  • Incidence of hyperthyroidism in Sweden

    • Introduction: The incidence of hyperthyroidism has been reported in various countries to be 23-93/100000 inhabitants per year. This extended study has evaluated the incidence for similar to 40% of the Swedish population of 9 million inhabitants. Sweden is considered to be iodine sufficient country. Methods:All patients including children, who were newly diagnosed with overt hyperthyroidism in the years 2003-2005, were prospectively registered in a multicenter study. The inclusion criteria are as follows:clinical symptoms and/or signs of hyperthyroidism with plasma TSH concentration below 0.2 mIE/l and increased plasma levels of free/total triiodothyronine and/or free/total thyroxine. Patients with relapse of hyperthyroidism or thyroiditis were not included. The diagnosis of Graves' disease (GD), toxic multinodular goiter (TMNG) and solitary toxic adenoma (STA), smoking, initial treatment, occurrence of thyroid-associated eye symptoms/signs, and demographic data were registered. Results:A total of 2916 patients were diagnosed with de novo hyperthyroidism showing the total incidence of 27.6/100 000 inhabitants per year. The incidence of GD was 21.0/100 000 and toxic nodular goiter (TNG=STA+TMNG) occurred in 692 patients, corresponding to an annual incidence of 6.5/100 000. The incidence was higher in women compared with men (4.2:1). Seventy-five percent of the patients were diagnosed with GD, in whom thyroid-associated eye symptoms/signs occurred during diagnosis in every fifth patient. Geographical differences were observed. Conclusion:The incidence of hyperthyroidism in Sweden is in a lower range compared with international reports. Seventy-five percent of patients with hyperthyroidism had GD and 20% of them had thyroid-associated eye symptoms/signs during diagnosis. The observed geographical differences require further studies.

    • Dendrimers up to the fourth generation (G1-G4) were successfully synthesized via the efficient copper catalyzed 1,3-dipolar cycloaddition between primary alkynes and azides (CuAAC), also referred to as a click reaction. The synthetic protocol involved the preparation of presynthesized dendron wedges that subsequently were attached to a tetra-valent alkyne functional cyclen core. These constructed structures integrated stable triazole groups "intra-locked'' between the cyclen and dendron wedges. The incorporation of a lanthanide metal ion, europium, into the interior of all cyclen dendrimers was monitored by FT-IR. Interestingly, the photophysical results showed that the proximate triazole not only acts as a stable linker but also as a sensitizers, transferring its singlet-singlet excitation in the ultraviolet region (270-290 nm) to the partially filled luminescent lanthanide 4f shell. An increase of luminescence decay time from the lanthanide D-5(0) -> F-7(2) emission was observed with increasing dendrimer size, indicating that the shielding effect of the dendron wedges is important for the relaxation of the photo-excitation and energy transfer. To the best of our knowledge, this is the first time a set of dendron wedges have successfully been attached to a cyclen metal ion cage via the versatile click reaction. Furthermore, the produced triazoles intra-locked in close proximity to the macrocycle core elucidated an interesting photophysical function.

The technique allows components tobenefit from the advantages of a real-time database managementsystem while still retaining desirable componentproperties, such as isolation and a high level of reusability.We propose that a database is integrated in the componentframework, and introduce the concept of databaseproxies to decouple components from the database schema.The resulting system fully benefits from the advantages ofcomponent-based software development, such as reusability,all component interaction through interfaces, etc, combinedwith the advantages of a real-time database managementsystem, i.e., system openness, controlled data access,and dynamic query language capabilities.

  • Fracture Incidence in Graves' Disease: A Population-Based Study.

    • Background: Population-based studies have indicated an increase in bone turnover in hyperthyroidism with a subsequent decrease in bone mineral density and an increased risk of fractures, especially in postmenopausal women. However, heterogeneity between studies prevents a definitive conclusion. Graves' disease (GD) is an autoimmune disease, and it is the most common cause of hyperthyroidism. The aim of this study was to investigate fracture risk in patients with GD. Methods: A total of 2134 patients with incident GD and 21,261 age, sex- and county-matched controls were included 16-18 years after diagnosis in a retrospective cohort study. Drug and patient national registries in Sweden were used to assess the risk of developing skeletal complications. Up to 10 years of age, sex- and county-matched controls per patient were selected from databases from the National Board of Health and Welfare and Statistics Sweden. Cox proportional hazards models were fitted to estimate hazard ratios (HR) and confidence intervals [CI]. Results: There were no significant differences in fracture rates between GD and controls but after adjustment for comorbidities, the data showed higher vertebral fracture rates in male GD patients aged >52 years compared to male controls, HR=2.83 [CI 1.05-7.64]. The rates of osteoporosis treatments as well as treatment with corticosteroids were higher in patients with GD. However, HR for the association between GD and fractures remained largely unchanged after adjustment for osteoporosis treatments and treatments with corticosteroids. Conclusions: There were no significant differences in total fracture rate between GD and the general population. However, men older than 52 years had a higher vertebral fracture rate. This study also shows that patients with treated GD receive more osteoporosis treatments compared to the general population.

    • Web corpora are a cornerstone of modern Language Technology. Corpora built from the web are convenient because their creation is fast and inexpensive. Several studies have been carried out to assess the representativeness of general-purpose web corpora by comparing them to traditional corpora. Less attention has been paid to assess the representativeness of specialized or domain-specific web corpora. In this paper, we focus on the assessment of domain representativeness of web corpora and we claim that it is possible to assess the degree of domain-specificity, or domainhood, of web corpora. We present a case study where we explore the effectiveness of different measures - namely the Mann-Withney-Wilcoxon Test, Kendall correlation coefficient, Kullback–Leibler divergence, log-likelihood and burstiness - to gauge domainhood. Our findings indicate that burstiness is the most suitable measure to single out domain-specific words from a specialized corpus and to allow for the quantification of domainhood.

    • Förvaltningsrapporten fokuserar på Snomed CT, eftersom det redan i dag finns rutiner för förvaltningar av termbanken och nationella hälsorelaterade klassifikationer. Ett särskilt utvecklingsarbete kommer att krävas för dessa delar.Rapporten tar upp syfte och mål med förvaltningen. Utöver det redogör rapporten för vilka konkreta ansvarsområden som ingår i förvaltningen av Snomed CT. Målet för förvaltningen är att Socialstyrelsen regelbundet ska kunna tillhandahålla en kontrollerad och uppdaterad release av Snomed CT. Det skulle möjliggöra användning inom vård och omsorg. Rapporten tar även upp behovet av kompetens, utbildning och finansiella resurser.

We will also talk about E-carehome project aiming at providing an IoT-based health-care system for elderly people at their homes. Furthermore, we refer to the role of SmartEnv in Ecarehome and how it needs to be further extended to achieve semantic interoperability as one of the challenges in development of autonomous health care systems at home.

  • Enhancing protein disaggregation restores proteasome activity in aged cells

    • The activity of the ubiquitin-proteasome system, UPS, declines during aging in several multicellular organisms. The reason behind this decline remains elusive. Here, using yeast as a model system, we show that while the level and potential capacity of the 26S proteasome is maintained in replicatively aged cells, the UPS is not functioning properly in vivo. As a consequence cytosolic UPS substrates, such as Delta ssCPY* are stabilized, accumulate, and form inclusions. By integrating a pGPD-HSP104 recombinant gene into the genome, we were able to constitutively elevate protein disaggregase activity, which diminished the accumulation of protein inclusions during aging. Remarkably, this elevated disaggregation restored degradation of a 26S proteasome substrate in aged cells without elevating proteasome levels, demonstrating that age-associated aggregation obstructs UPS function. The data supports the existence of a negative feedback loop that accelerates aging by exacerbating proteostatic decline once misfolded and aggregation-prone proteins reach a critical level.

    • SammanfattningEtt tillgängligt och använt nationellt fackspråk ska bidra till en god och säker vård och omsorg. Det ska även medverka till att kvaliteten och resultaten på området ska kunna följas upp och jämföras på ett mer effektivt sätt. Slutrapporten presenterar resultatet av projektet Nationellt fackspråk för vård och omsorg samt förslag till förvaltning och utveckling.ResultatetResultatet innefattar bland annat att det internationella begreppssystemet Snomed CT är översatt till svenska och att det är förberett för förvaltning och distribution. Socialstyrelsen har även tagit fram och testat metoder för förvaltning och utveckling av det nationella fackspråket i sin helhet. Därtill har representanter för målgrupperna informerats och fått kunskap.Rapporten innehåller en utförlig beskrivning av det nationella fackspråkets sammantagna innehåll: Socialstyrelsens termbank, klassifikationer och kodverk, den svenska versionen av Snomed CT, metoder för utveckling och förvaltning samt regler för användning.Förvaltning, införande och resursbehovI rapporten finns förslag till hur hela det nationella fackspråket kan tas omhand av Socialstyrelsen och hur det kan införas i vården och omsorgen. Projektets övergång till en långsiktigt hållbar organisation kräver resurser. Därför redogör rapporten för det förväntade resursbehovet för förvaltning och utveckling. Bland annat föreslås en treårig utbildningsinsats samt stimulansbidrag för införande.Krav på styrning, samordning och förtydligat ansvarRapporten betonar behovet av en samlad och medveten styrning av utvecklingen inom området. Socialstyrelsen vill ha en samordnande roll i utvecklingen och förvaltningen av det nationella fackspråket.     • OBJECTIVE:To facilitate the interchange of lexical information for multiple languages in the medical domain. To pave the way for the emergence of a generally available truly multilingual electronic dictionary in the medical domain.METHODS:An interchange format has to be neutral relative to the target languages. It has to be consistent with current needs of lexicon authors, present and future. An active interaction between six potential authors aimed to determine a common denominator striking the right balance between richness of content and ease of use for lexicon providers.RESULTS:A simple list of relevant attributes has been established and published. The format has the potential for collecting relevant parts of a future multilingual dictionary. An XML version is available.CONCLUSION:This effort makes feasible the exchange of lexical information between research groups. Interchange files are made available in a public repository. This procedure opens the door to a true multilingual dictionary, in the awareness that the exchange of lexical information is (only) a necessary first step, before structuring the corresponding entries in different languages.

    • Control over the photophysical properties and molecular organization of pi-conjugated oligothiophenes is essential to their use in organic electronics. Herein we synthesized and characterized a variety of anionic pentameric oligothiophenes with different substitution patterns of L- or D-tyrosine at distinct positions along the thiophene backbone. Spectroscopic, microscopic, and theoretical studies of L- or D-tyrosine substituted pentameric oligothiophene conjugates revealed the formation of optically active pi-stacked self-assembled aggregates under acid conditions. The distinct photophysical characteristics, as well as the supramolecular structures of the assemblies, were highly influenced by the positioning of the L- or D-tyrosine moieties along the thiophene backbone. Overall, the obtained results clearly demonstrate how fundamental changes in the position of the enantiomeric side-chain functionalities greatly affect the optical properties as well as the architecture of the self-assembled supramolecular structures.

    • Altered mitochondrial functionality can extend organism life span, but the underlying mechanisms are obscure. Here we report that inactivating SOV1, a member of the yeast mitochondrial translation control (MTC) module, causes a robust Sir2-dependent extension of replicative life span in the absence of respiration and without affecting oxidative damage. We found that SOV1 interacts genetically with the cAMP-PKA pathway and the chromatin remodeling apparatus. Consistently, Sov1p-deficient cells displayed reduced cAMP-PKA signaling and an elevated, Sir2p-dependent, genomic silencing. Both increased silencing and life span extension in sov1Δ cells require the PKA/Msn2/4p target Pnc1p, which scavenges nicotinamide, a Sir2p inhibitor. Inactivating other members of the MTC module also resulted in Sir2p-dependent life span extension. The data demonstrate that the nuclear silencing apparatus senses and responds to the absence of MTC proteins and that this response converges with a pathway for life span extension elicited by reducing TOR signaling.

    • The calcium-responsive phosphatase, calcineurin, senses changes in Ca2+ concentrations in a calmodulin-dependent manner. Here we report that under non-stress conditions, inactivation of calcineurin signaling or deleting the calcineurin-dependent transcription factor CRZ1 triggered the formation of chaperone Hsp100p (Hsp104p)-associated protein aggregates in Saccharomyces cerevisiae. Furthermore, calcineurin inactivation aggravated α-Synuclein-related cytotoxicity. Conversely, elevated production of the calcineurin activator, Cnb1p, suppressed protein aggregation and cytotoxicity associated with the familial Parkinson’s disease-related mutant α-Synuclein A53T in a partly CRZ1-dependent manner. Activation of calcineurin boosted normal localization of both wild type and mutant α-synuclein to the plasma membrane, an intervention previously shown to mitigate α-synuclein toxicity in Parkinson’s disease models. The findings demonstrate that calcineurin signaling, and Ca2+ influx to the vacuole, limit protein quality control in non-stressed cells and may have implications for elucidating to which extent aberrant calcineurin signaling contributes to the progression of Parkinson’s disease(s) and other synucleinopathies.     • With the introduction of EHR two-level modelling and archetype methodologies pioneered by openEHR and standardized by CEN/ISO, we are one step closer to semantic interoperability and future-proof adaptive healthcare information systems. Along with the opportunities, there are also challenges. Archetypes provide the full semantics of EHR data explicitly to surrounding systems in a platform-independent way, yet it is up to the receiving system to interpret the semantics and process the data accordingly. In this paper we propose a design of an archetype-based platform-independent testing framework for validating implementations of the openEHR archetype formalism as a means of improving quality and interoperability of EHRs.

    • The possibility of post-coordination of SNOMED CT concepts, especially by clinical users, is both an asset and a challenge for SNOMED CT implementation. To get insight in the applicability of post-coordination, we analyzed scenarios for user-directed coordination that are described in the documentation of SNOMED CT. The analyses were based on experiences from previous and ongoing research and implementation work, including national mapping projects, and investigations on collection of data for multiple uses. These scenarios show various usability and representation problems: high number of relationships for refinement and qualification, improper options for refinement, incorrect formal definitions, and lack of support for applying editorial rules. Improved user-directed coordination in SNOMED CT in real practice requires advanced sanctioning, increased consistency of definitions of concepts in SNOMED CT, and real-time analysis of the post-coordinate expression.

    • A spectral correlation algorithm for the analysis of hyperspectral fluorescence images is proposed by Ellingsen et al. [J. Biomed. Opt. 18, 020501 (2013)]. Here, it is applied to the analysis of double-stained A beta amyloid plaques being related to the Alzheimers disease (AD). Sections of APP/PS1 AD mice model brains are double stained with luminescent-conjugated oligothiophenes, known to bind to amyloid protein deposits. Hyperspectral fluorescence images of the brain sections are recorded and by applying the correlation algorithm the spectral inhomogeneity of the double-stained samples is mapped in terms of radial distribution and spectral content. To further investigate the progression of A beta amyloid plaque formation, 19 AD mice of different ages up to 23 months are characterized, enabling a statistical analysis of the plaque heterogeneity. In accordance with recent findings by Nystrom et al. [ACS Chem. Biol. 8, 1128-1133 (2013)], the spectral distribution within A beta plaques is found to vary with age throughout the lifespan of the mouse. With the new correlation algorithm, it is possible to quantify the spectral abundance of the two stains depending on the relative distance from the plaque center and mouse age. Thus, we demonstrate the use of the correlation analysis approach in double-staining experiments and how it is possible to relate these to structural/spectral changes in biological samples.     • PURPOSE: The aim of this study was the clinical evaluation of an independent dose and monitor unit verification (MUV) software which is based on sophisticated semi-analytical modelling. The software was developed within the framework of an ESTRO project. Finally, consistent handling of dose calculation deviations applying individual action levels is discussed. MATERIALS AND METHODS: A Matlab-based software ("MUV") was distributed to five well-established treatment centres in Europe (Vienna, Graz, Basel, Copenhagen, and Umeå) and evaluated as a quality assurance (QA) tool in clinical routine. Results were acquired for 226 individual treatment plans including a total of 815 radiation fields. About 150 beam verification measurements were performed for a portion of the individual treatment plans, mainly with time variable fluence patterns. The deviations between dose calculations performed with a treatment planning system (TPS) and the MUV software were scored with respect to treatment area, treatment technique, geometrical depth, radiological depth, etc. RESULTS: In general good agreement was found between calculations performed with the different TPSs and MUV, with a mean deviation per field of 0.2+/-3.5% (1 SD) and mean deviations of 0.2+/-2.2% for composite treatment plans. For pelvic treatments less than 10% of all fields showed deviations larger than 3%. In general, when using the radiological depth for verification calculations the results and the spread in the results improved significantly, especially for head-and-neck and for thorax treatments. For IMRT head-and-neck beams, mean deviations between MUV and the local TPS were -1.0+/-7.3% for dynamic, and -1.3+/-3.2% for step-and-shoot IMRT delivery. For dynamic IMRT beams in the pelvis good agreement was obtained between MUV and the local TPS (mean: -1.6+/-1.5%). Treatment site and treatment technique dependent action levels between +/-3% and +/-5% seem to be clinically realistic if a radiological depth correction is performed, even for dynamic wedges and IMRT. CONCLUSION: The software MUV is well suited for patient specific treatment plan QA applications and can handle all currently available treatment techniques that can be applied with standard linear accelerators. The highly sophisticated dose calculation model implemented in MUV allows investigation of systematic TPS deviations by performing calculations in homogeneous conditions

    • The aim of this study is to provide a survey of systematic reviews that have evaluated the effects of psychosocial interventions for adult people with intellectual disabilities and/or an autistic syndrome with concurrent mental health problems. Reviews for inclusion were identified through searches of 10 electronic databases. The authors found that 3 out of 126 published reviews met the inclusion criteria for interventions, population, and being considered a systematic review. The results imply a weak scientific support for behavioral therapy, cognitive-behavioral therapy, and some forms of integrated care and support. However, the primary studies included in the reviews have several methodological shortcomings. The results suggest future research initiatives in the direction of more effectiveness studies of good quality and reproduction of high-quality systematic reviews.

  • Centralized Routing for Prolonged Network Lifetime in Wireless Sensor Networks

    • In this thesis centralized routing methods for wireless sensor networks have been studied. The aim has been to prolong network lifetime by reducing the energy consumed by sensor-node communication.Wireless sensor networks are rapidly becoming common in application areas where information from many sensors is to be collected and acted upon. The use of wireless sensor networks adds flexibility to the network, and the cost of cabling can be avoided.Wireless sensor networks may consist of several hundreds of small devices, equipped with sensors (e.g. acoustic, seismic or image) that form a wireless network. Each sensor node collects information from its surroundings and sends it to a base station, either from sensor node to sensor node, or directly to the base station.We have made simulations that show that asymmetric communication with multihop extends the lifetime of large wireless sensor networks. We have also investigated the usefulness of enforcing a minimum separation distance between cluster heads in a cluster based wireless sensor network. The results show that our sensor network performs up to 150% better when introducing a minimum separation distance between cluster heads. The simulations also show that the minimum separation distance resulting in the lowest energy consumption in our network varies with the number of clusters. We have also made an initial study of maximum lifetime routing in sparse wireless sensor networks to be able to see how different heuristic routing algorithms influence the energy consumption of individual sensor nodes, and thus the lifetime of a sparse sensor network. We have also compared the maximum lifetime of the heuristic algorithms to the maximum lifetime of an optimal routing solution. These simulations showed that for some types of applications the choice of heuristic algorithm is more important to prolong network lifetime, than for other types of applications.

    • Background: Childhood pre-B acute lymphoblastic leukemia (ALL) is a bone marrow (BM) derived disease, which often disseminates out of the BM cavity, where malignant cells to a variable degree can be found circulating in the peripheral blood (PB). Normal pre-B cells are absolutely dependent on BM stroma for survival and differentiation. It is not known whether transformed pre-B ALL cells retain any of this dependence, which possibly could impact on drug sensitivity or MRD measurements. Results: Pre-B ALL cells, highly purified by a novel method using surface expression of CD19 and immunoglobulin light chains, from BM and PB show a very high degree of similarity in gene expression patterns, with differential expression of vascular endothelial growth factor (VEGF) as a notable exception. In addition, the cell sorting procedure revealed that in 2 out of five investigated patients, a significant fraction of the malignant cells had matured beyond the pre-B cell stage. Conclusion: The transition of ALL cells from the BM into the circulation does not demand, or result in, major changes of gene expression pattern. This might indicate an independence of BM stroma on the part of transformed pre-B cells, which contrasts with that of their normal counterparts. © 2008 Hansson et al, licensee BioMed Central Ltd.

    • Caloric restriction (CR) extends the lifespan of flies, worms, and yeast by counteracting age-related oxidation of H2O2-scavenging peroxiredoxins (Prxs). Here, we show that increased dosage of the major cytosolic Prx in yeast, Tsa1, extends lifespan in an Hsp70 chaperone-dependent and CR-independent manner without increasing H2O2 scavenging or genome stability. We found that Tsa1 and Hsp70 physically interact and that hyperoxidation of Tsa1 by H2O2 is required for the recruitment of the Hsp70 chaperones and the Hsp104 disaggregase to misfolded and aggregated proteins during aging, but not heat stress. Tsa1 counteracted the accumulation of ubiquitinated aggregates during aging and the reduction of hyperoxidized Tsa1 by sulfiredoxin facilitated clearance of H2O2-generated aggregates. The data reveal a conceptually new role for H2O2 signaling in proteostasis and lifespan control and shed new light on the selective benefits endowed to eukaryotic peroxiredoxins by their reversible hyperoxidation.

    • We introduce the concept of database proxies intended to mitigate the gap between two disjoint productivity-enhancing techniques: Component Based Software Engineering (CBSE) and Real-Time Database Management Systems (RTDBMS). The two techniques promote opposing design goals and their coexistence is neither obvious nor intuitive. CBSE promotes encapsulation and decoupling of component internals from the component environment, whilst an RTDBMS provide mechanisms for efficient and predictable global data sharing. A component with direct access to an RTDBMS is dependent on that specific RTDBMS and may not be useable in an alternative environment. For components to remain encapsulated and reusable, database proxies decouple components from an underlying database residing in the component framework, while providing temporally predictable access to data maintained in a database. Our approach provide access to features such as extensive data modeling tools, predictable access to hard real-time data, dynamic access to soft real-time data using standardized queries and controlled data sharing; thus allowing developers to employ the full potential of both CBSE and an RTDBMS. Our approach primarily targets embedded systems with a subset of functionality with real-time requirements. The implementation results show that the benefits of using proxies do not come at the expense of significant run-time overheads or less accurate timing predictions.

    • Efficient design-time management and documentation of run-time data elements are of paramount importance when developing and maintaining modern real-time systems. In this paper, we present the results of an industrial case-study in which we have studied the state of practice in data management and documentation. Representatives from five companies within various business segments have been interviewed and our results show that various aspects of current data management and documentation are problematic and not yet mature. Results show that companies today have a fairly good management of distributed signals, while internal ECU signals and states are, in many cases, not managed at all. This lack of internal data management results in costly development and maintenance and is often entirely dependent of the know-how of single individual experts.Furthermore, it has, in several cases, resulted in unused and excessive data in the systems due to the fact thatwhether or not a data is used is unknown.

We use stateof-the-art database pointer techniques to achieve predictabletiming, and database proxies to maintain component encapsulationand independence of data-management strategy. Thepaper illustrates the feasibility of our proposal when databaseproxies are used to manage the data communication betweencomponents and to perform run-time monitoring on the virtualfunction bus. Our implementation results show that the abovebenefits do not come at the expense of run-time overheadsor less accurate timing predictions. Under typical workloadconditions, our concept only introduces in the order of 4%CPU overhead.

  • Lessons learned from starting to implement SNOMED CT

    • National eHealth – the strategy for accessible and secure information in health and social care – is a Swedish initiative aiming at improving information management within the healthcare and social services sector. SNOMED CT is one tool in this strategy. To gain experience from SNOMED CT implementation, the National Board of Health and Welfare (host of the National Release Center of SNOMED CT) funded a number of pilot projects during 2013. Our project was one of these, focusing on facilitation of information sharing by using SNOMED CT in two application areas as described below. Furthermore, the project initiated a dialog with the EHR vendor concerning future SNOMED CT implementation.One area was transfer of discharge summaries from hospital care at Östergötland County Council to municipal home care. A discharge summary consists of a template with encoded headings, associated free text fields, and instructions about intended content. These headings were mapped to SNOMED CT.The other area was transfer of information from the EHR system at hospitals in three counties to the Swedish Stroke Register, a national quality register. The specification of information requested by the register was analyzed. Then, an EHR template was outlined, taking into account the clinical stroke process requirements, quality register requirements, and the information model of the EHR system. The draft template was based on mappings to SNOMED CT to facilitate a mutually agreed template. That means a template common for the three county councils, as well as information transfer to the register, and at the same time reduce the need for double documentation, information searching and other manual routines.Lessons learned include that mapping EHR template components and other EHR objects to SNOMED CT concepts holds potential benefits, regardless of whether one’s EHR system can handle mappings. Mapping activities may aid review and management of existing and development of new templates. Furthermore, mappings may be used as a common point of reference when information is shared. Mapping ambiguous template headings to SNOMED CT concepts was found to be time consuming and uncertain and mapping seems to be most useful when EHR contents are structured. These mapping source prerequisites imply that existing documentation practice needs to be revised and that organizations must support end users in that process. The project also concluded that mappings would be even more advantageous if EHR systems can handle mappings of different objects types. Organizations also need to allocate adequate resources for managing its own mappings as well as contributing to the development of SNOMED CT as such.Finally, the project found that training of mapping personnel benefits from integrating theoretical instruction and practical use of SNOMED CT, and that finding the correct concept in SNOMED CT calls for clinical expertise in order to be successful.

    • When we bring to mind something we have seen before, our eyes spontaneously unfold in a sequential pattern strikingly similar to that made during the original encounter, even in the absence of supporting visual input. Oculomotor movements of the eye may then serve the opposite purpose of acquiring new visual information; they may serve as self-generated cues, pointing to stored memories. Over 50 years ago Donald Hebb, the forefather of cognitive neuroscience, posited that such a sequential replay of eye movements supports our ability to mentally recreate visuospatial relations during episodic remembering. However, direct evidence for this influential claim is lacking. Here we isolate the sequential properties of spontaneous eye movements during encoding and retrieval in a pure recall memory task and capture their encoding-retrieval overlap. Critically, we show that the fidelity with which a series of consecutive eye movements from initial encoding is sequentially retained during subsequent retrieval predicts the quality of the recalled memory. Our findings provide direct evidence that such scanpaths are replayed to assemble and reconstruct spatio-temporal relations as we remember and further suggest that distinct scanpath properties differentially contribute depending on the nature of the goal-relevant memory.

    • An extensive body of research has shown that episodic remembering involves spontaneous eye movements that largely reproduce the gaze patterns that were present during encoding (e.g., Brandt & Stark, 1997; Johansson et al., 2012). Recent work has further shown that fixation locations that overlap between encoding and recall promote cortical episodic reconstruction (Bone et al., 2018; Johansson et al., 2018) and successful remembering (Johansson & Johansson, 2014). While such findings indicate that gaze location plays an active role during episodic reconstruction, the unfolding scanpaths also encompass more complex information over and above simple gaze locations, such as order, direction, shape, length and duration. Virtually nothing is known about how such spatio-temporal components contribute to episodic reconstruction. The present study investigated the encoding-retrieval overlap in scanpaths for 60 participants who encoded and recalled 36 visuospatial stimuli of two types: scenes and object arrangements. Results replicate and extend previous findings, by analyzing scanpath reinstatement over a multitude of spatio-temporal components. Critically, by combining subjective ratings of memory quality with a surprise test of forced-choice recognition, we demonstrate how such components contribute to successful remembering to different extents, and in different ways depending on the stimulus type. Results indicate that scanpath shape contributes to reconstructing the global scene structure whereas scanpath position, order and direction contribute to reconstructing the arrangement of individual objects in a spatial context. To our knowledge, this is the first systematic demonstration of how eye movement reinstatements contribute to episodic remembering in a multifaceted way.

    • Introduction: The aim of this work is to develop and test a system for storing and querying pre- and post-coordinated SNOMED CT expressions in an Electronic Health Record (EHR) system. SNOMED CT is here used as a reference terminology and to allow abstraction of EHR data for transfer to quality databases.Background: Enumerating all combinations of SNOMED CT concepts is not possible due to combinatorial explosion. E.g. pain may be qualified by severity (7 severities), pain character (152), body site (25888 sites), course (31) etc. giving a total of some 800 million possible, although not always sensible, different ways to express pain.For SNOMED CT to be useful, terminology services need to be integrated with the other parts of the information infrastructure, e.g. EHRs. As the boundary between terminology and information model representation may vary among use cases, care should be taken in implementing such integrations.Results: A system is developed to support the management of post-coordinated SNOMED CT expressions. This expression repository allows subsumption testing of pre- and post-coordinated expressions using a full view, i.e. historical views are possible. A transitive closure of the Is a-relation is updated continuously with new post-coordinated expressions stored in the repository. The repository is also integrated with an openEHR-based EHR system including archetype querying possibilities. The integrated system allows querying of terminology-bound archetype instances using pre- and post-coordinated SNOMED CT expressions.
