    • Aims: Recent trial data demonstrate beneficial effects of active rhythm management in patients with atrial fibrillation (AF) and support the concept that a low arrhythmia burden is associated with a low risk of AF-related complications. The aim of this document is to summarize the key outcomes of the 9th AFNET/EHRA Consensus Conference of the Atrial Fibrillation NETwork (AFNET) and the European Heart Rhythm Association (EHRA).Methods and results: Eighty-three international experts met in Munster for 2 days in September 2023. Key findings are as follows: (i) Active rhythm management should be part of the default initial treatment for all suitable patients with AF. (ii) Patients with device-detected AF have a low burden of AF and a low risk of stroke. Anticoagulation prevents some strokes and also increases major but non-lethal bleeding. (iii) More research is needed to improve stroke risk prediction in patients with AF, especially in those with a low AF burden. Biomolecules, genetics, and imaging can support this. (iv) The presence of AF should trigger systematic workup and comprehensive treatment of concomitant cardiovascular conditions. (v) Machine learning algorithms have been used to improve detection or likely development of AF. Cooperation between clinicians and data scientists is needed to leverage the potential of data science applications for patients with AF.Conclusions: Patients with AF and a low arrhythmia burden have a lower risk of stroke and other cardiovascular events than those with a high arrhythmia burden. Combining active rhythm control, anticoagulation, rate control, and therapy of concomitant cardiovascular conditions can improve the lives of patients with AF. Graphical Abstract

    • There are major challenges ahead for clinicians treating patients with atrial fibrillation (AF). The population with AF is expected to expand considerably and yet, apart from anticoagulation, therapies used in AF have not been shown to consistently impact on mortality or reduce adverse cardiovascular events. New approaches to AF management, including the use of novel technologies and structured, integrated care, have the potential to enhance clinical phenotyping or result in better treatment selection and stratified therapy. Here, we report the outcomes of the 6th Consensus Conference of the Atrial Fibrillation Network (AFNET) and the European Heart Rhythm Association (EHRA), held at the European Society of Cardiology Heart House in Sophia Antipolis, France, 17-19 January 2017. Sixty-two global specialists in AF and 13 industry partners met to develop innovative solutions based on new approaches to screening and diagnosis, enhancing integration of AF care, developing clinical pathways for treating complex patients, improving stroke prevention strategies, and better patient selection for heart rate and rhythm control. Ultimately, these approaches can lead to better outcomes for patients with AF.

    • Societal dependence on, and commercial and scientific exploitation of Earth-Oriented remote sensing from satellites is growing at an exponential rate. The comprehensive EU Copernicus programme provides a major contribution to the global effort, but even so, to achieve the necessary global and temporal coverage requires synergistic cooperation and associated interoperability of the Worlds sensors. For a user to exploit Earth Observation (EO) data there must exist confidence in data characteristics, quality and reliable delivery. Although long-term data records for climate may be the most demanding in nature, generation of analysis-ready operational data sets for applications, as diverse as food security to pollution monitoring, all require the user to have some quantitative level of confidence in the data and derived information. A long-term Calibration/Validation (Cal/Val) vision necessitates clear ownership and long-term funding. Delineating the roles of the European Commission (EC), space agencies and member states in long-term Cal/Val would provide clarity. It is clear that the space agencies have the responsibility to meet the mission requirement of their spaceborne instruments but long-term validation is often entrusted to interested parties bringing their own resources to the task. Furthermore, there is a critical need for Fiducial Reference Measurements (FRMs), acquired in operational mode, and comprehensive in coverage both spatially and temporally, to assure that the satellite product accuracies are met. This paper discusses the current status, gaps and challenges regarding long-term Cal/Val of EO satellites and recommends the creation of a European coordinating entity for satellite product calibration and validation. The proposed entity would be an integrative organization coordinating the European Cal/Val activities in partnership with the member states and the space agencies and working together with existing data providers to secure access to satellite and in-situ data of traceable FRM standards.

    • Venous thromboembolism (VTE) comprising deep venous thrombosis and pulmonary embolism is a major cause of morbidity and mortality. Short-term immobility-related conditions are a major risk factor for the development of VTE. Paradoxically, long-term immobilized free-ranging hibernating brown bears and paralyzed spinal cord injury (SCI) patients are protected from VTE. We aimed to identify mechanisms of immobility-associated VTE protection in a cross-species approach. Mass spectrometry-based proteomics revealed an antithrombotic signature in platelets of hibernating brown bears with heat shock protein 47 (HSP47) as the most substantially reduced protein. HSP47 down-regulation or ablation attenuated immune cell activation and neutrophil extracellular trap formation, contributing to thromboprotection in bears, SCI patients, and mice. This cross-species conserved platelet signature may give rise to antithrombotic therapeutics and prognostic markers beyond immobility-associated VTE.

  • Genetic Interactions with Age, Sex, Body Mass Index, and Hypertension in Relation to Atrial Fibrillation : The AFGen Consortium

    • It is unclear whether genetic markers interact with risk factors to influence atrial fibrillation (AF) risk. We performed genome-wide interaction analyses between genetic variants and age, sex, hypertension, and body mass index in the AFGen Consortium. Study-specific results were combined using meta-analysis (88,383 individuals of European descent, including 7,292 with AF). Variants with nominal interaction associations in the discovery analysis were tested for association in four independent studies (131,441 individuals, including 5,722 with AF). In the discovery analysis, the AF risk associated with the minor rs6817105 allele (at the PITX2 locus) was greater among subjects ≤ 65 years of age than among those > 65 years (interaction p-value = 4.0 × 10-5). The interaction p-value exceeded genome-wide significance in combined discovery and replication analyses (interaction p-value = 1.7 × 10-8). We observed one genome-wide significant interaction with body mass index and several suggestive interactions with age, sex, and body mass index in the discovery analysis. However, none was replicated in the independent sample. Our findings suggest that the pathogenesis of AF may differ according to age in individuals of European descent, but we did not observe evidence of statistically significant genetic interactions with sex, body mass index, or hypertension on AF risk.

    • The CryoGrid community model is a flexible toolbox for simulating the ground thermal regime and the ice-water balance for permafrost and glaciers, extending a well-established suite of permafrost models (CryoGrid 1, 2, and 3). The CryoGrid community model can accommodate a wide variety of application scenarios, which is achieved by fully modular structures through object-oriented programming. Different model components, characterized by their process representations and parameterizations, are realized as classes (i.e., objects) in CryoGrid. Standardized communication protocols between these classes ensure that they can be stacked vertically. For example, the CryoGrid community model features several classes with different complexity for the seasonal snow cover, which can be flexibly combined with a range of classes representing subsurface materials, each with their own set of process representations (e.g., soil with and without water balance, glacier ice). We present the CryoGrid architecture as well as the model physics and defining equations for the different model classes, focusing on one-dimensional model configurations which can also interact with external heat and water reservoirs. We illustrate the wide variety of simulation capabilities for a site on Svalbard, with point-scale permafrost simulations using, e.g., different soil freezing characteristics, drainage regimes, and snow representations, as well as simulations for glacier mass balance and a shallow water body. The CryoGrid community model is not intended as a static model framework but aims to provide developers with a flexible platform for efficient model development. In this study, we document both basic and advanced model functionalities to provide a baseline for the future development of novel cryosphere models.

  • Using SAR data time-series for regional glacier mapping

    • With dense SAR satellite data time series it is possible to map surface and subsurface glacier properties that vary in time. On Sentinel-1A and RADARSAT-2 backscatter time series images over mainland Norway and Svalbard, we outline how to map glaciers using descriptive methods. We present five application scenarios. The first shows potential for tracking transient snow lines with SAR backscatter time series and correlates with both optical satellite images (Sentinel-2A and Landsat 8) and equilibrium line altitudes derived from in situ surface mass balance data. In the second application scenario, time series representation of glacier facies corresponding to SAR glacier zones shows potential for a more accurate delineation of the zones and how they change in time. The third application scenario investigates the firn evolution using dense SAR backscatter time series together with a coupled energy balance and multilayer firn model. We find strong correlation between backscatter signals with both the modeled firn air content and modeled wetness in the firn. In the fourth application scenario, we highlight how winter rain events can be detected in SAR time series, revealing important information about the area extent of internal accumulation. In the last application scenario, averaged summer SAR images were found to have potential in assisting the process of mapping glaciers outlines, especially in the presence of seasonal snow. Altogether we present examples of how to map glaciers and to further understand glaciological processes using the existing and future massive amount of multi-sensor time series data.
