    • Immune checkpoint inhibitor therapies targeting PD‐1/PD‐L1 are now the standard of care in oncology across several hematologic and solid tumor types, including triple negative breast cancer (TNBC). Patients with metastatic or locally advanced TNBC with PD‐L1 expression on immune cells occupying ≥1% of tumor area demonstrated survival benefit with the addition of atezolizumab to nab‐paclitaxel. However, concerns regarding variability between immunohistochemical PD‐L1 assay performance and inter‐reader reproducibility have been raised. High tumor‐infiltrating lymphocytes (TILs) have also been associated with response to PD‐1/PD‐L1 inhibitors in patients with breast cancer (BC). TILs can be easily assessed on hematoxylin and eosin–stained slides and have shown reliable inter‐reader reproducibility. As an established prognostic factor in early stage TNBC, TILs are soon anticipated to be reported in daily practice in many pathology laboratories worldwide. Because TILs and PD‐L1 are parts of an immunological spectrum in BC, we propose the systematic implementation of combined PD‐L1 and TIL analyses as a more comprehensive immuno‐oncological biomarker for patient selection for PD‐1/PD‐L1 inhibition‐based therapy in patients with BC. Although practical and regulatory considerations differ by jurisdiction, the pathology community has the responsibility to patients to implement assays that lead to optimal patient selection. We propose herewith a risk‐management framework that may help mitigate the risks of suboptimal patient selection for immuno‐therapeutic approaches in clinical trials and daily practice based on combined TILs/PD‐L1 assessment in BC.

  • Adaptable transmitter for discrete and continuous variable quantum key distribution

    • We present a versatile transmitter capable of performing both discrete variable and continuous variable quantum key distribution protocols (DV-QKD and CV-QKD, respectively). Using this transmitter, we implement a time-bin encoded BB84 DV-QKD protocol over a physical quantum channel of 47 km and a GG02 CV-QKD protocol with true local oscillator over a 10.5 kin channel, achieving secret key rates of 4.1 kbps and 1 Mbps for DV- and CV-QKD, respectively. The reported transmitter scheme is particularly suitable for re-configurable optical networks where the QKD protocol is selected to optimize the performance according to the parameters of the links.

    • The secure transfer of information is critical to the ever-increasing demands of the digital world. Continuous-variable quantum key distribution (CV-QKD) is a promising technology that can provide high secret key rates over metropolitan areas, using conventional telecom components. In this study, we demonstrate the utilization of CV-QKD over a 15 km multi-core fiber (MCF), in which we take advantage of one core to remotely frequency lock Bob's local oscillator with Alice's transmitter. We also demonstrate the capacity of the MCF to boost the secret key rate by parallelizing CV-QKD across multiple cores. Our results indicate that MCFs are promising for the metropolitan deployment of QKD systems.

  • Detection of Genomically Aberrant Cells within Circulating Tumor Microemboli (CTMs) Isolated from Early-Stage Breast Cancer Patients

    • Circulating tumor microemboli (CTMs) are clusters of cancer cells detached from solid tumors, whose study can reveal mechanisms underlying metastatization. As they frequently comprise unknown fractions of leukocytes, the analysis of copy number alterations (CNAs) is challenging. To address this, we titrated known numbers of leukocytes into cancer cells (MDA-MB-453 and MDA-MB-36, displaying high and low DNA content, respectively) generating tumor fractions from 0–100%. After low-pass sequencing, ichorCNA was identified as the best algorithm to build a linear mixed regression model for tumor fraction (TF) prediction. We then isolated 53 CTMs from blood samples of six early-stage breast cancer patients and predicted the TF of all clusters. We found that all clusters harbor cancer cells between 8 and 48%. Furthermore, by comparing the identified CNAs of CTMs with their matched primary tumors, we noted that only 31–71% of aberrations were shared. Surprisingly, CTM-private alterations were abundant (30–63%), whereas primary tumor-private alterations were rare (4–12%). This either indicates that CTMs are disseminated from further progressed regions of the primary tumor or stem from cancer cells already colonizing distant sites. In both cases, CTM-private mutations may inform us about specific metastasis-associated functions of involved genes that should be explored in follow-up and mechanistic studies.

  • Plug-and-play continuous-variable quantum key distribution for metropolitan networks

    • We report a plug-and-play continuous variable quantum key distribution system (CV-QKD) with Gaussian modulated quadratures and a true local oscillator. The proposed configuration avoids the need for frequency locking two narrow line-width lasers. To minimize Rayleigh back-scattering, we utilize two independent fiber strands for the distribution of the laser and the transmission of the quantum signals. We further demonstrate the quantum-classical co-existing capability of our system by injecting high-power classical light in both fibers. A secret key rate up to 0.88 Mb/s is obtained by using two fiber links of 13 km and up to 0.3 Mb/s when adding 4 mW of classical light in the optical fiber used for transmitting the quantum signal. The reported performance indicates that the proposed QKD scheme has the potential to become an effective low-cost solution for metropolitan optical networks.
