  • Obesity and nocturnal gastro-oesophageal reflux are related to onset of asthma and respiratory symptoms

    • Several studies have identified obesity as a risk factor for asthma in both children and adults. An increased prevalence of asthma in subjects with gastro-oesophageal reflux (GOR) and obstructive sleep apnoea syndrome has also been reported. The aim of this investigation was to study obesity, nocturnal GOR and snoring as independent risk factors for onset of asthma and respiratory symptoms in a Nordic population. In a 5-10 yr follow-up study of the European Community Respiratory Health Survey in Iceland, Norway, Denmark, Sweden and Estonia, a postal questionnaire was sent to previous respondents. A total of 16,191 participants responded to the questionnaire. Reported onset of asthma, wheeze and night-time symptoms as well as nocturnal GOR and habitual snoring increased in prevalence along with the increase in body mass index (BMI). After adjusting for nocturnal GOR, habitual snoring and other confounders, obesity (BMI >30) remained significantly related to the onset of asthma, wheeze and night-time symptoms. Nocturnal GOR was independently related to the onset of asthma and in addition, both nocturnal GOR and habitual snoring were independently related to onset of wheeze and night-time symptoms. This study adds evidence to an independent relationship between obesity, nocturnal gastro-oesophageal reflux and habitual snoring and the onset of asthma and respiratory symptoms in adults.

  • Insomnia is more common among subjects living in damp buildings

    • BACKGROUND: Insomnia is a condition with a high prevalence and a great impact on quality of life. Little is known about the relation between and sleep disturbances and the home environment. AIM: To analyse the association between insomnia and building dampness. METHODS: In a cross-sectional, multicentre, population study, 16 190 subjects (mean age 40 years, 53% women) were studied from Reykjavik in Iceland, Bergen in Norway, Umea, Uppsala, and Goteborg in Sweden, Aarhus in Denmark, and Tartu in Estonia. Symptoms related to insomnia were assessed by questionnaire. RESULTS: Subjects living in houses with reported signs of building dampness (n = 2873) had a higher prevalence of insomnia (29.4 v 23.6%; crude odds ratio 1.35, 95% CI 1.23 to 1.48). The association between insomnia and different indicators of building dampness was strongest for floor dampness: "bubbles or discoloration on plastic floor covering or discoloration of parquet floor" (crude odds ratio 1.96, 95% CI 1.66 to 2.32). The associations remained significant after adjusting for possible confounders such as sex, age, smoking history, housing, body mass index, and respiratory diseases. There was no significant difference between the centres in the association between insomnia and building dampness. CONCLUSION: Insomnia is more common in subjects living in damp buildings. This indicates that avoiding dampness in building constructions and improving ventilation in homes may possibly have a positive effect on the quality of sleep.

  • Deciduous tooth biomarkers reveal atypical fetal inflammatory regulation in autism spectrum disorder.

    • Atypical regulation of inflammation has been proposed in the etiology of autism spectrum disorder (ASD); however, measuring the temporal profile of fetal inflammation associated with future ASD diagnosis has not been possible. Here, we present a method to generate approximately daily profiles of prenatal and early childhood inflammation as measured by developmentally archived C-reactive protein (CRP) in incremental layers of deciduous tooth dentin. In our discovery population, a group of Swedish twins, we found heightened inflammation in the third trimester in children with future ASD diagnosis relative to controls (n = 66; 14 ASD cases; critical window: -90 to -50 days before birth). In our replication study, in the US, we observed a similar increase in CRP in ASD cases during the third trimester (n = 47; 23 ASD cases; -128 to -21 days before birth). Our results indicate that the third trimester is a critical period of atypical fetal inflammatory regulation in ASD.

  • SwePep – A database designed for endogenous peptides and mass spectrometry

    • A new database, SwePep, specifically designed for endogenous peptides, has been constructed to significantly speed up the identification process from complex tissue samples utilizing mass spectrometry. In the identification process the experimental peptide masses are compared with the peptide masses stored in the database both with and without possible post-translational modifications. This intermediate identification step is fast and singles out peptides that are potential endogenous peptides and can later be confirmed with tandem mass spectrometry data. Successful applications of this methodology are presented. The SwePep database is a relational database developed using MySql and Java. The database contains 4180 annotated endogenous peptides from different tissues originating from 394 different species as well as 50 novel peptides from brain tissue identified in our laboratory. Information about the peptides, including mass, isoelectric point, sequence, and precursor protein, is also stored in the database. This new approach holds great potential for removing the bottleneck that occurs during the identification process in the field of peptidomics. The SwePep database is available to the public.

  • Remission of asthma : a prospective longitudinal study from northern Europe (RHINE study).

    • The aim of the present study was to investigate the remission rate of adult asthma in a general population sample in relation to age, sex, asthma symptoms, allergic rhinitis and smoking. A follow-up of the random population samples from the European Community Respiratory Health Survey in Northern Europe was conducted from 1999-2001 on 1,153 individuals (aged 2653 yrs) with reported asthma. Remission was defined as no asthmatic symptoms in two consecutive years and no current use of asthma medication. Remission rates per 1,000 personyrs were calculated and Cox regression models, adjusting for confounders, were used to estimate hazard ratios (HR) with 95% confidence intervals (Cl). An average remission rate of 20.2 per 1,000 person-yrs was found. There was no significant difference according to sex; the remission rates were 21.7 and 17.8 per 1,000 person-yrs in females and males, respectively. An increased remission rate was observed among subjects who quit smoking during the observation period. Subjects not reporting any asthma symptom at baseline had an increased remission rate. In the Cox regression model, ex-smoking (HR 1.65, 95% Cl 1.01-2.71) was associated with increased remission rate, and reporting any asthma symptom at baseline was associated with decreased remission rate (HR 0.7, 95% Cl 0.40-0.90). In conclusion, the present prospective longitudinal study showed that quitting smoking and the presence of mild disease appeared to favour remission.

  • Salivary Biomarkers and Oral Health in Liver Transplant Recipients, with an Emphasis on Diabetes

    • Salivary biomarkers have been linked to various systemic diseases. We examined the association between salivary biomarkers, periodontal health, and microbial burden in liver transplant (LT) recipients with and without diabetes, after transplantation. We hypothesized that diabetic recipients would exhibit impaired parameters. This study included 84 adults who received an LT between 2000 and 2006 in Finland. Dental treatment preceded transplantation. The recipients were re-examined, on average, six years later. We evaluated a battery of salivary biomarkers, microbiota, and subjective oral symptoms. Periodontal health was assessed, and immunosuppressive treatments were recorded. Recipients with impaired periodontal health showed higher matrix metalloproteinase-8 (MMP-8) levels (p < 0.05) and MMP-8/tissue inhibitor of matrix metalloproteinase 1 (TIMP1) ratios (p < 0.001) than recipients with good periodontal health. Diabetes post-LT was associated with impaired periodontal health (p < 0.05). No difference between groups was found in the microbial counts. Salivary biomarker levels did not seem to be affected by diabetes. However, the advanced pro-inflammatory state induced by and associated with periodontal inflammation was reflected in the salivary biomarker levels, especially MMP-8 and the MMP-8/TIMP-1 molar ratio. Thus, these salivary biomarkers may be useful for monitoring the oral inflammatory state and the course of LT recipients.

    • The multi-criteria decision analysis (MCDA) method provides for a comprehensive and transparent basis for performing sustainability assessments. Development of a relevant MCDA-method requires consideration of a number of key issues, e.g. (a) definition of assessment boundaries, (b) definition of performance scales, both temporal and spatial, (c) selection of relevant criteria (indicators) that facilitate a comprehensive sustainability assessment while avoiding double-counting of effects, and (d) handling of uncertainties. Adding to the complexity is the typically wide variety of inputs, including quantifications based on existing data, expert judgements, and opinions expressed in interviews. The SCORE (Sustainable Choice Of REmediation) MCDA-method was developed to provide a transparent assessment of the sustainability of possible remediation alternatives for contaminated sites relative to a reference alternative, considering key criteria in the economic, environmental, and social sustainability domains. The criteria were identified based on literature studies, interviews and focus-group meetings. SCORE combines a linear additive model to rank the alternatives with a non-compensatory approach to identify alternatives regarded as non-sustainable. The key strengths of the SCORE method are as follows: a framework that at its core is designed to be flexible and transparent; the possibility to integrate both quantitative and qualitative estimations on criteria; its ability, unlike other sustainability assessment tools used in industry and academia, to allow for the alteration of boundary conditions where necessary; the inclusion of a full uncertainty analysis of the results, using Monte Carlo simulation; and a structure that allows preferences and opinions of involved stakeholders to be openly integrated into the analysis. A major insight from practical application of SCORE is that its most important contribution may be that it initiates a process where criteria otherwise likely ignored are addressed and openly discussed between stakeholders.

    • Comparisons of transcriptional and translational expression in normal and abnormal states are important to reach an understanding of pathogenesis and pathophysiology. Maintaining the biochemical, molecular, and structural sample integrity is essential for correct sample comparisons. We demonstrate that both proteins and neuropeptides, including their PTMs, are subjected to massive degradation in the brain already 1 min postmortem. Further, markers for determining the integrity and status of a biological sample were identified. The protein fragment stathmin 2-20 correlated well with the general level of postmortem degradation and may serve as a sample quality indicator for future work, both in animal and human postmortem brains. Finally, a novel method for preventing degradation of proteins and peptides in postmortem tissue is presented using rapid and uniform conductive heat transfer on tissue prior to the actual sample preparation procedures, which enables the relatively low-abundant neuropeptides to remain intact, minimizes degradation of proteins by proteolysis, and conserves the PTMs of the neuropeptides.

    • There is an increasing demand amongst decision-makers and stakeholders for identifying sustainable remediation alternatives at contaminated sites, taking into account that remediation typically results in both positive and negative consequences. Multi-criteria analysis (MCA) is increasingly used for sustainability appraisal, and the Excel-based MCA tool Sustainable Choice Of REmediation (SCORE) has been developed to provide a relevant and transparent assessment of the sustainability of remediation alternatives relative to a reference alternative, considering key criteria in the economic, environmental and social sustainability domains, and taking uncertainty into explicit account through simulation. The focus of this paper is the use of cost-benefit analysis (CBA) as a part of SCORE for assessing the economic sustainability of remediation alternatives. An economic model is used for deriving a cost-benefit rule, which in turn motivates cost and benefit items in a CBA of remediation alternatives. The empirical part of the paper is a CBA application on remediation alternatives for the Hexion site, a former chemical industry area close to the city of Goteborg in SW Sweden. The impact of uncertainties in and correlations across benefit and cost items on CBA results is illustrated. For the Hexion site, the traditional excavation-and-disposal remediation alternative had the lowest expected net present value, which illustrates the importance of also considering other alternatives before deciding upon how a remediation should be carried out.

    • A 600 MW offshore wind farm is under construction in the Netherlands Exclusive Economic Zone at a site called Gemini situated 55 km north of the Wadden Sea island of Schiermonnikoog and 85 km from the nearest Dutch port ofEemshaven. This chapter investigates the option of introducing a multi-use design for the Gemini site by adding mussel cultivation (48 kt wet weight per year) andseaweed cultivation (480 kt wet weight per year) to the wind farm. An institutional analysis indicates a political will in the Netherlands to support the development ofadding uses to offshore wind farms, but a number of implementation obstacles are also identified. Those obstacles include an absence of licences for multi-use production and legal restrictions against third-party access to wind farms. There istherefore a need for a regulatory framework for multi-use and trust-building amongactors involved in multi-use installations. A financial and economic assessment,and a cost-benefit analysis also taking into account monetized changes in CO2 emissions, indicate that adding mussel cultivation to the wind farm is likely to be both financially and socio-economically viable. Including a seaweed cultivation function is probably not financially and socio-economically viable under currenttechnical and economic conditions. Knowledge gaps and uncertainties in these assessments with respect to, for example, missing site-specific data and non-monetizedexternalities suggest further research, also including pilot cultivations of mussels and seaweed in planned single-use or multi-use installations.
