  • Trends in cutaneous malignant melanoma in Sweden 1997-2011: Thinner tumours and improved survival among men

    • Background: Both patient survival and the proportion of patients diagnosed with thin cutaneous malignant melanoma (CMM) have been steadily rising in Sweden as in most western countries, though the rate of improvement in survival appears to have declined in Sweden at the end of last millennium.Objectives: To analyse the most recent trends in the distribution of tumour thickness (T-category) as well as CMM-specific survival in Swedish patients diagnosed 1997-2011.Methods: This nationwide population-based study included 30 590 patients registered in the Swedish Melanoma Register (SMR) and diagnosed with a first primary invasive CMM 1997-2011. The patients were followed through 2012 in the national Cause-of-Death Register.Results: Logistic and Cox regression analyses adjusting for age at diagnosis, tumour site, and health care region were carried out. The odds ratio for being diagnosed with thicker tumours was significantly reduced (P = 0·0008) and the CMM-specific survival significantly improved in men diagnosed 2007-2011 compared to men diagnosed 1997-2001 (hazard ratio=0·81; 95% CI 0·72-0·91, P = 0·0009) while the corresponding differences for women were not significant. Women were diagnosed with significantly thicker tumours during 2002-2006 and a tendency towards decreased survival was observed compared to those diagnosed earlier 1997-2001 and later 2007-2011.Conclusion: In Sweden, the CMMs of men are detected earlier over time and this seems to be followed by an improved CMM-specific survival for men. Women are still diagnosed with considerably thinner tumours and they experience a better survival than men.

  • Transfer of morphine along the olfactory pathway to the central nervous system after nasal administration to rodents

    • The aim of this study was to investigate whether morphine can be transferred along the olfactory pathway to the CNS, thereby circumventingthe blood–brain barrier, after nasal administration to rodents. Radiolabelled and unlabelled morphine were administered via the right nostrilto mice and rats. Olfactory bulbs, brain tissue and blood samples were collected. Morphine-derived radioactivity was measured using liquidscintillation (LS) and the concentrations of morphine and its metabolite morphine-3-glucuronide (M3G) were also assessed with highperformanceliquid chromatography. The location of morphine-derived radioactivity in the rat brain was visualised by autoradiography.Overall, the levels of morphine in the right olfactory bulbs (ROBs) significantly exceeded those in the left olfactory bulbs (LOBs) and braintissue samples 15, 60 and 240 min after right-sided nasal administration. Fifteen minutes after intravenous administration, there were nosignificant differences between olfactory bulbs and the other brain areas. Five minutes after nasal administration, autoradiography revealedradioactivity surrounding the ROB and reaching one of the ventricles in the brain. After 60 min, radioactivity had reached the peripheral partsof the ROB. All the techniques used in this study demonstrate that morphine was transferred along the olfactory pathway to the CNS afternasal administration to rodents.

    • The main aim of the Swedish physical activity and fitness cohort study (SPAF-1958) was to describe physical fitness, physical activity, health, and lifestyle across part of the lifespan, and to assess the influences on these factors from the environment, personal factors, and genetics. There is inevitable dropout from longitudinal studies, and it may be systematic. The aim of this first paper of the second follow-up of SPAF-1958 was to provide a dropout analysis to consider to what extent the participants, at 52 years of age, remain a representative sample of the original adolescent study population. Additional aims were to provide an overview of the study protocol and the ongoing study population. Ongoing study participants in SPAF born in 1958 were, at the second follow-up at the age of 52, still representative of the study cohort in terms of sex, adolescent geographical area, upper secondary school program, adolescent body composition, muscular strength, and muscular endurance. However, a higher physical activity and, among women, a higher aerobic capacity in adolescence decreased the risk for dropout. It is important when interpreting results from longitudinal studies to adjust for the systematic dropout that could bias the conclusions drawn from the results.

    • Jämställdhetsarbete väcker ofta motstånd, det har de flesta som arbetar med jämställdhet erfarenhet av. Men vilka strategier kan förändringsaktörer använda för att bemöta motstånd och komma vidare i jämställdhetsarbetet, är frågan som utforskas i denna bok.Jämställdhetsarbete berör ofta angelägna frågor där det kan finnas motstridiga intressen, till exempel kring fördelning av resurser, inflytande och ansvar. Inte sällan uppstår starka känslor hos människor. En del visar öppet sin oro. Andra praktiserar förnekelse på olika sätt. Vissa blir aggressiva. Många protesterar i tysthet. Denna ovilja mot förändring brukar formuleras i termer av motstånd och kan alltså komma till uttryck på många sätt.I boken ges ett perspektiv på makt och motstånd som syftar till att bidra till upplevelser av ett större handlingsutrymme för att möta motstånd mot jämställdhetsarbete. Ingången består av material som har skapats i ett forskningsprojekt där jämställdhetsarbetare, medarbetare och chefer, från myndigheter och kommuner har utforskat motstånd och strategier tillsammans med genusforskare. Materialet ger en inblick i vad det kan innebära att arbeta med jämställdhet, vad en förändringsaktör möter och förväntas hantera i jämställdhetsarbetet. Ett bidrag i boken är en rad av idéer på handlingsalternativ för att möta motstånd, som mejslats fram i olika former i projektet. Bokens bidrag handlar också om att förvalta de kunskaper som skapats gemensamt och hur teori kan hjälpa oss att fördjupa, förstå och agera.Boken är skriven av fyra genusforskare vid Karlstads universitet respektive Göteborgs universitet och riktar sig till alla som är intresserade av jämställdhet, förändring och utbildning i organisationer och verksamheter.

    • Som en del i utvärderingen av hur väl visioner och målsättningar för Västra Götaland uppnås, har den här rapporten fokuserat på en rad frågor som tillsammans speglar de vanor och attityder som finns hos invånarna i regionen. Med utgångspunkt i 2016 års västsvenska SOM-undersökning har medborgarnas demokratisyn, förtroende för politiker, utvärdering av Västra Götalandsregionens prestationer samt åsikter om infrastruktur och sjukvård kartlagts på detaljerad nivå. Vidare har invånarnas arbetsliv, livsstil och självskattade hälsa analyserats. Resultaten visar på många likheter i regionen men också på stora olikheter, varav många är direkt kopplade till socioekonomiska faktorer, utbildning och ålder. I detta avslutande kapitel sammanfattas och relateras rapportens resultat till Vision Västra Götaland – det goda livet.

    • The aim of this study was to investigate which principal human cytochrome P450 (CYP450) enzymes are affected by artemisinin and to what degree the artemisinin derivatives differ with respect to their respective induction and inhibition capacity. Seventy-five healthy adults were randomized to receive therapeutic oral doses of artemisinin, dihydroartemisinin, arteether, artemether or artesunate for 5 days (days 1–5). A six-drug cocktail consisting of caffeine, coumarin, mephenytoin, metoprolol, chlorzoxazone and midazolam was administered orally on days −6, 1, 5 and 10 to assess the activities of CYP1A2, CYP2A6, CYP2C19, CYP2D6, CYP2E1 and CYP3A, respectively. Four-hour plasma concentrations of parent drugs and corresponding metabolites and 7-hydroxycoumarin urine concentrations were quantified by liquid chromatography-tandem mass spectrometry. The 1-hydroxymidazolam/midazolam 4-h plasma concentration ratio (CYP3A) was increased on day 5 by artemisinin [2.66-fold (98.75% CI: 2.10–3.36)], artemether [1.54 (1.14–2.09)] and dihydroartemisinin [1.25 (1.06–1.47)] compared with day −6. The S-4'-hydroxymephenytoin/S-mephenytoin ratio (CYP2C19) was increased on day 5 by artemisinin [1.69 (1.47–1.94)] and arteether [1.33 (1.15–1.55)] compared with day −6. The paraxanthine/caffeine ratio (CYP1A2) was decreased on day 1 after administration of artemisinin [0.27 (0.18–0.39)], arteether [0.70 (0.55–0.89)] and dihydroartemisinin [0.73 (0.59–0.90)] compared with day −6. The α-hydroxymetoprolol/metoprolol ratio (CYP2D6) was lower on day 1 compared with day −6 in the artemisinin [0.82 (0.70–0.96)] and dihydroartemisinin [0.83 (0.71–0.96)] groups, respectively. In the artemisinin-treated subjects this decrease was followed by a 1.34-fold (1.14–1.58) increase from day 1 to day 5. These results show that intake of artemisinin antimalarials affect the activities of several principal human drug metabolizing CYP450 enzymes. Even though not significant in all treatment groups, changes in the individual metrics were of the same direction for all the artemisinin drugs, suggesting a class effect that needs to be considered in the development of new artemisinin derivatives and combination treatments of malaria.

    • A method to determine the absolute activity of 137Cs in irradiated nuclear fuel is presented. Using a well-known point-like calibration source in combination with measurements of the gamma-ray intensity from the nuclear fuel and Monte Carlo calculations based on the nominal measurement geometry, the activity content can be determined without prior knowledge of the intrinsic detection efficiency of the gamma-ray detector. The presented method is tested using measurements of the 137Cs intensity from spent nuclear fuel of the pressurized water type at the central interim storage in Sweden. Using an assumption of homogeneous distribution of 137Cs throughout the fuel, we demonstrate a linear relationship between measured activity and the activity calculated by a state-of-the-art simulation code. For future studies, we suggest some factors that potentially can decrease the uncertainty in the correlation between measured and calculated activity.

  • Action research improved general prerequisites for evidence-based practice

    • The present study was part of an action research project that was performed to implement a clinical pathway for patients on mechanical ventilation and simultaneously explore the implementation process in a Swedish intensive care unit. The aim of this questionnaire study was to evaluate whether an action research methodology could affect the general prerequisites for evidence-based practice (EBP). Informed by the Promoting Action on Research Implementation in Health Services (PARIHS) framework, the study included registered nurses, assistant nurses and anesthesiologists in the unit at start of the project (n = 50) and at follow-up (n = 44). Data was collected with the Evaluation Before Implementation Questionnaire and the Attitudes towards Guidelines Scale. The results revealed that the general prerequisites for EBP in the setting improved. Compared to baseline measurements, the staff at follow-up conversed significantly more about the importance of the patients' experiences, research utilization, context and facilitation, while changes with respect to clinical experiences were not significant. The attitudes towards guidelines were perceived as positive at baseline as well as at follow-up and did not significantly change. Longer professional experience was associated with a slightly lower probability of perceiving that the importance of research utilization was discussed and reflected upon, while belonging to a profession with longer education was associated with a higher probability of this perception. Compared to registered nurses and assistant nurses, the anesthesiologists perceived, to a greater extent, that the importance of clinical experience was discussed and reflected upon in the setting, while there was no significant association with the length of professional experience and/or specific professions regarding the other components. In conclusion, using action research to implement a clinical pathway methodology seems to set in motion various mechanisms that improve some but not all prerequisites that, according to the PARIHS framework, are advantageous for EBP.

  • Balancing intertwined responsibilities: A grounded theory study of teamwork in everyday intensive care unit practice

    • This study aimed to describe and explain teamwork and factors that influence team processes in everyday practice in an intensive care unit (ICU) from a staff perspective. The setting was a Swedish ICU. Data were collected from 38 ICU staff in focus groups with registered nurses, assistant nurses, and anaesthetists, and in one individual interview with a physiotherapist. Constant comparative analysis according to grounded theory was conducted, and to identify the relations between the emerged categories, the paradigm model was applied. The core category to emerge from the data was balancing intertwined responsibilities. In addition, eleven categories that related to the core category emerged. These categories described and explained the phenomenon's contextual conditions, causal conditions, and intervening conditions, as well as the staff actions/interactions and the consequences that arose. The findings indicated that the type of teamwork fluctuated due to circumstantial factors. Based on the findings and on current literature, strategies that can optimise interprofessional teamwork are presented. The analysis generated a conceptual model, which aims to contribute to existing frameworks by adding new dimensions about perceptions of team processes within an ICU related to staff actions/interactions. This model may be utilised to enhance the understanding of existing contexts and processes when designing and implementing interventions to facilitate teamwork in the pursuit of improving healthcare quality and patient safety.

  • Clinical Pathway Implementation and Teamwork in Swedish Intensive Care : Challenges in Evidence-Based Practice and Interprofessional Collaboration

    • Suboptimal quality of care is an evident issue in current healthcare services. Clinical pathways (CPs) have the potential to facilitate evidence-based practice and interprofessional teamwork, and thereby improve patient safety and quality of care.The overall aim of the thesis was to develop comprehensive empirical knowledge and understanding of CP implementation and teamwork in Swedish intensive care units (ICUs). Four studies were included (I-IV).Study I was a survey including all Swedish ICUs (N84) and a document analysis of CP examples (n12). In total, 17 (20%) ICUs used CPs and many had implementation plans. The quality, extent and content of the CPs (n56) varied greatly, with sometimes insufficient interprofessionalism, evidence base and renewal.Study II was a mixed method including ICUs using CPs. The implementation processes were retrospectively explored through questionnaire data (n15) and qualitative content analysis of interviews with key informants (n10). The CP implementation was revealed as a process directed at realizing the usefulness and creating new habits, which requires enthusiasm, support and time.Studies III and IV were grounded theory studies in an action research project in an ICU. Study III explored everyday teamwork through focus group interviews with registered nurses, assistant nurses and anesthesiologists, as well as an individual interview with a physiotherapist (n38). Teamwork was revealed as an act of ‘balancing intertwined responsibilities.’ The type of teamwork fluctuated as the team processes were affected by circumstantial factors and involved individuals. Study IV prospectively explored the implementation process of a CP during a five-year period through repeated focus groups and individual interviews, questionnaires and logbooks/field notes, including the interprofessional project group, staff and managers (n71), and retrospective screening of health records (n136). ‘Struggling for a feasible tool’ was revealed as a central phenomenon. The implementation process included contextual and processual circumstances that enforced negotiations to achieve progress, which made the process tentative and prolonged and had consequences on the process output.In conclusion, CP implementation processes are affected by multiple interplaying factors. Although progress has been achieved in evidence-based practice and interprofessional collaboration there is still potential for substantial improvements, emphasizing a need for further facilitation.

    • Rationale, aims and objectives Clinical pathways (CPs) can improve quality of care on intensive care units (ICUs), but are infrequently utilized and of varying quality. Knowledge regarding factors that facilitate versus hinder successful implementation of CPs is insufficient and a better understanding of the activities and individuals involved is needed. The aim of this study was to explore the implementation process of CPs within the context of ICUs. Methods An exploratory design with a sequential mixed method was used. A CP survey, including all Swedish ICUs, was used to collect quantitative data from ICUs using CPs (n = 15) and interviews with key informants (n = 10) were used to collect qualitative data from the same ICUs. Descriptive statistics and qualitative content analysis were used, and the quantitative and qualitative findings were integrated. Results The CP implementation was conceptualized according to two interplaying themes: a process to realize the usefulness of CPs and create new habits; and a necessity of enthusiasm, support and time. Multiple factors affected the process and those factors were organized in six main categories and 14 subcategories. Conclusions Bottom-up initiatives, interprofessional project groups and small ICUs seem to enhance successful implementation of CPs while inadequate electronic health record systems, insufficient support and time constrains can be barriers. Support regarding the whole implementation process from centralized units at the local hospitals, as well as cooperation between ICUs and national guidance, has the potential to raise the quality of CPs and benefit the progress of CP implementation.

    • RATIONALE, AIMS AND OBJECTIVESTo identify the prevalence of clinical pathways (CPs) in Swedish intensive care units (ICUs) and to explore the quality, content and evidence base of the documents.METHODSA descriptive and explorative survey of all Swedish ICUs (N84) and a review of submitted examples of CPs (n12) were conducted.RESULTSCPs were in use at 20% of the Swedish ICUs. There was a significant geographic variation but no relationship between the use of CPs and category of hospital, type of ICU, size of ICU or type of health record applied. In total, 56 CPs were reported within a range of scopes and extensions. The content of the ICUs' CPs, as well as the degree to which they were interprofessional, evidence based, and renewed varied.CONCLUSIONSProgress has been made in relation to CPs in recent years, but there is potential for further improvements. None of the ICUs had CPs that contained all key characteristics of a high-quality, interprofessional and evidence-based CP identified in the literature. Greater knowledge sharing and cooperation within the field would be beneficial, and further research is needed.

  • Struggling for a feasible tool - the process of implementing a clinical pathway in intensive care: a grounded theory study

    • BackgroundClinical pathways can enhance care quality, promote patient safety and optimize resource utilization. However, they are infrequently utilized in intensive care. This study aimed to explain the implementation process of a clinical pathway based on a bottom-up approach in an intensive care context.MethodsThe setting was an 11-bed general intensive care unit in Sweden. An action research project was conducted to implement a clinical pathway for patients on mechanical ventilation. The project was managed by a local interprofessional core group and was externally facilitated by two researchers. Grounded theory was used by the researchers to explain the implementation process. The sampling in the study was purposeful and theoretical and included registered nurses (n31), assistant nurses (n26), anesthesiologists (n11), a physiotherapist (n1), first- and second-line managers (n2), and health records from patients on mechanical ventilation (n136). Data were collected from 2011 to 2016 through questionnaires, repeated focus groups, individual interviews, logbooks/field notes and health records. Constant comparative analysis was conducted, including both qualitative data and descriptive statistics from the quantitative data.ResultsA conceptual model of the clinical pathway implementation process emerged, and a central phenomenon, which was conceptualized as Struggling for a feasible tool,' was the core category that linked all categories. The phenomenon evolved from the Triggers' (Perceiving suboptimal practice' and Receiving external inspiration and support'), pervaded the Implementation process' (Contextual circumstances,' Processual circumstances' and Negotiating to achieve progress'), and led to the process Output' (Varying utilization' and Improvements in understanding and practice'). The categories included both facilitating and impeding factors that made the implementation process tentative and prolonged but also educational.ConclusionsThe findings provide a novel understanding of a bottom-up implementation of a clinical pathway in an intensive care context. Despite resonating well with existing implementation frameworks/theories, the conceptual model further illuminates the complex interaction between different circumstances and negotiations and how this interplay has consequences for the implementation process and output. The findings advocate a bottom-up approach but also emphasize the need for strategic priority, interprofessional participation, skilled facilitators and further collaboration.

    • Sensitive and reproducible methods for the determination of oxycodone, oxymorphone and noroxycodone in Ringer solution, rat plasma and rat brain tissue by liquid chromatography/mass spectrometry are described. Deuterated analogs of the substances were used as internal standards. Samples in Ringer solution were analyzed by direct injection of 10 microL Ringer solution diluted by an equal volume of water. The limit of quantification was 0.5 ng/mL and the method was linear in the range of 0.5-150 ng/mL for all substances. To analyze oxycodone and oxymorphone in rat plasma, 50 microL of plasma were precipitated with acetonitrile, and the supernatant was directly injected onto the column. To analyze oxycodone, oxymorphone and noroxycodone in rat plasma, 100 microL of rat plasma were subjected to a C18 solid-phase extraction (SPE) procedure, before reconstituting in mobile phase and injection onto the column. For both methods the limit of quantification in rat plasma was 0.5 ng/mL and the methods were linear in the range of 0.5-250 ng/mL for all substances. To analyze the content of oxycodone, oxymorphone and noroxycodone in rat brain tissue, 100 microL of the brain homogenate supernatant were subjected to a C18 SPE procedure. The limit of quantification of oxycodone was 20 ng/g brain, and for oxymorphone and noroxycodone 4 ng/g brain, and the method was linear in the range of 20-1000 ng/g brain for oxycodone and 4-1000 ng/g brain for oxymorphone and noroxycodone. All methods utilized a mobile phase of 5 mM ammonium acetate in 45% acetonitrile, and a SB-CN column was used for separation. The total run time of all methods was 9 min. The intra-day precision and accuracy were <11.3% and <+/-14.9%, respectively, and the inter-day precision and accuracy were <14.9% and <+/-6.5%, respectively, for all the concentrations and matrices described.

    • The composition of serum proteins is reflecting the current health status and can, with the right tools, be used to detect early signs of disease, such as an emerging cancer. An earlier diagnosis of cancer would greatly increase the chance of an improved outcome for the patients. However, there is still an unmet need for proficient tools to decipher the information in the blood proteome, which calls for further technological development. Here, we present a proof-of-concept study that demonstrates an alternative approach for multiplexed protein profiling of serum samples in solution, using DNA barcoded scFv antibody fragments and next generation sequencing. The outcome shows high accuracy when discriminating samples derived from pancreatic cancer patients and healthy controls and represents a scalable alternative for serum analysis. Brofelth, Ekstrand et al use DNA barcoded scFv antibody fragments and next generation sequencing for multiplex profiling of proteins in serum from pancreatic cancer patients with high accuracy. This approach can potentially be used in high throughput precision diagnosis.

    • Malignant gastrointestinal stromal tumours (GIST) have a poor prognosis. Since these tumours are resistant to conventional radiation and chemotherapy, surgery has been the mainstay of treatment. However, surgery is usually inadequate for the treatment of malignant GIST. Imatinib, a KIT tyrosine kinase inhibitor, has recently been found to have a dramatic antitumour effect on GIST. In this centre-based study of 17 consecutive patients with high-risk or overtly malignant GIST, imatinib was used in three different settings - palliatively, adjuvantly, and neoadjuvantly. The treatment was found to be safe and particularly effective in tumours with activating mutations of exon 11 of the KIT gene. Clinical response to imatinib treatment correlated morphologically to tumour necrosis, hyalinisation, and reduced proliferative activity. The value of neoadjuvant imatinib treatment was illustrated in one case.

    • PURPOSE: The aim of the study was to investigate whether dopamine is transferred along the olfactory pathway to the brain following nasal administration to mice. METHODS: [3H]-Dopamine was administered nasally or intravenously to female mice. Brain tissue samples were excised and the radioactive content was measured. The precise localisation of dopamine radioactivity in the brain was studied using autoradiography. The presence of dopamine or its metabolites in the olfactory bulb and mucosa was ascertained using thin layer chromatography (TLC). RESULTS: After administration of [3H]-dopamine into the right nostril, the amount of dopamine in the right bulb increased with time until. after 4 h, it was 27 times higher than in the left bulb. Among the other brain tissue samples, significantly higher amount of radioactivity was detected in the lateral olfactory tract. Radioactivity in the right olfactory bulb was shown by autoradiography to be selectively located in the peripheral layers 1 to 4 h after administration. Selective uptake of radioactivity was not seen in other regions of the brain. TLC data indicated that approximately 75% and 10% of the radioactivity in the olfactory bulb and mucosa, respectively, coeluted with dopamine. CONCLUSIONS: The results indicate that unchanged dopamine is transferred into the olfactory bulb following nasal administration of [3H]-dopamine.

    • The aim was to develop a model to describe the population pharmacokinetics of nevirapine in South African human immunodeficiency virus (HIV)-infected patients who were taking nevirapine-based antiretroviral therapy concomitantly or in the absence of rifampicin-based tuberculosis therapy. Patients were divided into two groups: (1) patients receiving   nevirapine-containing antiretroviral regimen (200 mg twice daily) and continuation phase rifampicin-containing tuberculosis therapy (n = 27) in whom blood samples were obtained before and not less than 14 days after they completed tuberculosis therapy; (2) patients without tuberculosis who were receiving a nevirapine-containing antiretroviral regimen for at least 3 weeks (n = 26). The population pharmacokinetics of nevirapine was described using nonlinear mixed effects modelling   with NONMEM software. Based on the developed model, plasma concentration profiles after 300, 400 and 500 mg of nevirapine twice daily were simulated. Concomitant administration of rifampicin increased nevirapine oral clearance (CL/F) by 37.4% and reduced the absorption rate constant (k(a)) by almost sixfold. Rifampicin reduced the nevirapine average minimum concentration by 39%. Simulated doses of 300 mg twice daily elevated nevirapine concentrations above subtherapeutic levels in most patients, with minimum exposure above the recommended maximum concentration. The area under the concentration-time curve of 12-hydroxynevirapine was not different in the presence of rifampicin. 2-, 3- and 8-Hydroxynevirapine were not detectable (LLOQ = 0.025 mg/L). The developed model adequately describes nevirapine population   pharmacokinetics in a South African population when taken with/and in the absence of rifampicin treatment. The simulations suggest that an increased dose of 300 mg twice daily would achieve adequate nevirapine concentrations in most patients during rifampicin-containing treatment for tuberculosis.

    • This thesis investigates 5,000 descriptions of men and women in search for a life partner of the opposite sex through classifieds 1890-1980. The advertisers’ descriptions of who they are and what they want sketches a picture of the expectations everyday people have had on how men and women should be over almost a hundred years. Previous accounts of 20th century gender relations are based on governmental and political materials.The bipartite structure of the personals advertisements is used methodologically to differentiate between the so-called “ego-descriptions” and the “alter-descriptions” - what the advertiser had to offer and what the advertiser wanted. Men's descriptions of themselves – ego descriptions – is compared to women's descriptions of the man – alter description –, and women’s descriptions of themselves – ego-description – is compared to men’s descriptions of women – alter description. Six categories were created to capture the content of the personal advertisements: 1) Breadwinner qualifications 2) Parenting qualifications 3) Homemaker qualifications 4) Bodily aspects 5) Leisure 6) Personal interests and qualifications.This study shows that men and women increasingly are described on the grounds of similar qualifications, and that both men and women are increasingly depicted with characteristics that earlier was coded as female. It also shows that children and the family became more important. Stability and security were highly appreciated in the beginning of the period, but gave way to values as mobility and opportunity in the late 1900’s. Both women and men shift from favouring material conditions to an increased focus on feelings and thoughts, and in the end of the period the life partner relationship was more about emotional closeness and the company of a like-minded individual.However, despite these more general results, it is clear that neither the images of the man nor the images woman was unambiguous, and that men and women for most of the time didn't share the same ideals. When thousands of men and women describe their dreams and needs based on the lives they live, unlike government surveys, for example, a multitude of images of the nineteenth century man and woman appear.

    • Aim: To describe telenurses’ experiences of difficult calls.Design: A qualitative approach with a descriptive design was used to gain a deeper understanding of the telenurses’ experiences.Methods: The data were collected in spring 2017 through semi-structured interviews with 19 telenurses at call centres and primary healthcare centres and were analysed with qualitative content analysis.Results: Becoming emotionally concerned is central to the telenurse’s experiences of difficult calls. Difficult calls are accompanied by feelings such as inadequacy, uncertainty and anxiety, which can be described as emotional tension. Emotional tension refers to situations when the caller’s expressed emotions were conveyed to the telenurses and altered their state of mind. The telenurses stated that difficult calls that cause them to become anxious remain in their thoughts and go through their minds repeatedly, making a deep impression.

    • There is a need for accurate and efficient methods for quantification and characterisation of forest edges at the landscape level in order to understand and mitigate the effects of forest fragmentation on biodiversity.We present and evaluate a method for collecting detailed data on forest edges in aerial photographs by using line intersect sampling (LIS). A digital photogrammetric system was used to collect data from scanned colour infrared photographs in a managed boreal forest landscape.We focused on high-contrast edges between forest (height ≥10 m) and adjoining open habitat or young, regenerating forest (height ≤5 m). We evaluated the air photo interpretation with respect to accuracy in estimated edge length, edge detection, edge type classification and structural variables recorded in 20 m radius plots, using detailed field data as reference. The estimated length of forest edge in the air photo interpretation (52 ± 8.8 m ha-1; mean ± standard error) was close to that in the field survey (58 ± 9.3 m ha-1). The accuracy in edge type classification (type of open habitat) was high (88% correctly classified). Both tree height and canopy cover showed strong relationships with the field data in the forest, buttree height was underestimated by 2.3 m. Data collection was eight times faster and five times more cost-efficient in aerial photographs than in field sampling. The study shows that line intersect sampling in aerial photographs has large potential as a general tool for collecting detailed information on the quantity and characteristics of high-contrast edges in managed forest ecosystems.

    • Many of the photosynthetic genes are conserved among all higher plants, indicating that there is strong selective pressure to maintain the genes of each protein. However, mutants of these genes often lack visible growth phenotypes, suggesting that they are important only under certain conditions or have overlapping functions. To assess the importance of specific genes encoding the light-harvesting complex (LHC) proteins for the survival of the plant in the natural environment, we have combined two different scientific traditions by using an ecological fitness assay on a set of genetically modified Arabidopsis plants with differing LHC protein contents. The fitness of all of the LHC-deficient plants was reduced in some of the growth environments, supporting the hypothesis that each of the genes has been conserved because they provide ecological flexibility, which is of great adaptive value given the highly variable conditions encountered in nature.

    • The light-harvesting antenna of higher plant photosystem (PS) I is known to be composed of four different types of light-harvesting complex (LHC) proteins (Lhca1–4). However, the genomic sequence of Arabidopsis thaliana contains open reading frames coding for two additional LHC type proteins (Lhca5–6) that are presumably associated with PSI. While Lhca6 might not be expressed at all, ESTs have been detected for the Lhca5 gene in Arabidopsis and a number of other plant species. Here we demonstrate the presence of the Lhca5 gene product in the thylakoid membrane of Arabidopsis as an additional type of Lhca-protein associated with PSI. Lhca5 seems to be regulated differently from the other LHC proteins since Lhca5 mRNA levels increase under high light conditions. Analyses reported here of Lhca5 in plants lacking individual Lhca1–4 proteins show that it is more abundant in plants lacking Lhca1/4, and suggest that it interacts in a direct physical fashion with Lhca2 or Lhca3. We propose that Lhca5 binds chlorophylls in a similar fashion to the other Lhca proteins and is associated with PSI only in sub-stoichiometric amounts.

    • Vad behövs för att förstå arbetslivets och arbetsmarknadens genusbaserade segreger-ingsprocesser och ojämlika villkor på olika platser och i olika rum? Hur kan forskning som kombinerar perspektiven genus, plats och arbete bidra till ett arbetsliv som är hållbart och betydelsefullt för människor? Vilka frågor behöver ställas och besvaras i en kontext där arbetsmarknader och arbeten beskrivs som i ständig förändring, ofta i relation till megatrender som globalisering, urbanisering, digitalisering och automati-sering (till exempel Schwab 2016; OECD 2019)? Men där samtidigt skilda villkor och genusmässiga konsekvenser – här och nu – på lokala arbetsmarknader och i specifika organisationer och näringar synliggörs? Frågor om framtidens arbetsmarknad och arbetsliv handlar om komplexa skeenden på olika nivåer och platser och har med hela samhällsutvecklingen att göra. Vilka de största utmaningarna och förändring-arna är varierar men vi vet att de ofta får könade och genusifierade konsekvenser, att förändringar i arbetslivet drabbar kvinnor och män, rasifierade, migrerade, queera och funktionsvarierade olika, och att det hänger ihop med hur vi förstår ’arbete’. Till exempel att produktionen är överordnad reproduktionen, att normer om heltidsarbe-ten och lönearbeten osynliggör arbeten som utförs under osäkra och ’flexibla’ former. Även utbildningens förändrade roll villkorar människors möjligheter i både arbets- och vardagsliv, ofta i relation till plats/platser (till exempel Johansson med flera 2017; Dølvik och Steen 2018). Genom att samla bidrag som på olika sätt studerar och belyser arbete med utgångspunkt i teori om plats och rum i kombination med genusteori, vill vi visa hur denna forskning fortsatt har en viktig plats för att förstå de mekanis-mer och maktrelationer som skapar segregering i arbetslivet och i samhället i stort. Vi skriver fortsatt här för att vi anser att dessa perspektiv, som tidigare haft ett stort fokus inom såväl genus- och arbetslivsforskning som kulturgeografi, idag hamnat mer i skymundan.     • The aim of collaboration in municipal public sector is to ensure welfare and safety for its citizen, as well as to ensure economic sustainability in the municipality. Different forms of collaboration in the public sector can be an important part of sustaining a satisfactory service, especially in regions with a declining population, but also to give a faster and better service in all kind of regions. In this study we put focus on collaboration between municipal rescue service and home-help service, a collaboration that can be placed in civil and societal contingency and preparedness for unexpected events.Both the rescue service and the home-help service are highly gendered sectors. The point of departure for this study is that the gender coding of occupations and competences lay down conditions for the collaboration, why it is important to shed light on these codings and how they effect an effective collaboration and sustainable implementation. Our understanding is that gender is a social construction that is produced and reproduced through interaction. This construction have effect on how we are “made” as men and women, how we are formed as individuals, and how the society is both symbolic and structurally organized around what is conceived and perceived as male and female duties, occupations and places.The results of this study build on 30 interviews with staff in rescue services and home-care services (both employees and heads of these services). The results show that collaborations differ between municipalities, both what is included in the collaboration and how it is staged and performed, and that the processes of collaboration are complex and that they challenge ideal of organization, contents, responsibilities and who or whom should do different work tasks. We can see that conceptions of gender in male and female dominated work are often reproduced and reinforced in many of the studied cases, which hinder an effective service. But we can also notice that the collaboration by home-care service staff is perceived as a possibility for increased status and better resources when they collaborate with the rescue service.

This study contributes to this field by analysing a project for collaboration between the rescue service and the home-care service that has been introduced in a number of Swedish municipalities. The collaboration is intended to ensure welfare and safety for citizens, to guarantee a more efficient use of municipal resources, and to contribute to improved emergency management and civil protection. The rescue service and the home-care service are two clearly gender-coded occupations that also operate on gender-coded workplaces and places of work, which in our research project proved to be an important aspect of the (un)success of the collaboration processes. Furthermore, we argue that too little attention has been paid to the ways place affect the doing of gender in organisational studies. An overarching aim of the paper is therefore to contribute to gender studies of workplace and places of work, and more specific to discuss gendered obstacles and possibilities of the collaboration, in relation to place and work.

  • The Unchallenging Method is a Good One : about Measures for gender Equality and Diversity in the Swedish Rescue Service

    • Gender equality in the Swedish Rescue Services has been a political directive since the 1990ies, but has led to few visible changes. The Swedish Rescue Service is still a male-dominated organization with a traditional focus on firefighting. The call for change is made with references to new economic realities, conceptualizations of risk, and directives for gender and ethnic equality. The overriding concern of this paper is discursive constructions of equality work in relation to political directives on increasing equal opportunities in the Swedish Rescue Services. Gender is treated as a prevailing power structure and as an integral part of organizational practices. The paper is based on a questionnaire about the practical work made on equality in the Swedish Rescue Service, answered by over 1000 persons working in the Swedish Rescue Service. The questionnaire contains a large amount of open comments, which is analyzed in this paper. The result shows that two thirds of the respondents have a mainly positive attitude towards gender equality in the Rescue Service, but that the measures for equality are considered as "wrong methods". Our analysis discusses that only methods that doesn’t challenge the present organization are considered as acceptable.

    • Screening for gene copy-number alterations (CNAs) has improved by applying genome-wide microarrays, where SNP arrays also allow analysis of loss of heterozygozity (LOH). We here analyzed 10 chronic lymphocytic leukemia (CLL) samples using four different high-resolution platforms: BAC arrays (32K), oligonucleotide arrays (185K, Agilent), and two SNP arrays (250K, Affymetrix and 317K, Illumina). Cross-platform comparison revealed 29 concordantly detected CNAs, including known recurrent alterations, which confirmed that all platforms are powerful tools when screening for large aberrations. However, detection of 32 additional regions present in 2-3 platforms illustrated a discrepancy in detection of small CNAs, which often involved reported copy-number variations. LOH analysis using dChip revealed concordance of mainly large regions, but showed numerous, small nonoverlapping regions and LOH escaping detection. Evaluation of baseline variation and copy-number ratio response showed the best performance for the Agilent platform and confirmed the robustness of BAC arrays. Accordingly, these platforms demonstrated a higher degree of platform-specific CNAs. The SNP arrays displayed higher technical variation, although this was compensated by high density of elements. Affymetrix detected a higher degree of CNAs compared to Illumina, while the latter showed a lower noise level and higher detection rate in the LOH analysis. Large-scale studies of genomic aberrations are now feasible, but new tools for LOH analysis are requested.

    • The aim of this study was to gain a deeper understanding of the quality of daily living of persons with hyperacusis after a traumatic head injury 1 year after these persons had participated in a treatment programme at a Swedish hearing clinic. The study group consisted of 21 patients with a mean age of 36 years. In-depth interviews, conducted 1 year after the treatment programme, were analysed using the grounded theory method. Five emergent categories were labelled moderating vulnerability, awareness of restrictions, conditioned participation, structuring daily life and controlling mood changes. The core category, moderating vulnerability, describes the necessary balancing act between activity and recovery. The informants were aware of their new restrictions and managed daily life by structuring and planning each day in detail to minimize exposure to sensory stimuli. They had learned to prioritize their activities and, thereby, rationed their time. The evaluated programme appears to have positive effects and facilitates patients' adjustment process to hyperacusis with relatively restricted costs for the society.
