    • A polymorphism consisting of a deletion near the 5' splice site of exon 18 on the alpha2-macroglobulin (A2M) gene (A2M-2) has been suggested to be associated with Alzheimer's disease (AD) in family-based studies. We studied the A2M-2 allele together with the ApoE alleles in a large series on patients with AD (n = 449) and age-matched controls (n = 349). Neuropathologically confirmed diagnoses were available in 199 cases (94 AD and 107 control cases). We found no increase in A2M-2 genotype or allele frequencies in AD (27.5% and 14.6%) versus controls (26.4% and 14.9%). In contrast, a marked increase (p < 0.0001) in ApoE epsilon4 genotype or allele frequencies was found in AD (66.6% and 41.2%) as compared with controls (29.8% and 16.5%), suggesting sufficient statistical power in our sample. No relation was found between the A2M-2 and the ApoE epsilon4 allele. No change in A2M exon 17-18 mRNA size or sequence or A2M protein size was found in cases carrying the A2M-2 deletion, suggesting that there is no biological consequences of the A2M intronic deletion. No change in A2M protein level in cerebrospinal fluid was found in AD, suggesting that the A2M-2 allele does not effect the A2M protein expression in the brain. The lack of an association between the A2M-2 allele and AD in the present study, and the lack of abnormalities in the A2M mRNA or protein suggest that the A2M-2 allele is not associated with AD.

  • Antibodies to SARS-CoV-2 and risk of past or future sick leave

    • The extent that antibodies to SARS-CoV-2 may protect against future virus-associated disease is unknown. We invited all employees (n=15,300) at work at the Karolinska University Hospital, Stockholm, Sweden to participate in a study examining SARS-Cov-2 antibodies in relation to registered sick leave. For consenting 12,928 healthy hospital employees antibodies to SARS-CoV-2 could be determined and compared to participant sick leave records. Subjects with viral serum antibodies were not at excess risk for future sick leave (adjusted odds ratio (OR) controlling for age and sex: 0.85 [95% confidence interval (CI) (0.85 (0.43-1.68)]. By contrast, subjects with antibodies had an excess risk for sick leave in the weeks prior to testing [adjusted OR in multivariate analysis: 3.34 (2.98-3.74)]. Thus, presence of viral antibodies marks past disease and protection against excess risk of future disease. Knowledge of whether exposed subjects have had disease in the past or are at risk for future disease is essential for planning of control measures.Trial registration: First registered on 02/06/20, ClinicalTrials.gov NCT04411576.

  • The impact of macronutrient composition on metabolic regulation : An Islet-Centric view

    • Aim: The influence of dietary carbohydrates and fats on weight gain is inconclusively understood. We studied the acute impact of these nutrients on the overall metabolic state utilizing the insulin:glucagon ratio (IGR). Methods: Following in vitro glucose and palmitate treatment, insulin and glucagon secretion from islets isolated from C57Bl/6J mice was measured. Our human in vivo study included 21 normoglycaemia (mean age 51.9 ± 16.5 years, BMI 23.9 ± 3.5 kg/m2, and HbA1c 36.9 ± 3.3 mmol/mol) and 20 type 2 diabetes (T2D) diagnosed individuals (duration 12 ± 7 years, mean age 63.6 ± 4.5 years, BMI 29.1 ± 2.4 kg/m2, and HbA1c 52.3 ± 9.5 mmol/mol). Individuals consumed a carbohydrate-rich or fat-rich meal (600 kcal) in a cross-over design. Plasma insulin and glucagon levels were measured at −30, −5, and 0 min, and every 30 min until 240 min after meal ingestion. Results: The IGR measured from mouse islets was determined solely by glucose levels. The palmitate-stimulated hormone secretion was largely glucose independent in the analysed mouse islets. The acute meal tolerance test demonstrated that insulin and glucagon secretion is dependent on glycaemic status and meal composition, whereas the IGR was dependent upon meal composition. The relative reduction in IGR elicited by the fat-rich meal was more pronounced in obese individuals. This effect was blunted in T2D individuals with elevated HbA1c levels. Conclusion: The metabolic state in normoglycaemic individuals and T2D-diagnosed individuals is regulated by glucose. We demonstrate that consumption of a low carbohydrate diet, eliciting a catabolic state, may be beneficial for weight loss, particularly in obese individuals.

  • Patient-reported gastrointestinal and genitourinary toxicity after prostate cancer treatment : A comparison between radiotherapy including pelvic nodes and prostate-only radiotherapy

    • Background: To evaluate side effects of radiotherapy (RT) including the pelvic nodes in patients with prostate cancer. Methods: 143 patients with high risk prostate cancer (Pca) receiving whole pelvic radiotherapy (WPRT) was matched to a group of 142 patients receiving RT towards the prostate and seminal vesicles only (PORT). Both groups were given 78 Gy towards the prostate and 50 Gy towards the seminal vesicles. The WPRT group also received 50 Gy towards the pelvic nodes. WPRT was given with intensity modulated RT or volumetric modulated arc therapy technique and the group treated towards the prostate and seminal vesicles only was treated with 3-dimensional conformal RT. Gastrointestinal (GI) and genitourinary (GU) side effects were evaluated with patient-reported questionnaires and with the RTOG scale, scored by treating physician, at baseline and one-year after RT. Results: Overall the side effects were mild. At one-year follow up there were significant differences regarding morning urgency, mucus- and blood in stool between the two groups (Table 1). The WPRT group expressed lesser complications in all of these cases. In the RTOG GU and GI scores there were no differences between the groups. About 20% had RTOG grade 2 GU score and 2%/1% had RTOG grade 3 score in the PORT and WPRT groups, respectively. The numbers for RTOG grade 2 GI scores were 11% and 5% in the PORT and WPRT groups, respectively. None of the groups reported RTOG grade 3 GI symptoms. Conclusions: Whole pelvic RT can be done with a very satisfying profile of side effects. Sometimes even lesser gastrointestinal reported side effects are seen with the newer techniques compared to prostate only radiotherapy with older techniques.

    • Cancers have dysfunctional redox regulation resulting in reactive oxygen species production, damaging both DNA and free dNTPs. The MTH1 protein sanitizes oxidized dNTP pools to prevent incorporation of damaged bases during DNA replication. Although MTH1 is non-essential in normal cells, we show that cancer cells require MTH1 activity to avoid incorporation of oxidized dNTPs, resulting in DNA damage and cell death. We validate MTH1 as an anticancer target in vivo and describe small molecules TH287 and TH588 as first-in-class nudix hydrolase family inhibitors that potently and selectively engage and inhibit the MTH1 protein in cells. Protein co-crystal structures demonstrate that the inhibitors bindin the active site of MTH1. The inhibitors cause incorporation of oxidized dNTPs in cancer cells, leading to DNA damage, cytotoxicity and therapeutic responses in patient-derived mouse xenografts. This study exemplifies the non-oncogene addiction concept for anticancer treatment and validates MTH1 as being cancer phenotypic lethal.

    • Objective- Apolipoprotein CIII (apoCIII) is an independent risk factor for cardiovascular disease, but the molecular mechanisms involved are poorly understood. Here, we investigated potential proatherogenic properties of apoCIII-containing LDL from hypertriglyceridemic patients with type 2 diabetes. Research design and methods - LDL was isolated from controls and subjects with type 2 diabetes, and from apoB transgenic mice. LDL-biglycan binding was analyzed with a solid-phase assay using immunoplates coated with biglycan. Lipid composition was analyzed with mass spectrometry. Hydrolysis of LDL by sphingomyelinase was analyzed after labeling plasma LDL with [(3)H]sphingomyelin. ApoCIII isoforms were quantified after isoelectric focusing. Human aortic endothelial cells were incubated with desialylated apoCIII or with LDL enriched with specific apoCIII isoforms. Results- We showed that enriching LDL with apoCIII only induced a small increase in LDL-proteoglycan binding, and this effect was dependent on a functional Site A in apoB100. Our findings indicated that intrinsic characteristics of the diabetic LDL other than apoCIII per se are responsible for further increased proteoglycan binding of diabetic LDL with high endogenous apoCIII, and we showed alterations in the lipid composition of diabetic LDL with high apoCIII. We also demonstrated that high apoCIII increased susceptibility of LDL to hydrolysis and aggregation by SMase. In addition, we demonstrated that sialylation of apoCIII increased with increasing apoCIII content, and that sialylation of apoCIII was essential for its proinflammatory properties. Conclusions- We have demonstrated a number of features of apoCIII-containing LDL from hypertriglyceridemic patients with type 2 diabetes that could explain the proatherogenic role of apoCIII.

  • Systematic evaluation of SARS-CoV-2 antigens enables a highly specific and sensitive multiplex serological COVID-19 assay

    • Objective. The COVID-19 pandemic poses an immense need for accurate, sensitive and high-throughput clinical tests, and serological assays are needed for both overarching epidemiological studies and evaluating vaccines. Here, we present the development and validation of a high-throughput multiplex bead-based serological assay. Methods. More than 100 representations of SARS-CoV-2 proteins were included for initial evaluation, including antigens produced in bacterial and mammalian hosts as well as synthetic peptides. The five best-performing antigens, three representing the spike glycoprotein and two representing the nucleocapsid protein, were further evaluated for detection of IgG antibodies in samples from 331 COVID-19 patients and convalescents, and in 2090 negative controls sampled before 2020. Results. Three antigens were finally selected, represented by a soluble trimeric form and the S1-domain of the spike glycoprotein as well as by the C-terminal domain of the nucleocapsid. The sensitivity for these three antigens individually was found to be 99.7%, 99.1% and 99.7%, and the specificity was found to be 98.1%, 98.7% and 95.7%. The best assay performance was although achieved when utilising two antigens in combination, enabling a sensitivity of up to 99.7% combined with a specificity of 100%. Requiring any two of the three antigens resulted in a sensitivity of 99.7% and a specificity of 99.4%. Conclusion. These observations demonstrate that a serological test based on a combination of several SARS-CoV-2 antigens enables a highly specific and sensitive multiplex serological COVID-19 assay.

  • Incongruent odors suppress perceptual categorization of visual objects : Behavioral and ERP evidence

    • Visual stimuli often dominate non-visual stimuli during multisensory perception, and evidence suggests higher cognitive processes prioritize visual over non-visual stimuli during divided attention. Visual stimuli may therefore have privileged access to higher mental processing resources, relative to other senses, and should be disproportionally distracting when processing incongruent cross-sensory stimuli. We tested this assumption by comparing visual processing with olfaction, a “primitive” sensory channel that detects potentially hazardous chemicals by alerting attention. Behavioral and event-related brain potentials (ERPs) were assessed in a bimodal object categorization task with congruent or incongruent odor-picture pairings and a delayed auditory response target. For congruent pairings, accuracy was higher for visual compared to olfactory decisions. However, for incongruent pairings, reaction times (RTs) were faster for olfactory decisions, suggesting incongruent odors interfered more with visual decisions, thereby showing an “olfactory dominance effect”. Categorization of incongruent pairings engendered a late “slow wave” ERP effect. Importantly, this effect had a later amplitude peak and longer latency during visual decisions, likely reflecting additional categorization effort for visual stimuli. In sum, contrary to what might be inferred from theories of ”visual dominance”, incongruent odors may in fact uniquely attract mental processing resources during perceptual incongruence.

    • Purpose: In the digital society, people need to make use of a range of everyday technology (ET) when engaging in activities in various places outside home. The aim of this study was to describe and compare the use of ET and places visited outside the home and, also, to describe the relationship between them in people with different severity of disability after acquired brain injury (ABI).Materials and methods: Instruments addressing ET use, places visited outside the home and severity of disability were used to assess 74 individuals with ABI. Relationships were analyzed statistically.Results: A significantly higher use of personal ET related to public space and public space ET, a higher ability to use ET and more places visited outside the home was found in those with good recovery (GR) compared to those with moderate disability/severe disability (MD/SD). The use of ET was significantly correlated with places visited in the total sample and in those with MD/SD, but for those with GR, no significant correlations were found.Conclusions: To facilitate participation after ABI, the relationship between the use of ET and places visited outside the home needs to be assessed in rehabilitation.

    • Background: Being engaged in activities in places outside the home after a stroke can be challenging. Knowledge about what characterize places outside the home is important to support participation.Objectives: To explore patterns of participation in places for activities outside the home after stroke and whether these patterns were associated with personal and environmental aspects.Material and methods: Sixty-three people with stroke were interviewed using the Participation in Activities and Places Outside Home (ACT-OUT) questionnaire. A two-step cluster analysis was used to identify patterns of participation and non-parametric test was used to explore potential associations to the patterns of participation.Results: Four clusters of patterns of participation, based on frequency, familiarity of the place/the way to the place and perceived distance, were identified. The patterns were significantly associated with type of place, activity domain, retained or abandoned participation, transportation and being accompanied by someone. The severity of disability was significantly associated with groups of individuals with different patterns of participation.Conclusions: Different combinations of aspects facilitated and hindered whether or not participation changed. To support people with stroke in their endeavour to retain or recapture participation, social support is important to consider in relation to transportation, activities and places outside the home.

    • IntroductionKnowledge about the places people visit or do not visit after stroke is lacking. The aim of this study was to describe and compare the places visited for activities outside the home of people with stroke of working age and to explore the influence of the severity of disability, fatigue, driving a car and sociodemographic characteristics on the total number of places visited for activities outside the home.MethodsAn exploratory cross-sectional study was conducted and 63 people with stroke were interviewed with instruments that covered places outside the home, severity of disability and fatigue. Data were analysed using non-parametric tests, analysis of variance and the general linear model.ResultsSignificant difference in the total number of places visited for activities outside the home were found between all three groups of severity of disability. The good recovery group visited eight places to a significantly higher extent than those with severe/moderate disability. The severity of disability and driving a car were the only aspects that significantly influenced the total number of places visited.ConclusionsPlaces for activities outside the home people with stroke visit and changes therein can add valuable knowledge about participation that can be used in the design of rehabilitation.

    • Given the importance of prevention of complications in type 2 diabetes (T2D), we aimed to examine changes over time in consumption of fruits, vegetables and juice among men who were diagnosed with T2D in comparison with men without diabetes. The prospective Cohort of Swedish Men, aged 45-79 years in 1997, was used to examine changes in diet after diagnosis of T2D. Dietary intake was assessed using FFQ in 1997 and 2009. In all, 23 953 men who were diabetes free at baseline (1997) and completed both FFQ were eligible to participate in the study. Diagnosis of T2D was reported by subjects and ascertained through registers. Multivariable linear mixed models were used to examine changes in mean servings/week over time. In total, 1741 men developed T2D during the study period. Increased consumption of vegetables and fruits was observed among those who developed T2D (equivalent to 1·6 servings/week, 95 % CI 1·08, 2·03) and men who remained diabetes free (0·7 servings/week, 95 % CI 0·54, 0·84). Consumption of juice decreased by 0·6 servings/week (95 % CI -0·71, -0·39) among those who developed T2D and increased by 0·1 servings/week (95 % CI 0·05, 0·15) in those who were diabetes free. Changes over time and between groups were statistically significant. Although improvements in diet were observed, only 36 % of those with T2D and 35 % of those without diabetes consumed ≥5 servings of fruits and vegetables/d in 2009.

  • Diet and postprandial risk markers for complications in type 2 diabetes

    • The period after a meal is complex with fluctuation in blood glucose, lipids and other metabolic responses. This may induce and/or increase inflammation and contribute to future development of diabetic complications. Prevention of complications include well-controlled disease management, including diet. There are however gaps in the literature for dietary recommendations in diabetes, and whether those with type 2 diabetes (T2D) make dietary changes. The aims of this doctoral thesis were: 1) To study the acute effects of fructose loading on levels of serum uric acid, metabolic and inflammatory markers using isocaloric drinks; Coca-Cola (17.5 g fructose), blueberry drink (18 g fructose) and a pure fructose drink (35 g fructose), without and with a pizza. 3) To study the acute effect of meals with different compositions of high carbohydrate (HC) (52E%), HC & fibers (50E%, 15 g), low carbohydrate (LC, 32E%)+high fat (HF) (50E%) and LC (28E%)+high protein (HP) (41E%) on metabolic and inflammatory markers 4) Examine possible changes in fruits and vegetables consumption. The effects of acute fructose loading on levels of serum uric acid were examined in T2D (n=7), chronic kidney disease (n=3) and healthy subjects (HS) (n=6). Serum uric acid increased over time following fructose loading. The highest response was observed following fructose drink, and the lowest following the blueberry drink (p<0.05). The effect of acute fructose loading on glucose, insulin and inflammatory markers were examined in T2D and HS. The response in glucose and insulin was greater following Coca-Cola (p<0.05). MCP-1 decreased in both groups following blueberry drink and Coca-Cola (T2D; p=0.02, HS; p=0.03), probably secondary to the insulin response. The results suggests that drinks with added fructose should be avoided, and that blueberry is protective on uric acid and glucose response. The effect of meal composition on metabolic and inflammatory markers were examined in T2D and HS. HC meals induced the highest response in glucose and insulin, and LC+HF in triglycerides (p<0.05). The inflammatory marker VCAM-1 decreased following LC+HP meal (T2D; p=0.03, HS; p=0.003), while ICAM-1 decreased following LC+HF (p=0.02) in T2D and following LC+HP (p=0.03) in HS. PAI-1 decreased following HC (T2D; p=0.04, HS; p=0.006) and LC+HP (T2D; p=0.03, HS; p=0.01) and in T2D also following LC+HF (p=0.04). The responses did not differ between meals, probably due to the healthy composition of meals. Thus, LC meals with a healthy composition of fibers, vegetables, berries, mono and poly- unsaturated fat and plant-based proteins could be recommended to subjects with T2D. Possible changes in intake of fruits and vegetables consumption over time was explored in a prospective cohort of men using food frequency questionnaires in 1997 and 2009. 1 741 men developed T2D and 22 212 remained free from diabetes. Increased intake of fruit and vegetables was greater among those who developed T2D (1.6 servings/week, 95% CI 1.08; 2.03) compared to those remained free from diabetes (0.7 servings/week, 95% 0.54; 0.84). Although improvements in consumption were observed, only 36% of those with T2D consumed ≥5 servings per day in 2009. Thus, there is a need for nutritional education in T2D.

    • At the grave fields at Krankmårtenhögen and Smalnäset, Härjedalen, 200 bc?ad 200, there were depositions of skull fragments and antlers on top of the graves. Some skulls and antlers have been damaged. For instance, there are some cases where the antlers have been chopped off with an axe. This paper argues that this treatment and the depositions are ways of communicating a message of regeneration to the animals, with whom people had crucial and reciprocal social relations based on trust. By symbolically shedding the antlers, people intended to persuade the animals to make new antlers. Such an interpretation is based on the assumption that the dichotomies between nature and culture, and the religious and the profane, are irrelevant to understanding the conception of the environment of the people who made the grave fields. The depositions were made prior the introduction of reindeer herding, a process with the implication that the relation of trust is transformed into one of dominance. The depositions of animal bones and antlers can be seen as strategic means of maintaining good relations and communicating differences and similarities with animal persons as well as adjacent human groups in the process towards an early ethnic dualism in southern Sápmi. As such, they have a great value of their own and should not be considered as an anomaly from cultural expressions of clearly defined Sámi and Germanic ethnicities.

    • During multisensory experiences, visual stimuli typically suppress non-visual stimuli. Such ”visual dominance” effects might stem from inhibition across sensory systems. Does visual dominance generalize to odor-visual pairings? We developed a categorization task (fruits vs flowers) with congruent and incongruent odor-picture pairings and a delayed auditory response target that informed about categorization modality (olfactory vs visual). We investigated behavioral and cortical (ERP) responses. For congruent pairings, we found better accuracy for visual decisions. However, for incongruent pairings, we insteadobserved faster RTs for olfactory decisions. Incongruent olfactory stimuli thus interfere more with visualdecisions than vice versa. Our ERP results from auditory targets on incongruent trials gave supporting evidence of olfactory suppression over visual perception; higher P300 amplitudes were more strongly correlated with faster RTs during visual categorization. A late “slow wave” ERP effect had later onset andlonger latency during visual vs olfactory decisions. This indicates that in order to rapidly and successfully categorize visual stimuli (and ignore incongruent odors), participants need to allocate additional attentional and working memory resources. In sum, both behavioral and ERP effects suggest a higher level of interference from incongruent olfactory, compared to visual, input. These findings suggest that asymmetric inhibition across sensory systems is a fruitful way of studying sensory dominance, and that olfactory stimuli can dominate visual stimuli, refuting the general notion of ”visual dominance”.

  • Composing The Performance : An Exploration Of Musical Composition As A Dramaturgical Strategy In Contemporary Intermedial Theatre

    • This thesis explores musical composition in the context of contemporary theatre. It suggests methods where compositional thinking – broadly understood – can be used as a dramaturgical device in the creation of theatre performances. Such musicalization of theatre provides new methods for an artistic practice and questions traditional artistic hierarchies and working structures.   The investigation departs from a process of transference of radiophonic art to stage performances. From this a series of interdisciplinary works in the intersection between concert, theatre performance, radio play and music theatre has been created, each with different approaches to the roles and functions of musical composition. Through these works the creative processes and methods are examined and analysed.   The notion of vertical dramaturgy and polyphony in theatre contexts is discussed and demonstrated, which pinpoints the significance of interaction between the elements of a performance. The interplay between dramatic writing, acting methods and musical composition is under scrutiny, as is the way in which visual elements as video, light and scenography are integrated into polyphonic theatre experiences. Transformation is an essential compositional and dramaturgical tool as it allows a performance to move seamlessly between different art forms, continuously creating new relationships.   Through the research a model for interdisciplinary, collaborative work in performing arts emerges. The concepts of Shared space and Shared time are introduced and enable a collective ‘thinking-through-practice’ and expansion of the artistic roles. This joint arena of space and time opens up new connections, interactions and mutual exchanges between artists and art forms.   The outcome provides a range of artistic methods in the field of contemporary intermedial theatre where musical composition is one of the dramaturgical devices for composing the performances.

  • The β Pictoris system : Setting constraints on the planet and the disk structures at mid-IR wavelengths with NEAR

    • Context. β Pictoris is a young nearby system hosting a well-resolved edge-on debris disk, along with at least two exoplanets. It offers key opportunities for carrying out detailed studies of the evolution of young planetary systems and their shaping soon after the end of the planetary formation phase. Aims. We analyzed high-contrast coronagraphic images of this system, obtained in the mid-infrared, taking advantage of the NEAR experiment using the VLT/VISIR instrument, which provides access to adaptive optics, as well as phase coronagraphy. The goal of our analysis is to investigate both the detection of the planet β Pictoris b and of the disk features at mid-IR wavelengths. In addition, by combining several epochs of observation, we expect to constrain the position of the known clumps and improve our knowledge on the dynamics of the disk. Methods. We observed the β Pictoris system over two nights in December 2019 in the 10- 12.5 μm coronagraphic filter. To evaluate the planet b flux contribution, we extracted the photometry at the expected position of the planet and compared it to the flux published in the literature. In addition, we used previous data from T-ReCS and VISIR in the mid-IR, updating the star's distance, to study the evolution of the position of the southwest clump that was initially observed in the planetary disk back in 2003. Results. While we did not detect the planet b, we were able to put constraints on the presence of circumplanetary material, ruling out the equivalent of a Saturn-like planetary ring around the planet. The disk presents several noticeable structures, including the known southwest clump. Using a 16-yr baseline, sampled with five epochs of observations, we were able to examine the evolution of the clump. We found that the clump orbits in a Keplerian motion with a semi-major axis of 56.1-0.3+0.4 au. In addition to the known clump, the images clearly show the presence of a second clump on the northeast side of the disk as well as possibly fainter and closer structures that are yet to be confirmed. Furthermore, we found correlations between the CO clumps detected with ALMA and the northeastern and southwestern clumps in the mid-IR images. Conclusions. If the circumplanetary material were located at the Roche radius, the maximum amount of dust determined from the flux upper limit around β Pictoris b would correspond to the mass of an asteroid of 5 km in diameter. Finally, the Keplerian motion of the southwestern clump is possibly indicative of a yet-to-be detected planet or signals the presence of a vortex.
