the relation between measurements of lung deposition and properties of the lung. Nanoparticles deposit almost exclusively by diffusion in the distal lung. By measuring the half-life time of particles in the lung, effective airspace radii can be estimated which reflect lung geometry. Repeated measurements on three subjects with 15 -18 months between the measurements showed that the precision was on average ±7 μm. Particle deposition during the dynamic part of the breathing manoeuvres may also provide information about mainly the small conducting airways.The theory was evaluated by measurements on healthy subjects. It was shown that both the derived effective airspace radii and the particle deposition during the dynamic phase of the measurement were in agreement with expectations from the developed theory. It was also shown that the parameters correlated with subject characteristics and physiological measurements.An AiDA measurement protocol was developed which can be performed by most subjects in less than 15 minutes. AiDA measurements have been performed on more than 1000 subjects. This exceeds the total number of subjects included in previously published studies on lung deposition of nanoparticles. Preliminary results show that the technique fulfils several criteria for an efficient diagnostic technique. Preliminary data also indicates that the technique may give relevant medical information about the distal lung not easily obtainable by other means, and thus may be considered as a feasible future diagnostic tool for the lung.

    • Airspace Dimension Assessment (AiDA) is a technique to assess lung morphology by measuring lung deposition of inhaled nanoparticles. Nanoparticles deposit in the lungs predominately by diffusion, and average diffusion distances, corresponding to effective airspace radii (rAiDA), can be inferred from measurements of particle recovery after varied breath holds. Also, particle recovery after a 0-s breath hold (R0) may hold information about the small conducting airways. This study investigates rAiDA at different volumetric sample depths in the lungs of healthy subjects. Measurements were performed with 50-nm polystyrene nanospheres on 19 healthy subjects aged 17– 67 yr. Volumetric sample depths ranged from 200 to 5,000 ml and breath-hold times from 5 to 20 s. At the examined volumetric sample depths, rAiDA values ranged from ~200 – 600 m, which correspond to dimensions of the bronchiolar and the gas-exchanging regions of the lungs. R 0 decreased with volumetric sample depth and showed more intersubject variation than rAiDA. Correlations were found between the AiDA parameters, anthropometry, and lung function tests, but not between rAiDA and R0. For repeated measurements on 3 subjects over an 18-mo period, rAiDA varied on average within 7 m ( 2.4%). The results indicate that AiDA has potential as an efficient new in vivo technique to assess individual lung properties. The information obtained by such measurements may be of value for lung diagnostics, especially for the distal lungs, which are challenging to examine directly by other means. NEW & NOTEWORTHY This is the first study to measure effective airspace radii (rAiDA) at volumetric sample depths 200 –5,000 ml in healthy subjects by Airspace Dimension Assessment (AiDA). Observed rAiDA were 200 – 600 m, which corresponds to airspaces for the bronchiolar and the gas-exchanging regions around airway generation 14 –17. rAiDA correlated with lung function tests and anthropometry. Measurements of rAiDA on 3 subjects over 11–18 mo were within 7 m.

  • Effects and mechanisms of supramaximal High-Intensity Interval Training on extrapulmonary manifestations in people with and without Chronic Obstructive Pulmonary Disease (COPD-HIIT) : study protocol for a multi-centre, randomized controlled trial

    • Background: Beyond being a pulmonary disease, chronic obstructive pulmonary disease (COPD) presents with extrapulmonary manifestations including reduced cognitive, cardiovascular, and muscle function. While exercise training is the cornerstone in the non-pharmacological treatment of COPD, there is a need for new exercise training methods due to suboptimal adaptations when following traditional exercise guidelines, often applying moderate-intensity continuous training (MICT). In people with COPD, short-duration high-intensity interval training (HIIT) holds the potential to induce a more optimal stimulus for training adaptations while circumventing the ventilatory burden often associated with MICT in people with COPD. We aim to determine the effects of supramaximal HIIT and MICT on extrapulmonary manifestations in people with COPD compared to matched healthy controls.Methods: COPD-HIIT is a prospective, multi-centre, randomised, controlled trial with blinded assessors and data analysts, employing a parallel-group trial. In Phase 1, we will investigate the effects and mechanisms of a 12-week intervention of supramaximal HIIT compared to MICT in people with COPD (n = 92) and matched healthy controls (n = 70). Participants will perform watt-based cycling 2–3 times weekly. In Phase 2, we will determine how exercise training and inflammation impact the trajectories of neurodegeneration, in people with COPD, over 24 months. In addition to the 92 participants with COPD performing HIIT or MICT, a usual care group (n = 46) is included in phase 2. In both phases, the primary outcomes are change from baseline in cognitive function, cardiorespiratory fitness, and muscle power. Key secondary outcomes include change from baseline exercise tolerance, brain structure and function measured by MRI, neuroinflammation measured by PET/CT, systemic inflammation, and intramuscular adaptations. Feasibility of the interventions will be comprehensively investigated.Discussion: The COPD-HIIT trial will determine the effects of supramaximal HIIT compared to MICT in people with COPD and healthy controls. We will provide evidence for a novel exercise modality that might overcome the barriers associated with MICT in people with COPD. We will also shed light on the impact of exercise at different intensities to reduce neurodegeneration. The goal of the COPD-HIIT trial is to improve the treatment of extrapulmonary manifestations of the disease.Trial registration: Clinicaltrials.gov: NCT06068322. An instrument was developed to measure respiratory tract deposition of nanoparticles in a single breath. Monodisperse nanoparticles are generated, inhaled and sampled from a determined volumetric lung depth after a controlled residence time in the lung. The instrument was characterized for sensitivity to inter-subject variability, particle size (22, 50, 75 and 100 nm) and breath-holding time (3-20 s) in a group of seven healthy subjects. The measured particle recovery had an inter-subject variability 26-50 times larger than the measurement uncertainty and the results for various particle sizes and breath-holding times were in accordance with the theory for Brownian diffusion and values calculated from the Multiple-Path Particle Dosimetry model. The recovery was found to be determined by residence time and particle size, while respiratory flow-rate had minor importance in the studied range 1-10 L/s. The instrument will be used to investigate deposition of nanoparticles in patients with respiratory disease. The fast and precise measurement allows for both diagnostic applications, where the disease may be identified based on particle recovery, and for studies with controlled delivery of aerosol-based nanomedicine to specific regions of the lungs.

Experimental data are limited for vulnerable groups such as individuals with respiratory diseases. The aim of this study is to investigate the differences in lung deposition of nanoparticles in the distal lung for healthy subjects and subjects with respiratory disease. Methods: Lung deposition of nanoparticles (50 and 100 nm) was measured after a 10 s breath-hold for three groups: healthy never-smoking subjects (n = 17), asymptomatic (active and former) smokers (n = 15) and subjects with chronic obstructive pulmonary disease (n = 16). Measurements were made at 1300 mL and 1800 mL volumetric lung depth. Each subject also underwent conventional lung function tests, including post bronchodilator FEV1, VC, and diffusing capacity for carbon monoxide, DL,CO. Patients with previously diagnosed respiratory disease underwent a CT-scan of the lungs. Particle lung deposition fraction, was compared between the groups and with conventional lung function tests. Results: We found that the deposition fraction was significantly lower for subjects with emphysema compared to the other subjects (p = 0.001-0.01), but no significant differences were found between healthy never-smokers and smokers. Furthermore, the particle deposition correlated with pulmonary function tests, FEV1%Pred (p < 0.05), FEV1/VC%Pred (p < 0.01) and DL,CO (p < 0.0005) when all subjects were included. Furthermore, for subjects with emphysema, deposition fraction correlated strongly with DL,CO (Pearson's r = 0.80-0.85, p < 0.002) while this correlation was not found within the other groups. Conclusions: Lower deposition fraction was observed for emphysematous subjects and this can be explained by enlarged distal airspaces in the lungs. As expected, deposition increases for smaller particles and deeper inhalation. The observed results have implications for exposure assessment of air pollution and dosimetry of aerosol-based drug delivery of nanoparticles.

    • The time dependence of ice-nucleating particle (INP) activity is known to exist, yet for simplicity it is often omitted in atmospheric models as an approximation. Hitherto, only limited experimental work has been done to quantify this time dependency, for which published data are especially scarce regarding ambient aerosol samples and longer timescales. In this study, the time dependence of INP activity is quantified experimentally for six ambient environmental samples. The experimental approach includes a series of hybrid experiments with alternating constant cooling and isothermal experiments using a recently developed cold-stage setup called the Lund University Cold-Stage (LUCS). This approach of observing ambient aerosol samples provides the optimum realism for representing their time dependence in any model. Six ambient aerosol samples were collected at a station in rural Sweden representing aerosol conditions likely influenced by various types of INPs: marine, mineral dust, continental pristine, continental-polluted, combustion-related and rural continental aerosol. Active INP concentrations were seen to be augmented by about 40% to 100% (or 70% to 200%), depending on the sample, over 2h (or 10h). Mineral dust and rural continental samples displayed the most time dependence. This degree of time dependence observed was comparable to, but weaker than, that seen in previous published works. A general tendency was observed for the natural timescale of the freezing to dilate increasingly with time. The fractional freezing rate was observed to decline steadily with the time since the start of isothermal conditions following a power law. A representation of time dependence for incorporation into schemes of heterogeneous ice nucleation that currently omit it is proposed. Our measurements are inconsistent with the simplest purely stochastic model of INP activity, which assumes that the fractional freezing rate of all unfrozen drops is somehow constant and would eventually overpredict active INPs. In reality, the variability of efficiencies among INPs must be treated with any stochastic theory.

    • Present whiplash injury assessment tests, reflecting the seat performance only, provide limited insights into real-world whiplash injury protection needs. Virtual testing of braking followed by a rear-end impact, in addition to alternative initial sitting postures, were conducted to investigate if the current anthropomorphic test device can be used to cover a larger scope of the real-world context. Reconstruction of published 1.1 g braking volunteer tests showed that a BioRID FE model was capable of recreating human-like kinematics; with head and T1 kinematics just within a 1 SD corridor on the low side of the volunteer response, while vertical displacements and lap-belt forces were underpredicted. A simulation series including pre-impact braking prior to rear-end impact investigated two strategies to vary the backset, as well as pre-impact means of intervention, exemplified by pre-impact seatbelt pretensioning. Using virtual testing, the study demonstrates examples of expanding the whiplash assessment test setup, enabling inclusion of a variety of occupant sitting postures and a braking event preceding the rear-end impact, while still being feasible to execute. As a next step, a human body model capable of seamless pre-crash and crash prediction could even allow for more in-depth investigations, as well as inclusion of ranges of occupant sizes and posture setting possibilities.

    • This thesis consists of four studies focusing on empirical issues in industrial organization.Bid Rigging in Swedish Procurement Auctions: Using a unique data set of procurement auctions carried out by the Swedish National Road Administration, this paper addresses the issue of bid rigging in the Swedish asphalt-paving sector. Both market characteristics and the fact that the Swedish Competition Authority in 2003 initiated legal proceedings against a group of firms active in the market indicate the existence of collusive behaviour. If firms act competitively, they should submit independent bids, conditional on firm- and auction-specific differences. Reduced-form equations are estimated and the hypothesis of conditional independence is tested by analysing if the difference between observed and predicted bids correlates between firms. If negative correlation is observed, one possible explanation is bid rigging. The overall results indicate that collusion may be widespread in the industry and suggest further investigation of the market.Collusion in Procurement Auctions: Structural Estimation of Bidders' Costs: In this paper, the aim is to analyse the existence of collusion in asymmetric asphalt-procurement auctions. Firms that behave competitively should have private costs that are independent, conditional on available firm- and auction-specific information. The hypothesis of conditional independence can be tested and if it is rejected, a possible explanation is collusion. Using a constrained strategy equilibrium concept in solving for equilibrium bid strategies and firms' private costs makes it possible to test the hypothesis of conditional independence while at the same time controlling for firms' strategic considerations. The analysis is based on bid data from procurement auctions carried out in Sweden during the 1990's. The findings are that the hypothesis of conditional independence can be rejected for about half the firm-pairs that are tested. This suggests that collusive behaviour is plausible in the investigated market.An Econometric Analysis of the European Commission’s Merger Decisions: Using a sample of 96 mergers notified to the European Commission (EC) and logit-regression techniques, we analyse the EC’s decision process. We find that the probability of a phase-2 investigation and of a prohibition of the merger increases with the parties’ market shares. The probabilities increase also when the Commission finds high entry barriers or that the market structure post-merger is conductive to collusion. We do not find significant effects of “political” variables, such as the nationality of the merging firms.Comparing Merger Policies: The European Union versus the United States: Merger regulation affects large transactions in the market for corporate control in both the European Union (EU) and the United States (US). This paper compares and contrasts the relevant enforcement policies. US enforcement is broader-based as it attacks oligopoly, unilateral, and dominant firm concerns, while the EU policy focused almost entirely on market dominance. Our analysis of the enforcement policies shows that analytical and evidence considerations have more of an effect in the US, while institutional considerations matter more in the EU. Considering only cases subject to a theory of market dominance (dominant firm), the EU regime appears, on average, slightly more aggressive than US policy for relatively weak cases, while the US policy appears more aggressive for strong cases.

    • The processes controlling advance and retreat of outlet glaciers in fjords draining the Greenland Ice Sheet remain poorly known, undermining assessments of their dynamics and associated sea-level rise in a warming climate. Mass loss of the Greenland Ice Sheet has increased six-fold over the last four decades, with discharge and melt from outlet glaciers comprising key components of this loss. Here we acquired oceanographic data and multibeam bathymetry in the previously uncharted Sherard Osborn Fjord in northwest Greenland where Ryder Glacier drains into the Arctic Ocean. Our data show that warmer subsurface water of Atlantic origin enters the fjord, but Ryder Glacier’s floating tongue at its present location is partly protected from the inflow by a bathymetric sill located in the innermost fjord. This reduces under-ice melting of the glacier, providing insight into Ryder Glacier’s dynamics and its vulnerability to inflow of Atlantic warmer water.

    • Deep-water formation in the northern North Atlantic Ocean and the Arctic Ocean is a key driver of the global thermohaline circulation and hence also of global climate. Deciphering the history of the circulation regime in the Arctic Ocean has long been prevented by the lack of data from cores of Cenozoic sediments from the Arctic’s deep-sea floor. Similarly, the timing of the opening of a connection between the northern North Atlantic and the Arctic Ocean, permitting deep-water exchange, has been poorly constrained. This situation changed when the first drill cores were recovered from the central Arctic Ocean. Here we use these cores to show that the transition from poorly oxygenated to fully oxygenated (‘ventilated’) conditions in the Arctic Ocean occurred during the later part of early Miocene times. We attribute this pronounced change in ventilation regime to the opening of the Fram Strait. A palaeo-geographic and palaeo-bathymetric reconstruction of the Arctic Ocean, together with a physical oceanographic analysis of the evolving strait and sill conditions in the Fram Strait, suggests that the Arctic Ocean went from an oxygenpoor ‘lake stage’, to a transitional ‘estuarine sea’ phase with variable ventilation, and finally to the fully ventilated ‘ocean’ phase 17.5 Myr ago. The timing of this palaeo-oceanographic change coincides with the onset of the middle Miocene climatic optimum, although it remains unclear if there is a causal relationship between these two events.

    • Endogenous retroviruses (ERVs) make up a large fraction of mammalian genomes and are thought to contribute to human disease, including brain disorders. In the brain, aberrant activation of ERVs is a potential trigger for an inflammatory response, but mechanistic insight into this phenomenon remains lacking. Using CRISPR/Cas9-based gene disruption of the epigenetic co-repressor protein Trim28, we found a dynamic H3K9me3-dependent regulation of ERVs in proliferating neural progenitor cells (NPCs), but not in adult neurons. In vivo deletion of Trim28 in cortical NPCs during mouse brain development resulted in viable offspring expressing high levels of ERVs in excitatory neurons in the adult brain. Neuronal ERV expression was linked to activated microglia and the presence of ERV-derived proteins in aggregate-like structures. This study demonstrates that brain development is a critical period for the silencing of ERVs and provides causal in vivo evidence demonstrating that transcriptional activation of ERV in neurons results in an inflammatory response.
