  • Effects of cold and hand-arm vibration on the peripheral neurosensory and vascular system : an occupational perspective

    • Background In Swedish working life, exposure to cold and exposure to hand-arm vibration (HAV) are two common health hazards. Health effects of HAV in the neurosensory, vascular and musculoskeletal systems are collectively denoted hand-arm vibration syndrome (HAVS), and have been thoroughly studied. Effects of cold exposure in terms of effects on the peripheral neurosensory and vascular system are on the contrary limited, especially in an occupational setting. Effects of cold exposure or cold injury have not previously been assessed with quantitative sensory testing (QST). Commonly reported symptoms after exposure to HAV and after cold injuries, includes cold sensitivity and sensation of cold. Cold sensitivity can also occur without previous exposure to vibration or cold and may have a major impact on quality of life. Other possible risk factors for cold sensitivity need to be assessed. Sensation of cold hands could theoretically imply an early manifestation of damage to the neurosensory or vascular system, and therefore be of importance to enable early detection of vascular and neurosensory HAVS. The purpose of this thesis was to increase the knowledge about health effects from cold and HAV on the peripheral neurosensory and vascular system, with an occupational perspective. The aims were: first, to identify and evaluate health effects and sequelae in the peripheral neurosensory and vascular system due to cold injury and cold exposure; second, to investigate if sensation of cold hands is a predictor for future onset of Raynaud's phenomenon or paresthesia; and third, to identify possible risk factors associated with cold sensitivity.Methods A case series on 15 military conscripts with local cold injuries in the hands or feet, involving QST and symptom descriptions, was conducted to investigate the hypothesis that cold injuries can result in similar neurosensory and vascular impairments as in HAVS. To assess health effects of cold exposure, a cohort study on 54 military conscripts in cold winter military training, with cold exposure assessments, was conducted. Possible health effects were assessed after 14 months of military training, containing considerable cold exposure, by means of QST, Finger systolic blood pressure after local cooling (FSBP) and a questionnaire. To investigate if sensation of cold hands is a predictor for vascular or neurosensory HAVS we investigated a cohort of 178 employees at a manufacturing company where HAV was a common exposure. The cohort was followed during 21 years and both vibration exposure and health outcomes were assessed regularly. Questionnaire items were used to assess sensations of cold hands as well as signs of Raynaud’s phenomenon and paresthesia. To identify risk factors for cold sensitivity a case-control study was conducted involving 997iiiparticipants from the general population in northern Sweden. The study was cross-sectional and explored possible risk factors for cold sensitivity.Results Cold injuries and cold exposure were associated with reduced sensibility in QST and increase severity and prevalence of neurosensory and vascular symptoms. Our results did not show any impairment in peripheral blood flow due to cold exposure, detectable by FSBP. The risk of developing Raynaud's phenomenon was increased for workers previously reporting sensation of cold hands (OR 6.3, 95% CI 2.3-17.0). No increased risk for paresthesia in relation to a sensation of cold hands was observed. The identified risk factors for cold sensitivity were frostbite in the hands, rheumatic disease, nerve injury in upper extremities or neck, migraine and vascular disease. When analysing women and men separately, women’s risk factors were frostbite in the hands, rheumatic disease, migraine and cold exposure. Men’s risk factors were frostbite in the hands, vibration exposure and nerve injury in upper extremities or neck. BMI > 25 was a protective factor for both men and women.Conclusion Cold injury and cold exposure are associated with impairments in the neurosensory system, detectable by QST. Symptoms such as sensation of cold hands and white fingers indicate vascular involvement, even though no vascular impairments due to cold exposure could be detected by objective measurements. A sensation of cold hands is a risk factor for development of Raynaud´s phenomenon, but not for paresthesia. At the individual level, reporting cold hands does not appear to be useful information when considering the possibility of a future development of Raynaud’s phenomenon. Frostbite in the hands is a risk factor for cold sensitivity among both women and men. For women rheumatic disease, migraine and cold exposure are also independent risk factors, and for men, exposure to HAV. Being overweight is a protective factor for both women and men.

  • Taking Stock of Knowledge Transfer Studies: Finding Ways Forward

    • Knowledge transfer (KT) from academia to practice is important in many fields, but comprehensive studies on identifying the most effective forms of KT are scarce. This paper aims to provide an overview of KT theory and presents a cross-disciplinary scoping review of empirically oriented peer-reviewed articles. The review offers guidance for researchers seeking to communicate effectively with practitioners. It explores the effects of research communications, delves into the understanding and measurement of these effects, attempts to identify the most effective forms of communication, and highlights important considerations when designing KT strategies. Few studies in our sample (eight of 27) systematically measured effects of KT, and merely four studies compared multiple forms of KT. Nevertheless, most studies estimated effects from KT, regardless of the chosen form (e.g., workshops or lectures). Most studies estimated knowledge change as the primary outcome. Additionally, several studies explored altered beliefs such as increased self-efficacy. A third of the studies addressed how the knowledge was applied, ranging from sharing information to developing new habits. The identified effects were, however, both small and volatile. Our findings underscore the significance of continuity and repeated interactions to enhance the impact of KT initiatives. Furthermore, researchers need to develop a comprehensive set of tools to facilitate successful KT, considering factors such as expertise, communication skills, trust-building, and participant-centered approaches. By employing these strategies, researchers can effectively bridge the gap between academia and practice, facilitating successful KT in various fields.

  • Beliefs, social identity, and the view of opponents in Swedish carnivore management policy

    • In the policy sciences, the intractability of disputes in natural resource governance is commonly explained in terms of a “devil shift” between rival policy coalitions. In a devil shift, policy actors overestimate the power of their opponents and exaggerate the differences between their own and their opponents’ policy beliefs. While the devil shift is widely recognized in policy research, knowledge of its causes and solutions remains limited. Drawing insights from the advocacy coalition framework and social identity theory, we empirically explore beliefs and social identity as two potential drivers of the devil shift. Next, we investigate the potential of collaborative venues to decrease the devil shift over time. These assumptions are tested through statistical analyses of longitudinal survey data targeting actors involved in three policy subsystems within Swedish large carnivore management. Our evidence shows, first, that the devil shift is more pronounced if coalitions are defined by shared beliefs rather than by shared identity. Second, our study shows that participation in collaborative venues does not reduce the devil shift over time. We end by proposing methodological and theoretical steps to advance knowledge of the devil shift in contested policy subsystems.

  • The effect of personal values and the roles on representational principles in natural resource management decision-making

    • The aim of this article is to explore how personal values and the role of the representative influence representation principles when making decisions in natural resource management. This was tested in an empirical case of wildlife management in Sweden, the regional Wildlife Conservation Committees (WCCs). These WCCs consist of a mix of actors in collaborative settings, where both politicians and interest organization representatives make decisions on wildlife related issues. The results show that the value dimension of self-enhancement, associated with giving importance to values such as achievement and power, significantly affects a representational style associated with following the representative's personal preferences when making decisions, the trustee principle. The role of the representative also significantly affects representational style when making decisions in these cases, where the interest organizational representatives more often follow the party principle, i.e., the view of the parties or organization they represent, than the political actors. Age also had a significant impact where older representatives relied more on the trustee principle than their younger peers. The implications of these results are that personal values in this case matters for decision-making, which is in line with earlier research on decision-making on environmental issues. Further, politicians behave atypically for the Swedish context relying more on the trustee principle rather than the party principle, which policy makers should take into consideration when designing collaborative arenas similar to the WCCs.

  • What logics drive the choices of public decision-makers?

    • This thesis explores what logics drive the decisions of public decision-makers. More specifically, and drawing on new institutional theory, this topic is investigated from the perspective of how institutions, i.e. the formal and informal patterns of rules and practices, influence public decision-makers. New institutionalism has a variety of ideas on how this decision-making occurs and recent research in this tradition emphasizes the importance that context has for the empirical support of these ideas. At the same time, studies exploring, contrasting and converging new institutional ideas, and how these vary depending on context i.e. their conditionality, are lacking. In this thesis, I set out to address this knowledge gap and, moreover, to examine the role of personal values for the new institutional ideas on how institutions affect the public decision-makers. Personal values have not been emphasized in new institutional studies but successfully explained decision-making from other perspectives. By adding this dimension, I seek to explore whether individual factors, in this case the personal values the public decision-makers bring with them into the institutional context, affect the way they make decisions. Consequently, the aim of this thesis is to explore what decision-making logics that are at play among public-decision-makers and how this varies depending on context and personal values. This exploration is conducted by deriving and testing hypotheses on decision-making, from rational choice institutionalism and sociological institutionalism, in two different contexts, parliaments and collaborative management, within the same national arena. Parliaments have a homogenous composition of actors, i.e. politicians, whereas collaborative management arenas are constituted by the inclusion of both public and private actors in decision-making, resulting in a more heterogeneous composition. Through a study of local parliaments and wildlife conservation committees (a form of collaborative management on the regional level) in Sweden, the aim of the thesis is fulfilled by survey and interview analyses of decision-making in regards to different policy issues. The results show that there are different decision-making logics at play in the parliamentary case compared with the collaborative management case. Further, personal values influence the decision-making logics among the public decision-makers. The implications of these results are, firstly, that the conditionality of new institutionalism, as suggested in earlier research, is empirically prevalent in the studied cases and, secondly, that personal values play a role for what decision-making logics that are at play. Further research is encouraged to delve deeper into the results, preferably through qualitative studies that could complement the primarily quantitative focus of this thesis, and through studies of other national contexts than Sweden.

    • How do different multi-level governance models influence the adaptive capacity of environmental management? This paper examines the connection between different types of governance models, distinguished by diverse institutional features, and elements of adaptive capacity. The task is undertaken through a comparative study of two differently organized management systems within the same national context: Swedish water and large carnivore management. The systems’ governance models are defined through an institutional analysis of polycentric features, logics of design and knowledge arrangements. Assessments of adaptive capacity are based on survey data describing the involved actors' perceptions of the knowledge base, use of an experimental approach and the presence of learning. The empirical results suggest that institutional features influence some, but not all, elements of adaptive capacity. The results lend support to the idea that polycentric governance models, based on an ecological rationale, sustain participation in knowledge mobilization, support the use of an experimental approach and promote learning to a larger extent than more centralized and hierarchical governance models do; while there is no connection between governance model and the perceived reliability of knowledge base. The study contributes to environmental governance research, policy and practice by evaluating the adaptive capacity of current water and wildlife management systems in Sweden and by increasing our knowledge about how different governance models influence the adaptive capacity in environmental management.

  • The link between collaborative governance design and markers of legitimacy: Comparing Swedish water- and large carnivore management

    • This paper addresses how the design of collaborative regimes influences markers of legitimacy in Swedish water- and large carnivore governance. Based on institutional analysis and statistical analysis of survey data, the study examines two systems with different types of collaborative designs and compares them in relation to markers of legitimacy, in terms of perceived process quality, policy agreement and policy acceptance among the involved decision-making actors and concerned organizations. The findings show how the design of collaboration influences some, but not all, explored markers of legitimacy. First, the categories of actors involved; whom they are accountable to; and the authority given to the collaborative forums, effect perceptions of influence, the possibility of reaching joint agreements and the degree of policy agreement among involved decision-makers. Second, the findings indicate that the degree of policy agreement among concerned organizations is unaffected by differences in institutional design. Third, the degree of policy acceptance among involved decision-making actors and concerned organizations was unaffected by institutional design and notably high in both systems. The results both verify and develop previous research findings and the message to policymakers is to carefully consider the design when introducing new collaborative forums, including whom to invite, in what role, and with what mandate.
