  • Changes in Chromatin Structure in Curettage Specimens Identifies High-Risk Patients in Endometrial Cancer

    • Background: Most endometrial carcinoma patients are diagnosed at an early stage with a good prognosis. However, a relatively low fraction with lethal disease constitutes a substantial number of patients due to the high incidence rate. Preoperative identification of patients with high risk and low risk for poor outcome is necessary to tailor treatment. Nucleotyping refers to characterization of cell nuclei by image cytometry, including the assessment of chromatin structure by nuclear texture analysis. This method is a strong prognostic marker in many cancers but has not been evaluated in preoperative curettage specimens from endometrial carcinoma. Methods: The prognostic impact of changes in chromatin structure quantified with Nucleotyping was evaluated in preoperative curettage specimens from 791 endometrial carcinoma patients prospectively included in the MoMaTEC multicenter trial. Results: Nucleotyping was an independent prognostic marker of disease-specific survival in preoperative curettage specimens among patients with Federation Internationale des Gynaecologistes et Obstetristes (FIGO) stage I-II disease (HR = 2.9; 95% CI, 1.2-6.5; P - 0.013) and significantly associated with age, FIGO stage, histologic type, histologic grade, myometrial infiltration, lymph node status, curettage histology type, and DNA ploidy. Conclusions: Nucleotyping in preoperative curettage specimens is an independent prognostic marker for disease-specific survival, with potential to supplement existing parameters for risk stratification to tailor treatment. Impact: This is the first study to evaluate the prognostic impact of Nucleotyping in curettage specimens from endometrial carcinoma and shows that this may be a clinically useful prognostic marker in endometrial cancer.     • Background: Preoperative risk stratification is essential in tailoring endometrial cancer treatment, and biomarkers predicting lymph node metastasis and aggressive disease are aspired in clinical practice. DNA ploidy assessment in hysterectomy specimens is a well-established prognostic marker. DNA ploidy assessment in preoperative curettage specimens is less studied, and in particular in relation to the occurrence of lymph node metastasis. Methods: Curettage image cytometry DNA ploidy in relation to established clinicopathological variables and outcome was investigated in 785 endometrial carcinoma patients prospectively included in the MoMaTEC multicentre trial. Results: Diploid curettage status was found in 72.0%, whereas 28.0% were non-diploid. Non-diploid status significantly correlated with traditional aggressive postoperative clinicopathological features, and was an independent predictor of lymph node metastasis among FIGO stage I-III patients in multivariate analysis (OR 1.94, P = 0.033). Non-diploid status was related to shorter disease-specific survival (5-year DSS of 74.4% vs 88.8% for diploid curettage, P<0.001). When stratifying by FIGO stage and lymph node status, the prognostic effect remained. However, in multivariate regression analysis, preoperative histological risk classification was a stronger predictor of DSS than DNA ploidy. Conclusions: Non-diploid curettage is significantly associated with aggressive clinicopathological phenotype, lymph node metastasis, and poor survival in endometrial cancer. The prognostic effect was also observed among subgroups with (presumably) less aggressive traits, such as low FIGO stage and negative lymph node status.     • The Sub-millimetre and Millimetre Radiometer (SMR) is the main instrument on the Swedish, Canadian, Finnish and French spacecraft Odin. It consists of a 1.1 metre diameter telescope with four tuneable heterodyne receivers covering the ranges 486-504 GHz and 541-581 GHz, and one fixed at 118.75 GHz together with backends that provide spectral resolution from 150 kHz to 1 MHz. This Letter describes the Odin radiometer, its operation and performance with the data processing and calibration described in Paper II.

    • This paper presents a powerful method to analyzeantennas which can be considered principally two-dimensional(2-D) or cylindrical, except for some three-dimensional (3-D)physical or equivalent sources, e.g., dipoles or slots. It is shown byFourier transform techniques that such antennas can be analyzedas 2-D problems with harmonic longitudinal field variation. Theradiation pattern can often be determined directly from a finiteset of such 2-D solutions, each one obtained by any method, e.g.,the moment method. The mutual interaction between the cylindrical scatterer and the sources must be calculated to determine theexact current distribution on the sources and their impedances oradmittances. This is facilitated by performing an inverse Fouriertransform of an infinite spectrum of the numerical 2-D solutionsfollowed by a moment method solution in the spatial domain tosatisfy the boundary conditions on the 3-D equivalent sourcesthemselves. The inverse Fourier transform is simplified by theuse of asymptote extraction. The method is in itself a hybridtechnique as one method is used to solve the harmonic 2-Dproblem, and the other to solve for the source currents

    • As more and more advanced wireless technologies are incorporated into vehicles, there is a need to refine the test methods used for assessing the performance. Traditional test setups for multiple-input multiple-output (MIMO) communication systems have been developed for relatively small devices such as handsets and tablets. At the same time, there is a desire to replace vehicular field tests with lab testing, for more efficient and accurate testing. This paper evaluates over-the-air verification methods for MIMO communication systems, which may incorporate vehicle-sized antenna systems and that can be implemented in a lab. Different methods are compared by means of a round robin campaign. The results from the campaign indicate that performance assessment representative to real-world operation can be achieved.

    • In wireless communication systems, multipath interference has a significant impact on system design and performance. Fast fading is caused by the coherent summation of one or more echoes from many reflection points reaching the receive antenna. Antenna diversity can be used to mitigate multipath fading. The main challenge of antenna diversity in practical application is the integration of multiple antennas on a small ground plane. Two-element antenna structure based on coupling element antenna concept for diversity application has been studied in previous work and it has shown to be feasible for low frequency diversity application with stable and low envelope correlation across wide frequency range. The studied structure has been tuned to be operating at 1600 MHz for measurement evaluation purpose. This paper presents an analysis of diversity performance of the structure in terms of efficiencies, envelope correlation, and apparent and effective diversity gains in isotropic (i.e. statistically uniform) propagation channel, using spherical near-field technique in an ideal line-of-sight environment (anechoic chamber) and inside reverberation chamber. The envelope correlation is less than 0.1 and the diversity gain is equal to 10.2 dB at 99% reliability level using selection combining technique across simulation and both measurement methods. The measurement techniques are compared to show how accurately the diversity performance of a mobile terminal antenna can be estimated.

The National Center for Scientific Research (CNRS) in France launched a number of specific actions (SA) in order to identify the most challenging issues to be investigated by the French research community. One of these specific actions is dedicated to antenna miniaturisation. The aim of this action is to provide a state of the art and new direction of studies in the following topics: antenna miniaturisation techniques and designs, evaluation of small antenna Q and fundamental limitation in miniaturisations, software issues: computing and benchmark, comparison of different small antennas efficiency measurement methods. This last part gathers three French university laboratories (IETR, IREENA, LEAT) and two French companies (CEA-LETI, SAGEM). Recently, three companies (TCL & ALCATEL Mobile Phones, Bluetest AB, IMST GmbH) and the Chalmers University of Technology have been interested to participate to the efficiency round robin measurements. This paper presents the work already done from January to December 2004.

    • The paper deals with reverberation chambers for over-the-air (OTA) testing of wireless devices for use in multipath. We present a formulation of the S-parameters of a reverberation chamber in terms of the free space S-parameters of the antennas, and the channel matrix in the way this is known from propagation literature. Thereby the physical relations between the chamber and real-life multipath environments are more easily explained. Thereafter we use the formulation to determine the uncertainty by which efficiency-related quantities can be measured in reverberation chamber. The final expression shows that the uncertainty is predominantly determined by the Rician K-factor in the reverberation chamber rather than by the number of excited modes, assumed by previous literature. We introduce an average Rician K-factor that is conveniently expressed in terms of the direct coupling between the transmitting and receiving antennas (corresponding to a line-of-sight contribution) and Hill's transmission formula (corresponding to a multipath or non-line-of-sight contribution). The uncertainty is expressed in terms of this average K-factor and geometrical mode stirring parameters, showing strong reduction by platform and polarization stirring. Finally the formulations are verified by measurements, and the new understanding of uncertainty is used to upgrade an existing reverberation chamber to better uncertainty.

    • This contribution discusses the relationship between the number of degrees of freedom (DoF) and the maximum directivity for an arbitrary set of sources enclosed by a given spherical surface. In particular, it is shown that the number of DoF is equal to twice the maximum directivity. This provides a directivity limitation for arbitrarily sized antennas, thus, extending the validity of the known formulas for electrically large and electrically small antennas.

    • A novel dual polarized ultrawide-band (UWB) feed with a decade bandwidth is presented for use in both single and dual reflector antennas. The feed has nearly constant beam width and 11 dBi directivity over at least a decade bandwidth. The feed gives an aperture efficiency of the reflector of 66% or better over a decade bandwidth when the subtended angle toward the sub or main reflector is about 53° and an overall efficiency better than 47% including mismatch. The return loss is better than 5 dB over a decade bandwidth. The calculated results have been verified with measurements on a linearly polarized lab model. The feed has no balun as it is intended to be integrated with an active 180° balun and receiver. The feed is referred to as the Eleven antenna because its basic configuration is two parallel dipoles 0.5 wavelengths apart and because it can be used over more than a decade bandwidth with 11 dBi directivity. We also believe that 11 dB return loss is achievable in the near future.

    • The newly introduced parallel-plate ridge gap waveguide consists of a metal ridge in a metamaterial surface, covered by a metallic plate at a small height above it. The gap waveguide is simple to manufacture, especially at millimeter and sub-millimeter wave frequencies. The metamaterial surface is designed to provide a frequency band where normal global parallel-plate modes are in cutoff, thereby allowing a confined gap wave to propagate along the ridge. This paper presents an approximate analytical solution for this confined quasi-TEM dominant mode of the ridge gap waveguide, when the metamaterial surface is an artificial magnetic conductor in the form of a bed of nails. The modal solution is found by dividing the field problem in three regions, the central region above the ridge and the two surrounding side regions above the nails. The fields within the side regions are expressed in terms of two evanescent TE and TM modes obtained by treating the bed of nails as an isotropic impedance surface, and the field in the central ridge region is expanded as a fundamental TEM parallel-plate mode with unknown longitudinal propagation constant. The field solutions are linked together by equalizing longitudinal propagation constants and imposing point-continuity of fields across the region interfaces, resulting in a transcendental dispersion equation. This is solved and presented in a dispersion diagram, showing good agreement with a numerical solution using a general electromagnetic solver. Both the lower and upper cutoff frequencies of the normal global parallel-plate modes are predicted, as well as the quasi-TEM nature of the gap mode between these frequencies, and the evanescent fields in the two side regions decay very rapidly away from the ridge.

    • Radiation leakages are a considerable problem when measuring waveguide structures at high frequencies. In order to maintain good electrical contact, flanges need to be tightly and evenly screwed to the device under test. This can be a time-consuming operation, especially with repeated measurements. We present a metamaterial-based adapter, which prohibits leakage even in the presence of gaps at the interconnects. This so-called gap adapter has been fabricated from a metallized polymer (SU8). The reflection coefficient is below -20 dB throughout the band for a 50-mu m gap on both sides of the gap adapter. In comparison, a conventional waveguide with a 50-mu m gap on both sides has a reflection coefficient of -10 dB. The gap adapter can be used to perform fast measurements, since the normal flange screws are redundant. We compare the SU8 gap adapter with a Si version and to a smooth metal waveguide reference disc. The SU8 gap adapter performed better than the Si version and much better than the waveguide disc in all test cases. SU8 gap adapters were used to measure on a waveguide component. The SU8 gap adapters with 50-mu m gaps performed comparable with the waveguide component with the flange screws carefully tightened. The polymer also makes the gap adapter mechanically robust and easy to mass fabricate. [2015-0113]

    • A wideband 2x2-slot element for a 60-GHz antenna array is designed by making use of two double-sided printed circuit boards (PCBs). The upper PCB contains the four radiating cavity-backed slots, where the cavity is formed in substrate-integrated waveguide (SIW) using metalized via holes. The SIW cavity is excited by a coupling slot. The excitation slot is fed by a microstrip-ridge gap waveguide formed in the air gap between the upper and lower PCBs. The lower PCB contains the microstrip line, being short-circuited to the ground plane of the lower PCB with via holes, and with additional metalized via holes alongside the microstrip line to form a stopband for parallel-platemodes in the air gap. The designed element can be used in large arrays with distribution networks realized in such microstrip-ridge gap waveguide technology. Therefore, the present paper describes a generic study in an infinite array environment, and performance is measured in terms of the active reflection coefficient S11 and the power lost in grating lobes. The study shows that the radiation characteristics of the array antenna is considerably improved by using a soft surface EBG-type SIW corrugation between each 2x2-slot element in E-plane to reduce the mutual coupling. The study is verified by measurements on a 4x4 element array surrounded by dummy elements and including a transition to rectangular waveguide WR15.

    • Aims To explore sex-specific time trends in atrial fibrillation (AF) incidence and to estimate the impact of changes in risk factor levels using individual participant-level data from the population-based Tromso Study 1994-2016. Methods and results A total of 14 818 women and 13 225 men aged 25 years or older without AF were enrolled in the Tromso Study between 1994 and 2008 and followed up for incident AF throughout 2016. Poisson regression was used for statistical analyses. During follow-up, age-adjusted AF incidence rates in women decreased from 1.19 to 0.71 per 1000 person-years. In men, AF incidence increased from 1.18 to 2.82 per 1000 person-years in 2004, and then declined to 1.94 per 1000 person-years in 2016. Changes in systolic blood pressure (SBP) and diastolic blood pressure (DBP), body mass index (BMI), physical activity, smoking and alcohol consumption together accounted for 10.9% [95% confidence interval (CI): -2.4 to 28.6] of the AF incidence decline in women and for 44.7% (95% CI: 19.2; 100.0) of the AF incidence increase in men. Reduction in SBP and DBP had the largest contribution to the decrease in AF incidence in women. Increase in BMI had the largest contribution to the increase in AF incidence in men. Conclusion In the population-based Tromso Study 1994-2016, AF incidence decreased in women and increased following a reverse U-shape in men. Individual changes in SBP and DBP in women and individual changes in BMI in men were the most important risk factors contributing to the AF incidence trends.

    • The problem of determining the eigenmodes of a rectangular waveguide with one hard wall formed by longitudinal corrugations with grooves filled with dielectric is considered. The characteristic equation is derived by using the asymptotic boundary conditions for corrugated surfaces. It is shown analytically that if the groove depth is equal to the value 0.25 lambda/(epsilon - 1)(1/2) corresponding to the hard wall condition, the TE eigenmode spectrum of the waveguide contains an infinite set of new non-uniform quasi-TEM modes with different transverse propagation constants in the empty part and identical longitudinal propagation constants equal to the wavenumber k. Analytical solution for the case of excitation of the waveguide by a specified source is given, and an example of forming local quasi-TEM waves is considered and discussed.

    • The use of perforator flaps in breast reconstructions has increased considerably in the past decade. A disadvantage of the perforator flap is difficult dissection, which results in a longer procedure. During spring 2006, we introduced CT angiography (CTA) as part of the diagnostic work-up in perforator flap reconstructions to visualise each perforator more accurately. The main objectives were to reduce surgery time and the number of complications. A chart review was conducted 1 year after CTA introduction to investigate if these objectives were met. MATERIALS AND METHODS: Patients with a deep inferior epigastric perforator (DIEP) flap who underwent preoperative analysis through CTA were retrospectively evaluated. The population <0.001) than in the control group, 264 min (SD+/-62) versus 354 min (SD+/-83), respectively. There was a tendency for fewer complications in the CTA group compared with the control group. All flaps were successful in the CTA group. In the control group, one flap failed and partial necrosis occurred in three flaps. The differences were not statistically significant. CONCLUSIONS: Preoperative CTA in the assessment of vascular anatomy during perforator flap reconstruction was safe and reliable. It helped reduce surgery time, and may prevent the number of postoperative complications.

    • Characterization of efficiency is an important consideration to ensure the proper operation of an all-textile wearable antenna. In this work, a small, novel textile antenna is designed based on a planar inverted-F structure (PIFA). The structure incorporates an edge-feeding plate, symmetrical design, and well-placed slot, in order to ease fabrication. A thin conductive textile, Pure Copper Polyester Taffeta Fabric (PCPTF), and fleece is used to fabricate the structure. The efficiency is then verified and measured using a commercial reverberation chamber. Observed results indicate excellent agreements between simulations and measurements.

    • A new configuration is introduced to integrate diplexers and power dividers. The proposed configuration is based on coupling matrix. The design of the lumped element network is based on addition of an extra term to the conventional error function of the coupling matrix to decouple the two ports of the power divider. An optimized lumped element network is implemented successfully on an EBG based guiding technology known as ridge gap waveguide. The optimization of the physical structure is done efficiently by dividing the diplexer-power divider into many sub-circuits and analyzing the corrected delay response of them.

    • This paper describes a novel design for a direct-coupled cavity filter realization using a ridge gap waveguide technology. A ridge gap waveguide transmission line with two coaxial feed connectors is designed and operated within the frequency band of 10-13 GHz. A cavity is coupled to the transmission line to achieve bandstop filter characteristic. Then, the structure of the filter is modified in a way to generate a bandpass feature. The final manufactured prototype is a fourth-order bandpass filter, operating at the center frequency of 11.59 GHz with a bandwidth of 72 MHz. The proposed design has potential applications in channeling filters for telecommunication satellites.

    • The Finite Difference Time Domain (FDTD) method is applied to industrial microwave ovens used in the food industry. Theoretical and practical aspects of FDTD analysis for this purpose are dealt with. Two different open-ended applicator types are described and analyzed numerically and experimentally and their relative performance is discussed. One of these applicators is a novel design based on low impedance TM modes which reduces edge-overheating and load reflections. It is shown that the common belief that the heating pattern from open-ended microwave applicators can be predicted from their radiation fields alone is often in error, and that resonances in the whole system of applicators and food loads are of major importance.

    • This paper presents a wideband and high efficiency corporate-fed horn array antenna in the E-band. A horn antenna is used as the unit cell of the array. The radiation pattern and grating lobe levels of the unit cell are improved by splitting the horn's aperture with a septum in the E-plane. A ridge gap waveguide corporate distribution network feeds the horns with the same amplitude and phase via coupling apertures. A prototype consisting of 4 × 4-element horn array antenna is manufactured by Direct Metal Laser Sintering (DMLS) 3D printing technique. The proposed antenna has a relative bandwidth of 24% with input reflection coefficient better than -10 dB and simulated total antenna efficiency better than 85% over the 69-88 GHz frequency band.
