    • Climate change and population growth are exacerbating environmental challenges and natural hazards. Extreme events such as flooding due to heavy precipitation are occurring more frequently and becoming more severe throughout the world. Future projections indicate increasing risks and serious threats to human societies, particularly those living in urban areas. To mitigate associated socio-economic challenges and support sustainable urban development, nature-based solutions (NBS) are being introduced in mitigation and adaptation strategies. However, NBS need to be carefully located and suitably designed to achieve their full potential in terms of flood mitigation. Landscape connectivity, addressing various landscape processes and components, can potentially be utilized for land management and biodiversity conservation. For instance, hydrological connectivity within a catchment, as part of the landscape connectivity, describes the main water flow pathways and areas where runoff tends to accumulate, and can thus be useful in identifying the best locations for NBS from a hydrological viewpoint. In this chapter, we describe use of landscape connectivity to identify suitable locations for NBS to reduce flood risk in urban areas, and exemplify the process for two distinct urban catchments, located in Sweden and Portugal. The results showed good usefulness of the method and revealed the importance of integrating connectivity mapping into future NBS planning practices and decision support systems.

  • Inventering av stora rovdjur i Gävleborgs län 2012-2013

    • I Gävleborgs län finns fasta stammar av Sveriges samtliga stora rovdjur: björn, järv, kungsörn, lodjur och varg. Länsstyrelsen har det regionala ansvaret för rovdjursinventeringar, det vill säga att kartlägga rovdjursförekomsten i länet. I den här rapporten redovisas resultaten från rovdjurs-inventeringar samt länsstyrelsens bedömning av järvförekomst i Gävleborg inventeringssäsongen 2012/2013. Syftet med inventeringarna är att övervaka rovdjursstammarnas storlek, status och utveckling. Inventeringsresultaten utgör ett viktigt underlag i länsstyrelsens arbete med att förvalta rovdjuren utifrån uppsatta nationella och regionala mål. Kunskap om rovdjursstammarnas utveckling kan också vara av betydelse inom övrig viltförvaltning.  Jägares och allmänhetens rapportering av rovdjursobservationer till länsstyrelsen är till stor hjälp i arbetet med att göra så bra inventeringar som möjligt utifrån de resurser som finns. Rapporteringen gör enklast via hemsidorna www.rovobs.se och www.skandobs.se. Länsstyrelsens fältpersonal följer löpande upp inrapporterade observationer. Inventeringsmetoderna i fält efter lo och varg är huvudsakligen snoking och mindre områdesinventeringar utförda av länsstyrelsens personal. Vid lodjursinventeringen 2012/2013 kvalitetssäkrades 21 lodjursföryngringar, det vill säga 21 honor som fött ungar 2012. Antalet lodjur i Gävleborg kan därmed uppskattas till cirka 137 vintern 2012/2013. Två av föryngringarna delades med Uppsala län och en med Dalarnas län.  Gävleborg berördes av fem vargrevir vintern 2012/2013: Haverö, Prästskogen, Kukumäki, Sjösveden och Korså. I två av reviren, Prästskogen och Korså, bekräftades föryngringar 2012. Länsstyrelsen bedömer att föryngring också kan ha skett i Sjösvedenreviret, men det har inte kunnat bekräftas. Utifrån inventeringsresultatet kan länets vargpopulation vintern 2012/2013 uppskattas till 20-30 vargar. Björn inventerades hösten 2012 genom en spillningsinventering genomförd av länsstyrelsen och Jägareförbundet. Det preliminära resultatet visar att björnstammen i Gävleborg och Dalarna uppgår till 700-800 björnar, jämt fördelade över de två länen. Inventeringen visar också att björnarna finns i hela Gävleborg med kärnområden i Ljusdals, Ovanåkers, Bollnäs och Ockelbo kommuner, och att stammen är glesare vid kusten och i de södra länsdelarna. Ingen riktad järvinventering har gjorts i länet de senaste åren, men järvspår dokumenteras av länsstyrelsen under lodjurs- och varginventeringen. Vintern 2012/2013 dokumenterades järv och järvspår framför allt i länets nordliga och nordvästra delar. Länsstyrelsens bedömning är att järven ökar något i dessa områden och det finns indikationer på att stammen sprider sig söderut i länet. Kungsörn inventeras av Kungsörnsgruppen, på uppdrag av länsstyrelsen, genom besök vid kända boplatser under häckningstiden. Tre lyckade häckningar konstaterades i Gävleborg 2012.

    •  Länsstyrelsen ansvarar för att årliga rovdjursinventeringar genomförs i Gävleborgs län. Syftet är attövervaka rovdjursstammarnas storlek, utbredning, genetiska status och hur stammarna utvecklasöver tiden. I den här rapporten redovisas resultaten från inventeringar av lodjur, varg, järv och kungsörn, samtlänsstyrelsens bedömning av förekomsten av björn i Gävleborg under inventeringssäsongen2014/2015. Vid lodjursinventeringen 2014/2015 kvalitetssäkrades 15 familjegrupper, det vill säga 15 honor somfött ungar 2014. Lodjursstammen i Gävleborg kan därmed uppskattas till cirka 83 individer vintern2014/2015. Fyra av familjegrupperna delades med angränsande län. Gävleborg berördes av tio vargrevir vintern 2014/2015: Prästskogen/Haverö, Naggen, Kukumäki,Sjösveden, Tandsjön, Lingbo, Åmot/Ockelbo, Bondskog, Björnås och Glamsen. I sju av revirenbekräftades att föryngring skett våren 2014. Sex av reviren delades med andra län. Det innebär attGävleborg anses ha haft 4,7 familjegrupper vintern 2014/2015, enligt Viltskadecenters fördelning.Vargarna i Korsåreviret, som tidigare delades mellan Gävleborg och Dalarna, har denna säsong vistatsväster om länsgränsen och har inte spårats i Gävleborg. Tandsjö-reviret försvann underinventeringssäsongen genom skyddsjakt i februari 2015. Ingen inventering av björn har genomförts denna säsong. Länsstyrelsen bedömer att björnstammensstorlek och spridning i länet sannolikt överensstämmer med resultatet från 2012 års spillningsinventering. Björnstammen uppskattades då till cirka 380 individer i Gävleborgs län. En riktad järvinventering har för första gången på många år genomförts i Gävleborgs län vintern2014/2015. Med hjälp av fotostationer har bilder på järv erhållits på flera ställen i hela norra delen avGävleborg och en föryngring 2014 har kvalitetssäkrats. Vid inventeringen av kungsörn i Gävleborg 2014 konstaterades totalt 25 revir, varav 8 hade lyckadehäckningar och 3 av dessa var dubbelkullar, det vill säga kullar med två ungar. Storleken på länetsfasta kungsörnspopulation 2014 uppskattas till 50-65 individer. Inventeringarna av lo, järv och varg utfördes av länsstyrelsens personal, medan kungsörninventerades av Kungsörnsgruppen Gävleborg på uppdrag av länsstyrelsen. Allmänhetensrapportering av rovdjursobservationer har varit till stor hjälp i arbetet med att göra så brainventeringar som möjligt utifrån de resurser som finns.

To face these challenges, there is a need to develop new ways of working, such as co-production. It is a challenge to enable and enhance inclusive co-productive processes, but trust and self-efficacy are key concepts for success. Trust and self-efficacy can be considered as both enablers and products of co-production and are thus important to acknowledge together with contextual factors and to act upon at all organizational levels, starting with individual patients and users.

    • That people should take responsibility for their health is a prominent contemporary idea. But what does such responsibility actually entail, and what demands are being put on people? The objective of the dissertation is to describe and critically examine various ideas on personal responsibility for health. In the first step, I identify and describe a wide variation of uses of responsibility and in the second step, I problematize them. The analyzed material consists of Swedish government reports and various types of health advice literature, including medical books and alternative medicine literature. I employ a framework of philosophical, social scientific, and developmental psychology theories on responsibility to critically examine the material.The study shows that taking responsibility involves both body and mind. Common ideas are that it requires maintaining a healthy lifestyle and managing one's self-care. But it is also considered important to take responsibility for beliefs and emotions and to adopt an accepting attitude. Another idea is that spiritual insights expand the scope of responsibility. Some important abilities required to take responsibility are conscious healthy choices, self-knowledge, and critical thinking. The view of responsibility is also influenced by individual factors, cultural beliefs about health and disease, and social structures. In the literature, health responsibility is regarded both as a social duty and as a never-ending task performed by an active individual.Finally, I use Robert Kegan's theory of adult development to show that taking responsibility imposes psychological demands on people's awareness. The demands are often higher than many people can manage. Some of the stress and poor health that people experience may be the product of an inability to manage all of life's demands. The developmental perspective also asserts that people can develop the requisite capacity. One conceivable conclusion of the study is that if we want people to take more responsibility, we should not only invest resources in health information, but also in measures that generate self-knowledge, reflection, and personal development.

  • Parent rating of intelligibility: A discussion of the construct validity of the Intelligibility in Context Scale (ICS) and normative data of the Swedish version of the ICS.

    • Intelligibility can be defined as the speakers' ability to convey a message to the listener and it is considered the key functional measure of speech. The Intelligibility in Context Scale (ICS) is a parent rating scale used to assess intelligibility in children.To describe normative and validation data on the ICS in Swedish and to investigate how these are related to age, gender and multilingualism.Two studies were included. Study 1 included ICS forms from 319 Swedish-speaking children (3:2-9:2 years:months). Study 2 included video recordings and ICS forms from 14 children with speech sound disorder (SSD) and two with typical speech. The video recordings were transcribed in the validation process, resulting in intelligibility reference scores to which ICS scores were correlated.Study 1: The mean value of the ICS for the 319 children was 4.73. There were no differences in ICS score related to age or gender. The children in the multilingual group were significantly older than the monolingual group and had significantly lower ICS scores than the group of monolinguals. Study 2: There was a moderate correlation between the ICS score and the transcription-based intelligibility score, with the two children with typical speech excluded; however, this correlation was not significant.We contribute mean scores and percentiles on the ICS for Swedish-speaking children. The finding that the ICS does not provide valid measures of intelligibility for the included children with SSD suggests that the instrument measures a different construct.What is already known on the subject The ICS has been translated to numerous languages and validated against articulation measures in several previous studies. The validity of the Swedish version has been investigated against intelligibility based on transcription of single words. What this paper adds to existing knowledge The study provides normative values of the Swedish version of the ICS for children aged 3-9 years. This is the first study to use a gold standard measure of intelligibility in continuous speech to validate the ICS. The results show a somewhat dubious validity regarding ICS for the group of children with SSD included in the study. What are the potential or actual clinical implications of this work? The ICS's suitability as a measure of intelligibility is questionable; however, it might be of use for speech and language pathologists to give an overview of the parents' view of their child's ability to communicate, in order to make a decision on possible further assessment and intervention. The normative values of the Swedish version of the ICS could be of use in this decision process.

  • Maintaining the good store : lessons about caring practices from Swedish 100-year-old retail stores

    • This article is about small-scale independent single-store retailers that have been in business since the first half of the 20th century – an overlooked group of actors in retail research. Research has mapped the broader structural changes in the retail industry that have benefitted large-scale operations during the last decade. Still, there is an absence of explanations for the continuous existence of small-scale retail businesses with product lines competing with large-scale actors. Retail research tends to take economic profitability and growth as starting points to interrogate the capacity of businesses to successfully compete. Against this background, the longevity of inner-city small-scale independent stores dating back to the early 20th century remains a puzzle, which calls for looking towards alternative theories to find reasons for the persistence of this line of retail. We engage with this puzzle by exploring practices of valuation engaged by store owners in a selection of small-scale, independent stores established in Gothenburg and Stockholm (Sweden) before the 1950s. We draw on understandings of values as produced through social practices and inquire how forms of caring practices figure in the day-to-day maintenance of the stores as means through which they produce value. The article builds on ethnographic fieldwork focusing on in-store interviews with store owners, employees, and customers complemented with observations. We find that care figures in these stores as expressions of attention and presence, of maintenance and of tacit knowledge, and suggest that the retailers’ focus on the caring practices identified – rather than on prioritizing growth – is key to their continued, long-term existence. Anchored in an understanding of retail as situated in broader social and political processes, and as such also impacting society at large, we moreover emphasise the importance of attentiveness and valuation of, the caring practices that small-scale store owners engage in.

  • Measurement and outcomes of co-production in health and social care : A systematic review of empirical studies

    • Background: Co-production is promoted as an effective way of improving the quality of health and social care but the diversity of measures used in individual studies makes their outcomes difficult to interpret.Objective: The objective is to explore how empirical studies in health and social care have described the outcomes of co-production projects and how those outcomes were measured.Design and methods: A scoping review forms the basis for this systematic review. Search terms for the concepts (co-produc∗ OR coproduc∗ OR co-design∗ OR codesign∗) and contexts (health OR 'public service∗ OR "public sector") were used in: CINAHL with Full Text (EBSCOHost), Cochrane Central Register of Controlled trials (Wiley), MEDLINE (EBSCOHost), PsycINFO (ProQuest), PubMed (legacy) and Scopus (Elsevier). There was no date limit. Papers describing the process, original data and outcomes of co-production were included. Protocols, reviews and theoretical, conceptual and psychometric papers were excluded. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses guideline was followed. The Mixed Methods Appraisal Tool underpinned the quality of included papers.Results: 43 empirical studies were included. They were conducted in 12 countries, with the UK representing >50% of all papers. No paper was excluded due to the Mixed Methods Quality Appraisal screening and 60% of included papers were mixed methods studies. The extensive use of self-developed study-specific measures hampered comparisons and cumulative knowledge-building. Overall, the studies reported positive outcomes. Co-production was reported to be positively experienced and provided important learning.Conclusions: The lack of common approaches to measuring co-production is more problematic than the plurality of measurements itself. Co-production should be measured from three perspectives: outputs of co-production processes, the experiences of participating in co-production processes and outcomes of co-production. Both self-developed study-specific measures and established measures should be used. The maturity of this research field would benefit from the development and use of reporting guidelines.

  • High striving, high costs? : A qualitative examination of perfectionism in high-level dance.

    • This study provides the first in-depth investigation of how perfectionism is experienced in high-level dance. Seventy-seven students (M age = 15.52, SD = 2.30) completed a perfectionism questionnaire. Next, dancers with the highest and lowest levels of perfectionistic strivings (PS) and perfectionistic concerns (PC) were recruited for interview (N = 8), as representatives of the four subtypes of perfectionism in the 2 × 2 model. Distinct profiles emerged for the four quadrants of the 2 × 2 model in relation to four key themes: self-regulation, achievement goals, views on mistakes, and role of others. Dancers with high PS displayed the highest levels of self-regulation while dancers with low PC appeared most task-oriented. Dancers with high PC reported holding less favorable views on mistakes and placed greater emphasis on the opinions of others. Importantly, PS appeared to be both helpful and hurtful. Findings are discussed in relation to theory intermixed with practical recommendations.

I åtta av reviren bekräftades att föryngring skett våren2015. Sju av reviren delades med andra län. Utöver dessa elva revir har tre ensamma stationäravargar spårats. Dessa är inte DNA-profilerade men kvalitetsäkrade som stationära vargar enligtinventeringsföreskrifterna. Vid inventeringen av kungsörn i Gävleborg 2015 konstaterades totalt 23 intakta revir med 14häckande par, varav 8 hade lyckade häckningar. Storleken på länets fasta kungsörnspopulation 2015uppskattas till drygt 50 individer. Ingen inventering av björn har genomförts denna säsong. Länsstyrelsen bedömer att björnstammensstorlek i länet minskat något sedan den senaste inventeringen som genomfördes 2012.Björnstammen upp-skattades då till cirka 380 individer i Gävleborgs län. Järvinventeringen blev bristfällig vintern 2015/2016. Huvudsyftet med inventeringen är att fastställaantalet föryngringar som skett inom länet. Järvarna föder sina ungar i februari och honan lämnar sinlya efter någon vecka med ungarna. Snön försvann redan i början på mars och förutsättningarna attspåra en hona tillbaka till lyan försvann med snön. Länsstyrelsen bedömer att det sker mellan 4-8föryngringar inom Gävleborgs län. Spår av järv blir allt vanligare söderut i länet. Länets järvpopulationär som tätast i de norra och nordvästra delarna. Inventeringarna av lo, järv och varg utfördes av länsstyrelsens personal, medan kungsörninventerades av Kungsörnsgruppen Gävleborg på uppdrag av länsstyrelsen. Allmänhetensrapportering av rovdjursobservationer har varit till stor hjälp i arbetet med att göra så brainventeringar som möjligt utifrån de resurser som finns. Rapportering av rovdjursobservationer görs till skandobs.se eller till mobilapplikationen Skandobstouch, som är gratis att ladda ner.

Lodjursstammeni Gävleborg vintern 2016/2017 uppskattas till cirka 96 individer.Gävleborg berördes vintern 2016/2017 av 15 vargrevir, varav 8 delades med andra län. Länet hade5,3 länsegna familjegrupper och 1,5 revirmarkerande par.Resultatet från 2016 års kungsörnsinventering i länet är två lyckade häckningar och sex misslyckade.Storleken på länets fasta kungsörnspopulation 2016 uppskattas till cirka 70 individer.År 2012 uppskattades antalet björnar i Gävleborg till cirka 380.     • I denna rapport redovisas resultaten från länsstyrelsens inventeringar av lodjur ochvarg i Gävleborg under inventeringssäsongen 2011/2012, samt information om förekomstenav björn, järv och kungsörn i länet.Länsstyrelserna har det regionala ansvaret för rovdjursinventeringar i länen. Inventeringarnai Gävleborg genomförs i enlighet med Naturvårdsverkets föreskrifter ochallmänna råd (NFS 2007:10) om inventering av björn, varg, järv, lodjur och kungsörn.Resultaten från lodjurs- och varginventeringen är huvudsakligen baserade påsnöspårning genom snoking och mindre områdesinventeringar. I varginventeringenanvänds också DNA-analys av insamlad spillning. Jägares och allmänhetens rapporteringav rovdjursobservation till länsstyrelsens hemsida rovobs.se har varit till storhjälp i arbetet med att inventera länets rovdjur.Vid inventeringen av lodjur i Gävleborg 2011/2012 konstaterades totalt 19 familjegrupper/föryngringar (det vill säga hona med ungar), varav 1 delas med Uppsla län.Resultat från varginventeringen visar att Gävleborg 2011/2012 berördes av sex vargrevir,varav tre delas med andra län. I tre av länets revir bekräftades föryngringar.Vid slutet av spårsäsongen återfanns den så kallade Galvenhannen i länets norradelar. DNA-analys av spillning visade att han hade bildat par med en tik från norskaKynnareviret. Denna parbildning anses vara genetiskt mycket viktig för den Skandinaviskavargstammen. Parets nya revir har namnet Prästskogen. I vargreviret Korså2 har ett skifte av alfahane skett, vilket upptäcktes genom DNA-analys av spillning.Den nya hannen härstammar från reviret Kynna 2.Länets björnstam har inte inventerats på flera år. Länsstyrelsen bedömer dock attlänet har en fast björnstam med kärnområden i Ljusdals, Ovanåkers och Bollnäskommuner. Under hösten 2012 sker en länstäckande spillningsinventering.I länet har ingen riktad järvinventering utförts på flera år. Länsstyrelsen bedömerdock att järven ökar något i antal i länets norra och nordvästra delar, samt att populationentycks sprida sig söderut i länet.Inventering av kungsörn genomförs av Kungsörnsgruppen och inventeringsresultatenredovisas på deras hemsida: http://s-norell.se/k-orn/Resultat.htm

  • Asymmetric Transfer Hydrogenation of Aromatic Ketones : Catalyst Development and Mechanistic Studies

    • This thesis describes the development and evaluation of new chiral Ru(arene)(amino alcohol) catalysts for the transfer hydrogenation of aromatic ketones using isopropanol as the hydrogen source. Two mechanistic studies of the Ru(arene)(amino alcohol) catalyzed transfer hydrogenation of acetophenone were also conducted. The Ru(arene)[(1S,3R,4R)-3.(Hydroxymethyl)-2-azabicyclo[2.2.1]heptane] catalyst was optimized for catalytic asymmetric transfer hydrogenation of aromatic ketones. The effect of substituents on the arene ligand on both selectivity and reactivity was investigated. The performance of the catalyst was also optimized by altering the structure of the chiral amino alcohol ligand. These optimizations resulted in a highly active and selective catalyst, Ru(p-cymene)[(R)-1-[(1S,2R,6S,7R,9R)-4,4-Dimethyl-3,5-dioxa-8-aza-tricyclo[5.2.1.00,0]dec-9-yl]-ethanol], for asymmetric transfer hydrogenation of aromatic ketones. This catalyst was capable of reducing acetophenone in 96% ee in 4 minutes at room temperature at a substrate to catalyst ratio of 200. Full conversion was reached even at a substrate to catalyst ratio of 5000 and the high enantioselectivity of 96% ee was maintained. A range of prochiral aromatic ketones with electron withdrawing or electron donating substituents in any position on the aromatic ring were reduced in short reaction times and with high enantioselectivity, up to 99% ee. Bulky aryl alkyl ketones were also reduced with high enantioselectivity.A combined quantum chemical and kinetic investigation of the Ru(arene)(amino alcohol) catalyzed transfer hydrogenation of acetophenone was conducted. Three possible mechanisms were studied and the quantum chemical calculations indicated that the mechanism was concerted. In addition, a kinetic isotope study for the Ru(arene)(amino alcohol) catalyzed transfer hydrogenation of acetophenone was conducted. The determination of the kinetic isotope effect of the proton and hydride transfer showed that the proton and the hydride transfer were both rate-limiting and occurred in the same step. This result supports the proposed concerted mechanism.

    • Quality of care is determined not only by political decisions but also by how those policies are understood and managed by professionals when put into action. Home care services, the most common form of elder care in Sweden today, should include social support, which is very important for health and wellbeing. And yet, support for social participation seems to be lacking. Understanding prevalent social constructs and their possible impacts on focus and content of social practice in home care could reveal ways to address social support in home care. Therefore, this article highlights how professionals in home care provision talk about older home care recipients' loneliness and social needs, and how these repertoires are related to professionals' opportunities and obligations to support those social needs. The study included 22 persons from different professions in home care provision, from two municipalities in northern Sweden. Nine individual interviews and four group interviews were conducted, recorded, transcribed, and analyzed using a discourse psychology approach. The results show two interpretative repertoires in which notions of otherness and likeness guided definitions and support regarding loneliness, social needs, and social support. This study reveals assumptions that underpin and structure the practices of home care. As the interpretative repertoires provided differing and partly opposing views on how to provide social support and combat loneliness, it seems important to also address the broader issues of professional identities and how loneliness is defined and approached.

    • Purpose The aim of the study was to explore experiences and thoughts in the process of returning to work in employed patients with Exhaustion Disorder. Methods Twelve patients with Exhaustion Disorder (burnout) who had been referred to a Stress Rehabilitation Clinic were interviewed. All patients were employed but a majority was on full or part-time sick leave. Grounded Theory was used as the qualitative method. Results A core category, regaining the ability to work, was developed. Alongside, two categories, internal resources and the external support system, were experienced as being important to the process. The internal resources were expressed through three key features (sub-categories), perceived validation, insights and adaptive coping abilities. The external support system was diverse and described by the sub-categories practical/structural and/or emotional support. Four external support actors were identified; the workplace, health care, the Social Insurance Agency, and the union. The supervisor was described as the most important external actor. Conclusions Internal and external resources are intertwined in the process of regaining the ability to work. The internal resources and external support can directly increase the probability to regain the ability to work. Moreover, these resources can affect each other and thus indirectly have an effect on the process.

    • Introduction The aim of this study was to investigate the impact of psychosocial working conditions and coping strategies at work on change in sick leave level for patients on long-term sick leave due to burnout.Methods A cohort sample of patients (n = 117) on long-term sick leave due to burnout was analyzed. The patients answered a questionnaire at baseline and sick leave information was collected from the Swedish Social Insurance Agency at baseline and at follow-up 2 years later. Two groups were formed depending on whether the patients had "improved" and reduced their sick leave level (56%) or if the sick leave level was "unchanged" (44%) at follow-up. The association between change in sick leave and predictors measuring psychosocial working conditions and coping strategies at work were analyzed using logistic regression.Results The predictor, low control at work, was associated with unchanged sick leave at follow-up. When background characteristics were taken into account, usage of covert coping towards supervisors and covert coping towards workmates, respectively, also predicted unchanged sick leave level. High overcommitment was of borderline significance and associated with a reduced sick leave level at follow-up.Conclusions Patients with burnout who have experienced low control at work and used covert coping towards supervisors and/or workmates have a higher risk of not reducing their sick leave after rehabilitation. The workplace may contribute to a reduction of sick leave lengths with a more flexible work environment and improvement in communication strategies for employees and supervisors.

    • Alemtuzumab (ALZ) is a pulsed immune reconstitution therapy for multiple sclerosis (MS).To assess basic characteristics, therapeutic effects, and prognostic biomarkers on clinical and imaging parameters of disease activity for relapsing-remitting MS (RRMS) patients selected for ALZ, in a real-world long-term setting.Fifty-one RRMS patients [female=31; mean age 36 (standard deviation 7.1) years; median expanded disability status scale (EDSS) 2 (interquartile range (IQR) 1.5)] initiating ALZ treatment, were consecutively included. Patients were assessed at baseline and thereafter annually for 5years with clinical measures, symbol digit modality test (SDMT), and magnetic resonance imaging (MRI). Concentrations of glial fibrillary acidic protein (GFAP), reflecting astrogliosis, and neurofilament light (NfL), reflecting axonal damage, were measured in cerebrospinal fluid (CSF) and serum samples collected at baseline and after 2years in CSF, and annually in serum. Control subjects were symptomatic controls (SCs, n=27), who were examined at baseline and after 5years without evidence of neurological disease.While the mean annualized relapse rate was significantly reduced from baseline at each year of follow-up, disability was essentially maintained at a median EDSS of 1.5 and IQR between 1.13 and 2.25. New MRI activity was recorded in 26 patients (53%) over 5years. The proportion of patients who achieved no evidence of disease activity (NEDA-3), 6-months confirmed disability worsening (CDW), and 6-months confirmed disability improvement (CDI) at 5years were 33, 31, and 31%, respectively. The SDMT score was reduced for patients (p<0.001), but unchanged for SCs. ALZ treatment did not change GFAP levels, whereas there was a significant decrease for RRMS patients in median CSF and serum NfL levels at follow-up [CSF month 24: 456pg./mL (IQR 285.4) (p=0.05); serum month 24: 6.7pg/mL (IQR 4.7) (p<0.01); serum month 60: 7.2pg/mL (IQR 4.7) (p<0.01)], compared to baseline [CSF: 1014pg/mL (IQR 2832.5); serum 8.6pg/mL (IQR 17.4)].In this real-world mono-center population, we observed a progression-free survival of 69%, cumulative NEDA-3 of 33%, and reduced NfL levels, over a five-year follow-up. This confirms ALZ as an effective pulsed immune reconstitution therapy that significantly reduces neuro axonal loss, and therefore has the potential to reduce long-term neurological disability. ALZ did not appear to affect astrogliosis.

    • BACKGROUND: Co-production of health and care involving patients, families of patients, and professionals in care processes can create joint learning about how to meet patients' needs. Although barriers and facilitators to co-production have been examined previously in various health care contexts, the preconditions in Swedish chronic cardiac care contexts are yet to be explored. This study is set in the health system of the Swedish region of Jönköping County and is part of system-wide efforts to promote better health for persons with heart failure (HF).OBJECTIVE: The objective of this study was to test the usefulness of the Capability, Opportunity, and Motivation Behavior (COM-B) model when assessing the barriers to and facilitators of co-production of health and care perceived by patients with HF, family members of patients with HF, and professionals in a Swedish chronic cardiac care context as a guide for subsequent initiatives.METHODS: Data collection involved 1 focus group interview (FGI) with patients with HF (n=5), 1 FGI with family members of patients with HF (n=5), 1 FGI with professionals in primary care (n=7), and 1 FGI with professionals in cardiac care (n=4). In addition, patients with HF kept diaries of their thoughts regarding co-production. Using a deductive approach to content analysis, underpinned by the COM-B model, barriers and facilitators were categorized into capabilities, opportunities, and motivations to co-produce health and care.RESULTS: The participants showed limited understanding of co-production as a practice. They appeared to view it as a privilege to be offered to patients on top of traditional care and rarely as an approach for improving health care processes. The interviews revealed the limited health literacy among patients and the struggle of professionals to convey health information to these patients. Co-production was considered to be more resource-intensive than traditional care. Different expectations of stakeholders' roles were revealed: professionals expected older patients not to want to co-produce health and care, and all participants expected professionals to be in charge of health care services. The family members' position involved trying to balance their desire to support their relatives with understanding when, how, and with whom to co-produce. Presumed benefits motivated stakeholders: co-production was recognized to motivate patients to improve self-care. However, the participants recognized that motivation to get involved in health and care decisions varies over time among stakeholders.CONCLUSIONS: Co-production can be facilitated by the stakeholders' motivation. However, varying levels of understanding of co-production, patients' limited health literacy, unease with power sharing between patients and professionals, and resource constraints are barriers that need to be managed to promote co-produced care and better health for persons living with HF. Further research is warranted to explore how to co-produce health care services with patients with HF and how leaders can facilitate the inevitable cultural change it requires and represents.

    • Background An improvement initiative sought to improve care for atrial fibrillation (AF) patients; many felt insecure about how to cope with AF.Objective To reveal AF patients' and professionals' experiences of pilot-testing a Learning Café group education programme, aimed at increasing the patients' sense of security in everyday life.Design Using an organizational case study design, we combined quantitative data (patients' sense of security) and qualitative data (project documentation; focus group interviews with five patients and five professionals) analysed using inductive qualitative content analysis.Setting AF patients and a multiprofessional team at a cardiac care unit in a Swedish district hospital.Improvement activities Two registered nurses invited AF patients and partners to four 2.5-hour Learning Café sessions. In the first session, they solicited participants' questions about life with AF. A physician, a registered nurse and a physiotherapist were invited to address these questions in the remaining sessions.Results AF patients reported gaining a greater sense of security in everyday life and anticipating a future shift from emergency care to planned care. Professionals reported enhanced professional development, learning more about person-centredness and gaining greater control of their own work situation. The organization gained knowledge about patient and family involvement.Conclusions The Learning Café pilot test?exemplifying movement towards co-production through patient-professional collaboration?generated positive outcomes for patients (sense of security), professionals (work satisfaction; learning) and the organization (better care) in line with contemporary models for quality improvement and with Self-Determination Theory. This approach merits further testing and evaluation in other contexts.

  • Using stakeholders' experiences to redesign health services for persons living with heart failure : a case study protocol in a Swedish cardiac care setting [Protocol]

    • INTRODUCTION: Clinical guidelines promote recognising persons with heart failure (referred to as PWHF) as coproducers of their own care. Coproduction of healthcare-involving PWHF, families and professionals in care processes-aims to promote the best possible health. Still, it is unclear how to coproduce heart failure (HF) care. This study explores whether and how Experience-Based Co-Design (EBCD) involving PWHF, family members and professionals can be undertaken online, in a Swedish cardiac care setting, to codesign improved experiences of HF care.METHODS AND ANALYSIS: In EBCD, stakeholders' experiences are solicited to redesign healthcare services. First, we will undertake a thematic analysis of field notes from consultations and filmed/audio-recorded interviews with PWHF (n=10-12). This analysis will identify 'touchpoints' (emotionally positive/negative events that shape overall service experiences), edited into a 'trigger film'. Next, a thematic analysis of family members' (n=10-12) and professionals' (n=10-12) interviews will identify key themes mirroring their experiences. Separate feedback events with each stakeholder group will confirm identified touchpoints and key themes and identify areas for HF care improvement. At a joint event, prompted by the 'trigger film', stakeholders will agree on one area for HF care improvement. A team including PWHF, family members and professionals, led by an improvement adviser, will then plan, design, implement and evaluate an improvement activity addressing the identified problem area. A deductive thematic analysis of field notes, project documentation and stakeholder focus group interviews, underpinned by MUSIQ, will identify how organisational conditions influence the process. Quantitative measurements, describing the results of the improvement activity, will be integrated with qualitative data to strengthen the case. To reduce resource intensity, we will use online tools during the process.ETHICS AND DISSEMINATION: The Swedish Ethical Review Authority approved the study in May 2021. The results will be disseminated through seminars, conference presentations and publications.
