Därför stödjer vi deras krav, skriver 270 klimatforskare.

    • OBJECTIVES The aim of this study was to investigate the clinical outcomes of patients deferred from coronary revascularization on the basis of instantaneous wave-free ratio (iFR) or fractional flow reserve (FFR) measurements in stable angina pectoris (SAP) and acute coronary syndromes (ACS). BACKGROUND Assessment of coronary stenosis severity with pressure guidewires is recommended to determine the need for myocardial revascularization. METHODS The safety of deferral of coronary revascularization in the pooled per-protocol population (n = 4,486) of the DEFINE-FLAIR (Functional Lesion Assessment of Intermediate Stenosis to Guide Revascularisation) and iFR-SWEDEHEART (Instantaneous Wave-Free Ratio Versus Fractional Flow Reserve in Patients With Stable Angina Pectoris or Acute Coronary Syndrome) randomized clinical trials was investigated. Patients were stratified according to revascularization decision making on the basis of iFR or FFR and to clinical presentation (SAP or ACS). The primary endpoint was major adverse cardiac events (MACE), defined as the composite of all-cause death, nonfatal myocardial infarction, or unplanned revascularization at 1 year. RESULTS Coronary revascularization was deferred in 2,130 patients. Deferral was performed in 1,117 patients (50%) in the iFR group and 1,013 patients (45%) in the FFR group (p < 0.01). At 1 year, the MACE rate in the deferred population was similar between the iFR and FFR groups (4.12% vs. 4.05%; fully adjusted hazard ratio: 1.13; 95% confidence interval: 0.72 to 1.79; p = 0.60). A clinical presentation with ACS was associated with a higher MACE rate compared with SAP in deferred patients (5.91% vs. 3.64% in ACS and SAP, respectively; fully adjusted hazard ratio: 0.61 in favor of SAP; 95% confidence interval: 0.38 to 0.99; p = 0.04). CONCLUSIONS Overall, deferral of revascularization is equally safe with both iFR and FFR, with a low MACE rate of about 4%. Lesions were more frequently deferred when iFR was used to assess physiological significance. In deferred patients presenting with ACS, the event rate was significantly increased compared with SAP at 1 year. (C) 2018 The Authors.     • Believed to have a potential in reducing the NOxemission level, hydrogen peroxide was fumigated into theinlet duct of the AVL single cylinder research engine via astandard gasoline injector, normally used in the Volvo850-car engine. A small metallic sphere installed 3 cmdownstream the injector tip, improved the spray formationand the uniform distribution of the fumigated peroxidefluid upstream the intake valve. The hydrogen peroxideflow was varied according to the desired value via anelectronic pulse frequency generator.The engine, equipped with an electronic unit injector, wasinitially run without any fumigation fluid until thespecifications of the engine test point were reached andremained very stable. Further, the hydrogen peroxideinjection was activated with three different injection flows,and the engine performance, including emission levels,was compared to reference performance. To avoidsecondary effects on combustion processes andemission levels, the engine was run with the same fuel/airratio.The most expected effect of the hydrogen peroxidefumigation would be on the NOx emission reduction.However, it was observed that other emissions such asCO and HC were remarkably affected as well.Experimental results have shown that the hydrogenperoxide fumigation affects the engine thermochemistry,but not necessarily the fuel consumption and otherenergetic parameters of the engine. However, theincrease of the fumigation flow above a certain limit coulddeteriorate the combustion quality, thereby increase HC,CO and soot formation levels.Nevertheless, heat release and ignition delay, (evaluatedaccording to the “Dragon” software program andacquisition from “Burst to File” PC based samplingsystem)[1], [3] show that the optimal fumigated flow of thehydrogen peroxide might reduce the ignition delay, andthereby significantly improve the engine thermochemistryoutput.Verified effective possibilities for using hydrogen peroxidefumigation, would lead to a proper version of the intakemanifold design, which would bring to another solution forlow emission level diesel engines

  • 5-Year Outcomes of PCI Guided by Measurement of Instantaneous Wave-Free Ratio Versus Fractional Flow Reserve

    • BACKGROUND: Instantaneous wave-free ratio (iFR) is a coronary physiology index used to assess the severity of coronary artery stenosis to guide revascularization. iFR has previously demonstrated noninferior short-term outcome compared to fractional flow reserve (FFR), but data on longer-term outcome have been lacking.OBJECTIVES: The purpose of this study was to investigate the prespecified 5-year follow-up of the primary composite outcome of all-cause mortality, myocardial infarction, and unplanned revascularization of the iFR-SWEDEHEART trial comparing iFR vs FFR in patients with chronic and acute coronary syndromes.METHODS: iFR-SWEDEHEART was a multicenter, controlled, open-label, registry-based randomized clinical trial using the Swedish Coronary Angiography and Angioplasty Registry for enrollment. A total of 2,037 patients were randomized to undergo revascularization guided by iFR or FFR.RESULTS: No patients were lost to follow-up. At 5 years, the rate of the primary composite endpoint was 21.5% in the iFR group and 19.9% in the FFR group (HR: 1.09; 95% CI: 0.90-1.33). The rates of all-cause death (9.4% vs 7.9%; HR: 1.20; 95% CI: 0.89-1.62), nonfatal myocardial infarction (5.7% vs 5.8%; HR: 1.00; 95% CI: 0.70-1.44), and unplanned revascularization (11.6% vs 11.3%; HR: 1.02; 95% CI: 0.79-1.32) were also not different between the 2 groups. The outcomes were consistent across prespecified subgroups.CONCLUSIONS: In patients with chronic or acute coronary syndromes, an iFR-guided revascularization strategy was associated with no difference in the 5-year composite outcome of death, myocardial infarction, and unplanned revascularization compared with an FFR-guided revascularization strategy.     • Between the Promise of Specificity and the Demand for Evidence – A study of knowledge and the approach to knowledge in socially oriented non-profit, private and public sector organizationsIn the social work field it is possible to identify two parallel processes in time. On the one hand a qualitative developmental process―even towards a form of standardization―where central concepts are academisation, professionalization, scientifically produced knowledge, expertise and evidence based methods. Simultaneously, there is a drive to create the conditions for increased diversity, where hopes are especially being pinned on the non-profit sector. In spite of representations and expectations concerning the non-profit sector and its so-called specificity, however, much of existing research lacks a comparative perspective, i.e., studies where non-profit organizations are related to comparable activities in the private and public sector.The aim of the study―with special focus on issues concerning evidence based knowledge in social work―is to compare and analyse whether and in that case how employee conceptions differ between the sectors, and whether and in what way non-profit employees and their activities can be said to fulfil the expectations of contributing to increased diversity. Data is from a questionnaire directed to about 1300 social work employees.The results show a greater interest in research and more marked efforts at professionalization in the public sector, while above all in the non-profit sector there was skepticism about science paired with reservations about work carried out in a professional way. In the non-profit, but also in the private sector, issues of ethics, views of humanity and values were paid greater attention. Also stressed here was the importance of creating relationships, the unique human encounter, genuine commitment, and human kindness. However, there was significant uniformity across all sectors in the use of methods, where three dominated: solution focused measures, network support/therapy, and psychosocial work.

    • Cyclosporine A (CsA) has been shown to protect against myocardial ischemia and reperfusion (I/R) injury in small animal models. The aim of the current study was to evaluate the effects of CsA on myocardial I/R injury in a porcine model. Pigs were randomized between CsA (10mg/kg; n = 12) or placebo (n = 15) and anesthetized with either isoflurane (phase I) or pentobarbital (phase II). By catheterization, the left descending coronary artery was occluded for 45 minutes, followed by reperfusion for 2 hours. Hearts were stained to quantify area at risk (AAR) and infarct size (IS). Myocardial biopsies were obtained for terminal dUTP nick end labeling and immunoblot analysis of proapoptotic proteins (apoptosis-inducing factor [AIF], BCL2/adenovirus E1B 19-kd interacting protein 3 [BNIP-3], and active caspase-3). Cyclosporine A did not reduce IS/AAR compared with placebo (49% vs 41%, respectively; P = .21). Pigs anesthetized with isoflurane had lower IS/AAR than pigs anesthetized with pentobarbital (39% vs 51%, respectively; P = .03). This reduction in IS/AAR seemed to be attenuated by CsA. Apoptosis-inducing factor protein expression was higher after CsA administration than after placebo (P = .02). Thus, CsA did not protect against I/R injury in this porcine model. The data suggest a possible deleterious interaction of CsA and isoflurane.

    • Ground level ozone [ozone] is considered a harmful air pollutant but there is a knowledge gap regarding its long term health effects. The main aim of this study is to develop local Land Use Regression [LUR] models that can be used to study long term health effects of ozone. The specific aim is to develop spatial LUR models for two Swedish cities, Umea and Malmo, as well as a temporal model for Malmo in order to assess ozone exposure for long term epidemiological studies. For the spatial model we measured ozone, using Ogawa passive samplers, as weekly averages at 40 sites in each study area, during three seasons. This data was then inserted in the LUR-model with data on traffic, land use, population density and altitude to develop explanatory models of ozone variation. To develop the temporal model for Malmo, hourly ozone data was aggregated into daily means for two measurement stations in Malmo and one in a rural area outside Malmo. Using regression analyses we inserted meteorological variables into different temporal models and the one that performed best for all three stations was chosen. For Malmo the LUR-model had an adjusted model R-2 of 0.40 and cross validation R-2 of 0.17. For Umea the model had an adjusted model R-2 of 0.67 and cross validation adjusted R-2 of 0.48. When restricting the model to only including measuring sites from urban areas, the Malmo model had adjusted model R-2 of 0.51 (cross validation adjusted R-2 0.33) and the Umea model had adjusted model R-2 of 0.81 (validation adjusted R-2 of 0.73). The temporal model had adjusted model R-2 0.54 and 0.61 for the two Malmo sites, the cross validation adjusted R-2 was 0.42. In conclusion, we can with moderate accuracy, at least for Umea, predict the spatial variability, and in Malmo the temporal variability in ozone variation. (C) 2014 The Authors.     • OBJECTIVE: The main aim of this study was to identify foetal susceptibility genes on chromosome six for Ro/SSA autoantibody-mediated congenital heart block.SUBJECTS AND DESIGN: Single nucleotide polymorphism (SNP) genotyping of individuals in the Swedish Congenital Heart Block (CHB) study population was performed. Low-resolution HLA-A, -Cw and -DRB1 allele typing was carried out in 86 families comprising 339 individuals (86 Ro/SSA autoantibody-positive mothers, 71 fathers, 87 CHB index cases, and 95 unaffected siblings).RESULTS: A case-control comparison between index cases and population-based out-of-study controls (n=1710) revealed association of CHB with 15 SNPs in the 6p21.3 MHC locus at a chromosome-wide significance of p<2.59×10(-6) (OR 2.21-3.12). In a family-based analysis of association of SNP markers as well as distinct MHC class I and II alleles with CHB, HLA-DRB1*04 and HLA-Cw*05 variants were significantly more frequently transmitted to affected individuals (p<0.03 and p<0.05, respectively), while HLA-DRB1*13 and HLA-Cw*06 variants were significantly less often transmitted to affected children (p<0.04 and p<0.03). We further observed marked association of increased paternal (but not maternal) HLA-DRB1*04 transmission to affected offspring (p<0.02).CONCLUSIONS: HLA-DRB1*04 and HLA-Cw*05 were identified as novel foetal HLA allele variants that confer susceptibility to CHB in response to Ro/SSA autoantibody exposure, while DRB1*13 and Cw*06 emerged as protective alleles. Additionally, we demonstrated a paternal contribution to foetal susceptibility to CHB for the first time.

    • Objective Congenital heart block may develop in the fetuses of Ro/SSA-positive and La/SSB-positive mothers. Recurrence rates of only 10-20% despite persisting maternal antibodies indicate that additional factors are critical for the establishment of heart block. The authors investigated the influence of other maternal and fetal factors on heart block development in a Swedish population-based cohort. less thanbrgreater than less thanbrgreater thanMethods The influence of fetal gender, maternal age, parity and time of birth on heart block development was analysed in 145 families, including Ro/La-positive (n=190) and Ro/La-negative (n=165) pregnancies. less thanbrgreater than less thanbrgreater thanResults There was a recurrence rate of 12.1% in Ro/La-positive women, and no recurrence in Ro/La-negative women. Fetal gender and parity did not influence the development of heart block in either group. Maternal age in Ro/La-positive pregnancies with a child affected by heart block was, however, significantly higher than in pregnancies resulting in babies without heart block (pandlt;0.05). Seasonal timing of pregnancy influenced the outcome. Gestational susceptibility weeks 18-24 occurring during January-March correlated with a higher proportion of children with heart block and lower vitamin D levels during the same period in a representative sample of Swedish women and a corresponding higher proportion of children with heart block born in the summer (pandlt;0.02). Maternal age or seasonal timing of pregnancy did not affect the outcome in Ro/La-negative pregnancies. less thanbrgreater than less thanbrgreater thanConclusion This study identifies maternal age and seasonal timing of pregnancy as novel risk factors for heart block development in children of Ro/La-positive women. These observations may be useful for counselling when pregnancy is considered.

    • Multiple sclerosis (MS) is the most common chronic inflammatory disease of the central nervous system and also is regarded as an autoimmune condition. However, the antigenic targets of the autoimmune response in MS have not yet been deciphered. In an effort to mine the autoantibody repertoire within MS, we profiled 2,169 plasma samples from MS cases and population-based controls using bead arrays built with 384 human protein fragments selected from an initial screening with 11,520 antigens. Our data revealed prominently increased autoantibody reactivity against the chloride-channel protein anoctamin 2 (ANO2) in MS cases compared with controls. This finding was corroborated in independent assays with alternative protein constructs and by epitope mapping with peptides covering the identified region of ANO2. Additionally, we found a strong interaction between the presence of ANO2 autoantibodies and the HLA complex MS-associated DRB1*15 allele, reinforcing a potential role for ANO2 autoreactivity in MS etiopathogenesis. Furthermore, immunofluorescence analysis in human MS brain tissue showed ANO2 expression as small cellular aggregates near and inside MS lesions. Thus this study represents one of the largest efforts to characterize the autoantibody repertoire within MS. The findings presented here demonstrate that an ANO2 autoimmune subphenotype may exist in MS and lay the groundwork for further studies focusing on the pathogenic role of ANO2 autoantibodies in MS.

  • Coal Comminution Characterization for Industrial Scale Circulating Fluidized Bed

    • The management of the bed solids inventory and particle size distribution in fluidized bed combustors has long been recognized as a key factor for the efficient and trouble-free operation of industrial scale fluidized bed ombustors. From a coal particle to an ash particle getting out the circulating loop, the particle size reduction factor can be hundred. Detailed pilot scale test observations related to the size of circulating particles and ash particles, show very different behaviours for different coals. In order to describe those behaviours related to a large particle size spectrum, a general frame work is proposed based on several concepts. The key concept is represented by the Primary Ash Particle Size Distribution (PAPSD). According to this concept, Primary Ash Particles are liberated from the carbon matrix under the combined action of Combustion and Primary Attrition of the mother fuel particles. By Primary Attrition it is meant here a complex of particle comminution phenomena closely associated with the progress of reactions (devolatilization/char combustion), along pathways and with mechanisms that are extensively discussed by Chirone et al. (1991). Further (Secondary) attrition of ash particles can occur after char combustion is complete and is not associated with the parallel progress of chemical reactions but to mechanical attrition only. Those concepts are associated with coal characterization procedures in order to have information on the size reduction of a coal particle and the final size istribution of ash. The measurement obtained on a lab scale is validated by the analysis of some results of industrial-scale tests. In association with procedure definition, a qualitative approach allows prediction and inter-coal comparison. A modelisation work is also presented considering however that quantitative predictions of the particle size distribution inside a CFBC is still a far objective.

    • The FeCrAlRE (where RE is reactive element) alloy Kanthal AF was exposed isothermally at 600 and 800 degrees C for 72 h in dry O-2 and in O-2 with 10 vol % H2O. The mass gains were 3-5 times higher at the higher temperature. The presence of water vapor increased the oxidation rate at 800 degrees C, while no significant effect was observed at 600 degrees C. A thin two-layered oxide formed at 600 degrees C: an outer (Fe, Cr)(2)O-3 corundum-type oxide, containing some Al, and an inner, probably amorphous, Al-rich oxide. At 800 degrees C a two-layered oxide formed in both environments. The inner layer consisted of inward grown alpha-Al2O3. In dry O-2 the originally formed outward grown gamma-Al2O3 had transformed to alpha-Al2O3 after 72 h. Water vapor stabilized the outward grown gamma-Al2O3 and hence no transformation occurred after 72 h in humid environment. RE-rich oxide particles with varying composition (Y, Zr, and Ti) were distributed in the base oxide at both temperatures and in both environments. The RE-rich particles were separated from the alloy substrate by a layer of Al-rich oxide. At 800 degrees C the Y-rich RE particles were surrounded by thick oxide patches in both dry and humid O-2.

    • The FeCrAlRE (where RE is reactive element) alloy Kanthal AF was exposed isothermally at 600 and 800°C for 72 h in dry O2 and in O2 with 10 vol % H2O. The mass gains were 3–5 times higher at the higher temperature. The presence of water vapor increased the oxidation rate at 800°C, while no significant effect was observed at 600°C. A thin two-layered oxide formed at 600°C: an outer (Fe,Cr)2O3 corundum-type oxide, containing some Al, and an inner, probably amorphous, Al-rich oxide. At 800°C a two-layered oxide formed in both environments. The inner layer consisted of inward grown a-Al2O3. In dry O2 the originally formed outward grown g-Al2O3 had transformed to a-Al2O3 after 72 h. Water vapor stabilized the outward grown g-Al2O3 and hence no transformation occurred after 72 h in humid environment.     • The increasing contamination of freshwater with pharmaceuticals, surfactants, pesticides and other organic compounds are of major concern. As these contaminants are detected at trace levels in the environment it is important to determine if they elicit biological responses at the observed levels. In addition to chemical pollutants, there is also a concern for increasing levels of bacteria and other microorganisms in freshwater systems. In an earlier study, we observed the activation of inflammatory systems downstream of a wastewater treatment plant (WWTP) in southern Sweden. We also observed that the water contained unidentified components that were pro-inflammatory and potentiated the immune response in human urinary bladder epithelial cells. In order to determine if these effects were unique for the studied site or represent a common response in Swedish water, we have now performed a study on three WWTPs and their recipient waters in central Sweden. Analysis of immune responses in urinary bladder epithelial cells, monocyte-like cells and blood mononuclear cells confirm that these waters activate the immune system as well as induce pro-inflammatory responses. The results indicate that the cytokine profiles correlate to the endotoxin load of the waters rather than to the levels of pharmaceuticals or culturable bacteria load, suggesting that measurements of endotoxin levels and immune responses would be a valuable addition to the analysis of inland waters.

    • A Rapidly Solidified Powder (RSP) metallurgical FeCrAl alloy, Kanthal APMT, was exposed in dry and humid O-2 for 72 h at 900-1,100 A degrees C. The formed oxide scales were characterized using gravimetry in combination with advanced analysis techniques (SEM, EDX, TEM, XRD, AES and SIMS). The oxide scales were at all exposures composed of two-layered alpha-Al2O3 scales exhibiting a top layer of equiaxed grains and a bottom layer containing elongated grains. A Cr-rich zone, originating in the native oxide present before exposure, separated these two layers. The top alpha-Al2O3 layer is suggested to have formed by transformation of outwardly grown metastable alumina, while the inward-grown bottom alpha-Al2O3 layer had incorporated small Zr-, Hf- and Ti-rich oxide particles present in the alloy matrix. The scale also contained larger Y-rich oxide particles. Furthermore, in the temperature range studied, the presence of water vapour accelerated alloy oxidation somewhat and affected scale morphology.

    • High temperature resistant FeCrAl alloys are frequently used in high temperature applications Such as heating elements and metal based catalytic converter bodies. When exposed to high temperatures an adherent, slowly growing, dense aluminium oxide layer forms on the surface, which protects the underlying alloy from severe degradation. The composition, structure and properties of the formed oxide layer are strongly dependent on the alloy composition, temperature and oxidation environment. In this study, the Sandvik 0C404 FeCrAl alloy, in the form of 50 mu m thick foils, was exposed isothermally in the temperature range 500-900 degrees C for 168 hours in dry O-2 and in O-2 with 40 vol.% H2O. The surface morphology, composition and microstructure of the grown oxide scales were characterized by using scanning electron microscopy (SEM), transmission electron microscopy (TEM), energy dispersive X-ray spectroscopy (EDX), grazing incidence X-ray diffraction (GI-XRD), Auger electron spectroscopy (AES), and time of flight secondary ion mass spectrometry (TOF-SIMS). The oxidation process was faster at 900 degrees C than at 500 and 700 degrees C. At 500 degrees C a thin (10-20 nm) mixed oxide of Fe, Cr and Al was formed. Exposure at 700 degrees C resulted ill a similar (40-50 nm) duplex oxide, in both dry O-2 and in O-2 With 40 vol.% H2O. These oxide scales consisted of all inner and an outer relatively pure alumina separated by a Cr-rich band. This type of duplex oxide scale also formed at 900 degrees C with a thin inward growing alpha-Al2O3 at the oxide/metal interface and an outward growing layer outside a Cr-rich band. However, at 900 degrees C the Outward growing layer showed two types of oxide morphologies; a thin smooth base oxide and a much thicker nodular oxide grown on top of substrate ridges. In dry O-2 atmosphere, the main part of this outward growing layer had transformed to alpha-Al2O3. Only in the outer part of the thick oxide nodules, metastable alumina was found. When exposed in the presence of water vapour the main pall of the metastable alumina remained untransformed.

    • The Rosetta dual Langmuir probe instrument, LAP, utilizes the multiple powers of a pair of spherical Langmuir probes for measurements of basic plasma parameters with the aim of providing detailed knowledge of the outgassing, ionization, and subsequent plasma processes around the Rosetta target comet. The fundamental plasma properties to be studied are the plasma density, the electron temperature, and the plasma flow velocity. However, study of electric fields up to 8 kHz, plasma density fluctuations, spacecraft potential, integrated UV flux, and dust impacts is also possible. LAP is fully integrated in the Rosetta Plasma Consortium (RPC), the instruments of which together provide a comprehensive characterization of the cometary plasma.

    • Therapeutic vaccination targeting self-molecules is an attractive alternative to monoclonal antibody-based therapies for cancer and various inflammatory diseases. However, development of cancer vaccines targeting self-molecules has proven difficult. One complicating factor is that tumor cells have developed strategies to escape recognition by the immune system. Antigens specifically expressed by the tumor vasculature can therefore provide alternative targets. The alternatively spliced extra domain-A and B (ED-A and ED-B) of fibronectin are expressed during vasculogenesis in the embryo, but essentially undetectable under normal conditions in the adult. However, these domains are re-expressed during tumor angiogenesis and matrix remodeling, which renders them highly interesting for targeted cancer therapies. Using the MMTV-PyMT transgenic model of metastatic mammary carcinoma, we show that tumor burden can be significantly decreased by immunization against ED-A in a therapeutic setting. Furthermore, we found that in mice carrying anti-ED-A antibodies the number of metastases was reduced. ED-A immunization increased infiltration of macrophages and compromised tumor blood vessel function. These findings implicate an attack of the tumor vasculature by the immune system, through a polyclonal antibody response. We conclude that tumor vascular antigens are promising candidates for development of therapeutic vaccines targeting growth of primary tumors as well as disseminated disease.

    • Estrogens play a crucial role in the development of breast cancer. Estradiol can be produced in the breast tissue in situ, and one of the enzymes involved in this process is 17β-hydroxysteriod dehydrogenase (17β-HSD) type 1 that catalyzes the interconversion of estrone (E1) to the biologically more potent estradiol (E2). The gene coding for 17β-HSD type 1 (HSD17B1) is located at 17q12-21, close to the more studied ERBB2 and BRCA1. The aim of this study was to investigate if HSD17B1 shows an altered gene copy number in breast cancer. We used real-time PCR and examined 221 postmenopausal breast tumors for amplification of HSD17B1 and ERBB2. In all, 32 tumors (14.5%) showed amplification of HSD17B1 and 21% were amplified for ERBB2. Amplification of the two genes was correlated (P = 0.00078) and in 14 tumors (44%) with amplification of HSD17B1, ERBB2 was co amplified. The patients with amplification in at least one of the genes had a significantly worse outcome than patients without (P = 0.0059). For estrogen receptor (ER)-positive patients who received adjuvant tamoxifen, amplification of HSD17B1 was related to decreased breast cancer survival (P = 0.017), whereas amplification of ERRB2 was not. Amplification of HSD17B1 might be an indicator of adverse prognosis among ER-positive patients, and possibly a mechanism for decreased benefit from tamoxifen treatment.
