  • Inhaled ciclesonide in adults hospitalised with COVID-19 : a randomised controlled open-label trial (HALT COVID-19)

    • OBJECTIVE: To assess the efficacy of inhaled ciclesonide in reducing the duration of oxygen therapy (an indicator of time to clinical improvement) among adults hospitalised with COVID-19.DESIGN: Multicentre, randomised, controlled, open-label trial.SETTING: 9 hospitals (3 academic hospitals and 6 non-academic hospitals) in Sweden between 1 June 2020 and 17 May 2021.PARTICIPANTS: Adults hospitalised with COVID-19 and receiving oxygen therapy.INTERVENTION: Inhaled ciclesonide 320 µg two times a day for 14 days versus standard care.MAIN OUTCOME MEASURES: Primary outcome was duration of oxygen therapy, an indicator of time to clinical improvement. Key secondary outcome was a composite of invasive mechanical ventilation/death.RESULTS: Data from 98 participants were analysed (48 receiving ciclesonide and 50 receiving standard care; median (IQR) age, 59.5 (49-67) years; 67 (68%) men). Median (IQR) duration of oxygen therapy was 5.5 (3-9) days in the ciclesonide group and 4 (2-7) days in the standard care group (HR for termination of oxygen therapy 0.73 (95% CI 0.47 to 1.11), with the upper 95% CI being compatible with a 10% relative reduction in oxygen therapy duration, corresponding to a <1 day absolute reduction in a post-hoc calculation). Three participants in each group died/received invasive mechanical ventilation (HR 0.90 (95% CI 0.15 to 5.32)). The trial was discontinued early due to slow enrolment.CONCLUSIONS: In patients hospitalised with COVID-19 receiving oxygen therapy, this trial ruled out, with 0.95 confidence, a treatment effect of ciclesonide corresponding to more than a 1 day reduction in duration of oxygen therapy. Ciclesonide is unlikely to improve this outcome meaningfully.TRIAL REGISTRATION NUMBER: NCT04381364.

    • 1. Increased anthropogenic influence on the environment has accentuated the need to assess how climate and other environmental factors drive vital rates and population dynamics of different types of organisms. However, to allow distinction between the effects of multiple correlated variables, and to capture the effects of rare and extreme climatic conditions, studies extending over decades are often necessary.2. In this study, we used an individual-based dataset collected in three populations of Pulsatilla vulgaris subsp. gotlandica during 34 years, to explore the effects of variation in precipitation and temperature on vital rates and population dynamics.3. Most of the observed conspicuous variation in flowering among years was associated with differences in precipitation and temperature in the previous summer and autumn with a higher incidence of flowering following summers with high precipitation and low temperatures. In contrast, climatic variables had no significant effects on individual growth or survival.4. Although the weather-driven variation in flowering had only moderate absolute effects on the population growth rate, simulated persistent changes in average precipitation and temperature resulted in considerable reductions in population sizes compared with current conditions. Analyses carried out with subsets of data consisting of 5 and 10 years yielded results that strongly deviated from those based on the full dataset.5. Synthesis. The results of this study illustrate the importance of long-term demographic monitoring to identify key climatic variables affecting vital rates and driving population dynamics in long-lived organisms.

    • Theoretical and empirical data suggest that genes located on sex chromosomes may play an important role both for sexually selected traits and for traits involved in the build-up of hybrid incompatibilities. We investigated patterns of genetic variation in 73 genes located on the Z chromosomes of two species of the flycatcher genus Ficedula, the pied flycatcher and the collared flycatcher. Sequence data were evaluated for signs of selection potentially related to genomic differentiation in these young sister species, which hybridize despite reduced fitness of hybrids. Seven loci were significantly more divergent between the two species than expected under neutrality and they also displayed reduced nucleotide diversity, consistent with having been influenced by directional selection. Two of the detected candidate regions contain genes that are associated with plumage coloration in birds. Plumage characteristics play an important role in species recognition in these flycatchers suggesting that the detected genes may have been involved in the evolution of sexual isolation between the species.

  • Media and Basic Desires : An Approach to Measuring the Mediatization of Daily Life

    • The extended indispensability of media can be seen as a key indicator of mediatization, whereby our lives become adapted to media. But even though we can assume that the pervasive character of digital media essentially changes everyday life and the way people experience it, we cannot take consequences of technological transformation for granted. Following from this we argue there is a need for quantitative analyses that can verify mediatization processes in time and space. Here we present a tool that measures the extent to which media reach into the deeper layers of daily human life, used in an empirical study conducted in Sweden in 2017. The results show that perceived media indispensability is played out along three dimensions of daily human life; (1) (re)production; (2) recognition, and (3) civic life, and appears as a socially structured and structuring process, although not in a very strong way. We argue this tool, in diachronic analyses, works as a measurement of mediatization.

There has recently been a formulation of three tasks for mediatization research; historicity, specificity and measurability, needed to empirically verify mediatization processes across time and space. In this article, we present a tool designed to handle these tasks, by measuring the extent to which people experience that media reach into the deeper layers of daily human life. The tool was tested in an empirical study conducted in Sweden in 2017. The results show that perceived media reliance is played out in relation to three types of basic desires: (1) (re)productive desires, (2) recognition desires, and (3) civic desires, and is socially structured and structuring. We argue this tool, in diachronic analyses, can measure one important aspect of mediatization.

  • The Public Interest in the Data Society : Deconstructing the Policy Network Imaginary of the GDPR

    • When Facebook censored the Pulitzer Prize-winning photograph The Napalm Girl, it provoked a global outcry. It also showed digital media’s ability to redefine freedom of expression and information. Redefining fundamental rights and freedoms involves drawing limits upon other fundamental freedoms and rights like the right to privacy. It is therefore crucial to study the changing role of the public interest, which ensures conditions for all to exercise fundamental freedoms and rights, including the right to privacy. This thesis aims at analysing definitions and uses of the public interest during the policymaking process of the General Data Protection Regulation (GDPR). The policymaking process is approached through the policy network theory (Rhodes, 2007). Policy network theory permits researchers to study norms and beliefs that drive stakeholders’ strategies to construct knowledge during the policymaking process. The construction of knowledge is influenced by a shared imaginary (Ricœur, 1984), which frames shared knowledge at the societal level. Metaphors permit tracing imaginaries. The public interest, as a metaphor defined during the policymaking process of the GDPR, enables the reconstruction of the policy network imaginary of the GDPR. I use two methods, the assessment of the degree of preference attainment (DPA) and the discourse-historical approach (DHA), which belongs to the field of critical discourse studies. Assessing DPA allows a comparison of key definitions in the texts produced during the policymaking process by different groups of actors—like Google and the European Commission—with corresponding definitions in the GDPR. The DHA permits the contextualization of definitional changes, the identification of power games within the policy network, and the reconstruction of the policy network imaginary.Results show that the policy network imaginary corresponds to a techno-economic ideology.  This ideology underlies a preconceived context, the data society, and frames societal phenomena to regulate through beliefs and norms. This ideology is taking over the public interest ideology. Technological and economic determinism drove the design of the GDPR, which limits capacities to regulate and aligns with the interest of the dominant tech and economic actors. The member states favoured specific topics related to the public interest, like security, to be implemented at the EU level, at the expense of other topics, such as freedom of expression and information, to be implemented at the national level.

    • Herein we assess the phylogenetic relationships and taxonomy of geckos of the genus Phyllodactylus inhabiting the peninsula of Baja California, Mexico using five mitochondrial and two nuclear genes. Phylogenetic analysis using maximum parsimony (MP) and Bayesian inference (BI) recovered three distinct peninsular clades with high statistical support. Sequence divergence estimates between peninsular taxa approached 13%. Two of the species, P. unctus and P. xanti are Cape Region endemics, whereas P. nocticolus is widespread throughout much of the peninsula and extreme southern California. Monophyly of the peninsular taxa was strongly supported. In the MP analysis, P. unctus rooted at the base of the peninsular clade, resolving P. xanti and P. nocticolus as sister taxa. Conversely, BI placed P. nocticolus and P. unctus as sister taxa. These data provide further evidence for a trans-peninsular seaway near the Isthmus of La Paz, severing the Cape Region from the rest of the peninsula. The analysis also supports the validity of P. nocticolus as a distinct species and suggests a single invasion to the peninsula from mainland Mexico, presumably during tectonic activity during the Miocene.

    • Background The aim of the present study was to examine the outcomes and prognostic factors after surgery with curative intent for distal cholangiocarcinoma during a modern timespan, in a Swedish tertiary referral center. Methods All patients who underwent pancreaticoduodenectomy for distal cholangiocarcinoma between April 2008 and December 2015 were identified. Survival was estimated using the Kaplan-Meier analysis. Demographic, clinical, laboratory and histopathological data were evaluated for prognostic factors relating to mortality, using univariable and multivariable statistical analysis. Results Fifty-four patients were included. The mean age was 68±8 years and 21 (39%) of the patients were female. Jaundice was present at diagnosis in 73% of the patients. There was no 90-day mortality. Complications graded as Clavien-Dindo ≥3 occurred in 10 (19%) of the patients. Twenty-eight (52%) received adjuvant therapy. Overall survival rates at 1, 3, and 5 years were 80%, 21%, and 9.2%, respectively. Median survival was 22.2 months. The presence of lymph node metastases was found to be the only independent predictor of survival (hazard ratio 2.88, 95% confidence interval 1.22-6.84; P=0.016). The total number of lymph node metastases, lymph node ratio or total number of resected nodes did not improve the prediction. Conclusions We found that the recurrence rate was higher and the survival poorer after surgery for distal cholangiocarcinoma than has previously been reported. Lymph node status at the time of resection was the most important prognostic factor for survival in the current material.

    • X-ray and neutron powder diffraction has been used to determine the crystal and magnetic structure of Mn3Ni20P6. The crystal structure can be described as cubic with space group Fm3¯m (225) without any nuclear phase transformation within studied temperature interval from room temperature down to 4 K. The magnetic structure of Mn3Ni20P6 is complex with two independent magnetic positions for the Mn atoms and the compound passes three successive magnetic phase transitions during cooling. At 30 K the spins of the Mn atoms on the Wyckoff 4a site (Mn1) order to form a primitive cubic antiferromagnetic structure with propagation vector k=(0 0 1). Between 29 and 26 K the Mn atoms on the Wyckoff 8c site (Mn2) order independently on already ordered Mn1 magnetic structure forming a commensurate antiferromagnetic structure with propagation vector k=(0 0 12) and below 26 K, both Mn positions order to form an incommensurate helical structure with propagation vector k=(0 0 ~0.45). Magnetization vs. temperature curve of Mn3Ni20P6 shows a steep increase indicating some magnetic ordering below 230 K and a sharp field dependent anomaly in a narrow temperature range around 30 K.

    • Deep-ocean seaweed dumping is not an ecological, economical, or ethical answer to climate-change mitigation via carbon “sequestration.” Without sound science and sufficient knowledge on impacts to these fragile ecosystems, it distracts from more rational and effective blue-carbon interventions. We call for a moratorium on sinking seaweeds to deep-ocean ecosystems until its efficacy is established, and there is robust, evidence-based assessment of its environmental, economic, and societal sustainability.

    • Sequence variation in mitochondrial DNA (mtDNA) was traditionally considered to be selectively neutral. However, an accumulating body of evidence indicates that this assumption is invalid. Furthermore, recent advances indicate that mtDNA polymorphism can be maintained within populations via selection on the joint mitochondrial-nuclear genotype. Here, we review the latest findings that show mitochondrial and cytoplasmic genetic variation for life-history traits and fitness. We highlight the key importance of the mitochondrial-nuclear interaction as a unit of selection and discuss the consequences of mitochondrially encoded fitness effects on several key evolutionary processes. Our goal is to draw attention to the profound, yet neglected, influence of the mitochondrial genome on the fields of ecology and evolution.

  • Age-Independent Adult Mortality in a Long-Lived Herb

    • Relative to mammals and birds, little is known about the mortality trajectories of perennial plants, as there are few long-term demographic studies following multiple yearly cohorts from birth to death. This is particularly important because if reproductively mature individuals show actuarial senescence, current estimations of life spans assuming constant survival would be incorrect. There is also a lack of studies documenting how life history trade-offs and disturbance influence the mortality trajectories of plants. We conducted Bayesian survival trajectory analyses (BaSTA) of a 33-year individual-based dataset of Pulsatilla vulgaris ssp. gotlandica. Mortality trajectories corresponded to Type III survivorship patterns, with rapidly decreasing annual mortality rates for young plants, but with constant mortality for reproductively mature individuals. We found trade-off effects resulting in a cost of growth for non-reproductive plants but no apparent cost of reproduction. Contrarily to our expectation, young plants that had previously shrunk in size had a lower mortality. However, accounting for trade-offs and disturbance only had minor effects on the mortality trajectories. We conclude that BaSTA is a useful tool for assessing mortality patterns in plants if only partial age information is available. Furthermore, if constant mortality is a general pattern in polycarpic plants, long-term studies may not be necessary to assess their age-dependent demography.

    • Disconnecting from digital media is often mentioned in the public debate as a way of improving quality of life, productivity, sustainability, and so forth. However, not everyone can afford to disconnect, and media morality varies across social space. Based on data from a national Swedish survey (2019), this chapter applies correspondence analysis and a Bourdieusian theoretical framework to chart the extent to which different social groups prioritize disconnecting in different places, and the forms of digital unease associated with smartphone use. Such preferences are mapped onto a social space constructed around the distribution of economic and cultural capital in Swedish society, also illuminating how disconnection practices correspond to other lifestyle practices. The analysis reveals that the handling of digital (dis)connection (in different places) plays into overarching patterns of taste and cultural distinction. As such, disconnection manifests as an emerging moral-symbolic battleground in affluent societies.

    • This study sets out provide an understanding of internationally mobile life-conducts from a perspective that takes into account social costs that come with being away from localized, everyday life. We show that mobile elites are oftentimes reluctant travellers. A way of coping with the existential dilemmas of being away is to stay connected with family and friends with technologies of communication, which are deployed by the mobile elite under the regime of what Tomlinson calls “technologies of the hearth”. Furthermore, few informants ascribe any value to travelling in itself. Cosmopolitanism can here be understood as a form capital rather than a way of immersing the self into the culture of the other. We arrive at the concept of elastic mobility, which highlights central push-and-pull processes within mobile life-conducts. The concept forwards a perspective on the social consequences of globalization that goes beyond contemporary “flow speak”.

    • This book introduces and develops the concept of geomedia studies as the name of a particular subfield of communication geography. Despite the accelerating societal relevance of 'geomedia' technologies for the production of various spaces, mobilities, and power-relations, and the unquestionable emergence of a vibrant research field that deals with questions pertaining to such topics, the term geomedia studies remains surprisingly unestablished. By addressing imperative questions about the implications of geomedia technologies for organizations, social groups and individuals (e.g. businesses profiting from geo-surveillance, refugees or migrants moving across national borders, or artists claiming their rights to public space) the book also aims to contribute to ongoing academic and societal debates in our increasingly mediatized world.

    • The incidence of pancreatic cancer has fallen during the last ten years in Sweden. Early signs and symptoms of the disease are still undiscovered and when diagnosis is made the disease is incurable in most patients. Transabdominal ultrasonography is the first-line imaging test followed by spiral computed tomography (CT) and magnetic resonance imaging (MRI) if required for definite diagnosis. Spiral CT is also the imaging test of choice for assessment of resectability of the tumor. Surgical removal of the tumor is the only chance of cure. Markedly improved hospital mortality after pancreaticoduodenectomy is reported and an association between hospital volume and outcome of the operation has been established. Longterm survival after attempted curative resection continues to be dismal, however. Adjuvant treatment should not be given outside clinical studies. Palliative treatment has improved thanks to progress in the field of endoscopy, interventional radiology and in management of pain and nutrition. Palliative chemotherapy should only be given selectively outside clinical studies. Radiotherapy has no proven effects on survival. Special pancreatic cancer treatment teams with catchment areas of 2-4 million inhabitants are recommended by international authorities.

    • Transabdominellt ultraljud är förstahandsundersökning vid misstänkt pankreascancer, följt av spiral-DT eller MR för mer definitiv diagnos. Tumörmarkörer har ingen plats i rutindiagnostiken. Spiral-DT är basen i resektabilitetsbedömningen. Resektion av tumören är en förutsättning för bot. Ett samband har påvisats mellan antalet resektioner som görs vid ett sjukhus årligen och postoperativ mortalitet. Långtidsöverlevnaden efter resektion är oförändrat kort medan postoperativ mortalitet minskat dramatiskt vid enheter som rapporterat sina resultat. Adjuvant behandling efter resektion bör endast ges inom ramen för kliniska studier. Det palliativa omhändertagandet har förbättrats främst genom utveckling inom endoskopi, interventionell radiologi, smärt- och nutritionsbehandling. Palliativ cytostatikabehandling bör endast ges selektivt utanför kliniska studier. Radioterapi har ingen dokumenterad effekt på överlevnaden vid icke-resektabel pankreascancer.     • The concept of the Media Welfare State describes Nordic specificity in how media are organised and how they serve a lively and inclusive democracy. This article engages in a dialogue in regards to the contention that this media system has persisted in the midst of rapid social change. We synthesise previous research and documented changes in media policy in Sweden, covering the last three decades, to show the ways in which the Swedish media system has undergone significant transformations. Media use is becoming more polarised and connected to social class. The state is retreating from its involvement in media policy; consequently, the press and public service media are facing unprecedented challenges. Finally, the “consensual” relation between media companies and the state, which is said to be typical for the media welfare state, no longer characterises the media market. While some of the features of the media welfare state system remain in Sweden, the current media system is best characterised as a neoliberal media welfare state. The article discusses tensions and conflicts in the existing model and possible future developments.

    • Social class is one of the most enduring concepts in the social sciences and is associated with many of the key themes and topics across multiple disciplines. Thus, there are many uses to which the concept of social class in media and communication can be put. The ambition of this special issue is to offer a glimpse into the heterogeneity of its uses in the field. While attuned differently in relation to the various approaches and definitions of class, the eight contributions to this special issue collectively focus on the relationship between media and class. One of the ambitions of this special issue is to report on some of the current Nordic scholarship in this growing field of research and to highlight the particularities of the relationship between class, inequality and media in this region. While the special issue also includes contributions from other regions, the main focus is on social class in the Nordic countries.This introductory article begins with a discussion on the concept of social class and its analytical relevance in the contemporary social landscape. We detail the relationship between media and class as manifested in a growing body of research, followed by a brief presentation of the individual contributions to this special issue. Finally, we identify the road ahead and potential research areas for scholars of media and communication concerned about class and social inequality in the twenty-first century.

    • Previous research has neglected media audiences’ and citizens’ opinions on how the media should be organized, how they should function in society and what individual, corporate and state responsibilities should be in regard to these questions. In an attempt to understand the relationship between citizens’ broader political attitudes and their attitudes on media-related politics and responsibilities, this study uses a survey (n = 2003) of the adult Swedish population to investigate the distribution of a range of media political attitudes in the contemporary space of political positions. The results reveal overlaps between the space of media political attitudes and the broader political space, where support for a Nordic ‘media welfare state’ corresponds to leftist and GAL-oriented values, while TAN-oriented and right-wing attitudes link to scepticism towards state interventionism in the media landscape. A small but highly opinionated right-wing and TAN-oriented segment displays laissez-faire views on media policy that are reflected in current policy propositions from right-wing political parties in parliament.

In this article, we explore media welfare from a normative perspective. Our intention is to open up a discussion about the normative and political implications of the notion of media welfare and to bring the concept into the contemporary discussion on normative perspectives regarding the media.

    • This editorial introduces a thematic issue on "Rethinking Media and Social Space". By critically rethinking the relationship between media and social space this issue takes initial steps towards ensuring that media studies is appropriate for a mediatized world. Contemporary societies are permeated by media that play important roles in how people maneuver and position themselves in the social world. Yet, analyses of media-related social change too often fail to engage with the complex and situated nature of power relations. This editorial highlights three enduring problems: (1) the annihilation of the socially structured and structuring role of media technologies and practices; (2) the conflation of inherent social capacities of media technologies and discourses with existing mediations of power, and (3) the reduction of social space to one predominant dimension which overshadows all other forms of social power that media technologies, discourses, and practices are part of. As a response to these problems - and in bringing together the arguments of the five articles included in the thematic issue-this editorial calls for sociologized approaches to media technologies, discourses, and practices.
