Development of heart block in children of SSA/SSB-autoantibody-positive women is associated with maternal age and displays a season-of-birth pattern

- Objective Congenital heart block may develop in the fetuses of Ro/SSA-positive and La/SSB-positive mothers. Recurrence rates of only 10-20% despite persisting maternal antibodies indicate that additional factors are critical for the establishment of heart block. The authors investigated the influence of other maternal and fetal factors on heart block development in a Swedish population-based cohort. less thanbrgreater than less thanbrgreater thanMethods The influence of fetal gender, maternal age, parity and time of birth on heart block development was analysed in 145 families, including Ro/La-positive (n=190) and Ro/La-negative (n=165) pregnancies. less thanbrgreater than less thanbrgreater thanResults There was a recurrence rate of 12.1% in Ro/La-positive women, and no recurrence in Ro/La-negative women. Fetal gender and parity did not influence the development of heart block in either group. Maternal age in Ro/La-positive pregnancies with a child affected by heart block was, however, significantly higher than in pregnancies resulting in babies without heart block (pandlt;0.05). Seasonal timing of pregnancy influenced the outcome. Gestational susceptibility weeks 18-24 occurring during January-March correlated with a higher proportion of children with heart block and lower vitamin D levels during the same period in a representative sample of Swedish women and a corresponding higher proportion of children with heart block born in the summer (pandlt;0.02). Maternal age or seasonal timing of pregnancy did not affect the outcome in Ro/La-negative pregnancies. less thanbrgreater than less thanbrgreater thanConclusion This study identifies maternal age and seasonal timing of pregnancy as novel risk factors for heart block development in children of Ro/La-positive women. These observations may be useful for counselling when pregnancy is considered.

Att utvärdera och använda resultaten – vilka avtryck ger bibliotekens utvärderingar i samhället?

- Det har blivit allt viktigare för biblioteken att kunna visa på sin betydelse i samhället. Mätetal och utvärderingar är nödvändiga instrument för att kunna jämföra satsningar på biblioteken med satsningar på andra områden. Men finns det en synergi mellan mätetalen och verksamhetens planer och mål? Hur mäter vi måluppfyllese i biblioteksverksamheten? Finns det kvalitetsindikatorer som vi har gemensamma oavsett bibliotekstyp? Hur kan biblioteksstatistiken användas? Vad gör bibliotekens utvärderingar för avtryck i beslutsprocessen? Vad gör bibliotekens utvärderingar för avtryck i samhället?För att svara på frågorna ovan har expertgruppen påbörjat ett arbete med att kartlägga hur svenska bibliotek arbetar med utvärdering. I det arbetet har vi bland annat sett vikten av ett gemensamt språk mellan bibliotekstyperna i arbetet med utvärderingar och kvalitetsutveckling. Gruppen kommer att under år 2014 publicera ett utvärderingsverktyg för bibliotek. Där kan bibliotek finna vägledning och stöd i valet av utvärderingsmetod, i genomförandet och i nyttjandet av utvärderingars resultat i syfte att utveckla och synliggöra biblioteksverksamheten.I det här papret presenterar vi syftet med vårt arbete och den metod vi har använt samt mer ingående berättar om hur vi har tänkt kring det verktyg som vi i slutet av papret ger en beskrivning av – ett verktyg som går under namnet "Utvärderingshjälpen".Vid konferensen kommer vi att presentera hur en årscykel kan se ut vid ett bibliotek som väljer att använda sig av utvärderingsarbete och systematiskt kvalitetsutveckling i nära relation till måluppfyllelse och till dialog med beslutsfattare samt omgivande samhälle. Vi presenterar också hur "Utvärderingshjälpen" skall kunna användas som stöd i det arbetet.

Common Interactions between S100A4 and S100A9 Defined by a Novel Chemical Probe.

- S100A4 and S100A9 proteins have been described as playing roles in the control of tumor growth and metastasis. We show here that a chemical probe, oxyclozanide (OX), selected for inhibiting the interaction between S100A9 and the receptor for advanced glycation end-products (RAGE) interacts with both S100A9 and S100A4. Furthermore, we show that S100A9 and S100A4 interact with RAGE and TLR4; interactions that can be inhibited by OX. Hence, S100A4 and S100A9 display similar functional elements despite their primary sequence diversity. This was further confirmed by showing that S100A4 and S100A9 dimerize both in vitro and in vivo. All of these interactions required levels of Zn(++) that are found in the extracellular space but not intracellularly. Interestingly, S100A4 and S100A9 are expressed by distinct CD11b(+) subpopulations both in healthy animals and in animals with either inflammatory disease or tumor burden. The functions of S100A9 and S100A4 described in this paper, including heterodimerization, may therefore reflect S100A9 and S100A4 that are released into the extra-cellular milieu.

- Den svenska föreningsidrotten omfattar tusentals barn. Dessa barn tillbringar en stor del av sin fritid med att lära sig en viss sport under tränares överinseende. Idrott är också en arena där könsrelaterade normer formuleras, manifesteras och reproduceras.Syftet med denna artikel är att analysera könsrelaterade normer i två idrottsföreningar för flickor mellan nio och tolv år gamla: en traditionellt manlig sport, ishockey, och en traditionellt kvinnlig, gymnastik. Det empiriska materialet bygger på observationer av träningar och intervjuer med tränare och föreningsaktiva. De huvudsakliga resultaten är, förutom den uppenbara könsskillnaden i barnens idrottspreferenser, att könsrelaterade normer uttrycks på tre huvudsakliga sätt: genom (i) attribut och i framträdanden, såsom kläder och färger, genom (ii) en könshierarki i ledarens talspråk, och genom (iii) könsbaserade förväntningar. I de studerade idrottsföreningarna var maskulint genus normen.

Training and support for the role of facilitator in implementation of innovations in health and community care : a scoping review protocol

- BackgroundImplementing and sustaining innovations in clinical practice, such as evidence-based practices, programmes, and policies, is frequently described as challenging. Facilitation as a strategy for supporting implementation requires a facilitator, i.e. an individual with a designated role to support the implementation process. A growing number of studies report that facilitation can help tackle the challenges in implementation efforts. To optimise the potential contribution of facilitation as a strategy to improve the implementation of new practices, there is a need to enhance understanding about what training and support is required for individuals in the facilitator role.The objective of this scoping review is to map how facilitators have been trained for, and supported in, the facilitator role in implementation studies in health and community care. Specifically, the review aims to examine what is reported on training and support of facilitators in terms of learning outcomes, content, dose, mode of delivery, learning activities, and qualifications of the trainers and how the facilitators perceive training and support.MethodsThis scoping review will follow the guidance of the Joanna Briggs Institute and the PRISMA Extension for Scoping Review checklist. We will include articles in which (a) facilitation is deployed as an implementation strategy, with identified facilitator roles targeting staff and managers, to support the implementation of specified innovations in health or community care, and (b) training and/or support of facilitators is reported. We will exclude articles where facilitation is directed to education or training in specific clinical procedures or if facilitation supports the implementation of general quality improvement systems. All types of peer-reviewed studies and study protocols published in English will be included. A systematic search will be performed in MEDLINE (Ovid), Embase (embase.com), Web of Science Core Collection, and CINAHL (Ebsco).DiscussionThe proposed scoping review will provide a systematic mapping of the literature on the training and support of implementation facilitators and contribute useful knowledge within the field of implementation science to inform future facilitation initiatives.

- OBJECTIVE: Collagenous colitis (CC) is a major cause of chronic non-bloody diarrhoea, particularly in the elderly female population. The aetiology of CC is unknown, and still poor is the understanding of its pathogenesis. This possibly involves dysregulated inflammation and immune-mediated reactions in genetically predisposed individuals, but the contribution of genetic factors to CC is underinvestigated. We systematically tested immune-related genes known to impact the risk of several autoimmune diseases for their potential CC-predisposing role.DESIGN: Three independent cohorts of histologically confirmed CC cases (N=314) and controls (N=4299) from Sweden and Germany were included in a 2-step association analysis. Immunochip and targeted single nucleotide polymorphism (SNP) genotype data were produced, respectively, for discovery and replication purposes. Classical human leucocyte antigen (HLA) variants at 2-digit and 4-digit resolution were obtained via imputation from single marker genotypes. SNPs and HLA variants passing quality control filters were tested for association with CC with logistic regression adjusting for age, sex and country of origin.RESULTS: Forty-two markers gave rise to genome-wide significant association signals, all contained within the HLA region on chromosome 6 (best p=4.2×10(-10) for SNP rs4143332). Among the HLA variants, most pronounced risk effects were observed for 8.1 haplotype alleles including DQ2.5, which was targeted and confirmed in the replication data set (p=2.3×10(-11); OR=2.06; 95% CI (1.67 to 2.55) in the combined analysis).CONCLUSIONS: HLA genotype associates with CC, thus implicating HLA-related immune mechanisms in its pathogenesis.
