  • Guideline documents on caesarean section on maternal request in Sweden : varying usability with a restrictive approach

    • BackgroundGlobally, studies illustrate different approaches among health care professionals to decision making about caesarean section (CS) and that attitudes regarding the extent to which a CS on maternal request (CSMR) can be granted vary significantly, both between professionals and countries. Absence of proper regulatory frameworks is one potential explanation for high CSMR rates in some countries, but overall, it is unclear how recommendations and guidelines on CSMR relate to CSMR rates. In Sweden, CSMR rates are low by international comparison, but statistics show that the extent to which maternity clinics perform CSMR vary among Sweden’s 21 self-governing regions. These regions are responsible for funding and delivery of healthcare, while national guidelines provide guidance for the professions throughout the country; however, they are not mandatory. To further understand considerations for CSMR requests and existing practice variations, the aim was to analyse guideline documents on CSMR at all local maternity clinics in Sweden.MethodsAll 43 maternity clinics in Sweden were contacted and asked for any guideline documents regarding CSMR. All clinics replied, enabling a total investigation. We used a combined deductive and inductive design, using the framework method for the analysis of qualitative data in multi-disciplinary health research.ResultsOverall, 32 maternity clinics reported guideline documents and 11 denied having any. Among those reporting no guideline documents, one referred to using national guideline document. Based on the Framework method, four theme categories were identified: CSMR is treated as a matter of fear of birth (FOB); How important factors are weighted in the decision-making is unclear; Birth contracts are offered in some regions; and The post-partum care is related to FOB rather than CSMR.ConclusionIn order to offer women who request CS equal and just care, there is a pressing need to either implement current national guideline document at all maternity clinics or rewrite the guideline documents to enable clinics to adopt a structured approach. The emphasis must be placed on exploring the reasons behind the request and providing unbiased information and support. Our results contribute to the ongoing discussion about CSMR and lay a foundation for further research in which professionals, as well as stakeholders and both women planning pregnancy and pregnant women, can give their views on this issue.

  • Caesarean section on maternal request : a qualitative study of conflicts related to shared decision-making and person-centred care in Sweden

    • BackgroundToday, person-centred care is seen as a cornerstone of health policy and practice, but accommodating individual patient preferences can be challenging, for example involving caesarean section on maternal request (CSMR). The aim of this study was to explore Swedish health professionals' perspectives on CSMR and analyse them with regard to potential conflicts that may arise from person-centred care, specifically in relation to shared decision-making.MethodsA qualitative study using both inductive and deductive content analysis was conducted based on semi-structured interviews. It was based on a purposeful sampling of 12 health professionals: seven obstetricians, three midwives and two neonatologists working at different hospitals in southern and central Sweden. The interviews were recorded either in a telephone call or in a video conference call, and audio files were deleted after transcription.ResultsIn the interviews, twelve types of expressions (sub-categories) of five types of conflicts (categories) between shared decision-making and CSMR emerged. Most health professionals agreed in principle that women have the right to decide over their own body, but did not believe this included the right to choose surgery without medical indications (patient autonomy). The health professionals also expressed that they had to consider not only the woman's current preferences and health but also her future health, which could be negatively impacted by a CSMR (treatment quality and patient safety). Furthermore, the health professionals did not consider costs in the individual decision, but thought CSMR might lead to crowding-out effects (avoiding treatments that harm others). Although the health professionals emphasised that every CSMR request was addressed individually, they referred to different strategies for avoiding arbitrariness (equality and non-discrimination). Lastly, they described that CSMR entailed a multifaceted decision being individual yet collective, and the use of birth contracts in order to increase a woman's sense of security (an uncomplicated decision-making process).ConclusionsThe complex landscape for handling CSMR in Sweden, arising from a restrictive approach centred on collective and standardised solutions alongside a simultaneous shift towards person-centred care and individual decision-making, was evident in the health professionals' reasoning. Although most health professionals emphasised that the mode of delivery is ultimately a professional decision, they still strived towards shared decision-making through information and support. Given the different views on CSMR, it is of utmost importance for healthcare professionals and women to reach a consensus on how to address this issue and to discuss what patient autonomy and shared decision-making mean in this specific context. Person-centered care is today a widespread approach, but accommodating individual patient preferences can be challenging, for example involving caesarean section on maternal request (CSMR). This study examines Swedish health professionals' views on CSMR. Interviews with 12 health professionals reveal conflicts between CSMR and key aspects of person-centered care, in particular shared decision-making. While professionals acknowledge women's autonomy, they question CSMR without medical need. Concerns include for example treatment quality and patient safety, and avoiding treatments that harm others. The Swedish context, balancing collective solutions with individualized care, complicates decision-making. Unlike countries with more private healthcare, where CSMR support might be higher, Swedish health professionals emphasize shared decision-making despite viewing the mode of delivery as primarily a professional decision. This study sheds light on the challenges in integrating CSMR into person-centered care frameworks.

    • BackgroundYoung people in different healthcare settings are positive about using electronic patient-reported outcomes (ePROs), which are meant to increase the effectiveness and safety of interventions from the patient's perspective. Sweden offers free healthcare to young people aged 12-25 years at 275 youth health clinics (YHCs), whose goals are to strengthen young people and promote sexual, physical, and mental health. YHCs need effective ways to identify the overall picture of young people's health and health-related problems. To our knowledge, there is no ePRO for YHCs that provides an overview of young people's health from several health perspectives. The aim of this study was to explore young people's view on content and design of an ePRO to provide an overview of their health and health related problems when visiting a YHC, and their opinion on what healthcare needs to consider when using the ePRO. This was an explorative qualitative study. The participants were included from five YHCs, in different socioeconomic areas in central Sweden. Fifteen participants were included: 10 girls, three boys, and two non-binary participants with an age range of 16-22 years. Data were collected using a semi-structured interview guide and individual interviews, and inductive content analysis was performed.ResultsOne main theme, "ePRO created based on my needs is worth using" and two sub-themes, "Appealing content and design" and "Trusting healthcare", emerged. The participants wanted that an ePRO should include overall questions about mental-, physical-, and sexual health and social support. Participants also believed the ePRO must disclose the risks of self-harm or suicide. The participants noted the importance of emotional and digital security when using the ePRO and having a confidential conversation with a healthcare provider. To share health information means to trust to gain health.ConclusionsThe study participants' views on content and design can form the basis for designing an ePRO for young people. Their thoughts on safety and treatment in healthcare can be considered in the development process. This study is the starting point for developing an ePRO for young people at YHCs.

  • Identification of health-related problems in youth : a mixed methods feasibility study evaluating the Youth Health Report System

    • Background: Because poor health in youth risk affecting their entry in adulthood, improved methods for their early identification are needed. Health and welfare technology is widely accepted by youth populations, presenting a potential method for identifying their health problems. However, healthcare technology must be evidence-based. Specifically, feasibility studies contribute valuable information prior to more complex effects-based research. The current study assessed the process, resource, management, and scientific feasibility of the Youth Health Report System prototype, developed within a youth health clinic context in advance of an intervention study. Methods: This mixed-methods feasibility study was conducted in a clinical setting. The process, resource, management, and scientific feasibility of the Youth Health Report System were investigated, as recommended in the literature. Participants were youth aged 16–23 years old, attending a youth health clinic, and healthcare professionals from three clinics. The youth participants used their smart phones to respond to Youth Health Report System health questions and healthcare professionals used their computer to access the results and for registration system entries. Qualitative data were collected from interviews with healthcare professionals, which were described with thematic analysis. Youth participants’ quantitative Youth Health Report System data were analyzed for descriptive statistics. Results: Feasibility analysis of qualitative data from interviews with 11 healthcare professionals resulted in three themes: We expected it could be hard; Information and routines helped but time was an issue; and The electronic case report form was valuable in the health assessment. Qualitative data were collected from the Youth Health Report System. A total of 54 youth participants completed the evaluation questionnaire, and healthcare professionals retrieved information from, and made post-appointment system entries. Quantitative results revealed few missing items and acceptable data variability. An assessment template of merged qualitative and quantitative data guided a consensus discussion among the researchers, resulting in acceptable feasibility. Conclusions: The process-, resource-, management-, and scientific feasibility aspects were acceptable, with some modifications, strengthening the potential for a successful Youth Health Report System intervention study.

    • Background: Electronic Patient-Reported Outcomes (ePROs) have potential to improve health outcomes and healthcare. The development of health-technology applications, such as ePROs, should include the potential users and be theoretically grounded. Swedish Youth Health Clinics (YHCs) offer primarily sexual and psychological healthcare for young people aged 12 to 25 years old. Young people in healthcare settings are considered a vulnerable group. The development of a collection of Patient-Reported Outcomes (PROs) in an Electronic Health Report Form (eHRF) for identifying health and health-related problems in young people, was preceded by a qualitative interview study, exploring young people’s views on using an eHRF at YHCs and which questions about health an eHRF should contain. The aim of the current study was to develop and evaluate the usability of an eHRF prototype for identifying health and health-related problems in young people visiting YHCs. Methods: This study used a participatory design. During the development, an expert panel consisting of eight researchers and one Information Technology worker, participated. A wide literature search was performed to find PROs to construct an eHRF prototype to cover health areas. A mixed methods usability evaluation included 14 participants (young people, healthcare professionals, and an expert panel). Results: The development resulted in an eHRF prototype, containing ten reliable and valid health questionnaires addressing mental-, physical-, and sexual health and social support, a self-efficacy question, and background questions, in total 74 items. The interviews in the usability evaluation resulted in three categories describing the usability of the eHRF: ‘Captures the overall health of young people but needs clarification’, ‘Fun, easy, and optional and will keep young people’s interest’, and ‘Potential contribution to improve the health consultation’. The quantitative results support the usability of the eHRF for YHCs. Conclusions: The participatory approach contributed to development of the eHRF prototype to cover health areas adapted for the target population. The usability evaluation showed that the eHRF was usable and had the potential for self-reflection and contributions to cooperation between young people and healthcare professionals during the health consultation.

    • Background: Understanding young people's subjective well-being, such as how they experience equality, safety, and supportive environments, is particularly important for identifying possible preventive interventions in the school context. Aim: The aim of this study was to investigate similarities and differences between 15-year-old girls’ and boys’ subjective well-being, and its associations with psychosocial factors. Methods: The study was explorative and cross-sectional. A total of 195 adolescent girls and 116 boys in Grade 9 from four Swedish junior high schools completed a questionnaire on subjective well-being; emotional support opportunities; gender equality; experience of emotional, physical, and sexual violence in close relationships; insecurity due to harassment in the school environment or on social media; and pornography consumption. Results: Compared to boys, girls indicated worse subjective well-being and more harassment in school and on social media, and reported that boys received benefits based on gender. Boys reported higher pornography consumption compared with girls. Girls reporting decreased subjective well-being experienced more harassment on social media and in school, and more recent sexual violence. Among boys, with decreased subjective well-being, more psychological risks such as self-harm and suicidal thoughts was reported. Both girls and boys with decreased subjective well-being reported less availability of attachment and insecure close relations. Conclusion: The absence of a safe and supportive environment, such as feeling secure at school, and lack of close and trusting relationships are associated with 15-year-olds’ subjective well-being for the worse, regardless of gender. It is suggested that school personnel extend the relational possibilities in the school context.

    • ObjectivesTo describe and get a deeper understanding of how groups of young women and men reflect on and discuss pornography and its spread in the media and society, and its possible influence on sexual behaviour and relationships.MethodsSix focus group interviews were conducted with teenagers, three with women (n = 17) and three with men (n = 18). Open questions about pornography and its spread in the media and society were discussed. The interviews were tape-recorded and transcribed verbatim. Data were analysed according to Grounded Theory.ResultsThe core category 'A discriminatory sexuality' illustrates how participants felt regarding the messages conveyed by pornography portraying a man's role as dominant and a woman's role as subordinate. Pornographic messages were described as 'Fiction' depicting a distorted reality. Feelings of ambivalence towards pornography were expressed: anxiety and fear, but also inspiration. Participants said pornography occurred everywhere in the media and society, and felt pressured by messages relating to looks and sexual techniques.ConclusionsPornography and its spread in the media and society were considered as presenting a discriminatory image of body ideals, sexuality and relationships. Despite this awareness, both men and women considered pornography as sources of knowledge and inspiration: an apparent paradox.

  • Pornography consumption and psychosomatic and depressive symptoms among Swedish adolescents: a longitudinal study

    • Background: The aims of this longitudinal study were to identify predictors for continued pornography consumption and to investigate pornography consumption in relation to psychosomatic and depressive symptoms among a group of adolescents in Sweden.Methods and materials: A longitudinal study in classroom environment in 53 randomly selected senior high school classes in mid-Sweden in years 2011 and 2013. Out of 477 participating boys and 400 girls in 2011, 224 boys (47%) and 238 girls (60%) participated in 2013.Results: Higher pornography consumption at baseline and being born outside Sweden predicted continued pornography consumption at follow-up (adjusted R2 = 0.689).Psychosomatic symptoms at follow-up were predicted by higher pornography consumption at baseline (adjusted R2 = 0.254), being a girl, living with separated parents, and attending a vocational high school program. By contrast, depressive symptoms at follow-up were predicted by less pornography consumption at baseline (adjusted R2 = 0.122) and being a girl.Conclusions: Pornography consumption may, for some individuals, be associated to mental health issues. Differences between teenage boys and girls and between adolescents with diverse ethnic backgrounds imply that counseling and discussion about pornography need to be adjusted and individualized.

  • Pornography consumption, psychosomatic health and depressive symptoms among Swedish adolescents

    • Objective: The aims of the study were to investigate predictors for frequent use of pornography and to investigate such use in relation to psychosomatic and depressive symptoms among Swedish adolescents.Methods: Data collection was performed longitudinally in 13 randomly selected senior high schools in 53 classes in 2011 with a follow up in 2013. Two hundred twenty-four boys (47%) and 238 girls (60%) participated.Results: Using a multivariate general linear model (GLM) we found that being a boy born outside Sweden, living in a parent-owned dwelling, attending a vocational high school program and being a frequent user of pornography at baseline had major effects in relation to the frequent use of pornography at follow-up (adjusted R2 0.689). In a second GLM we found that being a girl, living with separated parents, attending a vocational high school program, and being a frequent user of pornography at baseline had major effects on psychosomatic symptoms at follow-up (adjusted R20.254).Conclusions: Being male, attending a vocational high school program and being a frequent user of pornography at baseline predicted frequent use of pornography at follow-up. Frequent use of pornography at baseline predicted psychosomatic symptoms at follow-up to a higher extent compared to depressive symptoms.

    • OBJECTIVE::To describe patterns of pornography use among high school boys and to investigate differences between frequent, average, and nonfrequent users of pornography with respect to sexual experiences, lifestyles, and self-rated health.METHODS::A population-based classroom survey among 16-year-old boys (n = 477), from 53 randomly selected high school classes in 2 towns in mid-Sweden.RESULTS::Almost all boys, 96% (n = 453), had watched pornography. Frequent users of pornography (everyday) (10%, n = 47) differed from average users (63%, n = 292) and nonfrequent users (27%, n = 126). Frequent users versus average users and nonfrequent users had more sexual experiences, such as one night stands (45, 32, 25%, respectively) and sex with friends more than 10 times (13, 10, 2%). A higher proportion of frequent users spent more than 10 straight hours at the computer several times a week (32, 5, 8%) and reported more relationship problems with peers (38, 22, 21%), truancy at least once a week (11, 6, 5%), obesity (13, 3, 3%), use of oral tobacco (36, 29, 20%), and use of alcohol (77, 70, 52%) versus average and nonfrequent users. One third of frequent users watched more pornography than they actually wanted. There were no differences between the groups regarding physical and psychological self-rated health.CONCLUSIONS::The boys, defined as frequent users of pornography, were more sexually experienced, spent more time at the computer, and reported an unhealthier lifestyle compared with average and nonfrequent users. No differences regarding self-rated health were detected even though obesity was twice as common among frequent users.

    • Objective: The aims were to gain a deeper understanding of how personnel, who work with adolescents, reason about the effect of pornography and its spread in the media, and to explore how well prepared they consider themselves to be in addressing sexual health and gender equality. Design and Sample: An inductive, exploratory, qualitative study with focus group discussions was selected. Seventeen participants with different professions were invited into five heterogeneous groups. Measures: Data were analyzed according to grounded theory. Results: Conflicting messages about sexuality became the core category. Participants were of the opinion that pornography conveyed a contradictory message compared with national public health goals, societal laws, and regulations. They believed that young people use pornography as a source of information and stimulation. Furthermore, they thought that pornography contributed to norm-creating ideals and a demanding sexuality, thus, confirming the traditional gender order. The participants opined that a professional approach was required when addressing sexuality and gender equality issues and requested better training tools and more cultural competence. Conclusions: Professionals working with adolescents perceived that pornography conveys a conflicting message about sexuality. They expressed a need for adequate tools for improving education on health and sexuality, including gender equality aspects and critical media analysis.

    • The present qualitative study is a part of the process evaluation of a complex intervention, the randomized controlled trial, "Prevention of human papillomavirus (HPV) in a school-based setting." We aimed to explore participating school nurses' perceptions and experiences of delivering the educational HPV intervention to adolescents aged 16. Focus group interviews were conducted with school nurses (n = 20) and analyzed with inductive qualitative content analysis. The overall theme Easily adapted into the existing role as a school nurse permeated the participants' views. The nurses were in favor of delivering an intervention that increased the HPV vaccination rates and improved beliefs and awareness about HPV prevention. It suits their work and health-promoting aspect of their role well and can easily be adapted into the current school health consultant curriculum. Having material in different languages to share with adolescents and their parents to promote equal health was deemed important.

    • The overall aim of this thesis was to investigate pornography consumption and its relation to sexual experiences, lifestyles, health and perceptions of sexuality and pornography. One qualitative study (focus group discussions) and one prospective longitudinal quantitative study (baseline and follow-up questionnaires) are included.The core category emerging from the focus group discussions, among personnel working with adolescents, was “Conflicting messages about sexuality”. The participants’ stated that the message conveyed by pornography was contradictory to the message conveyed by national public health goals and laws. A professional approach was emphasized, and adequate methods and knowledge to improve sexuality and relationship education were requested (I).Participants at baseline in 2011 were 477 boys and 400 girls, aged 16 years. Almost all boys (96%) and 54% of the girls had watched pornography. The boys were categorized into frequent users (daily), average users (every week or a few times every month) and nonfrequent users (a few times a year, seldom or never) of pornography. A higher proportion of frequent users reported experience of sex with friends, the use of alcohol, a sedentary lifestyle, peer-relationship problems and obesity. One-third watched more pornography than they actually wanted to (II).There were few differences between pornography-consuming girls and boys regarding fantasies about sexual acts, attempted sexual acts inspired by pornography and perceptions of pornography. Predictors for being sexually experienced included: being a girl, attending a vocational high school programme, stating that boys and girls are equally interested in sex, and having a positive perception of pornography. Boys were generally more positive towards pornography than girls (III).Participants at follow-up in 2013 were 224 boys (47%) and 238 girls (60%). Being male, attending a vocational high school programme and being a frequent user of pornography at baseline predicted frequent use at follow-up. Frequent use of pornography at baseline predicted psychosomatic symptoms to a higher extent at follow-up than depressive symptoms (IV).In conclusion, pornography has become a part of everyday life for many adolescents. Frequent users of pornography were mainly boys, and there were minor differences in sexual experiences between the male consumption groups. Frequent use was associated with lifestyle problems, such as the use of alcohol and a sedentary lifestyle to a higher extent than with sexual experiences and physical symptoms. In the longitudinal analyses frequent use of pornography was more associated to psychosomatic symptoms compared with depressive symptoms. Access to pornography will presumably remain unrestrained. It is therefore important to offer adolescents arenas for discussing pornography in order to counterbalance the fictional world presented in pornography, increase awareness regarding the stereotyped gender roles in pornography and address unhealthy lifestyles and ill health among adolescents.

    • Introduction While foremost mental health and psychosomatic symptoms are increasing in young people, little is known about the health in young people visiting Youth Health Clinics (YHC). Also, there is no unified health survey used to collect overall health data from young people nationally at YHCs.Aims To develop a health- and welfare technology tool to identify and increase knowledge about health-related problems in young people visiting YHCs.Methods The structured development of a health and welfare technology tool, the Health Report System (studies I to III) had a consultative level participatory approach. Study I was qualitative and contained interviews with young people on content and design for a digital questionnaire. Studies II and III had mixed methods design and included young people, healthcare professionals, and a researcher expert panel. Qualitative data came from interviews and quantitative data from questionnaires. The studies evaluated usability and feasibility. Study IV was quantitative and used data from the evaluation questionnaire, exploring the mediated association of variables on mental well-being, using a set of regression analyses.Results Study I: Young people (n=15) were positive to use a health and welfare technology tool if it was in accordance with their needs. It should give an overview of mental-, physical-, and sexual health and social support. The results should be discussed with healthcare professionals during a health assessment. Study II: The researchers used guidelines to develop the Health Report System and included ten validated and reliable health questionnaires. The usability of the Health Report System was evaluated by young people (n=4), YHC healthcare professionals (n=3), and an expert panel (n=7). The converged qualitative and quantitative data showed that the Health Report System was very usable for YHCs. Study III: The qualitative results from healthcare professionals’ interviews (n=11) and quantitative questionnaire results from young people (n=54) were merged. Using a matrix for assessment of feasibility, the researcher's consensus decision, was that progression and use of the Health Report System in an intervention study was feasible. Study IV: Results from young people’s (n=223) health data showed statistical significance for the mediating effect of social functioning between the sense of mastery and mental well-being. Conclusion This thesis presents that the Health Report System has the potential to identify health-related problems in young people, strengthen the health communication between young people and healthcare professionals, and influence the YHC organization.

    • Objectives: The purpose of the study was to contribute to knowledge about how midwives manage the separation between mother and child after a caesarean section and how they try to manage the difficulties they encounter. Methods: Data were collected from 12 interviews and subjected to inductive qualitative content analysis. Results: The findings showed the importance of enabling midwives to reflect on their daily work and indicated that the partner's role and participation after a caesarean section should receive greater focus and be part of routine care. Collaboration between the surgical and maternity wards could be improved by drawing up written guidelines to establish local routines. Together with national guidelines on minimising separation after a caesarean section, these suggestions could lead to more equal delivery of care for families.
