    • Four highly deformed structures in the odd-odd nucleus 13661Pm75 were observed via the 105Pd(35Cl,2p2n) reaction at 180 and 173 MeV using the GAMMASPHERE γ-ray spectrometer and the Microball charged-particle detector array. Quadrupole moment measurements were performed on all of the bands. In contrast to lighter odd-Ζ Pm and Pr nuclei, bands based on the g9/2[404]9/2 proton orbital were not observed. Instead, the four observed sequences are assigned as a coupling of an i13/2 neutron with the low-Ω h11/2 and mixed d5/2g7/2 orbitals. Comparisons with neighboring highly deformed structures are discussed and cranked Nilsson-Strutinsky calculations for 136Pm are presented.

While examples of special terminating states have been identified in a number of erbium isotopes, almost nothing is known about the states lying beyond band termination. In the present work the high-spin structure of Er-157 has been studied using the Gammasphere spectrometer. The subject of triaxial superdeformation and 'wobbling' modes in Lu nuclei has rightly attracted a great deal of attention. Very recently, four strongly or superdeformed (SD) sequences have been observed in Hf-174 and ultimate cranker calculations predict, such structures may have significant triaxial deformation. We have performed two experiments in an attempt to verify the possible triaxial nature of these bands. A lifetime measurement was performed to confirm the large (and similar) deformation of the bands. In addition, a high-statistics, thin-target experiment was run to search for linking transitions between the SD bands and possible wobbling modes.

  • Evolution of collectivity and evidence of octupole correlations in Br 73

    • High-spin states in the Br73 nucleus have been populated via the Cr50(Si28,αp)Br73 fusion evaporation reaction with a beam energy of 90 MeV. The deexciting gamma rays were detected using the Indian National Gamma Array (INGA). Using the γ-γ coincidence technique, two new bands and eight new interconnecting transitions have been added. The strong interconnecting E1 transitions, between positive and negative parity bands, ensure the existence of octupole correlations at low spin. Line shapes have been observed for 17 transitions, which were analyzed by the Doppler-shift attenuation method to determine the lifetime of excited states of the yrast negative parity band and its signature partner band along with the positive parity band. The deduced transitional quadrupole moments Qt for the ground state band decrease with increasing spin, with their values ranging from 2.88 to 1.00 eb. A similar trend in the quadrupole moment has also been observed for the signature partner as well as for the positive parity band. This decrease in Qt with increasing spin for these bands is interpreted in terms of the cranked Nilsson-Strutinsky model and total Routhian surface calculations, which indicate possible band termination at higher spin.

  • Coulomb excitation of a Am-242 isomeric target: E2 and E3 strengths, rotational alignment, and collective enhancement

    • A 98% pure Am-242m ( K = 5(-), t(1/2) = 141 years) isomeric target was Coulomb excited with a 170.5-MeV Ar-40 beam. The selectivity of Coulomb excitation, coupled with the sensitivity of Gammasphere plus CHICO, was sufficient to identify 46 new states up to spin 18h in at least four rotational bands; 11 of these new states lie in the isomer band, 13 in a previously unknown yrast K-pi = 6(-) rotational band, and 13 in a band tentatively identified as the predicted yrast K-pi = 5(+) band. The rotational bands based on the K-pi = 5- isomer and the 6(-) bandhead were populated by Coulomb excitation with unexpectedly equal cross sections. The gamma-ray yields are reproduced by Coulomb excitation calculations using a two-particle plus rotor model (PRM), implying nearly complete Delta K = 1 mixing of the two almost-degenerate rotational bands, but recovering the Alaga rule for the unperturbed states. The degeneracy of the 5(-) and 6(-) bands allows for precise determination of the mixing interaction strength V, which approaches the strong-mixing limit; this agrees with the 50% attenuation of the Coriolis matrix element assumed in the model calculations. The fractional admixture of the I-K(pi) = 6(6)(-) state in the nominal 6(5)(-) isomer band state is measured within the PRM as 45.6(-1.1)(+0.3)%. The E2 and M1 strengths coupling the 5(-) and 6(-) bands are enhanced significantly by the mixing, while E1 and E2 couplings to other low-K bands are not measurably enhanced. The yields of the 5(+) band are reproduced by an E3 strength of approximate to 15 W.u., competitive with the interband E2 strength. Alignments of the identified two-particle Nilsson states in Am-242 are compared with the single-particle alignments in Am-241.

  • High-angular-momentum structures in Zn-64

    • High-angular-momentum states in Zn-64 were populated in the Ca-40(Si-28,4p) reaction at a beam energy of 122 MeV. Evaporated, light, charged particles were identified by the Microball, while gamma rays were detected using the Gammasphere array. The main focus of this paper is on two strongly coupled, collective bands. The yrast band, which was previously known, has been linked to lower-lying states establishing the excitation energies and angular momenta of in-band states for the first time. The newly identified excited band decays to the yrast band but firm angular-momentum assignments could not be made. In order to interpret these structures cranked-Nilsson-Strutinsky calculations have been performed. The calculations have been extended to account for the distribution of nucleons within a configuration. The yrast collective band is interpreted as based on the pi(f(7/2))(-1)(p(3/2)f(5/2))(2)(g(9/2))(1)nu(p(3/2)f(5/2))(4)(g(9/2))(2) configuration. There are several possible interpretations of the second band but it is difficult to distinguish between the different possibilities.

    • Excited states in the Rh-99 nucleus were populated using the fusion-evaporation reaction As-75(Si-28,2p2n) at E-lab = 120 MeV and the de-excitations were investigated through in-beam gamma-ray spectroscopic techniques using the INGA spectrometer consisting of 18 clover detectors. The observed band structures are discussed in the framework of tilted axis cranking shell-model calculations. Level structures at low energies are identified as resulting from the rotational bands based on the pi p(1/2) and pi g(9/2) configurations. The Delta I = 1 coupled bands are observed at higher excitation energies and have been interpreted as based on the pi g(9/2) circle times nu g(7/2) circle times nu d(5/2), pi p(1/2) circle times nu h(11/2) circle times nu d(5/2) and pi g(9/2) circle times nu h(11/2) circle times nu g(7/2) configurations. Calculations based on cranked Nilsson- Strutinsky (CNS) formalism have been performed to interpret the favoured states with I-pi = (41/2(-), 43/2(-)) and (51/2(-), 53/2(-)) as maximal spin aligned states built on the valence space nu(d(5/2)g(7/2))(15/2)(3), (17/2)(h(11/2))(11/2)(1) configuration combined with the fully-aligned pi(g(9/2))(25/2)(5) configuration and the pi(g(9/2))(15/2)(5) configuration with one anti-aligned g(9/2) proton, respectively.

    • A superdeformed rotational band has been identified in the N = Z nucleus 36Ar, firmly linked to known low-spin states, and observed to its high-spin termination at Iπ = 16+. Lifetime measurements by the Doppler shift attenuation method establish a large low-spin deformation (β2 ≈ 0.46) and a decrease in the collectivity as the band approaches termination. Comparisons with cranked Nilsson-Strutinsky and large-scale spherical shell model calculations lead to a consistent description of the band based on a configuration in which four particles are promoted to the pf shell. With two major shells active for both protons and neutrons, yet a valence space dimension small enough to be approached from the shell model perspective, this band offers an excellent opportunity to investigate the microscopic structure of collective rotational motion in nuclei.

Cranked Nilsson-Strutinsky and spherical shell model calculations assign the band to a configuration in which four pf-shell orbitals are occupied, leading to a low-spin deformation β2 ≈ 0.45. Two major shells are active for both protons and neutrons, yet the valence space remains small enough to be confronted with the shell model. This band thus provides an ideal case to study the microscopic structure of collective rotational motion.

  • High mortality within 90 days of diagnosis in patients with Cushing's syndrome: results from the ERCUSYN registry

    • Objective: Patients with Cushing's syndrome (CS) have increased mortality. The aim of this study was to evaluate the causes and time of death in a large cohort of patients with CS and to establish factors associated with increased mortality. Methods: In this cohort study, we analyzed 1564 patients included in the European Registry on CS (ERCUSYN); 1045 (67%) had pituitary-dependent CS, 385 (25%) adrenal-dependent CS, 89 (5%) had an ectopic source and 45 (3%) other causes. The median (IQR) overall follow-up time in ERCUSYN was 2.7 (1.2-5.5) years. Results: Forty-nine patients had died at the time of the analysis; 23 (47%) with pituitary-dependent CS, 6 (12%) with adrenal-dependent CS, 18 (37%) with ectopic CS and two (4%) with CS due to other causes. Of 42 patients whose cause of death was known, 15 (36%) died due to progression of the underlying disease, 13 (31%) due to infections, 7 (17%) due to cardiovascular or cerebrovascular disease and 2 due to pulmonary embolism. The commonest cause of death in patients with pituitary-dependent CS and adrenal-dependent CS were infectious diseases (n = 8) and progression of the underlying tumor (n = 10) in patients with ectopic CS. Patients who had died were older and more often males, and had more frequently muscle weakness, diabetes mellitus and ectopic CS, compared to survivors. Of 49 deceased patients, 22 (45%) died within 90 days from start of treatment and 5 (10%) before any treatment was given. The commonest cause of deaths in these 27 patients were infections (n = 10; 37%). In a regression analysis, age, ectopic CS and active disease were independently associated with overall death before and within 90 days from the start of treatment. Conclusion: Mortality rate was highest in patients with ectopic CS. Infectious diseases the commonest cause of death soon after diagnosis, emphasizing the need for careful vigilance at that time, especially in patients presenting with concomitant diabetes mellitus.

  • East-West gradient in the incidence of inflammatory bowel disease in Europe: the ECCO-EpiCom inception cohort

    • Objective The incidence of inflammatory bowel disease (IBD) is increasing in Eastern Europe. The reasons for these changes remain unknown. The aim of this study was to investigate whether an East–West gradient in the incidence of IBD in Europe exists.Design A prospective, uniformly diagnosed, population based inception cohort of IBD patients in 31 centres from 14 Western and eight Eastern European countries covering a total background population of approximately 10.1 million people was created. One-third of the centres had previous experience with inception cohorts. Patients were entered into a low cost, web based epidemiological database, making participation possible regardless of socioeconomic status and prior experience.Results 1515 patients aged 15 years or older were included, of whom 535 (35%) were diagnosed with Crohn's disease (CD), 813 (54%) with ulcerative colitis (UC) and 167 (11%) with IBD unclassified (IBDU). The overall incidence rate ratios in all Western European centres were 1.9 (95% CI 1.5 to 2.4) for CD and 2.1 (95% CI 1.8 to 2.6) for UC compared with Eastern European centres. The median crude annual incidence rates per 100 000 in 2010 for CD were 6.5 (range 0–10.7) in Western European centres and 3.1 (range 0.4–11.5) in Eastern European centres, for UC 10.8 (range 2.9–31.5) and 4.1 (range 2.4–10.3), respectively, and for IBDU 1.9 (range 0–39.4) and 0 (range 0–1.2), respectively. In Western Europe, 92% of CD, 78% of UC and 74% of IBDU patients had a colonoscopy performed as the diagnostic procedure compared with 90%, 100% and 96%, respectively, in Eastern Europe. 8% of CD and 1% of UC patients in both regions underwent surgery within the first 3 months of the onset of disease. 7% of CD patients and 3% of UC patients from Western Europe received biological treatment as rescue therapy. Of all European CD patients, 20% received only 5-aminosalicylates as induction therapy.Conclusions An East–West gradient in IBD incidence exists in Europe. Among this inception cohort—including indolent and aggressive cases—international guidelines for diagnosis and initial treatment are not being followed uniformly by physicians.

  • Spectroscopy along flerovium decay chains. II. Fine structure in odd-A 289Fl

    • Fifteen correlated α-decay chains starting from the odd-A superheavy nucleus 289Fl were observed following the fusion-evaporation reaction 48Ca+244Pu. The results call for at least two parallel α-decay sequences starting from at least two different states of 289Fl. This implies that close-lying levels in nuclei along these chains have quite different spin-parity assignments. Further, observed α-electron and α-photon coincidences, as well as the α-decay fine structure along the decay chains, suggest a change in the ground-state spin assignment between 285Cn and 281Ds. Our experimental results, on the excited level structure of the heaviest odd-N nuclei to date, provide a direct testing ground for theory. This is illustrated by comparison with new nuclear structure calculations based on the symmetry-conserving configuration mixing theory.

    • A superdeformed rotational structure is reported in the stable nucleus Zn-68. It has a transition quadrupole moment (Q(t)) of 2.5(-0.4)(+0.7) e b (beta(2) approximate to 0.4), as measured by the residual Doppler shift method. Theoretical calculations suggest that two different configurations are possible candidates for this band. One is built without h(11/2) neutrons, as seen in superdeformed bands in the A similar to 60 region, while the other involves one h(11/2) neutron in analogy with superdeformed bands in the A similar to 80 region. The second of these candidate configurations is preferred, thus establishing a new, neutron-rich (relative to the line of beta stability) region of superdeformation with Z similar to 30 and N similar to 38.

  • Persistence of collective behavior at high spin in the N=88 nucleus Tb-153

    • Excited states in the N = 88 nucleus Tb-153 were observed up to spin similar to 40 in an experiment utilizing the Gammasphere array. The Tb-153 states were populated in a weak alpha 4n evaporation channel of the Cl-37 + Sn-124 reaction. Two previously known sequences were extended to higher spins, and a new decoupled structure was identified. The pi h(11/2) band was observed in the spin region where other N = 88 isotopes exhibit effects of prolate to oblate shape changes leading to band termination along the yrast line, whereas Tb-153 displays a persistent collective behavior. However, minor perturbations of the very highest state in both signatures of this h(11/2) band are observed, which perhaps signal the start of the transition towards band termination.

    • High-spin states in the neutron-deficient nucleus In-107 were studied via the Ni-58(Cr-52,3p) reaction. In-beam gamma rays were measured using the JUROGAM detector array. A rotational cascade consisting of ten gamma-ray transitions, which decays to the 19/2(+) level at 2.002 MeV, was observed. The band exhibits the features typical for smooth terminating bands that also appear in rotational bands of heavier nuclei in the A similar to 100 region. The results are compared with total Routhian surface and cranked Nilsson-Strutinsky calculations.

    • Excited states in the N=Z+1 nucleus Kr-73 have been investigated using the Ca-40(Ar-36,2pn) and Ca-40(Ca-40,alpha2pn) reactions at 145 and 160 MeV, respectively. gamma rays were detected using the Gammasphere array and events were recorded in coincidence with charged-particle and neutron detectors. The three previously observed bands were extended to high spin, and a new unfavored positive-parity band has been observed. The alignment characteristics and decay properties of the bands are all consistent with large-deformation prolate rotation, with no clear evidence for oblate bands or shape coexistence. This is quite different from neighboring Kr-72,Kr-74, indicating a strong shape-stabilizing role for the valence neutron. The experimental results are compared to extended total Routhian surface, cranked Nilsson Strutinsky, and cranked relativistic mean-field calculations. The results suggest that the paired calculations lack some important physics. Neutron-proton correlations may be the missing ingredient. There is also evidence for an unusual band crossing in the negative-parity bands, which may indicate the presence of T=0 pairing correlations. At high spin all the models can reproduce the experimental data.

    • For a level scheme investigation of Gd-142 an experiment with the gamma -spectrometer EUROBALLIII has been carried out and lifetimes have been measured with EUROBALL IV using the Doppler-shift attenuation method. The high-spin states have been populated in these experiments by means of the(99)Ru(Ti-48, 2p3n) reaction at a beam energy of 240MeV and the Sn-114(S-32, 2p2n) reaction at 160MeV, respectively. Reduced E2 transition probabilities B(E2) were determined for 15 members of four quadrupole bands. For the interpretation of the positive-parity even-spin quadrupole bands, calculations in the cranked Nilsson-Strutinsky as well as interacting boson models have been performed. From the former calculations it was concluded that the high-spin states of the (+ , 0)(1) band in Gd-142 represent a triaxial nucleus rotating around the longest principal axis.

Band terminating states were identified in the spin range I= (36-48)ℏ, and were found to compete with collective rotational cascades up to the highest observed spins. Several "sidebands" feeding the terminating structures were identified as well. A band dominated by M1 transitions was observed to terminate at Iπ = 42-. The data are interpreted within the framework of configuration-dependent cranked Nilsson-Strutinsky calculations without pairing.

    • High-spin states in the odd-odd N = Z nucleus 3774Rb37 were studied using the 40Ca(40Ca,αnp) reaction. A previously observed odd-spin T = 0 band has been extended to Iπ = (31+) and an even-spin T = 0 band has been observed for the first time to Iπ = (22+); both have a π(g9/2)⊗ν(g9/2) structure. A strongly coupled low-spin T = 0, K = 3 band has been interpreted as being based upon a π[312]3/2 ⊗ ν[312]3/2 configuration. Cranked relativistic Hartree-Bogoliubov calculations, which are corrected for the t = 1 np-pair field by restoring isospin symmetry, reproduce the observed spectrum. These new results provide evidence for the existence of an isovector pair field that contains a neutron-proton component with the proper strength for ensuring isospin conservation.

    • Rotational bands in 73Br have been investigated up to spins of I = 65/2 using the EUROBALL III spectrometer. One of the negative-parity bands displays the highest rotational frequency ℏω= 1.85 MeV reported to date in nuclei with A ≥ 25. At high frequencies, the experimental J(2) dynamic moment of inertia for all bands decreases to very low values, J(2) ≤ 10ℏ2 MeV-1. The bands are described in the configuration-dependent cranked Nilsson-Strutinsky model. The calculations indicate that one of the negative-parity bands is observed up to its terminating single-particle state at spin 63/2. This result establishes the first band termination in the A ≈ 70 mass region.

    • The Sn-124(Cl-37, 6n gamma) fusion-evaporation reaction at a bombarding energy of 180 MeV has been used to significantly extend the excitation level scheme of Ho-155(67)88. The collective rotational behavior of this nucleus breaks down above spin I similar to 30 and a fully aligned noncollective (band terminating) state has been identified at I-pi = 79/2(-). Comparison with cranked Nilsson-Strutinsky calculations also provides evidence for core-excited noncollective states at I-pi = 87/2(-) and (89/2(+)) involving particle-hole excitations across the Z = 64 shell gap. A similar core-excited state in Er-156(68)88 at I-pi = (46(+)) is also presented.

The experimental results are compared to cranked Nilsson-Strutinsky calculations and possible configurations for the band are discussed.

The level scheme of I-125 was extended considerably. In particular, maximally aligned states involving all eleven particles outside the Sn-114 core were observed. Comparison with cranked Nilsson-Strutinsky calculations suggests that three of these states are the final I-max states in terminating bands with all spin vectors aligned along a common axis. In two of these, one spin vector is antialigned and points in the opposite direction. In one of the states two spin vectors are antialigned. This is the first observation of a state with such a structure.

    • Flerovium isotopes (element Z = 114) were produced in the fusion-evaporation reactions 48Ca+242,244Pu and studied with an upgraded TASISpec decay station placed in the focal plane of the gas-filled separator TASCA at the GSI Helmholtzzentrum für Schwerionenforschung in Darmstadt, Germany. Twenty-nine flerovium decay chains were identified by means of correlated implantation, α decay, and spontaneous fission events. Data analysis aspects and statistical assessments, primarily based on measured rates of various events, which laid the foundation for the comprehensive spectroscopic information on the flerovium decay chains, are presented in detail. Various decay scenarios of an excited state observed in 282Cn are examined in depth with the help of GEANT4 simulations and assessed by predictions of beyond mean-field calculations including triaxial shape degrees of freedom. Previous, revised, and newly derived fission probabilities of even-even superheavy nuclei are compared with various theoretical predictions.

The good level of agreement between theory and experiment suggests that these structures can be interpreted in terms of configurations that involve three g92 particles and that both possess noncollective terminating states.

    • The nucleus Ge-68 has been studied by gamma-ray spectroscopy following its population at high spin in the reaction Ca-40(S-32,4p) Ge-68. The reaction channel was selected with the Microball array and gamma rays were detected with the Gammasphere array. The level scheme is very complex, reflecting the many different, and presumably mixed, excitation modes in this nucleus. Nevertheless, there appear to be some simplifications in the spin range above 18 (h) over bar where we have identified a superdeformed band and several terminating bands. The results are compared with a cranked Nilsson-Strutinsky model without pairing.

    • Background: Level sequences of rotational character have been observed in several nuclei in the A=60 mass region. The importance of the deformation-driving πf7/2 and νg9/2 orbitals on the onset of nuclear deformation is stressed. Purpose: A measurement was performed in order to identify collective rotational structures in the relatively neutron-rich Ni62 isotope. Method: The Mg26(Ca48,2α4nγ)Ni62 complex reaction at beam energies between 275 and 320 MeV was utilized. Reaction products were identified in mass (A) and charge (Z) with the fragment mass analyzer (FMA) and γ rays were detected with the Gammasphere array. Results: Two collective bands, built upon states of single-particle character, were identified and sizable deformation was assigned to both sequences based on the measured transitional quadrupole moments, herewith quantifying the deformation at high spin. Conclusions: Based on cranked Nilsson-Strutinsky calculations and comparisons with deformed bands in the A=60 mass region, the two rotational bands are understood as being associated with configurations involving multiple f7/2 protons and g9/2 neutrons, driving the nucleus to sizable prolate deformation.

    • Importance  Neurofilament light protein (NfL) is elevated in cerebrospinal fluid (CSF) of a number of neurological conditions compared with healthy controls (HC) and is a candidate biomarker for neuroaxonal damage. The influence of age and sex is largely unknown, and levels across neurological disorders have not been compared systematically to date.Objectives  To assess the associations of age, sex, and diagnosis with NfL in CSF (cNfL) and to evaluate its potential in discriminating clinically similar conditions.Data Sources  PubMed was searched for studies published between January 1, 2006, and January 1, 2016, reporting cNfL levels (using the search terms neurofilament light and cerebrospinal fluid) in neurological or psychiatric conditions and/or in HC.Study Selection  Studies reporting NfL levels measured in lumbar CSF using a commercially available immunoassay, as well as age and sex.Data Extraction and Synthesis  Individual-level data were requested from study authors. Generalized linear mixed-effects models were used to estimate the fixed effects of age, sex, and diagnosis on log-transformed NfL levels, with cohort of origin modeled as a random intercept.Main Outcome and Measure  The cNfL levels adjusted for age and sex across diagnoses.Results  Data were collected for 10 059 individuals (mean [SD] age, 59.7 [18.8] years; 54.1% female). Thirty-five diagnoses were identified, including inflammatory diseases of the central nervous system (n = 2795), dementias and predementia stages (n = 4284), parkinsonian disorders (n = 984), and HC (n = 1332). The cNfL was elevated compared with HC in a majority of neurological conditions studied. Highest levels were observed in cognitively impaired HIV-positive individuals (iHIV), amyotrophic lateral sclerosis, frontotemporal dementia (FTD), and Huntington disease. In 33.3% of diagnoses, including HC, multiple sclerosis, Alzheimer disease (AD), and Parkinson disease (PD), cNfL was higher in men than women. The cNfL increased with age in HC and a majority of neurological conditions, although the association was strongest in HC. The cNfL overlapped in most clinically similar diagnoses except for FTD and iHIV, which segregated from other dementias, and PD, which segregated from atypical parkinsonian syndromes.Conclusions and Relevance  These data support the use of cNfL as a biomarker of neuroaxonal damage and indicate that age-specific and sex-specific (and in some cases disease-specific) reference values may be needed. The cNfL has potential to assist the differentiation of FTD from AD and PD from atypical parkinsonian syndromes.

The aim of this study was to investigate differences in exposure to environmental factors prior to diagnosis in Eastern and Western European IBD patients.Methods: The EpiCom cohort is a population-based, prospective inception cohort of 1560 unselected IBD patients from 31 European countries covering a background population of 10.1 million. At the time of diagnosis patients were asked to complete an 87-item questionnaire concerning environmental factors.Results: A total of 1182 patients (76%) answered the questionnaire, 444 (38%) had Crohn's disease (CD), 627 (53%) ulcerative colitis (UC), and 111 (9%) IBD unclassified. No geographic differences regarding smoking status, caffeine intake, use of oral contraceptives, or number of first-degree relatives with IBD were found. Sugar intake was higher in CD and UC patients from Eastern Europe than in Western Europe while fibre intake was lower (p < 0.01). Daily consumption of fast food as well as appendectomy before the age of 20 was more frequent in Eastern European than in Western European UC patients (p < 0.01). Eastern European CD and UC patients had received more vaccinations and experienced fewer childhood infections than Western European patients (p < 0.01).Conclusions: In this European population-based inception cohort of unselected IBD patients, Eastern and Western European patients differed in environmental factors prior to diagnosis.     • Background and Aims: The EpiCom study and inception cohort was initiated in 2010 in 31 centers from 14 Western and 8 Eastern European countries, covering a 10.1 million person background population. Our aim was to investigate whether there is a difference between Eastern and Western Europe in health care and education of patients with inflammatory bowel disease (IBD).Methods: A quality of care (QoC) questionnaire was developed in the EpiCom group consisting of 16 questions covering 5 items: time interval between the onset of symptoms and diagnosis, information, education, empathy and access to health care providers.Results: Of 1,515 patients, 947 (217 east/730 west) answered the QoC questionnaire. Only 23% of all patients had knowledge about IBD before diagnosis. In Eastern Europe, significantly more patients searched out information about IBD themselves (77% vs. 68%, p < 0.05), the main source was the Internet (92% vs. 88% p = 0.23). In Western Europe, significantly more patients were educated by nurses (19% vs. 1%, p < 0.05), while in Eastern Europe, gastroenterologists were easier to contact (80% vs. 68%, p < 0.05).Conclusion: Health care differed significantly between Eastern and Western Europe in all items, but satisfaction rates were high in both geographic regions. Because of the low awareness and the rising incidence of IBD, general information should be the focus of patient organizations and medical societies. The aim was prospectively to assess and validate the pattern of HRQoL in an unselected, population-based inception cohort of IBD patients from Eastern and Western Europe.Methods: The EpiCom inception cohort consists of 1560 IBD patients from 31 European centres covering a background population of approximately 10.1 million. Patients answered the disease specific Short Inflammatory Bowel Disease Questionnaire (SIBDQ) and generic Short Form 12 (SF-12) questionnaire at diagnosis and after one year of follow-up.Results: In total, 1079 patients were included in this study. Crohn's disease (CD) patients mean SIBDQ scores improved from 45.3 to 55.3 in Eastern Europe and from 44.9 to 53.6 in Western Europe. SIBDQ scores for ulcerative colitis (UC) patients improved from 44.9 to 57.4 and from 48.8 to 55.7, respectively. UC patients needing surgery or biologicals had lower SIBDQ scores before and after compared to the rest, while biological therapy improved SIBDQ scores in CD. CD and UC patients in both regions improved all SF-12 scores. Only Eastern European UC patients achieved SF-12 summary scores equal to or above the normal population.Conclusion: Medical and surgical treatment improved HRQoL during the first year of disease. The majority of IBD patients in both Eastern and Western Europe reported a positive perception of disease-specific but not generic HRQoL. Biological therapy improved HRQoL in CD patients, while UC patients in need of surgery or biological therapy experienced lower perceptions of HRQoL than the rest.

  • Rotational structures and their evolution with spin in Gd-152

    • The fusion-evaporation reaction involving a 175 MeV S-36 beam and a Sn-124 target was performed, and the emitted gamma rays were observed with the Gammasphere spectrometer. Significant additions to the level scheme of Gd-152 were made in spite of the relative weakness of the alpha 4n exit channel, being only similar to 2% of the total fusion cross-section. The high-spin behavior of Gd-152 was compared with that of other N=88 nuclei. A striking similarity was observed with Dy-154 and it is therefore suggested that the angular-momentum-induced shape changes that take place in Dy-154 also occur in Gd-152 in the 30-40h spin range. This is supported by Cranked Nilsson-Strutinsky calculations which were used to interpret the high-spin bands. It is found that a better agreement between calculation and experiment is obtained if the Z=64 shell gap increases with a decreasing number of valence particles outside the doubly-closed Gd-146(64)82 nucleus.

    • The reaction 58Ni(32S, α2p) at E lab= 135 MeV was used to populate high-spin states in 84Zr. The complete GAMMASPHERE and MICROBALL arrays were used to obtain clean γ- γ- γ line shapes to be analyzed by the Doppler shift attenuation method and to determine 27 lifetimes in the ground-state band and in two excited bands. Side-feeding times were also measured by comparing the line shapes gated with transitions above and below the state under study. The deduced electric quadrupole moments for the ground-state band are consistent with a very slow reduction with frequency with values ranging between 2.4(3) and 2.0(1)e b. The negative-parity bands feature also an approximate constancy of quadrupole moment with values similar to those in the ground-state band. Cranking calculations agree with this behavior in both parity bands and suggest an interpretation of the upper states in the ground-state band as part of a very slowly terminating band. Shell-dependent cranked Nilsson calculations explain a fourth γ cascade as pertaining to a noncollective structure terminating at I = 20+.

    • The high spin states in the Ce-139 nucleus have been studied by in-beam gamma-spectroscopic techniques using the reaction Te-130(C-12,3n)Ce-139 at E-beam=65 MeV. A gamma detector array, consisting of five Compton-suppressed Clover detectors was used for coincidence measurements. 15 new levels have been proposed and 28 new gamma transitions have been assigned to Ce-139 on the basis of gamma gamma coincidence data. The level scheme of Ce-139 has been extended above the known 70 ns 19/2 isomer up to similar to 6.1 MeV in excitation energy and 35/2h in spin. The spin-parity assignments for most of the newly proposed levels have been made using the deduced Directional Correlation from Oriented states of nuclei (DCO ratio) and the Polarization Directional Correlation from Oriented states (PDCO ratio) for the de-exciting transitions. The observed level structure has been compared with a large basis shell model calculation and also with the predictions from cranked Nilsson-Strutinsky (CNS) calculations. A general consistency has been observed between these two different theoretical approaches.

    • Context Patients with adrenal insufficiency (AI) have excess mortality and morbidity, mainly due to cardiovascular (CV) diseases. The mechanisms for this is unclear. Objective To assess CV structure and function in AI patients on conventional replacement therapy and after switching to once-daily, modified-release hydrocortisone (OD-HC) in comparison with healthy matched controls. Methods This was a retrospective analysis of 17 adult AI patients (11 with primary AI, 6 with secondary AI) on stable replacement with cortisone acetate [median (minimum, maximum) 33.5 (12.5-50) mg] and, if needed, fludrocortisone [0.1 (0.05-0.2) mg], and 17 healthy matched controls. Ten patients were switched to an equivalent dose of OD-HC. Data from echocardiography, 24 h Holter-ECG and 24 h blood pressure monitoring were collected at baseline and 6 months after the switch to OD-HC. Results At baseline, AI patients had smaller left ventricular diastolic diameter (47.1 +/- 4.2 vs. 51.6 +/- 2.3 mm;P = 0.001) and left atrial diameter (34.9 +/- 4.7 vs. 38.2 +/- 2.6 cm;P = 0.018), and a higher ejection fraction (62.5 +/- 6.9% vs. 56.0 +/- 4.7%;P = 0.003) than controls. AI patients had lower nocturnal systolic and diastolic blood pressure than controls (108 +/- 15 mmHg vs. 117 +/- 8 mmHg;P = 0.038 and 65 +/- 9 mmHg vs. 73 +/- 7 mmHg;P = 0.008, respectively). After the switch to OD-HC, nocturnal diastolic blood pressure normalised. No significant changes were observed in echocardiographic and Holter-ECG parameters following the switch. Conclusions AI patients on conventional treatment display cardiovascular abnormalities that could be related to hypovolemia. Switch to OD-HC seems to have beneficial effect on blood pressure profile, but no effect on cardiovascular structure and function.

    • Endogenous/exogenous Cushing's syndrome is characterized by a cluster of systemic manifestations of hypercortisolism, which cause increased cardiovascular risk. Its biological basis is glucocorticoid excess, acting on various pathogenic processes inducing cardiovascular damage. Hypertension is a common feature in Cushing's syndrome and may persist after normalizing hormone excess and discontinuing steroid therapy. In endogenous Cushing's syndrome, the earlier the diagnosis the sooner management can be employed to offset the deleterious effects of excess cortisol. Such management includes combined treatments directed against the underlying cause and tailored antihypertensive drugs aimed at controlling the consequences of glucocorticoid excess. Experts on endocrine hypertension and members of the Working Group on Endocrine Hypertension of the European Society of Hypertension (ESH) prepared this Consensus document, which summarizes the current knowledge in epidemiology, genetics, diagnosis, and treatment of hypertension in Cushing's syndrome.

  • Spectroscopic Tools Applied to Element Z = 115 Decay Chains

    • Nuclides that are considered to be isotopes of element Z = 115 were produced in the reaction 48Ca + 243Am at the GSI Helmholtzzentrum für Schwerionenforschung Darmstadt. The detector setup TASISpec was used. It was mounted behind the gas-filled separator TASCA. Thirty correlated α-decay chains were found, and the energies of the particles were determined with high precision. Two important spectroscopic aspects of the offline data analysis are discussed in detail: the handling of digitized preamplified signals from the silicon strip detectors, and the energy reconstruction of particles escaping to upstream detectors relying on pixel-by-pixel dead-layer thicknesses.

  • Band termination spectroscopy in 157Er

    • The level scheme of 157Er has been extended from a spin region where the nucleus behaves as a prolate rotor to a region where the spin is produced by the alignment of all or most of the available valence nucleons along the symmetry axis of a weakly deformed oblate shape. The level scheme was established at high spin using up to four-fold gamma -ray coincidences detected in the Eurogam spectrometer following the reaction 114Cd( 48Ca,5n)157Er at a bombarding energy of 210 MeV. Particularly favoured states have been established at IK=69/2+, 81/2+, 71/2+, 77/2-, 87-/2 and 89-/2. Specific single-particle configurations are assigned to these special states by comparison with cranked Nilsson-Strutinsky calculations. These states are related to structures observed in the neighbouring nuclei 158Er and 157Ho. These data provide the spectrum of single-particle states for the lowest lying valence orbitals above the 146Gd closed core.

  • Characterization of Superdeformed Bands in 62Zn

    • Combined data from four fusion-evaporation reaction experiments were utilized to investigate deformed and superdeformed structures in Zn-62(30)32. Combination of the Gammasphere gamma-ray spectrometer and ancillary particle detection systems allowed for the connection of rotational bands to well-known, low-lying excited states in Zn-62, as well as spectroscopy of discrete high-spin states reaching excitation energies of E-x=42.5 MeV. Four well- or superdeformed bands in Zn-62 are characterized and described by means of cranked Nilsson-Strutinsky calculations.

    • High-spin states have been studied in 72,73Kr using 40Ca+40Ca and 36Ar+40Ca reactions at 160, 164 and 145 MeV respectively. The properties and configurations of the bands observed are interpreted with the aid of unpaired cranked Nilsson-Strutinsky/ cranked relativistic mean-field calculations and paired total routhian surface and cranked relativistic Hartree-Bogoliubov calculations. In 73Kr the three previously known structures have been extended to a spin of ~30[h-bar] and a fourth structure, which is believed to be the signature partner to the g9/2 band, has been identified. The negative parity bands show evidence for an unusual type of band crossing. It is currently unclear whether this is a consequence of T=0 n-p pairing. At the highest spins the unpaired calculations can reproduce the observed data, suggesting that there is little evidence for pairing correlations in this spin regime. In 72Kr a new band has been identified that has the properties expected from the missing doubly aligned S-band configuration. The properties of this band can be reproduced by the unpaired CNS/ CRMF calculations, again suggesting that the structure has little pairing. However, paired calculations appear to be necessary to reproduce the properties of the previously known structure in this nucleus. The 72Kr results are also compared with projected shell model calculations. ©2003 American Institute of Physics
