    • Stigma has been identified as one of the most important obstacles for a successful integration of people with mental illness into the society. Research about stigma has shown negative attitudes among the public towards people with mental illness. Studies so far have, however, put little emphasis on how these negative attitudes are perceived by the mentally ill persons. The aim of the present study was to investigate acceptability and internal consistency of the Swedish versions of two stigma scales, the Devaluation and Discrimination scale and the Rejection experiences scale. Forty individuals were subject to an interview, which also comprised assessments of needs for care, quality of life, therapeutic relationship and empowerment. The results showed that both the Devaluation and Discrimination scale and the Rejection experiences scale had a good internal consistency and acceptability. Stigma in terms of perceived devaluation and discrimination was found to be most markedly associated with empowerment and rejection experiences was found to be most associated with the number of previous psychiatric admissions. It is concluded that the Swedish versions of the Devaluation and Discrimination scale and the Rejection experiences scale may well be used in further studies of stigma among people with mental illness.

  • Delaktighet och dialog : Översiktsplanering i Kils kommun på annorlunda vis

    • I den planeringsprocess som lett fram till ett förslag till ny översiktsplan för Kils kommun (ÖP95) har nya former för medborgarinflytande prövats. Studiecirklar och dialogkonferenser har varit viktiga inslag i denna process. I rapporten återfinns dels en bakgrundsdiskussion om medborgarinflytande i planeringen, dels en dokumentation av hur arbetet med ÖP95 gått till och dels en utvärdering av i första hand studiecirkelverksamheten och hur den följts upp i den fortsatta processen.I samråd bl a med länsstyrelsen i Värmland beslöt Kils kommun att försöka utarbeta en ny översiktsplan på mänskligt vis, genom att låta arbetet inledas med en studiecirkelfas där vanliga medborgare skulle uttrycka sina önskemål och synpunkter. Fem cirkelgrupper sattes samman efter speciella kriterier: en kvinnogrupp, en mansgrupp, en ungdomsgrupp, en äldregrupp och en grupp med småbarnsföräldrar. Studiecirklarnas arbete och cirkeldeltagarnas upplevelser redovisas tämligen ingående i rapporten.Cirklarnas resultat överlämnades så småningom vid en s k dialogkonferens till en kommunal arbetgrupp bestående av politiker och tjänstemän, hälften män och hälften kvinnor. Tillsammans med kommunens planeringsansvariga utformade arbetsgruppen ett planförslag, som diskuterades vid en andra dialogkonferens med studiecirkeldeltagarna. Därefter lämnades förslaget till kommunstyrelsen för den fortsatta formella behandlingen.I rapporten konstateras bland annat att det i processen finns ett antal kritiska skeden, där kontinuiteten i medborgarinflytandet sätts på prov och där följsamhet och lyhördhet är viktigt. Det konstateras också att det slutliga planförslaget inte kom att bli dramatiskt annorlunda än en traditionell plan, men att det viktigaste kanske ändå är att medborgarna genom sitt deltagande i processen känner sig delaktiga i planen. Ett tecken på att man därvid lyckats väl är att cirkeldeltagarna vid den andra dialogkonferensen uppgav att de kände igen sig i arbetsgruppens planförslag.

    • The frequency of human papilloma virus (HPV) and its influence on clinical outcome was analyzed retrospectively in pre-treatment paraffin embedded biopsies from 110 patients with tongue cancer. The presence of HPV DNA was examined in 85 mobile tongue tumors and 25 base of tongue tumors by a polymerase chain reaction (PCR) with 2 general primer pairs, GP5+/6+ and CPI/IIG. When HPV-DNA was found, HPV-type specific primers and direct sequencing were used for HPV sub-type verification. Twelve of 110 (10.9%) samples were HPV-positive; 9 for HPV-16, 1 for HPV-33, 1 for HPV-35 and 1 could not be analyzed because of shortage of DNA. HPV was significantly more common in base of tongue tumors (10/25, 40.0%) compared to tumors of the mobile tongue (2/85, 2.3%). The influence of HPV on clinical outcome in mobile tongue cancer could not be studied, due to that HPV was present in too few cases. Of the 19 patients with base of tongue cancer that were included in the survival analysis, however, 7 patients with HPV-positive base of tongue cancer had a significantly favorable 5-year survival rate compared to the 12 HPV-negative patients. In conclusion, HPV is significantly more common in base of tongue cancer than in mobile tongue cancer, and has a positive impact on disease-specific survival in patients with base of tongue cancer.

    • Objective: In non-alcoholic fatty liver disease (NAFLD), liver fibrosis is the strongest predictor of adverse outcomes. We sought to investigate the relationship between liver fibrosis and cardiac remodeling in participants from the general population using magnetic resonance imaging (MRI), as well as explore potential mechanistic pathways by analyzing circulating cardiovascular biomarkers.Methods: In this cross-sectional study, we prospectively included participants with type 2 diabetes and individually matched controls from the SCAPIS (Swedish CArdioPulmonary bioImage Study) cohort in Linköping, Sweden. Between November 2017 and July 2018, participants underwent MRI at 1.5 Tesla for quantification of liver proton density fat fraction (spectroscopy), liver fibrosis (stiffness from elastography), left ventricular (LV) structure and function, as well as myocardial native T1 mapping. We analyzed 278 circulating cardiovascular biomarkers using a Bayesian statistica lapproach.Results: In total, 92 participants were enrolled (mean age 59.5 ± 4.6 years, 32 women). The mean liver stiffness was 2.1 ± 0.4 kPa. 53 participants displayed hepatic steatosis. LV concentricity increased across quartiles of liver stiffness. Neither liver fat nor liver stiffness displayed any relationships to myocardial tissue characteristics (native T1). In a regression analysis, liver stiffness was related to increased LV concentricity. This association was independent of diabetes and liver fat (Beta = 0.26, p = 0.0053), but was attenuated (Beta = 0.17, p = 0.077) when also adjusting for circulating levels of interleukin-1 receptor type 2.Conclusion: MRI reveals that liver fibrosis is associated to structural LV remodeling, in terms of increased concentricity, in participants from the general population. This relationship could involve the interleukin-1 signaling.

    • The aim of the study was to explore the relationship between stigmatizing rejection experiences and self-related variables. Our hypothesis was that rejection experiences would be negatively associated with perceptions of self-esteem, empowerment and sense of coherence. A cross-sectional study assessing rejection experiences, empowerment, sense of coherence and self-esteem was performed, including 200 persons in current or earlier contact with mental health services. The results showed that experiences of rejection were negatively associated with sense of coherence, empowerment and self-esteem. This exploratory investigation suggests that experiences of rejection might be a target for coping interventions. Mental health nurses are in a key position to identify patients' experiences of stigma and by that to understand what consequences of devaluation/discrimination can have for the afflicted.

PIMA-nätverket och över den regionala utvecklingen i Värmland. De därpå följande artiklarna behandlar ett brett spektrum av kulturgeografiska forskningsfält och ämnesvetenskapliga reflektioner och speglar därmed den mångfald av forskningsinriktningar och forskningsansatser som finns representerade bland kulturgeograferna i Karlstad.     • The basal plane resistivity varrho of stages 3–5 HNO3 intercalated graphite has been measured as a function of temperature T and pressure p in the range 130–320 K and 0–1.6 GPa (16 kbar). A contactless method was used. The phase boundary of the 250 K ordering transition at zero pressure was traced up to 320 K through the corresponding anomaly in varrho. The pressure dependence of varrho was strongly stage dependent, such that for stages 3, 4 and 5 the observed pressure coefficients (1/varrho)dvarrho/dp were approximately 0.08, −0.08, and −0.22 GPa−1, respectively, near zero pressure.

    • The overall aim of this thesis is to 1) investigate the prevalence of stigmatizing experiences and beliefs of devaluation and discrimination among persons with mental illness, and 2) to investigate the relationship between beliefs of devalua¬tion/¬discrimination, rejection experiences and sociodemographic/clinical patient characteristics, social networks, self esteem, empowerment and subjective quality of life. The thesis comprises four papers. Part one of the study was designed as a cross-sectional study using a convenience sample of 200 consumers in current contact with mental health services or with earlier experiences of this. In order to reach subjects with different experiences of mental illness recruitment were made at inpatient and outpatient settings, rehabilitation units and among members of user organizations. This part of the study is presented in paper I-III. In the second part of the study 25 mental health user's who participated in the cross-sectional study were interviewed with regard to experiences of rejection related to their mental illness (Paper IV). Paper I investigates prevalence of rejection experiences and beliefs of devaluation and discrimination and the relationship to sociodemographic and clinical patient charac¬teristics. Except for a few areas, more than half of the participants believed that psychiatric patients might face severe devaluation and discrimination. Almost half of the participants reported that they avoided people, because they felt that they look down on people with mental illness. Paper II examines beliefs of devaluation/¬discrimination, actual rejection experiences, empowerment, social network and the relationship of these factors to subjective quality of life among 150 individuals with mental illness. Paper III compares subgroups with low and high levels of rejection experiences with regard to self-esteem, sense of coherence and empowerment. In summary this thesis shows moderate associations between more frequent rejection experiences and a lower degree of global functioning, a greater number of admissions to inpatient psychiatric care and with current contact with social services (Paper I). In Paper II the factors most prominently related to subjective quality of life were overall empowerment and overall social network. For a subgroup of subjects suffering from psychotic disorders there was a significant negative relationship between subjective quality of life and rejection experiences. Subjects suffering from psychotic disorders also reported more frequent rejection experiences than subjects with affective disorders or other disorders. The main finding in Paper III was that more rejection experiences were associated with lower level of sense of coherence and to a lesser extent, with lower level of empowerment and self-esteem. Paper IV showed that experiences of devaluation and discrimination can be found in many areas, and seemed to result in more constricted social networks, unemployment and reductions of life opportunities.

  • Experiences of rejection by mental health patients - A qualitative study

    • Background and aim: People suffering from mental illness and their experiences of attitudes towards them are rarely investigated from the perspective of the individual. The aim was to gain an understanding of how a group of mental health patients experienced social relationships in personal settings as well as in society. Method: Open interviews with twenty-five mental health patients were conducted and analysed with a qualitative content analysis. Result: The essence of the result was that mental health patients’ experiences are still not taken enough into account, neither by mental health professionals nor by their social networks. This was underpinned by four core categories: Patients’ experience of deteriorated and reduced social contacts due to various degrees of acceptance and knowledge of people in general; service users reported on difficulties with social contacts in general, with family relationships, relationships with friends and workmates and with employers; reduced life opportunities were expressed, including violated self-image and poor coping competence and, ambivalent experiences of contact with the mental health services were reported. Conclusion and clinical implication: The present study contributes to the understanding of mental health patients’ experiences of professional services and of their social networks. This knowledge may strengthen the implications of patient-centred care essential for the outcome of the care.

  • "Jesus är för spanjorerna". Studier av den katolska missionens misslyckande i det tidigkoloniala Peru

    • The general aim of this study is to investigate the development of the Catholic mission in Peru between 1540 and 1622. Its point of departure is the fact that the efforts of the Catholic Church to Christianize the Indians failed. Its specific aims are twofold. First, to examine the veracity of the traditional images of the mission’s main protagonists, i.e., the priest who allegedly neglected his duties and abused his flock and the reputedly primitive but good-hearted Indian. Second, to examine whether there are only two parties, the priest and the Indian, responsible for the failure of the missionary endeavour. For my study I use “conciliar” sources such as minutes from church meetings (concilia and synods), and “non-conciliar” sources such as reports that were written outside the direct control of the Church, in order to provide different perspectives. In my view, historians like Rubén Vargas Ugarte and Pierre Duviols, both among the most accomplished specialists in the history of the Peruvian Church, have criticised the missionaries without adequately considering the frequently extreme circumstances that conditioned their work. Neither has the role of the Church as an institution been subject to sufficient critical scrutiny in previous research, while I argue that its leadership was indeed the third protagonist in the mission. There are reasons why the priest’s reputation is tarnished. When considered, the physical and mental circumstances of his situation might not excuse him but they do make his behaviour understandable. As far as the passive but benevolently inclined Indian is concerned, he has emerged as more actively opposing and resisting the conversion efforts by, for example, manipulating Spanish legal authorities to the detriment of the priest’s work. Examining the reputation of the priest and the Indian has also helped us see the role that the leadership of the Church played in the mission. The Church was in overall control of the mission, and no doubt its leaders were of great importance to it. However, when the problems and the final failure of the mission became increasingly apparent to all it was obvious that the Church had no interest in being held accountable, instead blaming the lower clergy, the missionaries on the ground. The conclusion of this study is that any discussion about the responsibility for the mission’s problems and final fiasco must include the Church. Its description of the main protagonists of the mission, where it excludes itself and its leaders, has essentially been adopted by Vargas Ugarte and Duviols. In my view they have accepted the portrayal of the priest and the Indian, as presented by the Church in the conciliar material, without applying adequate critical methods and without doing so in the context of the failed mission. Hence, their interpretation of the mission and its main protagonists will have to give way to the one presented in this investigation.

    • We have measured the pressure dependence up to 2.5 GPa of the electrical resistance of the acceptor intercalated graphites CnFeCl3, CnSbF5, and CnHNO3. The slope of resistance versus pressure is approximately the same in all cases. Two different types of high pressure transitions are observed in CnSbF5 and CnHNO3. The results of electrical resistance measurements under pressure in polypyrrole are consistent with a simple extension of the variable-range-hopping model. We infer for this material that the density of states at the Fermi level increases with pressure.

    • The resistivity of a commercial carbon-filled composite conducting polymer (ET-Semicon[large-closed-square]) has been measured as a function of temperature between 80 and 400 K and under pressure up to 1.5 GPa (15 kbar). Large changes in resistivity were observed. The resistivity was also very sensitive to the presence of certain solvents and hydrocarbons. The results are explained as percolation effects caused by changes in volume due to pressure, thermal expansion, or dissolved solvents. The material studied is found to have a wide range of potential applications for pressure measurements and as a transducer for gas or liquid concentration.

    • BACKGROUND: A major goal in mental health research and policy is to identify ways to reduce stigma among persons with mental illness. AIMS: The aims of the present study were to (1) investigate the prevalence of rejection and devaluation/discrimination in a cross-sectional sample of 200 individuals with experiences of mental illness and (2) investigate the relationship between sociodemographic and clinical, client characteristics and perceived devaluation/discrimination and experiences of rejection. METHODS: A total of 200 subjects in current contact with mental health services or with earlier experiences of this were interviewed regarding beliefs about devaluation/discrimination and rejection experiences. RESULTS: The results showed that subjects with a higher degree of global functioning perceived less devaluation/discrimination. With regard to rejections experiences associations were found between rejection experiences and global functioning, number of admissions to inpatient psychiatric care and current contact with social services. CONCLUSION: Further research should investigate the causal relationship between stigma experiences and psychosocial functioning. To understand what consequences perceived devaluation/discrimination has for the afflicted, a psychological and social approach in the research must be taken into consideration.

Methods: A cross-sectional study assessing beliefs of devaluation/discrimination, actual rejection experiences, empowerment, social network and subjective quality of life was performed on 150 subjects with mental illness. Results: The factors most prominently related to subjective quality of life were overall empowerment and overall social network. For a subgroup of subjects suffering from psychotic disorders there was a significant negative relationship between subjective quality of life and rejection experiences. This group also reported more frequent rejection experiences. Conclusion: The findings indicate that persons with mental illness are a heterogeneous group with regard to the influence of social network, stigma and empowerment on subjective quality of life. The negative impact of actual rejection experiences might express that, in order to increase subjective quality of life, stigma must be addressed as a separate and important factor in its own right.

    • Muscular adaptation to physical exercise has previously been described as a repair process following tissue damage. Recently, evidence has been published to question this hypothesis. The purpose of this study was to investigate inflammatory processes in human skeletal muscle and epimysium after acute physical exercise with large eccentric components. Three groups of subjects (n= 19) performed 45 min treadmill running at either 4 deg (n= 5) or 8 deg (n= 9) downhill or 4 deg uphill (n= 5) and one group served as control (n= 9). One biopsy was taken from each subject 48 h post exercise. Blood samples were taken up to 7 days post exercise. Compared to the control group, none of the markers of inflammation in muscle and epimysium samples was different in any exercised group. Only subjects in the Downhill groups experienced delayed onset of muscle soreness (DOMS) and increased serum creatine kinase activity (CK). The detected levels of immunohistochemical markers for T cells (CD3), granulocytes (CD11b), leukaemia inhibitory factor (LIF) and hypoxia-inducible factor 1beta (HIF-1beta) were greater in epimysium from exercised subjects with DOMS ratings >3 (0-10 scale) compared to exercised subjects without DOMS but not higher than controls. Eccentric physical exercise (downhill running) did not result in skeletal muscle inflammation 48 h post exercise, despite DOMS and increased CK. It is suggested that exercise can induce DOMS by activating inflammatory factors present in the epimysium before exercise. Repeated physical training may alter the content of inflammatory factors in the epimysium and thus reduce DOMS.

    • Rationale: Accumulating studies suggest that nitric oxide (NO) deficiency and oxidative stress are central pathological mechanisms in type 2 diabetes (T2D). Recent findings demonstrate therapeutic effects by boosting the nitrate-nitrite-NO pathway, which is an alternative pathway for NO formation. This study aimed at investigating the acute effects of inorganic nitrate on glucose and insulin signaling in adenosine A2B receptor knockout mice (A(2B)(-/-), a genetic mouse model of impaired metabolic regulation. Methods: Acute effects of nitrate treatment were investigated in aged wild-type (WT) and A(2B)(-/-) mice. One hour after injection with nitrate (0.1 mmol/kg, i.p.) or placebo, metabolic regulation was evaluated by intraperitoneal glucose and insulin tolerance tests. NADPH oxidase-mediated superoxide production and AMPK phosphorylation were measured in livers obtained from non-treated or glucose-treated mice, with or without prior nitrate injection. Plasma was used to determine insulin resistance (HOMA-IR) and NO signaling. Results: A(2B)(-/-) displayed increased body weight, reduced glucose clearance, and attenuated overall insulin responses compared with age-matched WT mice. Nitrate treatment increased circulating levels of nitrate, nitrite and cGMP in the A(2B)(-/-), and improved glucose clearance. In WT mice, however, nitrate treatment did not influence glucose clearance. HOMA-IR increased following glucose injection in the A(2B)(-/-), but remained at basal levels in mice pretreated with nitrate. NADPH oxidase activity in livers from A(2B)(-/-), but not WT mice, was reduced by nitrate treatment. Livers from A(2B)(-/-) displayed reduced AMPK phosphorylation compared with WT mice, and this was increased by nitrate treatment. Finally, injection with the anti-diabetic agent metformin induced similar therapeutic effects in the A(2B)(-/-) as observed with nitrate. Conclusion: The A(2B)(-/-) mouse is a genetic mouse model of metabolic syndrome. Acute treatment with nitrate improved the metabolic profile in it, at least partly via reduction in oxidative stress and improved AMPK signaling in the liver.

    • Projekt ”Långtidsfrisk i Värmland – ett vardagsbaserat hälsoarbete” har bedrivits i samarbete mellan LO, Försäkringskassan i Värmland och Karlstads universitet och har finansierats av Regeringen och försäkringskassan i Värmland. Projektet pågick från våren 2002 t.o.m. sommaren 2004. Forskningscirklar på åtta arbetsplatser inom offentlig sektor har kartlagt hälsovillkoren för de anställda, och med det som utgångspunkt formulerat förslag till åtgärder på arbetsplatsen för att främja de anställdas hälsa. Projektet har således haft dubbla syften, dels ett handlingsinriktat inåt de deltagande organisationerna och dels ett forskningsinriktat i syfte att ta fram mer generell kunskap. En utgångspunkt har varit att ”hela livet” påverkar en individs hälsa. Teoretiskt har krav-kontroll-modellen (Karasek & Theorell) använts och utvecklats i en ambition att förutom arbetslivet studera hem och familjeliv, fritidsliv och samhällsliv i övrigt. Höga sammanlagda krav liksom många förändringar samtidigt i livet är ogynnsamt för hälsan. I rapporten konstateras att ”livet självt” inte längre ges utrymme i arbetslivet, utan kvar är bara arbete. Det framgår vidare att skevheter i arbetsfördelningen mellan könen både på arbetsmarknaden och i hem och familjelivet är faktorer med betydelse för sjukskrivningslängd och rehabiliteringsprocess. Den avslutande diskussionen utmynnar i ett antal förslag till ett friskare arbetsliv.

    • The basal plane resistivity rho of stage 2 SbCl5 intercalated graphite has been measured as a function of temperature T and pressure p in the range 195-300K and 0-1.75 GPa, using a contactless method. rho is found to increase with increasing p. New evidence is found for the existence of the high-pressure (>1 GPa) phase suggested by Houser and co-workers (1984). The p/T phase boundaries for the low T(<230K) and intermediate p (>0.35 GPa) ordered phases are outlined. A re-analysis of previous results for stage 1 SbF5 intercalated graphite indicates a similar phase diagram for this material.

    • The basal plane resistivities of stage 1 SbF5 and stage 2 FeCl3 graphite intercalation compounds have been measured for the first time at high pressure, using a recently developed contactless method. The resistivity is found to increase with increasing pressure in both materials. In the SbF5 intercalated material a phase transition was found at 1.2 GPa. This could be the first pressure-induced stage transformation observed in an acceptor intercalation compound.

    • For the first time, a contactless inductive method has been devised for measurements of the electrical conductivity of metallic materials under very high pressures (>1 GPa). A sample in the form of a thin disc is placed in a pick-up coil in a steady-state AC magnetic field. The output voltage from the coil is analysed with a lock-in amplifier to find the component due to eddy currents in the sample, from which the conductivity is calculated, using a semi-empirical expression. The influence of the steel pressure vessel is eliminated by making simultaneous measurements on a known reference material. Experimental results are shown for tin and sodium up to 1.3 GPa.

    • Objective: A major assumption in cardiovascular medicine is that Q-waves on the electrocardiogram indicate major myocardial tissue damage. The appearance of a new Q-wave has therefore been considered the most reliable criterion for diagnosis of perioperative myocardial infarction (PMI) in cardiac surgery. In a study, originally intended to evaluate troponin-T as a marker of PMI, analysis of our data aroused the need to address the reliability of Q-wave criteria for diagnosis of PMI.Methods: In 302 consecutive patients undergoing coronary surgery, Q-wave and other electrocardiogram (ECG) criteria were compared with biochemical markers of myocardial injury and the postoperative course. All ECGs were analysed by a cardiologist blinded to the biochemical analyses and the clinical course.Results: The incidence of positive Q-wave criteria was 8.1%. Combined biochemical (CK-MB≥70 μg/l) and Q-wave criteria were found in 1.0%. Patients with new Q-waves did not have CK-MB or troponin-T levels significantly different from those without Q-waves. More than 25% of the Q-waves were associated with plasma troponin-T below the reference level (<0.2 μg/l) on the fourth postoperative day. Q-wave criteria alone did not influence the postoperative course. In contrast, biochemical markers correlated with clinical outcome.Conclusions: The majority of Q-waves appearing after coronary surgery were not associated with major myocardial tissue damage, and according to troponin-T one-fourth of the Q-waves were not associated with myocardial necrosis. Furthermore, the appearance of Q-waves had little influence on short term clinical outcome. Therefore, the use of Q-wave criteria as the gold standard for diagnosis of PMI may have to be questioned.
