(författare) Metabolism of 4 beta -hydroxycholesterol in humans 2002 Ingår i: Journal of Biological Chemistry. - 0021-9258 .- 1083-351X. ; 277:35, s. 31534-31540 Tidskriftsartikel (refereegranskat)abstract One of the major oxysterols in the human circulation is 4 beta-hydroxycholesterol formed from cholesterol by the drug-metabolizing enzyme cytochrome P450 3A4. Deuterium-labeled 4 beta-hydroxycholesterol was injected into two healthy volunteers, and the apparent half-life was found to be 64 and 60 h, respectively. We have determined earlier the half-lives for 7 alpha-, 27-, and 24-hydroxycholesterol to be approximately 0.5, 0.75, and 14 h, respectively. Patients treated with certain antiepileptic drugs have up to 20-fold increased plasma concentrations of 4 beta-hydroxycholesterol. The apparent half-life of deuterium-labeled 4 beta-hydroxycholesterol in such a patient was found to be 52 h, suggesting that the high plasma concentration was because of increased synthesis rather than impaired clearance. 4 beta-Hydroxycholesterol was converted into acidic products at a much slower rate than 7 alpha-hydroxycholesterol in primary human hepatocytes, and 4 beta-hydroxycholesterol was 7 alpha-hydroxylated at a slower rate than cholesterol by recombinant human CYP7A1. CYP7B1 and CYP39A1 had no activity toward 4 beta-hydroxycholesterol. These results suggest that the high plasma concentration of 4 beta-hydroxycholesterol is because of its exceptionally slow elimination, probably in part because of the low rate of 7 alpha-hydroxylation of the steroid. The findings are discussed in relation to a potential role of 4 beta-hydroxycholesterol as a ligand for the nuclear receptor LXR.

(författare) Hyperglycemia and insulin infusion in pancreatoduodenectomy : a prospective cohort study on feasibility and impact on complications 2023 Ingår i: International journal of surgery (London, England). - 1743-9159. ; 109:12, s. 3770-3777 Tidskriftsartikel (refereegranskat)abstract BACKGROUND: Hyperglycemia is a risk factor for postoperative complications but its impact on outcome after pancreatoduodenectomy (PD) is scarcely studied. This prospective cohort study aimed to assess the effect of continuous insulin infusion on postoperative complications and blood glucose, as well as to evaluate the impact of hyperglycemia on complications, after PD. MATERIALS AND METHODS: One hundred patients planned for PD at Skåne University Hospital, Sweden were prospectively included for perioperative continuous insulin infusion and a historic cohort of 100 patients was included retrospectively. Median blood glucose levels were calculated and data on complications were analyzed and compared between the historic cohort and the intervention group as well as between normo- and hyperglycemic patients. RESULTS: Median glucose levels were significantly lower in the intervention group compared to the historic cohort up to 30 days postoperatively (median glucose 8.5 mmol/l (interquartile range 6.4-11) vs. 9.1 mmol/l (interquartile range 6.8-17) ( P =0.007)). No significant differences in complication rates were recorded between these two groups. The incidence of complications classified as Clavien ≥3 was higher in hyperglycemic patients (100 vs. 27%, P =0.024). Among hyperglycemic patients the prevalence of preoperative diabetes was higher compared to normoglycemic patients (52 vs.12%, P <0.001). In patients with a known diagnosis of diabetes, a trend, although not statistically significant, towards a lower incidence of postoperative pancreatic fistula grade B and C, as well as postpancreatectomy hemorrhage grade B and C, was seen compared to those without preoperative diabetes (6.8 vs. 14%, P =0.231 and 2.3 vs. 7.0%, P =0.238, respectively). CONCLUSION: Insulin infusion in the early postoperative phase after PD is feasible in a non-ICU setting and significantly decreased blood glucose levels. The influence on complications was limited. Preoperative diabetes was a significant predictor of postoperative hyperglycemia and was associated with a lower incidence of clinically significant postoperative pancreatic fistula.

Patients undergoing pancreatoduodenectomy have an increased risk of hyperglycaemia after surgery which is aggravated by parenteral nutrition therapy. This study aims to evaluate glycaemic control and safety during insulin infusion in a surgical non-ICU ward, using a hybrid glucose monitoring approach with isCMG and periodic point-of-care (POC) testing. Methods: We prospectively included 100 patients with a resectable pancreatic tumour. After surgery, continuous insulin infusion was initiated when POC glucose was > 7 mmol/l and titrated to maintain glucose between 7 and 10 mmol/l. Glucose was monitored with isCGM together with intermittent POC, every 3–6 h. Median absolute relative difference (MARD) and hypoglycaemic events were evaluated. Mean glucose was compared with a historic control (n = 100) treated with multiple subcutaneously insulin injections, monitored with POC only. Results: The intervention group (isCGM/POC) had significantly lower POC glucose compared with the historic control group (8.8 ± 2.2 vs. 10.4 ± 3.4 mmol/l, p < 0.001). MARD was 17.8% (IQR 10.2–26.7). isCGM readings were higher than POC measurements in 91% of the paired cases, and isCGM did not miss any hypoglycaemic event. About 4.5% of all isCGM readings were < 3.9 mmol/l, but only six events were confirmed with POC, and none was < 3.0 mmol/l. Conclusions: A hybrid approach with isCGM/POC is a safe and effective treatment option in a non-ICU setting after pancreatoduodenectomy.

Vol. 4, Early Etruscan akroteria from Acquarossa and Poggio Civitate (Murlo) 1983 Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract Two kinds of early Etruscan akroteria - cut-out and relief-modelled - are presented and analyzed for the first time in this study. The material comes from the sites of Acquarossa and Poggio Civitate (Murlo), where it belonged to the decoration of residential architecture.Starting from the fragments (numbering 182), the minimum number of akroteria once present at the two sites is calculated (52), with the help of various criteria. Their relationship to the single buildings is discussed on the basis of find-places. An analysis of their arrangement on the roofs show them to conform to three different systems; one of these is closely parallelled by ash urns in the shape of houses from Cerveteri. In the matter of technique, the cut-out akroteria are distinguished by their manufacture individually through cutting, and so are a witness to a stage of production preceding the use of moulds. The relief-modelled akroteria were probably also handmade. Iconographically and stylistically the akroteria are shown to conform well with the tendencies of the early Etruscan arts in general, being sometimes adaptations of designs current in the minor arts. On the whole, they testify to the ambitions of socially leading classes to embellish not only the interior but also the exterior of their houses. A majority may be dated with the help of stratigraphy or iconography/style or a combination; most of the cut-out akroteria thus fall in the period of 625-575 B.C. (Late Orientalizing), the relief-modelled ones around 575 B.C. (beginning of the Archaic period).Similar material is clearly documented at Tuscania only, but indirect evidence suggests a widespread, original presence in Etruria of at least the cut-out akroterion. The apparent lack of corresponding akroteria in the Greek world, together with the early documented, Italic propensity towards roof decoration, leads to the conclusion that the akroteria in question were part of an old, local tradition. At the introduction in Italy of the Greek tile roof, they became prominent among the earliest architectural terracotta of the Etruscans. Later, they influenced, if indirectly, Etruscan akroteria produced as late as the fifth century B.C.

Om avfärdsmotivet i bildframställningar av den romerska apoteosen 2016 Ingår i: Tahiti. - 2242-0665. ; 4 Tidskriftsartikel (refereegranskat)abstract When the leading Roman magistrates of the Republic, and later on the Roman emperors, made official appearances in connection with important political and military events and functions, some urban locations in the cityscape became symbolically charged, in Rome especially Forum Romanum, Capitolium and the city gates. The ceremonial acting in front of the assembled public was dependent on measures adopted to organize space and movement. Studying the patterns of acting starting from space and movement—space and movement are signal words in actual research—one may detect factors of importance for the cognitive underpinning of the ceremonies.Taking my cue from space and movement, I study images of the imperial apotheosis of Roman emperors, i.e. the divinization of Roman emperors by way of a ceremonial act at the Field of Mars in Rome (Campus Martius). Most of the material is made up of marble reliefs for public exposure. A natural question is how space and movement as related to the actual performance of the ceremony were transposed, or not transposed, into the image. Another question concerns how the visual representation of the vertical transport of the soul of the deceased into heaven and the Olympus, i.e. the cognitive kernel of apotheosis, was realized. I point at the iconographic model that became dominant in the public imagery, viz. departure, and why: it is connected to the notion of becoming a god, in the presence of people witnessing the start of the process. I also indicate the relevance of the apparent associations of space with the living and movement with the deceased. The space-movement elaboration in fact forms part of the construction of the Roman apotheosis as a visual motif.

(författare) Integrerad behandling av missbruk och psykisk sjukdom : Pilotimplementering inom LVM-vården 2021 Rapport (populärvet., debatt m.m.)abstract Samsjuklighet bland personer med missbruk och/eller beroende är vanligt förekommande. Exempelvis är psykiska sjukdomar tre gånger så vanligt hos de som missbrukar alkohol, jämfört med hos personer som inte gör det. Särskilt vanligt förekommande är samsjuklighet av missbruk och psykiska sjukdomar bland klienter som befinner sig i missbruksvårdens sammanhang. Tidigare forskning har visat att behandling av missbruk och beroende med psykiatrisk samsjuklighet är mest effektiv när insatserna integreras. Trots detta saknas sådan behandling på bred basis. Syftet med föreliggande projekt var att pilottesta integrerad behandling i LVM-vårdens kontext, med huvudfokus på genomförbarhet och hållbarhet. Ett underordnat syfte var att beskriva effekterna av sådan behandling. Två behandlingsmodeller pilottestades vid tre SiS-institutioner. Den ena modellen, Integrating Combined Therapies (ICT), pilottestades vid två institutioner och den andra modellen, Enhanced Illness Management and Recovery (E-IMR), testades vid en institution. Totalt ingick 70 klienter i ICT-behandlingsgruppen, och elva klienter i E-IMR-gruppen. Utöver detta intervjuades 14 anställda vid de tre olika institutionerna. Implementeringen av E-IMR tog inte fäste, huvudsakligen på grund av yttre organisatoriska omständigheter. Vad gäller ICT, tyder resultaten på att modellen är genomförbar inom LVM-vården. Likaså föreligger goda tecken på hållbarhet. Det är dock viktigt att beakta att denna pilotimplementering är en relativt liten studie där det inte funnits någon kontrollgrupp. Utifrån designen på forskningsprojektet går det därför inte att dra säkra slutsatser om de positiva effekter av ICT som observera
