Och det gäller i högsta grad konstinstitutionen och vårdinstitutionen.Psykos- och rehabiliteringsavdelningarna 109 och 110 på Ulleråkers sjukhus ska renoveras. Enligt riktlinjer ska 1 % av byggkostnaden avsättas till konstnärlig gestaltning. I detta fall innebär det 210 000 kronor. Konstnärsgruppen love and devotion tar sig an det konstnärliga utsmyckningsuppdraget genom att ställa vårdmiljön snarare än konstobjektet i centrum.Inom den psykiatriska vården har antalet vårdplatser drastiskt minskat den senaste 10-årsperioden. Samtidigt är den fysiska vårdmiljön undermålig och avhandlas i stort sett aldrig. Utifrån en mängd olika perspektiv diskuterar denna bok frågor kring institutionens kris, liksom vårdens och konstens villkor i dagens samhälle.

  • Development of health-related quality of life and symptoms of anxiety and depression among persons diagnosed with cancer during adolescence : a 10-year follow-up study

    • Objective: The main aim was to investigate the development of health-related quality of life (HRQOL) and symptoms of anxiety and depression in a cohort diagnosed with cancer during adolescence from shortly after up to 10 years after diagnosis.Methods: Participants (n = 61) completed the SF-36 and the HADS shortly; six, 12, and 18 months; and two, three, four, and 10 years (n = 28) after diagnosis. Polynomial change trajectories were used to model development.Results: Polynomial change trajectories showed an initial increase which abated over time into a decrease which abated over time for the SF-36 subscales Mental Health and Vitality; an initial decline which abated over time into an increase for HADS anxiety; and an initial decline which abated over time into an increase which abated over time for HADS depression. The SF-36 mental component summary showed no change from two to 10 years after diagnosis whereas the SF-36 physical component summary showed an increase from two years after diagnosis which declined over time. Ten years after diagnosis 29% reported possible anxiety.Conclusions: Development of HRQOL and symptoms of anxiety and depression appears to be nonlinear among persons diagnosed with cancer during adolescence. Well into permanent survivorship an increase in symptoms of anxiety is shown and approximately a third of the participants report possible anxiety. The findings indicate the need for: studies designed to pinpoint the times of highest psychological risk, clinical follow-up focusing on psychological problems, and development of effective psychological interventions for survivors of adolescent cancer

  • Plasma glial fibrillary acidic protein and tau: predictors of neurological outcome after cardiac arrest.

    • The purpose was to evaluate glial fibrillary acidic protein (GFAP) and total-tau in plasma as predictors of poor neurological outcome after out-of-hospital (OHCA) and in-hospital cardiac arrest (IHCA), including comparisons with neurofilament light (NFL) and neuron-specific enolase (NSE).Retrospective multicentre observational study of patients admitted to an intensive care unit (ICU) in three hospitals in Sweden 2014-2018. Blood samples were collected at ICU admission, 12h, and 48h post-cardiac arrest. Poor neurological outcome was defined as Cerebral Performance Category 3-5 at 2-6months after cardiac arrest. Plasma samples were retrospectively analysed for GFAP, tau, and NFL. Serum NSE was analysed in clinical care. Prognostic performances were tested with the area under the receiver operating characteristics curve (AUC).Of the 428 included patients, 328 were OHCA, and 100 were IHCA. At ICU admission, 12h and 48h post-cardiac arrest, GFAP predicted neurological outcome after OHCA with AUC (95% CI) 0.76 (0.70-0.82), 0.86 (0.81-0.90) and 0.91 (0.87-0.96), and after IHCA with AUC (95% CI) 0.77 (0.66-0.87), 0.83 (0.74-0.92) and 0.83 (0.71-0.95). At the same time points, tau predicted outcome after OHCA with AUC (95% CI) 0.72 (0.66-0.79), 0.75 (0.69-0.81), and 0.93 (0.89-0.96) and after IHCA with AUC (95% CI) 0.61 (0.49-0.74), 0.68 (0.56-0.79), and 0.77 (0.65-0.90). Adding the change in biomarker levels between time points did not improve predictive accuracy compared to the last time point. In a subset of patients, GFAP at 12h and 48 h, as well as tau at 48h, offered similar predictive value as NSE at 48h (the earliest time point NSE is recommended in guidelines) after both OHCA and IHCA. The predictive performance of NFL was similar or superior to GFAP and tau at all time points after OHCA and IHCA.GFAP and tau are promising biomarkers for neuroprognostication, with the highest predictive performance at 48h after OHCA, but not superior to NFL. The predictive ability of GFAP may be sufficiently high for clinical use at 12h after cardiac arrest.

  • A generalizable data-driven model of atrophy heterogeneity and progression in memory clinic settings

    • Memory clinic patients are a heterogeneous population representing various aetiologies of pathological aging. It is unknown if divergent spatiotemporal progression patterns of brain atrophy, as previously described in Alzheimer's disease (AD) patients, are prevalent and clinically meaningful in this group of older adults. To uncover distinct atrophy subtypes, we applied the Subtype and Stage Inference (SuStaIn) algorithm to baseline structural MRI data from 813 participants enrolled in the DELCODE cohort (mean ± SD age = 70.67 ± 6.07 years, 52% females). Participants were cognitively unimpaired (CU; n = 285) or fulfilled diagnostic criteria for subjective cognitive decline (SCD; n = 342), mild cognitive impairment (MCI; n = 118), or dementia of the Alzheimer's type (n = 68). Atrophy subtypes were compared in baseline demographics, fluid AD biomarker levels, the Preclinical Alzheimer Cognitive Composite (PACC-5), as well as episodic memory and executive functioning. PACC-5 trajectories over up to 240 weeks were examined. To test if baseline atrophy subtype and stage predicted clinical trajectories before manifest cognitive impairment, we analysed PACC-5 trajectories and MCI conversion rates of CU and SCD participants. Limbic-predominant and hippocampal-sparing atrophy subtypes were identified. Limbic-predominant atrophy first affected the medial temporal lobes, followed by further temporal and, finally, the remaining cortical regions. At baseline, this subtype was related to older age, more pathological AD biomarker levels, APOE ε4 carriership, and an amnestic cognitive impairment. Hippocampal-sparing atrophy initially occurred outside the temporal lobe with the medial temporal lobe spared up to advanced atrophy stages. This atrophy pattern also affected individuals with positive AD biomarkers and was associated with more generalised cognitive impairment. Limbic-predominant atrophy, in all and in only unimpaired participants, was linked to more negative longitudinal PACC-5 slopes than observed in participants without or with hippocampal-sparing atrophy and increased the risk of MCI conversion. SuStaIn modelling was repeated in a sample from the Swedish BioFINDER-2 cohort. Highly similar atrophy progression patterns and associated cognitive profiles were identified. Cross-cohort model generalizability, both on the subject and group level, were excellent, indicating reliable performance in previously unseen data. The proposed model is a promising tool for capturing heterogeneity among older adults at early at-risk states for AD in applied settings. The implementation of atrophy subtype- and stage-specific end-points may increase the statistical power of pharmacological trials targeting early AD.

  • Aesthetic judgments and meaning-making during cooking in Home and Consumer Studies

    • In Swedish home and consumer studies (HCS), cooking forms apart of the core content, and students often experience the results in a sensuous way–by eating the food. Sensuous, or aesthetic, experiences may affect students’ meaning-making and thus what is learned within the subject. There is a lack of research concerning the aesthetic aspects of cooking in a learning context; therefore, this study aims to explore HCS students’ meaning-making by focusing on aesthetic judgments during formalized cooking practices. The research question is, in what ways do students use aesthetic judgments in meaning-making processes during cooking? The data comes from video-documented classroom observations where the students cook together. Using a pragmatic approach and practical epistemology analysis (PEA), three ways in which the students use aesthetic judgments are illustrated: as arguments in negotiations, as reference points when reactualizing experiences, and as nonverbal actions evaluating sensory qualities. Empirical examples exemplify how aesthetic judgments play a role in establishing power relations, entail social/normative values, and influence the “tacit knowing” of cooking. The study found that aesthetic experiences are integral and important in students’ meaning-making during cooking practices. Moreover, by adding a new classroom context to the methodology used, its applicability for investigating aesthetic experiences and meaning-making is confirmed and widened.

    • Presentationen baseras på en pågående fallstudie inom ämnet hem- och konsumentkunskap (HK), som genomförs i samverkan med två HK-lärare. Den del som presenteras här innefattar en praktisk epistemologisk analys (PEA) av filmade klassrumsobservationer där elever lagar mat i par. Under aktiviteten uppstår situationer som utgör hinder i matlagningsprocessen. Dessa blir särskilt synliga under moment som kräver en subjektiv bedömning baserad på sensoriska erfarenheter, exempelvis när eleverna ska bedöma om livsmedel är färdigtillagade. För att ta sig vidare i aktiviteten genomför eleverna repetitiva handlingar och söker bekräftelse från kamrater och lärare. Dessa processer synliggörs och ligger till grund för en vidare didaktisk diskussion kring lärares val av innehåll och metoder för undervisning i ämnet.

    • This study aims to contribute to the understanding of food for health as educational content in Home and Consumer Studies (HCS), specifically targeting three educational functions: (i) qualification, (ii) socialization, and (iii) subjectification. Data came from a comprehensive case study, where one school class and two HCS teachers were followed during one school year. Fourteen classroom observations and eight teacher interviews were included. It is shown how qualification functions comprise nutrition knowledge and cooking skills, how socialization functions entail, e.g., dichotomous values of food as “good” or “bad,” and how conscious consumers who take personal responsibility for themselves and the environment become promoted subjects. Taken together, the results illustrate how educational content can have consequences beyond those intended. Based on the results, an argument is made about the relevance for HCS teachers to critically reflect on their teaching and its potential educational consequences.

    • Helgo Zettervall (1831–1907) was one of Sweden’s principal architects and design talents. He was professionally active for 40 years, from the late 1850s to the mid-1890s – a period of intensive building activity in Sweden. For the first twenty years of his career, he had his practice in Lund, and in the subsequent twenty years he lived in Stockholm as head of Sweden’s state authority for public buildings. Zettervall was very productive. His portfolio contains 281 projects, of which 160 were executed, ranging from large cathedral restorations with thousands of detailed drawings and large public buildings to porch extensions and smaller residences.This dissertation highlights Zettervall’s works by putting them in a context and by analysing their qualitative aspects. Carried out as a historiographical case study, the framework for the dissertation is the architectural monograph. The individual architectural projects are the core of the presentation, assembling and making archive material readily accessible, and the method is leaning towards that of an oeuvre complète raisonné. The method includes three distinct approaches: a factual accounting of documents and drawings, a context-setting frame story that provides the conditions, and an architectural analysis.Zettervall belonged to the first generation of architects who could receive a well-planned and broad training in Sweden. All of this training took place against a backdrop of what a specific assignment demanded in terms of responsiveness and understanding. Zettervall developed a special talent for utilizing new construction methods and materials. Each assignment was a challenge to investigate new design principles and new spatial ideas. The investigation shows that every project was unique for Zettervall, and that his various solutions depended on the specific situation, regarding site and program. Zettervall was a distinct pragmatic. Every assignment had its unique conditions, and thus had a unique solution.

    • In 1947, Soviet architect Alexey Shchusev developed a large-scale urban renewal project for the post-war city of Chisinau, the then-capital of the SSR of Moldova. Part of the master plan was the construction of Boulevard D. Cantemir, which would cut through the city’s historic fabric. Only two sections of the boulevard were built before the project was abandoned. During the period of radical institutional political and economic shift towards a market economy in the early 1990s, initiatives to build the boulevard re-emerged through red lines, zoning documents, and planning regulations. The lack of political consensus caused planning paralyses over the city, creating a legal void where different actors competed to appropriate spaces. The power of the red lines has prompted various kinds of materializations of the boulevard. The real battle takes place in the sphere of the imaginary, and memory management is one of the main planning tools. Exploring the trajectory of the “Ghost Boulevard,” I reveal conflicting political and economic agendas and the many forces that constitute complex processes of planning today.

    • OBJECTIVE: To assess the role of resistin in primary Sjogren's syndrome (pSS) and its relation to local inflammation. METHODS: Blood and saliva were collected from 37 patients with pSS (duration of symptoms 12.6+/-1 yrs) and 32 healthy controls. Expression of resistin in salivary glands was visualized immunohistologically, and levels of resistin were detected by ELISA. Levels of resistin were evaluated at baseline and following oral dehydroepiandrosterone (DHEA) treatment (50 mg/day). The effect of DHEA treatment on the secretion of resistin was assessed in vitro in human leukocytes after challenge with insulin and lipopolysaccharide. RESULTS: Levels of resistin in saliva were significantly higher in patients with pSS than in controls, while circulating levels of resistin were similar in both groups. Resistin was expressed in the epithelial cells of striated ducts and in the lymphocytic foci. Resistin levels in saliva were related to the intensity of inflammation in the minor salivary glands of pSS patients. No changes of the levels of resistin in blood or saliva were observed during DHEA treatment. Exposure of naive leukocytes to DHEA in vitro induced significant expression of resistin compared to nonstimulated peripheral blood mononuclear cells (p=0.031). CONCLUSION: We showed that levels of resistin are upregulated locally in the salivary glands of patients with pSS; and that the levels of resistin correspond to the intensity of lymphocytic inflammation in patients with pSS. We suggest that resistin is expressed in the salivary glands of patients with pSS and may be a driving factor of local inflammation.

    • This report documents the conversations that occurred during two seminars, “Caring for Plans: Narratives of the Parallel Society Package”, held at the Copenhagen Architecture Festival CAFx, October 17, 2021,1 and “Solidarity in Times of Repressive Politics: A Seminar on the Effects of the Concepts ‘Particularly/Vulnerable Areas’”, held at Folkets Husby, October 15, 2022, in the Stockholm suburb of Husby.Narratives about the “failure” of large-scale housing from the postwar decades are now guiding major physical, social, and economic changes in neighborhoods all over Europe. Denmark and Sweden have long been known for their welfare-state systems and benevolent housing policies. However, in recent years, both countries have enacted new national “anti-segregation” measures that call for major physical and social changes to neighborhoods built in the 1960s and 1970s. In these processes, the opinions of local communities and residents of the neighborhoods have seldom been heard. By working with “witness seminars,” a method adopted from oral history, it is our aim to foreground residents’ perspectives and how they have enacted solidarity and collective resistance to these measures.

    • Narratives about the ‘failure’ of large-scale post-World War II housing are now guiding major physical, social, and economic changes in neighborhoods all over Europe. This is true even in Denmark and Sweden, which have long been known for their welfare states and benevolent housing policies. Today, however, both countries have enacted new national anti-segregation measures that call for major physical and social changes to neighborhoods built in the postwar era, even as the opinions of local communities and residents of such neighborhoods have been only sparsely heard – if at all. By working with the method ‘witness seminars’, we – as the research collective Aktion Arkiv – foreground residents’ perspectives and their collective resistance: the effects and affects of top-down changes. While sharing their lived experiences and actions, residents say that architects and planners can ‘simply say no’ and thereby refuse to participate in these actions.

  • Evaluation of NTRK immunohistochemistry as a screening method for NTRK gene fusion detection in non-small cell lung cancer

    • Purpose: The small molecule inhibitors larotrectinib and entrectinib have recently been approved as cancer agnostic drugs in patients with tumours harbouring a rearrangement of the neurotrophic tropomyosin receptor kinase (NTRK). These oncogenic fusions are estimated to occur in 0.1-3 % of non-small cell lung cancers (NSCLC). Although molecular techniques are most reliable for fusion detection, immunohistochemical analysis is considered valuable for screening. Therefore, we evaluated the newly introduced diagnostic immunohistochemical assay (clone EPR17341) on a representative NSCLC cohort.Methods: Cancer tissue from 688 clinically and molecularly extensively annotated NSCLC patients were comprised on tissue microarrays and stained with the pan-TRK antibody clone EPR17341. Positive cases were further analysed with the TruSight Tumor 170 RNA assay (Illumina). Selected cases were also tested with a NanoString NTRK fusion assay. For 199 cases, NTRK RNA expression data were available from previous RNA sequencing analysis.Results: Altogether, staining patterns for 617 NSCLC cases were evaluable. Of these, four cases (0.6 %) demonstrated a strong diffuse cytoplasmic and membranous staining, and seven cases a moderate staining (1.1 %). NanoString or TST170-analysis could not confirm an NTRK fusion in any of the IHC positive cases, or any of the cases with high mRNA levels. In the four cases with strong staining intensity in the tissue microarray, whole section staining revealed marked heterogeneity of NTRK protein expression.Conclusion: The presence of NTRK fusion genes in non-small cell lung cancer is exceedingly rare. The use of the immunohistochemical NTRK assay will result in a small number of false positive cases. This should be considered when the assay is applied as a screening tool in clinical diagnostics.

    • BACKGROUND: Hypopituitary patients with untreated GH deficiency and patients on inappropriately high doses of glucocorticoid (GC) share certain clinical features. OBJECTIVE: The aim of the study was to examine the influence of GC substitution on clinical characteristics in hypopituitary patients before and after GH replacement therapy. METHOD: A total of 2424 hypopituitary patients within the KIMS (Pfizer International Metabolic Database) were grouped according to ACTH status. Comparisons were performed between subjects on hydrocortisone (HC) (n = 1186), cortisone acetate (CA) (n = 487), and prednisolone/dexamethasone (n = 52), and ACTH-sufficient patients (AS) (n = 717) before and after 1 yr of GH treatment in terms of body mass index, waist and hip circumference, blood pressure, glucose, glycosylated hemoglobin (HbA1c), serum lipids, IGF-I, and comorbidity. Hydrocortisone equivalent (HCeq) doses were calculated, and measurements were adjusted for sex and age. RESULTS: At baseline, the HC group had increased total cholesterol, triglycerides, waist circumference, and HbA1c, and the prednisolone/dexamethasone group had increased waist/hip ratio as compared with AS. After HCeq dose adjustment, the HC group retained higher HbA1c than the CA group. GC-treated patients showed a dose-related increase in serum IGF-I, body mass index, triglycerides, low-density lipoprotein cholesterol and total cholesterol levels. Subjects with HCeq doses less than 20 mg/d (n = 328) at baseline did not differ from AS in metabolic endpoints. The 1-yr metabolic response to GH was similar in all GC groups and dose categories. All new cases of diabetes (n = 12), stroke (n = 8), and myocardial infarction (n = 3) during GH treatment occurred in GC-treated subjects. CONCLUSION: HCeq doses of at least 20 mg/d in adults with hypopituitarism are associated with an unfavorable metabolic profile. CA replacement may have metabolic advantages compared with other GCs.

    • This study aimed to evaluate the effects of hormone replacement therapy (HRT), known to prevent osteoporosis and fractures, on markers of bone and cartilage metabolism. Furthermore, we assessed whether changes in these markers corresponded to alterations in bone mineral density and radiographic joint destructions in postmenopausal women with rheumatoid arthritis. Eighty-eight women were randomized to receive HRT, calcium, and vitamin D3, or calcium and vitamin D3 alone, for 2 years. Bone turnover was studied by analyzing serum levels of C-terminal telopeptide fragments of type I collagen (CTX-I), C-terminal telopeptide of type I collagen (ICTP), bone sialoprotein, and C-terminal propeptide of type I procollagen (PICP) and cartilage turnover by urinary levels of collagen type II C-telopeptide degradation fragments (CTX-II) and cartilage oligomeric matrix protein (COMP) in serum. Treatment with HRT resulted in decrease in CTX-I (P < 0.001), ICTP (P < 0.001), PICP (P < 0.05), COMP (P < 0.01), and CTX-II (P < 0.05) at 2 years. Reductions in CTX-I, ICTP, and PICP were associated with improved bone mineral density. Of the markers tested, CTX-I reflected bone turnover most sensitively; it was reduced by 53 +/- 6% in the patients receiving HRT. Baseline ICTP (P < 0.001), CTX-II (P < 0.01), and COMP (P < 0.05) correlated with the Larsen score. We suggest that biochemical markers of bone and cartilage turnover may provide a useful tool for assessing novel treatment modalities in arthritis, concerning both joint protection and prevention of osteoporosis.

    • Hormone replacement therapy (HRT) modulates the imbalance in bone remodeling, thereby decreasing bone loss. Sex hormones are known to influence rheumatic diseases. The aim of this study was to investigate the effects of HRT on the serum levels of hormones and cytokines regulating bone turnover in 88 postmenopausal women with active rheumatoid arthritis (RA) randomly allocated to receive HRT plus calcium and vitamin D3 or calcium and vitamin D3 alone for 2 years. An increase in estradiol (E2) correlated strongly with improvement of bone mineral density in the hip (P < 0.001) and lumbar spine (P < 0.001). Both baseline levels and changes during the study of IL-6 and erythrocyte sedimentation rate were correlated positively (P < 0.001). HRT for 2 years resulted in an increase of the bone anabolic factor, insulin-like growth factor 1 (IGF-1) (P < 0.05) and a decrease of serum levels of soluble IL-6 receptor (sIL-6R) (P < 0.05), which is known to enhance the biological activity of IL-6, an osteoclast-stimulating and proinflammatory cytokine. Baseline levels of IL-6 and IGF-1 were inversely associated (P < 0.05), and elevation of IGF-1 was connected with decrease in erythrocyte sedimentation rate (P < 0.05) after 2 years. Interestingly, increase in serum levels of E2 was associated with reduction of sIL-6R (P < 0.05) and reduction of sIL-6R was correlated with improved bone mineral density in the lumbar spine (P < 0.05). The latter association was however not significant after adjusting for the effect of E2 (P = 0.075). The influences of IGF-1 and the IL-6/sIL-6R pathways suggest possible mechanisms whereby HRT may exert beneficial effects in RA. However, to confirm this hypothesis future and larger studies are needed.

    • OBJECTIVE: Hormone replacement therapy (HRT) is known to exert a positive effect in preventing bone loss and a beneficial effect on the disease activity in rheumatoid arthritis (RA). We evaluated the effects of HRT on bone mineral density (BMD) and on the course of established RA. METHODS: Eighty-eight postmenopausal women with RA were randomly allocated to receive HRT, vitamin D3, and calcium supplementation or vitamin D3 and calcium supplementation alone for 2 years. The effects of additional HRT on laboratory and clinical measures of disease activity, quality of life, and BMD and on radiographic joint damage were investigated. RESULTS: Treatment with HRT suppressed signs of inflammation as shown by reduction in erythrocyte sedimentation rate (ESR) (p = 0.025) and an elevation in hemoglobin concentration (p = 0.007), a better clinical outcome assessed by response on the Disease Activity Score 28 (DAS28) (p = 0.036), increased BMD in the forearm, proximal femur and spine (p < 0.01), and retarded (p = 0.026) progression of joint destruction among patients with radiological progressive disease. No significant effect on quality of life was seen. CONCLUSION: Two years of HRT in women with active RA had significant ameliorating effects on inflammation, DAS28 response, and BMD and was associated with slower progression of radiological joint destruction. The mechanisms by which HRT exerts its effects remain to be elucidated. We suggest HRT can be used in addition to conventional therapy in the management of postmenopausal patients with RA.

    • Purpose The purpose of this study is to identify, characterize and assess supplier flexibility measurement practices in the order-to-delivery process. Design/methodology/approach The study involved a survey; participants were 224 purchasing managers at Swedish manufacturing companies that had more than 20 employees. Findings Scrutiny of the details of measurement practices revealed that most respondents actually do not specifically measure supplier flexibility. Instead they measure other measures like delivery reliability, conduct qualitative follow-ups, or cannot specify how supplier flexibility is measured. It was acknowledged that they measure different supplier flexibility aspects, and the applied measures were characterized, e.g. in terms of which flexibility dimension they represent. Research limitations/implications Conceptual clarifications and adaptations to measuring supplier flexibility in the order-to-delivery process are provided. The identified measures can be a contribution in further developing literature on flexibility performance measurement. Practical implications Purchasing, logistics and supply chain managers in search of supplier flexibility performance measurement can find ways to measure and an extended flexibility vocabulary. This has the potential to improve flexibility in the supply chain. Originality/value Even though flexibility is claimed as being an important competitive advantage, few empirical studies and operationalized measures exist, particularly in the order-to-delivery process.

It aims to do this with a triadic approach, encompassing a customer, a supplier and a logistics service provider.Design/methodology/approach – The paper takes the form of a conceptual analysis and a triadic case study on performance measurement requirements in an OTD process characterized by delivery scheduling, and generating performance models.Findings – Two OTD process performance models, one for the supplier's delivery sub‐process and one for the customer's delivery scheduling, the logistics service provider's transportation and the customer's good receipt sub‐process, in delivery scheduling environments are generated.Research limitations/implications – A single case study limits the levels of external validity and reliability to analytical generalization.Practical implications – The generated performance models include definitions of four sub‐processes and outline ten performance dimensions that should be of relevance for several companies to apply.Originality/value – This is the first approach that generates performance models for a triadic OTD process for use in delivery scheduling environments.

Att ha flexibla leverantörer är ett alternativ för ett köparföretag till att själv hålla kostsamma buffertar för att säkerställa egen flexibilitet mot kunder. Frågan är hur och med vilka strategier företag kan uppnå kundorder-flexibilitet i order-till-leveransprocessen? Få empiriska studier förekommer på området. Syftet med denna studie är att utveckla ett ramverk bestående av strategier för hur kundorderflexibilitet i order-till-leveransprocessen kan uppnås, samt att empiriskt undersöka hur leverantörer försöker uppnå kundorderflexibilitet. En konceptuell analys av befintlig litteratur genomfördes och sammanställdes i form av ett ramverk med strategier kopplade till två flexibilitetsdimensioner. Flexibilitet i orderkvantitet och flexibilitet i leveranstid har studerats och täcker därmed order- respektive leverans-delprocesserna. Denna struktur ger fyra kombinationer som täcker 21 proaktiva såväl som 18 reaktiva strategier.Efter att ha sammanställt ramverket av strategier genomfördes en empirisk intervjustudie där nio viktiga leverantörer till fyra stora svenska tillverkare ingick. Leverantörerna är alla viktiga materialleverantörer, som hanterar kundorder med leveranstid, d. v. s.  tillämpar montering mot order (ATO) eller tillverkning mot order (MTO). Deras marknadschefer, key account managers, försäljningschefer eller exportchefer svarade på en intervju som utgick från flexibilitetsstrategierna i ramverket. Svaren avsåg hur de agerar för att uppnå flexibilitet mot den givna köparen, vilket skattades på en skala 1 (inte alls) till 5 (i mycket stor omfattning). De kunde även lägga till ytterligare använda strategier. Det visade sig att den strategi som hade högst medelvärde var “vi förbereder oss genom kontinuerliga kontakter med kundens inköpare”. Medelvärdet för använda strategier var högre i order-delprocessen än i leverans-delprocessen. Med de empiriskt genererade strategierna expanderade ramverket till 23 proaktiva och 19 reaktiva strategier. Det utvecklade ramverket är ett bidrag till flexibilitetslitteraturen. Leverantörer kan använda ramverket som ett hjälpmedel för att på ett systematiskt sätt utveckla och uppnå en högre kundorderflexibilitet i order-till-leveransprocessen. Köparföretagen kan använda ramverket som en checklista eller ett verktyg vid leverantörsutveckling.

The purpose is also to investigate how companies prioritize various strategies to achieve customer order flexibility.Design/methodology/approach: Based on a literature review, pre-tests and conceptual reasoning, a conceptual framework of strategies related to the order-to-delivery process was developed. The strategies were linked to the order quantity and delivery lead-time flexibility dimensions. This structure resulted in six groups covering enabling as well as remedial strategies. An empirical interview study of ten customer–supplier relationships was conducted.Findings: The interviews identified additional strategies, thereby expanding the framework. The enabling strategies with the highest median values were “have continuous contact with the customer's purchaser” and “use safety stock of raw materials/semi-finished products”. The remedial strategy with the highest median was “re-plan/re-prioritize the order backlog”. In the delivery sub-process, it was more common to apply remedial strategies for delivery lead-time than for order quantities.Research limitations/implications: The developed framework is a contribution to the literature on operational flexibility in and related to the OTD process. It complements existing knowledge by taking a supplier perspective.Practical implications: Suppliers can use the framework as a tool to understand and systematically achieve better customer order flexibility in and related to the OTD process. Customers can use the framework as a checklist for supplier evaluation and supplier development.Originality/value: Few identified studies include empirical data on customer order flexibility.

    • Flexibility is one enabler of efficient use of resources, and is therefore an antecedent to sustainability. The purpose of this article is to identify supplier flexibility variables in, and related to, the order-to-delivery (OTD) process and categorize them into a framework, followed by empirically exploring the framework. A perception-based survey was sent to Swedish purchasing managers. 289 responses were received. After descriptive gap analysis, exploratory factor analysis was applied to structure the responses into factors. This formed the basis for hierarchical linear regression analysis, explaining supplier flexibility. A conceptual framework, specifying supplier flexibility into volume, delivery and information exchange dimensions and relating these dimensions to the OTD process, was developed. Significant negative gaps between actual and demanded volume flexibility and delivery flexibility were identified, while significant positive gaps were found for information exchange flexibility. The factor analysis revealed three factors. The regression analysis verified that OTD-related information exchange flexibility and OTD-related volume and delivery flexibility explain the variation in OTD-specific flexibility, and are important antecedents for supplier flexibility in the OTD process. Research limitations/implications A contribution to research is the framework – with defined, related and empirically validated flexibility types. The study proposes a perception-based way to capture supplier flexibility in the OTD process, which is of practical relevance when evaluating suppliers. Identifying, conceptualizing and capturing types of supplier flexibility in the OTD process is new related to academic literature. The wide empirical study, mapping supplier flexibility gaps, is unique in its focus.

    • Leverantörers flexibilitet är av stor vikt för att kunna vara flexibel mot sina kunder. Trots att det generellt uttrycks ett behov av att ha flexibla leverantörer, är det få företag som uttryckligen använder det som ett mätetal. En enkätstudie som 2018 presenterades på Plans forsknings- och tillämpningskonferens kartlade leverantörsflexibilitet i svenska företag baserat på över 500 inköpschefers svar. Resultaten från den studien visade att inköpschefer generellt upplever stora negativa gap mellan bedömda behov och erhållen flexibilitet i kritiska leverantörers volym- och leveransflexibilitet. Dessa svar väckte åtskilliga nya frågor och ledde till denna studie. Syftet är att förstå och belysa varför det uppstår negativa gap mellan bedömt behov och erhållen leverantörsflexibilitet. Den använda metoden är en flerfallstudie hos fyra större tillverkare, där data har analyserats med hjälp av fiskbensdiagram och ställts mot litteraturen. Behovet av leverantörsflexibilitet, hur leverantörsval och leverantörsvärdering genomförs, hur kommunikationen sker med leverantören och vilken maktsituation som råder mellan köpare och leverantör har identifierats som bidragande orsaker till negativa gap i leverantörsflexibilitet. Förhållanden som identifierats i företagen innefattar bl.a. att flexibilitet inte avtalas med leverantörer, att andra urvalskriterier än flexibilitet används, att flexibilitet inte värderas och följs upp, att kommunikationen mellan köpare och leverantör inte fokuserar flexibilitet och att leverantörerna har stor makt över köparna. Dessa förhållanden har formulerats i åtta propositioner. Komplexa samband mellan dessa orsaker har också identifierats. Resultaten kan ge praktiker och forskare en grundförståelse för leverantörsflexibilitet i form av dess betydelse och orsaker till gap mellan bedömda behov och erhållen flexibilitet. Flera förslag till fortsatta studier presenteras.

    • This introduction to "Solids and Flows: Architecture and Capitalism" provides a context to the articles assembled in this issue, and reflects on the implications of what can be perceived as a common approach. Drawing on Nancy Fraser's "expanded conception of capitalism" combined with Isabelle Stengers' advocacy for "resisting the coming barbarism," we offer a framework for thinking architecture's relationship to capitalism that goes beyond the spheres of property and market, and places emphasis on our capacity to move across the categories. It seems that in view of "the social institutionalisation" of capitalism, and confronted with the neoliberal market set up as "thinking machine," there is no simple return to the analysis and strategies established in the past. Rather, it becomes a matter of " learning from now on" with an acute attention to detail of the various assemblages in which architecture now operates.

    • Background: Among Swedish mothers, breastfeeding duration has been declining in recent years. An instrument for early identification of women at risk for shorter breastfeeding duration may be useful in reversing this trend. Objectives: The aims of this study were to translate and psychometrically test the Swedish version of the Breastfeeding Self-Efficacy Scale-Short Form (BSES-SF), examine the relationship between breastfeeding self-efficacy and demographic variables, and evaluate associations with breastfeeding continuation plans in Swedish mothers. Methods: The BSES-SF was translated into Swedish using forward and back translation. The sample consisted of 120 mothers who, during the first week postpartum, came for a routine follow-up visit at the postnatal unit in a university hospital. The mothers were compared based on demographic data and their future breastfeeding plans. Results: The Cronbach’s alpha coefficient for internal consistency for the BSES-SF was 0.91 and the majority of correlation coefficients exceeded 0.3. A 1-factor solution was found that explained 46% of the total variance. There was no difference in confidence in breastfeeding between mothers with early hospital discharge and mothers who received postnatal care at the hospital. Primiparas who stayed longer at the hospital were less confident in breastfeeding than primiparas who had a shorter hospital stay. Breastfeeding mothers who planned to partially breastfeed in the near future had lower BSES-SF scores, compared to those who planned to continue exclusive breastfeeding. Conclusion: The Swedish version of the BSES-SF has good reliability, validity, and agreement with mothers’ plans regarding breastfeeding continuation and exclusivity.

    • PET with F-18-choline (F-18-FCH) is used in the diagnosis of prostate cancer and its recurrences. In this work, biodistribution data from a recent study conducted at Skane University Hospital Malmo were used for the development of a biokinetic and dosimetric model. Methods: The biodistribution of F-18-FCH was followed for 10 patients using PET up to 4 h after administration. Activity concentrations in blood and urine samples were also determined. A compartmental model structure was developed, and values of the model parameters were obtained for each single patient and for a reference patient using a population kinetic approach. Radiation doses to the organs were determined using computational (voxel) phantoms for the determination of the S factors. Results: The model structure consists of a central exchange compartment (blood), 2 compartments each for the liver and kidneys, 1 for spleen, 1 for urinary bladder, and 1 generic compartment accounting for the remaining material. The model can successfully describe the individual patients' data. The parameters showing the greatest interindividual variations are the blood volume (the clearance process is rapid, and early blood data are not available for several patients) and the transfer out from liver (the physical half-life of F-18 is too short to follow this long-term process with the necessary accuracy). The organs receiving the highest doses are the kidneys (reference patient, 0.079 mGy/MBq; individual values, 0.033-0.105 mGy/MBq) and the liver (reference patient, 0.062 mGy/MBq; individual values, 0.036-0.082 mGy/MBq). The dose to the urinary bladder wall of the reference patient varies between 0.017 and 0.030 mGy/MBq, depending on the assumptions on bladder voiding. Conclusion: The model gives a satisfactory description of the biodistribution of F-18-FCH and realistic estimates of the radiation dose received by the patients.
