    • In Sweden, personal assistance is one form of support for persons with certain functional impairment. The aim of personal assistance is to give compensatory support to individuals so that they can enjoy the right to live a life like anyone else. This article explores what kind of support parents who have functional impairments and apply for personal assistance may need when they have underage children. The article is focused on the following: What support needs in parenting are expressed in personal assistance applications and by whom? How are the expressed needs in parenting met by the intervention personal assistance, i.e. how do the decisions correspond with the expressed needs? The article is based on qualitative and quantitative document-analyses of 100 randomly selected applications for the right to personal assistance submitted to the Swedish Social Insurance Agency by applicants who were parents with children 0–17 years of age, during the years 2014–2017. The central concepts resulting from the analyses of the empirical material were support and care. The results show that in many cases the children were invisible in the material studied. In several investigations, there was no information at all about the applicant’s children. Many applications for assistance were refused. Few parents received assistance in their parenting role. In the documents, we found the following themes in statements about needs for support related to the role as a parent: practical support, care and supervision, support with communication, emotional support and safety, and support to be involved in the children’s lives.

  • Lack of Coordination between Health Care and Social Care in Multi-Professional Teamwork : the Obstacle for Coherent Care of Older People Suffering from Multi-Morbidity

    • The lack of a cohesive health and social care is a well-known problem of significance for ageing people in general and frail older people in particular. Responsibility for organising and conducting social care and healthcare for the elderly rests on different principals in different countries but difficulties with organisational coordination and collaboration between professions and authorities in social care and healthcare is an extensive concern worldwide. Regardless of the distribution of responsibilities, collaboration and coordination structures are complex and often lead to problems. However, the gap in the coordination between different organisations and the collaboration between professions, implying that frail older people with major care needs still living in their own homes are pinched, has received hardly any recognition. By closely following an implementation project focused on teamwork in order to improve collaboration and coordination between social care and healthcare, the purpose of this article is to fill this gap with the help of an example from Sweden. Data consisted of event diaries, observations, focus groups, structured questionnaires and interviews. Findings showed that multi-professional teams certainly were established, but did not work or last. Among the obstacles found the most prominent features were the various professions’ own organisations, territorial thinking and rivalries. The whole idea of the initiative to achieve a cohesive healthcare and social care for ageing frail older people fell through. By letting this happen, not only did the project hinder the development of better practice in serving older adults, but also cemented the problematic structures it was intended to dissolve.

  • Family caregivers and decision-making for older people with dementia

    • This article addresses the dilemmas concerning legislation, individual autonomy and the reality of everyday life for people coping with dementia. We describe and analyse decision-making in relation to older people with dementia in Sweden, within the area of social work regulated by the Social Services Act and the Parental Act. Swedish legislation is based on the individual's autonomy and capacity to consent to services without anyone having legal authority to decide on behalf of the individual. Based on data from interviews with family caregivers living at home, decision-making through family caregivers is discussed and formal guardianship is also considered. Swedish legislation leaves individuals with dementia and family caregivers in a vacuum between self-determination and full autonomy with the ideal of citizenship emphasised and recognised in the Social Services Act on the one hand, and on the other, a strong need for support in everyday life and with decision-making.

  • Personlig assistans och föräldraskap : Barnperspektiv i personlig assistans

    • I rapporten redovisas en studie om personlig assistans och föräldraskap i situationen då föräldrar med minderåriga barn har funktionsnedsättningar. Studien har genomförts med forskningsmedel från Försäkringskassans öppna utlysning. Syftet med studien var att undersöka vilket stöd föräldrar som har personlig assistans behöver för att kunna utöva föräldraskap på lika villkor som andra. Mer specifikt var avsikten att undersöka hur stöd i föräldraskap som ges genom personlig assistans utformas. Rapporten bygger på två delstudier. Den ena delstudien bygger på dokumentanalyser av ett slumpmässigt urval av Försäkringskassans utredningar (N=100) av rätten till assistansersättning för föräldrar med barn under 18 år. Den andra delstudien är en intervjustudie som bygger på intervjuer med föräldrar (N=11) och personliga assistenter (N=9) om deras erfarenheter av personlig assistans i föräldraskap.Resultaten av dokumentstudien visar att det i 31 procent av akterna inte hade dokumenterats att den sökande hade minderåriga barn. Av de sökande beviljades 26 procent assistansersättning och av dessa beviljades 9 procent specifik assistanstid för att vara föräldrar. Det var en högre andel män än kvinnor som fick rätt till assistansersättning. Analyser av handläggningsprocessen visar att inte alla sökande hade fått möjlighet att vid ett möte med handläggare beskriva sina behov. En förklaring till att få sökande fick rätt till assistans i föräldraskap var att föräldraskap inte inkluderas i definitionen av grundläggande behov, utan betraktas som ett annat personligt behov, vilket kan bli aktuellt först om den sökande bedöms ha grundläggande behov om i genomsnitt mer än 20 timmar per vecka.Av ansökningarna framgick att det fanns dokumentation om upplevda behov av assistans i föräldraskap. Behoven beskrevs av föräldrarna själva och intygas av professionella inom vården. Likartade behov har beskrivits av föräldrar och personliga assistenter i intervjustudien. Det handlar om behov av assistans med praktisk hjälp såsom hushållsarbete, omsorg och tillsyn av yngre barn, samt assistans för att kunna vara delaktig i barnens vardagsliv och kunna ge dem stimulans och aktivitet. Dessa identifierade områden kan förstås som grundläggande för barns omsorg och överensstämmer med de behov som tas upp i Socialstyrelsens handläggningsstöd Barns behov icentrum (BBIC), vilket används vid socialtjänstens barnutredningar. Det var också flera av de intervjuade föräldrarna som tog upp att omsorg om barn borde ingå i definitionen av grundläggande behov. Studien har visat exempel på att när samhällets insatser brister, och det inte heller finns ett stödjande nätverk som kan träda in, händer det att såväl minderåriga barn som unga vuxna får ta på sig uppgiften att ge omsorg till sina föräldrar. Studien visar på behovet av implementering av barnperspektiv i LSS där hänsyn tas till denna grupp föräldrar och barn.

    • The care for persons with dementia is involving a diversity of social and health care interventions. The systems for dementia care and services vary across European countries. However, there is a lack of knowledge about the complex system for dementia care on a European level. A more comprehensive view of dementia care provision in Europe may provide knowledge that can serve as a basis for the development of best practice in the care for persons with dementia. The objective of the study is to describe and analyse the European health and social care and welfare systems, advocacy and informal caregiver support systems for people with dementia. The presentation will focus on how data will be collected and how collaboration within the consortium will be performed.A consortium including members from eight European countries is invited in the study. The research teams that will collect the data represent Estonia, Finland, France, Germany, Netherlands, Spain, Sweden and United Kingdom. The Swedish research team coordinates the study. The consortium members will review a suggested terminology for overall issues regarding the care system for people with dementia as well as national directives and nancing. Further, data will comprise the terminology regarding descriptions of stages in the dementia trajectory, types of care provided and care providers. From this process, an overall agreement on the meaning of relevant terminology is going to be obtained, and this will make it possible to describe the health and social care system for older persons with dementia in a European perspective. A developed template will be used to collect data regarding health and social care and welfare system from each country included in the study.

  • Barns och ungdomars bilder av poliser : En studie i två lokala sammanhang

    • SammanfattningI fokus för denna licentiatuppsats är barns och ungdomars bilder av poliser. Syftet är att beskriva dessa bilder och placera dem i en senmodern samhällelig kontext. Vägledande i beskrivningarna är tre meningsbärande begrepp – föreställningar, erfarenheter och förväntningar – vilka har utkristalliserats ur det empiriska materialet. Utgångspunkten är att bilderna är socialt konstruerade, med hänsyn till maktordning, habitus, sociala positioner och symboliskt kapital. För att förstå den betydelse unga tillskriver poliser är ansatsen därtill kulturellt sociologisk.  Studien bygger på ett enkätmaterial med 1 945 barn och ungdomar utan specifik erfarenhet av kriminella handlingar, i åldern 10-22 år. 35 grundskolor och 11 gymnasieskolor har deltagit och svarsfrekvensen är 58 procent i vardera kommunen. Dessutom genomfördes samtalsintervjuer i grupp med sammanlagt 50 deltagare i direkt anslutning till enkätstudien. Analysmetoden är inspirerad av tematisk analys.Materialet visar att barns och ungdomars uppfattning om Polisen som institution är positiv, men att de är något mer kritiska till Polisen som organisation och betydligt mer kritiska till enskilda poliser. Den övervägande andelen av deltagarna har varit i kontakt med poliser, främst i skolan eller när de har ansökt om pass. Förväntningarna på poliser är att de ska rikta sin verksamhet mot vuxna. Den yngre gruppen, 10-12 år är i större utsträckning, positiva till poliser. Den något äldre gruppen, 13-16 år, är mer negativa, medan den äldsta gruppen, 17-22 år, har en betydligt mer nyanserad bild av poliser.Vidare visar det sig att föreställningar, erfarenheter och förväntningar intimt hänger samman. Föreställningar bygger på olika erfarenheter, vilket i sin tur genererar olika förväntningar mot bakgrund av olika förutsättningar i tillvaron. En del bilder är mer generellt förekommande medan andra framställs främst i mer avgränsade kategoriserade grupper med unga.Ungas bilder kan tolkas som en kamp inom fältet där habitus och symboliskt kapital utgör maktmedel. Om vi väljer att se på unga som seismografer kan deras bilder vara tecken på en allmänt hållen bild av poliser även bland vuxna. Det visar sig också finnas en mystik runt poliser. Samtidigt som de kan upplevas vara i vägen är det spännande att se dem agera i skarpt läge. Skrönor spär på mystiken samtidigt som Polisens sätt att vara förtegna om hur de verkar, bidrar till de bilder vi har av dem.

    • Becoming alone in old age can be a decisive life event that brings major changes depending on various causes as health status, financial resources, family situation, and available welfare services. This article discusses the situation of older people in Sweden who have transitioned from a two-person to single-person household in recent years and what impact this might have on their everyday lives. Through in-depth interviews with 18 older people, age 67–90, their experiences about life conditions and opportunities were examined. Findings showed large differences between the men and women. They all tried to live as they always had done and they used the same personal life strategies that they always had. But the men could live as before on their own financial merits, while the women needed assistance from children, grandchildren and the welfare system. Transportation options were central and clearly related to both private economy and social services available. Shortcomings in the welfare state's way of caring for the elderly were clearly uncovered. The gap between social policy promises of opportunities for autonomy and independence to live an active life in old age and the everyday reality for older people still seems to be wide.

    • Challenges with an ageing population are increasingly becoming a reality in the Western world. Since cognitive impairment increases with age, we can expect an increasing number of older people in need of care. The aim of this article is to describe, analyse and compare different focuses on care of older people with dementia, using examples from France, Portugal and Sweden. The questions are principally focused on the participants’ view about their tasks, the organisation of work, the professional role and cooperation with other professions. Everyday care was studied through observations and participant observations and the staff's opinion was explored by means of interviews. Twenty-two care settings for older people were included. The findings showed that France provided mainly ‘health care’, Sweden ‘social care’ and Portugal an integrated ‘health care and social care’. In a comparative perspective the Portuguese general care of older people, which focuses on integration of health care, social care and social work, also seems to provide care for older people suffering from dementia which best corresponds to the previously developed group living model.

    • Many countries in the Western world are facing great challenges posed by an ageing population. Since cognitive impairments and the prevalence of dementia increases with age, we can expect an increasing number of older people in need of care. However, severe problems with such care are already reported by researchers as well as in the mass media. The aim of this paper is to discuss how the identity confusion among older people suffering from dementia might influence relationships and the staff's behaviour, and in that sense, be part of the care problems reported. Three studies carried out between 1993 and 2003 are introduced aimed at illustrating the severe care problems identified. The interaction between the residents and the staff is focused on and discussed from a psychodynamic viewpoint. Data show that the caregivers also become confused and express a need of affirmation comparable with that of the older people suffering from dementia. That is, even the caregivers' behaviour seems to be influenced by an identity‐preserving ambition similar to the one of the dementia sufferers, and this might explain some of the problems in this form of care.

    • The gap between theory and practice in social work continues to be great, and research results seem to live a life of their own, far from the field they were intended to improve. This article considers research information as a useful contribution to the practice of social work with elderly demented people and to qualitative methods used in the area of social scientific research. Arising out of a project aimed at studying the usefulness of research information through an interactive exercise within focus groups, the author discusses how the results of research can be used to improve practice. When the researchers’ theoretical- oriented perspectives meet with the practitioners’ action-oriented perspectives, two different models enrich each other in the process of developing integrated knowledge.

    • The number of elderly people is gradually increasing, and with it the number of persons receiving some kind of care in their everyday lives. However, researchers and the mass media recurrently report great difficulties for many caregivers in their work situations. This article considers process-oriented supervision as a pedagogic method for training and support of nurse aids and managers in Swedish eldercare. With reference to a developmental project, the author discusses the clash between the supervisor's process-oriented manner and the staffs' problem-oriented attitudes. This shows that supervision in this form may not necessarily be an effective way of reforming care. Instead the caregivers point out organisational changes and better leadership as more adequate methods.
