  • DNA launched suicidal flaviviruses as therapeutic vaccine candidates

The relativelyhigh cost of the HCV treatment brings concerns about the accessibility, especially in the developing countries. Hence, there exists a need for cost effect interventions with high efficiency. We aim to develop therapeutic vaccine candidates against HBV, HCV and HDV using DNA based subgenomic flavivirus replicons as a delivery system. Tick-borne encephalitis virus (TBEV), Langat virus (LGTV), West-Nile virus (WNV), or Kunjinvirus (KUNV) replicon with firefly luciferase geneas a reporter were expressed and characterized in cell culture studies. WNV and KUNV replicons showed significantly higher replication compared to their respective negative controls with unfunctional viral RNA dependent RNA polymerase. KUNV and WNV replicons were chosen for cloning the HCV or HB/DV vaccine candidate gene by replacing luciferasegene. Owing to the self-replicating trait of the flavivirus subgenomic replicons, Western blotting demonstrated that the antigen expression by KUNV and WNV replicons was several folds higher than the positive control. These results suggest that DNA based KUNV and WNV replicons may function as carriers for the hepatitis vaccine candidate genes, and these replicons are currently used for in vivostudies in animal models.

In addition, the relatively high cost of the HCV treatment brings concerns about the accessibility, especially in the developing countries. Hence, there exists a need for cost effect vaccines with high efficiency to control and possibly eradicate Hepatitis viruses globally. The vaccine should induce either, or both, neutralizing antibodies and protective T cell responses. We have developed and utilized flavivirus replicons as delivery system to prime hepatitis-specific T cell responses. We generated subgenomic replicons of Tick-borne encephalitis virus (TBEV), Langat virus (LGTV), West-Nile virus (WNV), and Kunjin virus (KUNV) expressing either a fusion protein between the HCV NS3/4A and a stork hepatitis B virus core or a vaccine candidate gene of HB/DV. Transfection experiments showed that the antigen expression by KUNV and WNV replicons was several folds higher than the antigen expression of control DNA plasmid with CMV promoter. The immunogenicity of these flavivirus replicons was evaluated in mice. The KUNV replicon triggered a potent cellular immune response with respect to priming of HCV NS3/4A-specific T cells as determined by ELISpot, and polyfunctionality. In short, the newly developed KUNV replicon- based vaccine is an attractive candidate to provide protection against hepatitis viruses.

We therefore have developed DNA based flavivirus replicons as a potent delivery system that effectively prime HCV-specific T cell responses. We generated suicidal subgenomic DNA replicons of Tick-borne encephalitis virus (TBEV), Langat virus (LGTV), West-Nile virus (WNV), and Kunjinvirus (KUNV) expressing either a fusion protein between the HCV NS3/4A and a stork hepatitis B virus core or a vaccine candidate gene of HB/DV. Transfection experiments showed that the antigen expression by KUNV and WNV replicons was several folds higher than the antigen expression by standard DNA plasmid with CMV promoter. The immunogenicity of three suicidal flaviviral DNA replicons expressing HCV NS3/4A was tested in mice and compared to HCV NS3/4A expression by the standard DNA plasmid. The KUNV-HCV replicon was the best replicon-based immunogen with respect to priming of HCV NS3/4A-specific T cells as determined by ELISpot, dextramer staining, and polyfunctionality. Importantly, a mutant KUNV-HCV immunogen lacking replication failed to induce immune responses. Thus, the newly developed KUNV-based suicidal DNA launched replicon vaccine for HCV is a highly attractive candidate as a prophylactic vaccine against chronic hepatitis C. In addition, we are currently testing the immunogenicity of KUNV-HB/DV replicon in mice.

    • Understanding the responses of tundra systemsto global change has global implications. Most tundraregions lack sustained environmental monitoring and oneof the only ways to document multi-decadal change is toresample historic research sites. The International PolarYear (IPY) provided a unique opportunity for such researchthrough the Back to the Future (BTF) project (IPY project#512). This article synthesizes the results from 13 paperswithin this Ambio Special Issue. Abiotic changes includeglacial recession in the Altai Mountains, Russia; increasedsnow depth and hardness, permafrost warming, andincreased growing season length in sub-arctic Sweden;drying of ponds in Greenland; increased nutrient availabilityin Alaskan tundra ponds, and warming at mostlocations studied. Biotic changes ranged from relativelyminor plant community change at two sites in Greenland tomoderate change in the Yukon, and to dramatic increasesin shrub and tree density on Herschel Island, and in subarcticSweden. The population of geese tripled at one sitein northeast Greenland where biomass in non-grazed plotsdoubled. A model parameterized using results from a BTFstudy forecasts substantial declines in all snowbeds andincreases in shrub tundra on Niwot Ridge, Colorado overthe next century. In general, results support and provideimproved capacities for validating experimental manipulation,remote sensing, and modeling studies.

    • The conference Codex and Code: Aesthetics, Language and Politics in an Age of Digital Media (NorLit 2009)was held at the Royal Institute of Technology (KTH) in Stockholm, August 6–9, 2009. The conference was organized by the Nordic Association for Comparative Literature (NorLit); the Department of Culture and Communication, Linköping University; the School of Computer Science and Communication, Royal Institute of Technology (KTH); the Department of Comparative Literature, Stockholm University; the Department of Culture and Communication, Södertörn University College; and the Department of Comparative Literature, Uppsala University.    The aim of the conference was to develop the study of Comparative Literature through Nordic collaboration both in its own discipline and in Modern Language and Cultural studies. As the title for the conference suggests, the principal question for the conference was the challenge that the study of literature encounters in an age of digitalization and globalization. It was our aim to encourage discussion of how literary studies respond to the ongoing changes in media and technology, politics and economy. Many have argued that the Humanities currently are in a state of crisis. We believe that the discipline seldom has found itself in such an interesting and fruitful historical moment. Several of these questions have surfaced during earlier media system changes, in particular during Romanticism and Modernism, which provided the conference with an historical frame. The conference Codex and Code also addressed questions of authenticity and originality, identity and gender, literary genres and reading practices, media and materiality, culture and popular culture, language and history, world literature, work aesthetics, translations, and canon formation.    The conference Codex and Code wanted to stimulate interdisciplinary scholarly research of the literary in a broad sense. The conference was open to scholars in Comparative Literature and in Classical and Modern Languages, Aesthetics, Media and Communication studies, Film and Theatre studies, Philosophy and adjacent disciplines. The conference was organized around a number of thematic sessions in which researchers and scholars presented and discussed papers.    The conference has received generous financial support from the Bank of Sweden Tercentenary Foundation, Magnus Bergwall foundation, Granholms foundation, Linköping University, School of Computer Science and Communication, Royal Institute of Technology, Svenska litteratursällskapet; the Swedish Academy, Swedish Science Council, and Vitterhetsakademien.

    • Projektet har drivits som ett program för finansiering av forskning som ska utveckla ochdemonstrera tekniker som reducerar elenergiförbrukningen med 50% vid tillverkning avTMP och CTMP med bibehållna slutproduktegenskaper hos tryckpapper och kartong.Programmet är en del av skogsindustrins initiativ att under en tioårsperiod tillsammansmed svenska och norska finansiärer investera minst 200 Mkr för att nå detta radikalaeffektiviseringsmål. Ett uttalat mål för industriinitiativet är också att befästaforskningsnoderna vid FSCN i Sundsvall och PFI i Trondheim.Parallellt med Energimyndighetens finansiering, 30 Mkr, har Norges Forskningsråd satsat25 MNOK (2010‐14) i industriinitiativet, KK‐stiftelsen 36 MSEK (2011‐17) ochMittuniversitetet har finansierat12 MSEK. Industrins totala satsning kommer att överstiga100 MSEK redan vid utgången av 2017.Resultat från benchmarkingstudien BAT2012 av industrins modernaste TMP‐ och CTMPlinjersamt från demonstrationsskaleprojekt visas i rapporten. Projekten baseras delvis pågrundläggande forskningsprojekt genomförda inom FSCN´s KK‐stiftelse‐finansieradeforskningsprofil och projektet ”Filling the Gap” 31676‐, ISSN 1650‐5387 2014:57. Resultaten visar följande reduktionsnivåer; 28% TMP för news (Braviken), 14% TMP för SC(Kvarnsveden) och 21% CTMP för kartong (Skoghall).Utöver demoprojekten finns ytterligare tydliga potentialer beskrivna i övriga delprojekt:Processintensifiering och processmodifiering > 15%Processtabilitet via avancerad processanalys och reglering > 15%Kombinera effektivaste processavsnitt från benchmarking ca 25%Detta gör det troligt att det kommer att gå att i fullskaliga demonstrationsförsök validera50% elenergireduktion inom de tre produktområdena, förutsatt att fortsattforskningsfinansiering finns tillgänglig. Tre av de idéer till avknoppningsprojekt somframkommit under projektets gång har redan erhållit beslut om finansiering frånEnergimyndigheten 2015. Ytterligare projektförslag baserade på den här redovisadeforskningen kommer att ingå i ansökningar under 2016.     • Magnetic relaxation is exploited in innovative biomedical applications of magnetic particles such as magnetic particle imaging (MPI), magnetic fluid hyperthermia, and bio-sensing. Relaxation behavior should be optimized to achieve high performance imaging, efficient heating, and good SNR in bio-sensing. Using two AC susceptometers with overlapping frequency ranges, we have measured the relaxation behavior of a series of monodisperse magnetic particles and demonstrated that this approach is an effective way to probe particle relaxation characteristics from a few Hz to 10 MHz, the frequencies relevant for MPI, hyperthermia, and sensing.

    • We developed nanoparticles with tailored magnetic properties for sensitive detection of biomolecules directly in biological samples in a single step. Thermally blocked nanoparticles obtained by thermal hydrolysis are mixed with sample solutions and the variation of the magnetic relaxation due to surface binding is used to detect the presence of biomolecules. The binding events significantly increase the hydrodynamic volume of nanoparticles, thus changing their Brownian relaxation frequency which is measured by a specifically developed AC-susceptometer.The system was tested for the presence of Brucella antibodies in serum samples from infected cows and the surface of the nanoparticles was functionalized with lipopolysaccarides (LPS) from Brucella abortus. The hydrodynamic volume of functionalized particles increased by 25-35% as a result of the binding of the antibodies, as measured by changes in the susceptibility in an alternating magnetic field. The method has shown high sensitivity, with detection limit of 7 nmol·L-1 in serum without any pre-treatment of the biological samples.The detection method is very sensitive, cost-efficient and versatile, giving a direct indication if the animal is infected or not, making it suitable for point-of care applications. The functionalization of tailored magnetic nanoparticles can be modified to suit numerous homogenous assays for a wide range of applications.

  • Enhancing the process of environmental improvement in manufacturing industry

    • To enable competitive industrial production, it is crucial to understand how to design and operate competitive production systems. Today, much operations management research focuses on the environmental dimension of sustainable manufacturing. Even though the environmental impact of industrial and commercial activities has become more vivid and severe, evidence points to a slow diffusion of industry-led ideas and actions for improving the environmental performance. It seems as if the manufacturing industry is still in the beginning of the process towards designing and operating environmentally benign production systems. However, increasing raw material and energy prices as well as potential penalties for lacking compliance provide cost related incentives for manufacturing companies to reduce their environmental impacts. Since production typically determines several environmental impacts such as exploitation of resources, contamination of soil, water and air, generation and emission of greenhouse gases etc., an important opportunity to reduce the environmental impact is found within a company’s production activities. In this, environmental issues can be viewed as either constraints or opportunities for competition. Companies can choose to adopt different positions ranging on a continuum from re-active behaviour, merely complying with environmental laws and regulations, to pro-active positions where competitive advantages are sought for. Either way, the increased environmental concern requires manufacturing companies to develop strategies, technologies and practices that will reduce the environmental impact from production.This research focuses on the process of environmental improvement applied by companies within the manufacturing industry and aims to explore the current state as well as develop support for enhancing this process. By collecting and analysing the empirical and theoretical findings, common barriers facing the companies in their strive to reduce the environmental impact from production as well as enabling factors, supporting the progress towards increased environmental performance have been identified. Based on an analysis of the current state one example of a link of challenges impeding the process of environmental improvement has been identified. The identified link of causes and effects shows a path towards a reactive improvement approach when dealing with environmental issues in production. In order to reach a more proactive state this link had to be broken. A general pattern describing the stages from a reactive to a proactive approach was identified, indicating that an increased understanding of and commitment to environmental issues were crucial parameters in order to succeed. Based on the findings, a number of requirements for enhancing the process of environmental improvement in production were derived. It was concluded that developing a support in terms of an improvement process was one way of improving the current state. Using an existing visual structure as a baseline a process for Green Performance Mapping was developed in cooperation with the studied companies. Ultimately, the suggested process was evaluated against the stated requirements and suggested as a support for enhancing the process of environmental improvement in production within the manufacturing industry, thereby contributing to reducing the environmental impact from production.

  • Detecting deletions in families affected by a dominant disease by use of marker data

    • A method of testing for whether inherited deletions are a cause of a single-locus dominant disease was derived, involving analysis of the marker segregation within the pedigree of a single family that segregates for the disease. It is shown that markers can be used to test deductively for the presence of an inherited deletion. The probabilities of confirming or rejecting the presence of a deletion in an arbitrary pedigree without inbreeding are then derived. The power of the test is shown to be limited in single trios but to increase rapidly as the size of the pedigree increases. For larger pedigrees, the probabilities of confirming or rejecting a deletion are higher than 0.9 for SNPs having a minor allele frequency greater than 0.4. The probabilities are higher using multiallelic markers such as microsatellites, reaching levels as high as 0.9 in even rather small pedigrees. In certain cases the test outcome is not deductive, a deletion being neither confirmed nor rejected. It is shown to still be possible then to employ a statistical test for the presence of a deletion by use of an a priori probability for a deletion.

  • Balance and mobility in patients with newly diagnosed Parkinson's disease : a five-year follow-up of a cohort in northern Sweden

    • BACKGROUND: The presence of early balance impairment in patients with Parkinson's disease has not been fully investigated.PURPOSE: The purpose of this study was to examine balance and mobility, self-perceived unsteadiness, self-reported falls, and effects of medication on balance among patients at their first visit to a neurological clinic and during the ensuing five years.MATERIALS AND METHODS: The participants were collected from a prospective longitudinal study. One hundred and forty-five patients with idiopathic Parkinson's disease and 31 healthy controls were included. The outcome measures were the Berg Balance Scale, the Timed Up and Go, the Postural Stability test and a questionnaire.RESULTS: At their first visit to the neurological clinic, the patients performed less well on the Berg Balance Scale (p < 0.001, r = 0.36), the Timed Up and Go (p < 0.001, r = 0.32), and the Postural Stability test (p < 0.001, r = 0.35) compared with the controls. In addition, a higher percentage of the patients reported self-perceived unsteadiness (p < 0.001, phi = 0.47). During the ensuing five years, balance and mobility worsened both with and without medication (p < 0.01, r = 0.24-0.37), although with small median differences.CONCLUSIONS: Further studies are needed to confirm that minor balance impairments exist even at the time of diagnosis and worsen during the ensuing five years. IMPLICATIONS FOR REHABILITATION Impairments in balance and mobility may occur early in Parkinson's disease, especially in the elderly patients, and seem to worsen during the first five years. There is a need to use sensitive outcome measures and to ask the patients about unsteadiness and falls to detect balance impairment in this cohort. Parkinsonian medication has a limited effect on balance and may preferably be complemented with balance exercises to target balance impairment early in Parkinson's disease.

    • Rapporten presenterar resultat från spridningsberäkningar och mätningar av svavel- ochkvävenedfallet för perioden oktober 1994 — september 1995 (ett hydrologiskt år). Förberäkningarna används spridningsmodeller som utvecklats speciellt för Stockholms länoch som ingår i miljöövervakningssystemet som drivs av Luftvårdsförbundet i Stockholmslän.Modellberäkningarna ger halter av svaveldioxid och kväveoxider för varje timme med4x4 kilometers upplösning i hela länet. Torr- och våtdepositionen har summerats tillmånadsvärden. För beräkningarna används en detaljerad emissionsdatabas samt resultatenfrån ett stort antal mätningar av meteorologi, deposition, krondropp och föroreningshaltersom genomförts inom ramen för Luftvårdsförbundets och Länsstyrelsensregionala miljöövervakning. Beräkningarna jämförs med mätningar.Våtdepositionen av svavel och kväve är till största delen beroende av nederbördens geografiskafördelning, som varierar kraftigt p g a lokala klimatförhållanden. I genomsnittsyns två områden med förhållandevis hög nederbörd — inre delarna av Södertörn ochnorra delarna av Norrtälje kommun. I dessa områden är nederbördsmängden nästandubbelt så stor som över de yttre delarna av skärgården, där de minsta mängderna förekommer.Som mest noteras drygt 900 mm nederbörd för det hydrologiska året 1994/95.Detta är ca. 30% högre än de genomsnittligt högsta värdena för 10 år (1984 — 1993).Den relativa geografiska fördelningen av årsnederbörden 1994/95 skiljer sig inte väsentligtfrån den genomsnittliga fördelningen under de senaste 10 åren.Den högsta våtdepositionen av svavel sker över Södertörn med drygt 6 kg S/ha/år. I yttreskärgården ligger värdena på omkring 3 till 4 kg/ha/år. För kväve är motsvarande siffrordrygt 7 kg N/ha/år respektive 4 kg N/ha/år. Den geografiska fördelningen av torrdepositionenbestäms i hög grad av var utsläppen sker. Beräkningarna visar på mycket skarpagradienter med den högsta torrdepositionen i Stockholmsregionen och lägre värden utmot landsbygden och skärgården. Högsta torrdepositionen av svavel och kväve beräknasuppgå till 5 kg S/ha/år respektive 7 kg N/ha/år, vilket är drygt hälften av den totala depositionen.Som mest uppgår den totala depositionen (summan av våt- och torrdepositionen) avsvavel och kväve till drygt 9 kg S/ha/år respektive 12 kg N/ha/år. Det lägsta nedfallet,som sker över landsbygdsområden och skärgården, är på knappt hälften av de högstavärdena. I hela länet överskrids de undre kritiska belastningsgränserna för både svaveloch kväve.Bidragen till nedfallet från utsläpp av kväve och svavel i Stockholms län har beräknatsmed hjälp av spridningsmodellen. För kväve kan drygt 50% av den totala depositionen iStorstockholm härledas till utsläppen i länet. För svavel är motsvarande siffra 30% —40%.- 5 -Utanför tätortsområdena har utsläppen inom länet endast liten betydelse för den totaladepositionen. Här står intransporterade svavel- och kväveföreningar för huvuddelen.Mätningarna visar att halterna i luften som transporteras in är jämnt fördelade över länet.De geografiska variationerna i depositionen över landsbygden och skärgården ärdärför dels beroende av nederbördens fördelning, som bestämmer variationen i våtdeposition,dels markanvändningen, som påverkar variationen i torrdeposition.Det enda säkra sättet att bedöma noggrannheten i beräkningarna är att så långt det gårjämföra beräknade halter, deposition och nederbörd med uppmätta värden (i vissapunkter). För nederbörden erhålls en god beskrivning på de flesta platserna. I genomsnittför 12 mätstationer är beräknad årsnederbörd inom 2% av uppmätt värde. Under sommarenkan dock avvikelserna bli stora p g a mycket stora variationer i nederbördsmängdernaför olika platser i länet.När det gäller kväveoxidhalter (NOx och NO2) erhålls en god överensstämmelse mellanberäkningar och mätningar i Stockholms innerstad. Beräknade månadsmedelvärden liggerför de flesta månaderna inom 20% av uppmätta värden. För svaveldioxidhalterna(SO2) är skillnaden större speciellt för sommarmånaderna då halterna är mycket låga.Beräknat totalt nedfall av svavel stämmer relativt väl med krondroppsmätningarna. Beräknattotalt nedfall av kväve kan däremot inte jämföras med kvävenedfallet i krondropp,eftersom träden tar upp en avsevärd del av det kväve som tillförs skogen via luftoch nederbörd.

    • Inherited deficiency of protein S constitutes an important risk factor of venous thrombosis. Many reports have demonstrated that causative mutations in the protein S gene are found only in approximately 50% of the cases with protein S deficiency. It is uncertain whether the protein S gene is causative in all cases of protein S deficiency or if other genes are involved in cases where no mutation is identified. The aim of the current study was to determine whether haplotypes of the protein S gene cosegregate with the disease phenotype in cases where no mutations have been found. Eight protein S-deficient families comprising 115 individuals where previous DNA sequencing had failed to detect any causative mutations were analyzed using four microsatellite markers in the protein S gene region. Co-segregation between microsatellite haplotypes and protein S deficiency was found in seven of the investigated families, one family being uninformative. This suggests that the causative genetic defects are located in or close to the protein S gene in a majority of such cases where no mutations have been found.

    • BACKGROUND: The most common type of von Willebrand disease (VWD), type 1, has in only a few cases been explained by an identified causative mutation in the von Willebrand factor (VWF) gene. The ABO blood group and other modifier loci outside the VWF gene may contribute to the development of type 1 VWD. OBJECTIVES AND METHODS: Our aim was to determine whether there was genetic linkage to the VWF gene in 31 Swedish type 1 VWD families. Stringent diagnostic criteria in accordance with ISTH guidelines were used. Genetic linkage was investigated by using two highly informative dinucleotide microsatellite markers, which we have recently identified, located in introns six and 15 of the VWF gene. We also investigated the existence of common disease haplotypes and the relation between type 1 VWD and ABO blood group. RESULTS: We found genetic linkage to the VWF gene in 27 (87%) of the families. However, in four (13%) of the families, there was clearly no genetic linkage. We found the 4751A>G (Tyr1584Cys) sequence variation in exon 28, which is a common mutation in the Canadian VWD population (14.3%), in only one of the 31 families (3.2%). A possible common mutation was identified in six of the 27 (22%) families with genetic linkage. Blood group O was over-represented among type 1 VWD patients. CONCLUSION: We conclude that there is linkage between the VWF gene and hereditary type 1 VWD in a majority of families.

    • The aim of the conference was to develop the study of Comparative Literature through Nordic collaboration both in its own discipline and in Modern Language and Cultural studies. As the title for the conference suggests, the principal question for the conference was the challenge that the study of literature encounters in an age of digitalization and globalization. It was our aim to encourage discussion of how literary studies respond to the ongoing changes in media and technology, politics and economy. Many have argued that the Humanities currently are in a state of crisis. We believe that the discipline seldom has found itself in such an interesting and fruitful historical moment. Several of these questions have surfaced duringearlier media system changes, in particular during Romanticism and Modernism, which provided the conference with an historical frame. The conference Codex and Code also addressed questions of authenticity and originality, identity and gender, literary genres and reading practices, media and materiality, culture and popular culture, language and history, world literature, work aesthetics, translations, and canon formation.

    • Arctic and subarctic ecosystems are experiencing substantial changes in hydrology, vegetation, permafrost conditions, and carbon cycling, in response to climatic change and other anthropogenic drivers, and these changes are likely to continue over this century. The total magnitude of these changes results from multiple interactions among these drivers. Field measurements can address the overall responses to different changing drivers, but are less capable of quantifying the interactions among them. Currently, a comprehensive assessment of the drivers of ecosystem changes, and the magnitude of their direct and indirect impacts on subarctic ecosystems, is missing. The Torneträsk area, in the Swedish subarctic, has an unrivalled history of environmental observation over 100 years, and is one of the most studied sites in the Arctic. In this study, we summarize and rank the drivers of ecosystem change in the Torneträsk area, and propose research priorities identified, by expert assessment, to improve predictions of ecosystem changes. The research priorities identified include understanding impacts on ecosystems brought on by altered frequency and intensity of winter warming events, evapotranspiration rates, rainfall, duration of snow cover and lake-ice, changed soil moisture, and droughts. This case study can help us understand the ongoing ecosystem changes occurring in the Torneträsk area, and contribute to improve predictions of future ecosystem changes at a larger scale. This understanding will provide the basis for the future mitigation and adaptation plans needed in a changing climate.

  • Waveguide Truncation Using UPML in the Finite-Element Time-Domain Method

    • An important part of numerical waveguide modeling is the termination of the waveguide using artificial boundaries. In this paper we develop a perfectly matched layer (PML) for waveguides in the finite-element time-domain method (FETD). The PML is discretized by prism elements aligned with the direction of propagation of the waveguide. Assuming that the waveguide is discretized by tetrahedra such a grid is easily generated from a cross-sectional surface in the waveguide. The proposed method has the advantage of being general with regard to the geometry and material of the waveguide. Previous works on PML for FETD have reported problems with late-time instability. While still present in the current approach, our results indicate that the instability is less severe for the prism element PML compared to a tetrahedral PML. Moreover, it may be controlled by increasing the number of grid points in the absorbing layer. It should be noted that the instability appears long after the energy has been attenuated and therefore pose no problems in practical computations. The performance of the suggested scheme is demonstrated for several waveguide problems, including an inhomogeneous case.

    • Objective. The incidence of prostate cancer is rising rapidly in Sweden and there is a need to better understand the pattern of diagnosis, tumor characteristics and treatment. Material and methods. Between 1996 and 2005, all new cases of adenocarcinoma of the prostate gland were intended to be registered in the National Prostate Cancer Register (NPCR). This register contains information on diagnosing unit, date of diagnosis, cause of diagnosis, tumor grade, tumor stage according to the TNM classification in force, serum prostate-specific antigen (PSA) levels at diagnosis and primary treatment given within the first 6 months after diagnosis. Results. In total, 72 028 patients were registered, comprising >97% of all pertinent incident cases of prostate cancer in the Swedish Cancer Register (SCR). During the study period there was a considerable decrease in median age at the time of diagnosis, a stage migration towards smaller tumors, a decrease in median serum PSA values at diagnosis, a decrease in the age-standardized incidence rate of men diagnosed with distant metastases or with a PSA level of >100 ng/ml at diagnosis and an increase in the proportion of tumors with Gleason score ≤6. Relatively large geographical differences in the median age at diagnosis and the age-standardized incidence of cases with category T1c tumors were observed. Treatment with curative intent increased dramatically and treatment patterns varied according to geographical region. In men with localized tumors and a PSA level of <20 ng/ml at diagnosis, expectant treatment was more commonly used in those aged ≥75 years than in those aged <75 years. Also, the pattern of endocrine treatment varied in different parts of Sweden. Conclusions. All changes in the register seen over time are consistent with increased diagnostic activity, especially PSA testing, resulting in an increased number of cases with early disease, predominantly tumors in category T1c. The patterns of diagnosis and treatment of prostate cancer vary considerably in different parts of Sweden. The NPCR continues to be an important source for research, epidemiological surveillance of the incidence, diagnosis and treatment of prostate cancer
