  • God havsmiljö 2020 : Marin strategi för Nordsjön och Östersjön Del 2: God miljöstatus och miljökvalitetsnormer

    • Havsmiljöförordningens övergripande mål är att upprätthålla eller uppnå en god miljöstatus i de svenska förvaltningsområdena Nordsjön och Östersjön till år 2020. En av uppgifterna i den första förvaltningsperioden är att bestämma vad som kännetecknar god miljöstatus i respektive förvaltningsområde samt att ta fram miljökvalitetsnormer.   God miljöstatus baseras på ett ramverk av så kallade deskriptorer som anges i havsmiljödirektivet, det vill säga det EU-direktiv som i Sverige genomförs genom havsmiljöförordningen. Deskriptorerna beskriver god miljöstatus på en övergripande nivå för 11 temaområden. Till varje deskriptor hör en rad kriterier som anger vad som ska ingå i en bedömning av miljöstatus. I Sverige har god miljöstatus formulerats för samtliga 29 kriterier som ska beaktas enligt direktivet. Dessa kvalitativa beskrivningar anger vad som kännetecknar god miljöstatus i Nordsjön och Östersjön.   För att praktiskt bedöma om god miljöstatus har uppnåtts föreslås 37 nationella indikatorer. En uppsättning av indikatorer omfattar miljöns tillstånd och avspeglar ekosystemets komponenter i form av nyckelarter, samhällen, och livsmiljöer. En utgångspunkt vid val av indikatorer för miljöns tillstånd har varit koppling till de belastningar som i den inledande bedömningen av miljötillståndet i Nordsjön och Östersjön bedömts ha stor negativ påverkan på ekosystemet. En annan uppsättning indikatorer berör påverkan och belastning på miljön i form av tillförsel av näringsämnen och farliga ämnen, samt biologisk och fysisk störning av miljön. Tillsammans utgör indikatorerna ett verktyg för att följa utvecklingen av miljötillståndet och effekter av åtgärder i havsmiljön.  Vid val av indikatorer har arbetet delvis utgått från existerande miljöövervakning och redan utvecklade indikatorer. Med denna utgångspunkt uppfylls en rad av havsmiljödirektivets krav, bland annat en god uppföljning av effekter av tillförsel av näringsämnen. Funktionella indikatorer, det vill säga indikatorer som utvärderats och för vilka god miljöstatus har definierats, saknas dock för tio av havsmiljödirektivets 29 kriterier. För uppföljning av biologisk mångfald saknas bland annat miljöövervakning och metoder för att bedöma livsmiljöers tillstånd. För att kunna bedöma fysiska skador på havsbotten saknas en övergripande sammanställning av information om aktiviteter som påverkar havsbottnar samt metodik för att bedöma effekterna.  Det saknas också utvecklade indikatorer för det kriterium som berör uppföljning av storleks- och åldersstruktur hos fiskar. Brist på kunskap gör också att inga förslag på svenska indikatorer kan ges för effekter på levande organismer från marint avfall, undervattensbuller, och främmande arter samt att endast ett begränsat antal indikatorer tagits fram som speglar effekter av farliga ämnen. De indikatorer som fastställs i juli 2012 utgör således inte en slutlig lista för att följa upp havsmiljödirektivet. Bristerna kommer att beaktas i det fortsatta genomförandet av havsmiljöförordningen där nästa steg är att anpassa miljöövervakningsprogrammen till uppföljning av miljötillståndet med valda indikatorer senast år 2014 samt att ta fram åtgärdsprogram till år 2015.  God miljöstatus ska uppnås genom tillämpning av miljökvalitetsnormer det vill säga rättsligt bindande regler som avspeglar den lägsta godtagbara miljökvaliteten i Nordsjön och Östersjön. För att nå god miljöstatus har elva svenska miljökvalitetsnormer formulerats. Dessa miljökvalitetsnormer omfattar belastning i form av näringsämnen, farliga ämnen, främmande arter, uttag av arter, fysisk påverkan på havsbottnar och avfall i havsmiljön. Målsättningen har varit att utforma miljökvalitetsnormer som motsvarar alla de belastningar som i den inledande bedömningen har identifierats ha en stor påverkan på miljön.

  • Översyn av förordning om miljökvalitetsnormer för fisk- och musselvatten : Redovisning av regeringsuppdrag med Havs- och vattenmyndighetens analys av normerna och förslag till ändrad lagstiftning

    • Regeringen gav genom ett beslut den 22 oktober 2015 Havs- och vattenmyndigheten i uppdrag att göra en översyn av förordningen (2001:554) om miljökvalitetsnormer för fisk- och musselvatten (fisk- och musselvattenförordningen). Behovet av att göra denna översyn har framförts av Havs- och vattenmyndigheten, Naturvårdsverket och Länsstyrelsen i Västra Götalands län.  I rapporten görs en översyn av miljökvalitetsnormerna i förordningen (2001:554) om miljökvalitetsnormer för fisk- och musselvatten och en bedömning av om det finns normer som bör ändras, tas bort eller om samma skyddsnivå kan uppnås på annat sätt.  Havs- och vattenmyndigheten föreslår att fisk- och musselvattenförordningen upphävs. Som en följd av detta behöver ett antal föreskrifter upphävas eller ändras. Därutöver behöver viss vägledning tas fram för att täcka återstående skyddsbehov.Det är svårt att få en överblick av i vilken utsträckning fisk- och musselvattenförordningen har påverkat den övervakning som bedrivs och hur mycket övervakning som bedrivs enbart utifrån förordningens krav. Havs- och vattenmyndighetens bedömning är dock att ett upphävande av förordningen inte kommer att påverka den övervakning som utförs på ett betydande sätt.   Konsekvensutredningen visar att fisk- och musselvattenförordningen kan upphävas och att detta medför i huvudsak positiva effekter för såväl miljön som berörda myndigheter och verksamhetsutövare.

  • Öppna prioriteringar inom nya områden : logopedi, nutritionsbedömning, habilitering och arbetsterapi

    • Det finns fortfarande ett behov av att öka kunskapen om och stödja den praktiska tillämpningen av riksdagens riktlinjer för öppna prioriteringar inom svensk hälso- och sjukvård. Flera förslag på hur ett sådant stöd kan se ut har tagits fram de senaste åren. Spridning av goda exempel är ett sådant förslag, metodstöd ett annat (PrioriteringsCentrum 2007). En mer påtaglig form av metodstöd är den nationella modell som vuxit fram för att konkretisera innebörden i riktlinjerna (Carlsson m fl 2007). Den får idag anses som välbeprövad inom ett flertal områden och har bidragit till att samsynen och kommunicerbarheten kring prioriteringar har ökat i landet. Erfarenheter visar dock att det behövs pedagogisk vägledning i hur modellen kan tillämpas. För att möta upp efterfrågan på sådant metodstöd erbjuder Prioriteringscentrum handledning i grupp. Den första handledningsgruppen är nu avslutad och det är deltagarnas prioriteringsarbeten som presenteras i denna rapport i syfte att sprida konkreta exempel på försök att tillämpa prioriteringsriktlinjerna.I rapporten presenteras fyra prioriteringsarbeten med fokus på:   Regionsamverkan inom arbetsterapi   Logopedi   Yrkesspecifika prioriteringar på väg till teamet   Från projekt till integrerat redskapExemplet med prioriteringar i regionsamverkan utgörs av det prioriteringsarbete som genomförts i det s k femklövernätverket bestående av en samverkansgrupp för arbetsterapeuter i ledningsposition på sjukhusen i Uppland, Västmanland, Södermanland, Gävleborg och Dalarna. Arbetet var ett försök att skapa gemensamma prioriteringar i regionen för ett sjukdomsområde som kändes relevant. Valet kom att falla på arbetsterapi inom reumatologi. Arbetet har sedan huvudsakligen bedrivits i en projektgrupp, bestående av en representant från varje sjukhus där arbetet växlat mellan arbete på hemmaplan och avstämningsträffar i projektgruppen.Försöket har visat att det finns en samsyn inom regionen kring prioriteringar inom arbetsterapi och reumatologi. Säkerheten i prioriteringarna har ökat i och med att fem arbetsterapiorganisationer tillsammans bidragit med ett stort underlagsmaterial bl a genom att delge varandra sina kliniska erfarenheter. Förutsättningarna för en mer likartad vård i regionen har ökat. Arbetet har också gett upphov till frågor om i vilka situationer det är att föredra att prioriteringsarbete bedrivs lokalt, regionvis och/eller nationellt.

  • Practicing ’intellectus’ in rural entrepreneurship

    • In this paper we turn to recent philosophical investigations to analyse stories from entrepreneurial women in rural areas. They describe a variety of social activities they are engaged in to develop their companies, products and services, but also the local community, and society. These engagements are often described in passing, and not directly connected to the company according to conventional goal-oriented logic. Rather, they are seen as taken for granted– they are just done, and someone needs to do them. They concern care for others – for the children in the community, the elderly, the infrastructure, the sustainability of the industry etc. They are neither described as sacrifice, nor as benevolence, as is often emphasized in social entrepreneurship stories. Rather, these engagements signal something else. We argue that they illustrate a reflective practice of entrepreneurship, normally suppressed by an economic logic and described as play, passion and creativity in the sociologically inspired entrepreneurship literature.To better understand the role of reflexive practices in rural entrepreneurship, we turn to philosopher Jonna Bornemark who describes how humans in modern societies have become ‘prisoners’ of the measurable economic rationality (‘ratio’). With inspiration from the pre-renaissance philosopher, Nicholas Cusanus (1401-1464), she describes how the calculating ‘ratio’ has taken precedence over ‘intellectus’. Economic rationality (ratio) describes how we turn to rules of abstractions and generalization. Bornemark’s argument is that too much of ratio makes us loose contact with ourselves, others and the specific situation in a way that disables us to develop judgment. Instead we rely on external parameters to objectively guide our action. Practices built on intellectus, on the contrary, emphasize the subjective, emotional, temporary and our ability to ‘not know’, but to learn to cope with insecurity, instability, anxiety and find ways to act in such terrains. Bornemark’s point is that ratio and intellectus practices are interdependent – both are needed. But, in modern societies intellectus has been suppressed, overlooked and seen as state of lack of better knowledge.But, what if intellectus is a practice that is nurtured in rural contexts? A practice that not only makes it possible for rural areas to survive and thrive, but which we can learn from in the contemporary calls to change global society in a more thoughtful direction. By analyzing stories from 35 women pursuing different businesses and social activities, we set eyes at the question of if, and how, intellectus is practiced by rural entrepreneurial women.

  • Maternal obesity, duration of labor and the role of leptin

    • Background: The prevalence of obesity substantially increases in pregnant women. Maternal obesity is associated with adverse maternal and neonatal outcomes. The increased risk for cesarean section present in obese women has been related to potential impaired uterine contractility. The mechanism that underlies this theory is not clear. In vitro studies have shown that leptin, produced by adipose tissue and the placenta, exerts an inhibitory effect on myometrial contractility. The aim of this thesis was to evaluate the labor process in relation to maternal body mass index (BMI) and the clinical role of leptin in this process.Material and Methods: Studies I-IV are cohort studies. The first two studies analyze the association between labor duration and maternal BMI based on data from the Perinatal Revision South register and the Swedish Pregnancy Register. Study I included 63,829 nulliparous women with a spontaneous onset of labor between 1995 and 2009. Study II included 15,259 nulliparous women with induced labor between 2014 and 2017. In study III, the maternal leptin levels during and after pregnancy were analyzed in 343 obese women with respect to their obesity class (I-III) and degree of gestational weight gain (GWG). In study IV, the association between the maternal leptin levels measured in active labor and duration of the active phase of labor was analyzed in 914 women.Results: The duration of spontaneous labor significantly increased with an increasing maternal BMI; however, the duration of the pushing phase was inversely related to BMI. Time in induced labor increased with maternal BMI; however, the differences between the BMI categories were more pronounced in the latent phase than the active phase. Leptin levels were higher in women with obesity class III than women with class I during and after pregnancy. The degree of GWG in obese women was not associated with maternal leptin. No significant association between maternal leptin and the duration of the active phase of labor was identified in the adjusted analyses.Conclusions: Nulliparous obese women have a higher risk for a prolonged duration of spontaneous and induced labor. This is important to consider prior to diagnosing labor arrest that results in a cesarean delivery. As maternal leptin levels are increased with the degree of obesity during pregnancy, future research on the association of high maternal leptin levels and the duration of labor is warranted.

  • Feasibility studies of time-like proton electromagnetic form factors at PANDA at FAIR

    • Simulation results for future measurements of electromagnetic proton form factors at P ¯ ANDA (FAIR) within the PandaRoot software framework are reported. The statistical precision with which the proton form factors can be determined is estimated. The signal channel p¯ p→ e+e- is studied on the basis of two different but consistent procedures. The suppression of the main background channel, i.e.p¯ p→ π+π-, is studied. Furthermore, the background versus signal efficiency, statistical and systematical uncertainties on the extracted proton form factors are evaluated using two different procedures. The results are consistent with those of a previous simulation study using an older, simplified framework. However, a slightly better precision is achieved in the PandaRoot study in a large range of momentum transfer, assuming the nominal beam conditions and detector performance.

  • Low-grade intestinal inflammation two decades after pelvic radiotherapy.

    • Radiotherapy is effective in the treatment of cancer but also causes damage to non-cancerous tissue. Pelvic radiotherapy may produce chronic and debilitating bowel symptoms, yet the underlying pathophysiology is still undefined. Most notably, although pelvic radiotherapy causes an acute intestinal inflammation there is no consensus on whether the late-phase pathophysiology contains an inflammatory component or not. To address this knowledge gap, we examined the potential presence of a chronic inflammation in mucosal biopsies from irradiated pelvic cancer survivors.We biopsied 24 cancer survivors two to 20 years after pelvic radiotherapy, and four non-irradiated controls. Using tandem mass tag (TMT) mass spectrometry and mRNA sequencing (mRNA-seq), we charted proteomic and transcriptomic profiles of the mucosal tissue previously exposed to a high or a low/no dose of radiation. Changes in the immune cell populations were determined with flow cytometry. The integrity of the protective mucus layers were determined by permeability analysis and 16S rRNA bacterial detection.942 proteins were differentially expressed in mucosa previously exposed to a high radiation dose compared to a low radiation dose. The data suggested a chronic low-grade inflammation with neutrophil activity, which was confirmed by mRNA-seq and flow cytometry and further supported by findings of a weakened mucus barrier with bacterial infiltration.Our results challenge the idea that pelvic radiotherapy causes an acute intestinal inflammation that either heals or turns fibrotic without progression to chronic inflammation.   • COVID-19 in pregnancy—characteristics and outcomes of pregnant women admitted to hospital because of SARS-CoV-2 infection in the Nordic countries

    • Introduction: Population-based studies about the consequences of SARS-CoV-2 infection (COVID-19) in pregnancy are few and have limited generalizability to the Nordic population and healthcare systems. Material and methods: This study examines pregnant women with COVID-19 in the five Nordic countries. Pregnant women were included if they were admitted to hospital between 1 March and 30 June 2020 and had a positive SARS-CoV-2 PCR test ≤14 days prior to admission. Cause of admission was classified as obstetric or COVID-19-related. Results: In the study areas, 214 pregnant women with a positive test were admitted to hospital, of which 56 women required hospital care due to COVID-19. The risk of admission due to COVID-19 was 0.4/1000 deliveries in Denmark, Finland and Norway, and 3.8/1000 deliveries in the Swedish regions. Women hospitalized because of COVID-19 were more frequently obese (p < 0.001) and had a migrant background (p < 0.001) compared with the total population of women who delivered in 2018. Twelve women (21.4%) needed intensive care. Among the 56 women admitted due to COVID-19, 48 women delivered 51 infants. Preterm delivery (n = 12, 25%, p < 0.001) and cesarean delivery (n = 21, 43.8%, p < 0.001) were more frequent in women with COVID-19 compared with women who delivered in 2018. No maternal deaths, stillbirths or neonatal deaths were reported. Conclusions: The risk of admission due to COVID-19 disease in pregnancy was low in the Nordic countries. A fifth of the women required intensive care and we observed higher rates of preterm and cesarean deliveries. National public health policies appear to have had an impact on the risk of admission due to severe COVID-19 disease in pregnancy. Nordic collaboration is important in collecting robust data and assessing rare outcomes.

  • Experiences of eating disorders from the perspectives of patients, family members and health care professionals : a meta-review of qualitative evidence syntheses

    • BACKGROUND: Eating disorders are serious conditions that cause major suffering for patients and their families. Better knowledge about perceptions of eating disorders and their treatment, and which factors that facilitate or hinder recovery, is desired in order to develop the clinical work. We aimed to explore and synthesise experiences of eating disorders from the perspectives of those suffering from an eating disorder, their family members and health care professionals through an overarching meta-review of systematic reviews in the field.METHODS: A systematic literature search was conducted in the databases PubMed, PsycInfo, Scopus, and CINAHL. Inclusion criteria were systematic reviews of qualitative research on experiences, perceptions, needs, or desires related to eating disorders from the perspective of patients, family members or health care professionals. Systematic reviews that fulfilled the inclusion criteria were assessed for relevance and methodological limitations by at least two researchers independently. The key findings were analysed and synthesised into themes.RESULTS: We identified 17 systematic reviews that met our inclusion criteria. Of these, 13 reviews reported on the patients' perspective, five on the family members' perspective, and three on the health care professionals' perspective. The study population in the reviews was predominantly girls and young women with anorexia nervosa, whilst systematic reviews focusing on other eating disorders were scarce. The findings regarding each of the three perspectives resulted in themes that could be synthesised into three overarching themes: 1) being in control or being controlled, 2) balancing physical recovery and psychological needs, and 3) trusting relationships.CONCLUSIONS: There were several similarities between the views of patients, family members and health care professionals, especially regarding the significance of building trustful therapeutic alliances that also included family members. However, the informants sometimes differed in their views, particularly on the use of the biomedical model, which was seen as helpful by health care professionals, while patients and family members felt that it failed to address their psychological distress. Acknowledging these differences is important for the understanding of anorexia nervosa and other eating disorders, and may help clinicians to broaden treatment approaches to meet the expectations of patients and family members.

    • Objective: Intrapartum antibiotic prophylaxis (IAP) reduces the incidence of neonatal early onset group B streptococcal infections. The present study investigated if an automated PCR-assay, used bedside by the labor ward personnel was manageable and could decrease the use of IAP in a setting with a risk-based IAP strategy. Methods: The study comprises two phases. Phase 1 was a multicenter, randomized, controlled trial. Women with selected risk-factors were allocated either to PCR-IAP (prophylaxis given if positive or indeterminate) or IAP. A vaginal/rectal swab and superficial swabs from the neonate for conventional culture were also obtained. Phase 2 was non-randomized, assessing an improved version of the assay. Results: Phase 1 included 112 women in the PCR-IAP group and 117 in the IAP group. Excluding indeterminate results, the assay showed a sensitivity of 89% and a specificity of 90%. In 44 % of the PCR assays the result was indeterminate. The use of IAP was lower in the PCR group (53 versus 92%). Phase 2 included 94 women. The proportion of indeterminate results was reduced (15%). The GBS colonization rate was 31%. Conclusion: The PCR assay, in the hands of labor ward personnel, can be useful for selection of women to which IAP should be offered.

    • Detta förstudieprojekt har visat att vätgasdrift för timmerlastbilar ger något högre men ändå liknande kilometerkostnad som ren batteridrift, men snabbare tankning och längre körsträcka, vilket ger större flexibilitet för åkaren. Även biodrivmedel kan vara ett konkurrenskraftigt alternativ. Skogsindustrin är en av Sveriges största transportanvändare. För timmertransporter är lastbil det klart viktigaste transportslaget och skogsindustrins transporter motsvarar ca 17 % av Sveriges transporterade gods på väg. Ett alternativ för omställning av skogsindustrins transporter till fossilfrihet är förnybar vätgas, som kan produceras genom elektrolys med förnybar el. Precis som el ger vätgas inte upphov till några lokala emissioner vid användningen. Produktion av vätgas kan potentiellt ha synergier för skogsindustrins massabruk, som behov av syrgas och tillgång till överskottsel. Projektet har undersökt vätgas som alternativ för skogsindustrins transporter. Hela värdekedjan, inklusive produktion, komprimering, lagring, och användning inkluderas i analysen som beaktar kostnader, energieffektivitet och växthusgasutsläpp ur ett ”well-to wheel”-perspektiv. Studien inkluderar jämförelser med andra möjliga alternativ för att ställa om transporterna till fossilfrihet så som elektrifiering och biodrivmedel. Projektet har gett resultat som kommer att ligga till grund för en mer detaljerad projekteringsstudie inför ett framtida demonstrations- och pilotprojekt.     • In May 2024, the Swedish Reference Group on Antiviral Therapy updated the guidelines on management of HIV infection in pregnancy. The most important recommendations and revisions were: (i) ART during pregnancy should be started as early as possible and continue after delivery; (ii) Suppressive ART should normally not be modified; (iii) The treatment target of HIV RNA <20 copies/ml remains; (iv) Dolutegravir/emtricitabine/tenofovir DF is the first-line drug combination also in pregnant women and women planning pregnancy; (v) There is no evidence of an increased risk of neural tube defects associated with dolutegravir; (vi) Mode of delivery for women with effective ART and HIV RNA <200 copies/ml should follow standard obstetric procedures; (vii) Caesarean section is recommended if HIV RNA >= 200 copies/ml; (viii) Scalp electrode, foetal blood sampling and/or vacuum delivery should be used on strict indications, but does not necessitate intensified infant prophylaxis; (ix) Management and mode of delivery in case of premature or full-term rupture of membranes should follow standard obstetric procedures; (x) Recommended infant antiretroviral prophylaxis has been updated; (xi) The duration of infant antiretroviral prophylaxis (gestational age >= 35 weeks and mother on effective ART and HIV RNA <200 copies/ml) has been changed from 4 to 2 weeks; (xii) Infants born to women with HIV RNA >= 200 copies/ml should receive 4 weeks of combination prophylaxis; (xiii) Fertility evaluation and assisted reproduction should be offered to women on suppressive ART according to the same principles as for other women; (xiv) Women living with HIV should still be advised against breastfeeding; (xv) Women who nevertheless opt to breastfeed should be offered intensified support and follow-up.

  • Intramuscular pressure in the ECRB muscle of patients with lateral epicondylitis

    • An increased tension and a low intramuscular blood flow in the extensor carpi radialis (ECRB) muscle in patient with lateral epichondylitis has been suggested to contribute to the symptoms. The reason for the lower blood flow may be increased intramuscular pressure. The purpose of this pilot study was to determine intramuscular pressure and blood flow in affected and unaffected ECRB in ten patients with unilateral epicondylitis, with a mean duration of symptoms of 24 (13-48) months. Intramuscular pressure (IMP) was measured using the microcapillary infusion technique and intramuscular blood flow using the laser Doppler flowmetry technique, bilaterally with the non-affected arm serving as a control. The recordings were performed at rest, during isometric dorsal extension of the wrist for 1 min at 40 % and 2 min at 20 % of maximal voluntary contraction (MVC) and during recovery after contractions. At rest the median IMP was 7-8 mm Hg regardless of arm while the median intramuscular blood flow was lower on the affected side compared with the unaffected side (P=0.03). During contraction the IMP and blood flow increased regardless of arm. The IMP fell to pre-contraction level within 2 s after contraction, independent of arm and contraction force. Recovery, regarding the intramuscular blood flow, tended to be faster on the unaffected side than on the affected after contraction at 20 % MVC. After contraction at 40 % MVC the blood flow was equal on both sides. When the IMP value in the unaffected muscle was corrected for the higher force elicited compared to affected muscle the adjusted IMP was significantly lower (P=0.006) at 20 % MVC. In summary, affected ECRB has a lower intramuscular blood flow during rest and a higher intramuscular pressure in relation to the unaffected muscle at contraction for 2 min at 20 % MVC.

    • The microvascular response in extensor carpi radialis brevis (ECRB) muscle was studied in thirteen patients with unilateral epicondylitis. At rest the intramuscular blood flow was significantly lower in the affected arm than in the unaffected (P = 0.006). During dorsal extension for 1 min at 40% maximal voluntary contraction (MVC) and 2 min at 20% MVC, blood flow increased significantly in both arms (P = 0.02). Microcirculation remained above resting level both 1 and 2 min after contraction at 40% MVC but was back to the precontraction level 2 min after contraction at 20 % MVC. The reactive hyperaemic response after occlusion of the brachial artery for 5 min, showed no significant difference between the arms. The reason for the hampered microcirculation in ECRB at rest does not seem to be due to a decreased ability of the vessel wall to dilate. Other plausible reasons merits further investigation.

    • The exclusive charmonium production process in (P) over barp annihilation with an associated pi 0 meson (p) over barp -> J/psi pi(0) is studied in the framework of QCD collinear factorization. The feasibility of measuring this reaction through the J/psi -> e(+) e(-) decay channel with the AntiProton ANnihilation at DArmstadt ((P) over bar ANDA) experiment is investigated. Simulations on signal reconstruction efficiency as well as the background rejection from various sources including the (P) over barp -> pi(+)pi(-)pi(0) and (p) over barp -> J/psi pi(0)pi(0) reactions are performed with PANDAROOT, the simulation and analysis software framework of the (P) over bar ANDA experiment. It is shown that the measurement can be done at (P) over bar ANDA with significant constraining power under the assumption of an integrated luminosity attainable in four to five months of data taking at the maximum design luminosity.

    • Bound nuclear systems with two units of strangeness are still poorly known despite their importance for many strong interaction phenomena. Stored antiprotons beams in the GeV range represent an unparalleled factory for various hyperon-antihyperon pairs. Their outstanding large production probability in antiproton collisions will open the floodgates for a series of new studies of systems which contain two or even more units of strangeness at the PANDA experiment at FAIR. For the first time, high resolution gamma-spectroscopy of doubly strange Lambda Lambda-hypernuclei will be performed, thus complementing measurements of ground state decays of Lambda Lambda-hypernuclei at J-PARC or possible decays of particle unstable hypernuclei in heavy ion reactions. High resolution spectroscopy of multistrange Xi(-) -atoms will be feasible and even the production of Omega(-) -atoms will be within reach. The latter might open the door to the vertical bar S vertical bar = 3 world in strangeness nuclear physics, by the study of the hadronic Omega(-) -nucleus interaction. For the first time it will be possible to study the behavior of Xi(+) in nuclear systems under well controlled conditions.

  • Polycystic ovary syndrome and pregnancy complications : Focus on hyperandrogenism and comorbidity

    • Polycystic ovary syndrome (PCOS) is the most common endocrine disorder among women, affecting their lives in many ways. PCOS is characterised by ovulatory dysfunction, polycystic ovary morphology and hyperandrogenism, either clinical or biochemical. Women with PCOS face a higher risk of obstetric complications than women without PCOS. There are many factors that contribute to these complications, such as metabolic disturbances, insulin resistance, chronic inflammation, hyperandrogenism and factors related to infertility.The overall aim of the research presented in this thesis was to study factors that might affect the association between PCOS and pregnancy complications. The thesis consists of matched cohort studies based on data from the Uppsala Biobank of Pregnant Women (Papers I and II) and national register-based cohort studies (Papers III and IV). In the first two studies, we included women with PCOS (n = 159) and BMI-matched controls (n = 320), and the aim was to study the effect of high anti-Müllerian hormone (AMH) and testosterone on pregnancy complications. The third study (n = 138 219) explored whether the association between PCOS and preeclampsia depends on treated clinical hyperandrogenism and whether PCOS is associated with different subtypes of preeclampsia. In the fourth study (n = 281 806), the aim was to explore association and risk estimates for pregnancy outcomes in women with either or both PCOS and gestational diabetes mellitus (GDM).The main results were that women with PCOS have higher levels of AMH and testosterone and a higher free androgen index during second trimester pregnancy than non-PCOS controls. High AMH levels were not associated with adverse pregnancy outcome or birthweight. PCOS women with the highest testosterone levels had the highest risk for preeclampsia. Compared to non-PCOS controls, women with PCOS have increased risk of preeclampsia, especially the more severe subtypes of preeclampsia, early onset or with a birth of an infant born small for gestational age. With available data, we were unable to determine whether hyperandrogenism affects the risk of preeclampsia. The combination of PCOS and GDM exacerbates the risk of adverse pregnancy outcomes for both mother and infant compared with women with either PCOS or GDM alone.In conclusion, the research presented in this thesis adds important information about the association of PCOS and the more severe subtypes of preeclampsia and underpins the importance of an increased awareness of PCOS in antenatal care along with early screening for diabetes and hypertensive disorders.

    • Introduction: The Coronavirus 2019 Disease (COVID-19) pandemic reached the Nordic countries in March 2020. Public health interventions to limit viral transmission varied across different countries both in timing and in magnitude. Interventions indicated by an Oxford Stringency Index ≥50 were implemented early (March 13–17, 2020) in Denmark, Finland, Norway and Iceland, and on March 26, 2020 in Sweden. The aim of the current study was to assess the incidence of COVID-19-related admissions of pregnant women in the Nordic countries in relation to the different national public health strategies during the first year of the pandemic. Material and methods: This is a meta-analysis of population-based cohort studies in the five Nordic countries with national or regional surveillance in the Nordic Obstetric Surveillance System (NOSS) collaboration: national data from Denmark, Finland, Iceland and Norway, and regional data covering 31% of births in Sweden. The source population consisted of women giving birth in the included areas March 1–December 31, 2020. Pregnant women with a positive SARS-CoV-2 PCR test ≤14 days before hospital admission were included, and admissions were stratified as either COVID-19-related or non-COVID (other obstetric healthcare). Information about public health policies was retrieved retrospectively. Results: In total, 392 382 maternities were considered. Of these, 600 women were diagnosed with SARS-CoV-2 infection and 137 (22.8%) were admitted for COVID-19 symptoms. The pooled incidence of COVID-19 admissions per 1000 maternities was 0.5 (95% confidence interval [CI] 0.2 to 1.2, I2 = 77.6, tau2 = 0.68, P = 0.0), ranging from no admissions in Iceland to 1.9 admissions in the Swedish regions. Interventions to restrict viral transmission were less stringent in Sweden than in the other Nordic countries. Conclusions: There was a clear variation in pregnant women's risk of COVID-19 admission across countries with similar healthcare systems but different public health interventions to limit viral transmission. The meta-analysis indicates that early suppression policies protected pregnant women from severe COVID-19 disease prior to the availability of individual protection with vaccines.

Our results of the amplitude analysis are used to obtain a more precise measurement of the branching fraction of the D+s → K0SK−π+π+ decay, which is determined to be (1.46±0.05stat±0.05sys)%.

We find that the dominant intermediate process is D-s(+) -> rho(+)eta' and the significances of other resonant and nonresonant processes are all less than 3 sigma. The upper limits on the branching fractions of S-wave and P-wave nonresonant components are set to 0.10% and 0.74% at the 90% confidence level, respectively. In addition, the branching fraction of the D-s(+) -> pi(+)pi(0)eta' decay is measured to be (6.15 +/- 0.25(stat.) +/- 0.18(syst.))%, which receives significant contribution only from D-s(+) -> rho(+)eta' according to the amplitude analysis.

With the detection efficiency based on the amplitude analysis results, the absolute branching fraction is measured to be B(D-s(+) -> K+K-pi(+)) = (5.47 +/- 0.08(stat) +/- 0.13(sys))%.

The intermediate resonances f0(500), f0(980), and f0(1370) are observed for the first time in this channel.

    • The decay D+-> K-S(0)pi(+)pi(+)pi(-) is studied with an amplitude analysis using a data set of 2.93 fb(-1) of e(+)e(-) collisions at the psi(3770) peak accumulated by the BESIII detector. Intermediate states and nonresonant components, and their relative fractions and phases, have been determined. The significant amplitudes, which contribute to the model that best fits the data, are composed of five quasitwo-body decays K(S)(0)a(1)(1260)(+), (K) over bar (1)(1270)(0)pi(+) (K) over bar (1)(1400)(0)pi(+), (K) over bar (1)(1650)(0)pi(+), and (K) over bar (1460)(0)pi(+), a three-body decay K-S(0)pi(+)rho(0), as well as a nonresonant component K-S(0)pi(+)pi(+)pi(-). The dominant amplitude is K(S)(0)a(1)(1260)(+), with a fit fraction of (40.3 +/- 2.1 +/- 2.9)%, where the first and second uncertainties are statistical and systematic, respectively.

We use an e(+)e(-) collision data sample corresponding to an integrated luminosity of 3.19 fb(-1) collected with the BESIII detector at a center-of-mass energy of 4.178 GeV. We observe for the first time the W-annihilation dominant decays D-s(+) -> a(0)(980)(+)pi(0) and D-s(+) -> a(0)(980)(0)pi(+). We measure the absolute branching fraction B(D-s(+) -> a(0)(980)(+(0))pi(0(+)), a(0)(980)(+(0)) -> pi(+(0)())eta = (1.46 +/- 0.15(stat )+/- 0.23 sys)%, which is larger than the branching fractions of other measured pure W-annihilation decays by at least one order of magnitude. In addition, we measure the branching fraction of D-s(+) -> pi(+)pi(0)eta with significantly improved precision.

Our amplitude model includes the two-body decays D0 → ¯K*0ρ0, D0 → K−a+1(1260) and D0 → K−1(1270)π+, the three-body decays D0 →¯K*0π+π− and D0 → K−π+ρ0, as well as the four-body nonresonant decay D0 → K−π+π+π−. The dominant intermediate process is D0 → K−a+1(1260), accounting for a fit fraction of 54.6%.

    • Using 448.0 x 10(6) psi(3686) events collected with the BESIII detector, an amplitude analysis is performed for psi(3686) -> gamma chi(c1), chi(c1) ->eta pi(+)pi(-) decays. The most dominant two- body structure observed is a(0)(980)(+/-) pi(-/+); a(0)(980)(+/-) -> eta pi(+/-.) line shape is modeled using a dispersion relation, and a significant nonzero a(0) (980) coupling to the eta'pi channel is measured. We observe chi(c1) -> a(2)(1700)pi production for the first time, with a significance larger than 17 sigma. The production of mesons with exotic quantum numbers, J(PC) = 1(-+), is investigated, and upper limits for the branching fractions chi(c1) -> pi(1)(1400)(+/-)pi(-/+) , chi(c1) -> pi(1)(1600)(+/-)pi(-/+) and chi(c1) -> pi 1(2015)(+/-)pi(-/+) with subsequent pi(1)(X)(+/-) -> eta pi(+/-) decay, are determined.

    • Based on a sample of 1.31 x 10(9) J/Psi events collected with the BESIII detector, an amplitude analysis of the isospin-violating decays eta' -> pi(+)pi(-)pi(0) and eta' -> pi(0)pi(0)pi(0) is performed. A significant P-wave contribution from eta' -> rho(+/-)eta(-/+) is observed for the first time in eta' -> pi(+)pi(-)pi(0). The branching fraction is determined to be B(eta' -> rho(+/-)pi(-/+)) = (7.44 +/- 0.60 +/- 1.26 +/- 1.84) x 10(-4), where the first uncertainty is statistical, the second systematic, and the third model dependent. In addition to the nonresonant S-wave component, there is a significant sigma meson component. The branching fractions of the combined S-wave components are determined to be B(eta' -> pi(+)pi(-)pi(0))(S) = (37.63 +/- 0.77 +/- 2.22 +/- 4.48) x 10(-4) and B(eta' -> pi(0)pi(0)pi(0)) = (35.22 +/- 0.82 +/- 2.54) x 10(-4), respectively. The latter one is consistent with previous BESIII measurements.

    • An amplitude analysis of the KSKS system produced in radiative J/psi decays is performed using the (1310.6 +/- 7.0) x 10(6) nip decays collected by the BESIII detector. Two approaches are presented. A mass-dependent analysis is performed by parametrizing the KSKS invariant mass spectrum as a sum of Breit-aligner line shapes. Additionally, a mass-independent analysis is performed to extract a piecewise function that describes the dynamics of the KSKS system while making minimal assumptions about the properties and number of poles in the amplitude. The dominant amplitudes in the mass-dependent analysis include the f(0)(1710), f(0)(2200), and f(2)'(1525). The mass-independent results, which are made available as input for further studies, are consistent with those of the mass-dependent analysis and are useful for a systematic study of hadronic interactions. The branching fraction of radiative J/psi decays to KSKS is measured to be (8.1 +/- 0.4) x 10(-4), where the uncertainty is systematic and the statistical uncertainty is negligible.

    • Using the data sets taken at center-of-mass energies above 4 GeV by the BESIII detector at the BEPCII storage ring, we search for the reaction e(+)e(-) -> gamma(ISR) X(3872) -> gamma(ISR)pi(+)pi(-) J/psi via the Initial State Radiation technique. The production of a resonance with quantum numbers J(PC) = 1(++) such as the X(3872) via single photon e(+)e(-) annihilation is forbidden, but is allowed by a next-to-leading order box diagram. We do not observe a significant signal of X(3872), and therefore give an upper limit for the electronic width times the branching fraction Gamma B-X(3872)(ee)(X(3872) -> pi(+)pi(-) J/psi) < 0.13 eVat the 90% confidence level. This measurement improves upon existing limits by a factor of 46. Using the same final state, we also measure the electronic width of the psi(3686) to be Gamma(psi)(ee)(3686) ee = 2213 +/- 18(stat) +/- 99(sys) eV.

    • Using 2.93 fb(-1) of data taken at 3.773 GeV with the BESIII detector operated at the BEPCII collider, we study the semileptonic decays D+ -> (K) over bar (0)e(+)nu(e) and D+ -> pi(0)e(+)nu(e). We measure the absolute decay branching fractions B(D+ -> (K) over bar (0)e(+)nu(e)) = (8.60 +/- 0.06 +/- 0.15) x 10(-2) and B(D+ -> pi(0)e(+)nu(e)) = (3.63 +/- 0.08 +/- 0.05) x 10(-3), where the first uncertainties are statistical and the second systematic. We also measure the differential decay rates and study the form factors of these two decays. With the values of |V-cs| and |V-cd| from Particle Data Group fits assuming Cabibbo-Kobayashi-Maskawa (CKM) unitarity, we obtain the values of the form factors at q(2) = 0, f(+)(K)(0) = 0.725 +/- 0.004 +/- 0.012, and f(+)(pi)(0) = 0.622 +/- 0.012 +/- 0.003. Taking input from recent lattice QCD calculations of these form factors, we determine values of the CKM matrix elements |V-cs| = 0.944 +/- 0.005 +/- 0.015 +/- 0.024 and |V-cd| = 0.210 +/- 0.004 +/- 0.001 +/- 0.009, where the third uncertainties are theoretical.

The branching fraction of J/Psi -> KSKL is determined to be B(J/Psi -> KSKL) = (1.93 +/- 0.01 (stat) +/- 0.05 (syst)) x 10(-4), which significantly improves on previous measurements. No clear signal is observed for the J/Psi -> KSKS process, and the upper limit at the 95% confidence level for its branching fraction is determined to be B(J/Psi -> KSKS) < 1.4 x 10(-8), which improves on the previous searches by 2 orders in magnitude and reaches the order of the Einstein-Podolsky-Rosen expectation.

The product branching fractions of (psi 3686) -> gamma chi(c1), chi(c1) -> gamma J/psi (3686) -> gamma chi(c2), chi(c2) -> gamma J/psi and the branching fractions of chi(c1) -> gamma J/psi and chi(c2) -> gamma J/psi are also presented.

  • Complete Measurement of the Λ Electromagnetic Form Factors

    • The exclusive process e(+)e(-) -> Lambda(Lambda) over bar, with Lambda -> p pi(-) and (Lambda) over bar -> (p) over bar pi(+), has been studied at root s = 2.396 GeV for measurement of the timelike Lambda electric and magnetic form factors, G(E) and G(M). A data sample, corresponding to an integrated luminosity of 66.9 pb(-1), was collected with the BESIII detector for this purpose. A multidimensional analysis with a complete decomposition of the spin structure of the reaction enables a determination of the modulus of the ratio R = vertical bar G(E)/G(M)vertical bar and, for the first time for any baryon, the relative phase Delta Phi = Phi(E) - Phi(M). The resulting values are R = 0.96 +/- 0.14(stat) +/- 0.02(syst) and Phi(F) = 37 degrees +/- 12 degrees (stat) +/- 6 degrees (syst), respectively. These are obtained using the recently established and most precise value of the asymmetry parameter alpha(Lambda) = 0.750 +/- 0.010 measured by BESIII. In addition, the cross section is measured with unprecedented precision to be sigma = 118.7 +/- 5.3(stat) +/- 5.1(syst) pb, which corresponds to an effective form factor of vertical bar G vertical bar = 0.123 +/- 0.003(stat) +/- 0.003(syst). The contribution from two-photon exchange is found to be negligible. Our result enables the first complete determination of baryon timelike electromagnetic form factors.

    • We present a study of the process e(+)e(-) -> pi(+/-) (D (D) over bar*)(-/+) using data samples of 1092 pb(-1) at root s = 4.23 GeV and 826 pb(-1) at root s = 4.26 GeV collected with the BESIII detector at the BEPCII storage ring. With full reconstruction of the D meson pair and the bachelor pi(+) in the final state, we confirm the existence of the charged structure Z(c) (3885)(-/+) in the (D (D) over bar*)(-/+) system in the two isospin processes e(+)e(-) -> pi(+DD)-D-0*(-) and e(+)e(-) -> pi+D-D*(0). By performing a simultaneous fit, the statistical significance of Zc(3885)(-/+) signal is determined to be greater than 10 sigma, and its pole mass and width are measured to be M-pole = (3881.7 +/- 1.6(stat) +/- 1.6(syst)) MeV/c(2) and Gamma(pole) = (26.6 +/- 2.0(stat) +/- 2.1(syst)) MeV, respectively. The Born cross section times the (D (D) over bar*)(-/+) branching fraction (sigma(e(+)e(-) -> pi(+/-)Z(c)(3885)(-/+)) x Br(Z(c)(3885)(-/+) -> (D (D) over bar*)(-/+) )) is measured to be (141.6 +/- 7.9(stat) +/- 12.3(syst)) pb at root s = 4.23 GeV and (108.4 +/- 6.9(stat) +/- 8.8(syst)) pb at root s = 4.26 GeV. The polar angular distribution of the pi(+) - Z(c)(3885)(-/+) system is consistent with the expectation of a quantum number assignment of J(P) = 1(+) for Z(c)(3885)(-/+).

    • The cross sections of the process e(+)e(-) -> (KSKL0)-K-0 are measured at fifteen center-of-mass energies root s from 2.00 to 3.08 GeV with a total integrated luminosity of 582 pb(-1) and using the BESIII detector at the Beijing Electron Positron Collider. The results are found to be consistent with those obtained by BABAR. A resonant structure around 2.2 GeV is observed, with a mass and width of 2273.7 +/- 5.7 +/- 19.3 MeV/c(2) and 86 +/- 44 +/- 51 MeV, respectively, where the first uncertainties are statistical and the second ones are systematic. The product of its radiative width (Gamma(e+e-)) with its branching fraction to (KSKL0)-K-0 (Br-KS0KL0) is 0.9 +/- 0.6 +/- 0.7 eV.

    • Based on 14.7 fb(-1) of e(+)e(-) annihilation data collected with the BESIII detector at the BEPCII collider at 17 different center-of-mass energies between 3.7730 GeV and 4.5995 GeV, Born cross sections of the two processes e(+)e(-) -> p (p) over bar eta and e(+)e(-) -> p (p) over bar omega are measured for the first time. No indication of resonant production through a vector state V is observed, and upper limits on the Born cross sections of e(+)e(-) -> p (p) over bar eta and e(+)e(-) -> p (p) over bar omega at the 90% confidence level are calculated for a large parameter space in resonance masses and widths. For the current world average parameters of the psi(4230) of m = 4.2187 GeV=c(2) and Gamma = 44 MeV, we find upper limits on resonant production of the p (p) over bar eta and p (p) over bar omega final states of 7.5 pb and 10.4 pb at the 90% CL, respectively.

    • Using data samples with a total integrated luminosity of 20.1 fb(-1) collected by the BESIII detector operating at the BEPCII collider, the cross section of the process e(+) e(-) -> pi(+) pi(-) psi(3686) is measured at center-of-mass energies between 4.0076 and 4.6984 GeV. The measured cross section is consistent with previous results, and with much improved precision. A fit to the measured energy-dependent cross section, which includes three Breit-Wigner functions and a nonresonant contribution, confirms the existence of the charmonium-like states Y(4220), Y(4390), and Y(4660). This is the first observation of the Y(4660) at the BESIII experiment.

    • We measure the Born cross sections of the process e(+)e(-) -> K+ K- K+ K- at center-of-mass (c.m.) energies, root s, between 2.100 and 3.080 GeV. The data were collected using the BESIII detector at the BEPCII collider. An enhancement at root s = 2.232 GeV is observed, very close to the e(+)e(-) -> Lambda(Lambda) over bar production threshold. A similar enhancement at the same c.m. energy is observed in the e(+)e(-) -> phi K+ K- cross section. The energy dependence of the K+ K- K+ K- and phi K+ K- cross sections differs significantly from that of e(+)e(-) -> phi pi(+)pi(-).

The dependence of the cross section on root s shows a resonant structure with mass of (4218.5 +/- 1.6(stat) +/- 4.0(syst)) MeV/c(2) and width of (28.2 +/- 3.9(stat) +/- 1.6(syst)) MeV, respectively. This observation confirms and improves upon the result of a previous study. The angular distribution of the e(+) e(-) ->omega chi(C0) process is extracted for the first time.
