  • Predictors of mortality in solid organ transplant recipients with bloodstream infections due to carbapenemase-producing Enterobacterales : The impact of cytomegalovirus disease and lymphopenia

    • Treatment of carbapenemase-producing Enterobacterales bloodstream infections in solid organ transplant recipients is challenging. The objective of this study was to develop a specific score to predict mortality in solid organ transplant recipients with carbapenemase-producing Enterobacterales bloodstream infections. A multinational, retrospective (2004-2016) cohort study (INCREMENT-SOT, ClinicalTrials.gov NCT02852902) was performed. The main outcome variable was 30-day all-cause mortality. The INCREMENT-SOT-CPE score was developed using logistic regression. The global cohort included 216 patients. The final logistic regression model included the following variables: INCREMENT-CPE mortality score >= 8 (8 points), no source control (3 points), inappropriate empirical therapy (2 points), cytomegalovirus disease (7 points), lymphopenia (4 points), and the interaction between INCREMENT-CPE score >= 8 and CMV disease (minus 7 points). This score showed an area under the receiver operating characteristic curve of 0.82 (95% confidence interval [CI] 0.76-0.88) and classified patients into 3 strata: 0-7 (low mortality), 8-11 (high mortality), and 12-17 (very-high mortality). We performed a stratified analysis of the effect of monotherapy vs combination therapy among 165 patients who received appropriate therapy. Monotherapy was associated with higher mortality only in the very-high (adjusted hazard ratio [HR] 2.82, 95% CI 1.13-7.06, P = .03) and high (HR 9.93, 95% CI 2.08-47.40, P = .004) mortality risk strata. A score-based algorithm is provided for therapy guidance.

  • Microbiological diagnostics of bloodstream infections in Europe-an ESGBIES survey

    • Objectives: High-quality diagnosis of bloodstream infections (BSI) is important for successful patient management. As knowledge on current practices of microbiological BSI diagnostics is limited, this project aimed to assess its current state in European microbiological laboratories.Methods: We performed an online questionnaire-based cross-sectional survey comprising 34 questions on practices of microbiological BSI diagnostics. The ESCMID Study Group for Bloodstream Infections, Endocarditis and Sepsis (ESGBIES) was the primary platform to engage national coordinators who recruited laboratories within their countries.Results: Responses were received from 209 laboratories in 25 European countries. Although 32.5% (68/209) of laboratories only used the classical processing of positive blood cultures (BC), two-thirds applied rapid technologies. Of laboratories that provided data, 42.2% (78/185) were able to start incubating BC in automated BC incubators around-the-clock, and only 13% (25/192) had established a 24-h service to start immediate processing of positive BC. Only 4.7% (9/190) of laboratories validated and transmitted the results of identification and antimicrobial susceptibility testing (AST) of BC pathogens to clinicians 24 h/day. Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry from briefly incubated sub-cultures on solid media was the most commonly used approach to rapid pathogen identification from positive BC, and direct disc diffusion was the most common rapid AST method from positive BC.Conclusions: Laboratories have started to implement novel technologies for rapid identification and AST for positive BC. However, progress is severely compromised by limited operating hours such that current practice of BC diagnostics in Europe complies only partly with the requirements for optimal BSI management.

    • To foster a sustainable society within a sustainable environment, we must dramatically reshape our work and consumption activities, most of which are facilitated through software. Yet, most software engineers hardly consider the effects on the sustainability of the IT products and services they deliver. This issue is exacerbated by a lack of methods and tools for this purpose. Despite the practical need for methods and tools that explicitly support consideration of the effects that IT products and services have on the sustainability of their intended environments, such methods and tools remain largely unavailable. Thus, urgent research is needed to understand how to design such tools for the IT community properly. In this article, we describe our experience using design science to create the Sustainability Awareness Framework (SusAF), which supports software engineers in anticipating and mitigating the potential sustainability effects during system development. More specifically, we identify and present the challenges faced during this process. The challenges that we have faced and addressed in the development of the SusAF are likely to be relevant to others who aim to create methods and tools to integrate sustainability analysis into their IT products and services development. Thus, the lessons learned in SusAF development are shared for the benefit of researchers and other professionals who design tools for that end.

    • The sustainability analysis framework enables software developers to specifically consider environmental and social dimensions relative to technical and economic dimensions. The framework helps draw a more comprehensive picture of the relevant quality dimensions and, as a result, improve decision making. Sustainability is achievable only when accounting for all dimensions. Connections among the four dimensions involve different dependencies and stakeholders. The framework aims to capture the relevant qualities that characterize sustainability concerns of software systems, helping identify how these qualities influence each other with respect to the different aspects of sustainability. Moreover, interdependent quality requirements may influence one another, as in association/association-class influences among sustainability quality requirements. Integrating our four-dimensional sustainability analysis framework into the engineering processes of long-lived industrial systems provides valuable support to managers and engineers trying to satisfy not only economic and technical but also environmental and social sustainability requirements. The influences among the sustainability quality requirements must be determined by developers and/or stakeholders, as the framework can provide only the means for linking them but not the analysis itself. Constraints and parameters must be chosen by the developers, as it is not possible to list them in a way that is generic enough to be applicable in all circumstances and at the same time specific enough to be useful.
