    • Objectives: To investigate the incidence of hyperthyroidism in Stockholm County in those patients who were diagnosed with hyperthyroidism for the First time during the years 2003-2005. Design: All new cases of hyperthyroidism >= 18 years of age were prospectively registered to calculate the total incidence of hyperthyroidism, as well as the incidence of the subgroups: Graves' disease (GD), toxic multinodular goitre and solitary toxic adenoma (STA). Eight specialized units/hospitals in Stockholm County participated in the registration. The participating physicians were all specialists in medical endocrinology. oncology, nuclear medicine or surgery. Results: Duringa 3-year period, 1431 new patients of hyperthyroidism were diagnosed in a well-defined adult population (>18 years of age) of in average 1 457 036 inhabitants. This corresponds to a mean annual incidence of hyperthyroidism of 32.7/100 000. The incidence of GD was 24.5/100 000 per year. toxic nodular goitre was 3.3/100 000 per year and STA was 4.9/100 000 per year. Conclusions: The total incidence of hyperthyroidism in Stockholm County was found to be 32.7/100 000 per year. of which 75% had GD. There were a higher percentage of smokers among the patients with hyperthyroidism compared with the overall population in Stockholm, but no difference in the frequency of smoking between patients with GD and toxic nodular goitre.

  • Thyroid-Associated Ophthalmopathy after Treatment for Graves´Hyperthyroidism with Antithyroid Drugs or lodine-131

    • Context: Previous randomized trials have suggested an association   between radioiodine treatment for Graves' hyperthyroidism and   thyroid-associated ophthalmopathy (TAO).   Objectives: The aim of the study was to compare the occurrence of   worsening or development of TAO in patients who were treated with   radioiodine or antithyroid drugs.   Design: We conducted a randomized trial (TT 96) with a follow-up of 4   yr.   Patients, Setting, and Intervention: Patients with a recent diagnosis   of Graves' hyperthyroidism were randomized to treatment with iodine-131   (163 patients) or 18 months of medical treatment (150 patients). Early   substitution with T-4 was given in both groups.   Main Outcome Measure: Worsening or development of TAO was significantly   more common in the iodine-131 treatment group (63 patients; 38.7%)   compared with the medical treatment group (32 patients; 21.3%) (P <   0.001).   Results: The risk for de novo development of TAO was greater in   patients treated with iodine-131 (53 patients) than with medical   treatment(23patients). However, worsening of TAO in the 41 patients who   had ophthalmopathy already before the start of treatment was not more   common in the radioiodine group (10 patients) than in the medical group   (nine patients). Smoking was shown to influence the risk of worsening   or development of TAO, and smokers treated with radioiodine had the   overall highest risk for TAO. However, in the group of smokers,   worsening or development of TAO was not significantly associated with   the choice of treatment for hyperthyroidism.   Conclusions: Radioiodine treatment is a significant risk factor for   development of TAO in Graves' hyperthyroidism. Smokers run the highest   risk for worsening or development of TAO irrespective of treatment modality.

    • BACKGROUND: The survival for patients with atrioventricular septal defect has improved markedly over the last decades and, during the same period, the survival of children with Down's syndrome has also increased. The aim of our study was to investigate long-term survival in patients having atrioventricular septal defect with common valvar orifice, but without associated significant congenital heart defects, in the setting of Down's syndrome, comparing the findings to those in chromosomally normal children with the same malformation. METHODS AND RESULTS: In a population-based retrospective study, we scrutinised the medical records from 801 liveborn children with atrioventricular septal defect born in Sweden during the period 1973 through 1997. Data on gender, presence or absence of Down's syndrome, associated congenital heart defects, date of birth, operation and death were recorded and followed up until 2001. An isolated atrioventricular septal defect with common atrioventricular valvar orifice was present in 502 children, of whom 86% had Down's syndrome. We found a significant reduc tion over time in age at operation, and in postoperative mortality at 30 days, from 28 to 1%. Using a multiple logistic regression model, we found no significant differences in mortality between genders, nor between those with or without Down's syndrome. Early corrective surgery could not be identified as a significant independent factor for survival. The 5-year postoperative survival in patients with Down's syndrome increased from 65% over the period from 1973 through 1977, to about 90% in the period 1993 through 1997, and the same trend was observed in chromosomally normal patients. CONCLUSIONS: Survival in uncomplicated atrioventricular septal defect with common atrioventricular valvar orifice has greatly increased, and surgical correction is now equally successful in patients with Down's syndrome and chromosomally normal patients, and for both genders. Death in connection with surgery is no longer the major threat, and focus must now be on long-term follow-up.

  • Mortality in Down's syndrome in relation to congenital malformations

    • Down's syndrome (DS) is the most common form of intellectual disability. The syndrome is characterized by congenital malformations, especially of the heart and gastrointestinal tract, which can result in high mortality rates in the affected population. Many improvements have been made in the medical treatment of this syndrome during the past few decades and the survival of individuals with DS has increased in the industrial world. The aim of the present study was to investigate mortality in relation to congenital malformations. Medical records from all liveborn children with DS delivered between 1973 and 1980 in northern Sweden were studied, and malformations and causes of death were recorded. Out of the 219 children included in the study, a congenital heart defect was reported in 47.5% of subjects, 42.1% of whom had complete atrioventricular septal defect. Gastrointestinal tract malformations were present in 7.3% of subjects, and was frequently associated with a cardiac malformation and a very high mortality rate. Other major and minor congenital anomalies were present in 5.5% and 5.5% of subjects, respectively. In the 14.5-year follow-up of 213 children, the rate of survival was 75.6%. Mortality rates within one and 10 years after birth were 14.6% and 23.5%, respectively. Mortality within 10 years differed significantly between children with (44.1%) and without (4.5%) a congenital heart defect. A very high mortality rate was observed among children with a congenital heart defect, especially when it was combined with a gastrointestinal malformation.

    • Several studies have indicated that early pet keeping could protect the infant from later allergy development. Here, we investigate if there is a dose-dependent association between cat- and dog-keeping during the first year of life and subsequent allergy development.Two cohorts were investigated: a cross-sectional questionnaire-based study of 7- to 8-year-old children (N = 1029) from Mölndal and Kiruna, and a birth-cohort of children from the Västra Götaland county clinically evaluated for asthma and allergy by paediatricians up to the age of 8-9 years (N = 249). The cross-sectional study asked validated questions on asthma and allergy that had been used in two previous studies of children from the same areas. In the birth-cohort study, a diagnosis of asthma and allergy was based on predefined clinical criteria, and laboratory evaluation included blood eosinophils, skin-prick tests and specific immunoglobulin E analyses. Information on pets during first year of life was collected retrospectively in the Cross-Sectional Cohort and prospectively in the Birth Cohort.A dose-response association was seen, with less allergic manifestations (any of asthma, allergic rhinoconjunctivitis, or eczema) with increasing number of household cats and dogs during the first year of life. In the Cross-Sectional Cohort, allergy ever decreased from 49% in those with no pets to zero in those with five or more pets (P-value for trend 0.038), and from 32% to zero for allergy last year (P-value for trend 0.006). The same pattern was seen in Birth Cohort. Sensitization to animals, as well as pollens, also decreased with increasing number of animals in the household.The prevalence of allergic disease in children aged 7-9 years is reduced in a dose-dependent fashion with the number of household pets living with the child during their first year of life, suggesting a "mini-farm" effect, whereby cats and dogs protect against allergy development.

    • In this article, we quantitatively establish the centrality and importance of interviews in news and current affairs broadcasts. We show how segments of interviews (from soundbites to longer recorded or live question-and-answer interactions) are deployed as communicative resources in the construction and presentation of news in various ways. The data allows for a cross-national comparison in between the UK and Sweden that point to differences in practice between the  countries. We argue that our findings may be used to critically examine various conceptualisations of broadcast interviews in general and political interviews in particular. We also show how journalists outnumber politicians as interviewees in the news, a finding that is in need of further exploration from a range of perspectives We also believe that our study provides solid ground on which to base future critical studies of the authority of journalism, dialogical and soundbite journalism, and the alleged fragmentisation of news.

    • New technologies offer new interactional possibilities for news journalism, but they also pose a challenge to broadcasters who are accustomed to the practices of ‘old’ television news. The web is one such arena where broadcasters are in the process of mastering a sense of sociability (Scannell 1996, 2010) and ‘communicative ease’ (cf. Hutchby, 2006) in relation to audiences. They struggle to find ways to engage audiences in the roles of both viewers and users in line with the technological affordances of the web. Rather little attention has yet been paid to how the general sociability of broadcasting is influenced by the development of digital media. This study presents a case of how broadcasters orient to their audience(s) in a so-called live news co-production on the web. The main point is to highlight both possibilities and dilemmas in the management of audience-oriented activities on a new technological platform with its different conditions for production and reception. We argue that broadcasters interested in producing web news both need to adhere to the professional principles and standards of ordinary broadcasting, and at the same time show that they are competent enough to also produce unpolished, layman-like material normally associated with unprofessionality.
