  • Doxorubicin-melphalan with and without cisplatin in advanced ovarian cancer--ten-year survival results from a prospective randomized study by the Swedish Cooperative Ovarian Cancer Study Group

    • In a controlled prospective randomized study the regimen doxorubicin (A) 40 mg/m2 + melphalan (M) 0.4 mg/kg was compared with A + M + cisplatin (C) 50 mg/m2 given every four weeks in advanced ovarian cancer, FIGO stage III or IV and with serous or anaplastic histology. From 1981 to 1983, 300 patients entered the study and 295 patients were evaluable for response, toxicity and long-term survival. All patients were followed for at least 10 years. The majority of patients had large residual tumours >2 cm. Patients treated with MAC had a higher response rate compared with patients treated with MA (76% vs. 50%, p < 0.01) and treatment with MAC resulted in significantly more pathological complete responders than MA. There was a significant difference in median duration of response (19 months vs. 13 months, p < 0.006) and in median survival time (26 months vs. 19 months, p = 0.05). After 5- and 10 years a significant difference in progression-free and overall survival was found. The independent prognostic factors in this study were residual tumour after primary surgery, treatment with MAC, tumour grade, ascites, and stage. Objective and subjective side effects were significantly worse with MAC, although tolerable. In conclusion, this study shows that incorporating C into MA improves the duration of progression-free survival and overall survival in women with incompletely resected Stage III or Stage IV ovarian epithelial cancer. A 5- and 10-year survival of 25% and 18%, respectively, is impressive.

  • Long-term follow-up of the SBG 9401 study comparing tailored FEC-based therapy versus marrow-supported high-dose therapy

    • Background: The purpose was to investigate adjuvant marrow-supportive high-dose chemotherapy compared with an equitoxicity-tailored comparator arm. Patients and methods: Five hundred and twenty-five women below theage of 60 years with operated high-risk primary breast cancer were randomised to nine cycles of granulocyte colony-stimulating factor supported and individually tailored FEC (5-fluorouracil, epirubicin, cyclophosphamide), (n = 251) or standard FEC followed by marrow-supported high-dose therapy with CTCb (cyclophosphamide, thiotepa, carboplatin) therapy (n = 274), followed by locoregional radiotherapy and tamoxifen for 5 years. Results: There were 104 breast cancer relapses in the tailored FEC group versus 139 in the CTCb group (double triangular method by Whitehead, P = 0.046), with a median follow-up of all included patients of 60.8 months. The event-free survival demonstrated 121 and 150 events in the tailored FEC- and CTCb group, respectively [P = 0.074, hazard ratio (HR) 0.804, 95% confidence interval (CI) 0.633-1.022]. Ten patients in the tailored FEC regimen developed acute myeloid leukaemia (AML)/myelodysplasia (MDS). One hundred deaths occurred in the tailored FEC group and 121 in the CTCb group (P = 0.287, HR 0.866, 95% CI 0.665-1.129). Conclusion: The update of this study shows an improved outcome linked to the tailored FEC treatment in relation to breast cancer relapse, but also an increased incidence of AML/MDS. © 2007 Oxford University Press.

  • Levels and risk assessment for humans and ecosystems of platinum group elements in the airborne particles of some European cities

    • Traffic is the main source of platinum-group element (PGE) contamination in populated urban areas. There is increasing concern about the hazardous effects of these new pollutants for people and for other living organisms in these areas. Airborne and road dusts, as well as tree bark and grass samples were collected at locations in the European cities of Göteborg (Sweden), Madrid (Spain), Rome (Italy), Munich (Germany), Sheffield and London (UK). Today, in spite of the large number of parameters that can influence the airborne PGE content, the results obtained so far indicate significantly higher PGE levels at traffic sites compared with the rural or non-polluted zones that have been investigated (background levels). The average Pt content in airborne particles found in downtown Madrid, Göteborg and Rome is in the range 7.313.1 pg m−3. The ring roads of these cities have values in the range 4.117.7 pg m−3. In Munich, a lower Pt content was found in airborne particles (4.1 pg m−3). The same tendency has been noted for downtown Rh, with contents in the range 2.22.8 pg m−3, and in the range 0.83.0 and 0.3 pg m−3 for motorway margins in Munich. The combined results obtained using a wide-range airborne classifier (WRAC) collector and a PM-10 or virtual impactor show that Pt is associated with particles for a wide range of diameters.     • The ABC effect, a low-mass enhancement in the invariant pp mass, is observed in double-pionic fusion reactions leading to a bound nuclear system in the final state. From previous measurements there have been indications that this phenomenon is resctricted to the sigma channel of the pp system. With exclusive measurements of the pp -> d pi(+)pi(0) reaction at T-p = 1.1 GeV we demonstrate that, indeed, the ABC effect does not occur in the vector-isovector Delta Delta channel (rho channel) despite the fact that the Delta Delta excitation is oberved to be the dominant reaction process. We also show that this reaction is well described by a t-channel Delta Delta excitation with the subsequent decay into the vector-isovector Delta Delta channel given by the rho channel operator.

    • In most investigations the Roper resonance is sensed only very indirectly via complex partial-wave analyses. We find indications for its excitation in the invariant n pi(+) mass spectrum of the pp -> np pi(+) reaction at M approximate to 1360 MeV with a width of approximate to 150 MeV . The values fit very favorably to the most recent phase shift results as well as to the observations at BES. In the near-threshold two-pion production pp -> pp pi(0)pi(0) , where the Roper excitation and its subsequent decays via the routes N* -> Delta pi -> N pi pi and N* -> N sigma are the only dominant processes, we find its direct decay into the N sigma channel to be the by far dominating decay process -in favor of a monopole excitation of the Roper resonance.

    • The two-pion production in pp-collisions has been investigated in exclusive measurements from threshold up to T-p = 1.36 GeV. Total and differential cross sections have been obtained for the channels pn pi(+) pi(0), pp pi(+) pi(-), pp pi(0)pi(0) and also nn pi(+)pi(+). For intermediate incident energies T-p > 1 GeV, i.e. in the region, which is beyond the Roper excitation but at the onset of Delta Delta excitation the total pp pi(0)pi(0) cross section falls behind theoretical predictions by as much as an order of magnitude near 1.2 GeV, whereas the nn pi(+)pi(+) cross section is a factor of five larger than predicted. A model-unconstrained isospin decompostion of the cross section points to a significant contribution of an isospin 3/2 resonance other than the Delta(1232). As a possible candidate the Delta(1600) is discussed.

    • The pi(0)pi(0) production in pp-collisions has been investigated in exclusive and kinematically complete measurements from threshold up to T-p = 1.4 GeV. For incident energies T-p > 1 GeV, i.e. in the region beyond the Roper excitation, the Delta Delta excitation process takes over. The data are well explained by the t-channel Delta Delta process dominated by pion exchange. There is no low-mass enhancement (ABC effect) in the pi(0)pi(0)-invariant mass distribution beyond that given by the conventional t-channel Delta Delta process. This is also true for the limiting case, where the protons are in the quasi-bound He-2 state.

    • Many migratory species have experienced substantial declines that resulted from rapid and massive expansions of human structures and activities, habitat alterations and climate change. Migrants are also recognized as an integral component of biodiversity and provide a multitude of services and disservices that are relevant to human agriculture, economy and health. The plethora of recently published studies reflects the need for better fundamental knowledge on migrations and for better management of their ecological and human-relevant effects. Yet, where are we in providing answers to fundamental questions and societal challenges?. Engaging a broad network of researchers worldwide, we used a horizon-scan approach to identify the most important challenges which need to be overcome in order to gain a fuller understanding of migration ecology, and which could be addressed using radar aeroecological and macroecological approaches. The top challenges include both long-standing and novel topics, ranging from fundamental information on migration routes and phenology, orientation and navigation strategies, and the multitude of effects migrants may have on resident communities, to societal challenges, such as protecting or preventing migrant services and disservices, and the conservation of migrants in the face of environmental changes. We outline these challenges, identify the urgency of addressing them and the primary stakeholders – researchers, policy makers and practitioners, or funders of research.

  • Maintenance hemodialysis : patients´ experiences of their life situation

    • Aim. The aim of this study was to examine how patients suffering from CKD on maintenance haemodialysis experience their life situation. The focus was on how treatment encroaches on time and space and how patients experience care. The rationale was that this knowledge is necessary to provide professional support that takes into consideration a person's whole life situation. Background. The experiences of patients with chronic kidney disease (CKD) undergoing maintenance haemodialysis have been studied in many quantitative studies, which translate patients' subjective experiences into objectively quantifiable data. However, there are few qualitative studies examining the experiences of these patients' life situation and expressing their experiences within the context of a nursing and caregiver's perspective. Method. Data were collected by interviews with 41 patients between the ages of 29 and 86 years who participated in the study. A content analysis was used to identify common themes that describe the patients' experiences of their life situation. Results. Three main themes were identified, 'not finding space for living', 'feelings evoked in the care situation' and, 'attempting to manage restricted life'. The first theme 'not finding space for living' consisted of two sub-themes: 'struggling with time-consuming care' and 'feeling that life is restricted'. The second theme 'feelings evoked in the care situation' consisted of two sub-themes: 'sense of emotional distance' and 'feeling vulnerable'. Conclusions. The patients in this study indirectly expressed an existential struggle, indicating that encroachment of time and space were important existential dimensions of CKD. The findings indicated that caregivers were not always aware of this inducing a sense of emotional distance and a sense of vulnerability in the patients. Relevance to clinical practice. Caregivers in dialysis units have to consider haemodialysis patients' experience of a sense of emotional distance in their relationship to caregivers. Nurses and doctors need to create routines within nursing practice to overcome this.

    • Purpose: Multifocal soft contact lenses have been used to decrease the progression of myopia, presumably by inducing relative peripheral myopia at the same time as the central image is focused on the fovea. The aim of this study was to investigate how the peripheral optical effect of commercially available multifocal soft contact lenses can be evaluated from objective wavefront measurements. Methods: Two multifocal lenses with high and low add and one monofocal design were measured over the ±40° horizontal field, using a scanning Hartmann-Shack wavefront sensor on four subjects. The effect on the refractive shift, the peripheral image quality, and the depth of field of the lenses was evaluated using the area under the modulation transfer function as the image quality metric. Results: The multifocal lenses with a centre distance design and 2 dioptres of add induced about 0.50 dioptre of relative peripheral myopia at 30° in the nasal visual field. For larger off-axis angles the border of the optical zone of the lenses severely degraded image quality. Moreover, these multifocal lenses also significantly reduced the image quality and increased the depth of field for angles as small as 10°-15° Conclusions: The proposed methodology showed that the tested multifocal soft contact lenses gave a very small peripheral myopic shift in these four subjects and that they would need a larger optical zone and a more controlled depth of field to explain a possible treatment effect on myopia progression.

    • Breast cancer patients with c-erbB-2-positive tumours seem to benefit from anthracycline-based adjuvant chemotherapy. The predictive value of c-erbB-2 for taxane sensitivity is not yet clear. The purpose of this study was to assess whether c-erbB-2 expression is associated with clinical sensitivity to docetaxel (T) or sequential methotrexate and 5-fluorouracil (MF). A total of 283 patients with metastatic breast cancer were initially enrolled in a randomised multicentre trial comparing docetaxel with sequential MF in advanced breast cancer. Paraffin-embedded blocks of the primary tumour were available for 131 patients (46%). c-erbB-2 status was determined by immunohistochemistry using a polyclonal antibody to the c-erbB-2 protein. C-erbB-2 expression was scored in a semi-quantitative fashion using a 0 to 3+ scale. Staining scores 2+ or greater were considered positive, Response evaluation was performed according to World Health Organization (WHO) recommendations. Overall 54 (42%) patients had c-erbB-2-positive tumours. There was no association between treatment outcome and c-erbB-2 overexpression. The overall response rates (RR) (n = 128) among c-erbB-2 -negative and -positive patients were 35 and 44%, respectively (P=0.359). In the MF arm (n = 62), the RR was somewhat higher in the c-erbB-.2 overexpressors (33% versus 18%, P = 0. 18). In the docetaxel arm the RRs were very similar, regardless of the c-erbB-2 expression (53% versus 53%). While several studies have suggested a prognostic and putative predictive significance of c-erbB-2 overexpression in early breast cancer, the significance of c-erbB-2 expression as a predictive factor for response to various cytotoxic treatments in advanced breast cancer is still controversial. In this study, c-erbB-2 expression could not predict response to either MF or T. Thus, tumours over-expressing c-erbB-2 are not uniformly more sensitive to taxanes and c-erbB-2 expression cannot yet be applied clinically as a predictive factor for response in advanced breast cancer.

  • The scleritome of Paterimitra : an Early Cambrian stem group brachiopod from South Australia

    • Early Cambrian tommotiids are problematic fossil metazoans with external organophosphatic sclerites that have been considered to be basal members of the lophophorate stem group. Tommotiids are almost exclusively known from isolated or rarely fused individual sclerites, which made previous reconstructions of the actual organism highly conjectural. However, the recent discovery of the first articulated specimens of the tommotiid Eccentrotheca revealed a tubular sclerite arrangement (scleritome) that limited the possible life habit to sessile filter feeding and thus further supported a lophophorate affinity. Here, we report the first articulated specimens of a second tommotiid taxon, Paterimitra from the Early Cambrian of the Arrowie Basin, South Australia. Articulated specimens of Paterimitra are composed of two bilaterally symmetrical sclerite types and an unresolved number of small, asymmetrical and irregular crescent-shaped sclerites that attached to the anterior margin of the symmetrical sclerites. Together, the sclerites form an open cone in which the symmetrical sclerites are joined together and form a small posterior opening near the base of the scleritome, while the irregular crescent-shaped sclerites defined a broad anterior opening. The coniform scleritome of Paterimitra is interpreted to have attached to hard substrates via a pedicle that emerged through the small posterior opening ( sometimes forming a tube) and was probably a sessile filter feeder. The scleritome of Paterimitra can be derived from the tubular scleritome of Eccentrotheca by modification of basal sclerites and reduction in tube height, and probably represents a more derived member of the brachiopod stem group with the paired symmetrical sclerites possibly homologous to brachiopod valves.

    • OBJECTIVE: To investigate the potential of specific angiotensin II subtype 1 (AT1) receptor blockade to modify the mesenteric hemodynamic response to acute hypovolemia and retransfusion. DESIGN: Prospective, randomized, controlled experimental study. SETTING: University-affiliated animal research laboratory. SUBJECTS: Fasted, anesthetized, ventilated, juvenile domestic pigs of both sexes. INTERVENTIONS: Acute, graded hypovolemia by 20% and 40% of the total estimated blood volume followed by retransfusion in control animals (CTRL; n = 10) and animals pretreated with the AT1 receptor blocker candesartan (CAND; n = 10). MEASUREMENTS AND MAIN RESULTS: Invasive monitoring of arterial and central venous blood pressures, cardiac output, portal venous blood flow, and jejunal mucosal blood flow. Blood gases were repeatedly analyzed to calculate oxygen delivery and consumption. Thirty minutes after each level of hypovolemia at 20% and 40%, cardiac output was decreased in CTRL animals from a baseline of 2.9 +/- 0.1 to 1.8 +/- 0.2 and 1.1 +/- 0.2 L/min, with no differences compared with CAND animals. Cardiac output was restored to 3.0 +/- 0.3 L/min 30 mins after retransfusion in CTRL animals, with no significant intergroup differences. Baseline portal venous blood flow (Q(MES)) and jejunal mucosal perfusion (PU(JEJ)) were greater in CAND animals compared with CTRL animals. During graded hypovolemia, CAND animals maintained Q(MES) and PU(JEJ) at significantly higher levels compared with CTRL animals, particularly after 40% hemorrhage (+221% and + 244%, respectively, relative to the mean values in CTRL animals). The same pattern was observed after retransfusion. Moreover, the calculated mesenteric critical oxygen delivery was significantly greater in CTRL animals (74 mL/min) compared with CAND animals (34 mL/min). No animals died in the CAND group, whereas four animals died during 40% hypovolemia or retransfusion in the CTRL group. CONCLUSIONS: Specific AT1 blockade before acute hypovolemia significantly ameliorated mesenteric and, in particular, jejunal mucosal hypoperfusion. In addition, cardiovascular stability was improved, and mortality in conjunction with acute hypovolemia and retransfusion could be completely avoided. These findings support a fundamental role of the renin-angiotensin system in the mesenteric response to acute hypovolemia and indicate a substantial interventional potential for candesartan in conjunction with circulatory stress.
