  • Olfactory Function : The Influence of Demographic, Cognitive, and Genetic Factors

    • Olfactory function is affected by demographic, cognitive, and genetic factors. In the present thesis, three empirical studies investigated individual differences in olfactory ability. Study I explored demographic and cognitive correlates in common olfactory tasks; odor detection, odor discrimination, and odor identification. The results indicated that old age influenced performance negatively in all tasks, and that semantic memory proficiency and executive functioning were related to odor discrimination and odor identification performance. No cognitive influence was observed for measurements of olfactory threshold. Using population-based data, Study II investigated a potential influence of the ApoE gene on olfactory identification after controlling for health status, semantic memory, and preclinical and clinical dementia. The main finding was that the ApoE- ɛ4 allele interacted with age, such that older ɛ4-carriers had an impaired odor identification performance relative to older non-carriers. Importantly, the negative ApoE- ɛ4 effect on olfactory proficiency was independent of clinical dementia conversion within five years. Study III investigated the effects of the BDNF val66met polymorphism on olfactory change over a five-year interval, in a community dwelling sample of young and old age cohorts. The results showed that age-related decline in olfactory identification was influenced by the BDNF val66met. In middle-aged subjects, no effect of BDNF val66met was observed although older val homozygote carriers showed a selectively larger olfactory decline than the older met carriers. Overall, results suggest that the relative influence of demographic and cognitive factors vary across different olfactory tasks and that two genes (ApoE and BDNF) impact age-related deficits in odor identification. Potential theoretical and practical implications of the findings are discussed as well as potential limitations of association studies in genomics research.

  • Effects of Testing and Enactment on Memory

    • Learning occurs not only when we encode information but also when we test our memory for this information at a later time. In three empirical studies, I investigated the individual and combined effects of interleaved testing (via repeated rounds of study and test practice) and encoding (via motor enactment) during learning on later cued-recall performance for action phrases. Such materials (e.g., “water the flowers”) contain a verb and a noun and approximate everyday memory that typically revolves around past and future actions. Study I demonstrated that both interleaved testing (vs. study only) and enactment (vs. verbal encoding) individually reduced the forgetting rate over a period of 1 week, but these effects were nonadditive. That is, the direct testing effect on the forgetting rate occurred for verbal, but not for enactive encoding; enactment reduced the forgetting rate for the study-only condition, but not for the study–test condition. A possible explanation of these findings is that both study techniques sufficiently elicit verb–noun relational processing that cannot be increased further by combining them. In Studies II and III, I replicated these testing-effect results and investigated whether they varied as a function of recall type (i.e., noun-cued recall of verbs and verb-cued recall of nouns). For verbal encoding (Study II), the direct testing effect was of similar size for both noun- and verb-cued recall. For enactive encoding, the direct testing effect was lacking irrespective of recall type. In addition, interleaved tests enhanced subsequent re-encoding of action phrases, leading to an accelerated learning. This indirect testing effect was increased for the noun-cued recall of verbs—for both verbal and enactive encoding. A possible explanation is that because nouns are semantically more stable, in that the meaning of nouns changes less over time and across different contexts, they are more recognizable. Hence, associated information (e.g., about the recall status) may be more available to the learner during restudy that, in turn, can initiate more effective re-encoding. The two different testing benefits (i.e., direct and indirect) may, partly, engage different mechanisms, as they were influenced differentially by the manipulations of encoding type and recall type. The findings presented in the thesis provide new knowledge regarding the combined effects of strategies and materials that influence memory.

  • Blindness and Second Language Acquisition : Studies of Cognitive Advantages in Blind L1 and L2 speakers

    • The aim of this study is to investigate whether blind individuals display cognitive advantages over sighted individuals with regard to second language acquisition. Previous studies from neuropsychology have indicated that this is the case. It has been found that blind L1 speakers can compensate for loss of vision by developing better perceptual and cognitive skills compared to sighted individuals, skills that are highly relevant to language acquisition. These studies do not, however, investigate blind L2 speakers, for whom it is not clear whether these advantages are also found.  In all, 80 adults participated in the study: 40 L2 speakers of Swedish (11 early blind, 9 late blind, 20 sighted, AO<18) and a matching group and subgroups of L1 speakers. These speakers underwent tests on speech perception in noise, accentedness in an L2 and memory functions. The results revealed that L2 speakers are at a great disadvantage perceiving speech in noise compared to L1 speakers, and that there was no advantage associated with blindness. In the L1 speakers group, however, the results revealed that the early blind had advantages compared to the late blind and sighted in white noise, but that both blind groups were more negatively affected by babble noise than the sighted. The results in relation to accentedness in an L2 revealed that there were no advantages associated with blindness. The results further revealed there were no advantages associated with blindness on the episodic memory test. The results did, however, reveal that the early blind performed significantly better than the late blind and sighted on all phonological short-term memory tests and that both the early and late blind were significantly better than the sighted on recognition memory for new words, irrespective of language background. The conclusion is that blindness is associated with advantages in, for example, ability to learn new words and syntax, acquisition rate, ultimate L2 attainment, and language aptitude.

  • Subjective Cognitive Complaints in the Working Population : The Influence of Objective Cognitive Functioning and Working Conditions

    • Cognitive functioning is important for managing work and life in general. However, subjective cognitive complaints (SCC), involving self-perceived difficulties with concentration, memory, decision making, and clear thinking are common in the general and in the working population and can be coupled with both lowered well-being and work ability. The present thesis investigated the extent to which SCC among people in the work force can be explained by objective cognitive functioning (study I & II) and working conditions (study III), utilizing samples from the working population. The potential roles of other common psychological problems which often co-occur with SCC were also investigated in studies I-III.In Study I, high levels of SCC were associated with significantly poorer episodic memory performance during high executive demands and a trend was found towards poorer episodic memory, while not being associated with semantic memory. In Study II, high levels of SCC were associated with significantly poorer executive cognitive performance on all three executive cognitive tests used. Symptoms of depression, chronic stress and sleeping problems were found to play an important role in the relations between SCC and episodic memory during divided attention in study I and executive cognitive functioning in study II. In Study III, in all cross-sectional data analyses, high quantitative demands, information and communication technology (ICT) demands, underqualification in the work situation and inter-personal conflicts were positively associated with SCC, whereas social support, good resources at work and overqualification in the work situation were negatively associated with SCC. In all prospective data analyses, quantitative job demands, ICT demands and underqualification were positively associated with future SCC, including when adjusted for baseline cognitive complaints.The findings may guide prevention of and interventions for SCC among people in the work force.

  • Individual differences in the aging memory : Mediation accounts, moderators, and contextual factors

    • Within the field of cognitive aging, mediation accounts propose that age affects cognitive abilities through a mediator variable. Most of these mediation accounts are developed based on studies with cross-sectional designs. We had access to data from Betula, a longitudinal population-based multi-cohort project, and tested, in Study I, the well-known processing speed account (general age-related slowing of mental processing speed affects cognitive abilities negatively) (Salthouse, 1996). Interestingly, no support was found for the speed account. In Study II, a second mediation theory was tested, the common cause account (Lindenberger & Baltes, 1994). This notion suggests a link between sensory and cognitive abilities, where both abilities decline with age in a similar fashion because of a third factor, a common cause. Again, no support for a major account of cognitive decline was found. In Study III, interactions including vascular health and genetic status (APOE status) as potential interacting predictors of cognitive development were examined. A difference in the distribution of interaction effects on episodic and semantic memory development was found. Study IV, finally, consisted of a comparison of cognitive aging in two very different countries, Bangladesh (Poverty and Health in Ageing) and Sweden (Betula). The findings were surprising since chronological age, in Bangladesh, did not exert much effect on declarative memory in older people, in contrast to Betula and most other aging studies, predominantly performed in the Western world. Results from these four studies are discussed with respect to theoretical implications and methodological considerations. Recommendations for future research focus are made and implications for explanatory models of cognitive aging are elaborated on.

  • Noise improves cognitive performance in children with dysfunctional neurotransmission

    • Research on children with Attention Deficit Hyperactivity Disorder (ADHD) has shown that they are extremely sensitive to distraction from external stimuli that lead to poor cognitive performance. This thesis shows that cognitive performance can be improved if this external stimulus is smooth and continuous (e.g. auditory white noise). Control children attenuate their performance under such conditions. The first Study proposes the moderate brain arousal model (MBA). This neurocomputational model predicts selective improvement from noise in ADHD children. Noise through a phenomenon called stochastic resonance (SR), can be beneficial in dopamine deprived neural systems. The statistical phenomenon of SR explains how the signal-to-noise ratio can be improved by noise in neural systems where the passing a threshold is required. The second Study provides experimental support for the MBA-model by showing that ADHD children improve performance in a free recall task while exposed to auditive noise. Control children declines in the same condition. The third Study generalizes this finding among low achieving children, which it is argued have low dopamine levels. Noise exposure improves performance in low achievers, but inhibits performance in high achievers. The conclusion is that external auditory noise can restore low dopamine levels and thus improve cognitive performance. It is also proposed that dopamine levels modulate the SR effect; this means that low dopamine persons require more noise to obtain an SR effect. Both excessive and insufficient dopamine is detrimental for cognitive performance. The MBA model can be used to explain several shortcomings where changes in the dopamine system have been identified. The MBA model can also help create appropriate and adaptive environments, especially in schools, for persons with a deficient dopamine function, such as ADHD children.

  • Effects of family configuration on cognitive functions and health across the adult life span

    • This thesis examines whether childhood family configuration influences performance on cognitive functions and health in adulthood and old age. All studies examined participants in the Betula Prospective Cohort Study aged 35 to 85 years (Nilsson et al., 1997). Study Ι established whether there are reliable effects of sibship size and birth order in a large sample of participants in adulthood and old age. The results showed that the effects previously demonstrated in children and adolescents (e.g., Belmont & Marolla, 1973; Mercy & Steelman, 1982) have a long-lasting effect and can be demonstrated in an adult sample. These studies concluded that intelligence and executive functioning decreased as the sibship size increased. Birth order, in contrast, had only influenced executive functions and working memory: earlier born siblings performed at a higher level than later born siblings. Study ΙΙ examined whether the effects of sibship size and birth order can be replicated and extended to episodic memory and whether the effects of family configuration are stable over a five-year interval. The results showed that early born siblings and siblings belonging to a smaller sibship size performed at a higher level and that these effects on both recall and recognition were stable over a five-year interval. Study ΙΙΙ explored whether childhood family configuration influences chronic adult diseases (myocardial infarction and circulatory disorders, stroke, and hypertension). The overall results showed that being born in a large sibship is a risk factor for stroke, myocardial infarction /circulatory disorders, and hypertension in old age. The results also suggest that being born early in a sibship is a predictor of stroke.

  • Bilingual memory : A lifespan approach

    • Bilingualism and its effect on individuals have been studied within different disciplines. Although the first psychological study of bilingualism was couducted by Cattell as early as 1887, only a few studies have exclusively investigated the effect of bilingualism on memory systems’ functioning. In the field of cognitive psychology of bilingualism, there is some evidence for the positive influence of bilingualism on children’s cognitive ability across various domains but there is little knowledge about the relationship between bilingualism and memory in a lifespan perspective. This thesis’s main aim was to investigate memory systems’ functioning and development in bilingual individuals. To this end, two studies were performed: a cross-sectional study of bilingual children (Study I) and a longitudinal study of young and older adults (Study II). The purpose of Studies I and II was to determine whether there are differences between monolinguals and bilinguals regarding various memory systems’ functioning. Study I compared monolingual and bilingual children’s performance on episodic and semantic memory, and Study II investigated performance on episodic and semantic memory in bilingual younger and older adults. Specifically, these studies aimed to examine a) which memory systems will be affected more as a function of language, and b) to what extent the differences would manifest themselves during a subject’s lifespan. The purpose of Study III was to explain the relation among word representations, lexical access and lexical selection in a bilingual word production paradigm. In this study, a model of bilingual production was developed to explain the results and clarify the role of automatic and controlled processes in using two languages. The results of Studies I and II showed a superiority of bilinguals over monolinguals as well as a variation of association between memory performance and bilingualism across different periods of adulthood. It appears that the lifelong experience of managing two languages enhances control processes, which in turn play an important role in enhancing memory performance. Using a “dual mechanism model”, Study III explains the efficiency of inhibitory processing when having two languages activated.

    • This dissertation examines whether some aspects of engaged lifestyle, marital status and leisure activity, influence memory performance in adulthood and old age. Direct effects and indirect effects, via health, are investigated. All the studies in the dissertation examine participants in the Betula project, aged 35 to 85 years. Study I investigates whether there are reliable effects of marital status on memory function in a large sample of participants in adulthood and old age. The results demonstrate that marriage has an influence on some specific types of memory functions. They show that there are significant differences between married and single individuals in episodic memory, but not in semantic memory. Also, the extent of decline in episodic memory was found to be significantly larger for singles and widowed individuals than for married people over five years. Study II examines the relationships between different types of social and cognitive activities and episodic and semantic memory. The results show that a unidirectional effect of social activity on episodic memory was detectable on all test occasions. Also, episodic memory predicted change in cognitive activity during all test waves. However, there were no significant effects with regard to semantic memory and leisure activity in either direction. Study III explores longitudinally whetherengaged lifestyle, including marriage and leisure activity, directly affects memory performance, or whether the effect is mediated by health. The overall results demonstrate that marriage predicts episodic memory function directly. Leisure activity can also predict episodic memory performance ten years later, but indirectly via health. An active and engaged lifestyle can protect people against memory decline. The positive impact of engaged lifestyle on memory performance is discussed in terms of cognitive reserve theory, and in relation to the decrease in distress afforded by social support from other people.

    • Frågan är om det under de senaste femton åren funnits något mer populärt managementkoncept än processorientering. När en organsation genomför en förändring som införande av processorientering kan detta givetvis påverka ett antal olika aspekter i organisationen. Frågan som belyses i denna avhandling är vad som händer med styrningen vid förändringar mot ökad processorientering. Särskilt fokuseras vad som händer med betoningen av olika styrformer som regelstyrning, målstyrning och styrning med värderingar. För att belysa detta har två förändringsprocesser som syftat till ökad processorientering studerats, den ena i form av en aktionsforskningsstudie och den andra i form av en retrospektiv fallstudie. Dessa två fall illustrerar en stor spridning i processorienteringsansatserna. Den ena ansatsen som benämns mekanis­tisk uppvisar stora likheter med taylorism och fordism. Den andra som benämns organisk uppvisar snarare likheter med socioteknik och cultural management. Hur styrningen förändras i fallen visar sig vara kopplat till vilken processorienteringsansats som tillämpas. Trots olikheterna i ansatserna är syftena med processorientering likartade i de två fallen. Studien visar dock att den organiska ansatsen kopplar starkare till syftena än den mekanistiska. För att analysera varför respektive processorienteringsansats kommit att tillämpas och varför styrningen förändrats i olika riktningar har contingencyteori, institutionell och kognitiv organisationsteori använts.

    • An important aspect of precision medicine is to probe the stability in molecular profiles among healthy individuals over time. Here, we sample a longitudinal wellness cohort with 100 healthy individuals and analyze blood molecular profiles including proteomics, transcriptomics, lipidomics, metabolomics, autoantibodies andimmune cell profiling, complementedwith gut microbiota composition and routine clinical chemistry. Overall, our results show high variation between individuals across different molecular readouts, while the intra-individual baseline variation is low. The analyses show that each individual has a unique and stable plasma protein profile throughout the study period and that many individuals also show distinct profiles with regards to the other omics datasets, with strong underlying connections between the blood proteome and the clinical chemistry parameters. In conclusion, the results support an individual-based definition of health and show that comprehensive omics profiling in a longitudinal manner is a path forward for precision medicine.

  • Pulmonary function and atherosclerosis in the general population : causal associations and clinical implications

    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

  • The influence of social relationships and leisure activity on adult cognitive functioning and risk of dementia : Longitudinal population-based studies

    • Today, as we live longer, dementia diseases are becoming more prevalent around the world. Thus, further knowledge of how to maintain levels of cognitive functioning in old age and how to identify factors that postpone the onset of dementia are of acute interest. Lifestyle patterns and social life are important aspects to consider in this regard.This thesis includes three studies. Study I investigated the association between participation in various leisure activities in old age (≥65 years) and risk of incident all-cause dementia. Analyses of the total follow-up time period (15 years) showed that higher levels of “Social” and “Total” leisure activity were associated with decreased risk of dementia. In Study II, the aim was to investigate the association between various aspects of social relationships in old age (≥65 years) and risk of incidents of all-cause dementia and Alzheimer's disease. Results showed that over the total follow-up period (16 years) higher values on the relationship index were associated with reduced risk of both dementia and Alzheimer's disease. Visiting/visits of friends and acquaintances more than once a week was related to decreased risk for all-cause dementia, but not for Alzheimer's disease. However, in neither Study I nor II did any of these factors alter the risk of all-cause dementia or Alzheimer's disease when near-onset dementias were removed from the analyses (Study I, up to five years; Study II, up to three years).In Study III the aim was to investigate the association between social network size and cognitive ability in a middle-aged (40–60 years) sample. The idea was that if social network size can moderate negative age-related influence on memory functions, it might also put an individual on a cognitive trajectory that is beneficial in old age. Results from longitudinal analyses showed that baseline network size was positively related to five-year changes in semantic memory and with changes in both semantic and episodic memory at the ten-year follow-up. Social network size was unrelated to changes in visuospatial performance.Taken together, enrichment factors measured in old age (≥ 65 years) did not alter the risk of all-cause dementia or Alzheimer's disease when near-onset dementias were removed from the analyses. These results might reflect protective short-term effects or reverse causality, meaning that in the prodromal phase of dementia individuals tend to withdraw from activity. Social network size in middle age (40-60 years), however, appears to have beneficial long-term effects on cognitive functioning. The results highlight the importance of long follow-up periods and the need to adjust for the influences of reverse causality when investigating the impact of a socially and mentally active life on cognitive functioning.

  • Remembering in Alzheimer's disease : utilization of cognitive support

    • The aim of the present doctoral thesis was to investigate the ability of patients with Alzheimer's disease (AD) to utilize cognitive support in order to improve episodic remembering. A review of previous research indicated that most studies have failed to find beneficial effects of encoding support on memory in AD patients. The ability to utilize cognitive support (i.e., motoric activities, semantic organization, and semantic knowledge) for episodic remembering was investigated in five studies (Bäckman & Herlitz, 1990; Herlitz, Adolfsson, Bäckman, & Nilsson, in press; Herlitz & Bäckman, 1990; Herlitz & Viitanen, in press; Karlsson et al., 1989). Patients with mild, moderate, or severe AD, and normal older adults participated in the studies. On the basis of the results from these studies and the review of the literature, it was concluded that (a) AD patients, irrespective of dementia severity, perform at a lower level than normal older adults in episodic memory tasks; (b) provided that support is supplied at retrieval, AD patients may be sensitive to manipulations at encoding; (c) the strength of the encoding manipulation determines the size of the memory improvement in AD patients; and (d) depending on dementia severity, the type of encoding support also determines the magnitude of memory improvement obtained.

    • Background: The proportion of pregnant women delivered by cesarean section has increased steadily during the past three decades. The risk of infection is 10-fold augmented after elective cesarean section compared to vaginal delivery. Antibiotic prophylaxis may reduce endometritis by 62% and superficial wound infection by 38% after elective cesarean section. International guidelines recommend antibiotic prophylaxis in elective cesarean section, but this procedure is not routinely followed in Sweden. Studies of costs of antibiotic prophylaxis in cesarean section show conflicting results and are based on substantially different incidence of postoperative infections. No study of costs of antibiotic prophylaxis in elective cesarean section in a Swedish or Nordic context has been pursued. The aim of this study was to investigate if antibiotic prophylaxis is cost-reducing in elective cesarean section in orebro County, Sweden.Methods: All women undergoing elective cesarean in the Region orebro County health care system during 2011-2012 were eligible for inclusion. Postoperative infections and risk factors for infections were registered. A hypothetical situation in which all participants had received antibiotic prophylaxis was compared to the actual situation, in which none of them had received antibiotic prophylaxis. The reduction in the risk of postoperative infections resulting from antibiotic prophylaxis was based on a meta-analysis. Costs for in-patient care of postoperative infections were extracted from the accounting system, and costs for out-patient care were calculated according to standard costs. Costs for antibiotic prophylaxis were calculated and compared with the cost reduction that would be implied by the introduction of such prophylaxis.Results: The incidences of deep and superficial surgical site infection were 3.5% and 1.3% respectively. Introduction of antibiotic prophylaxis would reduce health care costs by 31 Euro per cesarean section performed (95% credible interval 4-58 Euro). The probability of cost-saving was 99%.Conclusions: Antibiotic prophylaxis in elective cesarean section is cost-reducing in this health care setting. Our results indicate that the introduction of antibiotic prophylaxis in elective cesarean section can also be cost-saving in low infection rate settings.Trial registration Ethical approval was given by the Regional Ethical Review Board in Uppsala (registration number 2013/484).

    • The aim of the present thesis was to study sex differences in memory and other cognitive bilities in healthy adults. In Study I, participants performed a number of episodic memory tasks that were more or less verbal in nature. Results showed that women performed on a higher level than did men in the episodic memory tasks where it was possible to use verbal labels, whereas men performed on a higher level than did women in a visuospatial episodic memory task. In Study II, women’s advantage in face recognition was investigated.Results showed that women performed at a higher level than did men only in the recognition of other women’s faces. In Study III, sex differences in cognitive tasks as well as brain measures were investigated in healthy older adults. Results showed that only the sex differences in a motor task could, to some extent, be explained by sex differences in one of the brain measures. The findings, as well as possible explanations for these patterns of results, are discussed in a theoretical context.

  • Hematopoietic Serine Proteases from the Mast Cell Chymase and Tryptase Loci - a Functional and Evolutionary Analysis

    • Mast cells are key effector cells in allergic and inflammatory diseases. However, their primary role is most likely in host defence against parasitic and bacterial infections. Mast cells are a particularly rich source of serine proteases. These proteases belong to the chymase or the tryptase family, which are encoded from the mast cell chymase and the multigene tryptase loci, respectively. To better understand the biological functions and the molecular evolution of these enzymes we have studied the organisation of these two loci in species ranging from fish to human. We show that the mast cell chymase locus has evolved from a single founder gene to a complex locus during the past 200 Myr of mammalian evolution. Forty-five fish candidate genes for hematopoietic serine proteases were also identified. However, in phylogenetic analyses none of them grouped with individual branches holding mammalian mast cell chymase locus genes, indicating an independent parallel evolution in fish. Studies of the evolution of the multigene tryptase locus showed that this locus has been highly conserved between marsupials and eutherians. However, no genes belonging to the individual subfamilies identified in eutherians could be identified in fish, amphibians or in birds, which also here indicates parallel evolution. To study the evolution of specific cleavage specificities associated with these proteases, the extended cleavage specificity of opossum α-chymase was determined and found to be nearly identical to human mast cell chymase and the major mouse mast cell chymase mMCP-4. This indicates a strong pressure to maintain this specificity during mammalian evolution. Basophils are rare blood cells with functions similar to mast cells that when mature almost completely lack mRNA. To study the proteome and to primarily characterize the granule protein content of basophils, an in vitro purification protocol was developed to obtain transcriptionally active umbilical cord blood-derived basophil precursors.

  • Streptococcus Species Abundance in the Gut Is Linked to Subclinical Coronary Atherosclerosis in 8973 Participants From the SCAPIS Cohort

    • Background: Gut microbiota have been implicated in atherosclerotic disease, but their relation with subclinical coronary atherosclerosis is unclear. This study aimed to identify associations between the gut microbiome and computed tomography-based measures of coronary atherosclerosis and to explore relevant clinical correlates.Methods: We conducted a cross-sectional study of 8973 participants (50 to 65 years of age) without overt atherosclerotic disease from the population-based SCAPIS (Swedish Cardiopulmonary Bioimage Study). Coronary atherosclerosis was measured using coronary artery calcium score and coronary computed tomography angiography. Gut microbiota species abundance and functional potential were assessed with shotgun metagenomics sequencing of stool, and associations with coronary atherosclerosis were evaluated with multivariable regression models adjusted for cardiovascular risk factors. Associated species were evaluated for association with inflammatory markers, metabolites, and corresponding species in saliva.Results: The mean age of the study sample was 57.4 years, and 53.7% were female. Coronary artery calcification was detected in 40.3%, and 5.4% had at least 1 stenosis with >50% occlusion. Sixty-four species were associated with coronary artery calcium score independent of cardiovascular risk factors, with the strongest associations observed for Streptococcus anginosus and Streptococcus oralis subsp oralis (P<1×10-5). Associations were largely similar across coronary computed tomography angiography-based measurements. Out of the 64 species, 19 species, including streptococci and other species commonly found in the oral cavity, were associated with high-sensitivity C-reactive protein plasma concentrations, and 16 with neutrophil counts. Gut microbial species that are commonly found in the oral cavity were negatively associated with plasma indole propionate and positively associated with plasma secondary bile acids and imidazole propionate. Five species, including 3 streptococci, correlated with the same species in saliva and were associated with worse dental health in the Malmö Offspring Dental Study. Microbial functional potential of dissimilatory nitrate reduction, anaerobic fatty acid β-oxidation, and amino acid degradation were associated with coronary artery calcium score.Conclusions: This study provides evidence of an association of a gut microbiota composition characterized by increased abundance of Streptococcus spp and other species commonly found in the oral cavity with coronary atherosclerosis and systemic inflammation markers. Further longitudinal and experimental studies are warranted to explore the potential implications of a bacterial component in atherogenesis.
