    • ObjectivesThe aim was to study the occurrence and how equality in health was expressed in policy and activities in four Swedish municipalities in the years 2006 and 2012.MethodsA deductive content analysis of interviews with local municipal key persons, politicians and chief executives and policy documents in the form of annual reports, in four strategically selected municipalities in the region of Mälardalen in Sweden.ResultsIn the documents, terms of equality, justice and vulnerable groups did not occur. However, these themes were mentioned by politicians and officials in most of the municipalities studied, and the issues of vulnerability and exclusion occur in both documents and interviews in most municipalities. These issues were conceptualized as various support measures, such as social and economic support, integration efforts, appropriate housing, promotion of employment and to provide security and a safe childhood. These issues occurred in all the documents and in some of the interviews. Priority groups in all municipalities were children, young people and older people but also immigrants. Barriers mentioned were the lack of qualified personnel such as social workers and support staff in schools.ConclusionsEquality, health equity and reducing the gaps are not explicitly stated in the study of the municipalities. But the insight that disadvantaged groups needs support through actions and tailored training programs and work based on the individuals’ different conditions was emphasized.

It is envisaged that the overall objective will be achieved through the efforts of different bodies, including agencies, county councils and municipalities. Responsibility for implementation is located at regional and local level, without any specified guidelines or, for example, economic incentives for how the work should be performed. This study’s intention is, from a bottom-up perspective, to investigate public-health policies in two municipalities, and to see whether municipal policies can be related to the national objective.  The bottom-up perspective focuses on the actors, the people responsible for implementation, and on the process as seen from their own perspective. The implementers play a key role, as mediators of empirical knowledge and bases for decisions to the national decision-makers. Thus, in the implementation of national policy at local level, decision-makers are dependent on the decisions made and priorities set by the municipalities in their own local settings, as these are related to competing or similar laws, policies, stakeholders and needs. Local and committed actors have an important function in the implementation process. Serious commitment and occupying a position of power are important aspects of success, but so too is professional expertise. At the same time, local actors’ opportunities to act and implement national targets within their own organization is restricted by surrounding structures, e.g. the party-political (partisan) composition of the municipality and its administrative organization, and also the presence or not of formal or informal support networks. Methods: Scrutiny of documents and interviews provided a foundation for a qualitative case study.Results:The results reveal a loose fit between policy implementation and the national public health policy. The local level does not regard the national objective as implementable; instead, it has, to varying degrees, chosen to redefine its goals according to municipal needs and conditions. A success-promoting factor in the two municipalities was the presence of committed and knowledgeable actors/implementers. Also, the municipality with a more centrally controlled and stable party-political leadership succeeded better in implementing structural and intersectoral community-wide policies for coordinated local public-health promotion. Discussion /Conclusions:Problems that accompany major, ambitious and non-specific goals, like those in the Swedish public health policy, are that they risk being integrated into a solely symbolic structure, entailing formal adoption but with only a superficial finish.  Below the surface, the informal, original organization remains in the administrations, which hinders implementation.Swedish municipal self-governance, with its enhanced scope for local action, offers an opportunity for communities to shape their own politics and policies. But, at the same time, this acts as a barrier to opportunities for decision-makers at national-government level to influence and guide the municipalities towards the national objective of more organized and structured health promotion focusing on the determinants of health enhanced knowledge of the policy, and of its specific focus on health determinants, is needed to increase understanding/motivation to implement the national policy locally.

  • Municipalities setting approaches in local health promotion : A swedish case study

    • Objective: Gain knowledge about municipalities’ setting approaches by studying four Swedish municipalities’ and how they practices settings in their organized and conducted local health promotion.Methods: A multiple case study design with content analysis of key person interviews and documents. Results: The municipalities stressed the importance of enable the inhabitants’ and staffs’ possibilities to maintain and achieve better health. In the case an explicit local health policy existed the focus was on specific target groups e.g. children, problem areas e.g. alcohol or on health outcomes as e.g. decreases in days of sick leave. Dependent on the nature of the problem, the strategies implicitly focused on intersectoral collaboration within the municipal administration and sectors or/and with external actors as official authorities, private companies, NGOs and neighboring municipalities, as well as between different levels and professions. Health promotion actions implemented were mainly established in more traditional and locally well-known local settings as schools, workplaces, family centers or libraries, but there were examples on efforts to create new venues to enable social wellbeing and supportive settings. Conclusions: The setting approach is consistent with the municipalities’ views and possibilities to handle complex public health issues, hence the setting approach is still going strong. Locally prioritized health issues based on local needs and prerequisites are preferably performed in traditional settings. The development and sustainability is seen as possible and achievable by intersectoral collaborations with engaged and committed actors in established arenas. Challenges in the municipalities’ setting approaches are related to factors like sectorial pipe-line organizations and e.g. confidentiality regulations in collaborations between the municipality and health care sector.

    • OBJECTIVES: In 2003 the wide-ranging Swedish National Public Health Policy (SNPHP), with a focus on health determinants, was adopted by the Swedish parliament. In the context of multilevel governance, SNPHP implementation is dependent on self-governed municipalities and counties. The aim of the study is, from a municipal perspective, to investigate public-health policies in two municipalities. METHODS: Content analysis of documents and interviews provided a foundation for an explorative case study. RESULTS: The SNPHP at national level is overriding but politically controversial. At local level, a health-determinants perspective was detectable in the policies implemented, but none regarding to health equality. At local level, the SNPHP is not regarded as implementable; rather, limited parts have, to varying degrees, been reconciled with local public-health goals, according to municipal needs and conditions. A success-promoting factor in the two municipalities was the presence of committed and knowledgeable actors/implementers. Also, the municipality with a more centrally controlled and stable party-political leadership succeeded better in implementing structural and intersectoral community-wide policies for coordinated local health promotion. CONCLUSIONS: The contents of national and local public-health policies differ, and municipalities that have implemented their own local health policies do not seem to regard the SNPHP as justifiable or adoptable. If the SNPHP overall aim regarding equal health is to be achieved homogeneously in Swedish municipalities, its contents and purpose need clearer management and negotiation, so that implementation of the national policy locally is understandable and motivated.

    • Obesity is globally prevalent and highly heritable, but its underlying genetic factors remain largely elusive. To identify genetic loci for obesity susceptibility, we examined associations between body mass index and ~2.8 million SNPs in up to 123,865 individuals with targeted follow up of 42 SNPs in up to 125,931 additional individuals. We confirmed 14 known obesity susceptibility loci and identified 18 new loci associated with body mass index (P < 5 × 10−8), one of which includes a copy number variant near GPRC5B. Some loci (at MC4R, POMC, SH2B1 and BDNF) map near key hypothalamic regulators of energy balance, and one of these loci is near GIPR, an incretin receptor. Furthermore, genes in other newly associated loci may provide new insights into human body weight regulation.

  • Detection of a particle shower at the Glashow resonance with IceCube

    • The Glashow resonance describes the resonant formation of a W− boson during the interaction of a high-energy electron antineutrino with an electron1, peaking at an antineutrino energy of 6.3 petaelectronvolts (PeV) in the rest frame of the electron. Whereas this energy scale is out of reach for currently operating and future planned particle accelerators, natural astrophysical phenomena are expected to produce antineutrinos with energies beyond the PeV scale. Here we report the detection by the IceCube neutrino observatory of a cascade of high-energy particles (a particle shower) consistent with being created at the Glashow resonance. A shower with an energy of 6.05 ± 0.72 PeV (determined from Cherenkov radiation in the Antarctic Ice Sheet) was measured. Features consistent with the production of secondary muons in the particle shower indicate the hadronic decay of a resonant W− boson, confirm that the source is astrophysical and provide improved directional localization. The evidence of the Glashow resonance suggests the presence of electron antineutrinos in the astrophysical flux, while also providing further validation of the standard model of particle physics. Its unique signature indicates a method of distinguishing neutrinos from antineutrinos, thus providing a way to identify astronomical accelerators that produce neutrinos via hadronuclear or photohadronic interactions, with or without strong magnetic fields. As such, knowledge of both the flavour (that is, electron, muon or tau neutrinos) and charge (neutrino or antineutrino) will facilitate the advancement of neutrino astronomy.

  • eV-Scale Sterile Neutrino Search Using Eight Years of Atmospheric Muon Neutrino Data from the IceCube Neutrino Observatory

    • The results of a 3 + 1 sterile neutrino search using eight years of data from the IceCube Neutrino Observatory are presented. A total of 305 735 muon neutrino events are analyzed in reconstructed energy-zenith space to test for signatures of a matter-enhanced oscillation that would occur given a sterile neutrino state with a mass-squared differences between 0.01 and 100 eV(2). The best-fit point is found to be at sin(2)(2 theta(24)) = 0.10 and Delta m(41)(2) = 4.5 eV(2), which is consistent with the no sterile neutrino hypothesis with a p value of 8.0%.

  • Follow-up of Astrophysical Transients in Real Time with the IceCube Neutrino Observatory

    • In multi-messenger astronomy, rapid investigation of interesting transients is imperative. As an observatory with a 4 pi steradian field of view, and similar to 99% uptime, the IceCube Neutrino Observatory is a unique facility to follow up transients, as well as to provide valuable insights for other observatories and inform their observational decisions. Since 2016, IceCube has been using low-latency data to rapidly respond to interesting astrophysical events reported by the multi-messenger observational community. Here, we describe the pipeline used to perform these followup analyses, and provide a summary of the 58 analyses performed as of July 2020. We find no significant signal in the first 58 analyses performed. The pipeline has helped inform various electromagnetic observation strategies, and has constrained neutrino emission from potential hadronic cosmic accelerators.

  • IceCube-Gen2 : the window to the extreme Universe

    • The observation of electromagnetic radiation from radio to gamma-ray wavelengths has provided a wealth of information about the Universe. However, at PeV (10(15) eV) energies and above, most of the Universe is impenetrable to photons. New messengers, namely cosmic neutrinos, are needed to explore the most extreme environments of the Universe where black holes, neutron stars, and stellar explosions transform gravitational energy into non-thermal cosmic rays. These energetic particles have millions of times higher energies than those produced in the most powerful particle accelerators on Earth. As neutrinos can escape from regions otherwise opaque to radiation, they allow an unique view deep into exploding stars and the vicinity of the event horizons of black holes. The discovery of cosmic neutrinos with IceCube has opened this new window on the Universe. IceCube has been successful in finding first evidence for cosmic particle acceleration in the jet of an active galactic nucleus. Yet, ultimately, its sensitivity is too limited to detect even the brightest neutrino sources with high significance, or to detect populations of less luminous sources. In this white paper, we present an overview of a next-generation instrument, IceCube-Gen2, which will sharpen our understanding of the processes and environments that govern the Universe at the highest energies. IceCube-Gen2 is designed to: (a) Resolve the high-energy neutrino sky from TeV to EeV energies (b) Investigate cosmic particle acceleration through multi-messenger observations (c) Reveal the sources and propagation of the highest energy particles in the Universe (d) Probe fundamental physics with high-energy neutrinos IceCube-Gen2 will enhance the existing IceCube detector at the South Pole. It will increase the annual rate of observed cosmic neutrinos by a factor of ten compared to IceCube, and will be able to detect sources five times fainter than its predecessor. Furthermore, through the addition of a radio array, IceCube-Gen2 will extend the energy range by several orders of magnitude compared to IceCube. Construction will take 8 years and cost about $350M. The goal is to have IceCube-Gen2 fully operational by 2033. IceCube-Gen2 will play an essential role in shaping the new era of multi-messenger astronomy, fundamentally advancing our knowledge of the high-energy Universe. This challenging mission can be fully addressed only through the combination of the information from the neutrino, electromagnetic, and gravitational wave emission of high-energy sources, in concert with the new survey instruments across the electromagnetic spectrum and gravitational wave detectors which will be available in the coming years.

  • IceCube Search for Neutrinos Coincident with Compact Binary Mergers from LIGO-Virgo's First Gravitational-wave Transient Catalog

    • Using the IceCube Neutrino Observatory, we search for high-energy neutrino emission coincident with compact binary mergers observed by the LIGO and Virgo gravitational-wave (GW) detectors during their first and second observing runs. We present results from two searches targeting emission coincident with the sky localization of each GW event within a 1000 s time window centered around the reported merger time. One search uses a model-independent unbinned maximum-likelihood analysis, which uses neutrino data from IceCube to search for pointlike neutrino sources consistent with the sky localization of GW events. The other uses the Low-Latency Algorithm for Multi-messenger Astrophysics, which incorporates astrophysical priors through a Bayesian framework and includes LIGO-Virgo detector characteristics to determine the association between the GW source and the neutrinos. No significant neutrino coincidence is seen by either search during the first two observing runs of the LIGO-Virgo detectors. We set upper limits on the time-integrated neutrino emission within the 1000 s window for each of the 11 GW events. These limits range from 0.02 to 0.7 . We also set limits on the total isotropic equivalent energy, E-iso, emitted in high-energy neutrinos by each GW event. These limits range from 1.7 x 10(51) to 1.8 x 10(55) erg. We conclude with an outlook for LIGO-Virgo observing run O3, during which both analyses are running in real time.

  • Searching for eV-scale sterile neutrinos with eight years of atmospheric neutrinos at the IceCube Neutrino Telescope

    • We report in detail on searches for eV-scale sterile neutrinos, in the context of a 3 + 1 model, using eight years of data from the IceCube Neutrino Telescope. By analyzing the reconstructed energies and zenith angles of 305,735 atmospheric nu(mu) and (nu) over bar (mu) events we construct confidence intervals in two analysis spaces: sin(2)(2 theta(24)) vs Delta m(41)(2) under the conservative assumption theta(34) = 0; and sin(2)(2 theta(24)) vs sin(2)(2 theta(34)) given sufficiently large Delta m(41)(2) that fast oscillation features are unresolvable. Detailed discussions of the event selection, systematic uncertainties, and fitting procedures are presented. No strong evidence for sterile neutrinos is found, and the best-fit likelihood is consistent with the no sterile neutrino hypothesis with a p value of 8% in the first analysis space and 19% in the second.

  • Detection of astrophysical tau neutrino candidates in IceCube

    • High-energy tau neutrinos are rarely produced in atmospheric cosmic-ray showers or at cosmic particle accelerators, but are expected to emerge during neutrino propagation over cosmic distances due to flavor mixing. When high energy tau neutrinos interact inside the IceCube detector, two spatially separated energy depositions may be resolved, the first from the charged current interaction and the second from the tau lepton decay. We report a novel analysis of 7.5 years of IceCube data that identifies two candidate tau neutrinos among the 60 "High-Energy Starting Events" (HESE) collected during that period. The HESE sample offers high purity, all-sky sensitivity, and distinct observational signatures for each neutrino flavor, enabling a new measurement of the flavor composition. The measured astrophysical neutrino flavor composition is consistent with expectations, and an astrophysical tau neutrino flux is indicated at 2.8 sigma significance.

  • First all-flavor search for transient neutrino emission using 3-years of IceCube DeepCore data

    • Since the discovery of a flux of high-energy astrophysical neutrinos, searches for their origins have focused primarily at TeV-PeV energies. Compared to sub-TeV searches, high-energy searches benefit from an increase in the neutrino cross section, improved angular resolution on the neutrino direction, and a reduced background from atmospheric neutrinos and muons. However, the focus on high energy does not preclude the existence of sub-TeV neutrino emission where IceCube retains sensitivity. Here we present the first all-flavor search from IceCube for transient emission of low-energy neutrinos, focusing on the energy region of 5.6-100 GeV using three years of data obtained with the IceCube-DeepCore detector. We find no evidence of transient neutrino emission in the data, thus leading to a constraint on the volumetric rate of astrophysical transient sources in the range of similar to 705- 2301Gpc(-3) yr(-1) for sources following a subphotospheric energy spectrum with a mean energy of 100 GeV and a bolometric energy of 10(52) erg.

  • IceCube high-energy starting event sample : Description and flux characterization with 7.5 years of data

    • The IceCube Neutrino Observatory has established the existence of a high-energy all-sky neutrino flux of astrophysical origin. This discovery was made using events interacting within a fiducial region of the detector surrounded by an active veto and with reconstructed energy above 60 TeV, commonly known as the high-energy starting event sample (HESE). We revisit the analysis of the HESE sample with an additional 4.5 years of data, newer glacial ice models, and improved systematics treatment. This paper describes the sample in detail, reports on the latest astrophysical neutrino flux measurements, and presents a source search for astrophysical neutrinos. We give the compatibility of these observations with specific isotropic flux models proposed in the literature as well as generic power-law-like scenarios. Assuming v(e): v(mu): v(tau) = 1:1:1, and an equal flux of neutrinos and antineutrinos, we find that the astrophysical neutrino spectrum is compatible with an unbroken power law, with a preferred spectral index of 2.87(-0.19)(+0.20) for the 68% confidence interval.

    • The flux of high-energy neutrinos passing through the Earth is attenuated due to their interactions with matter. The interaction rate is determined by the neutrino interaction cross section and affects the flux arriving at the IceCube Neutrino Observatory, a cubic-kilometer neutrino detector embedded in the Antarctic ice sheet. We present a measurement of the neutrino cross section between 60 TeV and 10 PeV using the high-energy starting event (HESE) sample from IceCube with 7.5 years of data. The result is binned in neutrino energy and obtained using both Bayesian and frequentist statistics. We find it compatible with predictions from the Standard Model. While the cross section is expected to be flavor independent above 1 TeV, additional constraints on the measurement are included through updated experimental particle identification (PID) classifiers, proxies for the three neutrino flavors. This is the first such measurement to use a ternary PID observable and the first to account for neutrinos from tau decay.

    • A search for point-like and extended sources of cosmic neutrinos using data collected by the ANTARES and IceCube neutrino telescopes is presented. The data set consists of all the track-like and shower-like events pointing in the direction of the Southern Sky included in the nine-year ANTARES point-source analysis, combined with the throughgoing track-like events used in the seven-year IceCube point-source search. The advantageous field of view of ANTARES and the large size of IceCube are exploited to improve the sensitivity in the Southern Sky by a factor of similar to 2 compared to both individual analyses. In this work, the Southern Sky is scanned for possible excesses of spatial clustering, and the positions of preselected candidate sources are investigated. In addition, special focus is given to the region around the Galactic Center, whereby a dedicated search at the location of SgrA* is performed, and to the location of the supernova remnant RXJ 1713.7-3946. No significant evidence for cosmic neutrino sources is found, and upper limits on the flux from the various searches are presented.

    • We present the results of the first combined dark matter search targeting the Galactic Center using the ANTARES and IceCube neutrino telescopes. For dark matter particles with masses from 50 to 1000 GeV, the sensitivities on the self-annihilation cross section set by ANTARES and IceCube are comparable, making this mass range particularly interesting for a joint analysis. Dark matter self-annihilation through the tau(+)tau(-) , mu(+)mu(-) , b (b) over bar, and W+W- channels is considered for both the Navarro-Frenk-White and Burkert halo profiles. In the combination of 2101.6 days of ANTARES data and 1007 days of IceCube data, no excess over the expected background is observed. Limits on the thermally averaged dark matter annihilation cross section are set. These limits present an improvement of up to a factor of 2 in the studied dark matter mass range with respect to the individual limits published by both collaborations. When considering dark matter particles with a mass of 200 GeV annihilating through the tau(+)tau(-)channel, the value obtained for the limit is 7.44 x 10(-24) cm(3) s(-1 )for the Navarro-Frenk-White halo profile. For the purpose of this joint analysis, the model parameters and the likelihood are unified, providing a benchmark for forthcoming dark matter searches performed by neutrino telescopes.

    • We report here an extension of the measurement of the all-particle cosmic-ray spectrum with IceTop to lower energy. The new measurement gives full coverage of the knee region of the spectrum and reduces the gap in energy between previous IceTop and direct measurements. With a new trigger that selects events in closely spaced detectors in the center of the array, the IceTop energy threshold is lowered by almost an order of magnitude below its previous threshold of 2 PeV. In this paper we explain how the new trigger is implemented, and we describe the new machine-learning method developed to deal with events with very few detectors hit. We compare the results with previous measurements by IceTop and others that overlap at higher energy and with HAWC and Tibet in the 100 TeV range.

    • Pulsar wind nebulae (PWNe) are the main gamma-ray emitters in the Galactic plane. They are diffuse nebulae that emit nonthermal radiation. Pulsar winds, relativistic magnetized outflows from the central star, shocked in the ambient medium produce a multiwavelength emission from the radio through gamma-rays. Although the leptonic scenario is able to explain most PWNe emission, a hadronic contribution cannot be excluded. A possible hadronic contribution to the high-energy gamma-ray emission inevitably leads to the production of neutrinos. Using 9.5 yr of all-sky IceCube data, we report results from a stacking analysis to search for neutrino emission from 35 PWNe that are high-energy gamma-ray emitters. In the absence of any significant correlation, we set upper limits on the total neutrino emission from those PWNe and constraints on hadronic spectral components.

    • Purpose – This article aims to focus attention on users of information technology (IT), especially mobile telephony. It focuses on what people actually say about mobile technology but also aims to pay attention to what they do not talk about, what is found in the silence, especially with new technology when much can be taken for granted. This latter is, according to Foucault, even more important to understand.Design/methodology/approach – The research draws on empirical research through 11 semi-structured interviews and interviews with five focus groups, comprising between four and eight care assistants in each group. The interviews were with three women and three men between 25-70 years old, five female public sector middle managers and care assistants from five focus groups at social services departments in the north of Sweden. A Foucauldian approach is adopted to interpret the findings and explore how their locations within the circuits of socio-technical networks engender uncertainty with mobile technology. The present spread of IT reinforces a belief that people are integrated into the circuits of socio-technical networks.Findings – The findings suggest, on the one hand, that new technologies like mobile communication can be used to organise our everyday lives, whilst, on the other, there are risks with the new technologies, which can discipline discourses.Originality/value – These issues are discussed from a sociological and informatics perspective.

    • This article discusses the so-called verb-oriented method and its role in the research project Gender and Work in early modern Sweden (GaW), which is based at Uppsala University. It provides a presentation of the GaW-database, which has been designed to allow analysis according to the verb-method. Finally, the article points out that this method can be combined with a number of different theoretical approaches as long as the focus is on practices. It is therefore compatible with the approaches of e.g., Judith Butler, Michel de Certeau, and Amartya Sen. Work is defined as "time-use with the purpose of making a living" and the article discusses why data on time-use, or actual work activities, are better suited for research into early modern Swedish working life than other types of data. It shows that activities are usually described in the sources by verb-phrases, and explains how and from what sources verb-phrases are collected and analyzed within the project. In order to allow for generalizations the verb-method presupposes large amounts of data. This is the rationale for the GaW-database, which at present includes around 5000 verb-phrases and 75000 data posts.

    • Approaches exploiting trait distribution extremes may be used to identify loci associated with common traits, but it is unknown whether these loci are generalizable to the broader population. In a genome-wide search for loci associated with the upper versus the lower 5th percentiles of body mass index, height and waist-to-hip ratio, as well as clinical classes of obesity, including up to 263,407 individuals of European ancestry, we identified 4 new loci (IGFBP4, H6PD, RSRC1 and PPP2R2A) influencing height detected in the distribution tails and 7 new loci (HNF4G, RPTOR, GNAT2, MRPS33P4, ADCY9, HS6ST3 and ZZZ3) for clinical classes of obesity. Further, we find a large overlap in genetic structure and the distribution of variants between traits based on extremes and the general population and little etiological heterogeneity between obesity subgroups.

    • Background The Molecular International Prognostic Scoring System (IPSS-M) is the new gold standard for diagnostic outcome prediction in patients with myelodysplastic syndromes (MDS). This study was designed to assess the additive prognostic impact of dynamic transfusion parameters during early follow-up. Methods We retrieved complete transfusion data from 677 adult Swedish MDS patients included in the IPSS-M cohort. Time-dependent erythrocyte transfusion dependency (E-TD) was added to IPSS-M features and analyzed regarding overall survival and leukemic transformation (acute myeloid leukemia). A multistate Markov model was applied to assess the prognostic value of early changes in transfusion patterns. Results Specific clinical and genetic features were predicted for diagnostic and time-dependent transfusion patterns. Importantly, transfusion state both at diagnosis and within the first year strongly predicts outcomes in both lower (LR) and higher-risk (HR) MDSs. In multivariable analysis, 8-month landmark E-TD predicted shorter survival independently of IPSS-M (p < 0.001). A predictive model based on IPSS-M and 8-month landmark E-TD performed significantly better than a model including only IPSS-M. Similar trends were observed in an independent validation cohort (n = 218). Early transfusion patterns impacted both future transfusion requirements and outcomes in a multistate Markov model. Conclusion The transfusion requirement is a robust and available clinical parameter incorporating the effects of first-line management. In MDS, it provides dynamic risk information independently of diagnostic IPSS-M and, in particular, clinical guidance to LR MDS patients eligible for potentially curative therapeutic intervention.

    • BACKGROUND: The World Health Organisation (WHO) 2013 diagnostic criteria for gestational diabetes mellitus (GDM) has been criticised due to the limited evidence of benefits on pregnancy outcomes in different populations when switching from previously higher glycemic thresholds to the lower WHO-2013 diagnostic criteria. The aim of this study was to determine whether the switch from previous Swedish (SWE-GDM) to the WHO-2013 GDM criteria in Sweden following risk factor-based screening improves pregnancy outcomes.METHODS AND FINDINGS: A stepped wedge cluster randomised trial was performed between January 1 and December 31, 2018 in 11 clusters (17 delivery units) across Sweden, including all pregnancies under care and excluding preexisting diabetes, gastric bypass surgery, or multifetal pregnancies from the analysis. After implementation of uniform clinical and laboratory guidelines, a number of clusters were randomised to intervention (switch to WHO-2013 GDM criteria) each month from February to November 2018. The primary outcome was large for gestational age (LGA, defined as birth weight >90th percentile). Other secondary and prespecified outcomes included maternal and neonatal birth complications. Primary analysis was by modified intention to treat (mITT), excluding 3 clusters that were randomised before study start but were unable to implement the intervention. Prespecified subgroup analysis was undertaken among those discordant for the definition of GDM. Multilevel mixed regression models were used to compare outcome LGA between WHO-2013 and SWE-GDM groups adjusted for clusters, time periods, and potential confounders. Multiple imputation was used for missing potential confounding variables. In the mITT analysis, 47 080 pregnancies were included with 6 882 (14.6%) oral glucose tolerance tests (OGTTs) performed. The GDM prevalence increased from 595/22 797 (2.6%) to 1 591/24 283 (6.6%) after the intervention. In the mITT population, the switch was associated with no change in primary outcome LGA (2 790/24 209 (11.5%) versus 2 584/22 707 (11.4%)) producing an adjusted risk ratio (aRR) of 0.97 (95% confidence interval 0.91 to 1.02, p = 0.26). In the subgroup, the prevalence of LGA was 273/956 (28.8%) before and 278/1 239 (22.5%) after the switch, aRR 0.87 (95% CI 0.75 to 1.01, p = 0.076). No serious events were reported. Potential limitations of this trial are mainly due to the trial design, including failure to adhere to guidelines within and between the clusters and influences of unidentified temporal variations.CONCLUSIONS: In this study, implementing the WHO-2013 criteria in Sweden with risk factor-based screening did not significantly reduce LGA prevalence defined as birth weight >90th percentile, in the total population, or in the subgroup discordant for the definition of GDM. Future studies are needed to evaluate the effects of treating different glucose thresholds during pregnancy in different populations, with different screening strategies and clinical management guidelines, to optimise women's and children's health in the short and long term.TRIAL REGISTRATION: The trial is registered with ISRCTN (41918550).

    • Mice deficient for the G protein subunit G alpha i2 spontaneously develop colitis, a chronic inflammatory disease associated with dysregulated T cell responses. We and others have previously demonstrated a thymic involution in these mice and an aberrant thymocyte dynamics. The G alpha i2(-/-) mice have a dramatically reduced fraction of double positive thymocytes and an increased fraction of single positive (SP) thymocytes. In this study, we quantify a number of critical parameters in order to narrow down the underlying mechanisms that cause the dynamical changes of the thymocyte development in the G alpha i2(-/-) mice. Our data suggest that the increased fraction of SP thymocytes results only from a decreased number of DP thymocytes, since the number of SP thymocytes in the Gai2(-/-) mice is comparable to the control littermates. By measuring the frequency of T cell receptor excision circles (TRECs) in the thymocytes, we demonstrate that the number of cell divisions the G alpha i2(-/-) SP thymocytes undergo is comparable to SP thymocytes from control littermates. In addition, our data show that the mature SP CD4(+) and CD8(+) thymocytes divide to the same extent before they egress from the thymus. By estimating the number of peripheral TREC+ T lymphocytes and their death rate, we could calculate the daily egression of thymocytes. G alpha i2(-/-) mice with no/mild and moderate colitis were found to have a slower export rate in comparison to the control littermates. The quantitative measurements in this study suggest a number of dynamical changes in the thymocyte development during the progression of colitis.

    • This study investigated organic laying hen farms for the presence of Erysipelothrix rhusiopathiae in the house environment and from potential carriers (i.e. insects and mice) during ongoing erysipelas outbreaks, and compared the obtained isolates with those from laying hens. The samples were investigated by selective culture followed by species-specific polymerase chain reaction on cultures. E. rhusiopathiae was isolated from the spleen, jejunal contents, manure, dust and swabs from water nipples. Three more samples from the house environment tested positive by polymerase chain reaction compared with selective culture alone. Selected isolates were investigated by pulsed-field gel electrophoresis (PFGE) and matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS). One farm was represented by isolates from laying hens only, and one of these isolates differed in one PFGE band from the others. Different banding patterns were observed for isolates from laying hens and manure on one farm. On the remaining two farms, the isolates from the house environment and laying hens were identical but differed between farms. Outbreaks reoccurred in the next flock on two of the farms, and different PFGE types were isolated from consecutive flocks. Our results suggest an external source of infection, which would explain the previously reported increased risk of outbreaks in free-range flocks. Contaminated manure and dust may represent sources of transmission. For the isolates, MALDI-TOF MS and biochemical typing results were in agreement but, since the type strain of Erysipelothrix tonsillarum was typed as E. rhusiopathiae using MALDI-TOF MS, further studies into this method are needed.

    • Smoking is a major heritable and modifiable risk factor for many diseases, including cancer, common respiratory disorders and cardiovascular diseases. Fourteen genetic loci have previously been associated with smoking behaviour-related traits. We tested up to 235,116 single nucleotide variants (SNVs) on the exome-array for association with smoking initiation, cigarettes per day, pack-years, and smoking cessation in a fixed effects meta-analysis of up to 61 studies (up to 346,813 participants). In a subset of 112,811 participants, a further one million SNVs were also genotyped and tested for association with the four smoking behaviour traits. SNV-trait associations with P < 5 × 10-8 in either analysis were taken forward for replication in up to 275,596 independent participants from UK Biobank. Lastly, a meta-analysis of the discovery and replication studies was performed. Sixteen SNVs were associated with at least one of the smoking behaviour traits (P < 5 × 10-8) in the discovery samples. Ten novel SNVs, including rs12616219 near TMEM182, were followed-up and five of them (rs462779 in REV3L, rs12780116 in CNNM2, rs1190736 in GPR101, rs11539157 in PJA1, and rs12616219 near TMEM182) replicated at a Bonferroni significance threshold (P < 4.5 × 10-3) with consistent direction of effect. A further 35 SNVs were associated with smoking behaviour traits in the discovery plus replication meta-analysis (up to 622,409 participants) including a rare SNV, rs150493199, in CCDC141 and two low-frequency SNVs in CEP350 and HDGFRP2. Functional follow-up implied that decreased expression of REV3L may lower the probability of smoking initiation. The novel loci will facilitate understanding the genetic aetiology of smoking behaviour and may lead to the identification of potential drug targets for smoking prevention and/or cessation.

    • One of the most widely used animal models for inflammatory bowel disease (IBD) is the dextran sulfate sodium (DSS)-induced colitis. We have previously reported that 5 days administration of DSS in C57Bl/6J mice induces a colonic inflammation that progresses into chronicity after DSS removal, whereas in BALB/cJ mice the inflammation resolves within 4 weeks post-DSS. Here we show that both thymic size and thymocyte numbers dramatically decreased in the acute phase of inflammation in C57Bl/6 mice, 7 days after DSS withdrawal. Mature, CD4(+) and CD8(+) single positive (SP) CD69(lo) CD62L(hi) thymocytes were enriched in these mice, accompanied by a major decrease in the number of immature double positive (DP) thymocytes. However, the different maturation stages within the DP thymocyte subset were unchanged between healthy and inflamed C57Bl/6J mice. Interestingly, as the inflammation progressed into the chronic phase, the thymus recovered and 2 weeks after the acute inflammatory phase all the thymic parameters investigated in this study were restored to normal. In contrast, BALB/cJ mice only develop mild thymic alterations. Nevertheless, we found that within the double negative (DN) thymocytes an increased frequency and also total numbers of CD44(+) CD25(-) (DN1) cells correlated with the severity of colitis, and that the frequency of CD44(-) CD25(-) (DN4) thymocytes decreased proportionally in the acute phase in BALB/cJ mice. Our observations suggest that the thymic effects are intimately connected to the intestinal inflammatory response in colitis regardless of the inflammatory stimuli.

The aim of this study was to evaluate colon wall thickness as a noninvasive marker in the dextran sodium sulfate (DSS) mouse model of colitis using micro-CT. METHODS: Mice were examined by micro-CT 1, 2, or 4 times between day 0 (d0) and d26 after induction of colitis to document the kinetics of changes in colon wall thickness and its relation to colitis development. RESULTS: DSS-treated mice displayed a significantly thicker colon wall at all timepoints (days 5, 8, 12, 19, and 26) investigated compared to healthy controls. Colon wall thickness showed a good correlation to the macroscopic grading of colitis (r = 0.81). The increase in colon wall thickness occurred mainly during the acute phase of colitis (between days 5 and 12) and did not progress much further in the chronic phase of colitis (d26). Colon wall thickness at d26 was thereby predicted by measurements at d12. All mice did not respond equally to DSS and this difference was manifest during the first 2 weeks of colitis, providing an important tool in stratifying responders from nonresponders. CONCLUSIONS: While the potential impact of handling and anesthesia should be considered on repeated micro-CT, irradiation exposure during repeated micro-CT did not affect the development of colitis. Thus, the results suggest that micro-CT can be used for monitoring and prediction of the inflammatory response in mouse colitis in future therapeutic studies.

    • The aim of this study was to investigate the relevance of mouse ex vivo cultures as a first screening model for new therapeutic agents of Inflammatory Bowel Disease (IBD). Two murine models (dextran sodium sulphate (DSS)-induced colitis and Gαi2-deficient mice) and two anti-inflammatory agents (methyl-prednisolone and the proteasome inhibitor MG132) were evaluated. The in vivo effects of methyl-prednisolone were assessed in both models. Ex vivo colonic tissue from both mouse models were cultured in the presence or absence of the drugs and TaqMan Low-Density arrays were used to assess the regulation of inflammatory genes before and after drug treatment. Colitis induced a similar inflammatory gene profile in both mouse models in in vivo studies and in ex vivo cultures. The differences encountered reflected the different phases of colitis in the models, e.g. innate cytokine/chemokine profile in the DSS model and T cell related markers in Gαi2-deficient mice. After steroid treatment, a similar pattern of genes was suppressed in the two mouse models. We confirmed the suppression of inflammatory gene expression for IL-1β, IL-6 and iNOS in ex vivo and in vivo colons from both mouse models by quantitative RT-PCR. Importantly, the inflammatory responses in the murine ex vivo culture system reflected the in vivo response in the inflamed colonic tissue as assessed by changes in inflammatory gene expression, suggesting that the murine culture system can be used for validation of future IBD therapies.

    • Including the voices and knowledge of service users is essential for developing recovery-oriented and evidence-based mental health services. Recent studies have however, suggested that challenges remain to the legitimization of user knowledge in practice. To further explore such challenges, a co-production study was conducted by a team of researchers and representatives from user organizations in Sweden. The aim of the study was to explore the barriers and facilitators to the legitimacy of user knowledge, as a central factor in sustainably implementing user influence in mental health practice. A series of workshops, with representatives of mental health services and user organizations were conducted by the research team to explore these issues. The analysis built on the theoretical framework of epistemic injustice, and the underlying aspects, testimonial, hermeneutic and participation-based injustice, were utilized as a framework for a deductive analysis. Results suggest that this is a useful model for exploring the complex dynamics related to the legitimacy of user knowledge in mental health systems. The analysis suggests that the legitimacy of user knowledge is related to the representativeness of the knowledge base, the systematic formulation of this knowledge inapplicable methods, access to resources and positions within the mental health system and participation in the process of integrating this knowledge-base in mental health contexts. Legitimizing user knowledge in practice additionally challenges mental health systems to support readiness for change in working environments and to address the power and role issues that these changes involve.
