    • PROBLEM: The current knowledge of evidence-based design for adults is not always implemented when hospital buildings are designed. Scientific data are sparse on the effects of hospital design in pediatric settings on health outcomes in children, parents, and staff. The objective of this review is to determine the evidence-based impact of the built environment in pediatric hospital facilities on health outcomes in children, parents, and staff.ELIGIBILITY CRITERIA: A systematic literature review was carried out on the electronic databases Cochrane Library, Embase, Medline and CINAHL from the period of 2008 to 2019. The review considered studies using either quantitative, qualitative, or mixed methodologies.SAMPLE: Out of 1414 reviewed articles the result is based on eight included articles.RESULTS: Two of these eight articles included health outcomes. The other six articles presented results on measures of perceptions and/or satisfaction for children, parents or staff with the built environment when transitioning to a new or renovated facility. These were generally higher for the new compared to the old facility.CONCLUSIONS: Given the small number of studies addressing the question posed in this review, no firm conclusions can be drawn.IMPLICATIONS: The review illustrates the need for more research in the pediatric setting assessing the evidence-based health outcomes of aspects of physical environmental design in pediatric hospitals or units in children, parents and staff.

  • Aggregation of factors causing cost overruns and time delays in large public construction projects: Trends and implications

    • PurposeThe purpose of this paper is to explore the impact that cost overruns and time delays exert on large public construction projects to clarify how past and current research regard factors causing cost overruns and time delays in large public construction projects.Design/methodology/approachThis paper, which is based on an analysis of a literature selection consisting of 40 journal articles, investigates and ranks the occurrence of and the explanations for cost overruns and time delays in large public construction projects. The study makes use of a kiviat diagram/radar chart in order to visualize multivariate data.FindingsAggregated rankings of important causes of cost overruns and time delays are reported. These show a strong emphasis on the management aspect as a primary cause of cost overruns and delays. Additionally, there seems to be a trend toward deemphasizing the role of financial considerations in explaining cost overruns and delays. It is argued that there needs to be a more rigorous assessment of the impact that each factor has on cost increases and delays based on factual observed data as opposed to retrospective accounts from questionnaire respondents.Research limitations/implicationsOnly public construction projects have been considered. The results will not be directly applicable to privately funded construction projects and/or projects of a smaller size.Originality/valueThe use of trend data, as illustrated in a kiviat diagram, showing how different ranking factors causing cost overruns and time delays has changed in importance over time.

    • Public clients in the construction sector face a number of challenges in designing, procuring and managing major construction projects. The client’s role in bringing about project delivery has more recently been emphasized, particularly with respect to developing capabilities that facilitate the coordinating of projects through its different phases. In line with these developments, this paper sets out to examine the management of capabilities in a client organization through the lens of the dynamic capabilities framework. In particular, what is investigated is how the client organization senses, seizes and transforms opportunities. In pursuit of this objective, an exploratory case study approach is adopted which examines one of Sweden’s largest public client organizations. The study explores the concept of dynamic capabilities and its applicability to the public construction context. Although the usefulness of dynamic capabilities as an interpretive framework is recognized, it is suggested that the concept of dynamic capabilities is inadequate for addressing the specific context in which public client organizations operate. Particularly with respect to the project-based characteristic of these organizations and the difficulty in framing what constitutes “competitive advantage” for public organizations.

  • Approaches to Safeguarding Sustainability Requirements in Public Construction Projects – the Client’s Perspective

    • In recent years, the concept of sustainability has penetrated much of modern political, social and industrial discourse. Its recent popularization, stemming from the Brundtland report of 1987, has led to sustainability becoming a household term in nearly every industry, of which the construction sector is no exception. Considering the importance that sustainability has in the construction industry, and how it is particularly emphasized in construction financed by public funds, questions need to be raised in terms how capable the construction client is in meeting and achieving the sustainability requirements, often set by politics, that exist whilst safeguarding project delivery. The study is based on four interviews targeting public clients in Sweden and it investigates how sustainability requirements are managed in large public construction projects. What is of particular interest is the degree to which public client organizations either develop or procure systems/staff to ensure that the criteria for social, environmental and cultural sustainability are maintained and that the consequences of different approaches are managed. The results support the idea of having a multifaceted approach to sustainable construction, arguing that terms such as social and cultural sustainability may instead be dealt with separately from the more strictly defined sustainability terms of toxicity, waste and energy consumption. There is also a suggestion that once the client organization begins incorporating a sustainability mind-set in all of its affairs, members of that organization may begin working with sustainability on a perfunctory basis without necessarily understanding the underlying reasons for their actions. Finally, the challenge with sustainability is perhaps not so much that there is a lack of capability as much as there is a lack of resources for working with sustainability.

    • Clients in the public sector face a large number of challenges in designing, procur-ing and managing major construction projects in a manner that is conducive to the organization’s overall goals. The role of the client in bringing about successful project completion has more recently been emphasized with a growing number of studies focusing on developing dynamic client capabilities that facilitate the han-dling of a project through all of its different phases. Though the capabilities of the client carries immense importance in all con-struction projects, the importance is further exacerbated by the sheer scale of the projects involved in major construction projects, a development which has prompted governmental agencies to inquire into ways to improve processes in the client organization. To address such inquiries, this paper which is based on a literature review, ex-plores the types of dynamic capabilities that emerge with respect to public con-struction clients and in particular, if and how a specific client capability influences a specific project outcome. The mapping of capabilities constitutes a theoretical foundation for a forthcoming empirical study on the same topic.

    • Purpose - The purpose of this paper is to examine the Company Dynamic Response Map (CDRM) risk management model that uses the dynamic capabilities concept. The study examines risks associated with strategic decision-making in construction projects and evaluates proposed methods that connect the dynamic capabilities of project-based organisations with risk management. Design/Methodology/Approach - This preliminary study examines risks associated with strategic decision-making in construction projects and evaluates a proposed model that connects the dynamic capabilities of project-based organisations with risk management. Specifically, the CDRM model is evaluated, a risk management model developed by Arena et al. (2013) to better respond to risks and opportunities based on the concept of dynamic capabilities. Findings - We argue that although the CDRM presents a promising development in that it uses dynamic capabilities prospectively in a risk management model to produce tangible results, there are, nonetheless, impediments to the CDRM being used by construction clients. The primary impediment relates to the issue of categorisation, the difficulty in assigning a specific identified risk to a particular category of dynamic capabilities. Research Limitations/Implications - A conceptual argument is made and not an empirical one. Practical Implications - The CDRM model was developed to be used in practice and this paper evaluates that model. Originality/Value - Contributes to both the dynamic capabilities literature as well as risk management literature. The paper ends with a discussion on the possible merits of the CDRM, and an evaluation on potential impediments to its use by construction clients.

    • Over a sustained period of time, organizational theorists have argued that public sector organizationsare more inefficient than their private counterparts. Recent studies have explained these inefficienciesas the result of having to do with capabilities rather than resources thereby calling for an improvementof the capabilities of public organizations. A key mechanism for achieving such improvements is thearticulation and codification of knowledge. This study examines an attempt at codifying knowledge inthe form of a set of shared construction guidelines (SCG) employed by 24 large public clientorganizations in the Municipality of Gothenburg, Sweden. Specifically, these guidelines are viewedwith respect to the framework of Zollo and Winter (2002) in terms of how organizations with high taskheterogeneity and low task frequency can more effectively develop dynamic capabilities through thedeliberate learning mechanism of knowledge codification. Preliminary results indicate that taskfrequency and organizational size are stronger indicators than heterogeneity in deciding whether apublic client organization utilizes knowledge codification systems such as the SCG.

    • For decades, the construction industry has been characterized by costs exceeding budgetary limits and completion times reaching further than what was set out ini-tially. This has been particularly noticeable for large public construction projects where cost overruns and time delays have long been regarded a common occur-rence. Due to the magnitude and frequency of these overruns, they have come to pose a significant financial risk to both clients and contractors, in addition to the impact exerted on the sustainability of the project. In dealing with this, researchers, auditors and practitioners have suggested a broad range of solutions, ranging from technical and economical to psychological and political approaches. In doing so, the contractor’s role has been emphasized whereas the role of the client organization has often been overlooked. This paper which is based on a literature review investigates the occurrence of and the expla-nations for cost overruns and time delays in major construction projects from the public client’s perspective. It also explores the implications of cost overruns and time delays; the purpose of which is to offer an extended understanding of the re-lationship between the client’s actions and effects on cost, time and sustainability parameters.

    • Having been overwhelmed with a strikingly large list of things that can go wrong, public clients have consistently found themselves subjected to many risks associated with construction projects. As the scale of projects increases, the severity of risks intensifies. This study sets out to highlight a particular set of risks associated with strategic decision making in public construction projects. Of particular importance stands the client organizations’ capacity to properly manage risks rooted in poor decision making that leads to many types of project failure. To curb the negative effects of poor decision making, varying organisational configurations have been adopted by clients. This paper investigates the six Swedish public client organizations’ risk management procedures, the risk types and the frequency by which they reference risk management in their internal documents. The purpose is to explore how organisational measures influence a client's ability to undertake major construction projects in a manner that is conducive to effective project delivery. The present study is part of an ongoing research project concerning the capabilities of public clients in managing large scale construction projects.

    • Purpose: Previous research within the dynamic capabilities literature has primarily targeted the strategic innovation of technology firms and significantly less interest has been given to project-based organizations that operate in the construction sector. A recent study by Davies and Brady (2016) places the dynamic capabilities concept in a project-based context, drawing upon research on dynamic capabilities and organizational ambidexterity. The purpose of this paper is to apply the aforementioned framework in a case involving public construction clients with the aim of examining their approaches for maintaining or developing project capabilities depending on the volatility of the environment. Design/methodology/approach: The study is based on a case study of a decentralized association of 16 healthcare client organizations in Swedish counties, which was compared to a centralized unit for healthcare planning and construction in Norway. In total, 19 interviews were conducted, alongside two workshops and a feedback questionnaire. Findings: The interviewees emphasized the lack of adequate support to handle the increasingly more complex projects. Results indicate the need for a more segmented approach for understanding how dynamic capabilities are managed in client organizations based not merely on the level of stability in the environment, but also taking into account the resources that are utilized. It is further argued that there is a need for a more granular research approach to studying the development of capabilities in a case-based setting, an approach that more specifically links the development of dynamic capabilities with their relevant antecedent activities. Originality/value: The study sheds light on how the various approaches for maintaining/developing project capabilities available to the public sector construction client depend on the volatility of the environment and the resources they require.

    • Organizations implement strategies in order to guide the organization into the future. Therefore, the interpretation of the strategies within the organization is important. This paper aims to clarify how an organization can transfer strategies to achieve a unison understanding in order to reach organizational goals. The main research question of this paper is; How does a municipality owned housing company implement their strategies within the organization? A theoretical framework of recent literature regarding strategy, communication and sensemaking is used and a case study of a municipality owned housing company is performed. The company is organized in divisions and departments in order to both maintain existing housing and to develop the company internally for future demands. Our method of research has been to interview three employees, working in the company’s departments. Results from the case study indicate that involvement from all employees creates a better understanding of the strategies. A clear structure of meetings, in which information is shared, is used to ensure the communication within the different divisions and departments. Our findings showed that workshops are viewed as a way to create mutual understanding. The study concludes that the involvement of employees and sufficient communication is a key factor of a successful implementation of strategies. Thereto, a unison sensemaking throughout the whole organization is needed.

    • This study explores how common spaces in assisted living schemes for older persons are used by the residents. Observation studies, group interviews with staff and individual interviews with residents, relatives, architects and key stakeholders in the context of Swedish elder-care were conducted. Common spaces are the major location for in-house social interaction on the units. The results show a higher presenceon the dementia units, compared to the somatic units. No significant correlation was found between the residents’ mobility limitations and their degree of presence in the common spaces. The results also suggest a contradiction between the staff’s intentions to provide a social context and the capabilities of the residents. Although common spaces are not used much between meals, the residents stress their importance for social interaction, suggesting that common spaces have important qualitative aspects, rather than quantitative. The results also how that few of the residents used the common spaces together with their relatives. The increasing use of assistive technology creates a shortage of space, suggesting a conflict between the efforts to create a home-like environment and the use of assistive technology.

    • To meet the demographic development, measures were initiated by the Swedish government entailing changed conditions for municipal eldercare, such as assisted living. Participant observation is used to study the daily use of common spaces in 14 assisted living units in Gothenburg. The results show examples where the currentuse diverges from the intended use and suggest diverging objectives and conflicts of use. The observations indicate that the degree of usability is determined by the physical environment itself in some cases, by the users and by changes in use in other cases, and by a combination of both in some cases.

    • Studies have shown how built environments for hospitals can influence psychological and physiological conditions and status for childbearing women. Sensory attributes of birth spaces can enhance comfort, feelings of wellbeing, and, to some extent, clinical outcomes. Recently, some case studies of multisensory rooms for the birth environment have been developed based on, e.g., Snoezelen room examples. The aim of this research is to develop an overview of how sensory aspects for birth environments are designed, used, and tested in current and recent studies. Case studies were selected according to sensory aspect significance, observing space factors, and relationship with the experience and comfort of users (women, partners, midwives). All case studies were analysed to collect information about the design and validation process. The collected data were organised in categories and compared for the selected case studies. Main findings were summarised in tables with the aim of underlining how sensory design processes and projects can positively influence comfort for birthing. Conclusions about how to bring forward the issue of sensory design so that it can be used and applied to support childbearing women is discussed.

    • BACKGROUND: An important prerequisite for optimal healthcare is a secure, safe and comfortable environment. There is little research on how the physical design of birthing rooms affects labour, birth, childbirth experiences and birthing costs. This protocol outlines the design of a randomised controlled superiority trial (RCT) measuring and comparing effects and experiences of two types of birthing rooms, conducted in one labour ward in Sweden. METHODS/DESIGN: Following ethics approval, a study design was developed and tested for feasibility in a pilot study, which led to some important improvements for conducting the study. The main RCT started January 2019 and includes nulliparous women presenting to the labour ward in active, spontaneous labour and who understand either Swedish, Arabic, Somali or English. Those who consent are randomised on a 1:1 ratio to receive care either in a regular room (control group) or in a newly built birthing room designed with a person-centred approach and physical aspects (such as light, silencer, media installation offering programmed nature scenes with sound, bathtub, birth support tools) that are changeable according to a woman's wishes (intervention group). The primary efficacy endpoint is a composite score of four outcomes: no use of oxytocin for augmentation of labour; spontaneous vaginal births (i.e. no vaginal instrumental birth or caesarean section); normal postpartum blood loss (i.e. bleeding < 1000 ml); and a positive overall childbirth experience (7-10 on a scale of 1-10). To detect a difference in the composite score of 8% between the groups we need 1274 study participants (power of 80% with significance level 0.05). Secondary outcomes include: the four variables in the primary outcome; other physical outcomes of labour and birth; women's self-reported experiences (the birthing room, childbirth, fear of childbirth, health-related quality of life); and measurement of costs in relation to the hospital stay for mother and neonate. Additionally, an ethnographic study with participant observations will be conducted in both types of birthing rooms. DISCUSSION: The findings aim to guide the design of birthing rooms that contribute to optimal quality of hospital-based maternity care. TRIAL REGISTRATION: ClinicalTrials.gov NCT03948815.     • In big and complex projects, where large numbers of project participants are involved, it is important to have a well functioning organisational structure. The more complex a project is the more planning and control is needed. The study presented in this paper investigates an installation companys formal and informal communication when constructing a new power plant for district heating, in Göteborg, Sweden. This is done in order to understand the informal communication and its effect on the project organisation. Interviews with managers involved in the project have been done as well a review of literature in the field of communication within an organisation. The results of the study indicate that the actual communication pattern differs from the planned structure. Actors involved, tend to contact each other outside the formal chain of order. The issue addressed in this paper concern how project teams with established informal communication paths might be a possibility to achieve success in projects.

    • Aim: To explore women’s experiences of physical features in a birthing room designed to be adaptable to personal wishes and needs during labor and birth. Background: Childbirth is a central life event influenced by numerous factors, including the healthcare environment; however, there is insufficient knowledge on how the physical design affects women during birth. Methods: This study was part of a randomized controlled trial in the Room4Birth research project, including women randomized to receive care in a new birthing room designed with physical features changeable according to personal wishes. Data consisted of responses to two questions analyzed with descriptive statistics (n = 202) and semi-structured interviews analyzed for content (n = 19). Results: A total of 93.6% (n = 189) assessed the physical features in the birthing room as meaningful to a very high or high extent. The overall impression of the room was positive and exceeded women’s expectations. They felt welcomed and strengthened by the room, which shifted the focus to a more positive emotional state. The room differed from traditional hospital birthing rooms, contained familiar features that maintained integrity, and had space for companions. The variety of physical features was appreciated. Of nine listed physical features, the bathtub was ranked most important, followed by the projection of nature scenery, and dimmable lighting, but the room as a whole appeared most important. Conclusions: When planning and designing hospital-based birthing rooms, it is crucial to offer possibilities to adapt the room and physical features according to personal wishes.

Several Evidence Based Design (EBD) studies highlight the deep interrelation between built and natural systems with human or organizations-related outcomes, but the effect on healthcare staff such as Medical Doctors (MD) is still underexplored. The paper investigates the assessment of self-reported satisfaction and wellbeing of MDs in healthcare facilities. A multidimensional assessment model composed of 53 Likert scale questions has been developed from literature review and existing tools, and submitted to a statistically significant sample of workers in 2 different office settings of an Italian hospital. Since MDs spend a considerable amount of their working time in offices, the qualities of such space are very important. The study highlights and confirms that localization, indoor environment, natural and artificial light are relevant drivers for staff satisfaction and wellbeing. Further investigations on a wider and diverse sample are encouraged.

    • Healthcare facilities are facing huge challenges due to the outbreak of COVID-19. Around the world, national healthcare contingency plans have struggled to cope with the population health impact of COVID-19, with healthcare facilities and critical care systems buckling under the ex-traordinary pressures. COVID-19 has starkly highlighted the lack of reliable operational tools for assessing the level sof flexibility of a hospital building to support strategic and agile decision making. The aim of this study was to modify, improve and test an existing assessment tool for evaluating hospital facilities flexibility and resilience. We followed a five-step process for collecting data by (i) doing a literature review about flexibility principles and strategies, (ii) reviewing healthcare design guidelines, (iii) examining international healthcare facilities case studies, (iv) conducting a critical review and optimization of the existing tool, and (v) assessing the usability of the evaluation tool. The new version of the OFAT framework (Optimized Flexibility Assessment Tool) is composed of nine evaluation parameters and subdivided into measurable variables with scores ranging from 0 to 10. The pilot testing of case studies enabled the assessment and verification the OFAT validity and reliability in support of decision makers in addressing flexibility of hospital design and/or operations. Healthcare buildings need to be designed and built based on principles of flexibility to accommodate current healthcare operations, adapting to time-sensitive physical transformations and responding to contemporary and future public health emergencies.

    • Aim: The aim was to investigate the content and quality of the governing documents created in the planning and design phase of new healthcare environments and in the related healthcare strategic and operational plans.Background: Quality deficits in buildings can often be traced back to the initial stages in the planning and design phase. Although, large investments have been made to improve the process of planning new healthcare environments and linking the requirements to health service strategies, healthcare organizations rarely relate their strategy goals to the built environment.Methods: A retrospective review of documents created in the planning and design stages of new healthcare environments and the operational plans of the target organizations was conducted.Results: The organizational operational plans did not contain any statements or information about the built environment or how a building could or should support the organization's goals. Important information was frequently absent from the documents governing the planning and design of buildings. The documents lacked information about what and how to follow-up and what to measure once a construction project had been completed. There were no references to evidence.Conclusions: Poor documentation might undermine the quality of the planning and design phase and ultimately the opportunity to create environments that support health outcomes. Therefore, more emphasis must be placed on the importance of documentation but above all to strengthen and clarify the relationship between the healthcare organization strategy to achieve an effective and efficient care process and the intention made in the planning and design process.

Thus, decisions concerning healthcare architecture are critical because it affects people and work processes for many years and requires a long-term financial commitment from society. In this paper, we describe and suggest several strategies (critical factors) to promote shared-decision making when planning and designing new healthcare environments. Discussion: This paper discusses challenges and hindrances observed in the literature and from the authors extensive experiences in the field of planning and designing healthcare environments. An overview is presented of the challenges and new approaches for a process that involves the mutual exchange of knowledge among various stakeholders. Additionally, design approaches that balance the influence of specific and local requirements with general knowledge and evidence that should be encouraged are discussed. Summary: We suggest a shared-decision making and collaborative planning and design process between representatives from healthcare, construction sector and architecture based on evidence and end-users' perspectives. If carefully and systematically applied, this approach will support and develop a framework for creating high quality healthcare environments.

    • The decentralisation of Swedish healthcare closer to citizens has been slow. Drawing from empirical material of the reform prior and amidst the COVID-19 pandemic, this paper argues that the pandemic has disrupted the healthcare ecosystem. Consequently, citizen-centred collaborations have accelerated integration of resources (such as knowledge and skills) across organisational, hierarchical and professional borders. However, collaborations have been delimited to traditional healthcare providers, neglecting the resources of citizens and other actors to be used to improve service delivery. The pandemic has revealed strengths and weaknesses with the prevailing healthcare ecosystem that post-COVID-19 public management must address, both theoretically and practically. Theoretically, the paper contributes to the development of a public service logic, addressing both strengths and difficulties with the logic in turbulent times. Practically, the empirical descriptions contribute to improved understanding of public service delivery reform and how it is impacted during the pandemic. © 2021 The Authors.     • Multidisciplinary collaboration and transformations in learning processes can be supported by activity-based campus retrofitting. The aim of this paper is to analyse the ongoing campus retrofitting processes at the three university campuses and to identify the elements of activity-based retrofitting. We answer the questions "What kind of examples of retrofitting are there at Nordic Campuses?" and "What kind of elements are typical for activity-based retrofitting concepts?" The 3-level framework of campus retrofitting processes was employed when conducting the three case studies. The cases were about the new ways of researching, collaborating and learning with the concepts of Living lab, Creative community for innovation and entrepreneurship and Network of learning hubs. The cases provided the first insights on retrofitting based on users' changing needs and the requirements of more sustainable solutions at campuses. Notably, a technical and spatial solution covers only part of retrofitting processes. The activity-based retrofitting consists on multidisciplinary collaboration and learning processes where diverse users have diverse roles during retrofitting processes.     • Real estate- and property owners' rationales behind the adoption of Life Cycle Costing (LCC) respectively how LCC is actually used in renovation projects, is investigated through empirical data from a questionnaire survey sent to managers in Swedish real estate organisations. The study shows a positive attitude towards LCC. It is perceived to as a flexible and multi-functional tool with a familiar monetary format. Nevertheless, the study also reveals simplistic and undeveloped views of how to use LCC. While much research has focused on developing sophisticated LCC tools, the findings indicate that practitioners' interest in these refinements seems limited. The importance of understanding that LCC is used in a context of multiple and partly competing institutional logics of renovation is emphasised. The paper contributes to a more informed research in development of LCC tools as well as better informed LCC use among real estate and property owners.

    • Objective: To study the effect of the birthing room design on nulliparous women’s childbirth experience up to 1 year after birth. Background: Although it is known that the birth environment can support or hinder birth processes, the impact of the birthing room design on maternal childbirth experience over time is insufficiently studied. Methods: The Room4Birth randomized controlled trial was conducted at a labor ward in Sweden. Nulliparous women in active stage of spontaneous labor were randomized (n = 406) to either a regular birthing room (n = 202) or a new birthing room designed with more person-centered considerations (n = 204). Childbirth experiences were measured 2 hr, 3 months, and 12 months after birth by using a Visual Analogue Scale of Overall Childbirth Experience (VAS-OCE), the Fear of Birth Scale (FOBS), and the Childbirth Experience Questionnaire (CEQ2). Results: Women randomized to the new room had a more positive childbirth experience reported on the VAS-OCE 3 months (p =.002) and 12 months (p =.021) after birth compared to women randomized to a regular room. Women in the new room also scored higher in the total CEQ2 score (p =.039) and within the CEQ2 subdomain own capacity after 3 months (p =.028). The remaining CEQ2 domains and the FOBS scores did not differ between the groups. Conclusions: These findings show that a birthing room offering more possibilities to change features and functions in the room according to personal needs and requirements, positively affects the childbirth experience of nulliparous women 3 and 12 months after they have given birth.

    • Objective: To evaluate if a birthing room designed with person-centred considerations improves labour and birth outcomes for nulliparous women when compared to regular birthing rooms. Methods: A randomised controlled trial was conducted at a Swedish labour ward between January 2019 and October 2020. Nulliparous women in spontaneous labour were randomised either to a birthing room designed with person-centred considerations (New room) or a Regular room. The primary outcome was a composite of four variables: vaginal non-instrumental birth; no oxytocin augmentation; postpartum blood loss < 1000 ml; and a positive childbirth experience. To detect a difference of 8% between the groups, 1274 study participants were needed, but the trial was terminated early due to consequences of the Covid-19 pandemic. Results: A total of 406 women were randomised; 204 to the New room and 202 to the Regular room. There was no significant difference in the primary outcome between the groups (42.2% versus 35.1%; odds ratio: 1.35, 95% Confidence Interval 0.90–2.01; p = 0.18). Participants in the New room used epidural analgesia to a lower extent (54.4% versus 65.3%, relative risk: 0.83, 95% Confidence Interval 0.71–0.98; p = 0.03) and reported to a higher degree that the room contributed to a sense of safety, control, and integrity (p=<0.001). Conclusions: The hypothesis that the New room would improve the primary outcome could not be verified. Considering the early discontinuation of the study, results should be interpreted with caution. Nevertheless, analyses of our secondary outcomes emphasise the experiential value of the built birth environment in improving care for labouring women.

    • Background: The provision of supportive environments is essential in clinical and environmental psychology. Mental health disorders are a major issue, and the experience of being at a mental health facility is affected by numerous factors related to the building's design. Aim: The aim of this study is to explore the expectations of a mental health facility planning group regarding the potential impact of a supportive design on patients' mental health and staff's therapeutic practices when planning and designing a new mental health facility. Methods: The new mental health facility is a case study and data were collected through qualitative in-depth interviews with nine participants and analyzed using a thematic analysis. The participants came from a mental health facility planning group in a new mental health facility in Norway. Results: The overall expectation of the new building was related to a future orientation to support patients' mental health and therapeutic practices. Three main themes were identified: toward a future orientation, supportive building design, and work environment. Conclusions: Supportive environments are expected to influence patients' mental health and staff's therapeutic practices, including providing options for novel treatment needs in contrast to older and more outdated buildings that are perceived as hindering appropriate treatment conditions.

The pilot study is part of a larger study investigating the efficiency in processes of providing facilities in the healthcare sector. The outset for the larger study is that a common opinion among representatives of core business and corporate real estate is that the finished projects do not meet expectations regarding usability and function for core business. A small number of Swedish public real estate companies have been transformed into service organizations. .A discussion whether to use a real estate organizational form or a service organization is rooted in the increased demands of achieving efficient and usable buildings satisfying the users needs. Two main questions were set up in this pilot study: i) In which ways does the organization form or structures impact the process of providing premises? and ii) How does the way of organization impact the individuals possibilities to act in the process of providing premises? The purpose of the study presented is thus to analyse the role of the organizational form and the individuals possibilities to act in the process of providing premises in terms of how they achieve fulfilment of expectations regarding usability for the healthcare core business. Tentative results in this paper based on the pilot study are that results from a project by means of meeting the clients´ expectations, quite contrary to an early assumption, do not depend on the organizational form or structure in the providing organization. The study is based on interviews and the result shows that all of the interviewed actors mean that the form of the organization has little direct impact on the result of the process of providing premises; it is how the actor views their work tasks that make the difference for the result. On the other hand, these views depend on the fundamental values in the organization.

This paper reports on a study that has analysed the process of providing premises for core businesses from a number of perspectives. The development of design theory throughout the past four decades has been one, the development of methods and theory in participatory design is another. The issues and definitions of usability and functionality that have been elaborated through the CIB workgroup WI 11 project Usability of Workspace are also used in the study. The aim of the study is to find better ways to provide premises from the point of view of the core business. This paper elaborates how the praxis of providing premises among Swedish corporate real estate companies in healthcare differs from the state-of-the-art in the above areas.

    • The field of FM and real estate is continuously challenged to utilize the experiences from the users in the planning of facilities and establishing of services related to facilities. The question raised in this paper is how FM professionals can achieve design solutions, and usability and functionality of premises that meet the users’ requirement and how close to the organizations activities the FM organization can get. To achieve design solutions that are state of the art can be achieved already today, but to deliver for the business activity usable and efficient facilities requires a process where the users are enabled to “own” their premises and where the FM-provider is professional in managing the meeting between core activities and FM processes. This paper, based on a case study of a hospital design project, discuss the process of how the users are related, engaged and integrated in the organizational development process in order to understand also the use and role of new facilities. The result of the study indicate that there needs to be a process for managing organizational culture and a communication strategy that address all affected by the organizational and facility changes. This is needed in order not to create a gap between those who are informed and those who are not, and by doing so decrease the risk of cumbersome and inefficient starting processes when new ways of working are to be implemented.

    • Purpose – The paper seeks to suggest methods that will enable innovative and effectivecommunication and collaboration between clients and construction project management professionals.Design/methodology/approach – The methodology involves workshops with construction clientsdocumented in working documents circulated to participants.Findings – Workshops revealed trends that urge a re-evaluation of the briefing process. The need forbetter briefing with the focus on end-users is increasing. The findings also pointed to difficulties forconstruction projects to deliver what the user-clients need. There was considered to be a lack ofsystems and methods to keep track of user client demands sufficiently and in a satisfactory way. Goalsneed to be iterated and validated on a regular and coherent basis throughout projects. An increasedinterest for process-oriented and strategic briefing was indicated.Research limitations/implications – Further studies are required to develop a client/user drivenconstruction process that is more than just new statements. Increased need for professional clients and the aim of construction project management professionals to engage in a wider range of tasks and activities during the briefing, design and production phase of construction projects is the main outset. The study presented addresses the relationship between the roles and goals of the client and the construction project management organisations/professionals. The overriding aim is to explore the managerial support, facilitation and implementation of client requirements, to study experience from collaboration between stakeholders and to create knowledge of the relationship between an effective achievement of clients goals and the construction project management process. Existing project management and processes concerned with the planning of facilities are discussed based on theories in the field as well as on an interview survey among client organisations and project management organisations. Throughout, the focus is on project management and how it succeeds to convert the construction clients corporate strategy and business into property investment or corporate real estate decisions. Such knowledge should make it possible to create innovative methods for facilitating construction briefing and project management. It will also support the provision of facilities and provide a deeper understanding of the consequences of strategies or methods for the improvement of the performance of the client organisations business objectives in construction projects.     • Samhällets omställning till ett hållbart samhälle tvingar till nytänkande. Byggsektorn hör till den sektor som förbrukar mest energi, åstadkommer mest avfall och är störst förbrukare av råvaror och övriga resurser. I en tid när man alltmer ifrågasätter resursförbrukning och övrig miljöpåverkan hamnar därför byggsektorn i fokus. Processer och metoder som strävar efter minskad resursförbrukning måste utvecklas.Olika aktörer i byggsektorn har under senare tid pekat på att alltför stor uppmärksamhet ägnas åt byggpriset i anbud och mycket lite åt de kostnader som uppstår under husets livslängd, dvs. det som man i dagligt tal brukar kalla för livscykelkostnader. Dit hör exempelvis energiåtgång, material och system som vid anbudslämning kan uppvisa låga priser men som i det långa loppet kan bli dyrbara för husets ägare och förvaltare. Ett fokus på investeringskostnader har gjort att FoU inriktad mot att utveckla metoder och processer som kan vara vägledande för sektorns aktörer har saknats.Studiens resultat visar att LCC är ett etablerat begrepp som det stora flertalet aktörer inom byggsektorn känner till. LCC är använt av såväl beställare som konsulter och entreprenörer. Då LCC-kalkylmodellerna ursprungligen är framtagna för att utvärdera maskiner och system som exempelvis ventilationssystem används i dag LCC för att göra olika systemval i byggandet. Däremot används inte LCC-analyser systematiskt i upphandlig av bostäder eller andra byggnader.

Inom detta område har diskussionen fokuserat på vilketsätt och hur byggherrens mål skall mätas och följas upp i projekt, här förstådda sombyggprojekt.Problemet som konstateras i studier av litteraturen är frågan om vilka faktorer som skallmätas, när detta skall göras och vilken omfattning (tidsperiod) som skall täckas. Utifrån ettbyggherreperspektiv är användningen av byggnader och lokaler kontinuerlig vilket innebär atten avgränsning måste göras om mätning skall kunna göras. En avgränsning som ter signaturlig, och som framkommit vid seminarier med byggherrar, är projektnivån. I dessa fall harprojektnivån satts till själva byggprojektet, eller det projekt som innebär i att byggherrenbygger nytt eller på något sätt påverkar eller förändrar en byggnad/lokaler. Detta innebär dockett dilemma då den verksamhet som byggherren företräder eller bedriver utvecklaskontinuerligt. För att kunna hantera detta måste därför projektmål fastställas och dessa sedanhållas aktuella eller justeras på ett genomtänkt sätt under projektets genomförande. Detta föratt säkerställa att organisationen som genomför projektet har målbilden klar samt attmotstridiga uppfattningar kan redas ut. Arbete hittills inom delprojekt 11 har kunnatkonstatera att en process som påminner om projektmålen, det vill säga repeterar ochkonfirmerar alternativt justerar, är ett sätt att nå framgång i projekt. Ett annat sätt att beskrivadetta är att få ”verksamhetsutnyttjaren” att ”kvittera på vägen”.En annan punkt som diskuterats är byggherrens ansvar att formulera projektmål inte bara förverksamheten utan också för byggprojektet. Byggherren skall här hantera intressenternasönskemål och kanalisera in dem i processen. Detta skall ske genom att byggherren formulerarmål som är användbara i byggprocessen, detta har uttryckts som att byggherren skall”översätta verksamhetsmål till byggproduktionsmål”. Detta pekar på betydelsen avbyggherrens kompetens och beslutsförmåga. Det är viktigt att understryka att här avses inte enmätning verksamhetsmål som sådana utan hur dessa kommer till uttryck/motsvaras av mål förbyggprocessen. Verksamhetsmål skall också mätas, men detta skall göras i verksamheten.

    • This paper is based on a study of a major sport event organization. Event organisations are interesting to study as they tend to change considerably during their life-time. Their work is also challenging as these organizations often consist of people with different backgrounds that have little or no experience of working together and the execution phase is in most cases very short. The focus of this paper is on organisational change within the Local Organizing Committee (LOC) formed to carry out the European Athletics Championships 2006 (EAC 2006) in Göteborg, Sweden. It focuses on the roles and functions of the people involved as well as the transformation of the organisation from planning to execution mode, in terms of organisational growth and organisational design. By contributing to knowledge within these areas, the development of appropriate organizations for projects with a short execution phase can be facilitated and researchers and project participants can be made aware of a number of potential organizational challenges. The study presented is part of a longitudinal study on the development of the LOC over a three year period.
