    • På uppdrag av Försvarsmakten genomförde Militärmedicinska forskningsgruppen vid Åstandlaboratoriet vid Gymnastik- och idrottshögskolan Stockholm (GIH), en observationsstudie gällande fysiologisk/medicinsk belastning på 14 flygplatsjägare under en fältövning med beteckningen "F 17 - Flygvapenövning 2014".Övningen var indelad i tre etapper, varav den första (c:a 42 tim) och sista (c:a 59 tim) planerades vara mer fysiskt och psykiskt krävande än den mellersta (c:a 114 tim). Registrering av hjärtfrekvens gjordes under hela övningen. Fysiologiska mätningar samt blodprov togs före och efter varje etapp samt efter c:a 12 timmars vila med sedvanliga kost efter övningens avslutande.Beräkningar av energiomsättningen för de tre etapperna inklusive viloperioder var c:a 308, 185 respektive 369 kcal/tim, resulterande i totalt c:a 13 000, 21 100 respektive 21 600 kcal per etapp eller totalt c:a 55 700 kcal för hela övningen. De höga fysiska belastningarna i etapp ett och tre är klart högre än tidigare uppmätta data i svenska och utländska militära operationer. Vissa delar av etapp tre resulterade i fysiska belastningar som var på ungefär samma nivå som vid tävlingar i längre uthållighetsidrotter.Den höga belastningen resulterade i stora förändringar i fysiologiska parametrar och medicinska markörer. Vissa mätningar av muskelstyrka i armar och ben liksom maximal syreupptagning försämrades. Testosteron sjönk under hela övningen kraftigt. Dessa och andra uppmätta data talar för att fysiska arbetsförmågan – i förlängningen "stridsvärdet" – blev klart försämrat under övningen.Mätningen 12 timmar efter övningens avslut visade oväntade resultat. Den subjektiva ansträngningskänslan under standardiserat cykelarbete var klart förbättrat av 12-timmarsvilan efter övningen. Däremot var flertalet medicinska och fysiologiska värden oförändrade jämfört med värden vid testerna 12 timmar tidigare och därmed var de klart lägre än vid testet innan övningens start. Det är uppenbart att återhämtning av stridsvärdet inte kan bedömas subjektivt utan måste avgöras genom reliabla psykologiska och fysiologiska mätningar.Undersökningen har gett underlag för fortsatta studier rörande bland annat typer av energitillskott under övning, olika strategier för snabb återhämtning samt försök att finna markörer för individuella reaktionsmönster på "stridsvärde" under långvariga militära operationer.

    • Denna observationsstudie av militär grundutbildning för kadetter visar mycket tydligt att förflyttning i fjällmiljö leder till stort energiunderskott, till viss del beroende på ökade energiutgifter i och med bärande av utrustning, men framför beroende på otillräckligt energiintag.Den genomsnittliga energiförbrukningen över övningens 100 timmarna och ca 78 km förflyttning var ca 260 kcal/h, medan energiintaget endast 135 kcal/h. Även om en typ av rations (mjukkonserv) gav i genomsnitt högre energiintag än två rations av frystorkat blev det genomsnittliga energiunderskottet för samtliga rations nästan 50 %.Mätningarna visade stora variationer mellan olika deltagare vilket dock endast delvis kan förklaras med tekniken av att bära tungt.Skjutprov (precision) visade att stridsvärdet påverkats negativt, genom en klart försämrad träffbild efter övningen. De fysiologiska testerna visar ökad hjärtfrekvens och upplevd ansträngning på submaximala belastningar, medan de maximala nivåerna av de fysiologiska kapaciteterna kondition, muskelstyrka i hand och ben i stort sett var oförändrade.Kommande undersökningar på motsvarande övningar bör inriktas på interventionsstudier på fördelning av utrustningsvikter, samt hur kostintaget bör förbättras för att nå ökat energiintag.På längre sikt bör träningsmodeller för att förbättra förmågan att bära tung utrustning utvärderas.

  • Slutövning GMU: ”Aldrig ge upp”, Amf1, Berga örlogsbas. : Rapport 4.

    • Energistatus och förändring i fysiskt stridsvärde studerades i samband med en nästan 8 dygn långa grundmilitär slutövning (GMU) vid AMF-1 Berga Örlogsskolor i månadsskiftet oktober november 2013.Totalt deltog 105 soldater i övningen, fördelat på 3 plutoner om vardera 35 soldater. En subgrupp på 24 soldater (8 kvinnor) bestående av 8 soldater från vardera pluton studerades mer ingående.Medelhjärtfrekvensen för de 6 soldater (2 kvinnor) som hade i stort sett kompletta mätningar från den 187 timmar långa övningen, inklusive viloperioder, var 88 ± 7 slag/minut. Utifrån hjärtfrekvensdata beräknades den totala energiomsättningen till i genomsnitt 44 000 ± 6 600 kcal, vilket motsvarar 5 600 ± 840 kcal per dygn och 235 ± 35 kcal/tim. Total energiförbrukning var i genomsnitt 39 000 kcal för kvinnorna och 46 500 kcal för männen. I övrigt noterades inte några betydande skillnader mellan kvinnor och män. Däremot var det stora individuella variationer i energiutgift, vilka till ca hälften berodde på skillnader i kroppsvikt. Med hänsyn till kroppsvikt och buren vikt var energiförbrukningen ca 3,1 ± 0,23 kcal per timme per kg totalvikt. Den individuella variationen beror på skillnad i buren vikt, på olika uppgifter och på individuella fysiologiska skillnader.Utifrån beräknat energiintag blev det totala energiunderskottet under övningen 12 000-15 000 kcal, vilket är ca 1 500-2 000 kcal per dygn. Viktminskningen under övningen var 2,9 kg för kvinnor och 3,7 kg för män. Denna viktminskning på >4 % leder troligen till försämrad uthållighetsförmåga.Den maximala muskelstyrkan i armar och ben var i stort sett oförändrade efter övningen, liksom den beräknade maximala syreupptagningsförmågan. Däremot upplevdes ett lågintensivt cykelarbete som betydligt tyngre efter övningen. Muskeluthållighet mättes inte i denna studie.Ett skjutprov om 5 skott i liggande på 100 m mot en tredjedelsfigur visade 64 deltagande soldater på en försämrad träffprocent från 90,5% före till 79,4 % efter övningen. Alla soldater hade minst en träff före medan 6 soldater hade alla bom efter övningen.Slutsatsen från studien är att GMU-övningen resulterade i ett stort energiunderskott. Stridsvärdet, bedömt från skjutprovet var klart försämrat. Maximala fysiologiska parametrar var i stort sett oförändrade, medan skattad ansträngning och därmed uthållighetsförmåga, försämrades.

  • Validation of modified D-max method for calculating individual anaerobic threshold in well trained male cyclists

    • IntroductionTo predict endurance performance and evaluate adaption to training in endurance athletes a laboratory test for retrieving the anaerobic lactate threshold is often used. The maximal rate of oxidative metabolism that can be sustained during prolonged exercise indicates preserved homeostasis and thereby sets the upper limit for long term endurance. Exercise intensities above the anaerobic threshold require an additional input from anaerobic energy sources resulting in blood lactate accumulation and reduced time to fatigue. The threshold is therefore a reliable and powerful predictor of performance in aerobic exercise lasting approximately 40-60 min. The work rate that elicits a blood lactate of 4 mmol/l (e.g. LT4) is widely used to determine the threshold, but by using a fixed lactate concentration, flexibility to account for inter- and intra-individual differences in aerobic and anaerobic metabolism is lost. Our hypothesis was that our modified D-max method (D-maxmod) would provide a more accurate performance related value by calculating the individual anaerobic threshold (IAT).Methods20 males cyclists age 36 ± 5 years, weight 79.7 ± 5.8 kg, VO2max 4.4 ± 0.4 l/min performed an incremental test on a cycle ergometer for calculation of LT (D-maxmod, and LT4) at two occasions separated by 8 weeks. The test consisted of 5 minutes long stages separated by 1 min of rest were lactate was sampled. The increase in work rate was 30 W/stage starting at 100 W and ended above LT4. Short after, an incremental test to fatigue (start at LT4 power, increase 20 W/min) was performed for assessment of VO2max, calculation of maximal aerobic power (MAP) and maximal power achieved in the test (Wmax). On a separate day a 40 minutes time-trial (TT40) was performed for assessment of aerobic endurance performance. IAT D-maxmod was defined as the derivate to the exponential curve created from exponentially lactate increase, including maximal lactate concentration plotted at MAP. The increase in lactate relative to power was defined as the increase in blood lactate from the point where the exponential curve crossed the lactate baseline (See also Cheng et al. 1992 and Zhou & Weston 1997).ResultsIAT D-maxmod was calculated to 5.0 ± 0.8 mmol/l. Both D-maxmod and LT4 were highly significantly correlated with both TT40 and Wmax. Coefficients of determination were higher for D-maxmod compared to LT4 for both TT40 (r2 = 0.78 vs 0.69) and Wmax (r2 = 0.89 vs 0.64).ConclusionThe calculated D-maxmod correlated better with performance than LT4 for parameters highly linked to performance in road- and mountain bike competitions.ReferencesCheng B, Kuipers H, Snyder AC, et al.     • In the kraft pulping process, the lignin-containing by-product kraft black liquor is currently combusted as an energy source. LignoBoost is a technique that extracts lignin from kraft black liquor, resulting in a lignin-lean black liquor, which is returned to the process, and an extracted kraft lignin. To facilitate the use of the extracted kraft lignin in high-value applications, it can be refined via fractionation to produce a more homogeneous starting raw material. Hence, the aim of this study is to investigate the behavior of dissolved softwood kraft lignin during cross-flow filtration. The effects of the lignin concentration, pH, and ionic strength on the fractionation of the dissolved lignin during cross-flow filtration are investigated. The results indicate that large amounts of low-molecular-weight kraft lignin can be produced from solutions having a low lignin concentration. Furthermore, the effects of pH and ionic strength on the fractionation of low-molecular-weight lignin are identified within the studied ranges.

  • "Distributed proton radiation therapy"--a new concept for advanced competence support.

    • The increased interest in high precision radiation therapy is to a large extent driven by the potential of modern imaging technology. The aim of this project was to analyse how an expensive proton facility best could support a multi-centre health care system. We have developed a model for distributed expert collaboration where all clinical experts will work close to their patients in regional centres. Patients who are candidates for proton therapy will be examined and dose-planned at their regional clinic, discussed in a fully information supported video conference and digitally made available at the proton treatment facility. The proton facility itself will be placed near a communication centre easily reached by all patients where they will be treated under full responsibility of their own physician at the home clinic. This concept has been analysed in detail both with respect to the overall functionality and with respect to possible weaknesses. It was found that the concept of distributed radiation therapy, as proposed here, will offer a stable clinical solution for advanced radiation therapy. It will support the spread of knowledge, serve as a fully developed backup system and the concept will further serve as an efficient base for clinical research.

    • Development of antibody drugs against novel targets and pathways offers great opportunities to improve current cancer treatment. We here describe a phenotypic discovery platform enabling efficient identification of therapeutic antibody-target combinations. The platform utilizes primary patient cells throughout the discovery process and includes methods for differential phage display cell panning, high-throughput cell-based specificity screening, phenotypic in vitro screening, target deconvolution, and confirmatory in vivo screening. In this study the platform was applied on cancer cells from patients with Chronic Lymphocytic Leukemia resulting in discovery of antibodies with improved cytotoxicity in vitro compared to the standard of care, the CD20-specific monoclonal antibody rituximab. Isolated antibodies were found to target six different receptors on Chronic Lymphocytic Leukemia cells; CD21, CD23, CD32, CD72, CD200, and HLA-DR of which CD32, CD200, and HLA-DR appeared as the most potent targets for antibody-based cytotoxicity treatment. Enhanced antibody efficacy was confirmed in vivo using a patient-derived xenograft model.

  • Effects of prolonged low intensity exercise with energy deficit (military training operation) on markers of muscle protein turnover.

    • IntroductionIt is well known that ultra-endurance exercise, such as Adventure racing and military operations, often induce substantial energy deficits. This suggests a catabolic state, but the exact effects on protein turnover have not yet been sufficiently investigated. The aim of this study was to examine several markers involved in muscle protein turnover before and after a multi-day physically demanding military training operation.MethodsSeven female (age 21 ± 5 years, weight 71.2 ± 6.6 kg) and seventeen male (age 20 ± 1 years, weight 76.6 ± 6.2 kg) performed a 185 hours military training operation. Energy intake was estimated from food supply and energy expenditure was calculated from continuous heart rate and accelerometer recordings. Muscle biopsies were taken from M Vastus Lateralis before and after the operation.ResultsA negative energy balance of 1,500-2,000 kcal/24 hours was estimated. Body weight declined 3.4 (95% CI 3.0-3.8) kg and muscle explosive strength, evaluated from squad and counter movement jumps, was reduced 5 and 6 %, respectively, after the operation with no difference between genders. Muscle glycogen content was reduced from 269 ± 58 to 181 ± 44 mmol/kg dry muscle (p<0.05) with no difference between genders. Muscle content of mTOR and p70 as well as MAFbx were unchanged while the protein content of MuRF-1 was significantly down regulated in both genders.DiscussionThe study indicated that prolonged low intensity exercise with substantial energy deficit reduces muscle function and muscle glycogen content. Proteins for muscle synthesis mTOR and p70 were unchanged while the down regulation of MuRF-1 indicates a protection against muscle break down during the energy deficit situation, preserving the muscle mass.

  • Increased autophagy signaling but not proteasome activity in human skeletal muscle after prolonged low-intensity exercise with negative energy balance

    • Little is known about the molecular regulation of skeletal muscle protein turnover during exercise in field conditions where energy is intake inadequate. Here, 17 male and 7 female soldiers performed an 8 day long field based military operation. Vastus lateralis muscle biopsies, in which autophagy, the ubiquitin-proteasome system and the mTORC1 signaling pathway where studied, were collected before and after the operation. The 187 h long operation resulted in a 15% and 29% negative energy balance as well as a 4.1% and 4.6% loss of body mass in women and men respectively. After the operation protein levels of ULK1 as well as the phosphorylation of ULK1Ser317 and ULK1Ser555 had increased by 11%, 39% and 13%, respectively, and this was supported by a 17% increased phosphorylation of AMPKThr172 (P<0.05). The LC3b-I/II ratio was 3-fold higher after compared to before the operation (P<0.05), whereas protein levels of p62/SQSTM1 were unchanged. The β1, β2, and β5 activity of the proteasome and protein levels of MAFbx did not change, while levels of MuRF-1 were slightly reduced (6%, P<0.05). Protein levels and phosphorylation status of key components in the mTORC1 signaling pathway remained at basal levels after the operation. Muscle levels of glycogen decreased from 269±12 to 181±9 mmol ∙ kg dry muscle-1 after the exercise period (P<0.05). In conclusion, the 8 days of field based exercise resulted in induction of autophagy without any increase in proteasome activity or protein ubiquitination. Simultaneously, the regulation of protein synthesis through the mTORC1 signaling pathway was maintained.

  • Cidea improves the metabolic profile through expansion of adipose tissue

    • In humans, Cidea (cell death-inducing DNA fragmentation factor alpha-like effector A) is highly but variably expressed in white fat, and expression correlates with metabolic health. Here we generate transgenic mice expressing human Cidea in adipose tissues (aP2-hCidea mice) and show that Cidea is mechanistically associated with a robust increase in adipose tissue expandability. Under humanized conditions (thermoneutrality, mature age and prolonged exposure to high-fat diet), aP2-hCidea mice develop a much more pronounced obesity than their wild-type littermates. Remarkably, the malfunctioning of visceral fat normally caused by massive obesity is fully overcome-perilipin 1 and Akt expression are preserved, tissue degradation is prevented, macrophage accumulation is decreased and adiponectin expression remains high. Importantly, the aP2-hCidea mice display enhanced insulin sensitivity. Our data establish a functional role for Cidea and suggest that, in humans, the association between Cidea levels in white fat and metabolic health is not only correlative but also causative.

    • Introduction: Different forms of graft-versus-host disease (GVHD) remain a major cause of morbidity and mortality after allogeneic hematopoietic stem cell transplantation (HSCT). The prognosis for steroid-refractory chronic GVHD (cGVHD) remains poor. Our aim was to evaluate extracorporeal photopheresis (ECP) treatment in cGVHD patients with different organ involvement to detect subgroups of patients with the best response.Material and methods: Thirty-four patients who underwent HSCT and developed moderate (n = 7) or severe (n = 27) steroid-refractory or steroid-dependent cGVHD treated with ECP were included in the analysis. A matched cGVHD control patient group untreated with ECP was collected for comparison.Results: Compared to the control group and the stable/progressive disease (SD/PD) patients, individuals with complete/partial remission have higher overall survival and lower transplant-related mortality. Furthermore, patients with complete and partial remission (CR/PR) had significantly higher levels of albumin and platelets after ECP treatment compared to patients with stable or progressive cGVHD (SD/PD). Corticosteroid treatment and other immunosuppressive agents could successfully be tapered in the CR/PR group compared to the SD/PD patients. In this study patients with skin cGVHD are those with the highest rate of CR/PR after ECP treatment.Conclusions: Our results suggest that ECP treatment is safe and effective for patients with predominantly skin, oral and liver cGVHD.

    • Lignin, which is the second most abundant biomass component and has carbon-rich phenolic content, is a promising renewable raw material for multiple applications, such as carbon fibers, adhesives, and emulsifiers. To use lignin efficiently, it is important to ensure its purity and homogeneity. As a result, the separation of lignin into fractions with high purity and narrow molecular-weight distributions is likely a prerequisite for several applications. Ultrafiltration using ceramic membranes has many advantages, including enabling direct lignin extraction from Kraft pulp cooking liquors without pH and temperature adjustment. One challenge with membrane filtration using such a system is the potential for reduced membrane performance over time, which is associated with fouling. In this study, LignoBoost Kraft lignin was fractionated using a ceramic membrane with a molecular weight cut-off of 1 kDa. The separation behavior during ultrafiltration fractionation was investigated and the antioxidant properties of the recovered low-molecular-weight (low-MW) lignin samples were evaluated. Using this model system, the permeate fluxes were unstable during the 100 h of membrane operation. However, a decrease in the average MW in the permeate over time was observed. The shift in MW was most pronounced for virgin membranes, while a more stable MW distribution was evident for membranes subjected to multiple cleaning cycles. According to 2D NMR analysis, low-MW lignin that was recovered after 100 h of operation, consisted of smaller lignin fragments, such as dimers and oligomers, with a high content of methoxy-groups. This was confirmed using the size exclusion chromatography method, which indicated an weigh average molecular weight in the range of 450–500 Da. 31P NMR spectroscopy showed that, despite the lower total content of phenolic OH groups, the low-MW sample had a higher proportion of non-condensed phenolic OH groups. The results of the antioxidant tests demonstrated the strong potential of lignin and its low-MW fraction as a natural antioxidant, particularly for lipid-containing systems. The low-MW lignin fraction showed better antioxidant activity than the non-fractionated LignoBoost lignin in the kinetic oxygen radical absorbance capacity (ORAC) test and demonstrated three-fold stronger inhibition of the substrate (fluorescein) than the reference antioxidant Trolox (a water-soluble derivative of vitamin E).

  • Innovation competition for nature-based solutions and ecosystem services in outdoor environments

    • Many outdoor environments are currently designed without consider natural-based solutions to promote ecosystem services, such as water regulation and pollination. In addition, there is an increasing trend in the use of non-natural and fossil materials, such as artificial grass and fall protection surfaces. The Swedish Environmental Protection Agency wants to see nature-based solutions being integrated into the planning of outdoor environments to a greater extent and therefore announced funding to conduct an innovation competition. The innovation competition for nature-based solutions and ecosystem services in outdoor environments was run as a project by IVL Swedish Environmental Research Institute (IVL) during the autumn of 2021. The competition’s implementation was a further development of the innovation competition on alternatives to artificial grass conducted by IVL in 2020–2021.The project included the following activities:Planning, launch, and marketing plan. Development of project plan, communication, and marketing plan, supporting documents, etc.Expressions of interest, where the project received contact information from 40 interested parties. These were given access to the competition documents, an invitation toa digital Q&A session, and regular reminders until the registration ended.Full registration, where the contestants were asked to describe their contributions in words and pictures. A total of 16 entries signed up.Screening. All entries underwent an initial review, where relevance to the competition was ensured and entries were divided into competition classes based on development and market maturity. All entries passed the screening and were divided into two competition classes.Examination by an expert group based on the screening criteria set by the project. The expert group began by making an initial assessment of the contributions, which identified questions and ambiguities. All the innovators were then interviewed before a final assessment was conducted and winners were selected.Final seminar with presentation of the winners and inspiration part focused on clients. To increase the benefits of the project and attract the right target group, the focus was on inspiration for potential buyers. More than 200 people signed up and120 participated live.Evaluation of the project and the final event was conducted together with the Swedish Environmental Protection Agency, Formas and Vinnova. Lessons from collaboration were focus for the discussion.In Phase 2, Vinnova and Formas invite the winning innovators to a discussion on whether they can offer additional support for the innovators.The winners of the competition were the entries Nyfiken på naturen (Curious About Nature) from Flora and Fauna, as well as Lekotoper (Playotopes) from Urbio and Örebromunicipality. Nyfiken på naturen is about creating educational gardens for children with special needs and Lekotoper is about creating green play environments that contribute with both play, learning, and ecosystem services.

    • The insulin-like growth factor (IGF)-IGF binding proteins (BP) and the pituitary-gonadal axes were investigated during ultra endurance exercise in 16 endurance-trained athletes (seven women). Median duration of the race was 6.3 days. Although food and drink were ad libitum, energy balance was negative. Blood samples were drawn before (PRE), at the end of (END) and 24 h after (POST24h) the race. Serum concentrations of total IGF-I (t-IGF-I) and free IGF-I (f-IGF-I) decreased by 33 (SD 38)% and 54 (19)%, respectively. The decrease in t-IGF-I appeared to be associated to the total energy deficit during the race. At END, the IGFBP-3 fragmentation and IGFBP-1 were increased but these changes did not predict changes in f-IGF-I. An increase in POST24h IGFBP-2 levels in women was the only sex difference. Testosterone was decreased by 67 (12)% in the men and estradiol became undetectable in the women without any detectable increase in LH and/or FSH. In conclusion ultra endurance exercise results in similar IGF-IGFBP responses in men and women reflecting a catabolic state. IGFBP-2 was the only exception, with increased levels in women after exercise. A concomitant decrease in gonadal hormones was not related to endocrine changes in the IGF-IGFBP axis but may be related to local changes in IGF-I expression.

    • Background: The last decade has seen a vast increase in the use of patient-reported outcomes. As patientreported outcomes are used in order to capture patients' perspectives of their health and illness, it is a prerequisite for accurate patient-reported outcome evaluations to use representative samples. In order to evaluate representativeness, the present study focussed on the comparison between participants and non-participants in the Swedish branch of the international study APPROACH-IS (Assessment of Patterns of Patient-Reported Outcomes in Adults with Congenital Heart disease - International Study), regarding demographic, clinical, and health status characteristics. Methods: Eligible patients for APPROACH-IS were identified and selected from SWEDCON, the Swedish registry for congenital heart disease (CHD). Overall, 912 eligible patients were identified, of whom 471 participated, 398 did not participate, and 43 were either unreachable or declined to participate in APPROACH-IS. The participants and nonparticipants were compared in terms of statistical significance and effect sizes. Results: Significant differences were observed between participants and non-participants for sex, age, primary diagnosis, number of cardiac operations, and fatigue; however, the effect sizes were in general small, except for the difference in primary diagnosis. No differences between the two groups were found in number of catheterisations, implanted device, the distribution of NYHA functional class, or health status and symptoms. Conclusions: This study shows that participants and non-participants are relatively comparable groups, which confirms the representativeness of the participants. The Swedish data from APPROACH-IS can therefore be reliably generalised to the population of adults with CHD in Sweden.

    • Background and objectives Mucositis is a common complication after allogeneic hematopoietic stem cell transplantation (HSCT), and is caused by a combination of conditioning-induced mucosal damage and severe neutropenia. The symptoms include oral and abdominal pain, inability to swallow food and fluids, and severe diarrhoea. Severe mucositis is associated with increased risk of Graft-versus-Host disease and infection. Granulocyte transfusions (GCX) could be a treatment option, and our objective was to study its feasibility and potential benefits. Material and methods This retrospective, single-centre study included 30 patients receiving GCX because of severe oral mucositis after HSCT during 2005-2017. Clinical outcome, response to GCX, change in opiate administration and adverse events were studied. Results Twenty-seven patients received GCX from donors pre-treated with steroids and G-CSF, and three from donors pre-treated with steroids only. Overall response was 83% (24/29 evaluable patients). Fifteen patients reached a complete response. In 14 of 24 responders, a reduction of the administration of opiate pain relief was seen. In eight patients this reduction was >= 50% of the dose. Adverse events (AEs) were reported in 14 cases, and were mild to moderate, and well manageable with symptomatic treatment. No life-threatening or fatal AEs were recorded. Conclusions These results indicate that GCX could be a safe and effective treatment for oral mucositis after HSCT with the potential to reduce the necessity of opiate analgesic treatment in this disorder. No severe AEs were seen in this study, but the risk for severe pulmonary AEs after GCX needs to be considered.

Here we report on the current inhomogeneous radionuclide distribution, on both large- and small scales (1 cm2 – 10ths of km2), which on average varies a factor of 4.

    • BACKGROUND: Multiple patient and clinical characteristics contribute to the variable outcome of spinal anesthesia (SPA). Acute reduction of cerebrospinal fluid (CSF) volume may alter the effect of SPA. The objective of the present study was to test if aspiration of 10 mL CSF immediately prior to SPA is associated with higher extent of sensory block. METHODS: Interventional cohort study. One hundred and two patients undergoing total hip arthroplasty (THA) were included. Fifty-one patients underwent sampling of 10 mL CSF prior to SPA (CSF aspiration group); 51 consecutive patients were used as controls. The primary outcome was the extent of sensory block to cold stimulus 20 minutes after injection of hyperbaric bupivacaine. Secondary outcome measures included duration of motor block and incidence of failed SPA. RESULTS: Acute reduction of CSF volume by 10 mL increased the extent of sensory anesthesia (mean thoracic level [T] 4.3+/-2.4 vs. 7.1+/-2.6, P<0.001). There were no significant between-group differences regarding motor block duration (P>/=0.30) or failed SPA (three of 51 [CSF aspiration group] vs. one of 51 [control group], P=0.31). In a retrospective data analysis, 10 of 13 patients in the CSF aspiration group who had previously received SPA had a higher sensory block after 10 mL CSF aspiration compared to the previous SPA (T4.1 [range, 0-11] vs. T8.2 [4-10], P<0.01). CONCLUSIONS: Acute reduction of CSF volume by 10 mL prior to SPA leads to a higher thoracic level of sensory block.

    • BACKGROUND: Proinflammatory mechanisms are implicated in pain states. Recent research indicates that patients with osteoarthritis (OA) with signs of central sensitization exhibit elevated cerebrospinal fluid (CSF) levels of interferon gamma-induced protein 10 (IP-10), Fms-related tyrosine kinase 1 (Flt-1), and monocyte chemoattractant protein 1 (MCP-1). METHODS: The current prospective cohort study, including 15 patients with OA, primarily aimed to evaluate associations among alterations in CSF IP-10, Flt-1, MCP-1, and pain sensitization following total hip arthroplasty (THA). Participants provided CSF and blood samples for analysis of 10 proinflammatory mediators, and underwent detailed clinical examination and quantitative sensory testing, immediately preoperative and 18 months after surgery. RESULTS: Neurophysiological measures of pain showed markedly reduced pain sensitivity long-term postoperative. Increases in remote site pressure pain detection thresholds (PPDTs) and decreased temporal summation indicated partial resolution of previous central sensitization. Compared to preoperative, CSF concentrations of IP-10 were increased (p = 0.041), whereas neither Flt-1 (p = 0.112) nor MCP-1 levels changed (p = 0.650). Compared to preoperative, plasma concentrations of IP-10 were increased (p = 0.006), whereas interleukin (IL)-8 was decreased (p = 0.023). Subjects who exhibited increases in arm PPDTs above median showed greater increases in CSF IP-10 compared to those with PPDT increases below median (p = 0.028). Analyses of plasma IP-10 and IL-8 indicated higher levels of peripheral inflammation were linked to decreased pressure pain thresholds (unadjusted beta = -0.79, p = 0.006, and beta = -118.1, p = 0.014, respectively). CONCLUSIONS: THA leads to long-term decreases in pain sensitivity, indicative of resolution of sensitization processes. Changes in CSF and plasma levels of IP-10, and plasma IL-8, may be associated with altered pain phenotype.

    • Neuroinflammation is implicated in the development and maintenance of persistent pain states, but there are limited data linking cerebrospinal fluid (CSF) inflammatory mediators with neurophysiological pain processes in humans. In a prospective observational study, CSF inflammatory mediators were compared between patients with osteoarthritis (OA) who were undergoing total hip arthroplasty due to disabling pain symptoms (n = 52) and pain-free comparison controls (n = 30). In OA patients only, detailed clinical examination and quantitative sensory testing were completed. Cerebrospinal fluid samples were analyzed for 10 proinflammatory mediators using Meso Scale Discovery platform. Compared to controls, OA patients had higher CSF levels of interleukin 8 (IL-8) (P = 0.002), intercellular adhesion molecule 1 (P = 0.007), and vascular cell adhesion molecule 1 (P = 0.006). Osteoarthritis patients with central sensitization possibly indicated by arm pressure pain detection threshold <250 kPa showed significantly higher CSF levels of Fms-related tyrosine kinase 1 (Flt-1) (P = 0.044) and interferon gamma-induced protein 10 (IP-10) (P = 0.024), as compared to subjects with PPDT above that threshold. In patients reporting pain numerical rating scale score >/=3/10 during peripheral venous cannulation, Flt-1 was elevated (P = 0.025), and in patients with punctate stimulus wind-up ratio >/=2, CSF monocyte chemoattractant protein 1 was higher (P = 0.011). Multiple logistic regression models showed that increased Flt-1 was associated with central sensitization, assessed by remote-site PPDT and peripheral venous cannulation pain, and monocyte chemoattractant protein-1 with temporal summation in the area of maximum pain. Multiple proinflammatory mediators measured in CSF are associated with persistent hip OA-related pain. Pain phenotype may be influenced by specific CSF neuroinflammatory profiles.

    • BACKGROUND: Sleep disturbance is thought to aggravate acute postoperative pain. The influence of preoperative sleep problems on pain control in the long-term and development of chronic postsurgical pain is largely unknown. METHODS: This prospective, observational study aimed to examine the links between preoperative sleep disturbance (Pittsburgh Sleep Quality Index, PSQI) and pain severity (Brief Pain Inventory, BPI) 6 months postoperative (primary outcome), objective measures of pain and postoperative pain control variables (secondary outcomes). Patients (n = 52) with disabling osteoarthritis (OA) pain undergoing total hip arthroplasty (THA) were included. Quantitative sensory testing (QST) was performed preoperatively on the day of surgery to evaluate pain objectively. Clinical data, as well as measures of sleep quality and pain, were obtained preoperatively and longitudinally over a 6-month period. RESULTS: Preoperatively, sleep disturbance (i.e., PSQI score >5) occurred in 73.1% (n = 38) of THA patients, and pain severity was high (BPI pain severity 5.4 +/- 1.3). Regression models, adjusting for relevant covariates, showed that preoperative PSQI score predicted pain severity 6 months postoperative (beta = 0.091 (95% CI 0.001-0.181), p = .048, R(2) = 0.35). Poor sleep quality was associated with increased pressure pain sensitivity and impaired endogenous pain inhibitory capacity (R(2) range 0.14-0.33, all p's < 0.04). Moreover, preoperative sleep disturbance predicted increased opioid treatment during the first 24 hr after surgery (unadjusted beta = 0.009 (95% CI 0.002-0.015) mg/kg, p = .007, R(2) = 0.15). CONCLUSIONS: Preoperative sleep disturbance is prevalent in THA patients, is associated with objective measures of pain severity, and independently predicts immediate postoperative opioid treatment and poorer long-term pain control in patients who have undergone THA. SIGNIFICANCE: Poor sleep quality and impaired sleep continuity are associated with heightened pain sensitivity, but previous work has not evaluated whether preoperative sleep problems impact long-term postoperative pain outcomes. Here, we show that sleep difficulties prior to total hip arthroplasty adversely predict postoperative pain control 6 months after surgery. Given sleep difficulties robustly predict pain outcomes, targeting and improving sleep may have salutary effects on postoperative pain reports and management.

Although pain is the most disabling symptom of osteoarthritis (OA), limited knowledge exists regarding the CNS mechanisms that amplify pain and drive sensitization processes in humans.Objectives:The main objective of this study was to evaluate associations between cerebrospinal fluid (CSF) monoamine metabolites, pain severity, and central sensitization in patients with OA undergoing total hip arthroplasty (THA).Methods:Patients with OA (n = 52) and pain-free controls (n = 30) provided CSF samples for measurement of serotonin (5-hydroxyindoleacetic acid [5-HIAA]), noradrenaline (3-methoxy-4-hydroxyphenylglycol [HMPG]), and dopamine (homovanillic acid [HVA]) monoamine metabolites. Patients with OA completed longitudinal evaluation of pain using clinical measures and quantitative sensory testing.Results:Patients with OA had higher HMPG levels when compared with controls (P = 0.036). Within patients with OA undergoing THA, higher 5-HIAA and HVA levels were consistently associated with higher preoperative pain severity. Higher concentrations of 5-HIAA and HVA were also associated with lower conditioned pain modulation levels, whereas higher HMPG levels were linked to more efficient conditioned pain modulation. Patients with higher levels of CSF HVA exhibited increased pressure pain sensitivity (arm pressure pain detection threshold < 250 kPa vs ≥ 250 kPa, P = 0.042). Higher preoperative levels of CSF 5-HIAA predicted poorer pain control 6 months postoperatively (brief pain inventory pain severity; adjusted β = 0.010, 95% CI 0.001-0.019).Conclusions:In OA patients with disabling pain, higher CSF levels of serotonin and dopamine metabolites are associated with increased pain severity and central sensitization. Increased noradrenergic activity may be associated with more efficient pain inhibitory capacity.

    • Physiological and medical effects of snuff have previously been obtained either in cross-sectional studies or after snuff administration to non-tobacco users. The effects of snuff cessation after several years of daily use are unknown. 24 participants with >2 years of daily snuff-use were tested before and after >6 weeks snuff cessation (SCG). A control group (CO) of 11 snuff users kept their normal habits. Resting heart rate (HR) and blood pressure (BP) were significantly lower in SCG after snuff cessation, and body mass was increased by 1.4 +/- 1.7 kg. Total cholesterol increased from 4.12 +/- 0.54 (95% CI 3.89-4.35) to 4.46 +/- 0.70 (95% CI 4.16-4.75) mM L-1 in SCG, due to increased LDL, and this change was significantly different from CO. Resting values of HDL, C-reactive protein, and free fatty acids (FFA) remained unchanged in both groups. In SCG group, both HR and BP were reduced during a four-stage incremental cycling test (from 50 to 80% of VO(2)max) and a prolonged cycling test (60 min at 50% of VO(2)max). Oxygen uptake (VO2), respiratory exchange ratio, blood lactate (bLa) and blood glucose (bGlu) concentration, and rate of perceived exertion (RPE) were unchanged. In CO group, all measurements were unchanged. During the prolonged cycling test, FFA was reduced, but with no significant difference between groups. During the maximal treadmill running test peak values of VO2, pulmonary ventilation (VE), time to exhaustion and bLa were unchanged in both groups. In conclusion, endurance exercise performance (VO(2)dmax and maximal endurance time) does not seem to be affected by prolonged snuff use, while effects on cardiovascular risk factors are contradictory. HR and BP during rest and submaximal exercise are reduced after cessation of regular use of snuff. Evidently, the long-time adrenergic stress on circulation is reversible.

  • Infiltration of Air into two World Heritage Farmhouses in Sweden during Winter Conditions

    • As a part of an ongoing study, we report measurements of air infiltration during winter conditions into two Decorated Farmhouses of Hälsingland designated as UNESCO World Heritage Sites. In winter these two-storied farmhouses are rarely heated, except for special occasions. In this measurement one farmhouse  was  unheated,  whereas  one  room  was  heated  for  a  brief  period  in  the  other  one.  The observed local mean ages of air measured with tracer gas techniques generally increase with height, both  locally  within  each  room  and  between  floors.  The  average  temperature  and  humidity  also increases from the first to the second floor. The indoor temperature follows the outdoor temperature with a time lag. The differences in water content between inside and outside air correlate with changes of the indoor relative humidity. The correlation is stronger for humidity increase than for humidity decrease, possibly due to moisture absorption by interior text.

    • High local concentrations of a pollutant can be the result of high local emission rates of the pollutant or insufficient ventilation. Using tracer gases to map the ventilation in multi-zone buildings combined with measurements of the local pollutant concentration provide the means to discriminate between these causes. In a similar manner, the propagation rate of pollutants from a source to a target zone and the emission rate of the pollutants at the source may be determined quantitatively.The paper presents both the theoretical framework for the techniques and experimental examples of the proposed methods.

    • This investigation was designed to evaluate changes in plasma and muscle levels of free amino acids during an ultra-endurance exercise and following recovery. Nine male ultra-endurance trained athletes participated in a 24-h standardized endurance trial with controlled energy intake. The participants performed 12 sessions of running, kayaking and cycling (4 x each discipline). Blood samples were collected before, during and after exercise, as well as after 28 h of recovery. Muscle biopsies were taken 1 week before the test and after exercise, as well as after 28 h of recovery. During the 24-h exercise, plasma levels of branched-chain (BCAA), essential amino acids (EAA) and glutamine fell 13%, 14% and 19% (P<0.05) respectively, whereas their concentrations in muscle were unaltered. Simultaneously, tyrosine and phenylalanine levels rose 38% and 50% (P<0.05) in the plasma and 66% and 46% (P<0.05) in muscle, respectively. After the 24-h exercise, plasma levels of BCAA were positively correlated with muscle levels of glycogen (r2=0.73, P<0.05), as was the combined concentrations of muscle tyrosine and phenylalanine with plasma creatine kinase (r2=0.55, P<0.05). Following 28-h of recovery, plasma and muscle levels of amino acids had either returned to their initial levels or were elevated. In conclusion, ultra-endurance exercise caused significant changes elevations in plasma and muscle levels of tyrosine and phenylalanine, which suggest an increase in net muscle protein breakdown during exercise. There was a reduction in plasma concentrations of EAA and glutamine during exercise, whereas no changes were detected in their muscle concentration after exercise.

    • Regular exercise and a physically active lifestyle have favorable effects on health. Several issues related to this theme are addressed in this report. A comment on the requirements of personalized exercise medicine and in-depth biological profiling along with the opportunities that they offer is presented. This is followed by a brief overview of the evidence for the contributions of genetic differences to the ability to benefit from regular exercise. Subsequently, studies showing that mutations in TP53 influence exercise capacity in mice and humans are succinctly described. The evidence for effects of exercise on endothelial function in health and disease also is covered. Finally, changes in cardiac and skeletal muscle in response to exercise and their implications for patients with cardiac disease are summarized. Innovative research strategies are needed to define the molecular mechanisms involved in adaptation to exercise and to translate them into useful clinical and public health applications.

    • Background It has been suggested that cerebrospinal fluid (CSF) CXCL13 is a diagnostic marker of Lyme neuroborreliosis (LNB), as its levels have been shown to be significantly higher in LNB than in several other CNS infections. Levels have also been shown to decline after treatment with intravenous ceftriaxone, but levels after treatment with oral doxycycline have previously not been studied. Like Borrelia burgdorferi, HIV also has neurotropic properties. Elevated serum CXCL13 concentrations have been reported in HIV patients, but data on CSF levels are limited. Methods We longitudinally analysed CSF CXCL13 concentrations in 25 LNB patients before and after oral doxycycline treatment. Furthermore, we analysed CSF CXCL13 concentrations in 16 untreated LNB patients, 27 asymptomatic untreated HIV-1 infected patients and 39 controls with no signs of infectious or inflammatory disease. Results In the longitudinal LNB study, initially high CSF CXCL13 levels declined significantly after doxycycline treatment, which correlated to a decreased CSF mononuclear cell count. In the cross-sectional study, all the LNB patients had CSF CXCL13 levels elevated above the lowest standard point of the assay (7.8 pg/mL), with a median concentration of 500 pg/mL (range 34–11,678). Of the HIV patients, 52% had elevated CSF CXCL13 levels (median 10 pg/mL, range 0–498). There was a clear overlap in CSF CXCL13 concentrations between LNB patients and asymptomatic HIV patients. All but one of the 39 controls had CSF CXCL13 levels below 7.8 pg/mL. Conclusions We confirm previous reports of highly elevated CSF CXCL13 levels in LNB patients and that these levels decline after oral doxycycline treatment. The same pattern is seen for CSF mononuclear cells. CSF CXCL13 levels are elevated in neurologically asymptomatic HIV patients and the levels overlap those of LNB patients. The diagnostic value of CSF CXCL13 in LNB remains to be established.

    • Importance  Neurofilament light protein (NfL) is elevated in cerebrospinal fluid (CSF) of a number of neurological conditions compared with healthy controls (HC) and is a candidate biomarker for neuroaxonal damage. The influence of age and sex is largely unknown, and levels across neurological disorders have not been compared systematically to date.Objectives  To assess the associations of age, sex, and diagnosis with NfL in CSF (cNfL) and to evaluate its potential in discriminating clinically similar conditions.Data Sources  PubMed was searched for studies published between January 1, 2006, and January 1, 2016, reporting cNfL levels (using the search terms neurofilament light and cerebrospinal fluid) in neurological or psychiatric conditions and/or in HC.Study Selection  Studies reporting NfL levels measured in lumbar CSF using a commercially available immunoassay, as well as age and sex.Data Extraction and Synthesis  Individual-level data were requested from study authors. Generalized linear mixed-effects models were used to estimate the fixed effects of age, sex, and diagnosis on log-transformed NfL levels, with cohort of origin modeled as a random intercept.Main Outcome and Measure  The cNfL levels adjusted for age and sex across diagnoses.Results  Data were collected for 10 059 individuals (mean [SD] age, 59.7 [18.8] years; 54.1% female). Thirty-five diagnoses were identified, including inflammatory diseases of the central nervous system (n = 2795), dementias and predementia stages (n = 4284), parkinsonian disorders (n = 984), and HC (n = 1332). The cNfL was elevated compared with HC in a majority of neurological conditions studied. Highest levels were observed in cognitively impaired HIV-positive individuals (iHIV), amyotrophic lateral sclerosis, frontotemporal dementia (FTD), and Huntington disease. In 33.3% of diagnoses, including HC, multiple sclerosis, Alzheimer disease (AD), and Parkinson disease (PD), cNfL was higher in men than women. The cNfL increased with age in HC and a majority of neurological conditions, although the association was strongest in HC. The cNfL overlapped in most clinically similar diagnoses except for FTD and iHIV, which segregated from other dementias, and PD, which segregated from atypical parkinsonian syndromes.Conclusions and Relevance  These data support the use of cNfL as a biomarker of neuroaxonal damage and indicate that age-specific and sex-specific (and in some cases disease-specific) reference values may be needed. The cNfL has potential to assist the differentiation of FTD from AD and PD from atypical parkinsonian syndromes.

    • Amyloid precursor protein (APP) is the precursor protein to amyloid β (Aβ), the main constituent of senile plaques in Alzheimer's disease (AD). Endogenous Aβ peptides reflect the APP processing, and greater knowledge of different APP degradation pathways is important to understand the mechanism underlying AD pathology. When one analyzes longer Aβ peptides by low-energy collision-induced dissociation tandem mass spectrometry (MS/MS), mainly long b-fragments are observed, limiting the possibility to determine variations such as amino acid variants or post-translational modifications (PTMs) within the N-terminal half of the peptide. However, by using electron capture dissociation (ECD), we obtained a more comprehensive sequence coverage for several APP/Aβ peptide species, thus enabling a deeper characterization of possible variants and PTMs. Abnormal APP/Aβ processing has also been described in the lysosomal storage disease Niemann-Pick type C and the major large animal used for studying this disease is cat. By ECD MS/MS, a substitution of Asp7 → Glu in cat Aβ was identified. Further, sialylated core 1 like O-glycans at Tyr10, recently discovered in human Aβ (a previously unknown glycosylation type), were identified also in cat cerebrospinal fluid (CSF). It is therefore likely that this unusual type of glycosylation is common for (at least) species belonging to the magnorder Boreoeutheria. We here describe a detailed characterization of endogenous APP/Aβ peptide species in CSF by using an online top-down MS-based method.

    • The one-hour workshop, containing both a demonstration and hands-on experiments on the topic of nanosafety, is based on current science on a topic of general interest. The workshop aims to provide a deeper knowledge and understanding of nanoparticles. The participants get an introduction to what nanoparticles are, why nanosized materials are interesting, how nanomaterials interact with biological molecules, and potential risks associated with nanoparticles. Furthermore, by participating in the workshop the audience gains insights into how research about nanoparticles is conducted. The participants carry out experiments to demonstrate that daily-used plastic products can be disintegrated into particles in the nanometer size range, which may have important implications for the environment.

  • Enhanced image analysis, a tool for precision metrology in the micro and macro world

    • The need for high speed and cost efficient inspection in manufacturing lineshas led to a vast usage of camera-based vision systems. The performance ofthese systems is sufficient to determine shape and size, but hardly to an accuracylevel comparable with traditional metrology tools. To achieve highprecision shape/position/defect measurements, the camera techniques haveto be combined with high performance image metrology techniques whichare developed and adapted to the manufactured components. The focus ofthis thesis is the application of enhanced image analysis as a tool for highprecision metrology. Dedicated algorithms have been developed, tested andevaluated in three practical cases ranging from micro manufacturing at submicronprecision to meter sized aerospace components with precision requirementsin the 10 μm range.The latter measurement challenge was solved by low cost standard consumerproducts, i.e. digital cameras in a stereo configuration and structured lightfrom a gobo-projector. Combined with high-precision image analysis and anew approach in camera calibration and 3D reconstruction for precise 3Dshape measurement of meter sized surfaces, the achievement was fulfilledand verified by two conventional measurement systems; a high precisioncoordinate measurement machine and a laser scanner.The sub-micron challenge was the implementation of image metrology forverification of micro manufacturing installations within a joint Europeaninfrastructure network, EUMINAfab. The results were an unpleasant surprisefor some of the participating laboratories, but became a big step forwardto improve the dimensional accuracy of the investigated laser micromachining, micro milling and micro-printing systems, since the accuracy ofthese techniques are very difficult to assess.The third high precision metrology challenge was the measurement of longrange,low-amplitude topographic structures on specular (shiny) aerodynamicsurfaces. In this case Fringe Reflection Technique (FRT) was appliedand image analysis algorithms were used to evaluate the fringe deformationas a measure of the surface slopes to obtain high resolution data. The resultwas compared with an interferometric analysis showing height deviation inthe range of tens of micrometers over a lateral extension of several cm.

    • Volatile organic compounds (VOCs) are one of the most commonly detected contaminants in water. The occurrence is mainly in gasoline and other petroleum-based products, fumigants, paints and plastics. Releases into the environment and the widespread use have an impact on the ecosystem such as humans and animals due to their toxicity, mutagenicity, and carcinogenicity. VOCs may persist in groundwater and may enter drinking water supplies. In this paper, a diamond-like carbon (DLC)-coated silicon waveguide in combination with a polymer film (ethylene/propylene copolymer, E/P-co) for enrichment of analytes was investigated to determine its suitability for ATR-FTIR (attenuated total reflection Fourier transform infrared) spectroscopic detection of VOCs. The DLC film was fluorine-terminated enhancing the adhesion of the hydrophobic polymer to the waveguide surface. The analytes diffuse into the hydrophobic polymer whereas water is excluded from the emanating evanescent field. Therefore, direct detection in aqueous systems is enabled. Nine VOCs, i.e., ethylbenzene (EB), trichloroethylene (TCE), tetrachloroethylene (TeCE), the xylene isomers (p-xylene, pXYL; m-xylene, mXYL; o-xylene, oXYL), naphthalene (NAPH), toluene (TOL), and benzene (BENZ), were evaluated simultaneously qualitatively and quantitatively showing the potential of DLC coatings revealing high sensitivities in the low ppb to ppm concentration range, i.e., 50 ppb for TeCE. To the best of our knowledge, this is the first time of IR spectroscopic detection of VOCs in aqueous solutions using DLC-coated waveguides in combination with a hydrophobic polymer. By utilizing a DLC-coated waveguide, a versatile sensor for real-time monitoring in harsh environments such as effluents, leaking pipelines, and underground storage tanks is feasible due to response times within a few minutes.

    • En intressant trend i motions- och tävlingsvärlden är att extrema uthållighetsidrotter blir alltfler och dessa får dessutom alltfler utövare. Multisport (engelska Adventure Racing) är en av dessa. Tävlingarna består i allmänhet av löpning, cykling och paddling men ibland läggs också olika fysiska aktiviteter såsom repklättring, inlines och simning in i tävlingen.
