  • Risk factors for neck pain among forklift truck operators : a retrospective cohort study

    • BACKGROUND: No previous research has been performed into neck pain among forklift operators. This is a common complaint among these workers, who number around 150,000 in Sweden and six million in Europe. The aim of the study was to examine long-term exposure to unnatural neck positions among forklift operators as a risk factor for neck pain.METHODS: A retrospective cohort study was conducted of all eligible employees at a high-level warehouse. Forklift operators and office workers answered an 18-page questionnaire comprising questions about joint pain, work tasks, work postures and year of start for all items. By using person years in the exposed and less-exposed groups before start of neck pain we were able to calculate Incident Rate ratios for various exposures.RESULTS: Forty nine percent of the forklift operators reported having experienced neck pain compared to 30 % of office workers. Being a forklift operator was associated with an increased risk of neck pain (OR = 5.1, 95% CI 1.4-18.2). Holding the head in an unnatural position resulted in significantly increased risks for neck pain, irrespective of type of position. The risks for neck pain remained after taking other ergonomic exposures and psychosocial aspects into consideration.CONCLUSIONS: This is the first published study showing that forklift operators have an increased risk of neck pain. The results are therefore of significance for improving work schedules, the adjustment of work tasks for these workers and the design of the vehicles.

    • The aim of this study was to evaluate the growth of infants born small for gestational age (SGA) as a result of 2 different feeding regimens during their hospital stay. A retrospective chart review was performed at 2 hospitals to assess the growth of 42 SGA infants (gestational age: median 37 weeks; range, 30-41 weeks) from birth up to 18 months corrected age. At one hospital, infants were fed according to a proactive nutrition regimen stipulating 200 mL milk/kg per day from day 2 to achieve better weight gain. At the other hospital, milk volumes were gradually increased to 170 mL/kg per day by day 9. Infants fed according to the proactive regimen had lower weight loss and regained their birth weight earlier but did not show better catch-up growth subsequently. The premature SGA infants (n = 20) showed catch-up growth before 40 weeks postmenstrual age. The lower the gestational age at birth, the less negative standard deviation score for length up to a corrected age of 18 months. Although infants fed according to a proactive regimen with liberal volumes of milk during the first days had lower weight loss and regained their birth weight earlier, no evidence was found that this would result in a different pattern of growth in later life.

    • To establish breastfeeding in preterm infants, test weighing before and after breastfeeding and gradual reduction of supplementation are both applicable regimens. Mothers can be encouraged to choose either of them, although test weighing may help infants attain exclusive breastfeeding at an earlier PMA.

    • The aim of these studies was to increase knowledge about hospital feeding routines in high-risk neonates. A retrospective medical chart review procedure was used to study routines at the neonatal units of two Swedish hospitals. In Papers I and II, the sample (Uppsala n=21 and Umeå n=21) comprised of small for gestational age (SGA) infants, in Papers III (Uppsala n=64 and Umeå n=59) and IV (n=127), the samples comprised of appropriate for gestational age (AGA) infants. Paper I indicated large enteral/oral milk volumes rendered i.v. administration of glucose unnecessary, reduced weight loss and helped SGA infants regain birth weight earlier. More rapid postnatal growth did not remain up to 18 months with corrected age in any growth variable (Paper II). In Paper III, effects were compared whether the infants’ volume of breast milk intake in hospital was estimated by “clinical indices” or determined by test-weighing. Infants treated in hospitals where test-weighing was practised attained exclusive breastfeeding at an earlier postmenstrual age (PMA), and they were discharged at an earlier PMA. However, the two study units were similar regarding the proportion of infants attaining exclusive breastfeeding. Paper IV revealed preterm AGA infants with higher standard deviation scores (SDS) at birth had more negative changes from birth to discharge for all growth variables. Conclusions: Papers I and II indicated that early initiation of enteral/oral feeding with proactive increases in milk volume was beneficial short term. No evidence was found for a proactive nutrition regimen with initial large volumes of milk resulting in a different pattern of growth up to the corrected age of 18 months. Test-weighing before and after breastfeeding might help infants to attain exclusive breastfeeding at an earlier PMA (study III). Finally, preterm AGA infants with higher SDS at birth are at higher risk of inadequate growth during their hospital stay (study IV).

    • Aim: The aim was to evaluate growth and breastfeeding up to 18 months corrected age (CA) among preterm appropriate for gestational age (AGA) infants whose mothers initiated breastfeeding during the infants' hospital stay. Methods: One hundred and twenty-seven preterm AGA infants with a median birth weight of 2320 (769-3250) g and gestational age 34.29 (25.00-35.86) weeks were evaluated up to a CA of 18 months. A retrospective, descriptive and comparative design was used. Data were obtained by chart review of hospital medical records and a questionnaire completed by the mothers. Results: The changes in standard deviation scores (SDS) during the infants' hospital stay were -0.9 for weight, -0.3 for length and -0.5 for head circumference (HC). Infants with higher SDS at birth showed more negative changes from birth to discharge. Median increments in SDS from discharge to a CA of 2 months were as high as, or higher than, the loss from birth to discharge. Conclusion: Preterm AGA infants with higher SDS for weight, length and HC at birth are at higher risk of inadequate growth during their hospital stay.

    • I Livsmedelsverkets beredskapsarbete ingår att göra uppskattningar av vilka nivåer av farliga ämnen som konsumenterna får i sig via livsmedel och att tidigt upptäcka exponering som kan resultera i skadliga hälsoeffekter. Detta arbete innebär att övervakningen behöver förbättras gällande risker för kemisk exponering. I händelse av kemisk exponering av befolkningen via livsmedel eller vatten, beroende på olyckor eller antagonistiska händelser, behövs en väl samordnad kedja inkluderande bl.a. operativa aktörer samt centrala och regionala myndigheter.Denna rapport är ett resultat av krisberedskapsprojektet Nätverk vid upptäckt av befolkningsexponering för farliga kemiska ämnen som finansierats med 2:4-anslag från MSB. Projektets syfte är att bilda ett nätverk mellan olika aktörer för att skapa förutsättningar för ett bättre utnyttjande av landets samlade kapacitet och kompetens avseende provtagnings- och laboratorieanalys i avsikt att identifiera farliga kemiska ämnen i prover från människa. Nätverket är till för att skapa en effektivare beredskap vid allvarliga händelser med kemiska ämnen och/eller för övervakning av exponering för kemiska ämnen hos den svenska befolkningen.Projektet fokuserade på att inventera och förbättra kunskapsläget och laboratorieberedskap för provtagning och analys av prover från människa vid upptäckt av befolkningsexponering för farliga ämnen. Detta gäller i livsmedel och dricksvatten då ämnen av kemiskt ursprung kan vara en potentiell farlig källa. Projektet skapade ett nätverk mellan myndigheter och regionala Arbets- och miljömedicinska (AMM) enheter för att skapa förutsättningar för ett bättre utnyttjande av landets samlade kapacitet, kompetens och laboratorier inom AMM-verksamheten. För att stärka såväl regional som nationell krisberedskap när det gäller hanteringen av oönskade händelser så har projektets alla AMM-enheter i Sverige kartlagt kompetens och laboratoriekapacitet samt testat förmågan att arbeta i fält med provtagning och efterhantering av prover.Målgruppen för rapporten är de som arbetar med identifiering och riskvärdering av farliga ämnen med kemiskt ursprung i befolkningen, både vid kris och i vardagen. Nätverkets arbete resulterade bl.a. i de manualer som används vid exponering för olika typer av CBRNE-ämnen dvs. inte enbart kemiska (C-ämnen), men också biologiska (B-ämnen), radiologiska (R-ämnen), nukleära (N-ämnen) och explosiva (E-ämnen). Projektet har drivits av Livsmedelsverket i ett nära samarbete med regionala AMM-enheter och Totalförsvarets Forskningsinstitut (FOI) samt i samverkan med Folkhälsomyndigheten, Naturvårdsverket, Socialstyrelsen, Kemikalieinspektionen och Karolinska Institutet.

    • The Active Ageing Index (AAI) can be applied across countries to compare active aging outcomes, evaluate the current situation regarding ageing, and identify the most desired directions for policy action. While the AAI-EU is addressed on a national level, it can also be used for assessing active ageing policies on the municipal level, which is the case in this study. The aim of this article was to assess active ageing amongst older people in western parts of Finland by adapting the methodology used for the AAI-EU. We used GERDA data (n=5,011) collected from people aged 66, 71, 76, 81 and 86 for our descriptive analyses. The results showed that the municipalities differed in their active ageing outcomes, and that men and Swedish speakers generally scored higher on the index, which is something that warrants further investigation. Based on the results, we argue that AAI could potentially be used as a tool to monitor various dimensions of active ageing on a regional level and that gender and ethno-linguistic characteristics should be a central concern for a discussion on ageing in our study region.

    • The study examined changes in active ageing among 65/66- and 75/76-year-olds in northern Sweden and western Finland. Data were retrieved from a repeated cross-sectional survey conducted in 2005 and 2016. Logistic regression was used to estimate the probability of ageing actively in terms of employment, social and political participation, confidence in healthcare, economic situation, self-rated health and social contacts. The results showed no significant changes in employment, social participation or economic situation. However, the level of political participation and self-rated good health was significantly higher in 2016 compared with 2005, whereas confidence in healthcare and social contacts was lower. Considering that the implementation of active ageing policies relies heavily on municipal and/or regional level in our study region, our results could be used as a framework for regional policymaking.

    • Objective: The aim of this study was to examine mortality causes and cancer incidence in a population cohort that have resided in close proximity to highly metal-contaminated sources, characterized by contamination of, in particular, arsenic (As), cadmium (Cd), and lead (Pb). Methods: Data from Swedish registers were used to calculate standardized mortality and cancer incidence ratios. An attempt to relate cancer incidence to metal contamination levels was made. Results: Significantly elevated cancer incidences were observed for overall malignant cancers in both genders, cancer in the digestive system, including colon, rectum, and pancreas, and cancers in prostate among men. Dose-response relationships between Cd and Pb levels in soil and cancer risks were found. Conclusions: Cancer observations made, together with previous studies of metal uptake in local vegetables, may imply that exposure to local residents have occurred primarily via oral intake of locally produced foodstuffs.

    • We consider an indoor tracking system consisting of an inertial measurement unit (IMU) and a camera that detects markers in the environment. There are many camera based tracking systems described in literature and available commercially, and a few of them also has support from IMU. These are based on the best-effort principle, where the performance varies depending on the situation. In contrast to this, we start with a specification of the system performance, and the design isbased on an information theoretic approach, where specific user scenarios are defined. Precise models for the camera and IMU are derived for a fusion filter, and the theoretical Cramér-Rao lower bound and the Kalman filter performance are evaluated. In this study, we focus on examining the camera quality versus the marker density needed to get at least a one mm and one degree accuracy in tracking performance.

    • Radar and sonar provide information of both range and radial velocity to unknown objects. This is accomplished by emitting a signal waveform and computing the round trip time and Doppler shift. Estimation of the round trip time and Doppler shift is usually done separately without considering the couplings between these two object related quantities. The purpose of this contribution is to first model the amplitude, time shift and time scale of the returned signal in terms of the object related states range and velocity, and analyse the Cramér-Rao lower bound (CRLB) for the joint range and velocity estimation problem. One of the conclusions is that there is negative correlation between range and velocity. The maximum likelihood (ML) cost function also confirms this strong negative correlation. For target tracking applications, the use of the correct covariance matrix for the measurement vector gives a significant gain in information, compared to using the variance of range and velocity assuming independence. In other words, the knowledge of the correlation tells the tracking filter that a too large range measurement comes most likely with a too small velocity measurement, and vice versa. Experiments with sound pulses reflected in a wall indoors confirm the main conclusion of negative correlation.

    • This paper presents a method for global pose estimation using inertial sensors, monocular vision, and ultra wide band (UWB) sensors. It is demonstrated that the complementary characteristics of these sensors can be exploited to provide improved global pose estimates, without requiring the introduction of any visible infrastructure, such as fiducial markers. Instead, natural landmarks are jointly estimated with the pose of the platform using a simultaneous localization and mapping framework, supported by a small number of easy-to-hide UWB beacons with known positions. The method is evaluated with data from a controlled indoor experiment with high precision ground truth. The results show the benefit of the suggested sensor combination and suggest directions for further work.

    • The number of patients with multiple primary malignancies has been increasing steadily in recent years. In the present study, we describe a unique case of an 81-year-old woman with 5 metachronous and synchronous primary malignant neoplasms. The patient was first diagnosed with an endometrium adenocarcinoma in 1997 and a colon adenocarcinoma in 2002. Eleven years after her colon surgery, in 2013, the patient presented with 3 other primary malignancies within a 4-month time span: An invasive malignant melanoma on the lower leg, an invasive mucinous breast carcinoma in the right breast, and a pleomorphic spindle cell sarcoma on the left upper arm. Subsequent routine medical checkups in 2013-2017 revealed no metastases of the primary malignancies. The patient mentioned a familial aggregation of malignant tumors, including 2 sisters with breast cancer and a brother with lung cancer. Interestingly, next-generation sequencing analysis of the patient's blood sample detected no mutations in the BRCA1, BRCA2, TP53, PTEN, CDH1, PALB2, RAD51C, RAD51D, MLH1, MSH2, MSH6, PMS2, EPCAM, APC, MUTYH, STK11, BMPR1A, SMAD4, PTEN, POLE, POLD1, GREM1, and GALNT12 genes. Therefore, whole genome sequencing is warranted to identify cancer-related genetic alterations in this patient with quintuple primary malignancies.

    • BackgroundLong‐term cognitive problems are common among elderly patients after surgery, and it has been suggested that inhalation anaesthetics play a role in the development of dementia. This study aims to investigate the hypothesis that patients with dementia have been more exposed to surgery and inhalational anaesthetics than individuals without dementia.MethodsUsing 457 cases from a dementia‐registry and 420 dementia‐free controls, we performed a retrospective case‐control study. The medical records were reviewed to determine exposure to anaesthesia occurring within a 20‐year timeframe before the diagnosis or inclusion in the study. Data were analysed using multivariate logistic regression and propensity score analysis.ResultsAdvanced age (70 years and older, with the highest risk in ages 80‐84 years) and previous head trauma were risk factors for dementia. History of exposure to surgery with anaesthesia was a risk factor for dementia (OR = 2.23, 95% CI 1.66‐3.00, P < 0.01). Exposure to inhalational anaesthetics with halogenated anaesthetics was associated with an increased risk of dementia, compared to no exposure to anaesthesia (OR = 2.47, 95% CI 1.17‐5.22, P = 0.02). Exposure to regional anaesthesia was not significantly associated with increased risk of dementia (P = 0.13).ConclusionIn this 20‐year retrospective case‐control study, we found a potential association between dementia and prior anaesthesia. Exposure to general anaesthetics with halogenated anaesthetic gases was associated with an increased risk of dementia.
