    • The cervical cancer screening programmes in Sweden and Norway have successfully reduced the frequency of cervical cancer incidence but have not implemented any form of evaluation for screening needs. This means that the screening frequency for individuals can he suboptimal, increasing either the cost of the programme or the risk of missing an early stage cancer development. We developed a framework for assessing an individual's risk of cervical cancer based on their available screening history and computing a primary risk factor called CRS from a data-driven separation model together with multiple derived attributes. The results show that this approach is highly practical, validates against multiple established trends, and can he effective in personalizing the screening needs for individuals.

  • Occupational risk of COVID-19 in the first versus second epidemic wave in Norway, 2020

    • Background: The occupational risk of COVID-19 may be different in the first versus second epidemic wave. Aim: To study whether employees in occupations that typically entail close contact with others were at higher risk of SARS-CoV-2 infection and COVID-19-related hospitalisation during the first and second epidemic wave before and after 18 July 2020, in Norway. Methods: We included individuals in occupations working with patients, children, students, or customers using Standard Classification of Occupations (ISCO-08) codes. We compared residents (3,559,694 on 1 January 2020) in such occupations aged 20–70 years (mean: 44.1; standard deviation: 14.3 years; 51% men) to age-matched individuals in other professions using logistic regression adjusted for age, sex, birth country and marital status. Results: Nurses, physicians, dentists and physiotherapists had 2–3.5 times the odds of COVID-19 during the first wave when compared with others of working age. In the second wave, bartenders, waiters, food counter attendants, transport conductors, travel stewards, childcare workers, preschool and primary school teachers had ca1.25–2 times the odds of infection. Bus, tram and taxi drivers had an increased odds of infection in both waves (odds ratio: 1.2–2.1). Occupation was of limited relevance for the odds of severe infection, here studied as hospitalisation with the disease. Conclusion: Our findings from the entire Norwegian population may be of relevance to national and regional authorities in handling the epidemic. Also, we provide a knowledge foundation for more targeted future studies of lockdowns and disease control measures.

    • Vi som har skrivit denna artikel är alla verksamma inom forskningsmiljön Svenska med didaktisk inriktning (SMDI) vid Malmö högskola. Frågor om språkutveckling i bred bemärkelse står här i centrum. För närvarande är vi engagerade i projektet ”SMDI och lärande i medielandskapet 2.0”. Vår teoretiska plattform kan beskrivas som medieekologisk, vilket kortfattat uttryckt innebär att vi är intresserade av de mångfaldiga och komplexa relationerna mellan medier och kommunikativa kompetenser (Elmfeldt & Erixon 2007; Erixon 2012; Hayles 2002). Dessa relationer förstås därför inte, som så ofta annars i skolsammanhang, i termer av enkelriktad påverkan eller effekter (exempelvis datorns och internets negativa inverkan på skriftspråket). Vår huvudpoäng i denna artikel är att medie- och informationskunnighet, MIK, handlar om, borde handla om, framför allt två saker: språkutveckling och demokrati.

    • Paper-reinforced plastics are gaining increased interest as packaging materials, where mechanical properties are of great importance. Strength and stress transfer in paper sheets are controlled by fibre-fibre bonds. In paper-reinforced plastics, where the sheet is impregnated with a polymer resin, other stress-transfer mechanisms may be more important. The influence of fibre-fibre bonds on the strength of paper-reinforced plastics was therefore investigated. Paper sheets with different degrees of fibre-fibre bonding were manufactured and used as reinforcement in a polymeric matrix. Image analysis tools were used to verify that the difference in the degree of fibre-fibre bonding had been preserved in the composite materials. Strength and stiffness of the composites were experimentally determined and showed no correlation to the degree of fibre-fibre bonding, in contrast to the behaviour of unimpregnated paper sheets. The degree of fibre-fibre bonding is therefore believed to have little importance in this type of material, where stress is mainly transferred through the fibre-matrix interface.

  • Risk stratification in cervical cancer screening by complete screening history : Applying bioinformatics to a general screening population

    • Women screened for cervical cancer in Sweden are currently treated under a one-size-fits-all programme, which has been successful in reducing the incidence of cervical cancer but does not use all of the participants' available medical information. This study aimed to use women's complete cervical screening histories to identify diagnostic patterns that may indicate an increased risk of developing cervical cancer. A nationwide case-control study was performed where cervical cancer screening data from 125,476 women with a maximum follow-up of 10 years were evaluated for patterns of SNOMED diagnoses. The cancer development risk was estimated for a number of different screening history patterns and expressed as Odds Ratios (OR), with a history of 4 benign cervical tests as reference, using logistic regression. The overall performance of the model was moderate (64% accuracy, 71% area under curve) with 61-62% of the study population showing no specific patterns associated with risk. However, predictions for high-risk groups as defined by screening history patterns were highly discriminatory with ORs ranging from 8 to 36. The model for computing risk performed consistently across different screening history lengths, and several patterns predicted cancer outcomes. The results show the presence of risk-increasing and risk-decreasing factors in the screening history. Thus it is feasible to identify subgroups based on their complete screening histories. Several high-risk subgroups identified might benefit from an increased screening density. Some low-risk subgroups identified could likely have a moderately reduced screening density without additional risk.

    • Polyfluorinated alkyl compounds (PFCs) are a group of chemicals of growing concern that have been detected in biological and abiotic samples worldwide. This study reports the concentrations of a suite of PFCs: perfluorooctyl sulfonate (PFOS), perfluorooctyl sulfonamide (PFOSA) and perfluorinated carboxylic acids (PFCAs) in guillemot (Uria aalge) eggs, collected in North-Western Europe, from Iceland, the Faroe Islands, Sweden and two locations in Norway. The highest concentrations of PFOS were found in samples from Sweden (mean 400 ng g(-1) wet weight (w.w.)), which were almost five times higher than concentrations found in Norwegian samples (mean 85 ng g(-1) w.w. from both sample sites). The concentrations found in Icelandic and Faroe samples were lowest (mean 16 and 15 ng g(-1) w.w., respectively). Only Swedish samples differed signiflcantly from the other locations. In general, PFCAs show a different spatial trend than PFOS. Perfluorooctanoic acid (PFOA) was not detected in any sample and perfluorononanoic acid (PFNA) was only detected in samples from Sweden. The most abundant PFCA was perfluoroundecanoic acid (PFUA) with highest concentrations in samples from Sweden (mean 82 ng g(-1) w.w.), samples from the Faroe Islands had the second highest concentration (mean 57 ng g(-1) w.w.) and samples from Iceland and Norway had concentrations ranging between 18 and 30 ng g(-1) w.w. The original hypothesis was based on the idea that PFC concentrations are the highest close to more densely populated and industrialized areas and lower levels in remote areas. However, the geographic pattern is more complicated than predicted and varies among different PFCs.

    • For more than a decade, extensive government-funded school improvement programs have aimed at strengthening quality and equality in Swedish schools. Since 2015, a specific focus has been on the group of newly arrived migrant students in terms of in-service training for teachers. Research about such initiatives is, however, lacking, particularly regarding how these efforts help teachers to better attend to newly arrived students’ language and content learning in various school subjects. The overall aim of the present thesis is to contribute new and in-depth knowledge about subject teachers’ possibilities for developing a language and content integrated approach in their subject teaching for newly arrived migrant students at secondary school level (students aged 13–16). The study was conducted in a school in tandem with an in-service training on language and content integrated teaching and learning from a second language and multilingual perspective. Using a linguistic ethnographic approach, two secondary school teachers’ meta-reflections and teaching practice in Science and Mathematics are in focus. The rich material produced over two school years during and after the in-service training consists of audio-recorded interviews, the teachers’ collegial meetings during the in-service training, audio-and video-recordings from classroom observations, fieldnotes, and photos. Using an ecological and critical framework, the teachers’ perspectives are related to the material and structural context. Central to the analyses is the teachers’ situated didactic agency and how it is shaped by power relations, available resources, and circulating discourses. These analyses are supported by the interconnected model of teacher professional growth (Clarke & Hollingsworth, 2002), which is expanded via the use of didactic agency. Findings show that the school’s mainstream policy, combined with a lack of adequate resources, limits the teachers’ opportunities to fully make use of in-service affordances and, in the long term, their possibilities for practice development. The findings also make visible how intertwined and colliding discourses about teaching in general, and newly arrived students in particular, impact on the teachers’ actions and investments. This does not only affect what the teachers can do, but also what they can learn and develop further in their teaching practices. This thesis argues that what teachers do – and do not do – needs to be understood in relation to their didactic agency. Consequently, the present thesis problematizes school-improvement initiatives where professional development for teachers is constructed as a taken-for-granted solution to complex structural problems. The thesis further argues that in-service training for teachers cannot be regarded as a substitute for the lack of material resources and spaces for manoeuvre. These findings are relevant for future school-improvement initiatives as well as professional development initiatives, especially regarding marginalized student groups. In the thesis, research conducted in collaboration with professionals in schools is encouraged, where conditions and possibilities for teachers to design teaching practices that support linguistically minority students could be further explored. In such studies, teachers’ didactic agency would serve well as a central analytical concept.

    • BACKGROUND: In April 2004, the Norwegian Institute of Public Health was notified about an outbreak of a respiratory illness among participants on a cultural-historical theme tour to Central America. One patient was examined in a Swedish hospital where the clinician suspected histoplasmosis, which later was verified by serology and by culture of Histoplasma capsulatum. In order to describe the outbreak and identify the possible source of infection, a clinical and epidemiological study was conducted. MATERIAL AND METHODS: A questionnaire on symptoms, treatment and participation in various activities was sent to all the tour participants. Reminders were sent by e-mail. In addition, some participants underwent a more detailed clinical examination. RESULTS: 19 of the 21 travellers completed the questionnaire. All travellers had visited several caves inhabited by bats in El Salvador and Guatemala. During the last 10 days of the journey, 16 of the respondents (84%) acquired a respiratory illness that lasted for more than 3 weeks. The diagnosis was confirmed by serology in 8 of the 14 tested cases. Antibacterial therapy was given to 11 patients and antifungal therapy to 3 patients. Only one patient was hospitalised. INTERPRETATION: The described outbreak of histoplasmosis was probably caused by exposure to Histoplasma capsulatum during a visit in bat-infested caves. Due to the increasing popularity of adventure and theme travels, this disease may become more frequent in our population. The condition should be considered in travellers with prolonged respiratory illness after visits to caves or other closed environments with little circulation of air that are potentially contaminated with bat or bird droppings. Persons with immunosuppression may develop severe illness, and should consider avoiding places with increased risk.
