    • P>Objectives To evaluate the extent of bone fill over 3 years following the surgical treatment of peri-implantitis with bone grafting with or without a membrane. Material and Methods In a non-submerged wound-healing mode, 15 subjects with 27 implants were treated with a bone substitute (Algipore (R)) alone and 17 subjects with 29 implants were treated with the bone substitute and a resorbable membrane (Osseoquest (R)). Implants with radiographic bone loss >= 1.8 mm following the first year in function and with bleeding and/or pus on probing were included. Following surgery, subjects were given systemic antibiotics (10 days) and rinsed with chlorhexidine. After initial healing, the subjects were enrolled in a strict maintenance programme. Results Statistical analysis failed to demonstrate changes in bone fill between 1 and 3 years both between and within procedure groups. The mean defect fill at 3 years was 1.3 +/- (SD) 1.3 mm if treated with the bone substitute alone and 1.6 +/- (SD) 1.2 mm if treated with an adjunct resorbable membrane, (p=0.40). The plaque index decreased from approximately 40-10%, remaining stable during the following 2 years. Conclusion Defect fill using a bone substitute with or without a membrane technique in the treatment of peri-implantitis can be maintained over 3 years.

  • Performance of sulfhydryl boron hydride in patients with grade III and IV astrocytoma: a basis for boron neutron capture therapy

    • This study investigated the rationale of boron neutron capture therapy (BNCT) for the treatment of Grade III and IV astrocytoma. The European Community joint research program on BNCT plans to use sulfhydryl boron hydride (BSH) in clinical trials. The work presented here, examines the performance of BSH in eight patients with Grade III and IV astrocytoma using a measurement technique which precisely correlates the boron uptake with the histology of the tumor and the peritumoral brain. Astrocytomas are exceptionally heterogeneous and spread migrating tumor cells into the surrounding brain. The patients were infused with 50 mg BSH per kilogram of body weight at 12, 18, 24 or 48 hours before surgery. At the time of operation, specimens were obtained of the tumor, skin, muscle, dura, blood, urine, and, when surgically possible, the brain adjacent to tumor. In three patients the intracellular boron distribution was investigated by subcellular fractionation. The blood clearance was biphasic with half-lives of 0.6 and 8.2 hours. After 3 days, approximately 70% of the dose injected was excreted in the urine. The maximum boron concentration in the tumor was 20 ppm, 12 hours after the infusion. The tumor-to-blood ratios ranged between 0.2 and 1.4, with the highest values after 18 to 24 hours. In the brain specimens the boron concentration never exceeded 1 ppm. This work confirms a selective uptake of boron in the tumor compared to the surrounding brain and that boron, to some extent, is incorporated in the tumor cells.

  • The Swedish CArdioPulmonary BioImage Study : objectives and design

    • Cardiopulmonary diseases are major causes of death worldwide, but currently recommended strategies for diagnosis and prevention may be outdated because of recent changes in risk factor patterns. The Swedish CArdioPulmonarybioImage Study (SCAPIS) combines the use of new imaging technologies, advances in large-scale 'omics' and epidemiological analyses to extensively characterize a Swedish cohort of 30 000 men and women aged between 50 and 64 years. The information obtained will be used to improve risk prediction of cardiopulmonary diseases and optimize the ability to study disease mechanisms. A comprehensive pilot study in 1111 individuals, which was completed in 2012, demonstrated the feasibility and financial and ethical consequences of SCAPIS. Recruitment to the national, multicentre study has recently started.

  • Weight monitoring in patients with severe heart failure (WISH). A randomized controlled trial.

    • AIMS: This multicentre, randomized controlled trial hypothesized that daily electronic transmission of body weight to a heart failure (HF) clinic will reduce cardiac hospitalization in patients recently hospitalized with HF.METHODS AND RESULTS: A total of 344 patients were randomized to either an intervention group (IG) or a control group (CG). Of the 319 patients included in the final analysis, the mean age was 73 years (SD 10.2), 75% were males, and 57% had a left ventricular ejection fraction (LVEF) <30%. Patients in both groups were recommended to weigh themselves daily and, in the case of sudden weight gain >2 kg in 3 days, to contact the HF clinic. Patients in the IG were given an electronic scale and the weight was automatically transmitted to and monitored at the HF clinic. No significant differences were found for the primary endpoint, cardiac re-hospitalization [70/153 CG, 70/166 IG; hazard ratio (HR) 0.90, 95% confidence interval (CI) 0.65-1.26, P = 0.54], or for the secondary endpoints, which included all-cause hospitalization (84/153 CG, 79/166 IG; HR 0.83, 95% CI 0.61-1.13, P = 0.24), death from any cause (8/153 CG, 5/166 IG; HR 0.57, 95% CI 0.19-1.73, P = 0.32), or the composite endpoint of cardiac hospitalization and death from any cause (78/153 CG, 75/166 IG; HR 0.90, 95% CI 0.65-1.26, P = 0.54). Subgroup analyses did not show any benefits for patients in the IG despite their more frequent monitoring; 398 occasions compared with 30 occasions in the CG.CONCLUSION: Daily electronic transmission of body weight and monitoring three times a week did not decrease hospitalization or death in HF patients followed up at a HF clinic.

  • Tillgänglighet, närhet och synlighet – gemensamma utmaningar för LUB-nätverket för att möta forskares behov av stöd vid Lunds universitet

    • This report presents a project at Lund University Libraries (LUB) during the fall of 2012, with the object to identify areas where the present research support services ought to be developed and strengthened. The project is three folded and contains a) a literature review focusing on definitions and examples of research support services as well as researchers’ needs and experiences of support services, b) a survey to get an overview of the support services offered today at LUB, the decentralized library network at Lund University, and c) focus group interviews with researchers, one group from each partaking faculty, to investigate key obstacles in their research processes. In all three parts a model of the research process schematically divided into four phases was applied; to start a project, to collect material, to process/analyze/write and to communicate the results. The first two stages of the project, the literature overview and the survey, form a background and both show that many support services are concentrated around the phases to collect material and to communicate results. The findings from the focus group interviews point to common needs and issues as well as special needs and issues related to different ways of doing and communicating research. Common for most researchers is limited time, a need for funding and an everyday fragmentation. They talk about the importance of networks, their efforts to navigate in an abundance of material and ways to find it and a need for overviews and help to understand new ways and demands to communicate. Most concern is expressed in the phases to start a project, to collect material and to communicate results. A major conclusion is that the research support services need to be available, visible and in close proximity to the researchers. Further, the development of support services needs to take into consideration both common traits as well as local variations in research practices. Therefore, discussions concerning LUB as a whole, need to be related to discussions at the single faculty libraries. Apart from knowledge about how to address the research support services at LUB, the project generates a model for how librarians can understand the research community that they are working with and with whom to create networks and partnerships for the future.

    • The octakis(DMSO) (DMSO = dimethylsulfoxide) neodymium(III), samarium(III), gadolinium(III), dysprosium(III), erbium(III), and lutetium(III) iodides crystallize in the monoclinic space group P21/n (No. 14) with Z = 4, while the octakis(DMSO) iodides of the larger lanthanum(III), cerium(III), and praseodymium(III) ions crystallize in the orthorhombic space group Pbca (No. 61), Z = 8. In all [Ln(OS(Me2)8]I3 compounds the lanthanoid(III) ions coordinate eight DMSO oxygen atoms in a distorted square antiprism. Up to three of the DMSO ligands were found to be disordered and were described by two alternative configurations related by a twist around the metal−oxygen (Ln−O) bond. To resolve the atomic positions and achieve reliable Ln−O bond distances, complete semirigid DMSO molecules with restrained geometry and partial occupancy were refined for the alternative sites. This disorder model was also applied on previously collected data for the monoclinic octakis(DMSO)yttrium(III) iodide. At ambient temperature, the eight Ln−O bond distances are distributed over a range of about 0.1 Å. The average value increases from Ln−O 2.30, 2.34, 2.34, 2.36, 2.38, 2.40 to 2.43 Å (Ln = Lu, Er, Y, Dy, Gd, Sm, and Nd) for the monoclinic [Ln(OSMe2)8]I3 structures, and from 2.44, 2.47 to 2.49 Å (Ln = Pr, Ce, and La) for the orthorhombic structures, respectively. The average of the La−O and Nd−O bond distances remained unchanged at 100 K, 2.49 and 2.43 Å, respectively. Despite longer bond distances and larger Ln−O−S angles, the cell volumes are smaller for the orthorhombic structures (Ln = Pr, Ce, and La) than for the monoclinic structure with Ln = Nd, showing a more efficient packing arrangement. Raman and IR absorption spectra for the [Ln(OS(CH3)2)8]I3 (Ln = La, Ce, Pr, Nd, Gd, Tb, Dy, Er, Lu, and Y) compounds, also deuterated for La and Y, have been recorded and analyzed by means of normal coordinate methods. The force constants for the Ln−O and S−O stretching modes in the complexes increase with decreasing Ln−O bond distance and show increasing polarization of the bonds for the smaller and heavier lanthanoid(III) ions.

Den skiljer sig också kraftigt mellan länder och mellan olika grupper inom länderna.Sex dödsorsaker står för mer än 90 procent av alla dödsfall före 5 års ålder: neonatal mortalitet, lunginflammation, diarré, ­malaria, mässling och HIV/aids. ­Undernäring beräknas vara ­delorsak till cirka en tredjedel av dessa dödsfall.Vi har kunskap och metoder att med kostnadseffektiva lösningar reducera barnadödligheten med två tredjedelar. Fortsatt inter­nationellt samarbete, utökade ­resurser samt lokal, nationell po­litisk vilja krävs för att lyckas.

  • Implementation of evidence-based assessment of upper extremity in stroke rehabilitation: From evidence to clinical practice

    • Objective: There is an evidence-practice gap in assessment of the upper extremities during acute and subacute stroke rehabilitation. The aim of this study was to target this gap by describing and evaluating the implementation of, and adherence to, an evidence-based clinical practice guideline for occupational therapists and physiotherapists. Methods: The upper extremity assessment implementation process at Sahlgrenska University Hospital comprised 5 stages: mapping clinical practice, identifying evidence-based outcome measures, development of a guideline, implementation, and evaluation. A systematic theoretical framework was used to guide and facilitate the implementation process. A survey, answered by 44 clinicians (23 physiotherapists and 21 occupational therapists), was used for evaluation. Results: The guideline includes 6 primary standardized assessments (Shoulder Abduction, Finger Extension (SAFE), 2 items of the Actions Research Arm Test (ARAT-2), Fugl-Meyer Assessment of Upper Extremity (FMA-UE), Box and Block Test (BBT), 9-Hole Peg Test (9HPT), and grip strength (Jamar hand dynamometer)) performed at specified time-points post-stroke. More than 80% (35 to 42) clinicians reported reported being content with the guideline and the implementation process. Approximately 60-90% of the clinicians reported good adherence to specific assessments, and approximately 50% reported good adherence to the agreed time-points. Comprehensive scales were more difficult to implement compared with the shorter screening scales. High levels of work rotation among staff, and the need to prioritize other assessments during the first week after stroke, hindered to implementation. Conclusion: The robustness of evidence, adequate support and receptive context facilitated the implementation process. The guideline enables a more structured, knowledge-based and consistent assessment, and thereby supports clinical decision-making and patient involvement.

    • Hemicelluloses constitute one of the most abundant renewable resources on earth. To increase their utilization, the isolation of hemicelluloses from industrial biomass side-streams would be beneficial. A method was investigated to isolate hemicelluloses from process water from a thermomechanical pulp mill. The method consists of three steps: removal of solids by microfiltration, preconcentration of the hemicelluloses by ultrafiltration, and purification by either size-exclusion chromatography (SEC) or diafiltration. The purpose of the final purification step is to separate hemicelluloses from small oligosaccharides, monosaccharides, and salts. The ratio between galactose, glucose, and mannose in oligo- and polysaccharides after preconcentration was 0.8: 1: 2.8, which is similar to that found in galactoglucomannan. Continuous diafiltration was performed using a composite fluoro polymer membrane with cutoff of 1000 Da. After diafiltration with four diavolumes the purity of the hemicelluloses was 77% (gram oligo- and polysaccharides/gram total dissolved solids) and the recovery was 87%. Purification by SEC was performed with 5, 20, and 40% sample loadings, respectively and a flow rate of 12 or 25 mL/min (9 or 19 cm/h). The purity of hemicelluloses after SEC was approx 82%, and the recovery was above 99%. The optimal sample load and flow rate were 20% and 25 mL/min, respectively. The process water from thermomechanical pulping of spruce is inexpensive. Thus, the recovery of hemicelluloses is not of main importance. If the purity of 77%, obtained with diafiltration, is sufficient for the utilization of the hemicelluloses, diafiltration probably offers a less expensive alternative in this application.

    • PURPOSE: To investigate heterotropia, heterophoria, head posture, nystagmus, stereo acuity, ocular motility and near point of convergence (NPC) in children with hydrocephalus treated surgically before 1 year of age. In addition, the effects of being born with hydrocephalus, the effect of the etiology of hydrocephalus, number of shunt revisions and the size of the ventricles on these variables were studied. METHODS: A population-based study was performed in 75 children and the results were compared with the results of an age- and sex-matched group (comp group) (n = 140). RESULTS: Heterotropia 68.9% (comp group 3.6%; p < 0.001), abnormal head posture 41.3% (comp group 0; p < 0.001), nystagmus 44.0% (comp group 0; p < 0.001), stereo acuity < or =60'' 33.8% (comp group 97.1%; p < 0.001) and ocular motility defects 69.7% (comp group 0.7%; p < 0.001) were more common among children with hydrocephalus than in the comparison group. Children with overt hydrocephalus at birth had significantly more heterotropia (p = 0.0006), esotropia (p = 0.002), abnormal head posture (p = 0.02) and motility defects (p = 0.003) compared to those with hydrocephalus developing during the first year of life. The etiology, number of shunt revisions and the size of the ventricles had no significant effect on any of the investigated variables. CONCLUSIONS: Children with hydrocephalus surgically treated before the age of one year commonly present orthoptic abnormalities. The etiology of hydrocephalus, number of shunt revisions and ventricle size seem to be of minor importance compared with the age of onset of hydrocephalus with regard to the risk for orthoptic abnormalities.

    • Hästflickan i rörelse: Generationsberättelser om hästboksläsningI vår forskning om hästböcker har vi kommit att intressera oss också för hästboksläsarna. I syfte att få kunskap om varför människor läser hästböcker har vi intervjuat cirka 60 unga och vuxna. Ett delresultat utifrån intervjuer med flickor 9-12 och i vissa fall deras (ibland hästintresserade) föräldrar presenteras. Där framkommer att flickorna i många fall är bokslukare, om än tämligen genretrogna. Ett tydligt samband mellan identiteten som hästtjej och läsare framträder, då läsningen ofta har spin-off-effekter på lekmönster. Orsakerna bakom hästboksläsningen handlar om identifikation i termer av ett flickskap som i stallkulturen står utanför normen och som präglas av mod och beslutsamhet, men där även spänning och kunskapsinhämtning nämns. Hästbokens status har höjts i både skolan och på bibliotek, där den tidigare har haft dåligt rykte, något som kan ses som att större uppmärksamhet ägnas åt något som intresserar många flickor.

    • This paper focuses on horse stories as depictions of identity formation withinequestrian cultures. The material used is Pia Hagmar’s three books on Millan,published 2012-2014. In these stories, the construction of girlhood interconnectswith the becoming of a person used to handle the large and potentially dangerousanimal, which is the horse. In Susanna Hedenborg´s study of the horse story genre(2013), she shows the image of a non-traditional girl, used to hard and dirty work,but also a classical fearful and shy girl outside of the interaction with horses. InHelen Asklund’s study, the investigation uncovers a construction of girlhood incontrast with antagonistic characters (Asklund 2013). This paper uses anintersectional perspective to explore how the main characters in Pia Hagmar’sseries about Millan and their identities are shaped by their interaction with bothhorses and fellow humans of both sexes within equine cultures, a milieu whichallows re-negotiations of what being a girl implies. Adults are positively portrayedas caring providers of knowledge about horses or as facilitators of the ridinginterest through financial support, or negatively as dysfunctional persons unable toact as adults. Boys of similar age and with an interest in horses are portrayed asgood friends, sometimes more, but the girls are, first and foremost, interested inhorses. Between girl characters, there are often conflicts and jealousy. The studyaims at mapping out the formation of identity by highlighting power asymmetriessuch as gender, age, species, class and sexuality.

    • This article analyzes how the horse story setting problematizes the conception of the good mother and normative mothering, by elucidating how the gazes of the young main characters on their mothers function in relation to the notion of the good mother, how this influences the mothering and how the mothering characters challenge this notion. Mothering is seen here as caring practices, which can be performed by people other than mothers. The analysis of three horse story series by Pia Hagmar, the books on Klara (1999–2008), the books on Millan (2012–2014) and the books on Juli (2019–2021), shows that the series depict motherhood and mothering at the intersection between the conception of the good mother and the good horse person. The common denominator is nurturing traits that make personal needs take the back seat – for the traditional good mother to the benefit ofher children, for the good horse person to the benefit of the horse. Hagmar normalizes the presence of several mothering characters in addition to their mothers around the horse girls. Furthermore, these characters, who function as mothering variations, contribute to the creation of a solid caring environment, reducing the demands on mothers who try to be everything to their children. Although the daughters’ views of their mothers are sometimes critical, Hagmar allows the mothers to be imperfect, partly incompetent, or simply women with their own agency, interests and needs outside of motherhood. In addition, a mother-daughter relationship built on a common interest in horses allows for a friendship with a common focus – the horse.

    • This dissertation examines the motif of the popular Viking in contemporary Anglophone fantasy literature, with a focus on masculinity, power, embodiment,and sexuality. The study draws on queer-theoretical perspectives on masculinity and the method of queer reading, and approaches the Viking as at once bound up with the legitimization of normative and hegemonic forms of masculinity and open to (queer) negotiations and possibilities beyond normative male masculinities.The material consists of contemporary gritty fantasy, a recent subgenre deeply invested in contemporary concerns regarding masculinity, masculine failure, and masculinity crisis narratives, where the Viking motif plays a major role. The texts under consideration are Joe Abercrombie’s The First Law (2006–2012) and The Shattered Sea (2014–2015), Richard K. Morgan’s A Land Fit for Heroes (2008–2016), Mark Lawrence’s The Red Queen’s War (2014–2016), and Elizabeth Bear and Sarah Monette’s The Iskryne Saga (2007–2015).Understanding the Viking as a motif that is intractably bound up with ideas of the past and the historical period of the Viking Age but not reducible to it, the thesis considers the fantasy Viking as a medial representation of spectacular hardbody action masculinity and puts it in relation to the fantasy text and fantasy worldbuilding as well as more generalized cultural ideas of the North and the Nordics. Furthermore, it asks how we can understand the masculinity of the Viking – long made symbolic of or associated with white supremacy, misogyny, homophobia, and reactionary gender roles – beyond an assumed direct relation to men or men’s concerns.Analytically, the thesis considers the Viking in relation to spatiality, temporality, and embodiment, finding that in the fantasy text, the Viking emerges with a strong focus on a mighty, muscular body and as a barbarian Other connected to the past and in direct opposition to civilization and futurity, making it an escapist possibility outside the disciplining power of neoliberal late-stage capitalism. Furthermore, connecting to postfeminist perspectives on masculinity in media, the thesis finds that the fantasy Viking has developed in ways that seemingly take into account feminist and queer critique of traditional, homophobic forms of masculinity, transforming the Viking and offering it up for (queer) objectification. At the same time, the Viking also becomes a safe site of traditional masculinity, where anxieties and concerns regarding a supposed loss of male power in modernity can be projected and ultimately resolved.

Socialstyrelsen har även tagit fram och testat metoder för förvaltning och utveckling av det nationella fackspråket i sin helhet.     • Meso-tetra(hydroxyphenyl)chlorin (mTHPC) (INN: Temoporfin) is a potent photodynamically active substance in clinical use today. Usually, the substance is given systemically and a known drawback with this administration route is a prolonged skin light sensitization. For the first time to our knowledge, a liposomal Temoporfin gel formulation for topical application was studied in connection with photodynamic therapy (PDT) of nonpigmented skin malignancies in humans. Intervals of 4 hr between drug administration and light irradiation were used. Sensitizer distribution within tumor and surrounding normal skin was investigated by means of point monitoring and imaging fluorescence spectroscopy before, during, and after PDT, showing high tumor selectivity. Furthermore, the bleaching of Temoporfin was studied during the PDT procedure by monitoring the fluorescence following excitation by using a therapeutic light. A 30−35% light-induced photometabolization was shown. No pain occurred during or after treatment. It was also observed that the treated area did not show any swollen tissue or reddening, as is often seen in PDT using topical δ-aminolevulinic acid. On controlling the patients one week after treatment, healing progress was observed in several patients and no complications were registered.

Both chemically uniform InAs wires as well as InAs wires containing one or several heterostructure interfaces were grown. The interfaces were characterized in a transmission electron microscope revealing atomically sharp interfaces and also that, barriers as thin as only 2-3 monolayers and up to several hundred nanometer could be inserted into homogenous InAs wires. Further more, electrical measurements on both homogenous wires and wires containing heterostructures have been performed to investigate the functionality of nanowire based device elements

In the development of quantum nanoelectronics, one-dimensional (1D) heterostructure devices are receiving a lot of interest. We report here functional 1D resonant tunneling diodes obtained via bottom-up assembly of designed segments of different semiconductor materials in III/V nanowires. The emitter, collector, and the central quantum dot are made from InAs and the barrier material from InP. Ideal resonant tunneling behavior, with peak-to-valley ratios of up to 50:1 and current densities of 1 nA/mum(2) was observed at low temperatures. (C) 2002 American Institute of Physics.

    • We report on the growth of designed heterostructures placed within semiconductor nanowhiskers, exemplified by the InAs/InP material system. Based on transmission electron microscopy, we deduce the interfaces between InAs and InP to be atomically sharp. Electrical measurements of thermionic emission across an 80-nm-wide InP heterobarrier, positioned inside InAs whiskers 40 nm in diameter, yield a barrier height of 0.6 eV. On the basis of these results, we propose new branches of physics phenomena as well as new families of device structures that will now be possible to realize and explore.

    • We report growth of one-dimensional semiconductor nanocrystals, nanowhiskers, in which segments of the whisker with different composition are formed, illustrated by InAs whiskers containing segments of InP. Our conditions for growth allow the formation of abrupt interfaces and heterostructure barriers of thickness from a few monolayers to 100s of nanometers, thus creating a one-dimensional landscape along which the electrons move. The crystalline perfection, the quality of the interfaces, and the variation in the lattice constant are demonstrated by high-resolution transmission electron microscopy, and the conduction band off-set of 0.6 eV is deduced from the current due to thermal excitation of electrons over an InP barrier.

    • Dual-energy computer tomography (DECT) can detect post-traumatic bone marrow lesions. Prospective studies of the knee with large numbers of participants and intra-observer agreement assessment are limited. Purpose To investigate the diagnostic accuracy of DECT in detecting bone marrow lesions as well as estimating the bone marrow lesion volume in patients with suspected anterior cruciate ligament trauma with magnetic resonance imaging (MRI) as reference standard. Material and Methods Forty-eight consecutive patients with suspected anterior cruciate ligament injury were imaged bilaterally with DECT within a mean of 25 days (range 4–55 days) following injury and MRI within seven days of DECT. Two readers analyzed DECT virtual non-calcium-blinded images. Consensus MRI was reference standard. Intra- and inter-observer agreement were determined using weighted kappa statistics. Sensitivity, specificity, and negative and positive predictive values were calculated. Bone marrow lesion volumes were measured; for comparison, intra-class correlation coefficient was used. Results The 48 patients (26 men, 22 women; mean age 23 years, age range 15–37 years) were imaged bilaterally yielding 52 knees with bone marrow lesions, of which 44 were in the femur and 41 were in the tibia. Intra- and inter-observer agreement to detect bone marrow lesions was moderate and fair to moderate (κ 0.54–0.66, 95% confidence interval [CI] 0.39–0.80 and 0.37–0.41, 95% CI 0.20–0.57) and overall sensitivity and specificity were 70.1% and 69.1%, respectively. Positive and negative predictive values were 72.9% and 66.1%, respectively. Bone marrow lesion volumes showed excellent intra- and inter-observer agreement (0.83–0.91, 95% CI 0.74–0.94 and 0.76–0.78, 95% CI 0.57–0.87). Conclusion The diagnostic performance of DECT to detect bone marrow lesions in the subacutely injured knee was moderate with intra- and inter-observer agreement ranging from moderate to substantial and fair to moderate. Bone marrow lesion volume correlation was excellent.

    • Background: CT is often used for fracture evaluation following knee trauma and to diagnose ACL injuries would also be valuable. Purpose: To investigate the diagnostic accuracy of dual energy CT (DECT) for detection of ACL tears in acute and subacute knee injuries. Material and Methods: Patients with suspected ACL injury were imaged with DECT and MRI. Clinically blinded DECT images were independently read twice by two radiologists. ACL was classified as normal or abnormal. Arthroscopy served as reference method. Sensitivity and positive predictive value (PPV) were calculated, and diagnostic performance between DECT and MRI was assessed. Results: 48 patients (26 M, 22 F, mean age 23 years, range 15-37 years) were imaged with a mean of 25 days following trauma. Of these, 21 patients underwent arthroscopy with a mean of 195 days after trauma. Arthroscopy revealed 19 ACL tears and 2 ACLs with no tear. The combined sensitivity was 76.3% (95% CI 66.8-85.9) and 86.8 (95% CI 71.9-95.6) for DECT and MRI, respectively. There was no statistically significant difference between these two methods (p = .223). The positive predictive value (PPV) was 93.5 (95% CI 84.3-98.2) and 91.7 (95% CI 77.5-98.3) for DECT and MRI, respectively. Conclusion: DECT has lower sensitivity to detect an ACL rupture than MRI, but the difference is not statistically significant. The PPV is high in both methods.

    • Purpose: To find the optimal imaging parameters for a photon-counting detector CT (PCD-CT) and to compare it to an energy-integrating detector CT (EID-CT) in terms of image quality and metal artefact severity using a metal-containing bovine knee specimen. Methods: A bovine knee with a stainless-steel plate and screws was imaged in a whole-body research PCD-CT at 120 kV and 140 kV and in an EID dual-source CT (DSCT) at Sn150 kV and 80/Sn150 kV. PCD-CT virtual monoenergetic 72 and 150 keV images and EID-CT images processed with and without metal artefact reduction algorithms (iMAR) were compared. Four radiologists rated the visualisation of bony structures and metal artefact severity. The Friedman test and Wilcoxon signed-rank test with Bonferronis correction were used. P-values of <= 0.0001 were considered statistically significant. Distributions of HU values of regions of interest (ROIs) in artefact-affected areas were analysed.Results: PCD-CT 140 kV 150 keV images received the highest scores and were significantly better than EID-CT Sn150 kV images. PCD-CT 72 keV images were rated significantly lower than all the others. HU-value variation was larger in the 120 kV and the 72 keV images. The ROI analysis revealed no large difference between scanners regarding artefact severity.Conclusion: PCD-CT 140 kV 150 keV images of a metal-containing bovine knee specimen provided the best image quality. They were superior to, or as good as, the best EID-CT images; even without the presumed advantage of tin filter and metal artefact reduction algorithms. PCD-CT is a promising method for reducing metal artefacts.

    • Bakgrund: Sjuksköterskestudenter är under sin verksamhetsförlagda utbildning (VFU) i en lärandesituation som ska bedömas och betygsättas. Hur examinationen är utformad påverkar hur studenterna planerar och genomför sina studier. Syftet var att utveckla och testa ett tregradigt bedömningsunderlag (för nivåer i år 1, m2 och 3 under utbildningen) med betygskriterier och tillhörande manual för VFUkurser i sjuksköterskeprogrammet. Metoder som användes var enkäter för utvärdering av bedömningsformulärens faktorer (dess innebörd och relevans), samtal, seminarier, pedagogiska diskussioner, fokusgrupp och kvantitativ metod. Enkäten för utvärdering besvarades av handledare och studenter. Dessutom fördes dialoger med lärare i sjuksköterskeprogrammet. Projektgruppen deltog även i VFU-seminarier på Hkr och i pedagogiska samtal i en fokusgrupp (LRC, Lärarresurscentrum). Dessutom tog projektgruppen del av resultat från regressions- och faktoranalys baserat på bedömningsunderlag. Resultat: Utifrån enkätsvaren reviderades bedömningsunderlaget, vilket resulterade i en sammanslagning av vissa faktorer för att få ett sammanhang samt att faktorerna verkligen anknöts till situationer från VFU. Benämning på VFU-samtalen (mellan klinisk lärare, handledare och student) bestämdes att vara planerings-, väglednings- och bedömningssamtal, för att spegla innehållet i de tre samtalen. Något som framkom som betydelsefullt angående samtalen var att lärare ställde fördjupande frågor under samtalen som vad, hur och varför. Resultatet från regressions- och faktoranalysen framhävde faktorer som självständighet och noggrannhet i omvårdnadsarbetet, mellanmänskliga relationer och samarbetsförmåga samt omsättning av forskning och lagar i praktiken. Dessa faktorer kunde användas som riktmärke vid bedömning och betygssättning av studenter under deras VFU-kurser. Slutsatser: Bedömningen ska utgå från studentens förmåga (inte endast från teoretisk kunskap), i sitt sammanhang och i varierande situationer under deras VFU. Bedömningsunderlaget tar inte bara upp studentens kunskap utan även färdigheter och värderingar.

    • Background: Nursing students are, during their work integrated learning (WIL) courses, in a learning situation that needs to be evaluated and graded. How the examination is shaped influence the students planning and carrying out of their studies.The aim was to develop and test three-graded, level (year 1, 2 and 3 during education) specific assessment forms with criteria for grading and an adherent manual to be used during nursing students’ WIL courses.Methods used were questionnaires for evaluation of the assessment forms for WIL (their meaning and relevance), dialogues, seminars, pedagogical discussions, focus group, and quantitative methodology. The questionnaire for evaluation was answered by supervisors and students. In addition dialogues with teachers in the nursing programme were carried out. The project group also participated in WIL-seminars at Kristianstad University and in pedagogical discussions in a focus group (at LRC, Teachers Resource Centre). In addition, the project group took part of results from regression- and factor-analyses based on the assessment forms.Results: Based on the questionnaire answers the assessment forms were revised, resulting in merging of some factors in order to coherent factors and it was secured that the factors were connected to situations in the WIL. The labelling of the WIL-dialogues (between the clinical teacher, the supervisor, and the student) was decided to be: “planning-“, “guiding-“, and “assessment dialogues”, in order to mirror the actual content in the three different dialogues. Something that emerged as important was that teachers gave deepening questions during the dialogues, such as asking students about what, how and why. The results from the regression-, and factor analyses highlighted factors such as independence, carefulness in caring, interpersonal relations, collaboration, and transforming research and laws into practice. These factors could be used as guidance for the assessments and grading of students during their WIL courses.Conclusions: The assessment shall be based on the students’ abilities (i.e. not only theoretical knowledge), the context, and different situations during their WIL. The developed assessment forms capture not only the students’ knowledge but also skills, and values.

    • Temporary no-take zones (NTZs) are increasingly introduced in Sweden as a fisheries management tool to restore populations of specific target species. This paper presents a cost-benefit analysis of two real case temporary NTZs closed during a 5–6 year period in the coastal zone of the Baltic Sea, using scenario analysis to account for uncertainty in both the biological and economic effects. A sensitivity analysis was added for certain key parameters. The results of the cost-benefit analyses for the two NTZs are positive in all scenarios relating to the most realistic case of no opportunity costs, i.e., assuming that all fishing activity could be relocated to adjacent areas without cost during the closed period. As an extreme case comparison, full opportunity costs were included, assuming that no fishing activity could be relocated to other areas during the closed period. One of the NTZs then exhibited a negative net result for most scenarios. For the other area the net result was positive even when the maximum opportunity costs of temporary lost fishing opportunities were included, largely depending on the strong positive change in the value of commercial fishing. By demonstrating potential costs and benefits of using temporary no-take zones in fisheries management this study may contribute to policy making, as well as to creating acceptance from stakeholder groups that incur short-term costs from closing areas to fishing.

    • Amyloid precursor protein (APP) is the precursor protein to amyloid β (Aβ), the main constituent of senile plaques in Alzheimer's disease (AD). Endogenous Aβ peptides reflect the APP processing, and greater knowledge of different APP degradation pathways is important to understand the mechanism underlying AD pathology. When one analyzes longer Aβ peptides by low-energy collision-induced dissociation tandem mass spectrometry (MS/MS), mainly long b-fragments are observed, limiting the possibility to determine variations such as amino acid variants or post-translational modifications (PTMs) within the N-terminal half of the peptide. However, by using electron capture dissociation (ECD), we obtained a more comprehensive sequence coverage for several APP/Aβ peptide species, thus enabling a deeper characterization of possible variants and PTMs. Abnormal APP/Aβ processing has also been described in the lysosomal storage disease Niemann-Pick type C and the major large animal used for studying this disease is cat. By ECD MS/MS, a substitution of Asp7 → Glu in cat Aβ was identified. Further, sialylated core 1 like O-glycans at Tyr10, recently discovered in human Aβ (a previously unknown glycosylation type), were identified also in cat cerebrospinal fluid (CSF). It is therefore likely that this unusual type of glycosylation is common for (at least) species belonging to the magnorder Boreoeutheria. We here describe a detailed characterization of endogenous APP/Aβ peptide species in CSF by using an online top-down MS-based method.

The aim of this study was to characterize the C-termini of sAPP alpha and sAPP beta in cerebrospinal fluid (CSF) by mass spectrometry (MS) and to evaluate whether different combinations of these fragments better separate between AD patients and controls by comparing two different sAPP immunoassays. Methods: Using immunoprecipitation and high resolution MS, the APP species present in CSF were investigated. CSF levels of sAPP alpha and sAPP beta from patients with AD (n=43) and from non-demented controls (n=44) were measured using AlphaLISA and MSD immunoassays that employ different antibodies for C-terminal recognition of sAPP alpha. Results: Four different C-terminal forms of sAPP were identified, sAPP beta-M671, sAPP beta-Y681, sAPP alpha-Q686, and 5APP alpha-K687 (APP770 numbering). Neither immunoassay for the sAPP species could separate the two patient groups. The correlation (R-2) between the two immunoassays was 0.41 for sAPP alpha and 0.45 for sAPP beta. Conclusion: Using high resolution MS, we show here for the first time that sAPP alpha in CSF ends at Q686 and K687.     • Cerebral accumulation of amyloid β (Aβ) is a pathological hallmark of Alzheimer's disease (AD). Proteolytic processing of amyloid precursor protein (APP) by α- or β-secretase results in two soluble metabolites, sAPPα and sAPPβ, respectively.     • Purpose. To examine whether the Sollerman hand function test is reliable in a test-retest situation in patients with chronic stroke. Method. Three independent examiners observed each patient at three experimental sessions; two days in week I (short-term test-retest) and one day in week 4 (long-term test-retest). A total of 24 patients with chronic stroke (mean age; 59.7 years, mean time since stroke onset 29.6 months) participated. The examiners simultaneously assessed the patients' ability to perform 20 subtests. Both ordinal data (generalized kappa) and total sum scores (Spearman's rank correlation coefficient (Spearman's rho), intra class correlation coefficient (ICC2,1) and mean differences) were used in the statistical analysis. Results. There was agreement (kappa >= 0.4) between the examiners for 15 out of the 20 subtests. Using total sum scores, the agreement within the examiners, both short- and long-term, was higher than 0.96 (for Spearman's rho and ICC, respectively). The mean differences were 0.29-1.0/80 points within each examiner. Agreement between the examiners at each session was higher than 0.96 (Spearman's rho) and 0.92 (ICC), respectively. Systematic differences (p < 0.05) were, however, found between examiners A and B/C for all sessions. Conclusions. The Sollerman hand function test seems to be a reliable test in patients with chronic stroke, but we recommend that the same examiner evaluates a patient's hand function pre- and post-treatment.
