  • Does occupational forward bending of the back increase long-term sickness absence risk? : A 4-year prospective register-based study using device-measured compositional data analysis

    • Objective Forward bending of the back is common in many jobs and a risk factor for sickness absence. However, this knowledge is based on self-reported forward bending that is generally imprecise. Thus, we aimed to investigate the dose-response relation between device-measured forward bending at work and prospective register-based risk of long-term sickness absence (LTSA). Methods At baseline, 944 workers (93% from blue-collar jobs) wore accelerometers on their upper back and thigh over 1-6 workdays to measure worktime with forward bending (>30 degrees and >60 degrees) and body positions. The first event of LTSA (>= 6 consecutive weeks) over a 4-year follow-up were retrieved from a national register. Compositional Cox proportional hazard analyses were used to model the association between worktime with forward bending of the back in an upright body position and LTSA adjusted for age, sex, body mass index (BMI), occupational lifting/carrying, type of work, and, in an additional step, for leisure time physical activity (PA) on workdays. Results During a mean worktime of 457 minutes/day, the workers on average spent 40 and 10 minutes on forward bending >30 degrees and >60 degrees in the upright position, respectively. Five more minutes forward bending >30 degrees and >60 degrees at work were associated with a 4% [95% confidence interval (CI) 1.01-1.07] and 8% (95% CI 1.01-1.16) higher LTSA risk, respectively. Adjustment for leisure-time PA did not influence the results. Conclusion We found a dose-response association between device-measured forward bending of the back and prospective LTSA risk. This knowledge can be integrated into available feasible methods to measure forward bending of the back for improved workplace risk assessment and prevention.

    • Background: Despite extensive efforts, issues like obesity and poor physical capacity remain challenges for a healthy work life in several occupations. The Goldilocks work principle offers a new approach, encouraging design of productive work to promote physical capacity and health. This paper presents the protocol for the Goldilocks-childcare study, a randomised controlled intervention trial aiming to evaluate the effectiveness of implementing the Goldilocks work principle in childcare. The primary aim of the intervention is to increase time in moderate to vigorous physical activity (MVPA) by having the childcare workers act as active role models for children in daily playful physical activities, and thereby improve cardiorespiratory fitness and health of the workers.Methods/ design: The study is a cluster-randomised trial with a usual-practice wait-list control group. The 10-week intervention consists of two phases. In the first, the childcare workers will participate in two participatory workshops aiming, a) to develop playful physical activities (‘Goldilocks-games’) for children in which childcare workers participate as active role models at MVPA intensity, and b) to develop action plans for implementation of the Goldilocks-games in daily work routines. In the second phase, childcare institutions will implement the Goldilocks-games. The primary outcome is working time spent in MVPA, and secondary outcomes are cardiorespiratory fitness, sleeping heart rate, perceived need for recovery, and productivity. Primary outcome and process evaluation will be based on direct measurements of physical activity and heart rate, determination of cardiorespiratory fitness, and questionnaires.Discussion: If proven effective, the Goldilocks work principle has a large potential for promoting sustainable health and working lives of childcare workers.

    • Childcare workers are reported to have poor cardiorespiratory fitness and health. The Goldilocks Work Principle argues that productive work should be designed with the right composition, intensity and alternations of physical behaviors so that workers get fit and healthy. The purpose of this study was to investigate: (1) composition, (2) intensity and (3) alternations of physical behaviors during work and leisure among childcare workers. Data were collected using accelerometers and heart rate monitors over five workdays among 51 childcare workers at five Danish childcare institutions. Workers mainly spent their work time sedentary (43.0%), spent little time (0.7%) at sufficiently high cardiometabolic intensity to increase cardiorespiratory fitness and often alternated between physical behaviors (67.0% occurred in bouts of <5 min). These findings indicate that the workers have a composition of behaviors at work dominated by sedentary time, little time with high cardiometabolic intensity, and frequent alternations between behaviors. During leisure, workers spent more time sedentary (59.4%), more time at high cardiometabolic intensity (3.4%) and less time occurred in bouts <5 min (38.7%). We see a potential for promoting cardiorespiratory fitness and health of childcare workers by redesigning the way they play with the children, so that work time with high cardiometabolic intensity is increased.

  • Effectiveness of a Goldilocks Work intervention in childcare workers – A cluster-randomized controlled trial

    • Objective Poor cardiorespiratory fitness and health is common among childcare workers. We designed “Goldilocks-games” according to the Goldilocks Work principle to provide high-intensity physical activity for childcare workers. We investigated the effectiveness of this Goldilocks Work intervention in increasing occupational high-intensity physical activity and improving work-related health.Methods In a two-arm cluster randomized trial, 16 childcare institutions with 142 workers were randomly allocated to either an 8-week Goldilocks Work intervention or a control group. The primary outcome was occupational time in high-intensity physical activity. Secondary outcomes were occupational time in active physical behaviors, heart rate during sleep, pain, physical exhaustion, energy at work, work productivity, and need for recovery. Results The intervention was successfully delivered and received. Both groups had a low amount of occupational high-intensity physical activity at baseline, and the intervention group reported playing the games 3.1 (SD 1.5) times/week for a duration of 112.2 (SD 175.0) min/week. However, the intervention did not increase high-intensity physical activity or the secondary outcomes, except for energy at work, measured on a scale from 0-10, increasing 0.65 (95% CI, 0.08-1.21), and need for recovery, measured on a scale from 1-5, decreasing -0.32 (95% CI, -0.54- -0.09).Conclusion The intervention was successfully delivered and received, but did not increase high-intensity physical activity. The intervention group increased their energy at work and decreased their need for recovery, but not the other health-related outcomes. Research on how to design and implement health-promoting work environment interventions in childcare are needed.

  • How does accelerometry-measured arm elevation at work influence prospective risk of long-term sickness absence?

    • Objective Elevated arm work is prevalent in many jobs. Feasible device-based methods are available to measure elevated arm work. However, we lack knowledge on the association between device-measured elevated arm work and prospective risk of long-term sickness absence (LTSA). We aimed to investigate this association.Methods At baseline, 937 workers wore accelerometers on the right arm and thigh over 1–5 workdays to measure work time spent with elevated arms in an upright position. Between baseline and 4-year prospective follow-up in the national registers, we obtained information on the individuals` first event of LTSA (≥6 consecutive weeks). We performed compositional Cox proportional hazard analyses to model the association between work time with arm elevation >30˚, >60˚, or >90˚ and the probability of LTSA.Results Workers spent 21% of their work time with >30˚ arm elevation, 4% with >60˚arm elevation, and 1% with >90˚ arm elevation; in the upright body position. We found a positive dose–response association between work time spent with elevated arm work and the risk of LTSA. Specifically, we found that increasing two minutes of work time spent with arm elevation at (i) >90˚ increased the risk of LTSA by 14% [hazard ratio (HR) 1.14, 95% confidence intervals (95% CI 1.04–1.25)] (ii) >60˚increased the LTSA risk by 3% (HR 1.03, 95% CI 1.03–1.06), and (iii) >30˚ increased the LTSA risk by 1% (HR 1.01, 95% CI 1.00–1.02).Conclusion Device-measured elevated arm work is associated with increased prospective LTSA. This information ought to be brought into preventive workplace practice by accessible and feasible device-based methods of elevated arm work.

  • Movement behavior profiles and obesity : a latent profile analysis of 24-h time-use composition among Danish workers

    • Background/objectivesAn element of obesity prevention is increasing total physical activity energy expenditure. However, this approach does not incorporate the balance of various movement behaviors—physical activity, sedentary behaviors and sleep - across domains of the day. We aimed to identify time-use profiles over work and leisure, termed ‘movement behavior profiles’ and to investigate their association with obesity.Subjects/methodsEight-hundred-and-seven workers completed (a) thigh accelerometry and diaries to determine their 24-h composition of behaviors (sedentary and standing, light physical activity and moderate-to-vigorous physical activity at work and leisure, and time in bed) and (b) obesity measurements. Movement behavior profiles were determined using latent profile analyses of isometric log-ratios of the 24-h composition, and labeled according to animal movement behavior traits. Linear models were applied to determine the association between profiles and obesity.ResultsFour profiles were identified, labeled as “Chimpanzees” (n = 226), “Lions” (n = 179), “Ants” (n = 244), and “Koalas” (n = 158). “Chimpanzees” work time was evenly distributed between behaviors while their leisure time was predominantly active. Compared to Chimpanzees, “Lions” were more active at work and sedentary during leisure and spent more time in bed; “Ants” were more active at work and during leisure; “Koalas” were more sedentary at work and leisure and spent similar time in bed. With “Chimpanzees” as reference, “Lions” had least favorable obesity indicators: +2.0 (95% confidence interval [CI] 0.6, 3.4) %body fat, +4.3 cm (1.4, 7.3) waist circumference and +1.0 (2.0, 0.0) Body Mass Index (BMI), followed by “Koalas” +2.0 (0.4, 3.7) %body fat, +3.1 cm (0.1, 6.0) waist circumference, and +0.8 (−0.30, 1.94) BMI. No significant differences were found between “Chimpanzees” and “Ants”.ConclusionsMovement behavior profiles across work and leisure time-use compositions are associated with obesity. Achieving adequate balance between work and leisure movement behaviors should be further investigated as a potential obesity prevention strategy.

  • Physical-behavior profiles and aerobic capacity: A latent profile analysis of 24-hour time-use composition among Danish workers

    • Introduction Obtaining a balance between exercise and recovery is an established approach in Exercise Science to improve aerobic capacity. However, it is unknown how different 24-h time-use profiles depicting a balance between physical behaviors (i.e. physical activities, sedentary behavior and sleep) across main domains of the day are associated with aerobic capacity. We aimed to identify such 'physical-behavior (PB) profiles' and to investigate their association with aerobic capacity. Methods Workers (n=807) participated in thigh-accelerometry to determine 24-h time-use composition of physical activity, sedentary and standing during work and leisure, as well as time in bed. Åstrand submaximal cycle ergometer test was used to estimate aerobic capacity. The PB profiles were determined using latent profile analysis of isometric log-ratios representing the 24-hour composition. Linear models were applied to determine the cross-sectional association between physical-behavior profiles and aerobic capacity. Results Four PB profiles were identified that were labeled based on animal traits; Chimpanzees (n=226), Lions (n=179), Ants (n=244), and Koalas (n=158). Compared to Chimpanzees (at work, in mins; sedentary=197, standing=145, physical activity=117; and at leisure in mins; physical activity=114, standing=121, time in bed= 440); Lions were more active at work, sedentary at leisure and spent more time in bed; Ants had more physical activity at work and similar physical activity and time in bed at leisure; Koalas were more sedentary at work and leisure and spent more time in bed. Compared to Chimpanzees, Koalas had lower aerobic capacity (mlO2/kg/min): -3.7 (95%CI -6.0,-1.5), followed by Lions -3.6 (-5.5,-1.7) and Ants -1.8 (-3.7,- 0.1). Conclusions Physical-behavior profiles based on 24-h time-use composition are associated with aerobic capacity. Obtaining a balance between physical behaviors at work and leisure may be a promising approach for improving aerobic capacity.

    • Data on the use of time in different exposures, behaviors and work tasks are common in occupational research. Such data are most often expressed in hours, minutes, or percentage of work time. Thus, they are constrained or “compositional”, in that they add up to a finite sum (e.g. 8 hours of work, or 100% work time). Due to their properties, compositional data need to be processed and analyzed using a set of specifically adapted methods called compositional data analysis (CoDA). CoDA has become an established framework to handle compositional data in various scientific fields such as nutritional epidemiology, geology and chemistry, but has only recently gained attention in public and occupational health sciences. In this paper, we introduce the reader to CoDA by explaining why CoDA should be used when dealing with compositional time-use data, showing how to perform CoDA, including a worked example, and pointing at some remaining challenges in CoDA. The paper concludes by emphasizing that CoDA in occupational research is still in its infancy, and stresses the need for further development and experience in the use of CoDA for time-based occupational exposures. We hope that the paper will encourage researchers to adopt and apply CoDA in studies of work exposures and health.

    • "Sit less-move more" has been the univocal advice to adults for better health. Predominantly, this advice is based on research of physical behaviors during leisure-time. A recent study among > 100,000 adults indicates a u-shaped association between leisure-time physical activity and risk for cardiovascular disease and mortality among adults in physically active occupations. This may be explained by the considerable difference in 24-h physical behaviors between adults in sedentary and physically active occupations. Thus, the advice "sit less-move more" might not be the best for health among adults in physically active occupations. To provide a scientific approach and encourage research on 24-h physical behaviors and health for those in physically active occupations, we propose the "Sweet-Spot Hypothesis." The hypothesis postulates that the "Sweet-Spot" of 24-h physical behaviors for better health differs between adults, depending on their occupation. Specifically, the hypothesis claims that the advice "sit less-move more" does not bring adults in physically active occupations toward their "Sweet-Spot" of 24-h physical behaviors for better health. The purpose of our paper is to encourage researchers to test this proposed hypothesis by describing its origin, its theoretical underpinning, approaches to test it, and practical implications. To promote health for all, and decrease social health inequalities, we see a great need for empirically testing the "Sweet-Spot Hypothesis." We propose the "Sweet-Spot Hypothesis" to encourage discussion, debates, and empirical research to expand our collective knowledge about the healthy "24-h physical behavior balance" for all.

  • Compositional analysis of sedentary behavior and physical activity during work and leisure among male and female office workers

    • Objective: To determine the extent to which male and female office workers differ in their time-use composition of sitting behaviors (SB) and physical activity (PA) during work and leisure.Methods: SB and PA was measured using thigh-worn accelerometers for up to 8 full days in 77 male and 104 female office workers. Daily time-use compositions during work and leisure were described in four exhaustive categories, i.e. sitting in short (<30  min) and long (≥30 min) bouts, standing, and active behaviors. Following a compositional data analysis procedure, isometric log-ratios (ilr) were calculated to express time in sitting relative to non-sitting, short relative to long sitting bouts, and standing relative to active behaviors. Differences between sexes (men and women) and domains (work and leisure) were examined on the basis of these ilrs using ANOVA.Results: At work, time spent sitting in short bouts, long bouts, standing, and active was, on average, 34%, 36%, 22% and 8% among men and 31%, 37%, 24% and 8% among women. Corresponding proportions during leisure were 34%, 25%, 28% and 13% among men and 29%, 28%, 31% and 12% among women. Time spent sitting relative to non-sitting differed significantly between work and leisure (ilr sitting-vs-non-sitting, partial eta squared=0.09, p<0.01). During leisure, men used proportionally more time than women in short sitting bouts (ilr short-vs-long, partial eta squared=0.06, p<0.01) and spent more time in active behaviors relative to standing (ilr standing-vs-active, partial eta squared=0.04, p<0.01). No significant sex differences were observed during work (p>0.05).Conclusions:  The leisure behavior observed among men is probably more beneficial for health than that observed for women. However, both men and women spent a major proportion of their time sitting, both at and outside their office work, and they were, in general, only little active. Thus, both men and women could benefit from interventions to reduce SB and increase PA both at work and during leisure.

  • Sitting, standing and moving during work and leisure among male and female office workers of different age: A compositional data analysis

    • Background Gendered patterns of physical activity behaviours may help explaining health inequalities between men and women. However, evidence on such patterns in the working population is sparse. This study aimed at documenting and comparing compositions of sitting, standing and moving at work and during leisure among male and female office workers of different age.Methods Sitting (including lying), standing and moving were measured using accelerometry for, on average, four working days in 55 male and 57 female Swedish office workers. Behaviours were described in terms of time spent in four exhaustive categories: sitting in short (<30 min) and long (≥30 min) bouts, standing, and moving. In a compositional data analysis approach, isometric log-ratios (ilr) were calculated for time sitting relative to non-sitting, time in short relative to long sitting bouts, and time in standing relative to moving. Differences between genders (men vs. women), domains (work vs. leisure), and according to age were examined for each ilr using ANOVA.Results At work, time spent sitting in short bouts, sitting in long bouts, standing, and moving was, on average, 29%, 43%, 21% and 7% among men, and 28%, 38%, 26% and 7% among women. Corresponding proportions during leisure were 34%, 27%, 27% and 13% among men and 28%, 27%, 32% and 13% among women. Men spent more time sitting relative to non-sitting (partial eta-squared=0.04, p=0.03) than women, and less time standing relative to moving (pes=0.07, p=0.01). At work compared to during leisure, both genders spent more time sitting relative to non-sitting (pes=0.47, p<0.01); within sitting more time was spent in long relative to short sitting bouts (pes=0.26, p<0.01), and within non-sitting, more time was spent standing than moving (pes=0.12, p<0.01). Older workers spent less of their non-sitting time moving than younger workers (pes=0.07, p=0.01).Conclusion Male office workers spent more time sitting relative to non-sitting than female workers, and more time moving relative to standing. Both genders were sitting more at work than during leisure. Older workers moved less than younger. These workers could likely benefit from interventions to reduce or break up prolonged sitting time, preferably by moving more.

    • ObjectivesContrary to leisure-time physical activity, occupational physical activity (OPA) may have harmful health effects, called the physical activity paradox. A proposed mechanism is that OPA can elevate the heart rate (HR) for several hours per day. We aimed to investigate the association between the mean intensity of OPA and HR variability (HRV) indices the following night.MethodsThree cohorts (NOMAD, DPhacto, and Physical Workload and Fitness) involving blue-collar workers front different sectors were merged in this study. HR monitors (Actiheart) recorded 24-hour inter-beat intervals (IBI) for up to four consecutive days. The relative intensity of the mean HR during work was estimated by HR reserve (%HRR), and time-domain indices of HRV were analyzed during the following night. Data were analyzed using a multilevel growth model to test the association between mean %HRR during work and HRV indices at night in a day-by-day analysis adjusted for age, BMI, alcohol consumption, smoking, and occupation.ResultsThe dataset included a sample of 878 Danish blue-collar workers, with a mean %HRR during work of 31%, and 42% worked at an intensity >= 30%HRR. The multilevel model showed negative within- and between-subject associations between %HRR during work and HRV indices at night.ConclusionsOur results indicate a higher %HRR during work to associate with lower HRV indices the following night and a higher HR, reflecting an imbalanced autonomic cardiac modulation. This finding supports a high mean HR during work to be a potential underlying mechanism for the harmful health effect of OPA.

    • Childcare workers are reported to have high variation in physical activity during work hours, but also to sit for about half of the workday and have almost no high intensity physical activity (HIPA). No study has investigated if their work can be re-designed to introduce HIPA, thus promoting fitness and health according to the Goldilocks principle. This study investigated the feasibility of designing pedagogical games (‘Goldilocks-games’) intended to lead to more HIPA. Heart rate was measured in nineteen childcare workers during Goldilocks-games, and compared to measurements during a regular workday. Worker perceptions of feasibility, and researcher observations of contextual factors were also collected. The Goldilocks-games (33 min) elicited significantly more HIPA (18/33 min) compared to the most active period of equal length on a regular workday (0.5/33 min). Seventy-four-percent of the childcare workers reported that it was feasible to integrate the Goldilocks-games pedagogically, and seventy-two-percent could see themselves using them. Thus, we found it possible to re-design a work task in childcare according to the Goldilocks principle so that it leads to substantial time with HIPA. The sustainability of Goldilocks-games in childcare, and their effectiveness in improving fitness and health among childcare workers, needs to be tested in further studies.

    • Background: The Goldilocks Work Principle expresses that productive work should be designed to promote workers’ health. We recently showed that it is feasible to develop and implement modifications to productive work that change physical behaviors (i.e. sitting, standing and being active) in a direction that may promote health among industrial workers. Therefore, the aim of the present study is to conduct a cluster randomised controlled trial investigating health effects of implementing the Goldilocks Work intervention among industrial workers. Methods: Our implementation plan consists of educating work teams, organizing implementation meetings, and providing feedback to workers. Three meetings with a preselected local workplace group will be scheduled. The first meeting educates the group to use a planning tool by which work can be planned to have ‘just right’ physical behaviors. The second and third meetings will focus on supporting implementation of the tool in daily work. An expected 28 clusters of work teams across two participating production sites will be randomized to either intervention or control group. Data collection will consist of 1) questionnaires regarding work and musculoskeletal health, 2) wearable sensor measurements of the physical behavior, and 3) assessment of general health indicators, including BMI, blood pressure, and fat percentage. The primary outcome is musculoskeletal health, measured by low back pain intensity, and secondary outcomes are 1) physical behaviors at work, 2) accumulated time in long bouts of sitting, standing, and being active and 3) perceived fatigue and energy during work. Furthermore, implementation and cost of the intervention will be evaluated based on questionnaires and data from the planning tool completed by the workers.Discussion: This study will evaluate the effectiveness and implementation of a 12- weeks Goldilocks Work intervention with the aim of improving musculoskeletal health among industrial workers. The cluster randomized controlled study design and the evaluation of the implementation, results and costs of the intervention will make it capable of contributing with valuable evidence of how productive work may be designed to promote industrial workers’ health.

    • The Goldilocks Work Principle expresses that productive work should be redesigned to comprise physical behaviors of different intensities in a composition promoting workers’ health and fitness. This study is the first to assess the feasibility of redesigning work in an industrial setting according to the Goldilocks Work Principle. We recruited workers (n=20) from a brewery in Denmark, and we conducted a participatory 16-week intervention including a workshop and two consultations. The workshop aimed to support the workers in modifying their work, while the consultations assisted the eventual implementation. Feasibility was evaluated as per three aspects: 1) developing modifications of work, 2) implementing these modifications, and 3) changing physical behavior and self-reported fatigue, pain and energy. The three aspects were addressed through records completed by the workers, measurements of workers’ physical behavior and intensity during ‘control’ workdays (i.e., usual work) and ‘intervention’ workdays (i.e. modified work), and self-reported fatigue, pain and energy level following both types of workdays. Five modifications to work were developed, and three of these five modifications were implemented. To some extent, physical behavior and intensity changed as intended during ‘intervention’ workdays compared to ‘control’ workdays. Workers were also less fatigued, had less pain, and had more energy after ‘intervention’ workdays. These results suggest that it is feasible to develop and implement modified work based on the Goldilocks Work Principle among industrial workers. However, we also identified several barriers to the implementation of such modifications.

  • Accelerometer-measured physical activity at work and need for recovery: A compositional analysis of cross-sectional data

    • ObjectivesPrevious research has shown strong associations between occupational physical activity (OPA) and need for recovery (NFR). However this research has only utilized self-reported measures of OPA which may be biased. Thus, there is a need for investigating if the previously documented association between self-reported OPA and NFR can be found when using technical measures of OPA. There is also the need to investigate whether older workers are particularly susceptible to increased NFR, since age-related declines in physical capacity mean that it is likely these workers will have a higher NFR for a given physical activity. The aim of this study was to investigate the association between technically measured OPA and NFR, and whether this relationship is modified by age.MethodsThis study utilized data from the Danish Physical Activity Cohort with Objective Measurements cohort—comprising Danish workers (n = 840) from the cleaning, manufacturing, and transportation sectors. OPA was measured by accelerometers attached to the thigh and upper back for at least one work day and classified into four physical behaviour categories (sedentary, standing, light, or moderate/vigorous). NFR was measured using a shortened version of the Danish NFR scale. Analysis was conducted using linear regression and isotemporal substitution analyses for compositional data.ResultsThe overall association between OPA and NFR was statistically significant in the unadjusted model (P < 0.001), but not when adjusted for age, sex, occupation, and shift work (P = 0.166). Isotemporal substitution showed small but significant reductions in NFR when increasing sedentary time relative to other behaviours (adjusted: ΔNFR = −0.010 [−0.019; −0.001]). There were no significant interactions between age and OPA (P = 0.409).ConclusionsThis study found significant associations between OPA and NFR, but the effect sizes were small. Reallocating 30 min to sedentary behaviours from other behaviours was associated with a reduced NFR, but the effect size may not be practically relevant. Moreover, no clear modifying effects of age were identified.
