  • Determinants of morbidity and mortality following emergency abdominal surgery in children in low-income and middle-income countries

    • Background: Child health is a key priority on the global health agenda, yet the provision of essential and emergency surgery in children is patchy in resource-poor regions. This study was aimed to determine the mortality risk for emergency abdominal paediatric surgery in low-income countries globally.Methods: Multicentre, international, prospective, cohort study. Self-selected surgical units performing emergency abdominal surgery submitted prespecified data for consecutive children aged <16 years during a 2-week period between July and December 2014. The United Nation's Human Development Index (HDI) was used to stratify countries. The main outcome measure was 30-day postoperative mortality, analysed by multilevel logistic regression.Results: This study included 1409 patients from 253 centres in 43 countries; 282 children were under 2 years of age. Among them, 265 (18.8%) were from low-HDI, 450 (31.9%) from middle-HDI and 694 (49.3%) from high-HDI countries. The most common operations performed were appendectomy, small bowel resection, pyloromyotomy and correction of intussusception. After adjustment for patient and hospital risk factors, child mortality at 30 days was significantly higher in low-HDI (adjusted OR 7.14 (95% CI 2.52 to 20.23), p<0.001) and middle-HDI (4.42 (1.44 to 13.56), p=0.009) countries compared with high-HDI countries, translating to 40 excess deaths per 1000 procedures performed.Conclusions: Adjusted mortality in children following emergency abdominal surgery may be as high as 7 times greater in low-HDI and middle-HDI countries compared with high-HDI countries. Effective provision of emergency essential surgery should be a key priority for global child health agendas.

    • Twenty years have passed since first light for the Sloan Digital Sky Survey (SDSS). Here, we release data taken by the fourth phase of SDSS (SDSS-IV) across its first three years of operation (2014 July-2017 July). This is the third data release for SDSS-IV, and the 15th from SDSS (Data Release Fifteen; DR15). New data come from MaNGA-we release 4824 data cubes, as well as the first stellar spectra in the MaNGA Stellar Library (MaStar), the first set of survey-supported analysis products (e.g., stellar and gas kinematics, emission-line and other maps) from the MaNGA Data Analysis Pipeline, and a new data visualization and access tool we call "Marvin." The next data release, DR16, will include new data from both APOGEE-2 and eBOSS; those surveys release no new data here, but we document updates and corrections to their data processing pipelines. The release is cumulative; it also includes the most recent reductions and calibrations of all data taken by SDSS since first light. In this paper, we describe the location and format of the data and tools and cite technical references describing how it was obtained and processed. The SDSS website (www.sdss.org) has also been updated, providing links to data downloads, tutorials, and examples of data use. Although SDSS-IV will continue to collect astronomical data until 2020, and will be followed by SDSS-V (2020-2025), we end this paper by describing plans to ensure the sustainability of the SDSS data archive for many years beyond the collection of data.

  • Impact of COVID-19 Lockdown in Eating Disorders : A Multicentre Collaborative International Study

    • Background. The COVID-19 lockdown has had a significant impact on mental health. Patients with eating disorders (ED) have been particularly vulnerable. Aims. (1) To explore changes in eating-related symptoms and general psychopathology during lockdown in patients with an ED from various European and Asian countries; and (2) to assess differences related to diagnostic ED subtypes, age, and geography. Methods. The sample comprised 829 participants, diagnosed with an ED according to DSM-5 criteria from specialized ED units in Europe and Asia. Participants were assessed using the COVID-19 Isolation Scale (CIES). Results. Patients with binge eating disorder (BED) experienced the highest impact on weight and ED symptoms in comparison with other ED subtypes during lockdown, whereas individuals with other specified feeding and eating disorders (OFSED) had greater deterioration in general psychological functioning than subjects with other ED subtypes. Finally, Asian and younger individuals appeared to be more resilient. Conclusions. The psychopathological changes in ED patients during the COVID-19 lockdown varied by cultural context and individual variation in age and ED diagnosis. Clinical services may need to target preventive measures and adapt therapeutic approaches for the most vulnerable patients.

    • Although optical components in Fourier transform infrared (FTIR) spectrometers are preferably wedged, in practice, infrared spectra typically suffer from the effects of optical resonances ("channeling") affecting the retrieval of weakly absorbing gases. This study investigates the level of channeling of each FTIR spectrometer within the Network for the Detection of Atmospheric Composition Change (NDACC). Dedicated spectra were recorded by more than 20 NDACC FTIR spectrometers using a laboratory mid-infrared source and two detectors. In the indium antimonide (InSb) detector domain (1900-5000 cm-1), we found that the amplitude of the most pronounced channeling frequency amounts to 0.1 ‰ to 2.0 ‰ of the spectral background level, with a mean of (0:68±0:48) ‰ and a median of 0.60 ‰. In the mercury cadmium telluride (HgCdTe) detector domain (700-1300 cm-1), we find even stronger effects, with the largest amplitude ranging from 0.3 ‰ to 21 ‰ with a mean of (2:45±4:50) ‰ and a median of 1.2 ‰. For both detectors, the leading channeling frequencies are 0.9 and 0.11 or 0.23 cm-1 in most spectrometers. The observed spectral frequencies of 0.11 and 0.23 cm-1 correspond to the optical thickness of the beam splitter substrate. The 0.9 cm-1 channeling is caused by the air gap in between the beam splitter and compensator plate. Since the air gap is a significant source of channeling and the corresponding amplitude differs strongly between spectrometers, we propose new beam splitters with the wedge of the air gap increased to at least 0.8. We tested the insertion of spacers in a beam splitter's air gap to demonstrate that increasing the wedge of the air gap decreases the 0.9 cm-1 channeling amplitude significantly. A wedge of the air gap of 0.8 reduces the channeling amplitude by about 50 %, while a wedge of about 2 removes the 0.9 cm-1 channeling completely. This study shows the potential for reducing channeling in the FTIR spectrometers operated by the NDACC, thereby increasing the quality of recorded spectra across the network.

    • The emergence of novel pervasive networks that consist of tiny embedded nodes have reduced the gap between real and virtual worlds. This paradigm has opened the Service Cloud to a variety of wireless devices especially those with sensorial and actuating capabilities. Those pervasive networks contribute to build new context-aware applications that interpret the state of the physical world at real-time. However, traditional Service-Oriented Architectures (SOA), which are widely used in the current Internet are unsuitable for such resource-constraint devices since they are too heavy. In this research paper, an internetworking approach is proposed in order to address that important issue. The main part of our proposal is the Knowledge-Aware and Service-Oriented (KASO) Middleware that has been designed for pervasive embedded networks. KASO Middleware implements a diversity of mechanisms, services and protocols which enable developers and business processing designers to deploy, expose, discover, compose, and orchestrate real-world services (i.e. services running on sensor/actuator devices). Moreover, KASO Middleware implements endpoints to offer those services to the Cloud in a REST manner. Our internetworking approach has been validated through a real healthcare telemonitoring system deployed in a sanatorium. The validation tests show that KASO Middleware successfully brings pervasive embedded networks to the Service Cloud.

    • Many applications and services have emerged in the frame of new Internet of Things paradigm. This novel view has opened the Web services to a variety of devices especially to tiny and resource-constrained devices. Wireless Sensor and Actuator Networks belong to that kind of devices. Those networks have become one of the more promising technologies to take part in the Future Internet. However, the integration of Sensor and Actuator Networks into the Service Cloud is a hard challenge requiring specific new architectures and protocols. This paper presents a middleware approach addressing this important issue. A Knowledge-Aware and Service-Oriented Middleware (KASOM) for pervasive embedded networks is proposed. The major aim of KASOM is to offer advanced and enriched pervasive services to everyone connected to Internet. In this sense, KASOM implements mechanisms and protocols which allow managing the knowledge generated in pervasive embedded networks in order to expose it to Internet users in a readable way. General functional requirements of embedded sensor and actuator platforms have been taken into account when designing KASOM, with special attention in energy consumption, memory and bandwidth. The KASOM evaluation and validation will be demonstrated through a real Wireless Sensor and Actuator Network deployment based on integral healthcare services in a sanatorium.

    • The increase in physical resources for the next-generation of embedded computing devices, as well as in the efforts carried out by the scientific and research communities, is paving the way for Smart Infrastructures based on Wireless Sensor Networks. In such manner, not only open and formally defined Service-Oriented Frameworks are required, but also efficient middleware technologies that ease the development of new sensor-based services. From the cornerstone analysis of pervasive computing principles applied to Smart Spaces creation, this article presents the μSMS (micro Subscription Management System) middleware. This approach specifies and develops the notion of virtual sensor services created for Smart Environments over sensor networks from tiny in-network services based on agent technology. In the framework of the μSWN European Research Project, this architecture has been validated in a Smart Hospital real-world scenario using a healthcare virtual sensor service. Considering such purpose, medical status monitoring, location tracking and perimeter surveillance agent-based services have been developed. This study is concluded by a comparative analysis of the system considering memory overhead, packet delivery ratio, average end-to-end delay and battery lifetime as evaluation metrics. The results show a lightweight middleware implementation with good overall system and network performance.

  • Insights into the accuracy of social scientists' forecasts of societal change

    • How well can social scientists predict societal change, and what processes underlie their predictions? To answer these questions, we ran two forecasting tournaments testing the accuracy of predictions of societal change in domains commonly studied in the social sciences: ideological preferences, political polarization, life satisfaction, sentiment on social media, and gender-career and racial bias. After we provided them with historical trend data on the relevant domain, social scientists submitted pre-registered monthly forecasts for a year (Tournament 1; N = 86 teams and 359 forecasts), with an opportunity to update forecasts on the basis of new data six months later (Tournament 2; N = 120 teams and 546 forecasts). Benchmarking forecasting accuracy revealed that social scientists' forecasts were on average no more accurate than those of simple statistical models (historical means, random walks or linear regressions) or the aggregate forecasts of a sample from the general public (N = 802). However, scientists were more accurate if they had scientific expertise in a prediction domain, were interdisciplinary, used simpler models and based predictions on prior data. How accurate are social scientists in predicting societal change, and what processes underlie their predictions? Grossmann et al. report the findings of two forecasting tournaments. Social scientists' forecasts were on average no more accurate than those of simple statistical models.

  • Enhanced survival prediction using explainable artificial intelligence in heart transplantation

    • The most limiting factor in heart transplantation is the lack of donor organs. With enhanced prediction of outcome, it may be possible to increase the life-years from the organs that become available. Applications of machine learning to tabular data, typical of clinical decision support, pose the practical question of interpretation, which has technical and potential ethical implications. In particular, there is an issue of principle about the predictability of complex data and whether this is inherent in the data or strongly dependent on the choice of machine learning model, leading to the so-called accuracy-interpretability trade-off. We model 1-year mortality in heart transplantation data with a self-explaining neural network, which is benchmarked against a deep learning model on the same development data, in an external validation study with two data sets: (1) UNOS transplants in 2017–2018 (n = 4750) for which the self-explaining and deep learning models are comparable in their AUROC 0.628 [0.602,0.654] cf. 0.635 [0.609,0.662] and (2) Scandinavian transplants during 1997–2018 (n = 2293), showing good calibration with AUROCs of 0.626 [0.588,0.665] and 0.634 [0.570, 0.698], respectively, with and without missing data (n = 982). This shows that for tabular data, predictive models can be transparent and capture important nonlinearities, retaining full predictive performance.

    • Providing necessary background for provisioning of a new generation of enriched services over Wireless Sensor Networks is the main effort that the scientific community is currently carrying out. These services have improved a great number of aspects related to pervasive systems such as saving resources, efficiency, reliability, scalability and low power consumption. In this paper, μSMS middleware, using an event-based service model, is presented. This novel approach makes up the design requirements previously mentioned by implementing a dynamic memory kernel and a variable payload multiplexing mechanism for the information events in order to provide advanced services. The results obtained over real-world deployments, especially those related with provision of e-Health services, reflect a significant improvement over other similar proposals, such as the RUNES approach: 50% lower memory overhead, 53% lower software components load time and 12% lower event's propagation time.

    • A wireless sensor network (WSN) is a wireless network composed of spatially distributed and tiny autonomous nodes - smart dust sensors, motes - which cooperatively monitor physical or environmental conditions. Nowadays these kinds of networks support a wide range of applications, such as target tracking, security, environmental control, habitat monitoring, source detection, source localization, vehicular and traffic monitoring, health monitoring, building and industrial monitoring, etc. Generally, these applications have strong and strict requirements for end-to-end delaying and loosing during data transmissions. In this paper, we propose a realistic scenario for application of the WSN field in order to illustrate selection of an appropriate approach for guaranteeing performance in a WSN-deployed application. The methodology we have used includes four major phases: 1) Requirements analysis of the application scenario; 2) QoS modeling in different layers of the communications protocol stack and selection of more suitable QoS protocols and mechanisms; 3) Definition of a simulation model based on an application scenario, to which we applied the protocols and mechanisms selected in the phase 2; and 4) Validation of decisions by means of simulation and analysis of results.     • Nowadays, proliferation of embedded systems is enhancing the possibilities of gathering information by using wireless sensor networks (WSNs). Flexibility and ease of installation make these kinds of pervasive networks suitable for security and surveillance environments. Moreover, the risk for humans to be exposed to these functions is minimized when using these networks. In this paper, a virtual perimeter surveillance agent, which has been designed to detect any person crossing an invisible barrier around a marked perimeter and send an alarm notification to the security staff, is presented. This agent works in a state of 'low power consumption' until there is a crossing on the perimeter. In our approach, the 'intelligence' of the agent has been distributed by using mobile nodes in order to discern the cause of the event of presence. This feature contributes to saving both processing resources and power consumption since the required code that detects presence is the only system installed. The research work described in this paper illustrates our experience in the development of a surveillance system using WNSs for a practical application as well as its evaluation in real-world deployments. This mechanism plays an important role in providing confidence in ensuring safety to our environment.

Finally, taking into account some particularities of the studied MAC- andnetwork-layer protocols, we have selected a real application scenario in order to show how to choose an appropriate approach for guaranteeing performance in a WSN deployed application.

    • The Knowledge and Information Society development has promoted the evolution of telematic services, such as those for e-govemment and e-health, which aim to offer feasible solutions to social and citizens problems that will strengthen the democracy, foster citizens equal opportunities and ease their participation in the processes of the public administration and the help to the needy. Though the different telematic services are internally constituted for similar or even identical subservices, the interoperability is not possible among them and components sharing either. In order to encourage the development of the above mentioned type of services, the present article proposes an architecture that contributes to the interoperability and sharing of subservices. This architecture derives from the study of two scenarios, one of e-health and other one of e-goverment, with similar needs of security for the user' or elements authentication and the operations authorization.

    • The Sentinel-5 Precursor (S5P) mission with the TROPOspheric Monitoring Instrument (TROPOMI) on board has been measuring solar radiation backscattered by the Earth's atmosphere and surface since its launch on 13 October 2017. In this paper, we present for the first time the S5P operational methane (CH4) and carbon monoxide (CO) products' validation results covering a period of about 3 years using global Total Carbon Column Observing Network (TCCON) and Infrared Working Group of the Network for the Detection of Atmospheric Composition Change (NDACC-IRWG) network data, accounting for a priori alignment and smoothing uncertainties in the validation, and testing the sensitivity of validation results towards the application of advanced co-location criteria. We found that the S5P standard and bias-corrected CH4 data over land surface for the recommended quality filtering fulfil the mission requirements. The systematic difference of the bias-corrected total column-averaged dry air mole fraction of methane (XCH4) data with respect to TCCON data is -0.26 +/- 0.56 % in comparison to -0.68 +/- 0.74 % for the standard XCH4 data, with a correlation of 0.6 for most stations. The bias shows a seasonal dependence. We found that the S5P CO data over all surfaces for the recommended quality filtering generally fulfil the missions requirements, with a few exceptions, which are mostly due to co-location mismatches and limited availability of data. The systematic difference between the S5P total column-averaged dry air mole fraction of carbon monoxide (XCO) and the TCCON data is on average 9.22 +/- 3.45 % (standard TCCON XCO) and 2.45 +/- 3.38 % (unscaled TCCON XCO). We found that the systematic difference between the S5P CO column and NDACC CO column (excluding two outlier stations) is on average 6.5 +/- 3.54 %. We found a correlation of above 0.9 for most TCCON and NDACC stations. The study shows the high quality of S5P CH4 and CO data by validating the products against reference global TCCON and NDACC stations covering a wide range of latitudinal bands, atmospheric conditions and surface conditions.
