  • Mortality outcomes with hydroxychloroquine and chloroquine in COVID-19 from an international collaborative meta-analysis of randomized trials

    • Substantial COVID-19 research investment has been allocated to randomized clinical trials (RCTs) on hydroxychloroquine/chloroquine, which currently face recruitment challenges or early discontinuation. We aim to estimate the effects of hydroxychloroquine and chloroquine on survival in COVID-19 from all currently available RCT evidence, published and unpublished. We present a rapid meta-analysis of ongoing, completed, or discontinued RCTs on hydroxychloroquine or chloroquine treatment for any COVID-19 patients (protocol: https://osf.io/QESV4/). We systematically identified unpublished RCTs (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform, Cochrane COVID-registry up to June 11, 2020), and published RCTs (PubMed, medRxiv and bioRxiv up to October 16, 2020). All-cause mortality has been extracted (publications/preprints) or requested from investigators and combined in random-effects meta-analyses, calculating odds ratios (ORs) with 95% confidence intervals (CIs), separately for hydroxychloroquine and chloroquine. Prespecified subgroup analyses include patient setting, diagnostic confirmation, control type, and publication status. Sixty-three trials were potentially eligible. We included 14 unpublished trials (1308 patients) and 14 publications/preprints (9011 patients). Results for hydroxychloroquine are dominated by RECOVERY and WHO SOLIDARITY, two highly pragmatic trials, which employed relatively high doses and included 4716 and 1853 patients, respectively (67% of the total sample size). The combined OR on all-cause mortality for hydroxychloroquine is 1.11 (95% CI: 1.02, 1.20; I-2=0%; 26 trials; 10,012 patients) and for chloroquine 1.77 (95%CI: 0.15, 21.13, I-2=0%; 4 trials; 307 patients). We identified no subgroup effects. We found that treatment with hydroxychloroquine is associated with increased mortality in COVID-19 patients, and there is no benefit of chloroquine. Findings have unclear generalizability to outpatients, children, pregnant women, and people with comorbidities. Hydroxychloroquine and chloroquine have been investigated as a potential treatment for Covid-19 in several clinical trials. Here the authors report a meta-analysis of published and unpublished trials, and show that treatment with hydroxychloroquine for patients with Covid-19 was associated with increased mortality, and there was no benefit from chloroquine.

  • Efficacy of Prolonged- and Immediate-release Tacrolimus in Kidney Transplantation : A Pooled Analysis of Two Large, Randomized, Controlled Trials

    • Background. Two large, prospective studies (12-03; OSAKA) compared the efficacy and tolerability of prolonged-release versus immediate-release tacrolimus in kidney transplant patients also receiving mycophenolate mofetil and low-dose corticosteroids (without induction therapy). Methods. Data were combined into one database to compare results over 24 weeks using 3 alternative endpoints: biopsy-confirmed acute rejection (BCAR); the Food and Drug Administration composite endpoint (graft loss, SCAR, and loss to follow-up), and the European Medicines Agency composite endpoint (graft loss, BCAR, and graft dysfunction). The 95% confidence intervals were calculated (10% noninferiority margin). Results. Overall, 633 patients received prolonged-release tacrolimus (12-03, n = 331; OSAKA, n = 302) and 645 received immediate-release tacrolimus (n = 336; n = 309). Baseline characteristics were comparable. Proportionately more patients receiving prolonged-release tacrolimus had trough levels of 5-15 ng/mL on day 1 (60.8%) and 2 (56.6%) versus immediate-release tacrolimus (42.5% and 43.9%, respectively, both P < .001). Efficacy of prolonged-release and immediate-release tacrolimus were similar as assessed by BCAR (13.9% vs 14.1%, respectively), European Medicines Agency composite endpoint (40.3% vs 38.3%) and US Food and Drug Administration composite endpoint (21.5% vs 19.8%). Conclusions. Novel efficacy endpoints as required by the European Medicines Agency and US Food and Drug Administration demonstrate noninferiority of prolonged-release versus immediate-release tacrolimus. Significantly more patients treated with prolonged release tacrolimus versus immediate-release tacrolimus achieved trough levels of 5 to 15 ng/mL early after transplantation.

    • We describe small-sized specimens of the metailurine felid Dinofelis from a new Plio-Pleistocene site in North Africa. Dinofelis is a genus of saber-toothed cats mainly recorded from East and South Africa with numerous leopard to jaguar-sized species. The described specimens, clearly smaller than all the other African Dinofelis, resemble isolated remains from the Late Pliocene of France and the Early Pleistocene of Africa. Present evidence suggests that our form represents a new species and/or new lineage of Dinofelis, smaller and probably occupying a different ecological niche compared to the previously known members of the genus, and thus it adds complexity to the high intraspecific competition among large carnivorans in the Plio-Pleistocene of Africa.

    • Medicinal plants cultivation has grown all over the world. This paper studies the role of Medicinal plants cultivation in sustainable development. The research takes the city of Fortaleza in Brazil as a case study. The results show that the proposed Living Pharmacy Program, which has been implanting for more than 20 years since 1990 in the municipality of Fortaleza, is enlarging the access of the population to the medicinal plants and physiotherapeutic medications, awakening their ecological conscience and thus contributing to economic and sustainable development of the municipality.

    • Removal of trace organic contaminants from aqueous solutions by air strippers (AS) and fixed bed carbon adsorper (FBCA) has been studied. A trace organic treatment tool has been developed to capture and adapt the best-known designprocedures and to have all regulated trace organics, their physical and chemical properties, and the corresponding maximum concentration limits. Outputs include the selected treatment method and the final design parameters of airstripper or fixed bed carbon adsorber. Running the model shows that water temperature is a very important factor in designing AS and FBA. It also shows that the best air pressure values, in AS, ranged between 150 -200 ATM. And there is a big relation between the column size and the packing material. On the other hand, it shows that FBA diameter has an obvious effect on the needed volume, and the best values ranged between 1.2 – 2.5 m.

    • Despite their imperative role in water resources management, distributed hydrological models like SWAT require calibration that can be challenging due to uncertainties of parameters involved. Prior to modelling of hydrological processes, these parameters and their uncertainty range need to be identified. The objective of this study was to conduct uncertainty analysis of hydrological processes and to calibrate the Soil and Water Assessment Tool (SWAT) for stream flow and sediment yield modelling in Thika River catchment. Sequential Uncertainty Fitting program (SUFI-2) was used to conduct sensitivity and uncertainty analysis. Stream flow was calibrated and validated between the years 1998 to 2013 for gauging stations 4CB05 and 4CB04. Manual sediments calibration was achieved by constraining the MUSLE parameters using the bathymetric survey data. Two uncertainty indices, p and r factor, were obtained as 0.72 and 0.65, 0.65 and 0.45 during calibration and validation, respectively. Statisticalperformance indicators showed a good match between the observed and simulated values which indicated that the model was well calibrated for simulation of stream flow and sediments yield in the catchment.

    • The widely used stainless steels and their deformed variants are anticorrosive in ambient conditions due to passivation layers composed of chromium oxides. Conventionally, corrosion and erosion of the steels are attributed to the breakdown of such layers but seldomly to the origin that depends on surface heterogeneity at the microscopic level. In this work, the nanometer-scaled chemical heterogeneity at the surface unveiled via spectro-microscopy and chemometric analysis unexpectedly dominates the breakdown and corrosion behavior of the cold-rolled Ce-modified 2507 super-duplex stainless steels (SDSS) over its hot-deformed counterpart. Though relatively uniformly covered by a native Cr2O3 layer revealed by X-ray photoemission electron microscopy, the cold-rolled SDSS behaved poorly in passivity because of locally distributed Fe3+ rich nano-islands over the Fe/Cr oxide layer. This atomic-level knowledge provides a deep understanding of corrosion of stainless steel and is expected to benefit corrosion controls of similar high-alloyed metals.
