    • To improve the accuracy of chemoradiation therapy in anal cancer patients PET/CT is frequently used in the planning of radiation therapy. A systematic review was performed to assess impact on survival, quality of life, symptom score, change in target definition and treatment intention. Systematic literature searches were conducted in Medline, EMBASE, the Cochrane Library, and Centre for Reviews and Dissemination. Ten cross-sectional studies were identified. No data were available on survival or quality of life. The summary estimate of the proportion of patients in which PET/CT had an impact on the target definition, was 23% (95% CI 16;33). The corresponding summary estimate of a change in treatment intent from curative to palliative was 3% (95% CI 2;6). Almost one in four patients had a change in target definition, which supports the use of PET/CT in radiation therapy planning, but the consequence regarding survival and quality of life is still uncertain.

    • The aim of this study is to understand and explain how street-level bureaucrats manage high workloads in public healthcare organisations. Data was collected through observations, interviews and documents. The results show that high workloads are reduced through use of collegial mechanisms at the organisation level. The study shows that workloads are reduced in a two-step process, the first is a negotiation between professionals and the second in the meeting with patients. The two-step process explains the limitation problems in more detail and makes an important contribution by explaining how high workloads are reduced in public health services.

  • Bio-propane from glycerol for biogas addition

    • In this report, the technical and economical feasibility to produce higher alkanes from bio-glycerol has been investigated. The main purpose of producing this kind of chemicals would be to replace the fossil LPG used in upgraded biogas production. When producing biogas and exporting it to the natural gas grid, the Wobbe index and heating value does not match the existing natural gas. Therefore, the upgraded biogas that is put into the natural gas grid in Sweden today contains 8-10 vol-% of LPG. The experimental work performed in association to this report has shown that it is possible to produce propane from glycerol. However, the production of ethane from glycerol may be even more advantageous. The experimental work has included developing and testing catalysts for several intermediate reactions. The work was performed using different micro-scale reactors with a liquid feed rate of 18 g/h. The first reaction, independent on if propane or ethane is to be produced, is dehydration of glycerol to acrolein. This was showed during 60 h on an acidic catalyst with a yield of 90%. The production of propanol, the second intermediate to producing propane, was shown as well. Propanol was produced both using acrolein as the starting material as well as glycerol (combining the first and second step) with yields of 70-80% in the first case and 65-70% in the second case. The propanol produced was investigated for its dehydration to propene, with a yield of 70-75%. By using a proprietary, purposely developed catalyst the propene was hydrogenated to propane, with a yield of 85% from propanol. The formation of propane from glycerol was finally investigated, with an overall yield of 55%. The second part of the experimental work performed investigated the possibilities of decarbonylating acrolein to form ethane. This was made possible by the development of a proprietary catalyst which combines decarbonylation and water-gas shift functionality. By combining these two functionalities, no hydrogen have to be externally produced which is the case of the propane produced. The production of ethane from acrolein was shown with a yield of 75%, while starting from glycerol yielded 65-70% ethane using the purposely developed catalyst. However, in light of this there are still work to be performed with optimizing catalyst compositions and process conditions to further improve the process yield. The economic feasibility and the glycerol supply side of the proposed process have been investigated as well within the scope of the report. After an initial overview of the glycerol supply, it is apparent that at least the addition of alkanes to biogas can be saturated by glycerol for the Swedish market situation at the moment and for a foreseeable future. The current domestic glycerol production would sustain the upgraded biogas industry for quite some time, if necessary. However, from a cost standpoint a lower grade glycerol should perhaps be considered. In the cost aspect, three different configurations have been compared. The three alternatives are ethane production, propane production with internal hydrogen supply and propane production with external hydrogen supply. The results from the base case calculations can be viewed in table ES1. The base case calculations are based on carburating the upgraded biogas, before introducing it to the natural gas grid, from a 24 GWh biogas production facility. This means that the production units supply an acceptable Wobbe index of the final upgraded biogas. The annual cost in table ES1 is the yearly cost of carburating the gas at a 24 GWh biogas site. From the base case, it is obvious that there are differences in glycerol consumption depending on what alternative is chosen. There are also investment cost differences. To further investigate the volatility of the prices, a blend of Monte Carlo techniques were used to generate multiple data sets. The conclusions from the simulations were that the ethane producing facility has a stronger dependence on the feedstock; it is hence more sensitive to changes in the feedstock cost. It is however not as sensitive to changes in investment cost. If the production cost is compared to the cost of fossil LPG used today, the cost of the LPG is 0.43 kr/kWh. This does however not include the taxation and transporting the fuel. Adding the taxation alone will put an additional 0.25 kr/kWh on the cost, totalling 0.68 kr/kWh. This compares well with the calculated production cost of 0.78 kr/kWh for ethane and with the 50% percentile acquired from the Monte Carlo simulations of 0.94 kr/kWh.

    • A distinguishing feature of Swedish child protection is the direct andvindirect influence on decision-making in individual cases by representatives appointed by their elected political parties. As members of local committees, they take the most interventionist and costly decisions themselves, informed by care proposals submitted by professional social workers. Other decisions are delegated to professional social workers. In direct decision-making, they are supposed to act as laypersons using their own judgement and experience, not as politicians. The aim of this paper is to describe and analyse these committees, their role and responsibilities, and possible influence of politics on child protection. A mixed method was used, with a survey sent to 467 representatives, structured interviews with 99 secretaries of local committees and data drawn from national statistics. The Swedish model is discussed as a hybrid system influenced not only by professional, bureaucratic, political and market governance logics but also by laypersons. One conclusion is that although child protection is directly influenced by politics, the reverse is also true. By exposing politicians to the difficult life circumstances and societal shortcomings experienced by vulnerable children, the system can, in turn, have an impact on politics at the municipal level.

    • Summary: In Sweden, decision making in child protection is heavily dependent on laypersons appointed to municipal child protection committees (CPCs). These laypersons have the power to decide upon proposals coming from professionals. Members of the committees are appointed from among their political party’s members. In the committee,they are supposed to act as judicious laypersons equipped with sound judgement. In this study, 31 committee meetings in three municipalities were followed using observational techniques and audio recordings. The appointed CPC members’ decisionmaking processes and communicative strategies to influence social work practice were analysed. Findings: The results show differences between the CPCs, indicating localized decisionmaking cultures. The representatives worked (not as party politicians, but) as a team with the professionals as counterparts. Communication strategies, most frequently questions, were used to govern the social work practice, over and above the decision making.Questions served both to obtain the information needed and also to exert normative influence by setting up a normative grid for ways to pass the committee. Applications: This research shows that social work in Sweden has a long way to go until it is acknowledged full professional status. The study also suggests that improvements in the CPCs’ decision making need to be based on a closer look at the laypersons’ knowledge, values and common sense and how these aspects affect decision making.

    • The establishment of strategic coordination bodies with members from different agencies and that are governed by various laws and regulations can be understood as an answer to the demand for improved coordinated care for citizens with complex needs, such as frail older people. However, this demand raises fundamental questions of democratic control and accountability in the modern welfare state. Although these issues are addressed in current literature on network governance, they have not been investigated empirically very much. The aim of this paper is to investigate coordination bodies as important actors in integrated care, and especially to investigate how the members of these governance networks perceive their own influence and how they are held accountable by their principals. This study is conceptually built on theories of network governance and accountability. The empirical investigation is based on a survey with 545 respondents from 73 different coordination bodies in Sweden. The analysis shows that there seems to be an imbalance between perceived influence and perceived demands from different stakeholders to account for the services. This imbalance provides an opening for a discussion of how to improve the current situation for vulnerable groups and about new perspectives on accountability and power in the modern welfare state.

This demand is also an example of the development ‘from government to governance’, but raises important fundamental questions of democratic control and accountability in the modern welfare state. These issues are addressed in current literature on network governance, but are not investigated very much empirically. The overall aim of the paper is to analyze collaboration bodies as a phenomenon in the modern welfare state. The specific aim is to investigate how the members of these governance networks perceive power and accountability, and are held accountable by their principals. The paper is conceptually built on theories of network governance and accountability. The empirical investigation is based on a survey with 981 respondents in 73 different coordination bodies in Sweden. The analysis shows that there seems to be an imbalance between perceived power and perceived demands from different stakeholders to account for the services. It opens up for a discussion of the meaning of the post-liberal state, and new perspectives on accountability and power in the modern welfare state.

The experiencesfrom the Swedish Open Comparisons (OC) system havebeen highlighted as a particularly interesting case because itbears similarities with both Anglo-Saxon countries concern-ing coverage, scope, and transparency, and also with Con-tinental European countries concerning voluntarism, sanc-tions, and policy sensitivity. The aim of this article is to inves-tigate how managers and senior officials within the Swedishsocial services use the OC-system, how they perceive itsconsequences, and also to understand how their use can beexplained.The study is based on concepts identified in previousresearch on use of performance information. The empiri-cal investigation is based on a survey to 269 managers andsenior officials in the social services in randomly selectedmunicipalities.The analysis shows that the OC system is mainly used forreporting to superior levels, identifying needs for improve-ments, and for evaluating the developments over time. Theuse is to a high extent determined by if the OC system has a strong support from the local leadership structure, andalso if the users perceive it as a reliable and mature system.The users’ perceived consequences of the system concernchanged attitudes, forms of governance, and also concreteoperational processes.

Since then, several ways of controlling and monitoring professionals have been introduced alongside the rise and reformulations of the modern welfare state. Professions have been challenged by, bureaucracy, market solutions and demands for transparency. These are examples of systematically developed counterforces to professional autonomy. A much less studied way of controlling and monitoring professionals is the institution of laypersons, which finds its legitimacy on the grounds that non-experts play the superior role to professionals. Through layperson committees, Swedish child protection advocates have extensive rights when deciding upon some of the most intrusive decisions that the state can make against families, namely, whether children are to be removed from their parents. This article analyses how laypersons challenge the authority of professionals in Swedish child protection. It is based on observations of layperson committees and semi-structured interviews with committee members. The results show that the jurisdictional boundaries are constructed in relation to groupthink, where the laypersons can be the cure for social workers developing unprofessional behaviours and remain as outsiders to the professionals. Another important finding of the study is the ambivalence and paradoxes that arise when it comes to understanding the layperson’s role in relation to at least five topics: formal training; identity as a professional, theoretical knowledge; the normative field laypersons should relate to; and the layperson’s impact. The ambivalence of the ideological boundaries can be seen as a weakness for the layperson.

The literature on professions appears to encompass at least two overarching key metaphors, one that likens professional groups to a hierarchy, and another that describes them as a landscape. This article provides an analysis of two key metaphors in the literature on professions, the purpose being to outline, analyse, and relate them to a number of other central concepts. In carrying out this analysis, questions relating to the similarities and differences between the two metaphors will be addressed. What are their epistemic virtues, and what are the consequences of privileging one metaphor over the other?

    • The article describes and analyses laypersons’ role in networks of expertise and their relationship to professions. It is suggested that networks of expertise are a profitable way of analysing claims-making activities, as it opens up for groups other than professions to be included in the discussion. In the first part of the article, three interest groups—professions, laypersons, and service users—who base their legitimacy on knowledge, are described, analysed, and compared. It is argued that what is special about laypersons’ claims-making activities is that they are built on general common sense instead of the more specialized knowledge that professions and service users build their legitimacy on. In the second part, child protection systems in three countries are compared. The US system is described as professionalized, the English one as semi-professional and the Swedish case as deprofessionalized (or laypersonalized). The third part of the article looks closer at Sweden and asks: who are the laypersons in the Swedish child protection system? How much can the laypersons be regarded as professionals working in a layperson system and how strong is the political affiliation? The conclusions include that laypersons are identified as having two roles: holding executive powers, as in child protection in Sweden and England, and a more consultative and supervisory role. Either way it can be seen as a weakness for a profession to be subordinated to laypersons, yet it can also be a strength for a specific network of expertise.

    • In recent years the welfare sector has encountered numerous and often challenging changes, such as marketisation, New Public Management, and demands for evidence-based practices. These changes can be seen as a shift between the two logics of creating and maintaining authority, and as a movement from occupational towards organizational professionalism. Perhaps the most important difference between these logics is that, in occupational professionalism, authority is built on trust in the professionals' education and ethics, whilst in organizational professionalism it is grounded in regulation and control expressed, for example, in rules, regulations, and routines. The article focuses on the ways in which two groups of social workers in Swedish social welfare offices orient themselves towards organizational and occupational professionalism in constructing themselves and in the construction of each other, as well as the work that is carried out by each group. The aim is thus to elaborate on what these logics mean at the micro-level. In this respect, pertinent questions such as 'How are they to be understood?', 'Regarded from the point of view of micro politics, what do these logics mean?' and 'How are the logics internalized by the social workers?' are posed.

    • The focus of this thesis is on boundary work both as a phenomenon and a theoretical tool, and consists of the manuscripts of four articles and an introduction. One way of perceiving professions is in terms of a landscape metaphor. In this perception of professions, boundaries and boundary work define and separate professional groups by means of dividing social space into sections, over which different groups struggle to establish and maintain control. Different actors make claims on a specific set of boundaries and, in this way, boundary work can be described as either an implicit or an explicit claim-staking activity. The first article, Key metaphors in the sociology of professions: Occupations as hierarchies and landscapes is written in English and offers a comparison between the landscape metaphor and another way of perceiving professions, that of the hierarchy metaphor. If boundaries and boundary work are the most central concepts in the landscape metaphor, closure plays a similar role in the imagery of hierarchies. One conclusion drawn in relation to the two metaphors is that, whilst the landscape provides a finer-tuned theoretical tool, an advantage with the hierarchy metaphor is that it has stronger rhetorical clout. The focus of the second article, which is written in Swedish, is on professional boundary work conducted in a Swedish social welfare department between two groups of qualified social workers, and analyses a specific example of intra-professional boundary work. Among 2 3 the conclusions is that both groups make different claims in the conduct of relational and motivational work carried out from a holistic perspective. The third article, Pragmatic professionalism: Micro level discourse in social work, focuses on boundary work in relation to two ideal types of logic; occupational and organizational professionalism. Perhaps the most important difference between these logics is that, in occupational professionalism, authority is built on trust in the professionals’ education and ethics, whilst in organizational professionalism it is grounded in regulation and control expressed, for example, in rules, regulations and routines. The article deals with how two groups of social workers in Swedish welfare departments position themselves in relation to these logics. Among the conclusions is that the groups relate to the logics in a pragmatic way, making claims on the occupational professionalism but, at times, falling back on organizational professionalism. The fourth and final article, The heroine and the capitalist: The professions´ debate about privatization of Swedish social work, analyzes boundary work conducted in four professional social work journals in the context of debate about privatization. The debate can be described as a struggle with a professional ambivalence towards privatization. This ambivalence is, for instance, evidenced in the metaphorical descriptions of those involved as either heroines or capitalists.

  • Strategic collaboration councils in the mental health services: what are they working with?

    • Introduction: In recent years collaboration has become an important part of the delivery of welfare services. One response to these collaborative efforts has been the introduction of strategic collaboration between different welfare agencies. Strategic collaboration is arguably the most open-ended form of service integration, as both purpose and membership are open to negotiation. This article will examine the work in strategic collaboration councils in the mental health services. Method: The study is based on observations in eight strategic collaboration councils in Sweden. The councils were observed over 12 months, and every meeting that was held during that time was observed and tape-recorded. Results: Four basic activities were identified: the exchange of information, the identification of problems, organizing events and activities, and organizing the councils. Even though these activities were identified, the main focus was to exchange information. The councils' work also varied in terms of how they make decisions and agreements, and whether their focus is more on internal or external issues. Conclusion: From the identified activities, the councils can be classified into four ideal types: the information council, the problem-identification council, the decision-making council, and the self-organizing council.

  • The heroine and the capitalist: The professions´debate about privatization of Swedish social work.

    • Since the 1990s, as a consequence of economic crisis and ideological and political redirections, welfare provision in Sweden has undergone processes of decentralisation, market orientation and privatisation. Publicly financed services provided by subcontracted private enterprises have increased substantially in every welfare sector, including social work. Furthermore, numbers of self-employed social workers also appear to be increasing. The aim of the article is to describe and analyse the debate on privatisation in professional social work journals. More than 700 articles gleaned from 1100 issues of four professional journals spanning the period 1985-2003 have been analysed using a combination of quantitative content and qualitative discourse analysis. The results show that the debate has shifted from ideology to pragmatics in the two major social work unions, although, despite de-polarisation tendencies, ambivalence towards private social work is evident.

    • Professionals within street-level organizations are essential for the delivery of public services to citizens. However, among a number of difficult dilemmas, they have to deal with an extensive workload. The police can be seen as a good example of this; they are expected to solve most crimes, including the so-called mass crimes and the more spectacular cases that make it into media headlines, and often on a continually decreasing budget. A key regulating mechanism for investigation departments in the Swedish police is the so-called balance. The balance can be described as a basket in which they put the cases that there is a desire and potential to work on but not in the immediate term. The purpose of this article is to analyse the balance as a way of rationing the workload within the Swedish police. Working with the balance consists of two processes: limiting and buffering the workload. Limiting is the practice of reducing the work in a situation. Buffering is the process of putting some work on hold to deal with later, of which the article identifies five kinds; functional, problematic, quasi, progressive, and symbolic buffering. The exploration of ‘the balance’ contributes to our understanding of how street level organizations attempt to defend their professional jurisdictions, their well-being, and their ability to complete their duties.

    • A novel method for the manufacturing of microchips for on-chip combinations of electrochemistry (EC) and sheathless electrospray ionisation mass spectrometry (ESI-MS) is described. The technique, which does not require access to clean-room facilities, is based on the incorporation of an array of gold microcoil electrodes into a poly(dimethylsiloxane) (PDMS) microflow channel equipped with an integrated graphite based sheathless ESI emitter. Electrochemical measurements, which were employed to determine the electroactive area of the electrodes and to test the microchips, show that the manufacturing process was reproducible and that the important interelectrode distance in the electrochemical cell could to be adequately controlled. The EC-ESI-MS device was evaluated based on the ESI-MS detection of the oxidation products of dopamine. The results demonstrate that the present on-chip approach enables full potentiostatic control of the electrochemical cell and the attainment of very short transfer times between the electrochemical cell and the electrospray emitter. The transfer times were 0.6 and 1.2 s for flow rates of 1.0 and 0.5 uL min-1, respectively, while the electrochemical conversion efficiency of the electrochemical cell was found to be 30% at a flow rate of 0.5 uL min-1. To decouple the electrochemical cell from the ESI-MS high voltage and to increase the user-friendliness, the on-line electrochemistry-ESI-MS experiments were performed using a wireless Bluetooth battery-powered instrument with the chip floating at the potential induced by the ESI high voltage. The described on-chip EC-ESI-MS device can be used for fundamental electrochemical investigations as well as for applications based on the use of electrochemically controlled sample pretreatment, preconcentration and ionisation steps prior to ESI-MS.

    • Auditing, accountability, and transparency are concepts that greatly impact the working conditions of today’s public sector professionals, including teachers. Documentation requirements have been on the increase for some time, which can be seen in the education sector’s Individual Development Plans (IDPs), for example. These IDPs are pedagogical tools, but can also be seen as governance tools, which is how we will treat them here. In this conceptual paper, we argue that to improve knowledge of education policy changes, there is a need to consider the changed documentation requirements from the perspective of sociology of professions. To this end, we use Evetts’ occupational and organisational logics and Abbott’s three aspects of professional work.

    • Hybrid capillary-poly(dimethysiloxane) (PDMS) microchips with integrated electrospray ionization (ESI) tips were directly fabricated by casting PDMS in a mould. The shapes of the emitter tips were drilled into the mould, which produced highly reproducible three-dimensional tips. Due to the fabrication method of the microfluidic devices, no sealing was necessary and it was possible to produce a perfect channel modified by PolyE-323, an aliphatic polyamine coating agent. A variety of different coating procedures were also evaluated for the outside of the emitter tip. Dusting graphite on a thin unpolymerised PDMS layer followed by polymerisation was proven to be the most suitable procedure. The emitter tips showed excellent electrochemical properties and durabilities. The coating of the emitter was eventually passivated, but not lost, and could be regenerated by electrochemical means. The excellent electrochemical stability was further confirmed in long term electrospray experiments, in which the emitter sprayed continuously for more than 180 h. The PolyE-323 was found suitable for systems that integrate rigid fused silica and soft PDMS technology, since it simply could be applied successfully to both materials. The spray stability was confirmed from the recording of a total ion chromatogram in which the electrospray current exhibited a relative standard deviation of 3.9% for a 30 min run. CE-ESI-MS separations of peptides were carried out within 2 min using the hybrid PDMS chip resulting in similar efficiencies as for fused silica capillaries of the same length and thus with no measurable band broadening effects, originating from the PDMS emitter.
