  • Adherence with advice and prescriptions in SLE is mostly good, but better follow up is needed: A study with a questionnaire.

    • The objective was to evaluate the efficacy of the information policy given to all systemic lupus erythematosus (SLE) patients. One hundred consecutive SLE patients were asked to answer anonymously a questionnaire covering demographic issues including education, adherence with prescriptions and advice given and methods of sourcing information. Seventy-three females and ten males responded. The demographic data showed that 34 had a university education, 29 high school and 19 primary school education. An inability to work due to disease was increased compared with the matched population (P < 0.001). Forty-two reported that they had received advice about physical training and forty of them followed this advice. Only 28 out of 46 smokers reported that they had got any advice about smoking, and out of these only 13 followed the advice. The patients with university background were less likely to smoke (P < 0.05) and followed the advice more often (P < 0.05). Most patients followed given advice about exposure to sunlight. Thirty-five percent of those prescribed glucocorticoids reported that they varied from the prescribed dosages without consultation with their specialist. This behaviour was more common in the university group (P < 0.05), this grouping also had a higher median dosage. Seventy-three patients had read the booklet about SLE provided by the clinic. Accessing internet information was more common for those with university education (P < 0.01). This study shows that on the whole SLE patients follow given advice, but adherence varies. Aside from the issue of glucocorticoid dosage adherence, educational level seems to be the most important predictor for adherence to advice. Thus, we conclude that a more individualized approach to delivery of information is required and better follow up is needed.

  • Mesenchymal stem cells stimulate antibody secretion in human B cells.

    • Mesenchymal stem cells (MSC) have immunomodulatory effects and inhibit T-cell responses to alloantigens and mitogens in vitro and in vivo. We wanted to examine the effect of MSC on human B cells. MSC stimulated IgG production, measured in an enzyme-linked immunospot (ELIspot) assay in blood and spleen lymphocytes. MSC only induced a low proliferation. When a semipermeable membrane separated MSC and mononuclear cells, the IgG production was stimulated in unfractionated lymphocytes. In contrast, enriched B cells required cell contact with MSC to produce IgG. Co-cultures of MSC and lymphocytes increased IFN-γ production. MSC produce IL-6, and addition of MSC to spleen cells dramatically increased IL-6 levels. After lymphocyte stimulation with lipopolysaccharide (LPS), cytomegalovirus or varicella zoster virus, MSC either stimulated or inhibited IgG response, depending on the level of stimulation by LPS or the viral antigens. Similar results were obtained for enriched B cells. To conclude, MSC stimulate B-cell antibody secretion. The IgG secretion by activated B cells may be stimulated or inhibited by the addition of MSC, depending on the level of stimulation.

    • A disc-shaped bone graft was harvested from the calvarium in ten rabbits and anchored as an onlay bone graft, using a titanium implant, at the proximal tibial metaphysis. The contralateral tibia served as the control, where an implant was placed without a graft with the implant head at a height corresponding to the thickness of the graft on the test side. Resonance frequency measurements were performed 4, 8, 16 and 24 weeks postoperatively and removal torque measurements were performed at 24 weeks. A statistically significant higher implant stability, as assessed by resonance frequency measurements (RFM), was measured from 4 weeks throughout the 24-week period. The mean peak removal torque for the test implants was 50.4+/-10.0 Ncm and 30.0+/-6.9 Ncm for the control implants, which was a statistically significant difference. Histologically, the grafted bone was well incorporated and morphometry revealed more bone around the test implants. Calculations of shear stresses indicated that the grafted bone had similar biomechanical properties to the cortical bone of the recipient site. It is concluded that the integration of titanium implants in autogenous onlay bone grafts results in an increased biomechanical support of the implant. The use of RFM may also serve as a useful instrument for noninvasive monitoring of implant stability in vivo.

    • Introduction: Vascularized autologous tissue grafts are considered 'gold standard' for the management of larger bony defects in the craniomaxillofacial area. This modality does however carry limitations, such as the absolute requirement for healthy donor tissues and recipient vessels. In addition, the significant morbidity of large bone graft is deterrent to fibula bone flap use. Therefore, less morbid strategies would be beneficial. The purpose of this study was to develop a printing method to manufacture scaffold structure with viable stem cells. Materials and Methods: In total, three different combinations of ground beta tri-calcium phosphate and CELLINK (bioinks) were printed with a nozzle to identify a suitable bioink for three-dimensional printing. Subsequently, a coaxial needle, with three different nozzle gauge combinations, was evaluated for printing of the bioinks. Scaffold structures (grids) were then printed alone and with additional adipose stem cells before being transferred into an active medium and incubated overnight. Following incubation, grid stability was evaluated by assessing the degree of maintained grid outline, and cell viability was determined using the live/dead cell assay. Results: Among the three evaluated combinations of bioinks, two resulted in good printability for bioprinting. Adequate printing was obtained with two out of the three nozzle gauge combinations tested. However, due to the smaller total opening, one combination revealed a better stability. Intact grids with maintained stability were obtained using Ink B23 and Ink B42, and approximately 80% of the printed stem cells were viable following 24 hours. Discussion: Using a coaxial needle enables printing of a stable scaffold with viable stem cells. Furthermore, cell viability is maintained after the bioprinting process.

  • Incidence and risk factors predisposing plate removal following orthognathic surgery

    • Objective. The aim of this study was to investigate the incidence and reasons for titanium fixation plate removal following orthognathic surgery, identify risk factors predisposing removal, and explore if discomfort was reduced postoperatively. Study Design. Medical records of 404 consecutive cases were retrospectively reviewed. All patients received a questionnaire for follow-up, and 323 patients answered the questionnaire, thus forming the sample group. Results. Of all the responding patients, 15% had plates removed, and 92% of these patients experienced relief from discomfort after removal of the plates. Infection was the most common reason for plate removal (10%). Smoking (hazard ratio 2.74) and surgery performed in the mandible (hazard ratio 2.40) increased the need for plate removal. For each plate added in the mandible, the risk for removal increased by 34%. Conclusions. Smoking, osteotomies, and additional numbers of plates in the mandible resulted in a higher incidence of plate removal. Most of the patients experienced relief from discomfort after plate removal.

  • Compounds activating VCP D1 ATPase enhance both autophagic and proteasomal neurotoxic protein clearance

    • Enhancing the removal of aggregate-prone toxic proteins is a rational therapeutic strategy for a number of neurodegenerative diseases, especially Huntington's disease and various spinocerebellar ataxias. Ideally, such approaches should preferentially clear the mutant/misfolded species, while having minimal impact on the stability of wild-type/normally-folded proteins. Furthermore, activation of both ubiquitin-proteasome and autophagy-lysosome routes may be advantageous, as this would allow effective clearance of both monomeric and oligomeric species, the latter which are inaccessible to the proteasome. Here we find that compounds that activate the D1 ATPase activity of VCP/p97 fulfill these requirements. Such effects are seen with small molecule VCP activators like SMER28, which activate autophagosome biogenesis by enhancing interactions of PI3K complex components to increase PI(3)P production, and also accelerate VCP-dependent proteasomal clearance of such substrates. Thus, this mode of VCP activation may be a very attractive target for many neurodegenerative diseases. Several neurodegenerative diseases are characterized by the aggregation of cytoplasmic proteins. Here, the authors demonstrate that the small molecule SMER28 activates VCP, which enhances both autophagic and proteasomal clearance of aggregate-prone proteins.
