    • CONTEXT: Bone marrow (BM) in adult long bones is rich in adipose tissue, but the functions of BM adipocytes are largely unknown. We set out to elucidate the metabolic and molecular characteristics of BM adipose tissue (BMAT) in humans. OBJECTIVE: Our aim was to determine if BMAT is an insulin-sensitive tissue, and whether the insulin sensitivity is altered in obesity or type 2 diabetes (T2DM). DESIGN: This was a cross-sectional and longitudinal study. SETTING: The study was conducted in a clinical research center. PATIENTS OR OTHER PARTICIPANTS: Bone marrow adipose tissue glucose uptake (GU) was assessed in 23 morbidly obese subjects (9 with T2DM) and 9 healthy controls with normal body weight. In addition, GU was assessed in another 11 controls during cold exposure. Bone marrow adipose tissue samples for molecular analyses were collected from non-DM patients undergoing knee arthroplasty. INTERVENTION(S): Obese subjects were assessed before and 6 months after bariatric surgery and controls at 1 time point. MAIN OUTCOME MEASURE: We used positron emission tomography imaging with 2-[18F]fluoro-2-deoxy-D-glucose tracer to characterize GU in femoral and vertebral BMAT. Bone marrow adipose tissue molecular profile was assessed using quantitative RT-PCR. RESULTS: Insulin enhances GU in human BMAT. Femoral BMAT insulin sensitivity was impaired in obese patients with T2DM compared to controls, but it improved after bariatric surgery. Furthermore, gene expression analysis revealed that BMAT was distinct from brown and white adipose tissue. CONCLUSIONS: Bone marrow adipose tissue is a metabolically active, insulin-sensitive and molecularly distinct fat depot that may play a role in whole body energy metabolism. © Endocrine Society 2020.

    • Asthma is the most common chronic lower respiratory disease in childhood throughout the world. Several guidelines and/or consensus documents are available to support medical decisions on pediatric asthma. Although there is no doubt that the use of common systematic approaches for management can considerably improve outcomes, dissemination and implementation of these are still major challenges. Consequently, the International Collaboration in Asthma, Allergy and Immunology (iCAALL), recently formed by the EAACI, AAAAI, ACAAI, and WAO, has decided to propose an International Consensus on (ICON) Pediatric Asthma. The purpose of this document is to highlight the key messages that are common to many of the existing guidelines, while critically reviewing and commenting on any differences, thus providing a concise reference. The principles of pediatric asthma management are generally accepted. Overall, the treatment goal is disease control. To achieve this, patients and their parents should be educated to optimally manage the disease, in collaboration with healthcare professionals. Identification and avoidance of triggers is also of significant importance. Assessment and monitoring should be performed regularly to re-evaluate and fine-tune treatment. Pharmacotherapy is the cornerstone of treatment. The optimal use of medication can, in most cases, help patients control symptoms and reduce the risk for future morbidity. The management of exacerbations is a major consideration, independent of chronic treatment. There is a trend toward considering phenotype-specific treatment choices; however, this goal has not yet been achieved.

  • Local variation in Bi crystal sites of epitaxial GaAsBi studied by photoelectron spectroscopy and first-principles calculations

    • Epitaxial Bi-containing III–V crystals (III-V1-xBix) have attracted increasing interest due to their potential in infrared applications. Atomic-scale characterization and engineering of bulk-like III-V1-xBix properties (e.g., Bi incorporation and defect formation) are challenging but relevant to develop applications. Toward that target, we report here that the traditional surface-science measurement of photoelectron spectroscopy (PES) is a potential, non-destructive method to be combined in the studies of bulk-like properties, when surface effects are properly removed. We have investigated epitaxial GaAs1-xBix films, capped by epitaxial AlAs layers, with high-resolution photoelectron spectroscopy. The Bi5d core-level spectra of GaAs1-xBix together with ab-initio calculations give direct evidence of variation of Bi bonding environment in the lattice sites. The result agrees with photoluminescence (PL) measurement which shows that the studied GaAs1-xBix films include local areas with higher Bi content, which contribute to PL but do not readily appear in x-ray diffraction (XRD). The measured and calculated Bi core-level shifts show also that Ga vacancies and Bi clusters are dominant defects.

ABSTRACT: A major part of the burden of asthma is caused by acute exacerbations. Exacerbations have been strongly and consistently associated with respiratory infections. Respiratory viruses and bacteria are therefore possible treatment targets. To have a reasonable estimate of the burden of disease induced by such infectious agents on asthmatic patients, it is necessary to understand their nature and be able to identify them in clinical samples by employing accurate and sensitive methodologies. This systematic review summarizes current knowledge and developments in infection epidemiology of acute asthma in children and adults, describing the known impact for each individual agent and highlighting knowledge gaps. Among infectious agents, human rhinoviruses are the most prevalent in regard to asthma exacerbations. The newly identified type-C rhinoviruses may prove to be particularly relevant. Respiratory syncytial virus and metapneumovirus are important in infants, while influenza viruses seem to induce severe exacerbations mostly in adults. Other agents are relatively less or not clearly associated. Mycoplasma and Chlamydophila pneumoniae seem to be involved more with asthma persistence rather than with disease exacerbations. Recent data suggest that common bacteria may also be involved, but this should be confirmed. Although current information is considerable, improvements in detection methodologies, as well as the wide variation in respect to location, time and populations, underline the need for additional studies that should also take into account interacting factors.

    • The lead isotopic compositions of galena in Early Proterozoic gold deposits have been determined for three districts in northern Sweden and central Finland. The deposits are hosted by a variety of 1870-1890 Ma Svecofennian host rocks including the volcanosedimentary succession within the Skellefte District island arc in Sweden as well as I-type tonalites at Jörn (Sweden) and Pohjanmaa (Finland). The deposits are epigenetic in relation to these Svecofennian rocks and are part of a goldbearing metallogenetic belt, which can be followed for 600 km parallel to the southwestern margin of the Archaean Domain. In spite of these epigenetic relationships, the lead isotopic data indicate that the deposits are not dramatically younger than the 1870-1890 Ma Svecofennian host rocks (probably not exceeding 10-20 million years). Two principal lead sources were activated when the gold deposits were formed. The most significant source is represented by the I-type tonalites, which constitute a relatively primitive ( = 9.3) and widely distributed source in the entire metallogenic belt. In addition, the volcanic components in the westernmost part of the Skellefte District constitute an extremely primitive ( <9.0) source, which only locally was an important contributor to the epigenetic deposits in this metallogenetic belt. The significantly different lead isotopic composition estimated for these sources indicates that the volcanic rocks in the western part of the Skellefte District were not comagmatic with the I-type tonalites recognized at Jörn and central Finland.

    • Contact with natural environments enriches the human microbiome, promotes immune balance and protects against allergies and inflammatory disorders. In Finland, the allergy &amp; asthma epidemic became slowly visible in mid 1960s. After the World War II, Karelia was split into Finnish and Soviet Union (now Russia) territories. This led to more marked environmental and lifestyle changes in the Finnish compared with Russian Karelia.The Karelia Allergy Study 2002–2022showed that allergic conditions were much more common on the Finnish side. The Russians had richer gene-microbe network and interaction than the Finns, which associated with better balanced immune regulatory circuits and lower allergy prevalence. In the Finnish adolescents, a biodiverse natural environment around the homes associated with lower occurrence of allergies. Overall, the plausible explanation of the allergy disparity was the prominent change in environment and lifestyle in the Finnish Karelia from 1940s to 1980s. The nationwideFinnish Allergy Programme 2008–2018implemented the biodiversity hypothesis into practice by endorsing immune tolerance, nature contacts, and allergy health with favorable results. A regional health and environment programme,Nature Step to Health 2022–2032, has been initiated in the City of Lahti, EU Green Capital 2021. The programme integrates prevention of chronic diseases (asthma, diabetes, obesity, depression), nature loss, and climate crisis in the spirit ofPlanetary Health. Allergic diseases exemplify inappropriate immunological responses to natural environment. Successful management of the epidemics of allergy and other non-communicable diseases may pave the way to improve human and environmental health.

    • We have used near-infrared spectroscopy (NIRS) to study hemodynamic auditory evoked responses on 7 full-term neonates. Measurements were done simultaneously above both auditory cortices to study the distribution of speech and music processing between hemispheres using a 16-channel frequency-domain instrument. The stimulation consisted of 5-second samples of music and speech with a 25-second silent interval. In response to stimulation, a significant increase in the concentration of oxygenated hemoglobin ([HbO2]) was detected in 6 out of 7 subjects. The strongest responses in [HbO2] were seen near the measurement location above the ear on both hemispheres. The mean latency of the maximum responses was 9.42±1.51 s. On the left hemisphere (LH), the maximum amplitude of the average [HbO2] response to the music stimuli was 0.76± 0.38 μ M (mean±std.) and to the speech stimuli 1.00± 0.45 μ± μM. On the right hemisphere (RH), the maximum amplitude of the average [HbO2] response was 1.29± 0.85 μM to the music stimuli and 1.23± 0.93 μM to the speech stimuli. The results indicate that auditory information is processed on both auditory cortices, but LH is more concentrated to process speech than music information. No significant differences in the locations and the latencies of the maximum responses relative to the stimulus type were found.

  • Effects of Different Exercise Training Protocols on Gene Expression of Rac1 and PAK1 in Healthy Rat Fast- and Slow-Type Muscles.

    • Purpose: Rac1 and its downstream target PAK1 are novel regulators of insulin and exercise-induced glucose uptake in skeletal muscle. However, it is not yet understood how different training intensities affect the expression of these proteins. Therefore, we studied the effects of high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on Rac1 and PAK1 expression in fast-type (gastrocnemius, GC) and slow-type (soleus, SOL) muscles in rats after HIIT and MICT swimming exercises.Methods: The mRNA expression was determined using qPCR and protein expression levels with reverse-phase protein microarray (RPPA).Results: HIIT significantly decreased Rac1 mRNA expression in GC compared to MICT (p = 0.003) and to the control group (CON) (p = 0.001). At the protein level Rac1 was increased in GC in both training groups, but only the difference between HIIT and CON was significant (p = 0.02). HIIT caused significant decrease of PAK1 mRNA expression in GC compared to MICT (p = 0.007) and to CON (p = 0.001). At the protein level, HIIT increased PAK1 expression in GC compared to MICT and CON (by ∼17%), but the difference was not statistically significant (p = 0.3, p = 0.2, respectively). There were no significant differences in the Rac1 or PAK1 expression in SOL between the groups.Conclusion: Our results indicate that HIIT, but not MICT, decreases Rac1 and PAK1 mRNA expression and increases the protein expression of especially Rac1 but only in fast-type muscle. These exercise training findings may reveal new therapeutic targets to treat patients with metabolic diseases.

  • Calibration and validation of a semi-empirical flux ecosystem model for coniferous forests in the Boreal region

    • Simple models are less input demanding and their calibration involves a lower number of parameters, however their general applicability to vast areas must be tested. We analysed if a simple ecosystem model (PRELES) can be applied to estimate carbon and water fluxes of Boreal forests at regional scale. Multi-site (M-S) and site-specific (S-S) calibrations were compared using evapotranspiration (ET) and gross primary production (GPP) measurements from 10 sites. The performances of M-S were similar to S-Ss except for a site with agricultural history. Although PRELES predicted GPP better than ET, we concluded that the model can be reliably used at regional scale to simulate carbon and water fluxes of Boreal forests. We further found that, in the calibration, the use of a long and carefully collected flux dataset from one site that covers a wide range of climate variability leads to better model performance in other sites as well.

  • Interpersonal similarity and knowledge sharing within multinational organizations’

    • Previous research has established that interpersonal similarity can influence knowledge sharing in such a way that similar people are more likely to share knowledge than those who are dissimilar. We contribute to the literature by showing that in the MNC context, cultural and functional similarity can become more powerful sources of bias than more commonly assumed demographic characteristics such as gender or seniority. This may be driven by the salience of such boundaries in the MNC: while demographic characteristics are more broadly distributed, cultural and functional barriers create more defined and observable faultlines, often coinciding with unit boundaries. We further argue that it may not be similarity as such that matters but rather its positive impact on different dimensions of social capital, which mediate the relationship between similarity and knowledge sharing. These microfoundations of inter-unit knowledge exchange point to important theoretical and practical implications for international management.

  • Rosette nebula globules : Seahorse giving birth to a star

    • Context. The Rosette nebula is an H II region ionized mainly by the stellar cluster NGC 2244. Elephant trunks, globules, and globulettes are seen at the interface where the H II region and the surrounding molecular shell meet.Aims. We have observed a field in the northwestern part of the Rosette nebula where we study the small globules protruding from the shell. Our aim is to measure their properties and study their star-formation history in continuation of our earlier study of the features of the region.Methods. We imaged the region in broadband near-infrared (NIR) JsHKs filters and narrowband H-2 1-0 S(1), P beta, and continuum filters using the SOFI camera at the ESO/NTT. The imaging was used to study the stellar population and surface brightness, create visual extinction maps, and locate star formation. Mid-infrared (MIR) Spitzer IRAC and WISE and optical NOT images were used to further study the star formation and the structure of the globules. The NIR and MIR observations indicate an outflow, which is confirmed with CO observations made with APEX.Results. The globules have mean number densities of similar to 4.6x10(4) cm(-3). P beta is seen in absorption in the cores of the globules where we measure visual extinctions of 11-16 mag. The shell and the globules have bright rims in the observed bands. In the Ks band 20 to 40% of the emission is due to fluorescent emission in the 2.12 mu m H-2 line similar to the tiny dense globulettes we studied earlier in a nearby region. We identify several stellar NIR excess candidates and four of them are also detected in the Spitzer IRAC 8.0 mu m image and studied further. We find an outflow with a cavity wall bright in the 2.124 mu m H-2 line and at 8.0 mu m in one of the globules. The outflow originates from a Class I young stellar object (YSO) embedded deep inside the globule. An H alpha image suggests the YSO drives a possible parsec-scale outflow. Despite the morphology of the globule, the outflow does not seem to run inside the dusty fingers extending from the main globule body.

  • Abdominal Aortic Calcifications Predict Survival in Peritoneal Dialysis Patioents

    • Background: Peripheral arterial disease and vascular calcifications contribute significantly to the outcome of dialysis patients. The aim of this study was to evaluate the prognostic role of severity of abdominal aortic calcifications and peripheral arterial disease on outcome of peritoneal dialysis (PD) patients using methods easily available in everyday clinical practice.Methods: We enrolled 249 PD patients (mean age 61 years, 67% male) in this prospective, observational, multicenter study from 2009 to 2013. The abdominal aortic calcification score (AACS) was assessed using lateral lumbar X ray, and the ankle-brachial index (ABI) using a Doppler device.Results: The median AACS was 11 (range 0 - 24). In 58% of the patients, all 4 segments of the abdominal aorta showed deposits, while 19% of patients had no visible deposits (AACS 0). Ankle-brachial index was normal in 49%, low (< 0.9) in 17%, and high (> 1.3) in 34% of patients. Altogether 91 patients (37%) died during the median follow-up of 46 months. Only 2 patients (5%) with AACS 0 died compared with 50% of the patients with AACS >= 7 (p < 0.001). The adjusted hazard ratio for all-cause mortality was 4.85 (95% confidence interval [CI] 1.94 - 24.46) for aortic calcification (AACS >= 7), 2.14 for diabetes (yes/no), 0.93 for albumin (per I g/L), and 1.04 for age (per year). A low or high ABI were not independently associated with mortality.Conclusions: Severe aortic calcification was a strong predictor of all-cause mortality in PD patients. The evaluation of aortic calcifications by lateral X ray is a simple method that allows the identification of high-risk patients.

    • GaAs1-xBix alloys have useful properties for many optoelectronic applications. Although the crystal growth by molecular beam epitaxy is influenced by kinetics, it is also important to understand the thermodynamics of the alloys. The Gibbs formation energies and the immiscibility curve of the totally disordered GaAs1-xBix (0 ≤ x ≤ 1) are determined using special quasi-random structures (SQS) of different sizes, different exchange-correlation functionals and Monte Carlo simulations. The local density approximation gives slightly larger mixing enthalpies and smaller Bi solubilities than the generalized gradient approximation for the GaAs1-xBix alloys, if the semiconducting GaAs and metallic Ga and Bi are reference states. The 64 and 512 atom SQS give similar mixing enthalpies, except at x = 0.5, where as eight atom SQS overestimate the mixing enthalpy significantly. The disordered alloys are more stable than the most stable ordered phases except at very low temperatures. The spinodal transformation temperature corresponds approximately to the latest experimental results, if the separated systems can conform to their own lattice constants. The spinodal transformation temperature is decreased very significantly, if the decomposing phases are constrained to maintain lattice coherence with the substrate. The strain energies of the alloys and decomposing phases are large also at the surfaces.

    • User psychology aims at understanding human-machine interaction from a psychological point of view. Its ultimate goal is to provide knowledge about human psychological properties for interaction designers. In this article, we are particularly interested in applying the theoretical concepts of mental contents (i.e., the information contents of users mental representations), in studying interaction with professional software. The immediate motivation for adopting such an approach arises from problems met in designing interaction processes in multiobjective optimization software. These types of software are meant to support complex thought and decision-making processes and this is why interaction design has to meet a number of theoretical problems, which do seldom arise in simpler interaction types. In four experiments, we investigated the types of knowledge required when developing ones capability of working with such a thought tool. We observed that in interaction design it is essential to take into account domain, professional and software specific mental content structures.

    • Background and purpose — 32-mm heads are widely used in total hip arthroplasty (THA) in Scandinavia, while the proportion of 36-mm heads is increasing as they are expected to increase THA stability. We investigated whether the use of 36-mm heads in THA after proximal femur fracture (PFF) is associated with a lower risk of revision compared with 32-mm heads. Patients and methods — We included 5,030 patients operated with THA due to PFF with 32- or 36-mm heads from the Nordic Arthroplasty Register Association database. Each patient with a 36-mm head was matched with a patient with a 32-mm head, using propensity score. The patients were operated between 2006 and 2016, with a metal or ceramic head on a polyethylene bearing. Cox proportional hazards models were fitted to estimate the unadjusted and adjusted hazard ratio (HR) with 95% confidence intervals (CI) for revision for any reason and revision due to dislocation for 36-mm heads compared with 32-mm heads. Results — 36-mm heads had an HR of 0.9 (CI 0.7–1.2) for revision for any reason and 0.8 (CI 0.5–1.3) for revision due to dislocation compared with 32-mm heads at a median follow-up of 2.5 years (interquartile range 1–4.4). Interpretation — We were not able to demonstrate any clinically relevant reduction of the risk of THA revision for any reason or due to dislocation when 36-mm heads were used versus 32-mm. Residual confounding due to lack of data on patient comorbidities and body mass index could bias our results. © 2020, © 2020 The Author(s).
