  • Mathematical Modelling of MASLD ‐ Towards Digital Twins in Liver Disease

    • Unhealthy dieting and a sedentary lifestyle are causing an increased prevalence of obesity related complications. One such complication is metabolic dysfunction-associated steatotic liver disease (MASLD) the manifestation of metabolic dysregulation and insulin resistance in the liver. Today, MASLD effect a third of the world’s population. One of the main characteristics of MASLD is accumulation of ectopic lipids in the liver, also denoted steatosis. Steatosis is not inherently dangerous but is an indication of metabolic dysregulation, and long-term MASLD can progress into severe conditions such as chronic hepatic inflammation denoted metabolic dysfunction-associated steatohepatitis (MASH), liver scarring (cirrhosis), and primary liver cancer (hepatocellular carcinoma, HCC). Moreover, these conditions can be further aggravated by alcohol consumption. The increase in potential patients with MASLD will have an enormous burden on future healthcare. Thus, future healthcare has a need for innovative solutions to lessen this burden. Such solutions should be capable of personalized and preventive measures, cost-effective high throughput screening methods, and frameworks integrating all available patient data, for all stages of MASLD. Today, some of these methodologies already exist, however there is still a need for ways to integrate different liver biomarkers into a user-friendly framework, with strong personalization and predictive capabilities. For this purpose, data-driven mathematical modelling is of use. Data-driven mathematical models has proven useful for such integration in other disease areas such as stroke. In this thesis, I have created and explored several mathematical models aimed at exploring different aspects of MASLD, as well as developed several models using data from example: our own collected magnetic resonance imaging (MRI) data from patients suffering from chronic liver disease or HCC, and pre-clinical mouse data of insulin resistance progression. The studies presented in this thesis investigate diet-driven insulin resistance development, steatosis development and screening, as well as lifestyle interventions for alcohol and dietary habits, and liver function evaluation at late-stage liver disease. Thus, this thesis presents a possible fundament to create a so-called digital twin of MASLD – a highly personalized model capable of making predictions based on lifestyle.

  • Residual stresses, fatigue and deformation in cast iron

    • The complex geometry of cylinder heads in heavy-duty diesel engines makes grey iron or compact graphite iron a preferred material choice due to its price, castability, thermal conductivity and damping capacity. Today’s strict emission laws have increased the demands on engine performance and engine efficiency. This means that material properties such as fatigue resistance need to be improved. Shot peening is often used to improve the fatigue resistance of components and the benefits of shot peening are associated with the induced compressive surface stresses and surface hardening. How different shot peening parameters can affect fatigue strength of grey and compact graphite iron has been investigated within the project underlying this thesis. To do this, X-ray diffraction (XRD) was utilized for residual stress measurements, scanning electron microscopy (SEM) for microstructural characterizations and mechanical fatigue testing for mechanical quantifications. The ultimate aim of this work has been to increase the fatigue resistance of cast iron by residual stress optimization.XRD measurements and SEM examinations revealed that the shot peening parameters shot size and peening intensity significantly influence residual stresses and surface deformation. Residual stress profiles, similar to the one general considered to improve the fatigue strength in steels, were obtained for both grey and compact graphite iron. Uniaxial push-pull fatigue testing on grey iron with these shot peening parameters reduced the fatigue strength with 15–20 %. The negative effect is likely related to surface damage associated with over peening and relatively high subsurface tensile residual stresses. With very gentle shot peening parameters, the uniaxial fatigue strength were unaltered from the base material but when subjected to bending fatigue an increase in fatigue strength were observed. An alternative way to increase the fatigue strength was to conduct a 30 min annealing heat treatment at 285 XC which increased the fatigue strength by almost 10 % in uniaxial loading. The improvement could be an effect of favourable precipitates forming during the annealing, which could hinder dislocation movement during fatigue.Measuring residual stresses using XRD and the sin2 -method demands accurate X-ray elastic constants (XEC) for meticulous stress analysis. The XEC referred to as 1~2s2 should therefore always be calibrated for the specific material used. The experiments conducted revealed that the XEC value is independent of the testing method used in this work. A small correction from the theoretical value should be applied when the material contains small amounts of residual stresses. The amount of residual stresses has a great impact on the XEC and thus on the stress analysis. Concluding that proper analysis of residual stresses in cast iron is not straight forward.

  • Walking the Line : Female Wedding Ngoma in Zanzibar

    • This thesis examines the music of two female ngoma, or music-dance events, that are associated with weddings in Zanzibar: unyago and msanja. Zanzibar is a Muslim island, and there are distinct social spheres for men and women, each with their own norms and expectations with regards to speech, dress, and behavior. Unyago and msanja songs not only represent these norms and expectations; they also negotiate and contest them.Based on the concept of heterotopia by Michel Foucault, this thesis understands music as an Other space, a space which is both connected to Zanzibari society, as well as existing separately in it and from it. While music as such is not a physical, actual site, it is inextricably tied to, and informs our sense of, space. Moreover, music is able to transform a space into one where different rules apply. Drawing on audio and video recordings made in Zanzibar as well as interviews and informal conversations, this study shows how an engagement with the music, in addition to lyrics and social context, can lead to a more profound understanding of how Zanzibari women play with, and challenge, societal expectations. The performance of, and discourse on, female wedding ngoma allow women to ‘walk the line’ towards knowledge about gendered expectations, and towards the ability to contest existing power relations at the same time.

  • Non-Alcoholic Fatty Liver Disease : Aspects on Diagnosis and Long-term Prognosis

    • Non-alcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease affecting approximately 25% of the global population and is commonly recognized as the hepatic manifestation of the metabolic syndrome. The histological spectrum of NAFLD ranges from isolated steatosis to non-alcoholic steatohepatitis (NASH), with risk of developing fibrosis and subsequent cirrhosis and hepatocellular carcinoma. The gold standard for diagnosing NAFLD is liver biopsy. However, because of its invasive nature, several non-invasive methods have been developed and validated in evaluating fat and fibrosis in patients with NAFLD.Liver fat content can be assessed using various methods. The conventional histopathological method consists of a visual semiquantitative approach in which the pathologist uses a four-point scale: grade 0 corresponds to fat deposition in <5% of hepatocytes and grade 1−3 (which is needed for the diagnosis of NAFLD) corresponds to ≥5%. An alternate approach is to quantitatively assess steatosis using stereological point counting (SPC) – which rely on liver biopsy. However, in vivo proton magnetic resonance spectroscopy (1H-MRS) is a reliable noninvasive method that can be used to quantitatively assess total hepatic lipid content, or proton density fat fraction (PDFF).In Paper I we compared the conventional semiquantitative histological method (grade 0-3) with SPC and 1H-MRS. We found a strong positive correlation between 1H-MRS and SPC, whereas the correlations between 1H-MRS or SPC and histopathological grading were substantially weaker. Using the widely used cut-off value of PDFF ≥5%, all participants were found to have steatosis (specificity 100%, sensitivity 53%). Reducing the cut-off value to 3% maintained 100% specificity while increasing sensitivity to 79%.In Paper IV we evaluated quantitative steatosis, by SPC, in 106 biopsy-proven NAFLD patients during a 20-year follow-up. SPC was independently associated with an increased risk of all-cause mortality and development of T2DM. Moreover, in the 59 patients with sequential biopsies (approximately 10 years apart), a reduction of quantitative hepatic steatosis decreased the all-time risk of developing T2DM.NASH is commonly seen as a histological feature portending a worse prognosis in NAFLD. Interestingly, no dual biopsy study has ever shown that NASH predicts fibrosis progression. Yet, NASH is seen as a surrogate marker in pharmaceutical trials – were resolution in NASH is equivalent to future resolution of fibrosis.In Paper II we conducted a long-term follow-up study (20 years) in a large cohort of biopsy-proven NAFLD patients (n=646), in a collaboration with Karolinska Institute. We could not ascertain that NASH had any effect on all-cause, or disease-specific mortality. However, higher stages of fibrosis predicted all-cause and disease specific mortality. In Paper III, we present 129 biopsy-proven NAFLD patients, in which we had prospective, longitudinal data. They were included between 1988 and 1993. All patients alive, were re-invited 2003-2005 and 2013-2015. Dual biopsies were present in 68 patients, and three consecutive biopsies were available in 33 patients. Results showed that NAFLD is a highly heterogeneous disease, with 9.3% developing end-stage liver disease and 16% progressing to advanced stages of fibrosis without any clinically significant baseline data predicting disease progression. In summary, when using 1H-MRS as a diagnostic method for NAFLD, the diagnostic cut-off should be reduced from 5% to 3%. Furthermore, quantitative amount of hepatic steatosis could be used to stratify patients with NAFLD related to future risk of developing T2DM. Moreover, we have shown that NASH does not predict future all-cause or disease-specific mortality nor end-stage liver disease, therefore a different surrogate marker should be used in clinical trials when assessing NAFLD improvement, so to not imbue false reliance in new therapies. Lastly, we have shown that NAFLD has a more dismal prognosis than previously reported, and that it is unexpectedly difficult to predict fibrosis progression in individual NAFLD patients, emphasizing the need for robust non-invasive biomarkers suitable to monitor large number of patients.

  • Residual Stresses and Fatigue of Shot Peened Cast Iron

    • The complex geometry of cylinder head in heavy-duty diesel engine makes grey cast iron or compact graphite iron a perfect material choice due to its castability, thermal conductivity and damping capacity. To increase the efficiency of the engine, the fatigue property of the material needs to be improved. Shot peening is often used to increase the fatigue strength of components. The benefits are associated with the compressive stresses induced and with surface hardening. In this research project, these effects on grey and compact iron have been analyzed for different shot peening parameters using XRD, SEM and fatigue testing methods. The ultimate aim of the project is to increase the fatigue strength of cast irons by optimization of residual stresses.The XRD measurements and SEM examinations revealed that the shot peening parameters including shot size and peening intensity had significant influences on the resulted residual stresses and strain hardening while changing the coverage made little difference. Also differences in the peening results between the two materials were observed, which were ascribed to an effect of the different graphite morphology. Nevertheless, a residual stress profile similar to the one general considered to improve the fatigue strength in steels could be obtained in both grey and compact iron after shot peening.The axial fatigue testing with R=-1 on the grey iron showed that peening using large shot size and high peening intensity (heavy shot peening) resulted in a fatigue strength reduction of 15-20% in comparison with the mechanically polished surface. The negative effects are likely related to surface damage and relatively high tensile residual stresses in subsurface induced by the heavy peening. Grey cast iron has low ductility in tension and therefore tensile residual stresses may promote multiple cracking and crack networking during cyclic loading.Shot peening using much smaller shots and lower intensity (gentle shot peening) which resulted in a much smaller residual stress field gave no significant changes in fatigue strength. However, a short time annealing at 285°C of specimens being gently shot peened increased the fatigue strength roughly by 10%. The improvement could be an effect of precipitates formed due to the heat treatment, which lock the dislocation movement under cyclic loading.

  • Functional Magnetic Resonance Imaging for Clinical Diagnosis : Exploring and Improving the Examination Chain

    • Functional Magnetic Resonance Imaging (fMRI) is a relatively new imaging technique, first reported in 1992, which enables mapping of brain functions with high spatial resolution. Functionally active areas are distinguished by a small signal increase mediated by changes in local blood oxygenation in response to neural activity. The ability to non-invasively map brain function and the large number of MRI scanners quickly made the method very popular, and fMRI have had a huge impact on the study of brain function, both in healthy and diseased subjects.The most common clinical application of fMRI is pre-surgical mapping of brain functions in order to optimise surgical interventions. The clinical fMRI examination procedure can be divided into four integrated parts: (1) patient preparation, (2) image acquisition, (3) image analysis and (4) clinical decision. In this thesis, important aspects of all parts of the fMRI examination procedure are explored with the aim to provide recommendations and methods for prosperous clinical usage of the technique.The most important results of the thesis were: (I) administration of low doses of diazepam to reduce anxiety did not invalidate fMRI mapping results of primary motor and language areas, (II) the choice of visual stimuli equipment can have severe impact on the mapping of visual areas, (III) three-dimensional fMRI imaging sequences did not perform better than two-dimensional imaging sequences, (IV) adaptive spatial filtering can improve the fMRI data analysis, (V) clinical decisions should not be based on activation results from a single statistical threshold.

    • Psalmer är perenna skapelser. Alla psalmer som sjungs idag har en gång först skrivits och komponerats. Därefter har de hållits vid liv, redigerats, översatts, omarbetats, restaurerats, utvalts och återaktualiserats. Titeln på denna årsbok: ”Arbete med psalm” syftar främst på denna senare typ av trädgårdsmästarliknande arbete. Utan psalmboksredaktörer, översättare och bearbetare av psalmer hade vi stått utan de psalmer vi har idag. Vi hade utanför rent akademiska studier möjligen stått helt utan arvet av äldre psalmer. Sådant arbete har nämligen både syftat och lett till bredare relevans och aktualitet i tider av förändring.Årsbokens sju artiklar belyser på olika sätt hur arbete med psalm lett fram till vår nuvarande situation. Här finns också bidrag som kastar ljus över hur psalmerna kan förstås idag och hur deras morgondag kan komma att te sig. Kommer det att finnas en psalmbok även i framtiden? I speciellt fokus står för flera av texterna den i flera samfund formellt gällande och dagligen sjungna Den svenska psalmboken av 1986. Dess ekumeniskt tillkomna första del var och är gemensam för en rad svenska kyrkosamfund och har också på olika sätt blivit en sångbok även långt utanför de kyrkliga sfärerna. Sist i årsboken följer ett tjugotal recensioner av nyutkommen litteratur inom liturgik, kyrkokonst, kyrkomusik och homiletik.

  • A multi-scale digital twin for adiposity-driven insulin resistance in humans : diet and drug effects

    • BACKGROUND: The increased prevalence of insulin resistance is one of the major health risks in society today. Insulin resistance involves both short-term dynamics, such as altered meal responses, and long-term dynamics, such as the development of type 2 diabetes. Insulin resistance also occurs on different physiological levels, ranging from disease phenotypes to organ-organ communication and intracellular signaling. To better understand the progression of insulin resistance, an analysis method is needed that can combine different timescales and physiological levels. One such method is digital twins, consisting of combined mechanistic mathematical models. We have previously developed a model for short-term glucose homeostasis and intracellular insulin signaling, and there exist long-term weight regulation models. Herein, we combine these models into a first interconnected digital twin for the progression of insulin resistance in humans.METHODS: The model is based on ordinary differential equations representing biochemical and physiological processes, in which unknown parameters were fitted to data using a MATLAB toolbox. RESULTS: The interconnected twin correctly predicts independent data from a weight increase study, both for weight-changes, fasting plasma insulin and glucose levels, and intracellular insulin signaling. Similarly, the model can predict independent weight-change data in a weight loss study with the weight loss drug topiramate. The model can also predict non-measured variables.CONCLUSIONS: The model presented herein constitutes the basis for a new digital twin technology, which in the future could be used to aid medical pedagogy and increase motivation and compliance and thus aid in the prevention and treatment of insulin resistance.

    • This study examined how teachers relate literacy teaching to their ordinary subject teaching in professional development settings. The study is conducted within the large Swedish professional development program the Literacy Boost (in Swedish “Läslyftet”), which can be viewed as an example of an international focus on reading ability beyond early and beginning reading. Such a focus may be well-grounded, but it also raises concerns of how teachers of different school subjects are addressed in such programs. The findings of this study show that participating teachers express four approaches of relating literacy teaching to subject teaching, indicating different types and degrees of coherence between professional development content and teachers’ teaching practice. While all teaching activities described by teachers aimed at improving students’ general literacy, this was mostly done in the form of additional activities rather than being embedded in the ordinary teaching concerning curricular objectives of school subjects. These results suggest that differences in how texts are used and interpreted in different school subjects should be given higher priority in the design of both content and form of professional development programs to better support subject teaching.

    • Throughout his long life span Marcus Meibom (c.1630–c.1710) was active as philologist, historian, librarian, music theorist and mathematician, but is today chiefly remembered for his writings concerning ancient Greek sources and artefacts. Meibom’s controversial personality caused great academic and personal conflicts, but appears also to have been an important factor in his scholarly preferences, his methods and the results therefrom. Through his residence at institutions throughout Europe (in present Sweden, Denmark, Holland and England) he has left his mark in many disciplines, which in turn enable scholars from a diversity of branches in the humanities to convene at this symposium, marking the tercentennial commemoration of Meibom’s death. Alongside the interdisciplinary exchange between specialised scholars that is the main purpose of the symposium, it is also hoped that the programme will attract interest from a broader audience with interest in the history of learning and European scholarly culture. The aim is to publish the conference proceedings in order to contribute to a deeper understanding of Meibom and to the particular research topics and contexts to which he contributed. The symposium will be held at the Swedish Royal Library in Stockholm (thus with an indirect connection to Meibom’s service in the Swedish court library). Applications are invited for short papers of 10-15 minutes duration, but the main focus of the conference will be panel discussions concerning different aspects of Meibom, his life, works and contexts. Proposals for papers and round-table sessions should be presented in the form of an abstract of c:a 250 words. Deadline for submitting proposals, which should be addressed to Mattias Lundberg (mattias.lundberg@muslib.se) is 1 November 2009. A full programme and practical details of the symposium will be communicated to participants and made available on-line before November 15 2009.     • Huldrich Zwingli held a highly censorious view of music in private and public worship. It has often been noted that his stance was almost the polar opposite of that of Martin Luther. A more specific disagreement between Zwingli and Luther, however, seems to have gone largely unnoticed in the vast modern literature on music and theology in sixteenth-century reformation movements, namely that of the ontology of music as sounding, sensory object vis-à-vis music as a silently imagined and grasped (ideal) entity. A number of passages from Zwingli’s writings suggest a rather peculiar version of the distinction between body and spirit as regards aural perception in general, and music in particular. The present study scrutinizes what seems to be Zwingli’s arguments against sound and music as sensory elements in conflict with the ideal realm of the highest order of music theory.

    • BACKGROUND: Liver cirrhosis, the advanced stage of many chronic liver diseases, is associated with escalated risks of liver-related complications like decompensation and hepatocellular carcinoma (HCC). Morbidity and mortality in cirrhosis patients are linked to portal hypertension, sarcopenia, and hepatocellular carcinoma. Although conventional cirrhosis management centered on treating complications, contemporary approaches prioritize preemptive measures. This study aims to formulate novel blood- and imaging-centric methodologies for monitoring liver cirrhosis patients.METHODS: In this prospective study, 150 liver cirrhosis patients will be enrolled from three Swedish liver clinics. Their conditions will be assessed through extensive blood-based markers and magnetic resonance imaging (MRI). The MRI protocol encompasses body composition profile with Muscle Assement Score, portal flow assessment, magnet resonance elastography, and a abbreviated MRI for HCC screening. Evaluation of lifestyle, muscular strength, physical performance, body composition, and quality of life will be conducted. Additionally, DNA, serum, and plasma biobanking will facilitate future investigations.DISCUSSION: The anticipated outcomes involve the identification and validation of non-invasive blood- and imaging-oriented biomarkers, enhancing the care paradigm for liver cirrhosis patients.     • The primary aim of this dissertation is to study the nationalistic worldview as it emerged in the German lands in the second half of the eighteenth century . The research focuses on how this influenced the artistic practices of Prussian composer, writer and court Kapellmeister Johann Friedrich Reichardt (1752–1814), and likewise how Reichardt’s activities influenced and formed this field. Reichardt was one of the musicians most actively involved in the development of this field at the time, both as a composer and as a writer. The nationalistic view that a society ought to have as its foundation a common cultural identity, and that this identity should be the guiding principle for organization and government, was groundbreaking at the time. In line with this Reichardt’s ambition involved a transformation of the composer’s/musician’s role in society, turning music into a means to influence and alter the state. As the dissertation shows, this was done in a variety of ways. Above all Reichardt was advocating, and helped shape, a German national cultural identity. This identity he sought to induce in the people, for instance through songs composed specifically for children.The study focuses on a variety of sources, above all a selection of Reichardt’s writings, songs and song collections. These are subjected to a substantial discourse analysis. Following the conviction that a discourse is formed and expressed not just in writing, but also in musical compositions, musical practices, institutions et cetera, the analytical approach vary throughout the study, depending on the researched material. Identified in the analyses are things and signs that point towards a new nationalistic system of meaning.Following a chapter on the historical background, including an analysis of Herderian nationalism, an exposition of the idea of expressive communities, and an account of Reichardt’s biography, the analytical part of the study is divided into two parts, dealing with two different kinds of music and fields of musical application. The first is devoted to music for the community, used in private and semi-private company. Here it is shown how Reichardt contributed to the formulation of a German national musical style founded on folksong ideals and the concept of Volkston. The second is devoted to music for school and education and study the strong interest and belief in education that characterized the eighteenth century, and which Reichardt fully embraced. Here Reichardt’s views on education, and their importance for his activities, are examined.A secondary aim of the study is the theoretical development of the concept expressive communities. This is founded on a central view of Johann Gottfried Herder’s (1744–1803) that acknowledges a natural expressive desire in man. On a community-level this leads to the cultural identity (national or other) being characterized by a particular expressiveness, and being acquired through an active cultural practice. This view is particularly applicable when regarding the late eighteenth century German lands.One result of the study is a better understanding of Reichardt’s activities and works. At the same time, as Reichardt was not the only one involved with the emerging nationalistic ideas, the study serves also as an example and its results are valid on a wider scale, providing a more nuanced understanding of the period on the whole.

    • This dissertation investigates folk hymn singing traditions in Denmark in the nineteenth and early twentieth century, which are rooted in the congregational singing from times of the Reformation. The melodic and vocal style of these traditions are closely connected to the hymnals of Thomas Kingo (1634–1703) and Hans Adolph Brorson (1694–1764). The folk hymn singing mainly thrived in environments of revivalism. The traditions were transmitted until modern times, where five musicologists made audio recordings with the groups of people who could still sing in the old style. The dissertation draws on theory in the intersection of hymnology – with emphasis on the hymnals and the liturgy - and ethnomusicology – with emphasis on music as practice, agency, memory, and experience. In this interdisciplinary field, issues of hymnal-culture and audio recording are investigated. The dissertation explores four cases of Kingo- and Brorson singing, developing a method that combines the diachronic axis of the analysis, which explores processes of tradition and memory, with a synchronic axis that explores the social and religious interaction of participatory singing at a given moment of time. The study especially focuses on the way in which the singers vocally and musically experience and embody the social and confessional community, first of all in terms of the texture of the singing. The dissertation identifies features of unison, of heterophony and of dense textures. The study also investigates how the musical experience can stimulate not only community, but also conflicts in the relationship to others. The dissertation concludes that the study of sounding music can lead to new views on history, and that Danish history of song culture is more multifacetted than is commonly known.
