  • Design and Baseline Characteristics of the Finerenone in Reducing Cardiovascular Mortality and Morbidity in Diabetic Kidney Disease Trial

    • <b><i>Background:</i></b> Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. <b><i>Patients and</i></b> <b><i>Methods:</i></b> The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m<sup>2</sup> and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. <b><i>Conclusions:</i></b> FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.   • Contribution of chemotherapy mobilization to disease control in multiple myeloma treated with autologous hematopoietic cell transplantation

    • In patients with multiple myeloma (MM) undergoing autologous hematopoietic cell transplantation (auto-HCT), peripheral blood progenitor cells may be collected following mobilization with growth factor alone (GF) or cytotoxic chemotherapy plus GF (CC+GF). It is uncertain whether the method of mobilization affects post-transplant outcomes. We compared these mobilization strategies in a retrospective analysis of 968 patients with MM from the Center for International Blood and Marrow Transplant Research database who received an auto-Ha in the US and Canada between 2007 and 2012. The kinetics of neutrophil engraftment (>= 0.5 x 10(9)/L) was similar between groups (13 vs 13 days, P=0.69) while platelet engraftment (>= 20 x 10(9)/L) was slightly faster with CC+GF (19 vs 18 days, P=0.006). Adjusted 3-year PFS was 43% (95% confidence interval (CI) 38-48) in GF and 40% (95% CI 35-45) in CC+GF, P=0.33. Adjusted 3-year OS was 82% (95% CI 78-86) vs 80% (95% CI 75-84), P=0.43 and adjusted 5-year OS was 62% (95% CI 54-68) vs 60% (95% CI 52-67), P=0.76, for GF and CC+GF, respectively. We conclude that MM patients undergoing auto-Ha have similar outcomes irrespective of the method of mobilization and found no evidence that the addition of chemotherapy to mobilization contributes to disease control.

  • Creating a specialist protein resource network : a meeting report for the protein bioinformatics and community resources retreat

    • During 11-12 August 2014, a Protein Bioinformatics and Community Resources Retreat was held at the Wellcome Trust Genome Campus in Hinxton, UK. This meeting brought together the principal investigators of several specialized protein resources (such as CAZy, TCDB and MEROPS) as well as those from protein databases from the large Bioinformatics centres (including UniProt and RefSeq). The retreat was divided into five sessions: (1) key challenges, (2) the databases represented, (3) best practices for maintenance and curation, (4) information flow to and from large data centers and (5) communication and funding. An important outcome of this meeting was the creation of a Specialist Protein Resource Network that we believe will improve coordination of the activities of its member resources. We invite further protein database resources to join the network and continue the dialogue.

  • Improved Outcomes After Autologous Hematopoietic Cell Transplantation for Light Chain Amyloidosis : A Center for International Blood and Marrow Transplant Research Study

    • Purpose Autologous hematopoietic cell transplantation, or autotransplantation, is effective in light-chain amyloidosis (AL), but it is associated with a high risk of early mortality (EM). In a multicenter randomized comparison against oral chemotherapy, autotransplantation was associated with 24% EM. We analyzed trends in outcomes after autologous hematopoietic cell transplantation for AL in North America. Patients and Methods Between 1995 and 2012, 1,536 patients with AL who underwent autotransplantation at 134 centers were identified in the Center for International Blood and Marrow Transplant Research database. EM and overall survival (OS) were analyzed in three time cohorts: 1995 to 2000 (n = 140), 2001 to 2006 (n = 596), and 2007 to 2012 (n = 800). Hematologic and renal responses and factors associated with EM, relapse and/or progression, progression-free survival and OS were analyzed in more recent subgroups from 2001 to 2006 (n = 197) and from 2007 to 2012 (n = 157). Results Mortality at 30 and 100 days progressively declined over successive time periods from 11% and 20%, respectively, in 1995 to 2000 to 5% and 11%, respectively, in 2001 to 2006, and to 3% and 5%, respectively, in 2007 to 2012. Correspondingly, 5-year OS improved from 55% in 1995 to 2000 to 61% in 2001 to 2006 and to 77% in 2007 to 2012. Hematologic response to transplantation improved in the latest cohort. Renal response rate was 32%. Centers performing more than four AL transplantations per year had superior survival outcomes. In the multivariable analysis, cardiac AL was associated with high EM and inferior progression-free survival and OS. Autotransplantation in 2007 to 2012 and use of higher dosages of melphalan were associated with a lowered relapse risk. A Karnofsky score less than 80 and creatinine levels 2 mg/m(2) or greater were associated with worsened OS. Conclusion Post-transplantation survival in AL has improved, with a dramatic reduction in early post-transplantation mortality and excellent 5-year survival. The risk-benefit ratio for autotransplantation has changed, and randomized comparison with nontransplantation approaches is again warranted.

  • Allotransplantation for Patients Age >= 40 Years with Non-Hodgkin Lymphoma : Encouraging Progression-Free Survival

    • Non-Hodgkin lymphoma (NHL) disproportionately affects older patients, who do not often undergo allogeneic hematopoietic cell transplantation (HCT). We analyzed Center for International Blood and Marrow Transplant Research data on 1248 patients age >= 40 years receiving reduced-intensity conditioning (RIC) or nonmyeloablative (NMA) conditioning HCT for aggressive (n = 668) or indolent (n = 580) NHL Aggressive lymphoma was more frequent in the oldest cohort 49% for age 40 to 54 versus 57% for age 55 to 64 versus 67% for age >= 65; P = .0008). Fewer patients aged >= 65 had previous autografting (26% versus 24% versus 9%; P = .002). Rates of relapse, acute and chronic GVHD, and nonrelapse mortality (NRM) at 1 year post-HCT were similar in the 3 age cohorts (22% [95% confidence interval (CI), 19% to 26%] for age 40 to 54, 27% [95% CI, 23% to 31%] for age 55 to 64, and 34% [95% CI, 24% to 44%] for age >= 65. Progression-free survival (PFS) and overall survival (OS) at 3 years was slightly lower in the older cohorts (OS: 54% [95% CI, 50% to 58%] for age 40 to 54; 40% [95% CI, 36% to 44%] for age 55 to 64, and 39% [95% CI, 28% to 50%] for age >= 65; P < .0001). Multivariate analysis revealed no significant effect of age on the incidence of acute or chronic GVHD or relapse. Age >= 55 years, Karnofsky Performance Status <80, and HLA mismatch adversely affected NRM, PFS, and OS. Disease status at HCT, but not histological subtype, was associated with worse NRM, relapse, PFS, and OS. Even for patients age >= 55 years, OS still approached 40% at 3 years, suggesting that HCT affects long-term remission and remains underused in qualified older patients with NHL.

    • The presence of monosomal karyotype (MK+) in acute myeloid leukemia (AML) is associated with dismal outcomes. We evaluated the impact of MK+ in AML (MK+AML, n = 240) and in myelodysplastic syndrome (MDS) (MK+MDS, n = 221) on hematopoietic cell transplantation outcomes compared with other cytogenetically defined groups (AML, n = 3360; MDS, n = 1373) as reported to the Center for International Blood and Marrow Transplant Research from 1998 to 2011. MK+AML was associated with higher disease relapse (hazard ratio, 1.98; P < .01), similar transplantation-related mortality (TRM) (hazard ratio, 1.01; P = .90), and worse survival (hazard ratio, 1.67; P < .01) compared with those outcomes for other cytogenetically defined AML. Among patients with MDS, MK+ MDS was associated with higher disease relapse (hazard ratio, 2.39; P < .01), higher TRM (hazard ratio, 1.80; P < .01), and worse survival (HR, 2.02; P < .01). Subset analyses comparing chromosome 7 abnormalities (del7/7q) with or without MK+ demonstrated higher mortality for MK+ disease in for both AML (hazard ratio, 1.72; P < .01) and MDS (hazard ratio, 1.79; P < .01). The strong negative impact of MK+ in myeloid malignancies was observed in all age groups and using either myeloablative or reduced-intensity conditioning regimens. Alternative approaches to mitigate disease relapse in this population are needed.

    • Bengal Delta region is currently confronted with largest groundwater arsenic calamity in history of human kind (BGS-DPHE, 2001; Mukherjee and Bhattacharya, 2001; Bhattacharya et al., 2002a; McArthur et al., 2001; Smedley and Kinniburgh, 2002; Mukherjee et al., 2006; Nath et al., 2005, 2007, 2008). Concentrations of arsenic in drinking water wells in the region often exceed the WHO drinking water guideline value (10 μg L-1) and the national safe limit of both India and Bangladesh for arsenic in drinking water (Smedley and Kinniburgh, 2002; RGNDWM, 2002; CGWB, 1999; Bhattacharya et al., 2002a). About one third (35 million) population inhabiting in this region (West Bengal and Bangladesh), currently at risk of long-term arsenic exposure (Bhattacharya et al., 2001; RGNDWM, 2002; Chakraborti et al., 2004; Kapaj et al., 2006), are being diagnosed with a wide spectrum of adverse health impacts including skin disorders such as hyper/hypo-pigmentation, keratosis and melanosis and are also in hot-spot areas of BDP which is reflected in a rise in the number of cancer cases (Guha Mazumdar et al., 1988). The distribution pattern of arsenic occurrence in BDP is patchy and there are numerous hotspots of arsenic contamination in the semi-confined shallow Holocene aquifer (Bhattacharya et al., 1997; Smedley and Kinniburgh, 2002). The scale of the problem is serious both in terms of hotspots and geographic area coverage (173 × 10 3 km2, eastern part of Hoogly-Bhagirathi/Western part of Ganga-Padma-lower Meghna flood plains).

    • The benefits of pre-transplant induction chemotherapy in light chain (AL) amyloidosis, a low burden plasma cell (PC) neoplasm associated with multiorgan dysfunction, is debatable, although with the availability of bortezomib, this approach is being increasingly pursued. We analyzed the outcomes of AL amyloidosis patients undergoing autologous hematopoietic cell transplant between 2014 and 2018 that were reported to the Center for International Blood and Marrow Transplant Research database. Of 440 patients, 294 received bortezomib-based induction, and 146 received no induction. Patients receiving induction had greater PC burden compared to no induction (PC 10% or more, 39% versus 11%; P < .01). At 2 years, the induction group compared to no induction had lower relapse/progression: 13% (9% to 18%) versus 23% (16% to 32%) (P = .02); better progression-free survival (PFS): 82% (77% to 87%) versus 69% (61% to 77%) (P < .01); and similar overall survival (OS): 92% (88% to 95%) versus 89% (84% to 94%) (P = .22), findings that were confirmed on multivariate analysis. A subset analysis limited to patientswith <10% PC also showed superior relapse/progression (hazard ratio [HR],.43; 95% confidence interval [CI],.24 to.78; P < .01) and PFS (HR,.43; 95% CI,.26 to .72; P < .01) for induction compared to no induction. Thus, we conclude that pre-transplant bortezomib-based induction was associated with improved relapse/progression and PFS in AL amyloidosis. Longer survival follow-up is warranted, as OS was excellent in both cohorts at 2 years.

    • Background. NT-proBNP may be useful for ruling out heart failure in primary health care. In this study we examined the analytical quality of NT-proBNP in primary health care on the Cobas h 232 point-of-care instrument compared with measurements performed in a hospital laboratory. Materials and methods. Blood samples requested for NT-proBNP were collected in primary health care (n = 95) and in a hospital laboratory (n = 107). NT-proBNP was measured on-site on Cobas h 232 instruments both in primary health care centres and at the hospital laboratory and all samples were also analyzed with a comparison method at the hospital. Precision, trueness, accuracy, and lot-variation were determined at different concentration levels and evaluated according to acceptance criteria. Furthermore user-friendliness was assessed by questionnaires. Results. For Cobas h 232 repeatability CV was 8.5-10.7% in the hospital setting and 5.3-10.0% in the primary health care and within the analytical quality specifications, but higher than with the comparison method (<4%). NT-proBNP results obtained in primary health care were significantly higher than by the hospital comparison method (bias ranged from 14.3-23.7%), whereas there was no significant bias when Cobas h 232 was used in the hospital setting (bias ranged from 4.9 to 7.0%). User-friendliness of Cobas h 232 was overall acceptable. Conclusion. Cobas h 232 point-of-care instrument for measurement of NT-proBNP performed satisfactorily with regard to precision, user-friendliness, and lot-variation. A decrease in NT-proBNP levels observed in samples transported to a central laboratory needs further attention and investigation.

    • A detailed analysis of optical and microphysical properties of aerosol particles during the dry winter monsoon season above the northern Indian Ocean is presented. The Cloud Aerosol Radiative Forcing Experiment (CARDEX), conducted from 16 February to 30 March 2012 at the Maldives Climate Observatory on Hanimaadhoo island (MCOH) in the Republic of the Maldives, used autonomous unmanned aerial vehicles (AUAV) to perform vertical in situ measurements of particle number concentration, particle number size distribution as well as particle absorption coefficients. These measurements were used together with surface-based Mini Micro Pulse Lidar (MiniMPL) observations and aerosol in situ and off-line measurements to investigate the vertical distribution of aerosol particles. Air masses were mainly advected over the Indian subcontinent and the Arabian Peninsula. The mean surface aerosol number concentration was 1717 +/- 604cm(-3) and the highest values were found in air masses from the Bay of Bengal and Indo-Gangetic Plain (2247 +/- 370cm(-3)). Investigations of the free tropospheric air showed that elevated aerosol layers with up to 3 times higher aerosol number concentrations than at the surface occurred mainly during periods with air masses originating from the Bay of Bengal and the Indo-Gangetic Plain. This feature is different compared to what was observed during the Indian Ocean Experiment (INDOEX) conducted in winter 1999, where aerosol number concentrations generally decreased with height. In contrast, lower particle absorption at the surface (sigma(abs)(520nm) = 8.5 + 4.2Wm(-1)) was found during CARDEX compared to INDOEX 1999. Layers with source region specific single-scattering albedo (SSA) values were derived by combining vertical in situ particle absorption coefficients and scattering coefficients calculated with Mie theory. These SSA layers were utilized to calculate vertical particle absorption profiles from MiniMPL profiles. SSA surface values for 550 nm for dry conditions were found to be 0 : 94 +/- 0 : 02 and 0 : 91 +/- 0 : 02 for air masses from the Arabian Sea (and Middle East countries) and India (and Bay of Bengal), respectively. Lidar-derived particle absorption coefficient profiles showed both a similar magnitude and structure as the in situ profiles measured with the AUAV. However, primarily due to insufficient accuracy in the SSA estimates, the lidar-derived absorption coefficient profiles have large uncertainties and are generally weakly correlated to vertically in situ measured particle absorption coefficients. Furthermore, the mass absorption efficiency (MAE) for the northern Indian Ocean during the dry monsoon season was calculated to determine equivalent black carbon (EBC) concentrations from particle absorption coefficient measurements. A mean MAE of 11.6 and 6.9m(2) g(-1) for 520 and 880 nm, respectively, was found, likely representing internally mixed BC containing particles. Lower MAE values for 880 and 520 nm were found for air masses originating from dust regions such as the Arabian Peninsula and western Asia (MAE(880 nm) = 5.6m(2) g(-1), MAE(520 nm) = 9.5m(2) g(-1)) or from closer source regions as southern India (MAE(880 nm) = 4.3m(2) g(-1), MAE(520 nm) = 7. 3m(2) g(-1)).

    • For the past decade, adoptive cell therapy including tumor-infiltrating lymphocytes, genetically modified cytotoxic lymphocytes expressing a chimeric antigen receptor, or a novel T-cell receptor has revolutionized the treatment of many cancers. Progress within exome sequencing and neoantigen prediction technologies provides opportunities for further development of personalized immunotherapies. In this study, we present a novel strategy to deliver in silico predicted neoantigens to autologous dendritic cells (DCs) using paramagnetic beads (EpiTCer beads). DCs pulsed with EpiTCer beads are superior in enriching for healthy donor and patient blood-derived tumor-specific CD8+ T cells compared to DC loaded with whole-tumor lysate or 9mer neoantigen peptides. A dose-dependent effect was observed, with higher EpiTCer bead per DC being favorable. We concluded that CD8+ T cells enriched by DC loaded with EpiTCer beads are tumor specific with limited tumor cross-reactivity and low recognition of autologous non-activated monocytes or CD8+ T cells. Furthermore, tumor specificity and recognition were improved and preserved after additional expansion using our Good Manufacturing Process (GMP)-compatible rapid expansion protocol. Phenotypic analysis of patient-derived EpiTCer DC expanded CD8+ T cells revealed efficient maturation, with high frequencies of central memory and effector memory T cells, similar to those observed in autologous expanded tumor-infiltrating lymphocytes. These results indicate that DC pulsed with EpiTCer beads enrich for a T-cell population with high capacity of tumor recognition and elimination, which are features needed for a T-cell product to be used for personalized adoptive cell therapy.

    • The Fractionation of Fe, Zn, Cu, Pb, Mn and Cd in the sediments of the Achankovil River, Western Ghats, India using a sequential extraction method was carried out to understand the metal availability in the basin for biotic and abiotic activities. Spatial distribution of heavy metals has been studied. Sediment grain size has significant control over the heavy metal distribution. The fluctuations in their concentration partly depend upon the lithology of the river basin and partly the anthropogenic activities. The sediments are dominated by sand and are moderately to strongly positively skewed and are very leptokurtotic in nature. The quartzite and feldspars are abundant minerals along with significant amount of mica with low clay content. The core sediments show increasing trend of heavy metal concentration with depth due to the recent addition of anthropogenic sources and post-diagenic activities. Significant amount of Cd (18%) was found in carbonate fraction, which may pose environmental problems due to its toxic nature. Small concentrations of metals, except Cd and Cu, are in exchangeable fraction, which indicate low bio-availability. Enrichment Factor (EF) for individual metals shows the contribution from terrregious and in part from anthropogenic sources. Selective Sequential Extraction (SSE) study shows the variation in specific metal distribution pattern, their distribution in different phases and their bio-availability. Maximum amount of the metals were bound to the non-residual fractions (mainly Fe-oxides). Overall, bio-availability of these micronutrients from sediments seems to be very less. Non-residual phase is the most important phase for majority of heavy metals studied. Among the non-residual fraction, maximum amount of the heavy metals bound to Fe-oxides. The study high lights the need for in-depth study of heavy metals distribution and fractionation in the smaller river basins to get precise information on the behavior and transport of heavy metals in the fluvial environment and their contribution to the world ocean.

    • Very limited efforts have been directed to determine the degree of Arsenic (As) and other trace elements contamination in food grains and understanding the associated risk to human through consumption of food in middle Gangetic plain of Bihar, India. Arsenic concentration in groundwater (>80%) samples found above BIS and WHO permissible limit, while 28% samples exceeded the previous WHO drinking water guideline (400 μg/L) for manganese (Mn). In dietary food grains As followed the sequence as rice > wheat > maize. The estimated daily intake by individual of As, Mn, Ni, Cd, Co, Pb, Zn and Cr from drinking water and dietary food grains were 169,14582, 474, 19, 26, 1449, 12955 and 882 μg/ kg. The Health Risk Index (HRI) were >1 for As in drinking water, vegetables and rice indicated the potential health risk to the residents of the study area. However, HRI <1 for wheat and maize indicate a relative absence of health risks associated with their ingestion.

    • Tectonic evolution of Himalayas is related to high erosional potential and substantial sediment transport. Fluvial deposition of clastic material in the Middle Gangetic plain (MGP) is mainly governed by crustal deformation and climatic condition of Himalayas (Singh M, Singh IB, Müller G, Geomorphology 86:144-175, 2007). Seven large Asian rivers-Ganga, Indus, Brahmaputra, Yangtze, Huang He or Yellow River, Salween and Mekong-are fed by Himalayan glaciers which are supplying ~30 % of the global sediments to the ocean (Milliman JD, Meade RH, J Geol 9:1-19, 1983; Singh VB, Ramanathan AL, Pottakkal JG, Kumar M, J Asian Earth Sci 79:224-234, 2014, 2005). High flux of sediment transported from different terrain of Himalayas is product of geologically young rock formation (Singh VB, Ramanathan AL, Pottakkal JG, Kumar M, J Asian Earth Sci 79:224-234, 2014). It provides an opportunity to study the fluvial system and post-depositional changes in sediment water interaction depending on the degree of mobility of element under the altered environmental conditions. Arsenic (As) contamination of groundwater is a global problem. Understanding of As mobilization from sediments to As-contaminated aquifers is important for water quality management in areas of MGP of India.

    • Groundwater with high geogenic arsenic (As) is extensively present in the Holocene alluvial aquifers of Ghazipur District in the middle Gangetic Plain, India. A shift in the climatic conditions, weathering of carbonate and silicate minerals, surface water interactions, ion exchange, redox processes, and anthropogenic activities are responsible for high concentrations of cations, anions and As in the groundwater. The spatial and temporal variations for As concentrations were greater in the pre-monsoon (6.4-259.5 mu g/L) when compared to the post-monsoon period (5.1-205.5 mu g/L). The As enrichment was encountered in the sampling sites that were close to the Ganges River (i.e. south and southeast part of Ghazipur district). The depth profile of As revealed that low concentrations of NO3- are associated with high concentration of As and that As depleted with increasing depth. The poor relationship between As and Fe indicates the As release into the groundwater, depends on several processes such as mineral weathering, O-2 consumption, and NO3- reduction and is de-coupled from Fe cycling. Correlation matrix and factor analysis were used to identify various factors influencing the gradual As enrichment in the middle Gangetic Plain. Groundwater is generally supersaturated with respect to calcite and dolomite in post-monsoon period, but not in pre-monsoon period. Saturation in both periods is reached for crystalline Fe phases such as goethite, but not with respect to poorly crystalline Fe phases and any As-bearing phase. The results indicate release of arsenic in redox processes in dry period and dilution of arsenic concentration by recharge during monsoon. Increased concentrations of bicarbonate after monsoon are caused by intense flushing of unsaturated zone, where CO2 is formed by decomposition of organic matter and reactions with carbonate minerals in solid phase. The present study is vital considering the fact that groundwater is an exclusive source of drinking water in the region which not only makes situation alarming but also calls for the immediate attention.

    • The present work is an attempt to study As and F+ coevality using laboratory based assays which couples fractionation and batch dissolution experiments. Sequential extraction procedure (SEP) resulting into five "operationally defined phases", was performed on sediment and soil samples collected from the Brahmaputra flood plains, Assam, India. High correlation between the Fe (hydr)oxide fraction and total As content of the soil/sediment sample indicates the involvement of Fe (hydr)oxides as the principal source of As. F- being an anion has high potential to be sorbed onto positively charged surfaces. Findings of the SEP were used to design the batch desorption experiments by controlling the Fe (hydr)oxide content of the soil/sediment. Desorption of As and F- was observed under acidic, neutral and alkaline pH from untreated and Fe (hydr)oxide removed samples. Highest amount of As and F- were found to be released from untreated samples under alkaline pH, while the amount leached from samples with no Fe (hydr) oxide was low. The study showed that the Fe (hydr)oxide fraction commonly found in the soils and sediments, had high affinity for negatively charged species like F- oxyanions of As, AsO43- (arsenate) and AsO33- (arsenite). Fe (hydr)oxide fraction was found to play the major role in co-evolution of As and F-. Two sorption coefficients were proposed based on easily leachable fraction and As present in the groundwater of sampling location for understanding of contamination vulnerability from the leaching.

    • A set of 96 water samples from shallow tubewells and 14 sediment samples from Samastipur district to know the level of arsenic (As) and its controlling factors were analyzed. Groundwater samples were collected from tubewells of different depths, and a wide range of concentrations in the range 0.19-135 μg L-1 was found. Scanning electron micrograph study of the sediments shows intense chemical weathering. Present study also support reductive dissolution of FeOOH triggered by organic matter oxidation as an As mobility factor in the aquifer sands.

    • The current study was focused on the characterization of recharge, weathering processes and to check the aptness of groundwater for household and agriculture utility in the Chhaprola industrial area located in Gautam Buddha Nagar district of Uttar Pradesh, India. Groundwater samples (n = 33) were analyzed for major cations and anions from which (n = 22) were analyzed for stable isotopic records (δ2H and δ18O). Solute chemistry revealed neutral to moderately basic nature (pH ranged 6.4–8.8) and showed a higher (ranged 552–3130 µS/cm) of electrical conductivity. Isotopic signals ranged from − 8.7 to − 3.3‰ for δ18O and − 60.1 to − 37‰ for δ2H. Bivariate plot of stable isotopes (δ18O and δ2H) showed that majority of the samples fell below the global meteoric water line (GMWL) and local meteoric water line (LMWL) of New Delhi, indicating enrichment of heavier isotopes in the aquifers, which explain that recharging water, has undergone evaporation before recharging the aquifer. Monsoonal precipitation was observed as the major source of aquifer recharge in the surveyed area. Vertical hydraulic connectivity of the aquifer layers was traced with the help of isotopic record which exhibited a more negative intercept for shallow depth tube well than the middle depth tube well. About 76% of the samples exhibited negative Schoeller index thereby indicated cation-anion exchange reactions while the remaining samples depicted the Base Exchange Reactions. Sodium-normalized Ca versus Na-normalized Mg and HCO3, indicated an equal contribution of silicate and carbonate type weathering on global-average weathering system. About 52% of the groundwater samples fell into the Ca-HCO3 - water type while remaining samples fell into 33% and 15% of Na-HCO3 and Mg-HCO3 type, respectively. Water quality indices (viz. residual sodium carbonate, Kelly index, permeability index, magnesium hazard, and Chloroalkaline indices) revealed unsuitability of the groundwater for drinking and irrigation purpose at few locations.

    • A geochemical assessment of seasonal dynamics in the groundwater chemistry of the National Capital Territory (NCT), Delhi, was attempted through geochemical modelling, mineral precipitation sequences with rainfall and water evaporation cycle. Saturation indices calculated using PHREEQC indicated that the degree of water-rock equilibrium changes significantly from pre-monsoon to post-monsoon. The schematic model of SI change with water table fluctuation showed that during monsoon, as rainwater percolates through the soil, partial pressure of CO2 becomes higher than that of the atmospheric value and led to the formation of more carbonic acid that react with the carbonate minerals to produce , Mg2+ and Ca2+. The thermodynamic stability relationships of water chemistry in the Na, K, Ca and Mg silicate systems showed that for the samples with higher EC equilibrium between clay and primary minerals is not likely to be the main processes controlling variation in the groundwater chemistry. Chloro-alkaline indices (CAI) are positive when the groundwater level is high and become negative with the lowering of water level, i.e. when water level is high, reverse ion exchange is dominant. In case of pre-monsoon season, lower and negative value of CAI-1 and CAI-2 indicates dominance of ion exchange process and increases dissolved solid concentration in groundwater. The conceptual geochemical model depicted that water table fluctuation resulting from heavy pumping/withdrawal and recharge in association with the variation in DO, and Fe regulates the water-mineral equilibrium. The conceptual geochemical model explained the hydrogeochemical processes and their variations with water table fluctuation and, thus, highlighted the descriptive capabilities of PHREEQC. The study suggested that in the subsurface environment, complex interactions are simultaneously functioning, and hence, significant seasonal variations are likely to be very influential due to monsoonal recharge and subsequent changes in the saturation states of the water.

    • In order to evaluate the sources and fate of organic matter (OM) in the Pichavaram mangrove–estuarine ecosystem, Corg, Ntot, C/Natm, and stable isotopes (δ13Corg and δ15Norg) of five 210Pb dated sediment cores were investigated. Core A was collected from the Vellar Estuary, cores B, C, and D from mangrove forests, and core E was from the Coleroon Estuary. In mangrove sediments, Corg, Ntot, and C/Natm concentration varied from 1.2 ± 0.52%, 0.08 ± 0.03%, and 16.6 ± 4.1%, respectively. In the estuaries, Corg, Ntot, and C/Natm varied from 0.37 ± 0.19%, 0.03 ± 0.01%, and 12.3 ± 2.9%, respectively. In mangrove forests the δ13Corg and δ15Norg values varied from − 24.3 ± 1.8‰ and 4.2 ± 0.86‰, whereas in estuaries these values were − 23.2 ± 1.6‰ and 5.1 ± 1.3‰, respectively. The variation represents variable inputs of marine and terrestrial OM in these area, OM degradation, replacement of native vegetation due to increasing salinity, and early diagenetic changes. Some of these changes could be attributed to anthropogenic activities that have been happening in the Pichavaram complex over the last few decades. OM once deposited in the sediments is rapidly decomposed or remineralized in both mangrove and estuarine sites. The rate of Corg burial is more in mangrove forests than estuaries. Early diagenetic changes are modeled in the cores based on C/N stoichiometry. This was most applicable in mangrove sediments (core C) where ~ 56% C was remineralized representing a first order diagenetic model.

    • The nature and distribution of lipid biomarkers (n-alkanes, n-alkanols, sterols and triterpenols) were investigated in five dated sediment cores from the Pichavaram mangrove-estuarine complex in order to: 1) identify the organic matter (OM) sources and its preservation and 2) trace recent changes associated with coastal processes and anthropogenic activities. The mangrove sediment extracts have higher biomarker concentration (22.6 +/- 13.3 mu g/g dry weight) than the estuarine extracts (6.42 +/- 4.92 mu g/g dry weight). Triterpenols are dominant biomarkers in both estuarine and mangrove sediments, and constitute greater than50% of the total lipid extracts. The presence of n-alkanols (n-C-26,C-28,C-30) and abundance of phytosterols (stigmasterol and beta-sitosterol) and triterpenols (taraxerol, beta-amyrin, germanicol, and lupeol) indicate that mangrove vegetation is the primary source of sedimentary OM. The high abundance and unimodal distribution of the long-chain n-alkanes (mainly n-C-25,C-27,C-29), and high values of Terrestrial Aquatic Ratio (TAR 2.4 to 41) and Carbon Preference Index (CPI greater than 8) indicate dominance (and better preservation) of higher plant derived immature OM in mangrove sediments. In contrast the weak dominance of high molecular weight n-alkanes, low CPI (0.75 to 0.90) and TAR (1.9 to 5.7) values, and the presence of high C-27 and C-28 sterols indicate that phytoplankton/algal derived OM is more pronounced in estuarine sediments. Diagenetic changes in sedimentary OM indicate that some of the lipid fractions are reactive, and as a result, they degrade more rapidly than bulk OM. These compounds follow first order decay kinetics, and concur with the downcore diagenetic changes in coastal areas. Resistance to degradation among the different lipid classes show the trend: n-alkane greater than sterol greater than triterpenol greater than n-alkanol. Finally, the study indicates that less freshwater discharge from the Coleroon River is causing a gradual change in mangrove vegetation. There is less input of OM derived from mangrove vegetation into recent sediments. Biomarker trends also indicate that less salt tolerant Rhizophora spp. are gradually replaced by more salt tolerant vegetation consisting of Sueda spp. and Aviccenia spp.

Study focus: We examined the origin, distribution and processes of As release by investigating the salient groundwater chemistry and subsurface sedimentological characteristics. Besides collection of groundwater samples from domestic and public water supply wells, sediment samples from boreholes were investigated for textural and colour linkages. New hydrological insights for the region: Arsenic concentrations above the WHO guideline value of 10. μg/L were present in 33 wells and above the previous Indian national drinking standard of 50. μg/L were present in 15 wells. The green-olive colour sediments were more likely to yield As-enriched groundwater. The supersaturation of groundwater with respect to Fe(II) minerals, such as siderite and vivianite, explained the poor correlation between dissolved As and Fe. The result reinforced the phenomenon of reductive dissolution of Fe(III) oxyhydroxides releasing As to groundwater. This study throws light on the processes and mechanisms involved with As release in groundwater. The homogenous floodplain terrain makes the hydrological As imprint unambiguous and the hydrogeological signatures untarnished. Considering the absence of anthropogenic sources in the study area, the conclusions on the nature and causes for As release to groundwater looked dependable although the final contamination at specific subsurface sites would be influenced by advection-dispersion of groundwater flow accompanied by retardation, ion exchange, surface complexation and possible biodegradation.

    • Groundwater from central Gangetic plain regions contains arsenic (As) concentrations few tens of time higher than the drinking water guideline value (10 μg/L) prescribed by the World Health Organization. The concentration of As near an inland lake was high where as it was moderate to low in the interior flood plains. The intermediate and deeper aquifers have higher As concentrations compared to shallow aquifers. These aquifers seem to be particularly at risk, due to the prevailing geochemical conditions in which oxidized and reduced waters mix, and where the amount of sulfate available for microbial reduction seem to be limited. The severity of As contamination necessitates restricted use of groundwater or look for alternative water sources like rivers. The rain water harvesting could be another alternative option along with a community based management and maintenance in this rural region.

    • The Pichavaram mangrove ecosystem is located between the Vellar and Coleroon Estuaries in south-eastern India. To document the spatial-depth-based variabilities in organic matter (OM) input and cycling, five sediment cores were collected. A comparative study was carried out of grain-size composition, pore water salinity, dissolved organic C (DOC), loss-on-ignition (LOI), elemental ratios (C/N and H/C), pigments (Chl a, Chl b, and total carotenoids), and humification indices. Sand is the major fraction in these cores ranging from 60% to 99% followed by silt and clay; cores from the estuarine margin have high sand content. In mangrove forests, pore-water DOC concentrations are high (32 +/- 14 mg L(-1)), whereas salinity levels are low (50 +/- 5.5 parts per thousand). Likewise, LOI, organic C and N, and pigment concentrations are high in mangroves. OM is mainly derived from upstream terrestrial matter and/or mangrove litter, and marine OM. The humification indices do not vary significantly with depth because of rapid OM turnover. The bulk parameters indicate that the Vellar and Coleroon Estuaries are more affected by anthropogenic processes than mangrove forests. Finally, greater variability and sometimes lack of specific trends in bulk parameters implies that the 2004 tsunami caused extensive mixing in sediments. (C) 2010 Elsevier Ltd. All rights reserved.

    • The sources and historical deposition of 16 polycyclic aromatic hydrocarbons (PAHs) were investigated in dated sediment cores from the Pichavaram mangrove–estuarine complex. The ΣPAH flux in mangrove and estuarine sediments was 0.064 ± 0.031 μg/cm2/yr and 0.043 ± 0.020 μg/cm2/yr, respectively. The PAH flux in sediments increased up-core, coinciding with rapid urbanization since the 1970s. The flux showed a decrease in recent years (since 1990), coinciding with less riverine discharge, and perhaps more effective implementation of environmental regulations. The sediments were dominated by low molecular weight PAHs, suggesting anthropogenic input. Ratios of specific PAH isomer pairs suggested a greater input of petrogenic vs. pyrogenic derived PAHs. Notably, the deposition of high molecular weight PAHs increase in mangrove surface sediments was due to lignite and firewood combustion. Because of their overall low concentration in sediments it is unlikely these PAHs pose an immediate ecological hazard.

  • Identifying a Safe and Just Corridor for People and the Planet

    • Keeping the Earth system in a stable and resilient state, to safeguard Earth's life support systems while ensuring that Earth's benefits, risks, and related responsibilities are equitably shared, constitutes the grand challenge for human development in the Anthropocene. Here, we describe a framework that the recently formed Earth Commission will use to define and quantify target ranges for a safe and just corridor that meets these goals. Although safe and just Earth system targets are interrelated, we see safe as primarily referring to a stable Earth system and just targets as being associated with meeting human needs and reducing exposure to risks. To align safe and just dimensions, we propose to address the equity dimensions of each safe target for Earth system regulating systems and processes. The more stringent of the safe or just target ranges then defines the corridor. Identifying levers of social transformation aimed at meeting the safe and just targets and challenges associated with translating the corridor to actors at multiple scales present scope for future work.

    • Autologous hematopoietic cell transplantation (AHCT) for plasma cell myeloma is performed less often in people >70 years old than in people <= 70 years old. We analyzed 11,430 AHCT recipients for plasma cell myeloma prospectively reported to the Center for International Blood and Marrow Transplant Research between 2008 and 2011, representing the majority of US AHCT activity during this period. Survival (OS) was compared in 3 cohorts: ages 18 to 59 years (n = 5818), 60 to 69 years (n = 4666), and >70 years (n = 946). Median OS was not reached for any cohort. In multivariate analysis, increasing age was associated with mortality (P = .0006). Myeloma-specific mortality was similar among cohorts at 12%, indicating an age-related effect on nonmyeloma mortality. Analyses were performed in a representative subgroup comparing relapse rate, progression-free survival (PFS), and nonrelapse mortality (NRM). One-year NRM was 0% for age >70 years and 2% for other ages (P = not significant). The three-year relapse rate was 56% in age 18 to 59 years, 61% in age 60 to 69 years, and 63% age >70 (P = not significant). Three-year PFS was similar at 42% in age 18 to 59 years, 38% in age 60 to 69 years, and 33% in age >70 years (P = not significant). Postrelapse survival was significantly worse for the older cohort (P = .03). Older subjects selected for AHCT derived similar antimyeloma benefit without worse NRM, relapse rate, or PFS.

    • Global pandemic of SARS-CoV-2 has infected more than 53 million people in more than 190 countries. The major transmission routes of most of the infectious viruses’ including SARS-CoV-2 remain aerosols and droplets. The source of which remains coughing, sneezing, urinary, and intestinal excretions. This book chapter focuses on the latter two sources of water and wastewater contamination with RNA of these contagious viruses. There has been studies regarding the presence of viral RNA of COVID-19 in wastewater in few of the countries like USA, Australia, Netherland, and India. This mainly happens due to the discharge of untreated wastewater from health care facilities and sewage treatment plants. Improper management of the waste has led to the contamination of surface water bodies. Fecal shedding of the virus is common in 50% of the infected individual, and urinal shedding in just around 5%. The former shedding is of the order of 103-105 copies/mL. The wastewater surveillance for SARS-CoV-2 RNA in wastewater gives the figure of 102-106 copies/L. The surveillance already known to track down many diseases, drugs in past has been a boon to the countries, where clinical testing is at a slow pace. Therefore, wastewater-based epidemiology is needed to have a better understanding of the water and wastewater surveillances, for prior detection of SARS-CoV-2 and other pathogens, sought to occur in future. The chapter deals with the occurrences, transport pathways, persistence, and removal of most of the reported infectious viruses in water and wastewater, with a special emphasize on SARS-CoV-2. Developing a proper framework and methodology containing environment and human health risk assessments will help the researchers and policyholders to look up for the alternative options. Even after a number of researches detecting the presence of SARS-CoV-2 in wastewater, there has been a limited knowledge in the potential role of water and wastewater in the transmission of these infectious viruses. A well summarized review would surely help in the assessment of the likely fate of most of these contagious human viruses.

  • Coulometric determination of NAD+ and NADH in normal and cancer cells using LDH, RVC and a polymer mediator

    • An electrochemical method for the measurement of NAD(+) and NADH in normal and cancer tissues using flow injection analysis (FIA) is reported. Reticulated vitreous carbon (RVC) electrodes with entrapped L-lactate dehydrogenase (LDH) and a new redox polymer containing covalently bound toluidine blue O (TBO) were employed for this purpose. Both NAD(+) and NADH were estimated coulometrically based on their reaction with LDH. The latter was immobilized on controlled pore glass (CPG) by cross-linking with glutaraldehyde and packed within the RVC. The concentrations of NAD(+) and NADH in the tissues, estimated using different electron mediators such as ferricyanide (FCN), meldola blue (MB) and TBO have also been compared. The effects of flow rate, pH, applied potential (versus Ag/AgCl reference) and adsorption of the mediators have also been investigated. Based bn the measurements of NAD(+) and NADH in normal and cancer tissues it has been concluded that the NADH concentration is lower, while the NAD(+) concentration is higher in cancer tissues. Amongst the electron mediators TBO was found to be a more stable mediator for such measurements. (C) 1999 Elsevier Science B.V. All rights reserved.
