    • Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data.

    • Compound 1, 2-(benzo[1,2,5]thiadiazole-4-sulfonylamino)-5-chloro-N-(3,4-dichloro-phenyl)-benzamide, was identified as a putative type III secretion inhibitor in Yersinia, and the compound thus has a potential to be used to prevent or treat bacterial infections. A set of seven analogues was synthesized and evaluated in a type III secretion dependent reporter-gene assay with viable bacterial to give basic SAR. A second set of 19 compounds was obtained by statistical molecular design in the building block and product space and subsequent synthesis. Evaluation in the reporter-gene assay showed that the compounds ranged from non-active to compounds more potent than 1. Based on the data multivariate QSAR models were established and the final Hi-PLS model showed good correlation between experimentally determined % inhibition and the calculated % inhibition of the reporter-gene signal.

  • Budskap bortom raderna : Design-baserad forskning om undervisning för tolkande läsning

    • Tolkande och reflekterande läsning av skönlitteratur är en viktig del av ämnena svenska och svenska som andraspråk, inte minst i fostran av demokratiska medborgare (Svedner, 2012). Detta gör litteraturundervisningen till ett viktigt studieobjekt. Även om det genomförts många klassrumsstudier om litteraturundervisning (t.ex. Hultin, 2006; Bergman, 2007; Langer, 2011), har ganska få involverat lärare i forskningsprocessen. Vår studie utgår ifrån en design-baserad ansats (Cobb et al., 2003), där nio lärare från grundskolans tre stadier tillsammans med forskare planerat och genomfört undervisning i syfte att utveckla elevers tolkande och reflekterade läsning av skönlitterära texters budskap. Texters budskap är ett vanligt, men också omstritt (t.ex. Lundström et al., 2011) begrepp. I studien används begreppet budskap pragmatiskt, som en konkret utgångspunkt för undervisning där begreppet sedan breddas och fördjupas genom en iterativ serie av undervisningsinterventioner. I studien har följande forskningsfrågor formulerats:  Vad innebär det att urskilja budskap i skönlitterära texter? Vad hindrar och möjliggör för elever att urskilja budskap i skönlitterära texter? Hur kan elevers förmåga att urskilja texters budskap fördjupas genom undervisning? Langers (2011) läsarpositioner används som ett teoretiskt och metodologiskt ramverk som väglett analys och undervisningsdesign. Det empiriska materialet består av elevtexter, inspelade elevsamtal, inspelade undervisningsavsnitt, lärares undervisningsmaterial och inspelade samtal mellan lärare och forskare.  Resultatet visar bland annat att specifika undervisningsaktiviteter, exempelvis att försätta elever i situationer av aktivt budskapsskapande fördjupar elevers tolkning av text. Vidare visar resultaten att lärarens stöttning både kan underlätta och hindra elevers tolkning av text.

    • Ungas erfarenheter av fiktion präglas alltmer av transmediering, i betydelsen att litterära element från det traditionella medieformatet, tryckt text, omskapas i andra medieformat (spel, filmer o.s.v.). Detta fenomen har i forskning begreppsliggjorts som textuniversum. I denna artikel diskuteras vilka typer av kompetenser som kan förknippas med textuniversumbaserad undervisning om skönlitteratur. Fyra dimensioner av kompetens ses som viktiga för textuniversumbaserad undervisning. Diskussionen utgår dels från tidigare forskning, dels från empiriska belägg hämtade från ett pågående forskningsprojekt i vilket nya former av läsning i skolan undersöks. Avslutningsvis i artikeln presenteras en didaktisk modell avsedd att hjälpa lärare i utformning av denna typ av undervisning.

  • High-throughput screening and radioligand binding studies reveal monoamine oxidase-B as the primary binding target for D-deprenyl

    • Aims: D-deprenyl is a useful positron emission tomography tracer for visualization of inflammatory processes. Studies with [C-11]-D-deprenyl showed robust uptake in peripheral painful sites of patients with rheumatoid arthritis or chronic whiplash injury. The mechanism of preferential D-deprenyl uptake is not yet known, but the existence of a specific binding site was proposed. Thus, in the present study, we sought to identify the binding site for D-deprenyl and verify the hypothesis about the possibility of monoamine oxidase enzymes as major targets for this molecule. Main methods: A high-throughput analysis of D-deprenyl activity towards 165 G-protein coupled receptors and 84 enzyme targets was performed. Additionally, binding studies were used to verify the competition of [H-3]D-deprenyl with ligands specific for targets identified in the high-throughput screen. Key findings: Our high-throughput investigation identified monoamine oxidase-B, monoamine oxidase-A and angiotensin converting enzyme as potential targets for D-deprenyl. Further competitive [3H] D-deprenyl binding studies with specific inhibitors identified monoamine oxidase-B as the major binding site. No evident high-affinity hits were identified among G-protein coupled receptors. Significance: Our study was the first to utilize a high-throughput screening approach to identify putative D-deprenyl targets. It verified 249 candidate proteins and confirmed the role of monoamine oxidase - B in D-deprenyl binding. Our results add knowledge about the possible mechanism of D-deprenyl binding, which might aid in explaining the increased uptake of this compound in peripheral inflammation. Monoamine oxidase-B will be further investigated in future studies utilizing human inflamed synovium.

  • Novel variants in Nordic patients referred for genetic testing of telomere-related disorders

    • Telomere-related disorders are a clinically and genetically heterogeneous group of disorders characterized by premature telomere shortening and proliferative failure of a variety of tissues. This study reports the spectrum of telomere-related gene variants and telomere length in Nordic patients referred for genetic testing due to suspected telomere-related disorder. We performed Sanger sequencing of the genes TERT, TERC, DKC1, and TINF2 on 135 unrelated index patients and measured telomere length by qPCR on DNA from peripheral blood leukocytes. We identified pathogenic or likely pathogenic variants in 10 index patients, all of which had short telomeres compared to age-matched healthy controls. Six of the 10 variants were novel; three in TERC (n.69_74dupAGGCGC, n.122_125delGCGG, and n.407_408delinsAA) and three in TERT (p.(D684G), p.(R774*), and p.(*1133Wext*39)). The high proportion of novel variants identified in our study highlights the need for solid interpretation of new variants that may be detected. Measurement of telomere length is a useful approach for evaluating pathogenicity of genetic variants associated with telomere-related disorders.

    • I denna designbaserade studie utforskas hur tolkning av skönlitteratur förändras när lågstadieelever i modersmålsämnet ryska själva intar en författarroll. En utgångspunkt är att meningsskapande i mötet med texter är dialogiskt till sin natur och inbäddat i ett historiskt, kulturellt och socialt sammanhang. Begreppet intertextualitet, det vill säga att texter bär med sig ett eko av andra texter, används dels för analys av transkriberade samtal mellan en modersmålslärare och hennes elever, dels för analys av elevernas egenförfattade berättelser. Resultaten visar att elevernas tolkningar förskjuts från textens yta till det som finns ”gömt” inne i texten, samt från förgivettaganden baserade på den egna livsvärlden i och utanför skolan till tolkningar med intertextuella kopplingar till undervisningens sammanhang. Tolkningarna blir även mer flerstämmiga, vilket bidrar till utveckling av en egen röst. Resultaten pekar på att eleverna genom undervisningsinterventionerna blir del av en tolkande gemenskap, där läsförståelseutvecklingen sker inbäddad i ett sammanhang, snarare än som en dekontextualiserad aktivitetet.

  • Additional value of magnetic resonance-targeted biopsies to standard transrectal ultrasound-guided biopsies for detection of clinically significant prostate cancer

    • OBJECTIVE: The aim of this study was to evaluate the additional value of magnetic resonance imaging-targeted biopsy (MRI-TB) to standard transrectal ultrasound-guided biopsy (SB) for detection of clinically significant prostate cancer (PCa). An additional aim was to compare the biopsy results to MRI evaluation using a Likert scale.MATERIALS AND METHODS: Patients with newly diagnosed localized PCa (n = 53) by clinical routine SB were prospectively included. The majority of the patients were scheduled for curative therapy before enrollment. The patients underwent multiparametric MRI (mpMRI) at 3 T using an endorectal coil followed by two MRI-TBs, using ultrasound with cognitive fusion. All included patients underwent MRI-TB, even those who had low to very low suspicion of significant PCa on mpMRI. The detection rate of significant cancer on SB versus SB + MRI-TB was compared in the 53 included patients and with whole-mounted histopathology as reference in 34 cases. Comparison of the biopsy results to MRI evaluation and interreader agreement calculation of five-point Likert score evaluation were performed.RESULTS: In total, 32 significant (Gleason ≥7) PCa were detected by SB, while SB + MRI-TB detected an additional five significant PCa. MRI-TB alone detected 20 and missed 17 significant PCa. Ten of the significant PCa cases missed by MRI-TB had a Likert score of 3 or lower. Interreader agreement using the Likert scale was high, with a kappa value of 0.77 (95% confidence interval 0.63-0.92, p < 0.0001).CONCLUSION: Detection of significant PCa increased by adding MRI-TB to SB. This may not be of enough clinical value to justify the use of targeted biopsies in this patient group.

  • The importance of species interactions in eco-evolutionary community dynamics under climate change

    • Eco-evolutionary dynamics are essential in shaping the biological response of communities to ongoing climate change. Here we develop a spatially explicit eco-evolutionary framework which features more detailed species interactions, integrating evolution and dispersal. We include species interactions within and between trophic levels, and additionally, we incorporate the feature that species interspecific competition might change due to increasing temperatures and affect the impact of climate change on ecological communities. Our modeling framework captures previously reported ecological responses to climate change, and also reveals two key results. First, interactions between trophic levels as well as temperature-dependent competition within a trophic level mitigate the negative impact of climate change on biodiversity, emphasizing the importance of understanding biotic interactions in shaping climate change impact. Second, our trait-based perspective reveals a strong positive relationship between the within-community variation in preferred temperatures and the capacity to respond to climate change. Temperature-dependent competition consistently results both in higher trait variation and more responsive communities to altered climatic conditions. Our study demonstrates the importance of species interactions in an eco-evolutionary setting, further expanding our knowledge of the interplay between ecological and evolutionary processes. Understanding the dynamics of species interactions can help predict community responses to climate change. A spatially explicit model finds that species interactions and competition mitigate the harmful impacts of climate change, and that temperature-dependent competition makes communities more variable and responsive to changing climates.

  • Guided Internet-Administered Self-Help to Reduce Symptoms Of Anxiety and Depression Among Adolescents and Young Adults Diagnosed With Cancer During Adolescence (U-CARE: YoungCan) : study protocol for a feasibility trial

    • Introduction A subgroup of adolescents and young adults diagnosed with cancer during adolescence reports elevated levels of anxiety and depressive symptoms and unmet needs for psychological support. Evidence-based psychological treatments tailored for this population are lacking. This protocol describes a feasibility study of a guided-internet-administered self-help programme (YoungCan) primarily targeting symptoms of anxiety and depression among young persons diagnosed with cancer during adolescence and of the planned study procedures for a future controlled trial. Methods/analysis The study is an uncontrolled feasibility trial with a pre-post and 3-month follow-up design. Potential participants aged 15-25years, diagnosed with cancer during adolescence, will be identified via the Swedish Childhood Cancer Registry. 30 participants will be included. Participants will receive YoungCan, a 12-week therapist-guided, internet-administered self-help programme consisting primarily of cognitive-behavioural therapy organised into individually assigned modules targeting depressive symptoms, worry and anxiety, body dissatisfaction and post-traumatic stress. Interactive peer support and psychoeducative functions are also available. Feasibility outcomes include: recruitment and eligibility criteria; data collection; attrition; resources needed to complete the study and programme; safety procedures; participants' and therapists' adherence to the programme; and participants' acceptability of the programme and study methodology. Additionally, mechanisms of impact will be explored and data regarding symptoms of anxiety, depression, post-traumatic stress, body dissatisfaction, reactions to social interactions, quality of life, axis I diagnoses according to the Mini International Neuropsychiatric Interview and healthcare service use will be collected. Exploratory analyses of changes in targeted outcomes will be conducted. Ethics/dissemination This feasibility protocol was approved by the Regional Ethical Review Board in Uppsala, Sweden (ref: 2016/210). Findings will be disseminated to relevant research, clinical, health service and patient communities through publications in peer-reviewed and popular science journals and presentations at scientific and clinical conferences.

    • OBJECTIVES: The aim of this study was to illuminate lived experience of having been in an acute confusional state (ACS) as narrated by elderly patients in orthopaedic care. METHOD: Qualitative study with phenomenological hermeneutic method for analysing the data based on narrative interviews. Fifty patients (67-96 years of age) who developed ACS during hospitalisation and in all cases the ACS ceased during their stay on the ward were interviewed once lucid again regarding the course of the event, their experiences, memories and interpretation of what had happened during the ACS. RESULTS: The meaning of the patients' lived experiences of being and having been confused was interpreted as 'Being trapped in incomprehensible experiences and a turmoil of past and present and here and there', comprising the themes trying to get a grip on the experience of the confusion, encountering past, present and the realm of the imagination as reality during the period of confusion and confronting the idea of having been confused. Contradictory to earlier research the patients remembered and could tell in great detail about their ACS. While confused, the confusional state means that impressions of all kinds invade the mind of the person and are experienced as reality, making him/her a victim of these impressions rather than the one who controls what comes into his/her mind. While in the middle of these experiences the person simultaneously senses that the impressions are unreal, thus indicating that he/she is in some sort of borderland between understanding and not understanding. The things that come into the mind of the person can either be frightening or neutral or enjoyable scenarios that seem to be mainly familiar but can also be unknown. These scenarios seem to be a mixture of past and present, of events and people while they seem to float from location to location. CONCLUSIONS: The findings indicates that what takes place during the ACS is not nonsense but probably a mix of the patient's life history, their present situation and above all a form of communication concerning their emotional state and inner experiences in this new situation. The findings also indicated that one possible approach to the patients is to confirm and support the patients in narrating their experiences both during the confusion and also after the ACS had ceased.

Etapp 1; scenarier och förslag på aktiviteter.Detta dokument är slutrapporten från den regionala framsynsprocessen som drevs 2008-2009 inom projektet DARE – Development Arena for Research and Entrepreneurship.Framsynen har verkat efter ett normativt och handlingsinriktat angreppssätt. Ledmotiv för processen har varit frågor som: Hur vill vi ha det? Varför är det inte så nu? Vad behöver vi göra nu och i framtiden? Regionen kan sägas kännetecknas av att den är rik på naturresurser och fattig på folk. Det är bakgrunden till de fokusområden som framsynen behandlat- Grönare näringsliv (hur kan nya och miljöeffektivare produkter och tjänsterutvecklas effektivare?)- Demografiutveckling (hur kan samhällsfunktionerna bestå med inverteradebefolkningspyramider och hur kan utvecklingen mot en åldrande befolkningvändas?)Lite tvärs över båda frågorna ovan hamnar ett annat fokus; regionens attraktivitet. Inom detta område har en ung framsyn genomförts med deltagare från olika delar av Norrbotten.Framsynen har gått steg för steg genom visioner, analys av framgångsfaktorer, scenarieskapande, förslag på åtgärder och prioritering av dessa. Under genomförandet har det uppstått ett antal avknoppningar i form av t ex nya samarbeten och projekt. Ett av de sistnämnda var den unga framsynen som genomfördes med finansiering av Norrbottens läns landsting.Bland de prioriterade förslagen som redovisas i denna rapport dominerar olika aktiviteter för att knyta samman olika aktörer och grupper inom regionen och på så sätt göra mer av mindre. Många av förslagen visar regionala aktörers förväntningar på universitetens roll i den regionala utvecklingen och markerar ett intresse av tätare samverkan meduniversiteten. Förslagen berör alla verksamheter inom universiteten, såväl utbildning, forskning och förvaltning. Bland de prioriterade förslagen finns:• Stimulera gröna affärs och teknikutvecklingsallianser för regionens småföretag• Stimulera ”den lille forskaren” – en modell för småföretagens forskarkollektiv• Utred grönare näringsliv som gemensamt profilområde för regionens universitet• Stärk företagsnyttig forskning & utvecklingsinsatser kring ”age management”• Etablera ett ”integrationsnätverk” för universitetens studenter & det regionalasamhället• Utveckla dialogen mellan universiteten & avnämargrupper i utvecklingen avutbildningar • Skapa regionalt utvecklingslab för bättre samverkan mellan universiteten ochomgivande samhälleInom DARE-projektet kommer nu det material som utvecklats inom framsynsprocessen att användas för utveckling av samverkan inom universiteten, mellan universiteten och med andra intressenter av den regionala utvecklingen, inte minst då de som deltagit i framsynen.

Rapporten beskriver projektets bakgrund, syfte, mål, metod, tidplan, organisation samt de identifierade områden och processer som har legat till grund för framtagandet av innehållet i den första 3-årstrategin.Projektet ”Nationell Standardiseringsstrategi för digital samhällsbyggnadsinformation” är ett strategiskt projekt inom Smart Built Environments temaområde Informationsinfrastruktur.Syftet med NSS är att få en branschgemensam samsyn på prioriteringar avseende Sveriges arbete med standardisering av samhällsbyggnadsinformation. Målet med projektet är att leverera ett ramverk för strategiarbetet fram till 2030 samt ett strategiskt program för den första 3-årsstrategin.Temaområdet Informationsinfrastruktur har för avsikt att uppnå specifika resultatmål, som i sin tur ska bidra till att åstadkomma ett antal kortsiktiga effekter 2021 respektive 2024. Dessa kortsiktiga effekter ska på sikt hjälpa till att uppnå programmets långsiktiga effektmål mot 2030. NSS-projektet avser att ha verkat för detta genom att tydligt formulera Smart Built Environments långsiktiga effektmål i projektets egen effektlogik.

    • Objectives: To explore how pictorial information on subclinical atherosclerosis affects GPs’ perception of patient cardiovascular disease (CVD) risk, their communication with patients, and GPs’ attitude to the treatment of CVD risk factors.Design, setting and subjects: Fifteen individual interviews were conducted between March 2014 and December 2016, with GPs who had received pictorial information regarding their patients’ subclinical atherosclerosis. The pictorial information was also received by the patients together with written information regarding atherosclerosis and CVD risk prior to the appointment with their GP. The interviews were recorded, transcribed and analyzed using qualitative content analysis.Results: Three categories were identified in the analysis. Increased knowledge makes a difference: When patients had more in-depth knowledge regarding atherosclerosis, the consultation became more patient-centered and moved towards shared decision making. This is real, not just a number: GPs described their risk assessment and the patient’s risk perception as more accurate with pictorial information about subclinical atherosclerosis. How to deal with the result–A passive to active approach: Some GPs acted promptly on the pictorial information while others took no action.Conclusion and implications: Pictorial information regarding patients’ subclinical atherosclerosis affected GPs’ assessment of CVD risk. The communication shifted towards shared decision-making although the GPs’ attitude to the result and treatment of CVD risk factors varied.     • We aimed to explore the prevalence of atherosclerosis by using multi-view ultrasound examination of the carotid arteries and its association with clinical risk factors in a middle-aged population at low to intermediate risk of cardiovascular disease (CVD). Carotid vascular ultrasound was performed in 3532 participants in the VIPVIZA trial. Mean and maximal carotid intima media thickness (cIMT) at prespecified angles and plaque presence were examined on the left and right side. Associations between CVD risk factors and ultrasound variables were quantified by partial least squares (PLS) regression. A combined ultrasound variable was computed using weights of the first PLS component. Associations between CVD risk factors and the combined multi-view ultrasound variable, single cIMT and plaque measurements, respectively, were determined using linear regression modelling. The participants’ mean age was 55.7 years and 52.9% were women. Plaque prevalence was 51.1% in men and 39.0% in women. cIMT was higher in men than in women and in the left compared with the right carotid artery. The strongest association of CVD risk factors was observed with the combined multi-view ultrasound variable (R2 = 24%), compared with single cIMT variables (R2 = 14–18%) and plaque presence (R2 = 15%). The pattern was similar in both sexes. The association with CVD risk factors and the combined ultrasound variable was stronger in 40-year olds (R2 = 22%) compared with 50- or 60-year olds (R = 12%). CVD risk factors are stronger associated with a combined ultrasound variable than plaque presence or single cIMT measures suggesting that carotid multi-view ultrasonography better captures the focality of early atherosclerosis.   • Pictorial presentation of subclinical atherosclerosis : a measure to reduce the risk for cardiovascular disease

    • The overall aim was improved cardiovascular disease (CVD) prevention through the VIPVIZA intervention. This includes the provision of pictorial information of subclinical atherosclerosis to participants and their general practitioners (GPs), follow-up phone call, and written information to the participant. VIPVIZA is a Prospective Randomized Open Blinded End-point (PROBE) trial nested within VIP, a CVD prevention program in primary health care in Västerbotten county, Sweden. Middle-aged individuals at low/intermediate CVD risk were enrolled to VIPVIZA and randomized 1:1 to an intervention (n=1749) or control group (n=1783 who received no pictorial or other information). Preventive measures were managed within primary health care.At baseline, clinical risk factors were measured and carotid ultrasound examination was performed. The prevalence of subclinical atherosclerotic disease was assessed as intima media thickness and presence of plaque. The association between clinical risk factors and measures of subclinical atherosclerosis was investigated. In addition to conventional risk factor-based risk evaluation, the impact of the VIPVIZA intervention on CVD risk, traditional risk factors and pharmacological treatment was evaluated after 1 and 3 years. Individual interviews were conducted with 15 GPs to explore how a pictorial representation of subclinical atherosclerosis affects physicians in their perception and communication of CVD risk. The interviews were analyzed by qualitative content analysis.The plaque prevalence was 44.7% in this population. Clinical risk factors explained more of the variation in a combined ultrasound measurement than single measurements. The results up to three years showed a VIPVIZA intervention effect, with lower and sustained CVD risk in the intervention as compared to the control group. The effect was partly mediated by differences in intake of lipid-lowering medication and partly by lifestyle behaviour. The GPs described their risk assessment and patients’ risk perception as more accurate with the VIPVIZA intervention. Informing patients about examination results prior to a consultation can facilitate shared decision-making and enhance adherence to preventive measures.The results show that the VIPVIZA intervention reduces CVD risk over three years. In the long run this has the potential to reduce the incidence of CVD events.

The objective was to investigate if pictorial information regarding subclinical carotid atherosclerosis provided to individuals and physicians gave sustained effects on cardiovascular risk beyond the previously reported effect after 1 year and up to 3 years. Methods: A Prospective Randomized Open Blinded End-point (PROBE) trial. Within a CVD prevention program in Västerbotten County, Sweden, 3532 healthy individuals aged 40, 50 or 60 years were enrolled and 1:1 ran- domized to intervention ( n = 1749; pictorial information with additional prevention materials to participants and physicians) or control group ( n = 1783; no pictorial information to participants and physicians). Preventive measures were managed within primary care. Participants were investigated at baseline during 2013–2016 and at follow-up after 1 and 3 years. Results: A beneficial effect on cardiovascular risk was observed at 3-year follow-up; Framingham Risk Score (FRS) was 13.38 for the intervention group and 14.08 for the control group ( p = 0.047) and SCORE was 1.69 vs. 1.82 ( p = 0.022). The effect observed at 1-year was sustained over 3 years after adjustment for sex and education and more pronounced among participants with a severe atherosclerotic picture at baseline.Conclusions: This study provides evidence of sustained beneficial effects on the adherence to prevention guidelines over 3 years of pictorial information about subclinical carotid atherosclerosis, resulting in lower cardiovascular risk regardless of sex and educational level. Direct visualization of the underlying still subclinical atherosclerotic disease, rather than just indirect information about risk factors and statistical risk of future myocardial infarction, stroke and death, is one way to tackle the problem of non-adherence to prevention of cardiovascular diseases.

    • This report describes how the work has been conducted in the project “A National Strategy for Standardisation of Digital Information Management within the BuiltEnvironment” (further on denoted NSS). This report describes the project background, aim, objective, methodology, schedule, organisation, and identified areas and processes that have been the basis for the development of the initial three-year strategy.The NSS project is a strategic project within Smart Built Environment’s theme“Information infrastructure”.The aim of NSS project is to get an industry-wide consensus on Sweden’s priorities regarding standardization of information management about the built environment. The objective is to deliver a framework which supports further development of the strategy until 2030 and a strategic programme for the initial three-year strategy.The theme “Information structure” has formulated specific performance targets whichin turn shall contribute to achieve some short-term effects within 2021 and 2024. These short-term effects aim to, in the long run, contribute to achieve the programme's long-term objectives by 2030. The intention of the NSS project is to contribute to the programme by clearly formulating Smart Built Environment's long-term objectives by following the project's effect logic.The NSS project consists of the following four phases:1. Initiation,2. Development of a three-year strategy (2021-2023),3. Development of a framework for subsequent strategies (2024–2026, 2027– 2029), and4. Closure.Each phase consists of several sub-goals (a total of 11), which in turn consists of several activities. These have been systematically processed and handled to achieve the project's aim and objectives.To identify the areas and processes that can streamline information flows within the built environment processes, the project team has used the process map (SBUF BIM Report 2013) as a basis.The following focus areas were identified:a) Geodata and existing conditions,b) Analysis, requirements, and review,c) Delivery from design to production, andd) Transfer between production and operations.For each focus area, several problems were identified, together with potential root causes and suggested remedy actions. A total of 19 problems or shortcomings were assessed with standards within the industry, which resulted in 39 suggested remedy actions. These actions were then further developed into eight development projectscategorized into three strategic focus areas: Application Instructions, Delivery 8 ARBETSRAPPORT NATIONELL STANDARDISERINGSSTRATEGISpecifications, and Common Information Structure. As a part of the strategy, the initiation and implementation of the above-mentioned projects is proposed to 2021 – 2023.The anchoring process of the NSS project and its framework has been an important sub-goal to ensure the project success and compliance. Continuous communication and anchoring efforts have been performed as a part of the project, partly through the various groups involved in the project (steering-, reference-, and expert support group), partly through coordination and discussions with important stakeholders and relevant parallel initiatives.The framework describes how the strategic work shall be accomplished and includes principles how to follow-up and revise the strategies. It also includes the strategic roadmap, that is a plan including objectives and milestones for navigation. The roadmap is divided into three different phases which all contain a set of projects to be conducted according to the figure below.The developed strategy and its framework are recommended to be owned and maintained by the newly created organization named “Samhällsbyggandets Regelforum”.The project also recommends how to practically follow-up the work, which is based on several identified indicators. A basic condition to ensure a quality assured and efficient implementation is “a modern and digital communication platform”. The project alsoincludes a suggested technical solution.

The report also contains a description of the vision and the objectives of the strategy. The project has been led by Pontus Bengtson, Autodesk, in collaboration with Lars Harrie, Lund University; Cristina Lázaro Morales, Plan B; Henrik Linderoth, Jönköping University; Anna Neidenström, Swedish Transport Administration; and Håkan Norberg, Plan B. The vision of the strategy is a public construction sector with a consensus on what to be prioritized regarding standardization to promote efficient information management. This will be achieved by jointly focusing on the prioritized development projects – projects that aim to create a common information structure with the greatest user benefit. There are eight projects categorized into three strategic focus areas, Application Instructions, Delivery Specifications and Common Information Structure recommended during the first strategy phase.     • Approximately half of all cases of Hoyeraal-Hreidarsson syndrome (HHS), a multisystem disorder characterized by bone marrow failure, developmental defects and very short telomeres, are caused by germline mutations in genes related to telomere biology. However, the varying symptoms and severity of the disease indicate that additional mechanisms are involved. Here, a 3-year-old boy with HHS was found to carry biallelic germline mutations in WRAP53 (WD40 encoding RNA antisense to p53), that altered two highly conserved amino acids (L283F and R398W) in the WD40 scaffold domain of the protein encoded. WRAP53 beta (also known as TCAB1 or WDR79) is involved in intracellular trafficking of telomerase, Cajal body functions and DNA repair. We found that both mutations cause destabilization, mislocalization and faulty interactions of WRAP53 beta, defects linked to misfolding by the TRiC chaperonin complex. Consequently, WRAP53 beta HHS mutants cannot elongate telomeres, maintain Cajal bodies or repair DNA double-strand breaks. These findings provide a molecular explanation for the pathogenesis underlying WRAP53 beta-associated HHS and highlight the potential contribution of DNA damage and/or defects in Cajal bodies to the early onset and/or severity of this disease.

    • PURPOSE: We investigated the incidence of prostate cancer after negative transrectal ultrasound (TRUS) guided multiple biopsies. Our secondary aim was to calculate the sensitivity of the extended protocol used.MATERIALS AND METHODS: A cohort of 547 men with elevated prostate specific antigen and/or abnormal digital rectal examination but with results negative for prostate cancer on a mean of 9 TRUS guided biopsies was followed through record linkage to the national cancer Registry. The observed number of prostate cancers was compared with the expected number during the same calendar period in an age matched male population to determine the standardized incidence ratio. The sensitivity of TRUS with multiple biopsies after 5 years of followup was calculated. Relative survival was estimated if there was an excess death rate due to undiagnosed prostate cancer.RESULTS: We found 11 men diagnosed with prostate cancer. The expected number in the age standardized male population was 15, resulting in a standardized incidence ratio of 0.8 (95% CI 0.4 to 1.2). Five-year sensitivity of the extended protocol of TRUS guided biopsies was 95.2% (95% CI 93.5 to 96.4) and relative survival was more than 100%, indicating a selection of men deemed candidates for curative treatment.CONCLUSIONS: Men with clinical suspicion of prostate cancer who are examined by an extended protocol of TRUS guided biopsies negative for cancer do not have an increased incidence of prostate cancer within 6 years compared with an age matched male population. Five-year sensitivity of this protocol was high.

    • A detailed study of the defect structure in a di-substituted -Bi2O3 type phase, -Bi5PbY2O11.5, is presented. Using a combination of conventional Rietveld analysis of neutron diffraction data, reverse Monte Carlo (RMC) analysis of total neutron scattering data and ab initio molecular dynamics (MD) simulations, both average and local structures have been characterized. -Bi5PbY2O11.5 represents a model system for the highly conducting -Bi2O3 type phases, in which there is a higher nominal vacancy concentration than in the unsubstituted parent compound. Uniquely, the methodology developed in this study has afforded the opportunity to study both oxide-ion vacancy ordering as well as specific cation-cation interactions. Oxide-ion vacancies in this system have been found to show a preference for association with Pb2+ cations, with some evidence for clustering of these cations. The system shows a non-random distribution of vacancy pair alignments, with a preference for 100 ordering, the extent of which shows thermal variation. MD simulations indicate a predominance of oxide-ion jumps in the 100 direction.

  • DNA methylation variations and epigenetic aging in telomere biology disorders

    • Telomere Biology Disorders (TBDs) are characterized by mutations in telomere-related genes leading to short telomeres and premature aging but with no strict correlation between telomere length and disease severity. Epigenetic alterations are also markers of aging and we aimed to evaluate whether DNA methylation (DNAm) could be part of the pathogenesis of TBDs. In blood from 35 TBD cases, genome-wide DNAm were analyzed and the cases were grouped based on relative telomere length (RTL): short (S), with RTL close to normal controls, and extremely short (ES). TBD cases had increased epigenetic age and DNAm alterations were most prominent in the ES-RTL group. Thus, the differentially methylated (DM) CpG sites could be markers of short telomeres but could also be one of the mechanisms contributing to disease phenotype since DNAm alterations were observed in symptomatic, but not asymptomatic, cases with S-RTL. Furthermore, two or more DM-CpGs were identified in four genes previously linked to TBD or telomere length (PRDM8, SMC4, VARS, and WNT6) and in three genes that were novel in telomere biology (MAS1L, NAV2, and TM4FS1). The DM-CpGs in these genes could be markers of aging in hematological cells, but they could also be of relevance for the progression of TBD.

    • Climate change is predicted to have major implications for global biodiversity. Dispersal and evolution may become crucial for species survival, as species must either adapt or migrate to track the changing climate. However, migration and evolution do not occur in vacuum – the biotic community in which these processes play out may modulate their effect on biodiversity. Here, we use an eco-evolutionary, spatially explicit, multi-species model that allows us to examine the interactive effects of competition, adaptation and dispersal on species richness in plant communities under global warming. We find that there is a larger decline in global species richness when interspecific competition is strong. Furthermore, there is a three-way interaction between interspecific competition, evolution and dispersal that creates a complex pattern of biodiversity responses, in which both evolution and dispersal can either increase or decrease the magnitude of species loss. This interaction arises for at least two reasons: 1) different levels of dispersal, evolution and competition creates differences in local and global community structure before climate change, and 2) competitive interactions determine whether the benefits of dispersal and/or evolution (climate tracking and adaptation) outweighs the risks (competitive exclusion).

    • Stress among healthcare students is a growing problem. As self-comfort is assumed to be a way of coping with stressful emotions, the aim of this study was to describe the patterns of self-comforting actions that healthcare students usually use in distress. One hundred and sixty-eight healthcare students volunteered to write down accounts of what they do when they comfort themselves. Their accounts were analysed using qualitative content analysis. The findings reveal two themes: Ingressing and Transcending. Ingressing comprises the sub-themes Unloading, Distracting, Nurturing oneself, Withdrawing and Reassuring. Transcending comprises the sub-themes Opening up and Finding new perspectives. These findings are in line with some stress-reducing strategies described in the literature on stress management. Winnicott’s theory about the phenomenon of transition is used to interpret the findings. In the light of Winnicott’s theory, self-comforting measures can be comprehended as the ability to transfer early childhood experiences of being nurtured and comforted into well-adapted strategies to effect relaxation and gain strength.

    • Health care often involves ethically difficult situations that may disquiet the conscience. The purpose of this study was to develop a questionnaire for identifying various perceptions of conscience within a framework based on the literature and on explorative interviews about perceptions of conscience (Perceptions of Conscience Questionnaire). The questionnaire was tested on a sample of 444 registered nurses, enrolled nurses, nurses' assistants and physicians. The data were analysed using principal component analysis to explore possible dimensions of perceptions of conscience. The results showed six dimensions, found also in theory and empirical health care studies. Conscience was perceived as authority, a warning signal, demanding sensitivity, an asset, a burden and depending on culture. We conclude that the Perceptions of Conscience Questionnaire is valid for assessing some perceptions of conscience relevant to health care providers.

    • The aim of this study is to illuminate the meaning of encounters with a troubled conscience among psychiatric therapists. Psychiatric care involves ethical dilemmas which may affect conscience. Conscience relates to keeping or losing a sense of personal integrity when making judgments about one's actions. Ten psychiatric therapists were interviewed in June 2006. The interviews were tape-recorded, transcribed verbatim and interpreted using a phenomenological-hermeneutic method. Two themes 'Facing inadequacy' and 'Struggling to view oneself as being 'good enough'' are presented. In the therapists interviewed, awareness of their use of power, a sense of powerlessness and a sense of blame gave rise to feelings of betrayals and shameful inadequacy. By sharing their inadequacy with co-workers, they managed to endure the sense of their inadequacy which otherwise would have threatened to paralyse them. Finding consolation in sharing wearing feelings, becoming realistic and attesting their worthiness, they reached reconciliation and found confirmation of being good enough. The findings are interpreted in light of Lögstrup's ethics of trust, according to which conscience alerts us to silent but radical ethical demand and the risk of self-deception.

    • We introduce the 4-metre Multi-Object Spectroscopic Telescope (4MOST), a new high-multiplex, wide-field spectroscopic survey facility under development for the four-metre-class Visible and Infrared Survey Telescope for Astronomy (VISTA) at Paranal. Its key specifications are: a large field of view (FoV) of 4.2 square degrees and a high multiplex capability, with 1624 fibres feeding two low-resolution spectrographs (R = λ/Δλ ~ 6500), and 812 fibres transferring light to the high-resolution spectrograph (R ~ 20 000). After a description of the instrument and its expected performance, a short overview is given of its operational scheme and planned 4MOST Consortium science; these aspects are covered in more detail in other articles in this edition of The Messenger. Finally, the processes, schedules, and policies concerning the selection of ESO Community Surveys are presented, commencing with a singular opportunity to submit Letters of Intent for Public Surveys during the first five years of 4MOST operations.

    • The aim of this study was to evaluate the effects of one year of systematic clinical supervision, and supervised individually planned care on the mood and general behavior of patients with dementia in relation to their cognitive function and level of confusion. The intervention was carried out in a ward devoted to the care of patients with severe dementia (EW), with a similar ward (CW) where no changes were made serving as a control. Each ward housed 11 patients, of whom 7 patients in each ward survived throughout the study time. Assessment of the patients' mood, general behavior, cognitive functions (MMSE), orientation and confusion (OBS) was made at baseline, and after 6 and 12 months of intervention. Significant deteriorations for the patients in CW were seen during the study period as regards the factor strength, functional performance and orientation in the ward, and speech performance and psychomotor slowing. For the patients in the EW, deterioration was seen only in the factor accessibility, and improvement was seen in the factor sensitivity and euphoria. The differences indicated that the development in the two wards went in opposite directions to the benefit of the patients in the EW. Because of the small sample size, the findings are more the result of a pilot trial, rather than generalizable. The intervention, however, seemed to have a positive effect on the secondary symptoms of the disease. (C) 1999, Editrice Kurtis.

    • BACKGROUND: According to international guidelines, standard treatment (ST) with curative intent in cervical cancer (CC) comprises radical hysterectomy and pelvic lymphadenectomy in early stages (International Federation of Gynecology and Obstetrics (FIGO) 2009 IB1, IIA1), adjuvant chemoradiation is recommended based on risk factors upon final pathology. Definitive chemoradiation is recommended in locally advanced stages (FIGO 2009 IB2, IIA2, IIB). Total mesometrial resection (TMMR) with therapeutic lymph node dissection (tLND) without adjuvant radiation has emerged as a promising treatment. Here we compare oncologic outcome by TMMR + tLND or ST.METHODS: In this observational cohort study, women treated according to international guidelines were identified in the population-based registries from Sweden and women treated with TMMR were identified in the Leipzig Mesometrial Resection (MMR) Study Database (DRKS 0001517) 2011-2020. Relevant clinical and tumour related variables were extracted. Recurrence-free survival (RFS) and overall survival (OS) by ST or TMMR was analysed with log-rank test, cumulative incidence function and proportional hazard regression yielding hazard ratios (HR) with 95% confidence intervals (CI), adjusted for relevant confounders.FINDINGS: Between 2011 and 2020, 1007 women were included in the final analysis. 733 women were treated according to ST and 274 with TMMR. RFS at five years was 77.9% (95% CI 74.3-81.1) and 82.6% (95% CI 77.2-86.9) for the ST and TMMR cohorts respectively (p = 0.053). In early-stage CC, RFS was higher after TMMR as compared to ST, 91.2% vs 81.8% (p = 0.002). In the adjusted analysis, TMMR was associated with a lower hazard of recurrence (HR 0.39; 95% CI 0.22-0.69) and death (HR 0.42; 95% CI 0.21-0.86) compared to ST. The absolute difference in risk of recurrence at 5 years was 9.4% (95% CI 3.2-15.7) in favor of TMMR. In locally advanced CC, no significant differences in RFS or OS was observed.INTERPRETATION: Compared to ST, TMMR without radiation therapy was associated with superior oncologic outcomes in women with early-stage cervical cancer whereas no difference was observed in locally advanced disease. Our findings together with previous evidence suggest that TMMR may be considered the primary option for both early-stage and locally advanced cervical cancer confined to the Müllerian compartment.FUNDING: This study was supported by grants from Centre for Clinical Research Sörmland (Sweden) and Region Stockholm (Sweden).
