    • Background: There is tremendous potential for genome sequencing to improve clinical diagnosis and care once it becomes routinely accessible, but this will require formalizing research methods into clinical best practices in the areas of sequence data generation, analysis, interpretation and reporting. The CLARITY Challenge was designed to spur convergence in methods for diagnosing genetic disease starting from clinical case history and genome sequencing data. DNA samples were obtained from three families with heritable genetic disorders and genomic sequence data were donated by sequencing platform vendors. The challenge was to analyze and interpret these data with the goals of identifying disease-causing variants and reporting the findings in a clinically useful format. Participating contestant groups were solicited broadly, and an independent panel of judges evaluated their performance. Results: A total of 30 international groups were engaged. The entries reveal a general convergence of practices on most elements of the analysis and interpretation process. However, even given this commonality of approach, only two groups identified the consensus candidate variants in all disease cases, demonstrating a need for consistent fine-tuning of the generally accepted methods. There was greater diversity of the final clinical report content and in the patient consenting process, demonstrating that these areas require additional exploration and standardization. Conclusions: The CLARITY Challenge provides a comprehensive assessment of current practices for using genome sequencing to diagnose and report genetic diseases. There is remarkable convergence in bioinformatic techniques, but medical interpretation and reporting are areas that require further development by many groups.

    • Takotsubo cardiomyopathy (TTC) is a distinct clinical entity characterized by the presence of transient left ventricular wall dysfunction without significant culprit obstructive coronary artery disease. Invasive coronary angiography and ventriculography are the 'gold standard' for definitive diagnosis, with an integrated multi-modality imaging approach offering advantages in various clinical scenarios. Echocardiography is a widely available, first-line, non-invasive imaging technique appropriate both in emergency setting to confirm diagnosis, assess for various potential acute complications, and in serial follow-up to track myocardial recovery. Cardiac magnetic resonance (CMR) may be helpful to discriminate TTC from other acute cardiac syndromes with troponin elevation and ventricular dysfunction. Echocardiography, CMR, and nuclear imaging may also provide new insights into possible underlying pathophysiological mechanisms, and myocardial (123)I-metaiodobenzyl-guanidine imaging may have a role for retrospective diagnosis in the subacute phase of late-presenting cases. The potential diagnostic role of coronary computed tomography angiography in the emergency room requires a further study.

    • Cardiovascular (CV) imaging is an important tool in baseline risk assessment and detection of CV disease in oncology patients receiving cardiotoxic cancer therapies. This position statement examines the role of echocardiography, cardiac magnetic resonance, nuclear cardiac imaging and computed tomography in the management of cancer patients. The Imaging and Cardio-Oncology Study Groups of the Heart Failure Association (HFA) of the European Society of Cardiology (ESC) in collaboration with the European Association of Cardiovascular Imaging (EACVI) and the Cardio-Oncology Council of the ESC have evaluated the current evidence for the value of modern CV imaging in the cardio-oncology field. The most relevant echocardiographic parameters, including global longitudinal strain and three-dimensional ejection fraction, are proposed. The protocol for baseline pre-treatment evaluation and specific surveillance algorithms or pathways for anthracycline chemotherapy, HER2-targeted therapies such as trastuzumab, vascular endothelial growth factor tyrosine kinase inhibitors, BCr-Abl tyrosine kinase inhibitors, proteasome inhibitors and immune checkpoint inhibitors are presented. The indications for CV imaging after completion of oncology treatment are considered. The typical consequences of radiation therapy and the possibility of their identification in the long term are also summarized. Special populations are discussed including female survivors planning pregnancy, patients with carcinoid disease, patients with cardiac tumours and patients with right heart failure. Future directions and ongoing CV imaging research in cardio-oncology are discussed.

    • Cardiogenic shock (CS) is a complex multifactorial clinical syndrome with extremely high mortality, developing as a continuum, and progressing from the initial insult (underlying cause) to the subsequent occurrence of organ failure and death. There is a large spectrum of CS presentations resulting from the interaction between an acute cardiac insult and a patients underlying cardiac and overall medical condition. Phenotyping patients with CS may have clinical impact on management because classification would support initiation of appropriate therapies. CS management should consider appropriate organization of the health care services, and therapies must be given to the appropriately selected patients, in a timely manner, whilst avoiding iatrogenic harm. Although several consensus-driven algorithms have been proposed, CS management remains challenging and substantial investments in research and development have not yielded proof of efficacy and safety for most of the therapies tested, and outcome in this condition remains poor. Future studies should consider the identification of the new pathophysiological targets, and high-quality translational research should facilitate incorporation of more targeted interventions in clinical research protocols, aimed to improve individual patient outcomes. Designing outcome clinical trials in CS remains particularly challenging in this critical and very costly scenario in cardiology, but information from these trials is imperiously needed to better inform the guidelines and clinical practice. The goal of this review is to summarize the current knowledge concerning the definition, epidemiology, underlying causes, pathophysiology and management of CS based on important lessons from clinical trials and registries, with a focus on improving in-hospital management.

  • Transforming knowledge systems for life on Earth : Visions of future systems and how to get there

    • Formalised knowledge systems, including universities and research institutes, are important for contemporary societies. They are, however, also arguably failing humanity when their impact is measured against the level of progress being made in stimulating the societal changes needed to address challenges like climate change. In this research we used a novel futures-oriented and participatory approach that asked what future envisioned knowledge systems might need to look like and how we might get there. Findings suggest that envisioned future systems will need to be much more collaborative, open, diverse, egalitarian, and able to work with values and systemic issues. They will also need to go beyond producing knowledge about our world to generating wisdom about how to act within it. To get to envisioned systems we will need to rapidly scale methodological innovations, connect innovators, and creatively accelerate learning about working with intractable challenges. We will also need to create new funding schemes, a global knowledge commons, and challenge deeply held assumptions. To genuinely be a creative force in supporting longevity of human and non-human life on our planet, the shift in knowledge systems will probably need to be at the scale of the enlightenment and speed of the scientific and technological revolution accompanying the second World War. This will require bold and strategic action from governments, scientists, civic society and sustained transformational intent.

    • Takotsubo syndrome was featured in various sessions and posters at the recent Heart Failure Congress and World Congress on Acute Heart Failure 2019 held in Athens. The importance and increasing recognition of this acute heart failure syndrome is reflected by the growing activity at Heart Failure Association congresses over the last 8 years. Two dedicated sessions to Takotsubo syndrome with comprehensive lectures from experts in the field and several posters highlighted new scientific progress, important aspects of epidemiology, pathophysiology, risk stratification, and management of the syndrome and discussed gaps in knowledge of this intriguing entity. This paper will summarize the topics discussed in these sessions including the most recent data from large registries, clinical, and pre‐clinical studies presented at the meeting.

  • Building a coherent hydro-climatic modelling framework for the data limited Kilombero Valley of Tanzania

    • This thesis explores key aspects for synthesizing data across spatiotemporal scales relevant for water resources management in an Eastern Africa context. Specifically, the potential of large scale global precipitation datasets (GPDs) in data limited regions to overcome spatial and temporal data gaps is considered. The thesis also explores the potential to utilize limited and non-continuous streamflow and stream water chemistry observations to increase hydrological process understanding. The information gained is then used to build a coherent hydro-climatic framework for streamflow modelling. In this thesis, Kilombero Valley Drainage Basin (KVDB) in Tanzania is used as an example of a data limited region targeted for rapid development, intensification and expansion of agriculture. As such, it is representative for many regions across the Eastern Africa. With regards to the data synthesis, two satellite products, three reanalysis products and three interpolated products were evaluated based on their spatial and temporal precipitation patterns. Streamflow data from KVDB and eight subcatchments were then assessed for quality with regards to missing data. Furthermore, recession analysis was used to estimate catchment-scale characteristic drainage timescale. Results from these streamflow analyses, in conjunction with a hydrological tracer-based analysis, were then used for improved understanding of streamflow generation in the region. Finally, a coherent modelling framework using the HBV rainfall-runoff model was implemented and evaluated based on daily streamflow simulation. Despite the challenges of data limited regions and the often large uncertainty in results, this thesis demonstrates that improved process understanding could be obtained from limited streamflow records and a focused hydrochemical sampling when experimental design natural variability were leveraged to gain a large  signal to noise ratio. Combining results across all investigations rendered information useful for the conceptualization and implementation of the hydro-climatic modelling framework relevant in Kilombero Valley. For example, when synthesized into a coherent framework the GPDs could be downscaled and used for daily streamflow simulations at the catchment scale with moderate success. This is promising when considering the need for estimating impacts of potential future land use and climate change as well as agricultural intensification.

  • Advancing understanding in data limited conditions : Estimating contributions to streamflow across Tanzania’s rapidly developing Kilombero Valley

    • Large natural variability in, for example, climate signals and experimental design may help to overcome the data limitations and difficult conditions that typify much of the global south. This, in turn, can facilitate the application of advanced techniques to help inform management with science (which is sorely needed for guiding development). As an example on this concept, we used a limited amount of weekly water chemistry as well as stable water isotope data to perform end-member mixing analysis (EMMA) in a generalized likelihood uncertainty estimation (GLUE) framework in a sub-catchment of Kilombero Valley, Tanzania. How water interacts across the various storages in this region, which has been targeted for rapid agricultural intensification and expansion is still largely unknown, making estimation of potential impacts (not to mention sustainability) associated with various development scenarios difficult. Our results showed that there were, as would be expected, considerable uncertainties related to the characterization of end-members in this remote system. Regardless, some robust estimates could be made on contributions to seasonal streamflow variability. For example, it appears that there is a low connectivity between the deep groundwater and the stream system throughout the year. Also, there is a considerable wetting up period required before overland flow occurs. These process insights, in turn, help interpreting hydrochemical data thereby potentially improving understanding at larger scales. Thus, in spite of large uncertainties our results highlight how improved system understanding of hydrological flows can be obtained even when working under less than perfect conditions.

  • Comparing global precipitation data sets in eastern Africa : a case study of Kilombero Valley, Tanzania

    • In the face of limited or no precipitation data, global precipitation data sets (GPDs) may provide a viable alternative to gauge or ground radar data. This study aims to provide guidance to the choice of GPDs targeting scales relevant to water resources management in data poor regions. Specifically, the 34 000 km(2) Kilombero Valley in central Tanzania, where water resource management is seen as integral to poverty reduction and food security, is used as a case study for performance evaluation of seven GPDs and their ensemble mean against the Tropical Rainfall Measuring Mission (TRMM) multi-satellite precipitation analysis research-grade product v7 (TRMMv7). The GPDs include one satellite rainfall product [Climate Prediction Center morphing technique v1.0 CRT (CMORPH)], three reanalysis products [Climate Forecasting System Reanalysis (CFSR), European reanalysis interim (ERA-i) and Modern Era Retrospective-Analysis for Research and Applications (MERRA)] and three interpolated data sets [Climate Research Unit Time Series 3.21 (CRU), Global Precipitation and Climatology Center v6 data set (GPCC) and University of Delaware Air Temperature and Precipitation v3.01 data set (UDEL)]. Standard statistical performance measures and spatial patterns were evaluated for the common overlap time period 1998-2010. For this region, the principal seasonality of the climatology was well represented in all GPDs; however, the intraseasonal variability and the spatial precipitation patterns were less well represented. The ensemble mean and GPCC had the best performance with regard to the analysis of the time series while CMORPH and GPCC had the best performance with regard to the spatial pattern analysis. These results indicate that the spatial scale intended for application is a major factor impacting the suitability of a given GPD for hydrometrological studies that form a basis for development of water management strategies.

    • This study explored the potential for bias correction of global precipitation datasets (GPD) to support streamflow simulation for water resource management in data limited regions. Two catchments, 580 km(2) and 2530 km(2), in the Kilombero Valley of central Tanzania were considered as case studies to explore three GPD bias correction methods: quantile mapping (QM), daily percentages (DP) and a model based (ModB) bias correction. The GPDs considered included two satellite rainfall products, three reanalysis products and three interpolated observed data products. The rainfall-runoff model HBV was used to simulate streamflow in the two catchments using (1) observed rain gauge data; (2) the original GPDs and (3) the bias-corrected GPDs as input. Results showed that applying QM to bias correction based on limited observed data tends to aggravate streamflow simulations relative to not bias correcting GPDs. This is likely due to a potential lack of representativeness of a single rain gauge observation at the scale of a hydrological catchment for these catchments. The results also indicate that there may be potential benefits in combining streamflow and rain gauge data to bias correct GPDs during the model calibration process within a hydrological modeling framework.

  • Statistical downscaling of global precipitation datasets in data limited regions : a case study of Kilombero Valley, Tanzania

    • This study explored the potential for spatial downscaling of Global precipitation datasets (GPD) to improve streamflow simulation in data limited regions. Specifically, we used Kilombero Valley in central Tanzania as an example of an area characterized by rapid large scale agricultural intensification where observational data are limited. Two catchments in Kilombero Valley were considered as case studies to explore the potential of downscaling GPDs based on the three downscaling methods: quantile mapping (QM), daily percentages (DP) and model based bias correction (ModB). The downscaled GPDs were then evaluated based on streamflow simulations. A simple bucket-type runoff model (HBV) was used to simulate streamflow for the two catchments using rain gauge, non-downscaled and downscaled precipitation data as input. Investigated GPDs include two satellite rainfall products, three reanalysis products and three interpolated products. Results showed that applying QM based on limited observed data tends to aggravate streamflow simulations due to a potential lack of representativeness of a rain gauge observation at the scale of a hydrological catchment. ModB improved results for all GPD downscaling and combining QM and ModB improved simulations yet further (but only in some cases). These results indicate that there are benefits in using an integrated approach, for example combining streamflow and rain gauge data to downscale GPDs, in order to bridge the scale mismatch between precipitation data and water management scale.

    • To identify genetic loci influencing central obesity and fat distribution, we performed a meta-analysis of 16 genome-wide association studies (GWAS, N = 38,580) informative for adult waist circumference (WC) and waist-hip ratio (WHR). We selected 26 SNPs for follow-up, for which the evidence of association with measures of central adiposity (WC and/or WHR) was strong and disproportionate to that for overall adiposity or height. Follow-up studies in a maximum of 70,689 individuals identified two loci strongly associated with measures of central adiposity; these map near TFAP2B (WC, P = 1.9x10(-11)) and MSRA (WC, P = 8.9x10(-9)). A third locus, near LYPLAL1, was associated with WHR in women only (P = 2.6x10(-8)). The variants near TFAP2B appear to influence central adiposity through an effect on overall obesity/fat-mass, whereas LYPLAL1 displays a strong female-only association with fat distribution. By focusing on anthropometric measures of central obesity and fat distribution, we have identified three loci implicated in the regulation of human adiposity.

    • Takotsubo syndrome is an acute reversible heart failure syndrome that is increasingly recognized in modern cardiology practice. This Position Statement from the European Society of Cardiology Heart Failure Association provides a comprehensive review of the various clinical and pathophysiological facets of Takotsubo syndrome, including nomenclature, definition, and diagnosis, primary and secondary clinical subtypes, anatomical variants, triggers, epidemiology, pathophysiology, clinical presentation, complications, prognosis, clinical investigations, and treatment approaches. Novel structured approaches to diagnosis, risk stratification, and management are presented, with new algorithms to aid decision-making by practising clinicians. These also cover more complex areas (e.g. uncertain diagnosis and delayed presentation) and the management of complex cases with ongoing symptoms after recovery, recurrent episodes, or spontaneous presentation. The unmet needs and future directions for research in this syndrome are also discussed.

    • Takotsubo syndrome (TTS) has been a recognized clinical entity for 31 years, since its first description in 1990. TTS is now routinely diagnosed in patients who present with acute chest pain, electrocardiographic changes, troponin elevation, unobstructed coronary arteries, and a typical pattern of circumferential left ventricular wall motion abnormalities that usually involve the apical and midventricular myocardium. Increasing understanding of this intriguing syndrome stems from wider recognition, possible increasing frequency, and a rising number of publications focused on the pathophysiology in clinical and laboratory studies. A comprehensive understanding of TTS pathophysiology and evidence-based treatments are lacking, and specific and effective treatments are urgently required. This paper reviews the pathophysiology of this fascinating syndrome; what is known from both clinical and preclinical studies, including review of the evidence for microvascular dysfunction, myocardial beta-adrenergic signaling, inflammation, and electrophysiology; and where focused research needs to fill gaps in understanding TTS.

    • In the UK, television cookery programmes engage the attention and enthusiasm of large audiences. Celebrity chefs are household names whose books nearly always top best-seller lists. Magazines and newspapers routinely include meal recipes and reports on food-related issues. In this sense, the domestic preparation of food has probably never attracted greater public interest. Paradoxically, much is also now said and written about the general loss of practical cooking skills. The latter takes on a special significance especially in relation to adverse changes in UK eating patterns and food safety problems. The paper contributes to an understanding of this paradox by examining what has happened to food preparation skills. The 20th century was a context for massive social and technological changes, and these were reflected in the domestic environment. Among younger cohorts many of traditional food preparation skills have atrophied but to some extent we can view this in terms of changed timescales of acquisition. Also, on the basis of historical evidence, there may be grounds for optimism in a re-evaluation of the extent and diversity of cooking skills in the past.

K values were determined using streamflow recession analysis with a Brutsaert-Nieber solution to the linearized Boussinesq equation. Estimated K values were variable, comparing between wet and dry seasons for the relatively small catchments draining upland positions. For the larger catchments draining through valley bottoms, K values were typically longer and more consistent across seasons. Variations in K were compared with long-term averaged, Moderate-resolution Imaging Spectroradiometer-derived monthly evapotranspiration. Although the variations in K were potentially related to evapotranspiration, the influence of data quality and analysis procedure could not be discounted. As such, even though recession analysis offers a potential approach to explore aquifer release timescales and thereby gain insight to a region's hydrology to inform water resources management, care must be taken when interpreting spatiotemporal shifts in K in connection with process representation in regions like the Kilombero Valley.

    • Background: Takotsubo syndrome (TS) is a potentially life-threatening acute cardiac syndrome with a clinical presentation similar to myocardial infarction and for which the natural history, management, and outcome remain incompletely understood. Our aim was to assess the relative short-term mortality risk of TS, ST-segment-elevation myocardial infarction (STEMI), and non-STEMI (NSTEMI) and to identify predictors of in-hospital complications and poor prognosis in patients with TS.Methods and Results: This is an observational cohort study based on the data from the SCAAR (Swedish Coronary Angiography and Angioplasty Registry). We included all patients (n=117 720) who underwent coronary angiography in Sweden attributed to TS (N=2898 [2.5%]), STEMI (N=48 493 [41.2%]), or NSTEMI (N=66 329 [56.3%]) between January 2009 and February 2018. We compared patients with TS to those with NSTEMI or STEMI. The primary end point was all-cause mortality at 30 days. Secondary outcomes were acute heart failure (Killip Class ≥2) and cardiogenic shock (Killip Class 4) at the time of angiography. Patients with TS were more often women compared with patients with STEMI or NSTEMI. TS was associated with unadjusted and adjusted 30-day mortality risks lower than STEMI (adjusted hazard ratio [adjHR], 0.60; 95% CI, 0.48-0.76; P<0.001), but higher than NSTEMI (adjHR, 2.70; 95% CI, 2.14-3.41; P<0.001). Compared with STEMI, TS was associated with a similar risk of acute heart failure (adjHR, 1.26; 95% CI, 0.91-1.76; P=0.16) but a lower risk of cardiogenic shock (adjHR, 0.55; 95% CI, 0.34-0.89; P=0.02). The relative 30-day mortality risk for TS versus STEMI and NSTEMI was higher for smokers than nonsmokers (adjusted P interaction STEMI=0.01 and P interaction NSTEMI=0.01). Conclusions: The 30-day mortality rate in TS was higher than in NSTEMI but lower than STEMI despite a similar risk of acute heart failure in TS and STEMI. Among patients with TS, smoking was an independent predictor of mortality.

    • Despite improvements in cancer survival, cancer therapy–related cardiovascular toxicity has risen to become a prominent clinical challenge. This has led to the growth of the burgeoning field of cardio-oncology, which aims to advance the cardiovascular health of cancer patients and survivors, through actionable and translatable science. In these Global Cardio-Oncology Symposium 2023 scientific symposium proceedings, we present a focused review on the mechanisms that contribute to common cardiovascular toxicities discussed at this meeting, the ongoing international collaborative efforts to improve patient outcomes, and the bidirectional challenges of translating basic research to clinical care. We acknowledge that there are many additional therapies that are of significance but were not topics of discussion at this symposium. We hope that through this symposium-based review we can highlight the knowledge gaps and clinical priorities to inform the design of future studies that aim to prevent and mitigate cardiovascular disease in cancer patients and survivors.

    • Cardiomyopathies are a heterogeneous group of heart muscle diseases and an important cause of heart failure (HF). Current knowledge on incidence, pathophysiology and natural history of HF in cardiomyopathies is limited, and distinct features of their therapeutic responses have not been systematically addressed. Therefore, this position paper focuses on epidemiology, pathophysiology, natural history and latest developments in treatment of HF in patients with dilated (DCM), hypertrophic (HCM) and restrictive (RCM) cardiomyopathies. In DCM, HF with reduced ejection fraction (HFrEF) has high incidence and prevalence and represents the most frequent cause of death, despite improvements in treatment. In addition, advanced HF in DCM is one of the leading indications for heart transplantation. In HCM, HF with preserved ejection (HFpEF) affects most patients with obstructive, and similar to 10% of patients with non-obstructive HCM. A timely treatment is important, since development of advanced HF, although rare in HCM, portends a poor prognosis. In RCM, HFpEF is common, while HFrEF occurs later and more frequently in amyloidosis or iron overload/haemochromatosis. Irrespective of RCM aetiology, HF is a harbinger of a poor outcome. Recent advances in our understanding of the mechanisms underlying the development of HF in cardiomyopathies have significant implications for therapeutic decision-making. In addition, new aetiology-specific treatment options (e.g. enzyme replacement therapy, transthyretin stabilizers, immunoadsorption, immunotherapy, etc.) have shown a potential to improve outcomes. Still, causative therapies of many cardiomyopathies are lacking, highlighting the need for the development of effective strategies to prevent and treat HF in cardiomyopathies.

    • Heart failure (HF) is common and associated with a poor prognosis, despite advances in treatment. Over the last decade cardiovascular outcome trials with sodium-glucose co-transporter 2 (SGLT2) inhibitors in patients with type 2 diabetes mellitus have demonstrated beneficial effects for three SGLT2 inhibitors (empagliflozin, canagliflozin and dapagliflozin) in reducing hospitalisations for HF. More recently, dapagliflozin reduced the risk of worsening HF or death from cardiovascular causes in patients with chronic HF with reduced left ventricular ejection fraction, with or without type 2 diabetes mellitus. A number of additional trials in HF patients with reduced and/or preserved left ventricular ejection fraction are ongoing and/or about to be reported. The present position paper summarises recent clinical trial evidence and discusses the role of SGLT2 inhibitors in the treatment of HF, pending the results of ongoing trials in different populations of patients with HF.

  • Estimating Aquifer Transmissivity Using the Recession-Curve-Displacement Method in Tanzania's Kilombero Valley

    • Information on aquifer processes and characteristics across scales has long been a cornerstone for understanding water resources. However, point measurements are often limited in extent and representativeness. Techniques that increase the support scale (footprint) of measurements or leverage existing observations in novel ways can thus be useful. In this study, we used a recession-curve-displacement method to estimate regional-scale aquifer transmissivity (T) from streamflow records across the Kilombero Valley of Tanzania. We compare these estimates to local-scale estimates made from pumping tests across the Kilombero Valley. The median T from the pumping tests was 0.18 m(2)/min. This was quite similar to the median T estimated from the recession-curve-displacement method applied during the wet season for the entire basin (0.14 m(2)/min) and for one of the two sub-basins tested (0.16 m(2)/min). On the basis of our findings, there appears to be reasonable potential to inform water resource management and hydrologic model development through streamflow-derived transmissivity estimates, which is promising for data-limited environments facing rapid development, such as the Kilombero Valley.

    • This position paper focusses on the pathophysiology, diagnosis and management of women diagnosed with a cardiomyopathy, or at risk of heart failure (HF), who are planning to conceive or present with (de novo or previously unknown) HF during or after pregnancy. This includes the heterogenous group of heart muscle diseases such as hypertrophic, dilated, arrhythmogenic right ventricular and non-classified cardiomyopathies, left ventricular non-compaction, peripartum cardiomyopathy, Takotsubo syndrome, adult congenital heart disease with HF, and patients with right HF. Also, patients with a history of chemo-/radiotherapy for cancer or hematological malignancies need specific pre-, during and post-pregnancy assessment and counseling. We summarize the current knowledge about pathophysiological mechanisms, including gene mutations, clinical presentation, diagnosis, and medical and device management, as well as risk stratification. Women with a known diagnosis of a cardiomyopathy will often require continuation of drug therapy, which has the potential to exert negative effects on the fetus. This position paper assists in balancing benefits and detrimental effects.

    • We have simulated the magnetic Bragg scattering in transmission electron microscopy in two antiferromagnetic compounds, NiO and LaMnAsO. This weak magnetic phenomenon was experimentally observed in NiO by Loudon (2012). We have computationally reproduced Loudon's experimental data, and for comparison we have performed calculations for the LaMnAsO compound as a more challenging case, containing lower concentration of magnetic elements and strongly scattering heavier non-magnetic elements. We have also described thickness and voltage dependence of the intensity of the antiferromagnetic Bragg spot for both compounds. We have considered lattice vibrations within two computational approaches, one assuming a static lattice with Debye-Waller smeared potentials, and another explicitly considering the atomic vibrations within the quantum excitations of phonons model (thermal diffuse scattering). The structural analysis shows that the antiferromagnetic Bragg spot appears in between (111) and (000) reflections for NiO, while for LaMnAsO the antiferromagnetic Bragg spot appears at the position of the (010) reflection in the diffraction pattern, which corresponds to a forbidden reflection of the crystal structure. Calculations predict that the intensity of the magnetic Bragg spot in NiO is significantly stronger than thermal diffuse scattering at room temperature. For LaMnAsO, the magnetic Bragg spot is weaker than the room-temperature thermal diffuse scattering, but its detection can be facilitated at reduced temperatures.

    • We have modeled the magnetic Bragg scattering in two antiferromagnetic materials, NiO and LaMnAsO, using transmission electron microscopy. Experimentally, Loudon detected these weak magnetic phenomena in NiO. As a more difficult situation with a lower concentration of magnetic elements and higher concentration of heavier non-magnetic elements that significantly scatter, we did computations for the LaMnAsO compound in order to compare our computational replication of Loudon's experimental data. Additionally, we have discussed the antiferromagnetic Bragg spot's thickness and voltage dependency for both compounds. We used two computational methods, one assuming a static lattice with smeared Debye-Waller potentials and the other explicitly taking into account the atomic vibrations within the quantum excitations of phonons model (thermal diffuse scattering). According to the structural study, the antiferromagnetic Bragg spot in NiO is located between the (111) and (000) reflections. However, in LaMnAsO, it is located at the site of the (110) reflection in the diffraction pattern, which is a forbidden reflection of the crystal structure. According to calculations, the magnetic Bragg spot in NiO has an intensity that is much greater than thermal diffuse scattering at room temperature. The magnetic Bragg spot for LaMnAsO is weaker than the thermal diffuse scattering at room temperature, but its identification can be made easier at lower temperatures.

    • Takotsubo syndrome is a well-characterized cause of acute yet reversible heart failure associated with periods of intense emotional stress, often mimicking on presentation an acute coronary syndrome. Animal models of Takotsubo syndrome have been developed, either through the application of a stressor, or administration of exogenous catecholamine. We found that in a model of isoproterenol-induced Takotsubo syndrome in anesthetized rats hyperthermia (40-41°C) would occur after the administration of isoproterenol. Maintenance of this hyperthermia would result in an apical hypocontractility typical of the syndrome, whereas prevention of hyperthermia with active cooling to maintain a euthermic core body temperature prevented (but did not subsequently reverse) apical hypocontractility. In vitro experimentation with isolated cardiomyocytes showed no effect of hyperthermia on either baseline contractility or contractility change after beta-adrenoceptor stimulation. We suggest that the rise in body temperature that is characteristic of catecholamine storm may be a component in the development of Takotsubo syndrome.
