    • An experiment will be reported which assessed speech intelligibility and free recall of spoken words in Swedish (native tongue) and in English heard under different signal-to-noise (S/N) ratios (+3 and +12 dB), and with/without the spoken words being repeated back orally directly after presentation (shadowing). All participants encountered all experimental conditions. Twelve wordlists with 12 words each were generated in English as well as in Swedish. The words were chosen according to their ranks in category norms for the two languages, and no category was the same for the two languages. Blocks of counter balanced presentation orders, S/N-ratios and shadowing/no shadowing were generated. After each wordlist the participants wrote down the words they could recall. Pre-experimental measures of working memory capacity were taken. The basic hypotheses for the recall of the words were that working memory would be overloaded when the S/N-ratio was low, there was no shadowing and when the language was English. A low score on working memory capacity was expected to further enhance these effects. While writing this abstract data collection is still in progress but results will be presented at the conference.

  • A direct observation instrument for assessment of nurses’ patient transfer technique (DINO)

    • The aim of this project was to develop a direct observation instrument to assess the work technique of nursing personnel during patient transfers and to test the validity and reliability of the instrument. An expert group developed this instrument, called DINO (Direct Nurse Observation instrument for assessment of work technique during patient transfers), which contains 16 items divided into three phases of a transfer: the preparation, performance and result phases. To quantify the assessments a scoring system was constructed, giving an overall score for each transfer, depending on the level of musculoskeletal health and safety. Four observers assessed 45 patient transfers at hospital wards and showed in an evaluation that the inter-observer reliability and criterion-related validity of DINO was satisfactory. The assessments with the DINO instrument are done directly when the transfer occurs, without costly equipment. Therefore, it has a wide range of applications. For example, when evaluating training in work technique or when identifying an unsafe work technique as a risk factor for musculoskeletal problems in epidemiological studies. (C) 2004 Elsevier Ltd.   • Radiation-induced cystitis treated with hyperbaric oxygen therapy (RICH-ART): a randomised, controlled, phase 2–3 trial

    • © 2019 Elsevier Ltd Background: Late radiation cystitis is an adverse effect of cancer treatment with radiotherapy in the pelvic region. Symptoms of late radiation cystitis can be assessed with the Expanded Prostate Index Composite Score (EPIC). Previous reports indicate that hyperbaric oxygen therapy reduces symptoms from late radiation cystitis, but the evidence is predominantly based on non-randomised and retrospective studies. We aimed to assess whether hyperbaric oxygen therapy would mitigate symptoms of late radiation cystitis. Methods: We did a randomised, controlled, phase 2–3 trial (RICH-ART [Radiation Induced Cystitis treated with Hyperbaric oxygen—A Randomised controlled Trial]) at five Nordic university hospitals. All patients aged 18–80 years, with pelvic radiotherapy completed at least 6 months previously, a score of less than 80 in the urinary domain of the Expanded Prostate Index Composite Score (EPIC), and referred to participating hyperbaric clinics due to symptoms of late radiation cystitis, were eligible for inclusion. Exclusion criteria were ongoing bleeding requiring blood transfusion exceeding 500 mL in the past 4 weeks, permanent urinary catheter, bladder capacity less than 100 mL, fistula in the urinary bladder, previous treatment with hyperbaric oxygen therapy for late radiation injuries, and contraindications to hyperbaric oxygen therapy. After computer-generated 1:1 randomisation with block sizes of four for each stratification group (sex, time from radiotherapy to inclusion, and previous invasive surgery in the pelvic area), patients received hyperbaric oxygen therapy (30–40 sessions, 100% oxygen, breathed at a pressure of 240–250 kPa, for 80–90 min daily) or standard care with no restrictions for other medications or interventions. No masking was applied. The primary outcome was change in patient-perceived urinary symptoms assessed with EPIC from inclusion to follow-up at visit 4 (6–8 months later), measured as absolute change in EPIC urinary total score. RICH-ART closed enrolment on Dec 31, 2017; the last follow-up data will be compiled in 2023. RICH-ART is registered with ClinicalTrials.gov, number NCT01659723, and with the European Medicines Agency, number EudraCT 2012-001381-15. Findings: Of 223 patients screened between May 9, 2012, and Dec 20, 2017, 87 patients were enrolled and randomly assigned to either hyperbaric oxygen therapy (n=42) or standard care (n=45). After excluding eight patients who withdrew consent directly after randomisation (one in the hyperbaric oxygen therapy group and seven in the standard care group), 79 were included in the intention-to-treat analyses (n=41 in the hyperbaric oxygen therapy group, n=38 in the standard care group). Median time from randomisation to visit 4 was 234 days (IQR 210–262) in the hyperbaric oxygen therapy group and 217 days (195–237) in the standard care group. The difference between change in group mean of EPIC urinary total score at visit 4 was 10·1 points (95% CI 2·2–18·1; p=0·013; 17·8 points [SD 18·4] in the hyperbaric oxygen therapy group vs 7·7 points [15·5] in the standard care group). 17 (41%) of 41 patients in the hyperbaric oxygen therapy group experienced transient grade 1–2 adverse events, related to sight and hearing, during the period of hyperbaric oxygen therapy. Interpretation: Our results suggest that hyperbaric oxygen therapy relieves symptoms of late radiation cystitis. We conclude that hyperbaric oxygen therapy is a safe and well tolerated treatment.   • Measures against preschool noise and its adverse effects on the personnel : an intervention study

    • The aim of the study was to analyze the exposure effects of different types of noise measures carried out at preschools. The project was carried out as an intervention study. The investigation included 89 employees at 17 preschools in the northern part of Sweden. Individual noise recordings and recordings in dining rooms and play halls were made at two departments in each preschool. The adverse effects on the employees were analyzed with validated questionnaires and saliva cortisol samples. Evaluations were made before and 1 year after the first measurement. Between the two measurements, measures were taken to improve the sound environments at the preschools. The effects of the measures varied a lot, with respect to both the sound environments and health. Regarding acoustical measures, significant changes were seen for some of the variables analyzed. For most of the tested effects, the changes, however, were very small and non-significant. The effects of organizational measures on the objective and subjective noise values were in overall less pronounced. Acoustical measures improved the subjectively rated sound environment more than organizational measures. This may be due to the high work effort needed to implement organizational measures. Even though the sound level was not lower, the personnel experienced improvements of the sound environment.

  • Minimizing speech contribution using different microphone noise dosimeter positions

    • Use of noise dosimeter recordings in low sound level environments is often problematic due to the voice contribution from the carrier. The aim of the study was to find a microphone position with low speech contribution while still providing an accurate sound level recording. An experiment with different microphone positions was conducted in a low sound reverberation room. Two types of noise dosimeters were used (Brüel & Kjaer 4445 and Larson Davis Spark 706-Atex). In repeated measurements the carrier was instructed to read a text chapter during 60 seconds and at a speech of level about 60 dB(A), acquired by practice, with different background noise and noise levels. White noise and preschool noise was used at sound levels 50, 60, 70 and 80 dB(A). The voice contribution to the background noise level was then measured. Three microphone positions were tested; on the shoulder, above the ear and on the back of the head. The position with the microphone placed behind the head of the carrier had the lowest speech contribution, less than 2 dB(A) to the background noise at 70 dB(A). This compared to positions right above the ear 6 dB(A and on the shoulder 12 dB(A).

The investigation included 101 employees at 17 preschools in Umea County, located in northern Sweden. Individual noise recordings and recordings in dining rooms and play halls were made at two departments from each preschool. The adverse effects on the employees were analyzed by use of different validated questionnaires and by saliva cortisol samples. Stress and energy output were pronounced among the employees, and about 30 of the staff experienced strong burnout syndromes. Mental recovery after work was low, indicated by remaining high levels of stress after work. The burnout symptoms were associated with reduced sleep quality and morning sleepiness. Cortisol levels supported the conclusion about pronounced daily stress levels of the preschool employees.

    • Hearing impairments and tinnitus are being reported in an increasing extent from employees in the preschool. The investigation included 101 employees at 17 preschools in Umeå county, Sweden. Individual noise recordings and stationary recordings in dining rooms and play halls were conducted at two departments per preschool. The effects of noise exposures were carried out through audiometric screenings and by use of questionnaires. The average individual noise exposure was close to 71 dB(A), with individual differences but small differences between the preschools. The noise levels in the dining room and playing halls were about 64 dB(A), with small differences between the investigated types of rooms and preschools. The hearing loss of the employees was significantly higher for the frequencies tested when compared with an unexposed control group in Sweden. Symptoms of tinnitus were reported among about 31% of the employees. Annoyance was rated as somewhat to very annoying. The voices of the children were the most annoying noise source. The dB(A) level and fluctuation of the noise exposure were significantly correlated to the number of children per department. The preschool sound environment is complex and our findings indicate that the sound environment is hazardous regarding auditory disorders. The fluctuation of the noise is of special interest for further research.

    • This thesis is based on noise recordings and health evaluations carried out at preschools in the northern part of Sweden. Sound level recordings were made on individuals and by use of stationary devices in dining rooms and play halls. Health evaluations were based on ratings by use of questionnaires and by analyses of cortisol.The average equivalent individual noise exposure was 71 dB(A). The average equivalent noise levels in the dining room and playing halls were 64 dB(A). The hearing loss of the employees was significantly higher for the frequencies tested than in an unexposed control group. Symptoms of tinnitus were reported among 31% of the employees. Noise annoyance was rated as somewhat to very annoying, and the voices of the children were the most annoying noise source. The dB(A) level and fluctuations of the noise exposure were significantly correlated with the number of children per department. Stress and energy output were pronounced among the employees. About 30% of the staff experienced strong burnout syndromes. Mental recovery was low as indicated by noise fatigue and high levels of stress after work. Increased cortisol levels during work were associated with higher number of children present at the department.An essential finding of the thesis was that noise and noise sources may impair the pedagogic work, thereby increasing the work load of employees. It is concluded that noise exposure in the preschool, isolated or in combination with other stressors, plays a fundamental role in the building up of acute as well as long term stress. An intervention study implementing six acoustical and seven organizational measures was tested, aimed to improve the noise situation in the departments. Acoustical measures improved the noise situation as well as the rated noise experiences better than the organizational measures.

    • The study included 93 employees at 17 preschools in the county of Umeå located in northern part ofSweden. Personal daily noise dosimeter recordings were made at five representative work days.Stationary noise recordings were made during the same days at two departments of each preschool, in theplaying halls and in the dining rooms. Besides audiometric tests, the employees rated their experiences ofthe noise, hearing and tinnitus as well as well as different health effects, on validated questionnaires.Tinnitus was reported among 31 per cent of the participants. The study group was dichotomized intoemployees with or without tinnitus. Employees with tinnitus reported higher prevalence of subjectivehearing loss, higher experiences of elevated sound levels at work, anxiety of the noise at work, chestpressure/pain, burn out symptoms, depression and reduced sleep quality. Significant differences wereseen for shoulder tension/pain. No group differences were seen for the objective personal or stationarynoise measures or the number of children present at the department. The results of the study are discussedin terms of underlying causes and the way in which the symptom interfere with experiences and healtheffects of the employees.

  • Work related stress and stressors in the preschool environment

    • The study included six departments, with two employees at each department, characterized by high levels of stress and burnout and six departments, with two employees at each department, characterized by low levels of stress andburnout. A total of 24 females with a mean age of 43.5 years participated in the study. The employees rated stress at work and burnout by use of different questionnaires. Cortisol samples were collected at wake up, one hour afterwake up, at 11:00 am and at 09:00 pm. An observation study was made to create an overview of the interactionbetween the children and the personnel during periods described as overloading.More than 50% of the employees stated that all members of the staff did not have equal commitment to the work. About 8% considered this stressful to a high degree. More than half found their increased administrative work task as stressful to a high degree. Half of the personnel felt it stressful to a high degree, not being able to give all the children the attention they needed.The highest stressors regarding the organization of the work were when conflicts occurred among the personnel. The highest stressors regarding noise were the children´s voices closely followed by noise when changing the children´sclothes. The highest stressor regarding the constitution of the child group was when the personnel experienced that they had a child in the group they thought needed special support. In general, the high stress group rated all stressors as higher compared to the low stress group. Significant differences were pronounced among stressors regarding“organization of the work” and “work situations”. The results also indicate that individuals who are highly stressed by noise from the childrens activities rated their rewards as lower. The high stress group had more communications to and from the children, both before lunch and during lunch.An essential conclusion, is also that noise and noise sources, may impair the work situation of the employees fundamentally, thereby increasing the stress levels. It is assumed that noise exposures in the preschool, isolated or in combination with other stressor, plays a fundamental role in the building up acute as well as long term stress. One of the stand points of the study is that overloaded communication between the personnel and children might be a pronounced factor behind the situational and longterm stress.

    • Objective: To investigate the amount and pattern of changes of maxillary front teeth 7 years postretention, which previously were retained with a bonded retainer. Materials and Methods: The study group consisted of 27 patients. Study models before treatment (T1), at debonding (T2), 1 year after removal of the upper bonded retainer (T3), and 7 years postretention (T4) were present. The irregularity index (sum of contact point displacements) and the rotations of front teeth toward the raphe line were calculated. Results: The irregularity index of the maxillary front teeth changes very little or not at all during the first year postretention. Further change long term resulted in an irregularity index of mean 2.0 (range 0.0–5.8). The contact relationship between the laterals and centrals seems to be the most critical. Forty rotated teeth in 21 patients were corrected more than 20°. Mean relapse during the first year postretention was 6.7° (range 0.0°–14.7°). Mean changes during 7 years was 8.2° (range 0.0°–19.3°). Conclusions: Relapse of upper front teeth retained with a bonded retainer is minor in both the short and long term. If permanent retention is required after 3 years of retention, it is enough to retain the incisors.

    • The proposed European Union directive on minimum wages has met strong criticism in Scandinavia. But up to now the debate has lacked specific instances of what statutory minimum wages would mean for the Swedish labour market. A key feature of the Swedish industrial-relations system is that unions and employers regulate most aspects of the labour market through collective agreements. The Swedish-or rather Nordic-model has repeatedly shown its vitality, solving difficult problems and contributing to high employment and wage growth, while largely avoiding government intervention. This model is threatened by the envisaged EU minimum-wage legislation.

    • For the past months, Finland has been shaken by recurring strike waves, involving hundreds of thousands of workers in all sectors of the economy. The strikes are political, not directed at any particular company or related to bargaining demands. Instead, the primary target is the conservative Finnish government, and its plan to implement a wide set of policies aimed at improving the manufacturing sector’s competitiveness after many years of weak economic growth. If passed, these policies would weaken unions significantly, and contribute to the employer strategy to dismantle the Finnish sectoral bargaining model.

    • Noise sources with a dominating content of low frequencies (20-200 Hz) are found in many occupational environments. This study aimed to evaluate effects of moderate levels of low-frequency noise on attention, tiredness and motivation in a low demanding work situation. Two ventilation noises at the same A-weighted sound pressure level of 45 dB were used: one of a low-frequency character and one of a flat frequency character (reference noise). Thirty-eight female subjects worked with six performance tasks for 4 h in the noises in a between-subject design. Most of the tasks were monotonous and routine in character. Subjective reports were collected using questionnaires and cortisol levels were measured in saliva. The major finding in this study was that low-frequency noise negatively influenced performance on two tasks sensitive to reduced attention and on a proof-reading task. Performances of tasks aimed at evaluating motivation were not significantly affected. The difference in work performance was not reflected by the subjective reports. No effect of noise was found on subjective stress or cortisol levels.

    • Noise sources with a dominant content of low frequencies (20-200 Hz) are found in many occupational environments. Work efficiency has been found in two earlier studies to be impaired to a larger degree when working in a low frequency ventilation noise than when working in a flat frequency ventilation noise at the same A-weighted sound pressure level. Other previous studies indicate that different sound characteristics found in low frequency noise are important for the way in which low frequency noise affects humans, and better knowledge of these characteristics could lead to better methods for assessing low frequency noise in occupational environments. The aim of this study was to evaluate the influence of frequency balance between low and high frequencies, which affects the slope of the frequency spectra, and the modulation frequency on subjects´ perception of a pleasant low frequency noise. The results showed that the subjects preferred either a higher or a lower modulation frequency as compared to that of the original low frequency noise, both choices leading to less perceivable modulations. Furthermore, the subjects preferred a lower relative content of frequencies below 500 Hz, but only as long as the original low frequency noise contained modulations and the variations were made within a constant A-weighted sound pressure level. The results are discussed in relation to improved guidelines and to the previously suggested theory of slope of the frequency spectra and problems involved with the implementation of that theory.

    • BACKGROUND: More knowledge is needed of occupational attainment of persons with disabilities, i.e. the relationship between their educational level and their profession, and factors of importance for this relationship.OBJECTIVE: To compare occupational attainment among persons with and without a disability.METHOD: 3396 informants with disabilities and 19004 non-disabled informants participated (control group) in a survey study by Statistics Sweden. The informants with disabilities were divided into six groups.RESULTS: Occupational attainment did not differ between the disability groups, neither between persons with and without a disability. Follow-up analysis showed that men with disabilities with primary or secondary school had an occupation above their educational level to a significantly larger extent than women with disabilities. This pattern was even clearer in comparison with the control group. Persons without disabilities, with secondary or higher education, were more successful in the labor market than persons with disabilities. Occupational attainment increased with age in both groups.CONCLUSIONS: Young women with disabilities who only have primary or secondary education run a higher risk of having a job that is below their educational level than men at the same educational level. This indicates discriminating mechanisms in the society related to gender and ability.

    • The employment status of groups with different disabilities wasanalysed as were potentially important moderating factors (workability, structural and individual factors). A secondary analy-sis was performed on 4359 respondents with disabilities fromStatistics Sweden's Labour Market Investigation. The respondentswere divided into six disability groups (communicative-hearing,communicative-speech-reading, communicative-vision, psycho-logical disability, medical disability, physical disability). Logisticregression analyses showed that the probability of being employedwas highest among respondents with hearing disabilities andrespondents with psychological disabilities were least likely tobe employed. Being a woman (very young or old) with onlyprimary education and with partially or very impaired workability, reduced employment opportunities. Higher education didnot increase employment opportunities for respondents withimpaired work ability. In summary, the type of disability is essentialfor employment opportunities, and differences between disabil-ity groups cannot be explained by differences in other variables.

    • The overall aim of this thesis is to explore how the labour market situation appears to persons with different types of disabilities in Sweden. Study I (n=4 359) examined the importance of the person’s type of disability for the possibility of having a job. Study II (n=3 396) examined whether there were differences in occupational attainment between persons with different types of disabilities and between persons with and without a disability (n=19 004). Study III (n=4 359) examined perceived discrimination in the workplace among persons with different types of disabilities. Study IV (n=2 268) examined whatsupport and adaptation persons with different types of disabilities need in their actual workplace or to enter the labour market. In studies I–III the respondents were divided into six disability types, and in study IV the respondents were divided into four disability types.Overall, it can be concluded that a person’s type of disability is of importance for the situation in the labour market, where persons with psychological disabilities encounter barriers to a greater extent than other groups, both difficulties at the present workplace and difficulties entering the labour market. Further, they were subjected to discrimination and they expressed a great need for adaptations to acquire employment and at their actual workplace. It can also be concluded that there are discriminatory mechanisms against women, who have more difficulties than men in acquiring employment, and that men, despite lower education, can more easily progress in the labour market. It is suggested that, in order to achieve inclusion and recognition, factors in the environment, such as existing support systems, need to be changed and developed, and social prejudices vis-à-vis people with disabilities must also be addressed.

    • This study aimed to examine support and adaptation needed by persons with four different types of disabilities, either at their workplace or in order to enter the labour market. A secondary analysis was conducted, based on the regular labour market survey (AKU), and an additional survey which included questions specifically posed to persons with disabilities. All participants had either partly or very reduced work ability. There was more need for adaptation than for support, independently of work ability and current employment status. Around 60% of respondents with a psychological disability reported a need for adapted work pace and working time, both at their workplace and in order to enter the labour market. Adapted work tasks were an important factor, especially for persons who were not currently working; around half of the non-employed participants said this adaptation could make it possible for them to work, regardless of type of disability.

    • Emåns vattensystem är känt som ett av få områden i Sverige som fortfarande har ett livskraftigt bestånd av den europeiska malen (Silurus glanis, L. 1758). Malen är klassad som akut hotad i Naturvårdsverkets rödlista över hotade djur- och växtarter i Sverige och är sedan länge fridlyst och således skyddad mot allt fiske (SFS 1994:1716). Under den senaste 20-årsperioden har utvecklingen av Sveriges malbestånd varit relativt okänd. I samarbete med Kronobergs län införskaffades därför ett antal parryssjor som testredskap för fiske efter mal. Under provfisken i Möckelns vattensystem sommaren 2003 (Lessmark 2003) och 2005 (Lessmark 2005) visade sig ryssjorna ge goda resultat för att indikera rekryteringsframgång (yngre årsklasser). Dessutom upplevdes metoden som praktisk och skonsam för fångad fisk. Syftet med 2006 års provfiske i Emån var att göra ett inventeringsfiske till grund för utvecklingen av ett övervakningsprogram för mal, få lokalkännedom om lämpliga fiskeplatser (stationer) samt att få en känsla av hur metoden fungerade vid olika förhållanden med tanke på vegetation- och bottenförhållanden samt strömhastighet. Dessutom var det värdefullt att få en uppfattning om malbeståndets storlek i samband med arbeten kring utrivningen av Emsfors kraftstation. Fisket utfördes från den 21:e augusti t.o.m. den 8:e september 2006 med 54 st parryssjor. 30 st med knutlös nätduk resp. 24 st med knuten duk. Fångstarmarnas fiskande längd och höjd skiljde sig mellan de två olika ryssjetyperna, 6,2 och 8,0 m resp. 0,54 och 0,62 m med den knutlösa variantens siffror nämnda först. Även första ingångsbågens diameter skiljde sig något (0,52 och 0,55 m). Stationerna utgjordes av länkar om 5, 10, 14 eller 15 st ryssjor. Utvalda stationer ligger alla i Emåns huvudfåra, från Ryngen uppströms Blankaström ca 65 km från mynningen till nedströms Karlshammar ca 5 km från mynningen. Avfiskade sektioner i ån, totalt 8 st, har värderats utifrån tidigare malbiotopskartering (Bäckstrand 1999), praktiska aspekter som båtisättningsmöjligheter, forspassager och tidsåtgång. Stationernas placering inom valda sektioner har bestämts visuellt på plats genom bedömning av troliga malbiotoper. I den mån det förekommit har också olika bottensubstrat och strömförhållanden fiskats av. Ryssjorna vittjades på morgon/förmiddag efter 1 dygns fiske med undantag för två tillfällen då vittjning skedde efter 2 dygn (Högsbyplatån och Karlshammarmagasinet). Fångade malar längdmättes till närmaste cm. Vattentemperatur noterades på 1 m djup vid varje vittjningstillfälle. Till grund för släktskapsanalys (DNA) mellan olika malar i Emåsystemet samlades det även in 83 st fenprover, 22 av dessa malar märktes dessutom med P.I.T.-tags (Fliserydsplatån). Provfisket genomfördes med 924 ansträngningar (1 ansträngning = 1 parryssjas fiske under 1 dygn). Totalt fångades 209 malar. Storleken varierade mellan 9 och 121 cm. Sektionerna Ryngen-Blankaström (8/9) och Blankaström-Högsby (7/9) fiskades med 84 resp. 98 ansträngningar utan att någon mal fångades. På övriga sektioner fångades mal och redovisas sammanfattningsvis i figur 2 och karta 1 och enskilt i figur 3 till 8 och karta 2 till 7. Som indikation på malbeståndets storlek mellan sektioner redovisas en kvot av antal fångade malar och ansträngning (tab. 1). Vattentemperaturen föll sakta från 20 till 17° C under fiskets gång (fig. 1).

    • The aim of the present study was to test two structural models of the relationship between appraised psychologicla workload and musculoskeletal symptoms from the neck, shoulder, and upper and lower back with different aspects of perceived fatigue as mediating variables. In this cross-sectional study a questionnaire survey was conducted among employees at three Swedish assembly plants (n=305). The proposed models were tested for one general fatigue dimension - lack of energy - and four specific fatigue dimensions - physical discomfort, physical exertion, lack of motivation, and sleepiness - using structural equation modeling. The results indicate that the role of perceived fatigue in the relationship between appraised workload and musculoskeletal symptoms is different for different aspects of fatigue. The general fatigue dimension, lack of energy, does not mediate the relationship. As regards the specific fatigue dimensions, the relationship is partially mediated by physical discomfort and lack of motivation but not by physical exertion or sleepiness. Appraised psychological workload has a unique effect on musculoskeletal symptoms not mediated by fatigue.

Methods 40 patients undergoing unicompartmental knee arthroplasty (UKA) were randomized into 2 groups in a double-blind study (ClinicalTrials.gov identifier: NCT00653926). In group A (active), 200 mg ropivacaine, 30 mg ketorolac, and 0.5 mg epinephrine (total volume 106 mL) were infiltrated intraoperatively into the soft tissue, while in group P (placebo), no injections were given. 21 hours postoperatively, 150 mg ropivacain, 30 mg ketorolac, and 0.1 mg epinephrine were injected intraarticularly via a catheter in group A, whereas patients in group P were injected with the same volume of saline (22 mL). Results Median hospital stay was shorter in group A than in group P: 1 (1-6) days as opposed to 3 (1-6) days (p 0.001). Postoperative pain in group A was statistically significantly lower at rest after 6 h and 27 h and on movement after 6, 12, 22, and 27 h. Morphine consumption was statistically significantly lower in group A for the first 48 h, resulting in a lower frequency of nausea, pruritus, and sedation. Postoperatively, there were improved functional scores (Oxford knee score and EQ-5D) in both groups relative to the corresponding preoperative values. Interpretation Local injection of analgesics periarticularly at the end of the operation and intraarticularly at 21 h postoperatively provided excellent pain relief and earlier home discharge following UKA. There was a high degree of patient satisfaction in both groups after 6 months.
