    • Older peoples usage of mobile devices is increasing. This study is part of the GASEL project and seeks to increase the understanding of the use of mobile information technology and opinions towards it. Opinions are compared between genders and age groups. The population based data were collected by conducting a questionnaire survey. A random sample of 1,500 adults 65 years of age or older was obtained from the Finnish Population Register Centre. The number of respondents was 918 with a response rate of 61.2 %. About one fifth (n=169) had used a tablet computer and approximately one third (n=226) had used a mobile phone with a touch screen in the past 12 months without encountering major difficulties. The respondents had mostly thought that using the devices would not be easy. They had also thought that the usage would not be too expensive for them. Men and younger old people held more positive opinions towards these new technologies. Older people are slower in adapting and accepting new technologies and this should be taken into account when designing services, applications and content, as these technologies have the potential to enrichen the lives of this specific population group.

  • Relationship Between Everyday Health Information Literacy and Attitudes Towards Mobile Technology Among Older People

    • In order to benefit from advanced mobile information technology (AMIT) in e-health services, people need competencies in finding, evaluating, and understanding health-related information in varying everyday life situations, that is, everyday health information literacy (EHIL). This study focused on the relationship between EHIL and AMIT use and attitudes towards it among older adults. A paper questionnaire was posted to a random sample of 1,500 home dwelling subjects aged 65 or more living in Northern Finland. A variety of themes were addressed in the questionnaire including a 4-item version of an EHIL screening tool. The response rate was 61% (n=918). Older adults with negative attitudes towards or having less experience with mobile information technology were likely to have poor self-estimated EHIL skills. Older people are at risk of marginalization in regard to m-services, and these results should be utilized by decision-makers and software designers in digitalization of services.

    • Falls are an increasing problem of aging population, both in home-dwelling and institutionalized people. Automatic fall detection systems are a choice in supporting the independent and secure living of the older people. Typically, health technology applications such as fall detection systems are tested in experimental falls of young adults. However, sensitivity and specificity, and acceptability and usability of these systems in real-life conditions in end users should be the ultimate aim. This paper overviews our set of studies on the technology and algorithms for fall detection, from laboratory-based experiments to long-term real-life field tests. The data obtained during the incremental set of studies suggest that automatic accelerometric fall detection systems might offer a tool for improving safety among older people. Additional studies are needed for further improvement of fall detection sensitivity and decreasing the false alarm rate, and for the implementation of the technology to elderly care ICT platforms.

  • Comparison of low-complexity fall detection algorithms for body attached accelerometers

    • The elderly population is growing rapidly. Fall related injuries are a central problem for this population. Elderly people desire to live at home, and thus, new technologies, such as automated fall detectors, are needed to support their independence and security. The aim of this study was to evaluate different low-complexity fall detection algorithms, using triaxial accelerometers attached at the waist, wrist, and head. The fall data were obtained from standardized types of intentional falls (forward, backward, and lateral) in three middle-aged subjects. Data from activities of daily living were used as reference. Three different detection algorithms with increasing complexity were investigated using two or more of the following phases of a fall event: beginning of the fall, falling velocity, fall impact, and posture after the fall. The results indicated that fall detection using a triaxial accelerometer worn at the waist or head is efficient, even with quite simple threshold-based algorithms, with a sensitivity of 97-98% and specificity of 100%. The most sensitive acceleration parameters in these algorithms appeared to be the resultant signal with no high-pass filtering, and the calculated vertical acceleration. In this study, the wrist did not appear to be an applicable site for fall detection. Since a head worn device includes limitations concerning usability and acceptance, a waist worn accelerometer, using an algorithm that recognizes the impact and the posture after the fall, might be optimal for fall detection.

    • Background: About a third of home-dwelling older people fall each year, and institutionalized older people even report a two- or threefold higher rate for falling. Automatic fall detection systems have been developed to support the independent and secure living of the elderly. Even though good fall detection sensitivity and specificity in laboratory settings have been reported, knowledge about the sensitivity and specificity of these systems in real-life conditions is still lacking. Objective: The aim of this study was to evaluate the long-term fall detection sensitivity and false alarm rate of a fall detection prototype in real-life use. Methods: A total of 15,500 h of real-life data from 16 older people, including both fallers and nonfallers, were monitored using an accelerometry-based sensor system with an implemented fall detection algorithm. Results: The fall detection system detected 12 out of 15 real-life falls, having a sensitivity of 80.0%, with a false alarm rate of 0.049 alarms per usage hour with the implemented real-time system. With minor modification of data analysis the false alarm rate was reduced to 0.025 false alarms per hour, equating to 1 false fall alarm per 40 usage hours. Conclusion: These data suggest that automatic accelerometric fall detection systems might offer a tool for improving safety among older people.

    • About one third of home-dwelling people over 65 years of age fall each year. Falling, and the fear of falling, is one of the major health risks that affects the quality of life among older people, threatening their independent living. In our pilot study, we found that fall detection with a waist-worn triaxial accelerometer is reliable with quite simple detection algorithms. The aim of this study was to validate the data collection of a new fall detector prototype and to define the sensitivity and specificity of different fall detection algorithms with simulated falls from 20 middle-aged (40-65 years old) test subjects. Activities of daily living (ADL) performed by the middle-aged subjects, and also by 21 older people (aged 58-98 years) from a residential care unit, were used as a reference. The results showed that the hardware platform and algorithms used can discriminate various types of falls from ADL with a sensitivity of 97.5% and a specificity of 100%. This suggests that the present concept provides an effective method for automatic fall detection.

    • The Nordic eHealth Research Network, a subgroup of the Nordic Council of Ministers eHealth group, is working on developing indicators to monitor progress in availability, use and outcome of eHealth applications in the Nordic countries. This paper reports on the consecutive analysis of National eHealth policies in the Nordic countries from 2012 to 2016. Furthermore, it discusses the consequences for the development of indicators that can measure changes in the eHealth environment arising from the policies. The main change in policies is reflected in a shift towards more stakeholder involvement and intensified focus on clinical infrastructure. This change suggests developing indicators that can monitor understandability and usability of eHealth systems, and the use and utility of shared information infrastructure from the perspective of the end-users - citizens/patients and clinicians in particular.
