  • Laser-induced fluorescence diagnostics of basal cell carcinomas of the skin following topical ALA application

    • Fourteen patients with superficial basal cell carcinomas (BCCs) and fifteen patients with nodular BCCs were investigated by means of laser-induced fluorescence (LIF) in connection with photodynamic therapy (PDT). Topical application of delta-amino levulinic acid (ALA) was performed six hours prior to the treatment session. Fluorescence spectra were recorded, using a point-monitoring system with an excitation wavelength of 405 nm. The measurements were performed in scans over the lesion and the surrounding normal skin before application of ALA, and immediately before and after the laser treatment. The selective uptake of the photosensitiser resulted in a fluorescence intensity ratio of 2.4:1 for superficial BCCs and 2.5:1 for nodular BCCs. If the fluorescence intensity was divided by the autofluorescence, this resulted in a contrast enhancement of about a factor 6 for tumour tissue. In seven patients (five with nodular BCC and two with superficial BCC), additional fluorescence measurements were performed two and four hours following the ALA application, and two hours after the PDT procedure. Thus, the kinetics of the transformation of ACA to protoporphyrin IX (PpIX) could be followed, which indicated that the synthesis of PpIX was more rapid in the tumour than in the normal tissue. After four hours, the PpIX level inside the tumour was saturated, while there still was an accumulation in the surrounding skin. The highest contrast between tumour and normal skin was reached within two hours after the ALA application.

  • Impact on follow-up strategies in patients with primary sclerosing cholangitis

    • BACKGROUND & AIMS: Evidence for the benefit of scheduled imaging for early detection of hepatobiliary malignancies in primary sclerosing cholangitis (PSC) is limited. We aimed to compare different follow-up strategies in PSC with the hypothesis that regular imaging improves survival.METHODS: We collected retrospective data from 2,975 PSC patients from 27 centers. Patients were followed from the start of scheduled imaging or in case of clinical follow-up from January 1, 2000, until death or last clinical follow-up alive. The primary endpoint was all-cause mortality.RESULTS: A broad variety of different follow-up strategies were reported. All except one center used regular imaging, ultrasound (US) and/or magnetic resonance imaging (MRI). Two centers used scheduled ERCP in addition to imaging for surveillance purposes. The overall HR (CI95%) for death, adjusted for sex, age and start year of follow-up, were 0.61 (0.47-0.80) for scheduled imaging with and without ERCP; 0.64 (0.48-0.86) for US/MRI and 0.53 (0.37-0.75) for follow-up strategies including scheduled ERCP. The lower risk of death remained for scheduled imaging with and without ERCP after adjustment for cholangiocarcinoma (CCA) or high-grade dysplasia as a time-dependent covariate, HR 0.57 (0.44-0.75). Hepatobiliary malignancy was diagnosed in 175 (5.9%) of the patients at 7.9 years follow-up. Asymptomatic patients (25%) with CCA had better survival if scheduled imaging had been performed.CONCLUSIONS: Follow-up strategies vary considerably across centers. Scheduled imaging was associated with improved survival. Multiple factors may contribute to this result including early tumor detection and increased endoscopic treatment of asymptomatic benign biliary strictures.

    • Dymola and Modelica has been used at KTH Vehicle Dynamics (KTHVD) for research work since 2000, see e.g. [1]. With the Vehicle Dynamics Library (VDL) [2], Modelica has become far more accessible for both researchers and students in the field of vehicle dynamics. Therefore a project aiming at introducing it as a tool in education was initiated in order to evaluate the current state of Dymola and Modelica as tools for wider use in education at the division. The work presented in this paper was realized as a part of a PhD course, where one of the tasks were to design dedicated exercises to illustrate fundamentals of vehicle dynamics for students.

    • Neural stem cells reside in defined areas of the adult mammalian brain, including the dentate gyrus of the hippocampus. Rat neural stem/progenitor cells (NSPCs) isolated from this region retain their multipotency in vitro and in vivo after grafting into the adult brain. Recent studies have shown that endogenous or grafted NSPCs are activated after an injury and migrate toward lesioned areas. In these areas, reactive astrocytes are present and secrete numerous molecules and growth factors; however, it is not currently known whether reactive astrocytes can influence the lineage selection of NSPCs. We investigated whether reactive astrocytes could affect the differentiation, proliferation, and survival of adult NSPCs by modelling astrogliosis in vitro, using mechanical lesion of primary astrocytes. Initially, it was found that conditioned medium from lesioned astrocytes induced astrocytic differentiation of NSPCs without affecting neuronal or oligodendrocytic differentiation. In addition, NSPCs in coculture with lesioned astrocytes also displayed increased astrocytic differentiation and some of these NSPC-derived astrocytes participated in glial scar formation in vitro. When proliferation and survival of NSPCs were analyzed, no differential effects were observed between lesioned and nonlesioned astrocytes. To investigate the molecular mechanisms of the astrocyte-inducing activity, the expression of two potent inducers of astroglial differentiation, ciliary neurotrophic factor and leukemia inhibitory factor, was analyzed by Western blot and shown to be up-regulated in conditioned medium from lesioned astrocytes. These results demonstrate that lesioned astrocytes can induce astroglial differentiation of NSPCs and provide a mechanism for astroglial differentiation of these cells following brain injury.

    • This study focuses on how science teachers' Pedagogical content knowledge (PCK) can be captured and developed with the support of Content representation (CoRe) in combination with video-based reflection when they plan, teach, and reflect on their teaching in sustainable development (SD). The theoretical framework is based on PCK, and the Refined consensus model (RCM) is used as a theoretical lens for conceptualising links between teaching practice and PCK development. Eleven upper-secondary school science teachers participated in the study. Data from two teachers were chosen to illustrate representative examples from the analysis. The findings indicate that the use of CoRe and video-based reflection provides opportunities for teachers to make their knowledge of practice explicit and help them begin to identify aspects within their own practice that are important for the further development of teaching SD. The use of RCM as an analytical tool also makes an important contribution to how RCM can be used to identify and capture teachers' PCK.

    • It is vital that upper secondary students gain a rich and deep knowledge of sustainability issues, as they will enter adulthood and working life within a short time. The students belong to a generation that will be intensely involved in managing several environmental issues to achieve sustainable development for our Earth. However, earlier research indicates that many teachers have a low self-efficacy for teaching sustainability issues. This study aimed to explore how science teacher teams in Swedish upper secondary schools can develop their knowledge and expertise in sustainable development (SD) through collective reflections with the support of the reflective tool Content Representation (CoRe). Science teachers' pedagogical content knowledge (PCK) development in teaching SD was examined. Twelve in-service science teachers participated in the study. The qualitative research design included semi-structured interviews and science teacher teams' collective meetings. The findings covered four themes about how the collective reflections with support of the CoRe tool stimulated teachers' PCK development: (1) creating structure and a focus for learning conversations, (2) modification of the teaching approach, (3) development of new content knowledge and pedagogical knowledge in SD, and (4) shared language to stimulate equal opportunities for students to learn SD. The conclusion is that with the support of CoRe, the teacher team's collective PCK in SD was developed; therefore, we recommend that decision-makers within the school system organize regular meetings in science teacher teams and introduce CoRe to the teachers.

    • The pursuit of sustainable development is a necessity for life on Earth and the future of humanity. Education is important in this endeavour; therefore, creating knowledge about teachers' teaching for sustainable development is essential. This study aimed to investigate how Swedish upper secondary science teachers adjust their teaching of the environmental dimension in sustainable development to the different vocational education programmes their students attend. Data were collected through individual semi-structured interviews with 15 in-service science teachers and analysed by thematic coding. The result showed that most teachers (but not all) adjusted their teaching to the students' vocational educational programme; however, they did it to different extents and in different ways. The result presents teachers' descriptions of and arguments for adjusting their teaching linked to the pedagogical content knowledge (PCK) framework. The PCK framework is used as a theoretical lens in this study. Further research is needed to investigate how science teachers' adjustments to students' vocational education affect the students' knowledge of the environmental dimension in sustainable development and how that affects actions in their future professional life.

This paper aims to examine how teachers describe the content in SD and how they justify this content concerning their teaching practice. The Pedagogical Content Knowledge (PCK) framework, with its reflective tool Content Representation (CoRe), was used in combination with semi-structured interviews to capture 18 Swedish upper-secondary science teachers' reflections. The results indicate four themes of content that the teachers emphasise: energy and global warming, ecosystem services and biodiversity, presence of harmful substances and materials in nature, and imbalance of natural substances in nature. The teachers justified these content themes in terms of the importance of students' gaining different perspectives (local, global, ecological, social, and economic), belief in the future, action competence, and general scientific education. The paper contributes to sustainability education research, as it focuses on teaching sustainability in the light of the PCK framework.     • Optical properties of human whole blood were measured in vitro, at 633 nm with a double integrating sphere set-up, The blood was kept at constant flow through a flow cell while slowly heating the blood from approximately 25 degrees C to 55 degrees C in a heat exchanger, The results show a small but distinct decrease in the g-factor of 1.7 +/- 0.6\% and a similar increase in the scattering coefficient, mu(s), of 2.9 +/- 0.6\% at approximately 45-46 degrees C. When studying the thermal effect on the blood cells under a white-light transmission microscope, the changes in the scattering properties could be correlated to a sudden change in the shape of the red blood cells, from disc-shaped to spherical, at approximately the same temperature. Furthermore, a continuous manifest increase in the absorption coefficient, mu(a), was seen with temperature rise, on average 83.8 +/- 68.1\% when reaching the temperature 50 degrees C. This might be due to heat-induced haemolysis of the red blood cells, resulting in free light absorbing haemoglobin in the surrounding plasma and thus higher effective light absorption.

    • During the past decade, early childhood teachers have been faced with new needs to develop their content knowledge and pedagogi- cal content knowledge (PCK) for different science subject areas. In order to meet these challenges there is a strong need for professional learning programmes for early childhood teachers that focus on the development of knowledge, and skills to work with science activities in their own context. In this article, 46 early childhood teachers participated in a professional development programme aimed at helping teachers capture and further develop their content knowl- edge and PCK for teaching science through the use of content representations (CoRes). A CoRe is a holistic reflective tool for making explicit the different dimensions of, and links between, knowledge of content, teaching, and learning about a particular topic. The aim of the project was to investigate in what ways the use of the CoRe helped the teachers in planning and reflecting on their science teaching and in developing their PCK. Thus, the project was based on opportunities for teachers to develop and reframe their knowl- edge of science teaching in ways that stimulated children's curiosity for scientific phenomena. The result indicates that with its content- specific focus, the CoRe helped the teachers to highlight the science content in their activities, not only the general pedagogical knowl- edge. The use of the CoRes also helped the preschool teachers to establish the fundamental ideas of the topic they were teaching and to develop confidence in what they were teaching, why, and how. © 2017 Association for Science Teacher Education

  • Vadstena krigsmanshus : En studie av den svenska kronans inrättning för sårade och gamla soldater cirka 1640–1780

    • Den här avhandlingen handlar det underhåll till sårade och gamla soldater som den svenskastatsmakten organiserade under tidigmodern tid. Undersökningen tar sin utgångspunkt i den tid närstatsmakten började organisera mer omfattande hjälp för soldater, vilket var slutet på Gustav Vasasoch under Erik XIV:s regeringstid i mitten av 1500-talet. Motiven till att statsmakten organiseradehjälp för en del soldater var att dessa skulle ha offrat sin hälsa och arbetsförmåga i kronans tjänst, menså länge statsmakten fortsatte att vara relativt löst organiserad var även hjälpen till soldaterna avganska liten omfattning. Det ändrades under Gustav II Adolfs regering då planerna på att skapa ettkrigsmanshus i Vadstena etablerades, vilka senare genomfördes efter hans död och institutionen kundeta emot de första soldaterna senast 1640. I krigsmanshuset fick ett trettiotal soldater med familjeruppehälle, samtidigt som soldater boende i andra delar av landet fick stöd från krigsmanshuskassansom också administrerades från Vadstena. Mot slutet av 1600-talet blev kassan proportionellt merbetydelsefull än krigsmanshuset och runt år 1700 försörjde kassan flera tusen soldater runtom i riket.1700-talet innebar stora förändringar, bland annat genom att krigen blev färre, att krigaryrket intelängre var lika attraktivt som karriärväg för adelsmän och att statsmakten utvecklade andra mer civiladelar. Behovet av ett krigsmanshus fanns till sist inte längre och institutionen i Vadstena stängde förboende våren 1784, men kassan fanns kvar in på 1970-talet.

  • Medication dosage calculation among nursing students : does digital technology make a difference? A literature review.

    • BACKGROUND: Patient safety is a major part of nursing care and following patients' medication orders is considered one of the greatest responsibilities of individual nurses and nursing Failure to make safe drug calculations poses serious risks to patient safety. It is therefore important to strengthen nursing students' numeracy skills and conceptual abilities during their education. Research suggests that digital technologies play an increasingly important role in promoting nursing students' knowledge and medication dosage calculation (MDC) skills. The present review aims to identify and critically evaluate research investigating how the use of digital technologies informs the development of nursing students' MDC skills.METHODS: A systematic literature review was performed within Scopus (Elsevier), Academic Search Elite (Ebsco), Cinahl (Ebsco), ERIC (Ebsco), Web of Science and PubMed. Research papers on MDC using digital technologies were considered for inclusion. Starting from 2843 sources, eighteen research articles met the inclusion criteria.RESULTS: The results show that use of digital technologies can reduce nursing students' medication errors. Interestingly, web-based courses were the most commonly used digital technologies aimed at developing nursing students' MDC skills. However, such courses had limited impacts the development of these skills.CONCLUSION: The present review concludes by mapping the current knowledge gaps and making suggestions for further research.

  • Att sänka cyklisters hastighet på cykelbanor : acceptans, konsekvenser och förutsättningar

    • Syftet med projektet var att öka kunskapen om hur hastighetssänkande åtgärder på cykelvägar kan användas och vilka effekter de kan ha. Litteraturstudier och intervjuer gjordes för att samla aktuell kunskap. Fältstudier genomfördes med hastighetsmätningar för att studera den eventuella effekten av en skyltad hastighetsbegränsning till 20 km/h och vägkantsintervjuer för att undersöka cyklisters acceptans för olika typer av hastighetsbegränsningar på cykelbanor. Resultaten visade att skyltningen inte tycktes ge någon hastighetssänkning, vilket delvis kan förklaras av att de flesta cyklar saknar hastighetsmätare. Intervjuerna visade att cyklister är skeptiska till hastighetssänkande åtgärder eftersom de ofta utgör en säkerhetsrisk. I sammanhanget är det viktigt att skilja på reshastighet respektive punkthastighet. Genomsnittlig punkthastighet hos cyklister på sträcka ligger vanligtvis runt 20 km/h med få som cyklar snabbare än 30 km/h. Reshastigheten som inbegriper stopp och väntetider är oftast lägre. För att uppnå målen om god tillgänglighet och ökad cykling bör cyklister erbjudas en hög reshastighet. Situationer där det är motiverat att begränsa cyklisters punkthastighet är vid dålig sikt, i anslutning till vägarbeten, på gångytor eller cykelbanor förbi t.ex. skolor. Fasta hinder, ojämnheter, avsmalningar eller skarpa kurvor för att begränsa cyklisters hastighet bör inte användas eftersom de bidrar till högre olycks- och skaderisk. Via Trafikförordningen är cyklisters hastighet redan reglerad med regler som är generella för alla fordon och vägar. Det går inte att förbudsskylta med lägre hastighet än 30 km/h på cykelvägar, men med anvisningsskyltar kan lägre hastigheter rekommenderas. Kunskapen om trafiksäkerhetseffekter av cyklisters hastighet är begränsad. Tidigare forskning pekar i vissa fall på att ”hög fart” kan ha bidragit till cykelolyckor som inträffat, dock utan att definiera vilken hastighet som avses, medan andra studier inte kunnat visa på några samband.

    • We herein outline the rationale for a Swedish cohort consortium, aiming to facilitate greater use of Swedish cohorts for world-class research. Coordination of all Swedish prospective population-based cohorts in a common infrastructure would enable more precise research findings and facilitate research on rare exposures and outcomes, leading to better utilization of study participants' data, better return of funders' investments, and higher benefit to patients and populations. We motivate the proposed infrastructure partly by lessons learned from a pilot study encompassing data from 21 cohorts. We envisage a standing Swedish cohort consortium that would drive development of epidemiological research methods and strengthen the Swedish as well as international epidemiological competence, community, and competitiveness.

    • Development of strategies that allow for identification of secreted factors in conditioned media is of significance for a wide range of research areas. Secreted factors are involved in intercellular communication and might also be biomarkers of potential clinical importance, used for early detection and diagnosis of disease. The aim of this study was to investigate whether metabolic labeling combined with mass spectrometry could be used to identify secreted proteins in serum-containing conditioned medium. Earlier proteomic studies of conditioned media have been performed on cells cultured in serum-free media. In the present study the fact that only the proteins derived from the cells contain the incorporated isotopically labeled amino acid was taken advantage of, making it possible to differentiate released proteins from medium proteins. The second objective was to examine whether any quantitative differences in the secretion profiles between primary astrocytes and astrocytes in a scratch injury model of reactive astrogliosis could be found. To our knowledge, this is the first study to identify secreted proteins in serum-containing medium using a proteomic approach involving stable isotope labeling by amino acids in cell cultures and mass spectrometry.
