    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

    • Elevated body weight in adolescence is associated with early cardiovascular disease, but whether this association is traceable to weight in early adulthood, weight in midlife or to weight gain is not known. The aim of this study is to assess the risk of midlife coronary atherosclerosis being associated with body weight at age 20, body weight in midlife and body weight change.We used data from 25,181 participants with no previous myocardial infarction or cardiac procedure in the Swedish CArdioPulmonary bioImage Study (SCAPIS, mean age 57 years, 51% women). Data on coronary atherosclerosis, self-reported body weight at age 20 and measured midlife weight were recorded together with potential confounders and mediators. Coronary atherosclerosis was assessed using coronary computed tomography angiography (CCTA) and expressed as segment involvement score (SIS).The probability of having coronary atherosclerosis was markedly higher with increasing weight at age 20 and with mid-life weight (p<0.001 for both sexes). However, weight increase from age 20 until mid-life was only modestly associated with coronary atherosclerosis. The association between weight gain and coronary atherosclerosis was mainly seen in men. However, no significant sex difference could be detected when adjusting for the 10-year delay in disease development in women.Similar in men and women, weight at age 20 and weight in midlife are strongly related to coronary atherosclerosis while weight increase from age 20 until midlife is only modestly related to coronary atherosclerosis.

    • BackgroundElevated body weight in adolescence is strongly associated with early cardiovascular disease, but whether this association is traceable to weight in early adulthood, or to weight gain with subsequent high adult weight is not known. Using data from the Swedish CArdioPulmonary bioImage Study (SCAPIS), we investigated the association between weight at age 20, weight gain to midlife and coronary artery calcium score (CACS) at midlife.MethodsIn the first 15,810 participants in SCAPIS (mean age 58 years, 52% women), data on CACS at midlife, self-reported body weight at age 20 and weight at examination in SCAPIS were recorded.ResultsCACS in midlife was significantly higher with increasing weight at age 20 (p<0.001 for both sexes), and then increased with weight gain until midlife at all levels of body weight at age 20 after adjusting for age, height, smoking, alcohol intake, education level, exercise levels and LDL cholesterol. However, the association with weight gain was only significant in men (p = 0.047), not in women (p=0.474). No significant interaction was seen between weight at age 20 and midlife weight with CACS. The effect of weight at age 20 on CACS was significantly more marked in men than in women, as was the effect of weight gain (p<0.001 for both interactions).ConclusionWeight at age 20 and weight gain to midlife were both related to CACS, but much more markedly so in men than in women, indicating a generally larger effect of both early adult weight and further weight gain until midlife on CACS in men, compared to women.

  • Implementation of the Strategic Approach to International Chemicals Management in Cambodia : effects of regime design

    • Over the past 20 years, a cluster of international environmental agreements has developed aiming at reducing the risks associated with production and use of chemicals. The Strategic Approach to International Chemicals Management (SAICM) is the newest addition to this cluster and serves to guide efforts to meet the 2020 goal to use and produce chemicals in a way that minimizes significant adverse effects on human health and the environment. SAICM differs from other chemical and waste agreements on several key points: It is a policy framework that is legally non-binding; it comprises a broad scope of activities; and it allows for active participation of non-governmental stakeholders. A central aim of SAICM is to decrease the gap between developed and developing countries in terms of capacities to manage chemicals safely. This article examines the early implementation of SAICM, based on a national-level case study in Cambodia and interviews with SAICM stakeholders. The results show that SAICM has generated a clear momentum in Cambodia and has led to several implementation projects. Based on the interviews, the overall conclusion is that design features of the SAICM—its broad scope, multi-stakeholder participation and voluntary status—are appropriate for the purpose of improving chemicals management in a developing country like Cambodia. However, these features also bring about difficulties in measuring progress on implementation. The future development of SAICM therefore needs to balance the benefits of its key design features with the need to more effectively and precisely monitor progress toward the 2020 goal.

  • Miljöpåverkan från svensk konsumtion - nya indikatorer för uppföljning. Slutrapport för forskningsprojektet PRINCE

    • NATURVÅRDSVERKET RAPPORT 6842 Miljöpåverkan från svensk konsumtion - nya indikatorer för uppföljning Sammanfattning Projektet Policy Relevant Indicators for Consumption and Environment (PRINCE) har haft som syfte att analysera potentiell miljöpåverkan kopplad till svensk konsumtion, både i Sverige och utomlands, och att kvantifiera denna med indikatorer. För att kunna producera de föreslagna indikatorerna har projektet utvecklat en ny matematisk metod för att kombinera svenska miljöekonomiska data med en multiregional input-outputmodell, som är relativt lättarbetad och bevarar en flexibilitet som möjliggör ett byte till andra multiregionala modeller om så önskas. Baserat på detta har projektet konstruerat en ny indikatormodell, kallad PRINCE-modellen, och uppskattat en lång rad utsläpp och resursanvändning kopplat till svensk konsumtion med den. Detta inkluderar förutom växthusgasutsläpp och luftföroreningar, en rad naturresurser och utsläpp som tidigare inte har studerats lika mycket, såsom markanvändning, vattenanvändning, användning och utsläpp av kemikalier och växthusgasutsläpp från avskogning. För vissa variabler har projektet tagit fram indikatorer för ett visst år, och för vissa andra har vi producerat tidsserier för åren 2008-2014. För växthusgaser, utsläpp av kväveoxider, svaveldioxid och partiklar har utsläppen från svensk konsumtion minskat under denna tid. Även arealen mark som tagits i anspråk kopplat till den svenska konsumtionen har minskat något under denna tid, medan totala materialflöden ökat. Under denna tidsperiod har förädlingsvärdet, som är ett mått på ekonomisk aktivitet, ökat vilket visar på en absolut frikoppling mellan ekonomisk utveckling och utsläpp av växthusgaser (med reservation för att inga höghöjdseffekter av utsläpp från flyg har ingått i denna studie). Utsläppsminskningen är dock betydligt mindre än den som krävs för att vara i linje med globala överenskommelser om klimatet så frikopplingen har inte varit tillräckligt stor. Det svenska generationsmålet anger bland annat att de svenska miljöproblemen ska lösas utan att orsaka ökade miljö- och hälsoproblem utanför Sverige. För de indikatorer där en tidsserie producerats (växthusgaser, utsläpp av kväveoxider, svaveldioxid och partiklar) har andelen av utsläppen som sker i Sverige respektive utomlands kopplat till den svenska konsumtionen varit relativt oförändrad under den studerade tidsperioden. Då de totala utsläppen minskat så har även utsläppen som skett utomlands under perioden minskat något. Projektet har med indikatorernas hjälp studerat vilka geografiska områden där miljöpåverkan från svensk konsumtion är störst och vilka produktkategorier som ger stort utslag i uppföljningen. Resultaten är något olika för olika typer av miljöpåverkan men produkter från byggsektorn, livsmedel och jordbruksprodukter, samt hushållens direkta utsläpp bland annat från fossila bränslen är kategorier med stor miljöpåverkan. Miljöpåverkan kopplat till svensk konsumtion sker i många olika länder. Förutom i Sverige sker utsläppen bland annat i stora länder som Kina, Ryssland, Tyskland och USA men också i många andra länder som Sverige importerar ifrån. Projektet har också tagit fram en grupp med indikatorer som kan mäta kemikalieanvändningen och utsläpp på en övergripande nivå. De täcker både användning av farliga kemiska produkter och utsläpp av vissa farliga ämnen. Resultaten pekar bland annat på att både användning och utsläpp av farliga kemikalier i stor utsträckning sker utomlands. Till exempel sker endast 22% av användningen av farliga kemikalier och 20% av utsläppen av farliga ämnen, som ingår i indikatorn, för svensk konsumtions räkning inom Sveriges gränser. Kemikalieindikatorerna kräver ett visst fortsatt utvecklingsarbete, exempelvis med framtagande av tidsserier. Vidare har projektet tagit fram nya förslag på hur man kan följa upp miljöpåverkan av fiskkonsumtion beroende på typ av fisk som konsumeras och vilken fångstmetod som använts. Även fiskindikatorerna kräver fortsatt arbete innan de kan läggas till ordinarie uppföljningsprogram. Projektet har också undersökt metoder för att fördjupa analysen av miljöpåverkan av vattenanvändning i produktionen genom att urskilja vilka områden som har särskilda problem med vattenbrist. Detta är inte möjligt att göra med någon större precision i dagsläget. Två olika metoder provades inom projektet, en som använde vattenåtgång för jordbruksprodukter och en som använde data om vattenbrist per land. Slutligen har specialstudier även gjorts av viktiga produktgrupper som mat och dryck, IKT-sektorn (informations- och kommunikationsteknik), fossilbränsleanvändningen i sjöfarten med två olika metoder, samt socioekonomisk påverkan av svensk konsumtion i Kina. Baserat på resultaten har projektet genererat ett antal rekommendationer för hur arbetet med konsumtionsbaserade indikatorer kan drivas vidare: • SCB ändrar den tidigare beräkningsmetoden för att beräkna utsläpp från konsumtion och använder de multiregionala data som finns tillgängliga i EXIOBASE för att beräkna miljöpåverkan från import. • För utsläpp av växthusgaser samt för utsläpp till luft av kväveoxider, svaveldioxid och partiklar: generationsmålet följs upp med den utvecklade PRINCE-modellen som kombinerar svensk miljöekonomisk data med multiplikatorer som beräknas med en multiregional input-outputmodell. • Det internationella arbetet med förbättrad tillgång till harmoniserad miljöstatistik stöttas. • Arbetet med vidareutveckling av indikatorer för användning av naturresurser (som markanvändning och dess koppling till avskogning och biologisk mångfald, vattenanvändning, materialflöden och energi) fortsätter bland annat avseende kopplingen mellan dessa flöden och miljöpåverkan, de svenska miljömålen och de globala hållbarhetsmålen. • Forsknings- och utvecklingsarbetet med kemikalieindikatorerna fortsätter och tidsserier tas fram för att så småningom kunna ta in dem i analysen av generationsmålet. • Metoderna som föreslagits för att bedöma miljöpåverkan från fiskkonsumtion vidareutvecklas. • Metoderna som föreslagits för att beräkna utsläpp från internationella transporter vidareutvecklas.

  • Degradation of dissolved organic matter in oxic and anoxic lake water

    • Decades of conflicting results have fueled a debate about how O-2 affects organic matter (OM) degradation and carbon cycling. In a laboratory study, using both OM taken directly from a humic lake and chemically isolated fulvic acid, we monitored the mineralization of dissolved OM in freshwater under purely oxic and anoxic conditions, under oxic then anoxic conditions, and under anoxic then oxic conditions, for 426 d. Between 5% and 24% of the initial OM was mineralized, with most extensive mineralization occurring under purely oxic and anoxic-oxic conditions. A sequential change in the O-2 regime did not result in greater overall degradation, but initially anoxic conditions favored subsequent oxic mineralization. A substantially greater fraction of the OM was degraded than in previous shorter studies, with as much as 50% of the total OM degradation occurring after 147 d into the experiment. Three fractions of the degradable OM were identified: OM degraded only under oxic conditions (68-78%), OM degraded more rapidly under anoxic conditions than under oxic conditions (16-18%), and OM degraded at equal rates under both oxic and anoxic conditions (6-14%). The degradation patterns of natural dissolved OM from a humic lake and chemically isolated fulvic acid were very similar, which indicates a similar level of bioavailability. The difference between anoxic and oxic degradation was greater in our long-term studies than in previous short-term experiments, which indicates that the oxic and anoxic degradation potentials vary with increasing overall OM recalcitrance and that similar oxic and anoxic degradation rates can be expected in short-term experiments in which <30% of the long-term degradable OM is allowed to decompose.

  • A genecentric Human Protein Atlas for expression profiles based on antibodies

    • An attractive path forward in proteomics is to experimentally annotate the human protein complement of the genome in a genecentric manner. Using antibodies, it might be possible to design protein-specific probes for a representative protein from every protein-coding gene and to subsequently use the antibodies for systematical analysis of cellular distribution and subcellular localization of proteins in normal and disease tissues. A new version (4.0) of the Human Protein Atlas has been developed in a genecentric manner with the inclusion of all human genes and splice variants predicted from genome efforts together with a visualization of each protein with characteristics such as predicted membrane regions, signal peptide, and protein domains and new plots showing the uniqueness (sequence similarity) of every fraction of each protein toward all other human proteins. The new version is based on tissue profiles generated from 6120 antibodies with more than five million immunohistochemistry-based images covering 5067 human genes, corresponding to approximately 25% of the human genome. Version 4.0 includes a putative list of members in various protein classes, both functional classes, such as kinases, transcription factors, G-protein-coupled receptors, etc., and project-related classes, such as candidate genes for cancer or cardiovascular diseases. The exact antigen sequence for the internally generated antibodies has also been released together with a visualization of the application-specific validation performed for each antibody, including a protein array assay, Western blot analysis, immunohistochemistry, and, for a large fraction, immunofluorescence-based confocal microscopy. New search functionalities have been added to allow complex queries regarding protein expression profiles, protein classes, and chromosome location. The new version of the protein atlas thus is a resource for many areas of biomedical research, including protein science and biomarker discovery.

    • Even though more than 30 years have passed since the eradication of smallpox, high titers of smallpox-specific antibodies are still detected in the blood of subjects vaccinated in childhood. In fact, smallpox-specific antibody levels are maintained in serum for more than 70 years. The generation of life-long immunity against infectious diseases such as smallpox and measles has been thoroughly documented. Although the mechanisms behind high persisting antibody titers in the absence of the causative agent are still unclear, long lived plasma cells (LLPCs) play an important role. Most of the current knowledge on LLPCs is based on experiments performed in mouse models, although the amount of data derived from human studies is increasing. As the results from mouse models are often directly extrapolated to humans, it is important to keep in mind that there are differences. These are not only the obvious such as the life span but there are also anatomical differences, for instance the adiposity of the bone marrow (BM) where LLPCs reside. Whether these differences have an effect on the function of the immune system, and in particular on LLPCs, are still unknown. In this review, we will briefly discuss current knowledge of LLPCs, comparing mice and humans. © 2007 - 2018 Frontiers Media S.A.

  • Quantifying shedding of synthetic fibers from textiles; a source of microplastics released into the environment

    • Microplastics in the environment are a subject of intense research as they pose a potential threat to marine organisms. Plastic fibers from textiles have been indicated as a major source of this type of contaminant, entering the oceans via wastewater and diverse non-point sources. Their presence is also documented in terrestrial samples. In this study, the amount of microfibers shedding from synthetic textiles was measured for three materials (acrylic, nylon, polyester), knit using different gauges and techniques. All textiles were found to shed, but polyester fleece fabrics shed the greatest amounts, averaging 7360 fibers/m(-2)/L-1 in one wash, compared with polyester fabrics which shed 87 fibers/m(-2)/L-1. We found that loose textile constructions shed more, as did worn fabrics, and high twist yarns are to be preferred for shed reduction. Since fiber from clothing is a potentially important source of microplastics, we suggest that smarter textile construction, prewashing and vacuum exhaustion at production sites, and use of more efficient filters in household washing machines could help mitigate this problem.

  • Exploring the planetary boundary for chemical pollution

    • Rockström et al. (2009a, 2009b) have warned that humanity must reduce anthropogenic impacts defined by nine planetary boundaries if “unacceptable global change” is to be avoided. Chemical pollution was identified as one of those boundaries for which continued impacts could erode the resilience of ecosystems and humanity. The central concept of the planetary boundary (or boundaries) for chemical pollution (PBCP or PBCPs) is that the Earth has a finite assimilative capacity for chemical pollution, which includes persistent, as well as readily degradable chemicals released at local to regional scales, which in aggregate threaten ecosystem and human viability. The PBCP allows humanity to explicitly address the increasingly global aspects of chemical pollution throughout a chemical's life cycle and the need for a global response of internationally coordinated control measures. We submit that sufficient evidence shows stresses on ecosystem and human health at local to global scales, suggesting that conditions are transgressing the safe operating space delimited by a PBCP. As such, current local to global pollution control measures are insufficient. However, while the PBCP is an important conceptual step forward, at this point single or multiple PBCPs are challenging to operationalize due to the extremely large number of commercial chemicals or mixtures of chemicals that cause myriad adverse effects to innumerable species and ecosystems, and the complex linkages between emissions, environmental concentrations, exposures and adverse effects. As well, the normative nature of a PBCP presents challenges of negotiating pollution limits amongst societal groups with differing viewpoints. Thus, a combination of approaches is recommended as follows: develop indicators of chemical pollution, for both control and response variables, that will aid in quantifying a PBCP(s) and gauging progress towards reducing chemical pollution; develop new technologies and technical and social approaches to mitigate global chemical pollution that emphasize a preventative approach; coordinate pollution control and sustainability efforts; and facilitate implementation of multiple (and potentially decentralized) control efforts involving scientists, civil society, government, non-governmental organizations and international bodies.

This paper presents an analysis of which products and services cause the largest environmental pressures in terms of resource use and emissions and in which countries or regions these pressures occur. The results have been calculated using a hybrid model developed in the PRINCE project combining the multi-regional input-output database Exiobase with data from the Swedish economic and environmental accounts. The following environmental pressures are analysed: Use of Land, Water and Material resources, Emissions of Greenhouse gases (GHG), Sulphur dioxides (SO2), Nitrogen oxides (NOx), and Particulate Matters (PM 2.5 and 10). The product groups include a range of goods and services bought for private or public consumption and investments. The results show that Sweden is a net importer of all embodied environmental pressures, except for land use and material use. The most important product groups across environmental pressures are construction, food products and direct emissions from households (except for sulphur dioxide emissions and material use for the latter). Other recurrent product groups across several indicators are wholesale and retail services, architecture and engineering, dwellings, motor vehicles and machinery and equipment. However, for the three natural resource pressures Use of Water, Land and Material resources, agricultural products are a relatively important product group along with products from forestry for the last two indicators. The environmental pressures occur to a large degree in Sweden but also some other countries stand out as particularly important. One significant country is China, which is among the top ten countries for all indicators. Other highly relevant countries or regions are Rest of Asia and Pacific, Russia, Denmark, Germany and Spain. This variation indicates the need to work on policies at various levels: national, EU, bilateral and international.

    • In order to produce goods and services that are consumed in Sweden, natural resources are extracted and pollutants are emitted in many other countries. This paper presents an analysis of the goods and services consumed in Sweden that cause the largest environmental pressures in terms of resource use and emissions, identifying in which countries or regions these pressures occur. The results have been calculated using a hybrid model developed in the PRINCE project combining the multi-regional input-output database EXIOBASE with data from the Swedish economic and environmental accounts. The following environmental pressures are analysed: Use of Land, Water and Material resources, Emissions of Greenhouse gases (GHG), Sulphur dioxides (SO2), Nitrogen oxides (NOx), and Particulate Matters (PM 2.5 and 10). The product groups are those goods and services bought for private or public consumption and capital investments, as listed in the Swedish economic accounts. The results show that Sweden is a net importer of all embodied environmental pressures, except for land use and material use. The most important product groups across environmental pressures are construction, food products and direct emissions from households (except for sulphur dioxide emissions and material use for the latter). Other product groups that are found to have environmental pressures across several indicators are wholesale and retail services, architecture and engineering, dwellings, motor vehicles and machinery and equipment. However, for the three natural resource pressures Use of Water, Land and Material resources, agricultural products are a relatively important product group along with products from forestry for the last two indicators. A considerable proportion of the environmental pressure occurs in Sweden, but when comparing those of domestic origin and that occurring internationally, the majority of all pressures for Swedish consumption occur abroad (except for land use).     • In this paper we will discuss humor as a gendered resource which opens up spaces for creativity in music education. We will do so by re-analysing empirical material from our two doctoral dissertations focusing on music education in secondary schools in Sweden. Humor is often presented as something positive when it comes to education. Humor has for example been pointed out as a resource to create motivation within school-subjects otherwise not regarded as interesting among students. Humor has also been proven fruitful for teachers in order to create a friendly environment and to balance teachers formal position with a more familiar position. In other words, humor seems, within education, to be constructed as a resource to accomplish different educational goals within the classroom. In this paper though, we will discuss difficulties to keep in mind when humor appears in students interactions within the music-classroom. In the following we will draw the attention to some themes we want to discuss which have become visible in our material. When it comes to gender, humor does not seem to be a neutral resource. In studies conducted within the secondary school music-education, humor seems to be more frequently used to express masculinity. Students positioned as boys are also more expected to be humoristic and make jokes than students positioned as girls. Similarly, in research regarding the humor-business, humor seems to represent a masculine domain. Even if this is a hierarchical order that seems to change, it is a discourse that is still being reproduced and thus still have a possible explanatory power. Humor as a masculine domaine is in the material also apparent as students positioned as boys expressing humor are not being questioned, which is the case among students positioned as girls. Language creates binaries. These binaries are contextual and changes over time but are however inevitably producing positions in the particular context. Humor is in our material frequently constructed as the opposite to seriousity. This does not mean that making jokes could not be “serious business”, but rather that the distance to seriousity in itself could be used in interaction to create space, which we would like to discuss in terms of creativity. Making jokes in the different music classrooms can function as resources to create distance to seriousity. In other words, making jokes makes it possible to express yourself in otherwise not socially accepted ways without being criticized. When draped in humor, expressions such as sexism, violence, racism, homofobia etcetera could be articulated and yet not followed by criticism. Even if this could be understood as a somewhat negative way of using humor, it points towards the creative potential that comes with humor in social interaction. This kind of creativity is primarily connected to the positioning work that is being done in interactions. Even creativity connected to musical expression is, following our material, facilitated by expressions of humor. Singing in different ways in order to make other people laugh is connected to breaking (and thus at the same time expressing) musical conventions in different contexts. Singing in a funny playful way can also be used as a didactic method to encourage pupils to sing, or as a positive approach during the rehearsal phases of musical processes in school. In secondary school music classrooms the demands from public policy to teach creativity in music is not seldomly done by letting students write songs. In our material we have studied an example in which the students are seriously engaged in expressing humor within their composing. We will argue that this kind of humor increases creativity. Attempting to be humoristic can in other words be very closely connected to creativity. Humor could also be understood in negative terms, not in what it produces in terms of creativity in order to make a joke, but rather in what or who is the aim of the joke. Something or someone is always pointed out in jokes. Jokes have, in other words, a normative effect. In our material making jokes could be used to legitimize pointing out someone's mistakes for example, even if teasing is not socially accepted within the classroom. Finally, these different aspects of humor combined creates different opportunities for musical creativity within music education. These differences are, we argue, important to consider when using humor as a didactic resource.

    • Surgical excision followed by histopathological examination is the gold standard for the diagnosis and staging of melanoma. Reoperations and unnecessary removal of healthy tissue could be reduced if non-invasive imaging techniques were available for presurgical tumor delineation. However, no technique has gained widespread clinical use to date due to shallow imaging depth or the absence of functional imaging capability. Photoacoustic (PA) imaging is a novel technology that combines the strengths of optical and ultrasound imaging to reveal the molecular composition of tissue at high resolution. Encouraging results have been obtained from previous animal and human studies on melanoma, but there is still a lack of clinical data. This is the largest study of its kind to date, including 52 melanomas and nevi. 3D multiwavelength PA scanning was performed ex vivo, using 59 excitation wavelengths from 680 nm to 970 nm. Spectral unmixing over this broad wavelength range, accounting for the absorption of several tissue chromophores, provided excellent contrast between healthy tissue and tumor. Combining the results of spectral analysis with spatially resolved information provided a map of the tumor borders in greater detail than previously reported. The tumor dimensions determined with PA imaging were strongly correlated with those determined by histopathological examination for both melanomas and nevi.

    • Background: Multiple sclerosis (MS) is a demyelinating disease affecting millions of people worldwide. Hereditary susceptibility and environmental factors contribute to disease risk. Infection with Epstein–Barr virus (EBV) and human herpesvirus 6A (HHV-6A) have previously been associated with MS risk. Other neurotropic viruses, such as rubella virus (RV), are possible candidates in MS aetiopathogenesis, but previous results are limited and conflicting.Methods: In this nested case–control study of biobank samples in a Swedish cohort, we analysed the serological response towards RV before the clinical onset of MS with a bead-based multiplex assay in subjects vaccinated and unvaccinated towards RV. The association between RV seropositivity and MS risk was analysed with conditional logistic regression.Results: Seropositivity towards RV was associated with an increased risk of MS for unvaccinated subjects, even when adjusting for plausible confounders including EBV, HHV-6A, cytomegalovirus and vitamin D (adjusted odds ratio [AOR] = 4.0, 95% confidence interval [CI] 1.8–8.8). Cases also had stronger antibody reactivity towards rubella than controls, which was not seen for other neurotropic viruses such as herpes simplex or varicella zoster. Furthermore, we observed an association between RV seropositivity and MS in vaccinated subjects. However, this association was not significant when adjusting for the aforementioned confounders (AOR = 1.7, 95% CI 1.0–2.9).Conclusions: To our knowledge, these are the first reported associations between early RV seropositivity and later MS development. This suggests a broadening of the virus hypothesis in MS aetiology, where molecular mimicry between rubella epitopes and human central nervous system molecules could be an attractive possible mechanism.

    • Background: : A two- to three-fold increase in the risk of multiple sclerosis (MS) after infectious mononucleosis (IM) has been observed in cohort and case control studies. However, this association has not been investigated prospectively from IM. It remains to be determined whether long-term immunospecific sequelae with features consistent with presymptomatic MS occur after IM. Methods: : Sera were obtained from individuals with acute IM from 2003-2007 (n = 42) and from the same individuals at a follow-up (FU) study approximately 10 years after IM. These were assayed for antibodies against a variety of Epstein-Barr virus (EBV) antigens, including gp350, a novel recombinant glycoprotein from the EBV envelope. Similarly, single-protein antigens were used to assess measles and varicella-zoster reactivity (Ncore and varicella-zoster glycoprotein E [VZVgE]). The FU study also included cerebrospinal fluid (CSF) samples from 21 of these individuals to test for IgG antibodies against the same viral antigens. As controls, CSF and serum samples were obtained from 15 EBV-seropositive volunteers who denied a history of IM, and serum samples were obtained from 24 EBV-seropositive blood donors. Anti-gp350, anti-Ncore and anti-VZVgE IgG levels were also analysed in sera and CSF samples from 22 persons with MS. Results: : The FU assays showed higher anti-gp350 IgG (p = 0.007, univariate) than among healthy controls, with no difference in serum anti-VCA or anti-EBNA1 IgG levels and no difference in anti-gp350 in the CSF samples. Anti-Ncore IgG and anti-VZVgE were higher in acute IM samples (p < 0.001 and p < 0.0001, respectively) than at FU, although anti-Ncore remained heightened in an age-adjusted analysis at FU (p = 0.014) compared to the control group. In the MS group, the serum anti-gp350 and anti-Ncore IgG levels were significantly higher than among the control group, but the anti-VZVgE levels were not. The CSF anti-gp350 and VZVgE levels were slightly higher among persons with MS than among the control group, whereas anti-Ncore IgG was markedly higher in persons with MS than in the control group. Conclusion: : In the present study IM showed certain similarities with MS. Increased anti-gp350 reactivity persisted more than a decade after IM, reminiscent of the established increased anti-EBV reactivity in presymptomatic MS. Acute IM was associated with increased anti-measles and anti-VZV immunoreactivity, similar to the MRZ reaction in MS, with some evidence suggesting that this measles reactivity persisted after a decade.

    • BackgroundIn patients undergoing resection for pancreatic cancer, adjuvant modified fluorouracil, leucovorin, irinotecan, and oxaliplatin (FOLFIRINOX) improves overall survival compared with alternative chemotherapy regimens. We aimed to compare the efficacy and safety of neoadjuvant FOLFIRINOX with the standard strategy of upfront surgery in patients with resectable pancreatic ductal adenocarcinoma.MethodsNORPACT-1 was a multicentre, randomised, phase 2 trial done in 12 hospitals in Denmark, Finland, Norway, and Sweden. Eligible patients were aged 18 years or older, with a WHO performance status of 0 or 1, and had a resectable tumour of the pancreatic head radiologically strongly suspected to be pancreatic adenocarcinoma. Participants were randomly assigned (3:2 before October, 2018, and 1:1 after) to the neoadjuvant FOLFIRINOX group or upfront surgery group. Patients in the neoadjuvant FOLFIRINOX group received four neoadjuvant cycles of FOLFIRINOX (oxaliplatin 85 mg/m2, irinotecan 180 mg/m2, leucovorin 400 mg/m2, and fluorouracil 400 mg/m2 bolus then 2400 mg/m2 over 46 h on day 1 of each 14-day cycle), followed by surgery and adjuvant chemotherapy. Patients in the upfront surgery group underwent surgery and then received adjuvant chemotherapy. Initially, adjuvant chemotherapy was gemcitabine plus capecitabine (gemcitabine 1000 mg/m2 over 30 min on days 1, 8, and 15 of each 28-day cycle and capecitabine 830 mg/m2 twice daily for 3 weeks with 1 week of rest in each 28-day cycle; four cycles in the neoadjuvant FOLFIRINOX group, six cycles in the upfront surgery group). A protocol amendment was subsequently made to permit use of adjuvant modified FOLFIRINOX (oxaliplatin 85 mg/m2, irinotecan 150 mg/m2, leucovorin 400 mg/m2, and fluorouracil 2400 mg/m2 over 46 h on day 1 of each 14-day cycle; eight cycles in the neoadjuvant FOLFIRINOX group, 12 cycles in the upfront surgery group). Randomisation was performed with a computerised algorithm that stratified for each participating centre and used a concealed block size of two to six. Patients, investigators, and study team members were not masked to treatment allocation. The primary endpoint was overall survival at 18 months. Analyses were done in the intention-to-treat (ITT) and per-protocol populations. Safety was assessed in all patients who were randomly assigned and received at least one cycle of neoadjuvant or adjuvant therapy. This trial is registered with ClinicalTrials.gov, NCT02919787, and EudraCT, 2015-001635-21, and is ongoing.FindingsBetween Feb 8, 2017, and April 21, 2021, 77 patients were randomly assigned to receive neoadjuvant FOLFIRINOX and 63 to undergo upfront surgery. All patients were included in the ITT analysis. For the per-protocol analysis, 17 (22%) patients were excluded from the neoadjuvant FOLFIRINOX group (ten did not receive neoadjuvant therapy, four did not have pancreatic ductal adenocarcinoma, and three received another neoadjuvant regimen), and eight (13%) were excluded from the upfront surgery group (seven did not have pancreatic ductal adenocarcinoma and one did not undergo surgical exploration). 61 (79%) of 77 patients in the neoadjuvant FOLFIRINOX group received neoadjuvant therapy. The proportion of patients alive at 18 months by ITT was 60% (95% CI 49–71) in the neoadjuvant FOLFIRINOX group versus 73% (62–84) in the upfront surgery group (p=0·032), and median overall survival by ITT was 25·1 months (95% CI 17·2–34·9) versus 38·5 months (27·6–not reached; hazard ratio [HR] 1·52 [95% CI 1·00–2·33], log-rank p=0·050). The proportion of patients alive at 18 months in per-protocol analysis was 57% (95% CI 46–67) in the neoadjuvant FOLFIRINOX group versus 70% (55–83) in the upfront surgery group (p=0·14), and median overall survival in per-protocol population was 23·0 months (95% CI 16·2–34·9) versus 34·4 months (19·4–not reached; HR 1·46 [95% CI 0·99–2·17], log-rank p=0·058). In the safety population, 42 (58%) of 73 patients in the neoadjuvant FOLFIRINOX group and 19 (40%) of 47 patients in the upfront surgery group had at least one grade 3 or worse adverse event. 63 (82%) of 77 patients in the neoadjuvant group and 56 (89%) of 63 patients in the upfront surgery group had resection (p=0·24). One sudden death of unknown cause and one COVID-19-related death occurred after the first cycle of neoadjuvant FOLFIRINOX. Adjuvant chemotherapy was initiated in 51 (86%) of 59 patients with resected pancreatic ductal adenocarcinoma in the neoadjuvant FOLFIRINOX group and 44 (90%) of 49 patients with resected pancreatic ductal adenocarcinoma in the upfront surgery group (p=0·56). Adjuvant modified FOLFIRINOX was given to 13 (25%) patients in the neoadjuvant FOLFIRINOX group and 19 (43%) patients in the upfront surgery group. During adjuvant chemotherapy, neutropenia (11 [22%] patients in the neoadjuvant FOLFIRINOX group and five [11%] in the upfront surgery group) was the most common grade 3 or worse adverse event.InterpretationThis phase 2 trial did not show a survival benefit from neoadjuvant FOLFIRINOX in resectable pancreatic ductal adenocarcinoma compared with upfront surgery. Implementation of neoadjuvant FOLFIRINOX was challenging. Future trials on treatment sequencing in resectable pancreatic ductal adenocarcinoma should be biomarker driven.

    • Purpose: To describe what patients with chronic pain expect from a multimodal pain rehabilitation programme. Material and method: Qualitative interviews were used to uncover expectations about a multimodal rehabilitation programme offered at the Pain and Rehabilitations Centre, The University Hospital; Linkoping, Sweden. Sixteen women and two men (mean age 37 years; standard deviation 10 years) with chronic benign pain participated. The interviews were analysed using qualitative content analyses. Results: To participate actively in the multimodal pain rehabilitation programme and to learn adequate coping strategies to improve daily life emerged as a main category. It was based on the following four categories comprising expectations about: participating actively in the programme, interacting with the professionals and fellow patients, cognitive effects of the programme and tools for coping, and explicit effects from the programme. Conclusions: Many patients expressed expectations which may reflect that the information before the programme had started rehabilitation process at the time point for this study. The results could be applied in rehabilitation programmes by acknowledging expectations to interact with professional team members and fellow patients, by early addressing of positive and negative expectations about the future pain and by incorporating and strengthen expectations of learning to cope with pain.

    • Background: The closely related herpes simplex viruses 1 and 2 can cause inflammations of the central nervous system (CNS), where type 1 most often manifest as encephalitis (HSE), and type 2 as meningitis (HSM). HSE is associated with severe neurological complications, while HSM is benign in adults. We proposed that studying the chemokine and cytokine production in cerebrospinal fluid (CSF) and serum could indicate why two closely related viruses exhibit different severity of their accompanied CNS inflammation. Methods: Secretion patterns of 30 chemokines and 10 cytokines in CSF of adult patients with acute HSE (n = 14) and HSM (n = 20) in the initial stage of disease were analyzed and compared to control subjects without viral central nervous system infections and to levels in serum. Results: Most measured chemokines and cytokines increased in CSF of HSE and HSM patients. Overall, the CSF chemokine levels were higher in CSF of HSM patients compared to HSE patients. However, only five chemokines reached levels in the CSF that exceeded those in serum facilitating a positive CSF-serum chemokine gradient. Of these, CXCL8, CXCL9, and CXCL10 were present at high levels both in HSE and HSM whereas CXCL11 and CCL8 were present in HSM alone. Several chemokines were also elevated in serum of HSE patients but only one in HSM patients. No chemokine in- or efflux between CSF and serum was indicated as the levels of chemokines in CSF and serum did not correlate. Conclusions: We show that HSM is associated with a stronger and more diverse inflammatory response in the CNS compared to HSE in the initial stage of disease. The chemokine patterns were distinguished by the exclusive local CNS production of CXCL11 and CCL8 in HSM. Inflammation in HSM appears to be restricted to the CNS whereas HSE also was associated with systemic inflammation.

  • Pulp Particle Classification Based on Optical Fiber Analysis and Machine Learning Techniques

    • In the pulp and paper industry, pulp testing is typically a labor-intensive process performed on hand-made laboratory sheets. Online quality control by automated image analysis and machine learning (ML) could provide a consistent, fast and cost-efficient alternative. In this study, four different supervised ML techniques—Lasso regression, support vector machine (SVM), feed-forward neural networks (FFNN), and recurrent neural networks (RNN)—were applied to fiber data obtained from fiber suspension micrographs analyzed by two separate image analysis software. With the built-in software of a commercial fiber analyzer optimized for speed, the maximum accuracy of 81% was achieved using the FFNN algorithm with Yeo–Johnson preprocessing. With an in-house algorithm adapted for ML by an extended set of particle attributes, a maximum accuracy of 96% was achieved with Lasso regression. A parameter capturing the average intensity of the particle in the micrograph, only available from the latter software, has a particularly strong predictive capability. The high accuracy and sensitivity of the ML results indicate that such a strategy could be very useful for quality control of fiber dispersions.

    • AimsThe aim of this study was to investigate the association between estimated glucose disposal rate (eGDR), a proxy for insulin resistance, and retinopathy or kidney disease, i.e. micro-, or macroalbuminuria, in young individuals with type 1 diabetes (T1D).Material and MethodsUsing data from the Swedish pediatric registry for diabetes (SweDiabKids) and the registry for adults (NDR), all individuals with T1D with a duration of diabetes of less than 10 years between 1998 and 2017 were included. We calculated the crude incidence rates with 95% confidence intervals (CIs) and used multivariable Cox regression to estimate crude and adjusted hazard ratios (HRs) for two cohorts: retinopathy cohort or kidney disease cohort, stratified by eGDR categories: < 4, 4 to 5.99, 6 to 7.99, and >= 8 mg/kg/min (reference).ResultsA total of 22 146 (10 289 retinopathy cohort, and 11 857 kidney disease cohort with an overlapping of 9575) children and adults with T1D (median age 21 years, female 42% and diabetes duration of 6 and 7 years, respectively for the cohorts) were studied. During a median follow-up of 4.8 years (IQR 2.6-7.7) there were 5040 (24.7%), 1909 (48.1%), 504 (52.3%) and 179 (57.6%) events for retinopathy in individuals with an eGDR >= 8, 7.99 to 6, 5.99 to 4, and < 4 mg/kg/min, respectively. Corresponding numbers for kidney disease was 1321 (6.5%), 526 (13.3%), 255 (26.8%) and 145 (46.6%). After multiple adjustments for different covariates, individuals with an eGDR 7.99 to 6, 5.99 to 4 and < 4 mg/kg/min, had an increased risk of retinopathy compared to those with an eGDR >= 8 mg/kg/min (adjusted HRs, 95% CIs) 1.29 (1.20 to 1.40); 1.50 (1.31 to 1.71) and 1.74 (1.41 to 2.14). Corresponding numbers for kidney disease was (adjusted HRs, 95% CIs) 1.30 (1.11 to 1.52); 1.58 (1.25 to 1.99) and 1.33 (0.95 to 1.86), respectively.ConclusionseGDR, a proxy for insulin resistance, is associated with retinopathy and kidney disease in young adults with T1D. The risk of retinopathy increased with lower eGDR. The risk of kidney disease also increased with lower eGDR; however results show no association between the lowest eGDR and kidney disease. eGDR can be helpful to identify young T1D individuals at risk.

  • A systematic approach for major renovation of residential buildings

    • In Sweden, buildings are responsible for about 40 % of total energy use and about 10 % of total CO2 emissions Today more than 60 % of existing Swedish residential buildings are over 40 years old and are in need of major renovation. In addition, 15 % of all multi-family buildings and 27 % of all single-family houses were built before 1945. The increased energy use and threat from CO2 emissions of the building sector create a need for energy efficiency. The important role that renovation of residential buildings will play in reducing the total energy used by the Swedish building sector as well as in reducing primary energy use and CO2 emissions on both the national and global levels has been the impetus for the studies included in this thesis.The aim of the current research is to develop a methodology from a system perspective which can be used to analyze the energy use, optimal life cycle cost (LCC), energy efficiency measure (EEM) package, indoor environment, CO2 emissions, and primary energy use of a building or a community during major renovation. The developed methodology accomplished at three different levels, i.e. building level, cluster level and district level. The methodology considers both energy efficiency and economic viability during building renovation and will also play an important role in overall urban planning. The studied buildings include both non-listed and listed residential buildings and the tools used include building energy simulation (BES), survey, technical measurements, LCC optimization and building categorization.The results show that the combination of BES, technical measurements and surveys provides a holistic approach for evaluation of energy use and indoor environment of the studied residential buildings. The results from the current study also show that the 2020 energy target, i.e., reduction of energy use by 20 %, for the building sector can be achieved by all the studied building types and that the total LCC of these buildings are below the cost-optimal point. In comparison, the 2050 energy target, i.e., reduction of energy use by 50 %, for the building sector may be achieved by the non-listed buildings, but when the constraints relevant to listed buildings are added the cost-optimality changes as some EEMs in direct conflict with the building’s heritage value may not be implemented.The investigation of primary energy use and CO2 emissions by the residential buildings show that the higher the energy saving, the lower the primary energy use becomes, and vice versa. With the same energy saving, the heating system with higher primary energy factor results in higher primary energy use. From a CO2 emissions point of view, EEM packages proposed to help buildings connected to a CHP based district heating system, to reduce the energy use or LCC are not consistently effective. Since these EEM packages will reduce district heating demand, the electricity produced in the CHP plant will also decrease. When the biomass is considered a limited resource, measures such as investment in a biofuel boiler are not favourable from the CO2 emissions point of view. The current study has also shown that combining building categorization method and LCC optimization method will help the community to reduce its energy use, primary energy use and CO2 emissions in a systematic and strategic way.

Other chemical pollution-related planetary boundaries likely exist, but are currently unknown. A chemical posed an unknown planetary boundary threat if it simultaneously fulfills three conditions (1) it has an unknown disruptive effect on a vital Earth system process; (2) the disruptive effect is not discovered until it is a problem at the global scale, and (3) the effect is not readily reversible. In this paper, we outline scenarios in which chemical could fulfill each of the three conditions, then use the scenarios as the basis to define chemical profiles that fit each scenario. The chemical profiles are defined in terms of the nature of the effect of the chemical and the nature of exposure of the environment to the chemical. Priortization of chemicals in commerce against some of the profiles appears feasible, but there are considerable uncertainites and scientific challenges that must be addressed. Most challenging is prioritizing chemicals for the potential to have a currently unknown effect on a vital. Earth system process. We conclude that the most effective strategy currently available to identify chemicals that are planetary boundary threats is prioritization against profiles defined in terms of environmental exposure combined with monitoring and study of the biogeochemical process that underlie vital Earth system processes to identify currently unknown disruptive effects.

Sverige måste ta ansvar och göra sin rättvisa andel av det globala klimatarbetet. I strid med Parisavtalet ökar utsläppen av växthusgaser i en takt som gör att 1,5-gradersmålet kan överskridas om några år. De globala effekterna blir allt mer synliga med ständiga temperaturrekord, smältande isar, havshöjning och extremväder som torka, förödande bränder och skyfall med enorma översvämningar, som i Pakistan nyligen. Försörjningen av befolkningen utsätts för allvarliga hot i många länder.Minskningen av den biologiska mångfalden är extrem. Klimatkrisen är enligt WHO det största hotet mot människors hälsa i hela världen och barn utgör en särskilt sårbar grupp. Med Sveriges nordliga läge sker uppvärmningen här dubbelt så fort som det globala genomsnittet. Det förskjuter utbredningsområden för växtlighet och sjukdomsbärande insekter och ökar förekomsten av extremväder såsom värmeböljor, skogsbränder och översvämningar samt av många olika sorters infektioner och allergier. När extremväder ökar, ökar även stressen och risken för mental ohälsa. Värmeböljor ökar risken för sjukdom och död hos sårbara grupper som äldre, små barn och personer med kroniska sjukdomar. De negativa effekterna på hälsan kommer att öka i takt med klimatkrisen och barn riskerar att drabbas av ackumulerade negativa hälsoeffekter under hela sina liv. Redan i dag är mer än hälften av unga mellan 12 och 18 år i Sverige ganska eller mycket oroliga för klimat och miljö. Detta är förståeligt när våra beslutsfattare inte gör vad som krävs.Den juridiska och moraliska grunden för arbetet mot klimatförändringarna är att varje land måste göra sin rättvisa andel av det globala klimatarbetet. Centralt i det internationella klimatramverket är att rika länder med höga historiska utsläpp, däribland Sverige, måste gå före resten av världen. Dessa länder måste också bidra till att finansiera klimatomställningen i länderna i det Globala Syd, som är minst ansvariga för klimatkrisen men drabbas hårdast. Denna rättviseprincip är tydlig i Parisavtalet och var en het diskussionsfråga under COP27 i Sharm el-Sheikh, men lyser med sin frånvaro i det svenska klimatarbetet. Sverige har satt mål för att minska sina utsläpp. Men de är helt otillräckliga: minskningstakten är för låg och målen tillåter samtidigt att åtgärder skjuts på framtiden. Dessutom exkluderas merparten av Sveriges utsläpp från de svenska nationella utsläppsmålen; bland annat utelämnas utsläpp som svensk konsumtion orsakar utanför Sveriges gränser, utsläpp från utrikes transporter och utsläpp från markanvändning och skogsbruk, exempelvis utsläpp från förbränning av biobränslen eller utsläpp från dikade våtmarker (Prop. 2016/17:146 s.25-28).Sverige saknar dessutom ett eget mål för att öka upptaget av växthusgaser genom utökat skydd och restaurering av ekosystem, något som krävs för att begränsa de värsta konsekvenserna av klimatkrisen (IPCC s.32). Trots dessa låga ambitioner misslyckas Sverige med att nå sina utsläppsmål, konstaterar både Klimatpolitiska rådet och Naturvårdsverket. En klimatpolitik i linje med Parisavtalet kräver både att alla typer av växthusgasutsläpp minskar samtidigt som – inte i stället för – upptaget av växthusgaser maximeras: i dag misslyckas Sverige på bägge fronter.Slutsatsen är tydlig. Sverige vidtar inte de åtgärder som krävs för att skydda barns och ungdomars rättigheter enligt Europakonventionen till skydd för de mänskliga rättigheterna. Detta medför allvarliga risker för liv och hälsa för unga generationer, människor i andra länder och särskilt utsatta grupper. Detta kan inte fortsätta. Därför ställer vi oss bakom Auroras krav att Sverige börjar göra sin rättvisa andel och omedelbart sätter igång ett omfattande och långtgående klimatarbete som vilar på vetenskaplig grund och sätter rättvisa i centrum.

    • ABSTRACT: BACKGROUND: In order to improve child survival there is a need to target neonatal mortality. In this pursuit, valid local and national statistics on child health are essential. We analyze to what extent births and neonatal deaths are unreported in a low-income country and discuss the consequences at local and international levels for efforts to save newborn lives. METHODS: Information on all births and neonatal deaths in Quang Ninh province in Northern Vietnam in 2005 was ascertained by systematic inventory through group interviews with key informants, questionnaires and examination of health facility records. Health care staff at 187 Community Health Centers (CHC) and 18 hospitals, in addition to 1372 Village Health Workers (VHW), were included in the study. Results were compared with the official reports of the Provincial Health Bureau. RESULTS: The neonatal mortality rate (NMR) was 16/1000 (284 neonatal deaths/17 519 births), as compared to the official rate of 4.2/1000. The NMR varied between 44/1000 and 10/1000 in the different districts of the province. The under-reporting was mainly attributable to a dysfunctional reporting system and the fact that families, not the health system, were made responsible to register births and deaths. This under-reporting has severe consequences at local, national and international levels. At a local level, it results in a lack of awareness of the magnitude and differentials in NMR, leading to an indifference towards the problem. At a national and international level the perceived low mortality rate is manifested in a lack of investments in perinatal health programs. CONCLUSION: This example of a faulty health information system is reportedly not unique in low and middle income countries where needs for neonatal health reforms are greatest. Improving reporting systems on births and neonatal deaths is a matter of human rights and a prerequisite for reducing neonatal mortality in order to reach the fourth millennium goal.

    • Epstein-Barr virus (EBV) glycoprotein 350 (gp350) is the most abundant glycoprotein expressed on the EBV envelope, the major target for neutralizing antibodies and also essential for virion attachment to B lymphocytes. Several studies have addressed EBV gp350 as a vaccine candidate, but less commonly as a potential antigen for serological assays. The aim of the current study was to develop a diagnostic tool to quantify EBV gp350-specific IgG in previously EBV-infected individuals. A construct encoding the extracellular domain of EBV gp350 (amino acid (aa) 1-860) was developed for expression in Chinese hamster ovary cells. Serum samples (n = 360) with known IgG serostatus against viral capsid antigen (VCA) and Epstein-Barr nuclear antigen 1 (EBNA1) were divided into three groups based on the differences in their serostatus: VCA + EBNA1+ (n = 120), VCA + EBNA1-(n = 120) and VCA-EBNA1-(n = 120). The samples were analyzed by indirect ELISA using recombinant EBV gp350 aa 1-860 as antigen. A clear majority, 108 of the 120 VCA + EBNA1+ samples, had detectable EBV gp350-specific IgG. Of the 120 VCA + EBNA1-samples, 79 had detectable EBV gp350-specific IgG. Only 2 of the 120 VCA-EBNA1-samples had detectable EBV gp350-specific IgG. The results reported here show that use of the EBV gp350 aa 1-860 ELISA can serve as a sensitive method for EBV-specific IgG detection in serum samples.

    • Background Patients with multiple sclerosis (MS) demonstrate higher seroprevalence of Epstein-Barr virus (EBV) and increased anti-EBV IgG levels in serum compared with healthy controls. Intrathecal antibody production to measles virus (MeV) is a common finding in patients with MS. Objective To measure serum IgG reactivity to EBV glycoprotein 350 (gp350) and MeV nucleocapsid protein (N-CORE) in patients with MS and healthy controls and to determine if reactivity changed in patients during interferon beta (IFN beta) and/or natalizumab (NAT) treatment. A secondary aim was to determine the seroprevalence of EBV in patients and controls. Methods Patients with MS (n=728) were included from the Swedish pharmacovigilance study for NAT. Paired serum samples from 714 patients drawn before and during NAT treatment and paired samples from 170 patients during prior IFN beta treatment were analysed. In total, 156 patients were included in both groups. Samples from 144 matched blood donors served as controls. Indirect ELISA was applied using recombinant EBVgp350 and MeV N-CORE as antigens. EBVgp350 IgG seronegative samples were also analysed using EBV nuclear antigen 1 and viral capsid antigen (VCA). Results Patients with MS showed higher serum levels of anti-EBVgp350 and anti-MeV N-CORE IgG compared with controls. During NAT treatment, the levels of anti-EBVgp350 and anti-MeV N-CORE IgG declined, compared with the relatively stable levels noted during prior IFN beta treatment. Ten patients failed to demonstrate anti-EBVgp350 IgG but did show detectable anti-VCA IgG, indicating EBV seropositivity. In contrast, 10/144 controls were EBV seronegative. Conclusions Treatment with NAT, which is considered a selective immunosuppressive agent with a compartmentalised effect on the central nervous system, appeared to be associated with a moderate decrease in circulating IgG levels to EBVgp350 and MeV N-CORE. All patients with MS were EBV IgG seropositive, supporting the potential role of EBV in the pathogenesis of MS.

  • Viral proteins as serological antigens - Development and clinical applications

    • Serological methods are based on the detection of antibodies and antigens in mainly serum but also in other body fluids such as cerebrospinal fluid (CSF). Conventional whole virus antigens are widely used in viral serological assays. These antigens usually contain a mixture of proteins from the virus of interest together with residual cell components from antigen production, which can cause diagnostic problems with cross-reactive antibodies between closely related viruses and antibodies that bind to cellular components. The methods can become more specific by using antigens based on recombinant single viral proteins that differ between closely related viruses but to which the immune system reacts strongly (immunodominant proteins). The aim of the research has been to develop specific serological assays to detect antibodies to varicella-zoster virus (VZV), Epstein-Barr virus (EBV) and measles virus (MeV). This has been accomplished by recombinantly producing single, specific, immunodominant viral proteins, VZV glycoprotein E (gE), EBV glycoprotein 350 (gp350) and the core part of the MeV nucleocapsid protein (NCORE), for use as serological antigens in enzyme-linked immunosorbent assay (ELISA). In Paper I, we show that VZVgE functions well as ELISA antigen to detect anti-VZVgE IgG antibodies. The antigen has thereafter been used in the routine diagnostics at the Department of Clinical Microbiology, Sahlgrenska University Hospital. In Paper II, we demonstrate that EBVgp350 performs well as serological antigen in ELISA for the detection of anti-EBVgp350 IgG. In Paper III, we found that patients with multiple sclerosis (MS) and their clinically healthy siblings with similar MS findings in CSF, i.e. a suspect hyperimmune phenotype, still show an increased IgG response to MeV in both serum and CSF compared with healthy controls when the previously used complex MeV whole virus antigen was replaced with MeV NCORE. Our results indicate that the reactivity is indeed specific and not caused by cross-reacting autoantibodies to cellular proteins. In Paper IV, patients with MS show higher IgG levels in both serum and CSF to MeV and EBVgp350 compared with healthy controls. In addition, we observed that patients with serologically verified acute infectious mononucleosis have higher serum IgG levels to EBVgp350 at follow-up after 10 years compared with healthy controls, suggesting that EBV-induced mononucleosis affects the immune system in a powerful and long-lasting way. In Paper V, patients with MS treated with interferon beta (IFNβ) had higher anti-EBVgp350 and anti-MeV NCORE IgG levels in serum compared with healthy blood donors. Following initiation of treatment with the monoclonal antibody natalizumab, patients' serum IgG levels decreased against both antigens, whereas levels were relatively stable during previous IFNβ treatment. Another finding was that all 728 patients with MS in the study were EBV IgG seropositive while 10 of the 144 blood donors in the control group were EBV IgG seronegative. This finding further strengthens the potential role of EBV in the pathogenesis of MS. The developed ELISA methods can, through increased specificity, offer new diagnostic possibilities for detecting antibodies to EBV, VZV and MeV in viral infections, for control of immunity after infection/vaccination, in epidemiological investigations and in autoimmune diseases such as MS.

Here, we propose that there is no single chemical pollution planetary boundary, but rather that many planetary boundary issues governed by chemical pollution exist. We identify three conditions that must be simultaneously met for chemical pollution to pose a planetary boundary threat. We then discuss approaches to identify chemicals that could fulfill those conditions, and outline a proactive hazard identification strategy that considers long-range transport and the reversibility of chemical pollution.

    • Qualitative analysis of lake water dissolved organic matter (DOM) was carried out using electrospray ionization mass spectrometry (ESI MS) without prior clean-up or chromatography, showing the potential of ESI MS as a rapid method to analyse qualitative differences between DOM with different environmental properties. Spectra were evaluated in terms of the relative ion abundance in certain mass to charge ranges (m/z(range)), and the mass-to-charge average (m/z(average)). Lake water DOM from two different sources was used. In addition, DOM from both sources that had been subjected to microbial degradation under different oxygen regimes was analysed. ESI MS results were compared with those of UV absorbance and pollutant binding capacity (pyrene and atrazine). The results from all methods indicated qualitative differences between the DOM from different lakes, but only minor qualitative changes when comparing oxic and anoxic degradation. To detect those small differences the m/z(range) proved to be the more sensitive evaluation method, compared to the m/z(average).

    • Increased chemical use is causing a growing number of environmental problems and chemical products are pervasive in societies within animal and crop-based agriculture, in industrial processes and in households. National environmental targets, as well as the global chemical-related goals in the 2030 Agenda, call for the monitoring of chemical use and emissions. The growing international trade of goods, where use and regulation of chemical inputs vary highly between countries, complicates measurements. This paper addresses these issues by deriving a set of indicators on chemical use and emissions and connect the global impacts to a country's total consumption, here using the case of Sweden. The indicators are based on a hybrid model combining the multi-regional input-output analysis database EXIOBASE with data from the Swedish System of Economic and Environmental Accounts together with a novel set of environmental extensions. A review of databases is conducted and discussed in relation to the driver-pressure-state-impact-response (DPSIR) framework for indicators. Five indicators are calculated, showing the chemical use and emissions connected to consumption, both within a country and abroad. The indicators are: use of hazardous chemical products, use of pesticides, use of antimicrobial veterinary medicines, emissions of hazardous substances, and of the potential toxicity of these emissions. The results show that the impact of Swedish consumption in terms of use and emissions of hazardous substances is largely taking place outside the Swedish borders. Only 10e24% of the pressure from Swedish consumption is shown to occur within Sweden's borders, depending on the indicator. The use of hazardous chemical products and veterinary medicines related to Swedish consumption primarily takes place in other EU countries, whereas the use of pesticides as well as reported emissions of pollutants occur mainly in countries outside the EU. The results highlight the need for improved international accounting of chemical flows, as well as for strengthened policy frameworks to address cross-border impacts of consumption of hazardous chemical products.

However, there is a lack of economic evaluations of bullying prevention programs assessing the “value for money.” The aim of this study was to assess the cost-effectiveness of the Finnish bullying prevention program KiVa in comparison to “status quo” (treatment as usual) in a Swedish elementary school setting (grades 1 to 9). The cost-effectiveness analysis was carried out using a payer perspective based on a Markov cohort model. The costs of the program were measured in Swedish kronor and Euros, and the benefits were measured using two different metrics: (1) the number of victim-free years and (2) the number of quality adjusted life years (QALYs). Data on costs, probability transitions, and health-related quality of life measures were retrieved from published literature. Deterministic and probabilistic sensitivity analyses were carried out to establish the uncertainty of the cost-effectiveness results. The base-case analysis indicated that KiVa leads to an increased cost of €829 for a gain of 0.47 victim-free years per student. In terms of the cost per gained QALY, the results indicated a base-case estimate of €13,823, which may be seen as cost-effective given that it is lower than the typically accepted threshold value in Swedish health policy of around €50,000. Further research is needed to confirm the conclusions of this study, especially regarding the treatment effects of KiVa in different school contexts

    • Monocular depth estimation is a highly challenging problem that is often addressed with deep neural networks. While these use recognition of high level image features to predict reasonably looking depth maps,the result often has poor metric accuracy. Moreover,the standard feed forward architecture does not allow modification of the prediction based on cues other than the image.In this paper we relax the monocular depth estimation task by proposing a network that allows us to complement image features with a set of auxiliary variables. These allow disambiguation when image features are not enough to accurately pinpoint the exact depth map and can be thought of as a low dimensional parameterization of the surfaces that are reasonable monocular predictions. By searching the parameterization we can combine monocular estimation with traditional photoconsistency or geometry based methods to achieve both visually appealing and metrically accurate surface estimations. Since we relax the problem we are able to work with smaller networks than current architectures. In addition we design a self-supervised training scheme,eliminating the need for ground truth image depth-map pairs. Our experimental evaluation shows that our method generates more accurate depth maps and generalizes better than competing state-of-the-art approaches.
