  • Kidneys from marginal donors: views of patients on informed consent.

    • BACKGROUND:Considering the fact that donor age is a major risk factor for graft survival, and taking into account the importance attached to the principle of autonomy in the Swedish Health Care Law, we decided that allocation of kidneys from marginal donors should be restricted to patients who have given their informed consent. Written information was given to the patients on the waiting list for kidney transplantation in the southern part of Sweden. Patients were asked to state whether they would accept either a single kidney or dual transplantation kidneys from marginal donors. The aim of this study was to investigate the views of patients on the information they received about kidneys from marginal donors and their reaction to being asked to make a decision on this issue. METHODS:A questionnaire was posted to 61 patients who had already replied to the question of whether or not they would accept kidneys from a marginal donor for themselves. The median age of the patients was 52 years (range 22-74 years). Answers were given anonymously. RESULTS:Among the 53 respondents, 48 considered the information to be comprehensible. The extent of the information was considered sufficient by 43 patients. No patient thought that the information was too extensive. According to 41 patients, it is totally right to be asked to make a decision on this type of issue. Two patients thought it was totally wrong. Finally, 33 patients thought it was easy to make a decision on this issue. CONCLUSIONS:This study indicates that patients on the waiting list for kidney transplantation accept information on donor-related risk factors and most patients want to be involved in the decision concerning transplantation with a kidney from a marginal donor.

  • Xenotransplantation public perceptions: rather cells than organs.

    • The aim of this study was to describe some of the factors that might play a role in influencing attitude to xenotransplantation: first, the consideration of receiving cells and tissue from xenotransplants in relation to whole xeno-organs; secondly, the fact that there is greater uncertainty regarding the result and risk of infection associated with xenotransplantation than with allotransplantation. We also describe the attitude to research on xenotransplantation, and the relationship between the attitude to receiving a xenotransplant and an allotransplant. Finally, we describe the attitude to xenotransplantation in relation to treatment for renal failure and waiting-time for allotransplantation. A questionnaire was sent to randomly selected members of the public aged 18 to 75 (n=1000) and to all patients in the same age range who were waiting for kidney transplants in Sweden in the spring of 1998 (n=460). The response rate was 60% among the public and 87% among the patients. Both study groups were positive to a greater extent in their attitude to receiving cells and tissue than to receiving a whole organ such as a kidney. The response `rather positive' to receiving organs was generally favored by the public, whereas the most generally favored response to receiving cells and tissue was `very positive'. When there was suggested to be a greater uncertainty regarding the outcome with xenotransplantation compared with allotransplantation, the number of negative and uncertain respondents increased, both among the public and the patients. Eighty percent of the public and about 90% of the patients were in favor of continued research on xenotransplantation. Of those members of the public who responded, the attitude to receiving an organ from a human was positive in 86% of cases, with an emphasis on `very positive'. There was a moderate relation between the attitude to receiving an organ from a human and to receiving a xenotransplant. Among the patients, there was no systematic or strong relation between the attitude to xenotransplantation and the kind of dialysis treatment they were on. Neither was there any systematic or strong relation to the waiting-time. The overall impression is that the attitude to xenotransplantation seems to be most influenced by whether the xenotransplant would involve whole organs or cells and uncertainty regarding the outcome.

  • Serially connected EAP based tape yarns for in-air actuation using textile structures

    • Smart textiles that have the capability for actuation are of great interest for creating wearables and haptic devices. Through the use of textile fabric production processes electroactive polymeric materials in the form of film-based yarns can be integrated and combined with passive yarns to create soft, actuating fabrics. This way single EAP materials can be transformed into segments consisting of multiple EAP yarns working together. Furthermore, these segments can be positioned within a fabric to work individually or simultaneously in different patterns by use of incorporated conductive yarn paths. While the chase for additivity in force is a long-standing part of developing new actuator structures, so is the need for additivity in displacement motion. Here we construct an actuating textile fabric through the process of weaving that is able to operate in-air using polypyrrole-based tape yarns with choline acetate ionic liquid. Finding the balance between the weaving parameters turned out to be key. We found that in a vertically suspended arrangement, a three-segment serially connected fabric assembly demonstrated an accumulative effect in displacement and a joint-like motion behaviour. This opens up for more complicated motion patterns to be created through textile processing of EAP materials.

    • We have used seven different techniques to measure the anisotropic refractive index of poly(vinylcarbazole) films. These techniques are: two types of variable angle spectroscopic ellipsometry (VASE) with multiple sample analysis, Interference enhanced VASE, Transmittance combined with VASE, Polarised Reflectance, beta-scan VASE, and prism coupling. We have found the average ordinary and extraordinary indices at 633 nm to be no = nTE = 1.675 ± 0.008, and ne = nTM = 1.722 ± 0.018, respectively, consistent amongst methods and conclusive on the magnitude of Δn in polymer films.

  • Quantifying shedding of synthetic fibers from textiles; a source of microplastics released into the environment

    • Microplastics in the environment are a subject of intense research as they pose a potential threat to marine organisms. Plastic fibers from textiles have been indicated as a major source of this type of contaminant, entering the oceans via wastewater and diverse non-point sources. Their presence is also documented in terrestrial samples. In this study, the amount of microfibers shedding from synthetic textiles was measured for three materials (acrylic, nylon, polyester), knit using different gauges and techniques. All textiles were found to shed, but polyester fleece fabrics shed the greatest amounts, averaging 7360 fibers/m(-2)/L-1 in one wash, compared with polyester fabrics which shed 87 fibers/m(-2)/L-1. We found that loose textile constructions shed more, as did worn fabrics, and high twist yarns are to be preferred for shed reduction. Since fiber from clothing is a potentially important source of microplastics, we suggest that smarter textile construction, prewashing and vacuum exhaustion at production sites, and use of more efficient filters in household washing machines could help mitigate this problem.

    • Educators must select the best tools to teach anatomy to future physicians and traditionally, cadavers have always been considered the "gold standard" simulator for living anatomy. However, new advances in technology and radiology have created new teaching tools, such as virtual dissection, which provide students with new learning opportunities. Virtual dissection is a novel way of studying human anatomy through patient computed tomography (CT) scans. Through touchscreen technology, students can work together in groups to "virtually dissect" the CT scans to better understand complex anatomic relationships. This article presents the anatomic and pedagogic limitations of cadaveric dissection and explains what virtual dissection is and how this new technology may be used to overcome these limitations.

  • EMIm-OTf Ionogel Coated Fibres - Characterisation and Development, Aiming at Ionic Smart Textiles

    • Ions are prevalent within bioelectronics, as they are the main charge carriers in living systems. In contrast to electronic systems, ionic ones are closer to what can be found in our body; in muscles, neurons and nerves.Textiles are a much-used biomedical material, both in vivo and in vitro due to its membrane character, highly efficient area, softness, biocompatibility and biodegradability. Modifying the physicochemical properties of the core or the surface of textile has been reported a countless number of times, but still, its use in a bioelectrical context is limited.Fibres are the building blocks of textiles and what make textiles an architected class of material. Then ionically conductive fibres are of great interest.Here, we show the preparation of iono-conductive textile fibres through the (semi-)continuous dip-coating of ionogel on the cellulose-based viscose.Ionogels are composed of salts in liquid state and a 3-dimensional solid network, in our case an ionic liquid (IL), 1-Ethyl-3-methylimidazolium trifluoromethanesulfonate, commonly named EMIm OTf or EMIm Triflate, and a thiol acrylate network, allowing the mobility of the ions within or in/out of the gel. This specific combination is a first effort towards the development of ionic textile fibres and ionic smart textiles, as a variety of ILs with different cations and anions exists, potentially allowing a large number of different combinations.We investigate how the coating of this ionogel affects the mechanical properties as well as the conductivity in AC or DC arrangement and their relation to temperature and humidity. Also, the thermal stability and sensitivity of degradation of the fibre system is studied.Moreover, we introduce different textile structures, and potential applications directed to bioelectronics.

  • Investigating ionic liquid-based click-ionogels by thiol-ene photopolymerisation onto textile yarns/fibres

    • Electronic textiles’ primordial component are the connections that allow a circuit to be formed. As for today, the catalogue of conductive yarns is expanded to highly conductive metals such as copper, silver and steel, or electroconductive plastics composed of conductive polymers and electroconductive fillers such as metal particles or carbon allotropes.Ionic liquids are also able to carry electrical charges, and their capacity to conduct electricity has yet to be investigated as a yarn component, e.g. an ion conducting coating.Here, we report on attempts to coat ionic liquid-based click-ionogel on fibres, using thiol-ene reactions with the help of a photobase generator.Ionogel precursors, composed of plurithiol precursors, acrylate monomers and a triflate ionic-liquid, are applied on yarn and then cured by UV irradiation, initiating the Michael reaction and creating the thiol-acrylate-triflate network around the yarn.The aim of the present study is to prepare and characterise yarns coated with such ionogels, while developing a continuous yarn coating process.Several different ionogel compositions and different yarn topologies are investigated, comparing their structure, electrical conductivity, mechanical properties, thermal stability, behaviour to chemical reagents, as well as the different surface tensions and interfacial interactions.Textile processability is explored by the manufacture of simple fabrics.An application for those ionic conductive coating is the ion supply for electroactive polymers coated yarns that currently rely on electrolytes. This novel coating will render the light-weight property of textile valuable, and therefore broadening their application as wearables.

  • EPOS trial: The effect of air filtration through a plasma chamber on the incidence of surgical site infection in orthopaedic surgery: A study protocol of a randomised, double-blind, placebo-controlled trial

    • Introduction There is controversy regarding the importance of air-transmitted infections for surgical site infections (SSIs) after orthopaedic surgery. Research has been hindered by both the inability in blinding the exposure, and by the need for recruiting large enough cohorts. The aim of this study is to investigate whether using a new form of air purifier using plasma air purification (PAP) in operating rooms (ORs) lowers the SSI rate or not. Methods and analysis Multicentre, double-blind, cluster-randomised, placebo-controlled trial conducted at seven hospitals in 2017-2022. All patients that undergo orthopaedic surgery for minimum 30 min are included. Intervention group: patients operated in OR with PAP devices turned on. Control group: patients operated in OR with PAP devices turned off. Randomisation: each OR will be randomised in periods of 4 weeks, 6 weeks or 8 weeks to either have the devices on or off. Primary outcome: any SSI postoperatively defined as a composite endpoint of any of the following: use of isoxazolylpenicillin, clindamycin or rifampicin for 2 days or more, International Classification of Diseases codes or Nordic Medico-Statistical Committee codes indicating postoperative infection. In a second step, we will perform a chart review on those patients with positive indicators of SSI to further validate the outcome. Secondary outcomes are described in the Methods section. Power: we assume an SSI rate of 2%, an SSI reduction rate of 25% and we need approximately 45 000 patients to attain a power of 80% at a significance level of 0.05. Ethics and dissemination The study is approved by the Swedish Ethical Review Authority. The interim analysis results from the study will be presented only to the researchers involved unless the study thereafter is interrupted for whatever reason.   • Tissue reactions to polyethylene implants with different surface topography

    • This study investigates the importance of implant surface topography on soft tissue response. The tissue response in the rat abdominal wall to discs of low density polyethylene with smooth to coarse surfaces was evaluated after one, six or 12 weeks. Capsule thickness and immunohistochemical quantification of monocytes-macrophages were used as measures. The macrophage specific antibody ED1 was used for identification of newly recruited macrophages and the ED2 antibody for the mature tissue macrophages. The smoother surfaces gave a thicker capsule than the rougher surfaces, and at one week also larger total numbers of cells and ED1 positive macrophages at interface. The capsule thickness increased over time for the smooth and intermediate surface topographies. In contrast, the cell numbers generally decreased over time. In conclusion, a coarse surface elicited lesser tissue reaction compared with a smooth surface.

    • Two photoactive iron N-heterocyclic carbene complexes [FeII(btz)2(bpy)]2+ and [FeIII(btz)3]3+, where btz is 3,3’-dimethyl-1,1’-bis(p-tolyl)-4,4’-bis(1,2,3-triazol-5-ylidene) and bpy is 2,2’-bipyridine, have been investigated by Resonant Photoelectron Spectroscopy (RPES). Tuning the incident X-ray photon energy to match core-valence excitations provides a site specific probe of the electronic structure properties and ligand-field interactions, as well as information about the resonantly photo-oxidised final states. Comparing measurements of the Fe centre and the surrounding ligands demonstrate strong mixing of the Fe t 2 g levels with occupied ligand π orbitals but weak mixing with the corresponding unoccupied ligand orbitals. This highlights the importance of π-accepting and -donating considerations in ligand design strategies for photofunctional iron carbene complexes. Spin-propensity is also observed as a final-state effect in the RPES measurements of the open-shell Fe III complex. Vibronic coupling is evident in both complexes, where the energy dispersion hints at a vibrationally hot final state. The results demonstrate the significant impact of the iron oxidation state on the frontier electronic structure and highlights the differences between the emerging class of Fe III photosensitizers from those of more traditional Fe II complexes.

    • We present the first experimental study of the frontier orbitals in an ultrathin film of the novel hexa-carbene photosensitizer [Fe(btz)3]3+, where btz is 3,3′-dimethyl-1,1′-bis(p-tolyl)-4,4′-bis(1,2,3-triazol-5-ylidene). Resonant photoelectron spectroscopy (RPES) was used to probe the electronic structure of films where the molecular and oxidative integrities had been confirmed with optical and X-ray spectroscopies. In combination with density functional theory calculations, RPES measurements provided direct and site-selective information about localization and interactions of occupied and unoccupied molecular orbitals. Fe 2p, N 1s, and C 1s measurements selectively probed the metal, carbene, and side-group contributions revealing strong metal-ligand orbital mixing of the frontier orbitals. This helps explain the remarkable photophysical properties of iron-carbenes in terms of unconventional electronic structure properties and favorable metal-ligand bonding interactions - important for the continued development of these type of complexes toward light-harvesting and light-emitting applications.

  • A Randomized, Doubleblind, Placebo-Controlled, Study of Single-Dose Rituximab as Induction in Renal Transplantation

    • We performed a prospective, double blind, randomized, placebo-controlled multicenter study on the efficacy and safety of rituximab as induction therapy, together with tacrolimus, mycophenolate mofetil, and steroids. The primary endpoint was defined as acute rejection, graft loss, or death during the first 6 months. Secondary endpoints were creatinine clearance, incidence of infections, and incidence of rituximab-related adverse event. Results. We enrolled 140 patients (44 living donor and 96 deceased donor), and of those, 68 rituximab and 68 placebo patients fulfilled the study. In all the patients receiving rituximab, there was a complete depiction of CD 19/CD20 cells, whereas there was no change in the number of CD19/CD20 cells in the placebo group. There were 10 treatment failures in the rituximab group versus 14 in the placebo group (P=0.348). There were eight rejection episodes in the rituximab group versus 12 in the placebo group (P=0.317) Creatinine clearance was 66 +/- 22 mL/min in the study group and 67 +/- 23 mL/min in the placebo group. There was no difference in the number of bacterial infections, cytomegalovirus infections, and BK virus infections or fungal infections. Conclusion. We performed a placebo-controlled study of rituximab induction in renal transplantation. There was a tendency toward fewer and milder rejections during the first 6 months in the rituximab group. Although induction with one dose of rituximab induced a complete depletion B cells, there was no increase in the incidence of infectious complications or leukopenia and it seems safe, therefore, to conduct further studies on the use of rituximab in transplantation.
