    • The restricted immunoglobulin (Ig) repertoire found in B-cell chronic lymphocytic leukemia (CLL) implies a role for antigen(s) in the leukemogenesis. The nature of the antigens has, however, not been characterized, although examples of autoantigens have been demonstrated. We have analyzed a panel of 28 CLL cell lines and primary cultures, producing monoclonal Ig with different Ig heavy-chain variable region gene usage and mutational status, including several complementarity determining region 3 homology subset members. Using mass-spectrometry, immunoassays, or protein macroarrays, we have discovered novel antigens binding to CLL Igs. These antigens included cytoskeletal proteins vimentin, filamin B, and cofilin-1, but also phosphorylcholine-containing antigens (eg, Streptococcus pneumoniae polysaccharides and oxidized low-density lipoprotein [oxLDL]). Additional new antigens identified were cardiolipin and proline-rich acidic protein-1. Remarkably, these antigens represent molecular motifs exposed on apoptotic cells/blebs and bacteria, and several CLL Igs bound to apoptotic Jurkat cells. In conclusion, these intriguing data, showing a limited target structure recognition, indicate that CD5(+) CLL B cells are derived from a cell compartment that produces "natural antibodies," which may be instrumental in elimination and scavenging of apoptotic cells and pathogenic bacteria.

  • Apartment price determinants : A comparison between Sweden and Germany

    • Similar development of economic fundamentals in Germany over the last two decades did not lead to the same dramatic house price increases as it is in Sweden. What can explain this house price stability over a long period? This thesis attempts to find the answer this question.The first paper in this thesis contains an extended literature review on the studies focused on the factors affecting house prices in the short and in the long run. Existing literature adopts a broad variation of approaches and reaches different conclusions attempting to answer the question about what are the key drivers of house prices. Conclusions often depend on the model specifications and econometric methods applied. Though there is a considerable agreement in real estate economics theory regarding the main factors that affect house prices (or so called “fundamental determinants”), it is hard to find a consistent definition regarding what factors can be considered as “fundamentals” and what factors belong to “non-fundamentals”. The dominating factors that are presented in the majority of the studies are income, population, interest rate, housing stock and unemployment. Studies done after the recent financial crisis put more attention on such factors as the behavior of the market participants, financing conditions and regulations. The characteristics of the bank lending and valuation policies as well as regulations on the rental market have received attention in the research literature, but the impact of these factors on house price dynamics is not measured and not well described. Therefore the other two papers in this thesis aim to provide a better insight in to the factors that create fluctuations in housing markets.The second paper investigates the effects of macroeconomic indicators such as population, income housing stock, mortgage interest rate on house prices. Estimation is done by applying panel data methodology on regional data for major cities in Germany and Sweden and by using yearly observations from 1995 to 2010. Results suggest that the long-run development of apartment prices in Sweden can be explained by changes in such factors as population, disposable income per capita, mortgage interest rate, housing stock, and prices per square meter in the previous period. The price for the previous period has the highest impact in comparison with other factors in Sweden. At the same time for Germany this is the only factor that is valid for long-term house price development. Estimates for fundamental factors such as population, disposable income, mortgage interest rate and housing stock appeared as not significant in house price development in the long run in Germany. A closer analysis has shown that the fundamental factors developed in a similar way in both countries during the analyzed period, though the house prices dynamic is very different. The conclusion is that fundamental factors cannot provide an explanation for the differences in house price developments in two countries and further analysis of institutional differences in the housing markets is done in the third paper.Third paper applies a comparative analysis approach and hypothetico-deductive method in order to examine the differences in the banking policies on mortgage financing and approaches to valuation of mortgage properties in Germany and Sweden.  The results suggest that the extreme rise in Swedish house prices above the long-term trend was created by expanding bank lending policies that was supported by the general macroeconomic factors and regulation environment on the housing market. The main difference between countries in approaches to valuation for mortgage purposes is that in Germany that mortgage is based not on the market value as it is in Sweden, but on the long-run sustainable value, so called “fundamental” value. Mortgage lending value is determined in such a way that is also develops in the same tempo as fundamentals in the long-run and is not that procyclical as market value. Using a long-term sustainable value has a restrictive effect on the housing prices and in such a way stabilizes the market.  One more factor that gives stability to the housing market in Germany is the well-functioning rental market. Third paper contributes to a better understanding of necessary conditions for the house prices to rise in the long run above the fundamentals level and suggests policy solutions that can reduce the risks of housing bubbles and increase financial stability.

  • Body weight at age 20 and in midlife is more important than weight gain for coronary atherosclerosis: Results from SCAPIS.

    • Elevated body weight in adolescence is associated with early cardiovascular disease, but whether this association is traceable to weight in early adulthood, weight in midlife or to weight gain is not known. The aim of this study is to assess the risk of midlife coronary atherosclerosis being associated with body weight at age 20, body weight in midlife and body weight change.We used data from 25,181 participants with no previous myocardial infarction or cardiac procedure in the Swedish CArdioPulmonary bioImage Study (SCAPIS, mean age 57 years, 51% women). Data on coronary atherosclerosis, self-reported body weight at age 20 and measured midlife weight were recorded together with potential confounders and mediators. Coronary atherosclerosis was assessed using coronary computed tomography angiography (CCTA) and expressed as segment involvement score (SIS).The probability of having coronary atherosclerosis was markedly higher with increasing weight at age 20 and with mid-life weight (p<0.001 for both sexes). However, weight increase from age 20 until mid-life was only modestly associated with coronary atherosclerosis. The association between weight gain and coronary atherosclerosis was mainly seen in men. However, no significant sex difference could be detected when adjusting for the 10-year delay in disease development in women.Similar in men and women, weight at age 20 and weight in midlife are strongly related to coronary atherosclerosis while weight increase from age 20 until midlife is only modestly related to coronary atherosclerosis.

    • Background: Early detection of coronary atherosclerosis using coronary computed tomography angiography (CCTA), in addition to coronary artery calcification (CAC) scoring, may help inform prevention strategies. We used CCTA to determine the prevalence, severity, and characteristics of coronary atherosclerosis and its association with CAC scores in a general population.Methods: We recruited 30 154 randomly invited individuals age 50 to 64 years to SCAPIS (the Swedish Cardiopulmonary Bioimage Study). The study includes individuals without known coronary heart disease (ie, no previous myocardial infarctions or cardiac procedures) and with high-quality results from CCTA and CAC imaging performed using dedicated dual-source CT scanners. Noncontrast images were scored for CAC. CCTA images were visually read and scored for coronary atherosclerosis per segment (defined as no atherosclerosis, 1% to 49% stenosis, or ≥50% stenosis). External validity of prevalence estimates was evaluated using inverse probability for participation weighting and Swedish register data.Results: In total, 25 182 individuals without known coronary heart disease were included (50.6% women). Any CCTA-detected atherosclerosis was found in 42.1%; any significant stenosis (≥50%) in 5.2%; left main, proximal left anterior descending artery, or 3-vessel disease in 1.9%; and any noncalcified plaques in 8.3% of this population. Onset of atherosclerosis was delayed on average by 10 years in women. Atherosclerosis was more prevalent in older individuals and predominantly found in the proximal left anterior descending artery. Prevalence of CCTA-detected atherosclerosis increased with increasing CAC scores. Among those with a CAC score >400, all had atherosclerosis and 45.7% had significant stenosis. In those with 0 CAC, 5.5% had atherosclerosis and 0.4% had significant stenosis. In participants with 0 CAC and intermediate 10-year risk of atherosclerotic cardiovascular disease according to the pooled cohort equation, 9.2% had CCTA-verified atherosclerosis. Prevalence estimates had excellent external validity and changed marginally when adjusted to the age-matched Swedish background population.Conclusions: Using CCTA in a large, random sample of the general population without established disease, we showed that silent coronary atherosclerosis is common in this population. High CAC scores convey a significant probability of substantial stenosis, and 0 CAC does not exclude atherosclerosis, particularly in those at higher baseline risk.

    • RATIONALE: Chronic obstructive pulmonary disease (COPD) includes respiratory symptoms and chronic airflow limitation (CAL). In some cases, emphysema and impaired diffusing capacity for carbon monoxide (DLCO) are present, but characteristics and symptoms vary with smoking exposure.OBJECTIVES: To study the prevalence of CAL, emphysema and impaired DLCO in relation to smoking and respiratory symptoms in a middle-aged population.METHODS: We investigated 28,746 randomly invited individuals (52% women) aged 50-64 years across six Swedish sites. We performed spirometry, DLCO, high-resolution computed tomography (HRCT) and asked for smoking habits and respiratory symptoms. CAL was defined as post-bronchodilator forced expiratory volume in 1 second divided by forced expiratory volume (FEV1/FVC)<0.7.RESULTS: The overall prevalence was for CAL 8.8%, for impaired DLCO (DLCOCONCLUSIONS: In this large population-based study of middle-aged people, CAL and impaired DLCO were associated with common respiratory symptoms. Self-reported asthma was not associated with CAL in never-smokers. Our findings suggest that CAL in never-smokers signifies a separate clinical phenotype that may be monitored and, possibly, treated differently from smoking-related COPD.   • Pulmonary function and atherosclerosis in the general population : causal associations and clinical implications

    • Reduced lung function is associated with cardiovascular mortality, but the relationships with atherosclerosis are unclear. The population-based Swedish CArdioPulmonary BioImage study measured lung function, emphysema, coronary CT angiography, coronary calcium, carotid plaques and ankle-brachial index in 29,593 men and women aged 50–64 years. The results were confirmed using 2-sample Mendelian randomization. Lower lung function and emphysema were associated with more atherosclerosis, but these relationships were attenuated after adjustment for cardiovascular risk factors. Lung function was not associated with coronary atherosclerosis in 14,524 never-smokers. No potentially causal effect of lung function on atherosclerosis, or vice versa, was found in the 2-sample Mendelian randomization analysis. Here we show that reduced lung function and atherosclerosis are correlated in the population, but probably not causally related. Assessing lung function in addition to conventional cardiovascular risk factors to gauge risk of subclinical atherosclerosis is probably not meaningful, but low lung function found by chance should alert for atherosclerosis.

    • Kustfiskprojektet är ett specialprojekt inom den nationella miljöövervakningen och har finansierats av Naturvårdsverket, Fiskeriverket samt länsstyrelserna. Deltagare i projektet sedan starten 2002 har varit länsstyrelserna i Norrbottens, Västerbottens, Västernorrlands, Gävleborgs, Uppsala, Stockholms och Södermanlands län, Fiskeriverkets Kustlaboratorium i Öregrund, Fiskeriverkets Utredningskontor i Luleå och Härnösand samt Umeå Marina Forskningscentrum. Föreliggande rapport tillsammans med undersökningstyp i Naturvårdsverkets Handbok för Miljöövervakning och områdesvisa resultatblad tillgängliga på Fiskeriverkets hemsida utgör slutredovisning av projektet.SMHI:s indelning i kustvattentyper (Håkansson och Hansson 2003) har legat till grund för ett förslag till dimensionering av ett samordnat övervakningsprogram av kustfisk. För att möjliggöra uppföljning av förändringar på havsbassängnivå har projektet dimensionerat ett övervakningsprogram med minst ett referensområde per kustvattentyp av inre- och mellantyp, alternativt minst två områden per havsbassäng, vilket ger elva referensområden i Östersjön. I den föreslagna dimensioneringen har inte ett eventuellt behov av regional förtätning av provfiskeområden i Egentliga Östersjöns kustvatten (söder om Södermanlands läns kustvatten) beaktats. Referensområdena är utvalda enligt de kriterier som tagits fram inom projektet. Så långt möjligt har urvalet av nya områden gjorts så att både inneslutna kustmiljöer och mer exponerade kuststräckor är representerade.Den reviderade provtagningsstrategi och metodik, Djupstratifierat provfiske med Nordiska kustöversiktsnät, som har testats inom kustfiskprojektet har visats bättre kunna skatta kustfiskbestånden i ett provfiskeområde samt bättre kunna följa förändringar i biologisk mångfald och rekryteringsframgång än tidigare provtagningsmetodik. Provfiskemetodiken är inriktad mot varmvattenarter så som abborre, gers, gös, gädda och de flesta karpfiskar som mört, braxen etc. Abborre fungerar som modellart, vilket innebär att programmet är fördjupat för den arten med beskrivning av köns- och åldersfördelning. Projektet rekommenderar att den färdigställda undersökningstypen tillämpas vid provfiske i kustvatten där syftet är att följa förändringar i fisksamhället.Projektet lämnar förslag till indikatorer för uppföljning av förändringar i kustfisksamhället. Indikatorer har valts inom olika indikatorgrupper; Fisksamhällets diversitet, fisksamhällets kvantitet och struktur, fisksamhällets funktion samt individdata för abborre.Förekomst av rekryteringsområden i provfiskeområdets närhet är ett av de kriterier som ligger till grund för val av referensområden för kustfiskövervakning. Inom projektet har potentiellt intressanta områden för fiskrekrytering i kustmiljö identifierats för samtliga provfiskeområden i det samordnade programmet. Projektet förordar att vattentemperatur ska följas i en representativ grund miljö i området samt att fysisk påverkan bör kartläggas och följas upp regelbundet i provfiskeområdets närhet.Projektet föreslår en organisation och ansvarsfördelning som ska säkerställa driften av det samordnade programmet på lång sikt. Kustlaboratoriet i Öregrund ansvarar för samordningen av det nationella/regionala programmet för kustfiskövervakning i Östersjön. Det regionala och nationella nätverk som har byggts upp under projektets gång skapar förutsättningar för bättre samordning av den regionala och nationella marina miljöövervakningen. De Marina Forskningscentra bör vara naturliga intressenter i den kustsamverkan som utvecklas i Östersjön.Det är kustfiskprojektets förhoppning att erfarenheter och strategier som tagits fram inom projektet ska kunna användas som modell för nationell/regional samverkan avseende övervakning av kustfisk i andra kustområden i Östersjön och för annan typ av övervakning i marina ekosystem än kustfisk. Samordning av resurser, metoder och strategier mellan nationell och regional tidsserieövervakning är en förutsättning för att på lång sikt kunna upprätthålla en hög kvalitet på kustfiskövervakning i Östersjön.

  • Hearing-related symptoms and occupational noise exposure among women: An intervention study in preschool and obstetrics care

    • Recent cross-sectional studies have showed that preschool teachers and obstetrics personnel risk developing hearing-related symptoms associated with noise exposure. An ongoing cohort follow-up and intervention study will provide causal interpretations and evaluation of preventive factors. This presentation will include an overview of the project with focus on the intervention study, assessing effects on self-reported noise exposure and hearing-related symptoms. The intervention was performed at one preschool and one obstetrics care unit with two control workplaces each. A participatory approach was used, involving personnel, managers and health and safety representatives. The process followed a common structure in identifying, selecting, implementing and evaluating the interventions, whereas the interventions were tailored to each workplace. Survey data was collected at baseline and two follow-ups, and analysed using generalised estimating equation (GEE). We did not find significant positive effects of the intervention. Factors such as limited resources for implementation and other work environment deteriorations probably explain some of the negative results. Meanwhile, qualitative data indicated positive experiences of the interventions. Thus, we suggest further studies with control of implementation and other changes in the workplace.

    • A participatory-based intervention was performed in Sweden, aimed at improving the sound environment in one preschool (n=20) and one obstetric ward (n=50), with two controls each (n=28, n=66). Measured sound levels, and surveys of noise annoyance, hearing-related symptoms and emotional exhaustion were collected before, and three and nine months after the interventions, comparing intervention and control groups over time. The results of this first implementation in a limited number of workplaces showed significantly worsening of hyperacusis, sound-induced auditory fatigue, emotional exhaustion and increased sound levels in the preschool, and worsening of noise annoyance in both intervention groups. Increased risk awareness, limited implementation support and lack of psychosocial interventions may explain the worsening in outcomes, as might the worse baseline in the intervention groups. The complexity of the demands in human-service workplaces calls for further intervention studies.

    • Personnel in communication-intense workplaces are exposed to noise, occasionally so high they risk hearing-related disorders. Still, intervention studies are few, with ambiguous results. To increase the impact and sustainability of the interventions, we used a participatory method in which personnel and managers at one preschool and one obstetrics ward were actively involved in the intervention process. To identify work environment problems to target in the intervention, we performed qualitative focus group interviews prior to the intervention among 16 personnel and managers. Four main themes emerged from the inductive thematic analysis: a challenging and harmful sound environment, the sound environment causing hearing-related symptoms and health effects, a good sound environment not being prioritised, and resourceful and motivated personnel. As the participatory intervention entailed implementation of several changes, we also performed interviews post-intervention among nine personnel and managers from the preschool and the obstetrics ward where interventions had been implemented. Five main themes emerged from the inductive thematic analysis: increased awareness of the sound environment and risks to hearing and health, increased control over the sound environment, influence of the building and interior design, other factors influencing the intervention process (concurrent work environment issues such as stress, and limited resources), and motivation to maintain the positive changes. We conclude that the personnel perceive a risk to their hearing and health and that sound environment improvements should be prioritised in communication-intense workplaces. A participatory approach may facilitate motivation and empowerment to change, but resources and support are needed for comprehensive and effective implementation.

Purpose: To assess whether working in preschools increases the risk of hearing-related symptoms and whether age, occupational noise, and stressful working conditions affect the risk. Methods: Questionnaire data on hearing-related symptoms were analysed in women aged 24–65 (4718 preschool teachers, and 4122 randomly selected general population controls). Prevalence and risk ratio (RR) of self-reported hearing loss, tinnitus, difficulty perceiving speech, hyperacusis and sound-induced auditory fatigue were assessed by comparing the cohorts in relation to age and self-reported occupational noise and stressful working conditions (effort–reward imbalance and emotional demands). RR was calculated using log-binomial regression models adjusted for age, education, income, smoking, hearing protection, and leisure noise. Incidence rates and incidence rate ratios (IRR) were calculated for retrospectively reported onset of all symptoms except sound-induced auditory fatigue. Results: Compared to the controls, preschool teachers had overall more than twofold RR of sound-induced auditory fatigue (RR 2.4, 95% confidence interval 2.2–2.5) and hyperacusis (RR 2.3, 2.1–2.5) and almost twofold for difficulty perceiving speech (RR 1.9, 1.7–2.0). Preschool teachers had a threefold IRR of hyperacusis (IRR 3.1, 2.8–3.4) and twofold for difficulty perceiving speech (IRR 2.4, 2.2–2.6). Significantly although slightly less increased RR and IRR were observed for hearing loss and tinnitus. RR and IRR were generally still increased for preschool teachers when stratified by age and occupational exposure to noise and stress. Conclusions: This large cohort study showed that working as preschool teacher increases the risk of self-reported hearing-related symptoms, indicating a need of preventative measures.

    • Abstract BACKGROUND: Noise is a common workplace problem that can affect health and performance. High sound levels have been found in sectors that largely has been overlooked in noise research such as health care and education. In these communication-intense environments the work requires speech communication, thus making it difficult to wear hearing protection. OBJECTIVE: To explore nurses’ and preschool teachers’ experiences of taking part in a participatory intervention project aiming to improve the sound environment and the psychosocial work environment. METHODS: One preschool and one obstetrics ward took part in the study, and a qualitative design was used to evaluate the experience of the participatory intervention approach. RESULTS: Five main themes were found in the analysis: Awareness; Taking control of the sound environment; Influence of the building and interior design; Circumstances influencing the intervention process; and Motivation to maintain change. CONCLUSIONS: Despite demanding working situations and lack of financial resources, preschool and obstetrics staff described being creative in planning and implementing several different solutions to improve the sound environment at their workplaces, while interventions specifically improving the psychosocial work environment were fewer. Hence, our study suggest that a participatory intervention approach may facilitate participation and motivation, but resources and support are needed for a comprehensive and effective implementation.

    • Hösten 2008 och våren 2009 gav ISV4 vid Linköpings universitet en uppdragsutbildning som omfattade 7.5 högskolepoäng. Kursen hette Brottsprevention och samhällsarbete. Kursen vände sig till personer som arbetar brottspreventivt hos polisen, åklagarmyndigheten, kommunen, kriminalvården, skattemyndigheten, fastighetsbolagen och frivillig-organisationerna mfl. I Kursen deltog 18 personer och kursansvariga var fil.dr. Kerstin Johansson, lektor vid ISV tillsammans med Christina Söderberg adjunkt vid ISV och drogsamordnare i Norrköpings kommun. Antologin presenterar kursens innehåll och idé. Vidare presenteras kursens hörnstenar vilka var samhällsarbete, prevention och samverkan. I första hand har vi dock samlat kursdeltagarnas slutexaminationer. Det främsta syftet med antologin är att visa ett, som vi tycker och kursvärderingarna visade, gott exempel på det goda mötet mellan yrkesverksamma och s.k. vetenskaplig kunskap och hur vi tillsammans tog några steg mot ett ”bättre preventivt arbete”. I kapitel 1 beskrivs antologin. I kapitel 2, som bygger på en av kursens föreläsningar, gör Kerstin Johansson en översiktlig beskrivning och problematisering av socialt samhällsarbete i Sverige. I kapitel 3 beskriver Christina Söderberg begreppen prevention och samverkan samt något av den aktuella forskningen kring dessa fält. Under kursen hölls flera föreläsningar på dessa teman och de löpte som en röd tråd under kursens gång. Första examensarbetet presenteras i kapitel 4. Denna grupp valde att kartlägga och analysera det brottsförebyggande arbetet i Linköpings kommun utifrån struktur och mandat samt möjliga förbättringsområden. I kapitel 5 presenteras det brottpreventiva arbetet i Ystad och Staffanstorp utifrån ett samverkansperspektiv. Gruppen har också haft fokus på resurser i arbetet och eventuella effekter. En grupp valde att utifrån framgångsfaktorer som BRÅ tagit fram undersöka Norrköpings brottsförebyggande råd. Gruppen ville se om aktuell forskning och kunskap påverkat rådets arbete och även föreslå förbättringsområden. I kapitel 6 presenteras resultatet. Kapitel 7 utgör ett examensarbete där gruppen valde att granska de överenskommelser om samverkan som ingåtts mellan polismyndigheten i Östergötland och några av Östergötlands kommuner. Överenskommelserna syftar till att minska ungdomskriminaliteten i kommunerna och utgår från de riktlinjer som Rikspolisstyrelsen utfärdade 2007. I kapitel 8 presenteras det sista examensarbetet, där en grupp utifrån aktuell forskning arbetade fram ett förslag till utformning av föräldramöten och då utifrån samverkan mellan skola och andra samhällsaktörer som grundtanke.

    • Selenium treatment of the doxorubicin-resistant cell line, U-1285dox, derived from human small cell carcinoma of the lung, resulted in massive apoptosis. This effect appeared maximal at 2 days after addition of selenite. The apoptosis was caspase-3 independent as revealed by Western blot analysis, activity measurement and by using caspase inhibitors. Induction of apoptosis was significantly more pronounced and occurred after addition of lower concentrations of selenite in the doxorubicin-resistant cells compared to the parental doxorubicin-sensitive cells. High levels of selenite caused necrosis in the doxorubicin-sensitive cells. Analysis of enzymatic activity (insulin reduction) of thioredoxin reductase (TrxR) and TrxR protein concentration, measured by ELISA, revealed increasing activity and protein levels after treatment with increasing concentrations of selenium. Maximum relative increase was induced up to 1 μM in both sublines and at this selenium level the concentrations of TrxR measured as insulin reducing activity or ELISA immunoreactivity were nearly identical. Increasing concentrations of selenite up to 10 μM resulted in increased activity and concentration of TrxR in the sensitive subline but decreasing levels in the resistant subline. The level of truncated Trx (tTrx) was higher in the resistant U-1285dox cells but the level did not change with increasing selenite concentrations. Our results demonstrate pronounced selective selenium-mediated apoptosis in therapy-resistant cells and suggest that redox regulation through the thioredoxin system is an important target for cancer therapy.

    • Background and purpose — For want of national guidelines for lower limb amputation (LLA) the quality registry SwedeAmp was started in 2011 to increase knowledge around LLA and prosthetic rehabilitation. We now present data from the first 8 years of registration. Patients and methods — We present descriptive data from the first 8 years (2011–2018) of registration. Patient-reported outcome was collected at baseline and at follow-up 12 and 24 months after surgery for patients with prosthetic supply and included generic (EQ-5D-5L) and amputee-specific (e.g., LCI-5L and Prosthetic Use Score) measures. Sex differences were investigated. Results — As at December 31, 2018, 5,762 patients, 7,776 amputations, 2,658 prosthetic supplies, 1,848 baselines, and 2,006 follow-ups were registered. 61% of the patients were male, and mean age by the time of the first registered amputation was 74 years (SD 14). Women were older, more frequently had vascular disease without diabetes and more often underwent amputation at a higher level compared with men (p < 0.001). Time from amputation to fitting of first individual prosthesis was median 69 days (6–500) after transtibial amputation (TTA) and 97 days (19–484) after transfemoral amputation (TFA). The outcomes were lower after TFA than after TTA. Interpretation — SwedeAmp shows sex differences concerning amputation level, diagnosis, and age, leading to the conclusion that women have worse preconditions for successful prosthetic mobility after LLA. With increasing coverage, SwedeAmp can provide deeper knowledge with regard to patients undergoing LLA in Sweden.

    • The purpose of this study was to develop an equation with which to determine the tube current to be used in order to obtain a certain image noise level for differently sized children undergoing multi-slice computed tomography examination. The relationship between image noise and detector dose for different examination protocols was established for a LightSpeed Ultra, an eight slice CT from GEMS, using homogeneous water phantoms of different sizes. Three different anatomical areas (head, thorax and abdomen) were studied in 111 patients between 0 and 17 y of age. The mean ratio between the calculated and the measured noise in patient images was established for the different areas. Head examinations showed the best correlation (measured-to-calculated noise ratio = 1.01). In the thorax, the calculated noise was generally higher than the measured noise (ratio = 0.74), and in the abdomen, the opposite result was found (ratio = 1.20).

    • Introduction: The in-hospital rehabilitation of patients who have undergone surgery for hip fracture requires a team-based effort, in which nurses play an all-embracing role throughout the patients’ hospital stays. Although discharge planning has been widely studied, little is known about discharge planning from hospitals to homes in rural settings. Aim: To describe nurses’ views on discharge planning for older patients after hip fracture surgery who live in their own homes in rural areas. Methods: A qualitative method was used. Four focus group interviews were conducted with 18 nurses who work at an orthopaedic clinic. The interview texts were analysed with qualitative content analysis. Findings: Nurses expressed that patients needed support from healthcare personnel as well as relatives in order to prepare for life at home. They also expressed that patients were not supported in all aspects of discharge planning because they faced difficulties in having their voices heard. Nurses described that many of those aspects were beyond their own control, which had left them with little to non-ability to influence discharge planning. Findings additionally indicate that discharge planning seems not affected by occurring in rural settings. Conclusions: Although discharge planning is intended to meet the unique wishes and needs of each patient given the realities of existing resources, nurses’ responsibilities in discharge planning are unclear. This study shows an organisation in which healthcare personnel continue to make decisions for patients. Significance for nurses to perform a discharge planning that support patients’ participation seems to be a communication based on shared understanding.

    • Purpose The aim of this study was to elucidate meanings of participation in care for older people after hip fracture surgery and nurses working in an orthopaedic ward. Methods A qualitative phenomenological hermeneutical design was used. We conducted personal interviews with a narrative approach with 11 older people recovering from hip fracture surgery and 12 nurses working in an orthopaedic ward. Results The results show that for older people, participation meant being a co-creator in their own care, founded on being met with sensitivity and support, being told what is going to happen, taking responsibility and asking questions and being able to influence care. For nurses, patient participation meant meeting the patients' needs and requests by being open and allowing them to influence care while at the same time recognizing that the patients' possibility to influence care was limited. Conclusion The study shows that for older people and nurses, the phenomenon of participation has similar meanings but also differences. When older people participate in their care, they become a co-creator in care and confirmed as a person. This highlights the importance of a nurse-patient relationship built on trust, connectedness and communication based on a shared understanding.

    • This thesis falls within the field of nursing and focuses on older people’s recovery after hip fracture surgery in a rural context. A fall-related hipfracture is a common and severe health issue that affects older people, leaving them vulnerable and in need of support from close relatives and healthcare personnel during recovery. The overall aim of this doctoral thesis was to explore and describe the recovery of older people after hip fracture surgery from the perspectives of older people, close relatives, and nurses. Data were collected through qualitative individual interviews (I), focus group interviews (II), and narrative interviews (III, IV). The interviews were analyzed using qualitative content analysis (I, II) and phenomenological hermeneutics (III,IV).The findings show that older people found themselves in a new and vulnerable situation in which they were dependent on others for everyday activities they used to take for granted prior to the hip fracture. They were grateful to be able to return home after discharge from the hospital, although being at home would not have been possible without support from close relatives (I). For nurses, preparing patients for a life at home was vital, as they recognized the amount of support that was needed after hip fracture surgery. Nurses shared that certain aspects of discharge planning did not benefit the patient, but the nurses could not influence these (II). Furthermore, this study shows that for older people, participation in care means being a co-creator in care that is founded on being met with sensitivity and support, being told what is going to happen, taking responsibility, and asking questions and being able to influence care. For nurses, participation in care meant meeting the patients’ needs and requests by being open and allowing them to influence their care while also recognizing that the patients’ ability to influence care was limited. For older people and nurses, participation in care had somewhat different meanings, but it was associated with managing better at home (III). Being a close relative to an older person recovering from hip fracture surgery meant being a helper and involved facing the unimaginable yet expected, putting oneself aside, encountering the healthcare personnel, giving support, feeling concern and fear, and noticing recovery (IV). In conclusion, the findings of this thesis show that being affected by an acute, life-altering event such as a hip fracture greatly impacts the older person’s life. However, it seems that older people are given rather small opportunities to participate in and influence decisions about their care and discharge planning. Recovery after hip fracture surgery and the transition in daily life toward health seems to be influenced by the older person possessing selfdetermination and expectations of recovery, being a co-creator in care, and having supportive close relatives.

The interview texts were analyzed with qualitative content analysis.RESULTS: Patients described finding themselves in a new and vulnerable situation, dependent on others for simple everyday chores. They struggled to regain independence while staying positive, convinced that they would recover. Fear of another fall, as well as lack of information, made recovery at home difficult.CONCLUSION: Older people who experience hip fractures need support during the recovery process. Because recovery begins at the hospital, this study highlight patients' need to participate in recovery planning, as well as the need to have information about what it means to be affected by a hip fracture and how to prevent it from happening again. This study emphasizes that nurses' awareness of patients' need to participate in planning the recovery process is crucial for enabling patients' return to a daily life that is similar to their prefracture life.

    • This thesis deals with individual-oriented crime prevention interaction (ICPI) aimed at young people who commit - or are deemed to be at risk of committing - offences. Youth crime prevention is always topical, and a great deal of attention has been paid to such initiatives in recent years. One point of departure is that crime prevention practices are influenced by prevailing political ideologies. Another is that forms of interaction result in tension between care and control.Given the above points of departure, the purpose of this thesis is to investigate further how perceptions of individual-oriented crime prevention interaction are expressed in narratives, primarily from representatives of interacting organisations.The following questions have provided guidance for the study:How are individual-oriented crime prevention interaction and young people subject to ICPI described in local interaction practice?How do interacting representatives describe the criteria and difficulties in establishing interaction practice such as ICPI?How can interaction meetings take place in the practice of ICPI, and what importance do they have?To what extent – and how – is the tension between care and control expressed in local interaction practice?Particular interest is focused on the role of social services in ICPI and potential implications for social services and individual social workers.The theoretical framework consists of new institutional theory and the concept of human service organisations, Stanley Cohen’s theory of social control and Karl E. Weick’s theory of sense-making.This is a qualitative study, and the methods used involve group and individual interviews with representatives from interacting organisations, as well as individual interviews with a number of young people and their guardians. Observations from interaction meetings in respect of ICPI have also taken place.Face-to-face interaction meetings have served as a point of departure for the presentation of results in this thesis; and as regards meetings, earlier research indicates that meetings are important for interaction, but that they do not always fulfil the intended purpose. The thesis further develops arguments about meetings (the magic of meetings) and analyses how consensus, but also tensions, are expressed in ICPI meetings.ICPI, like other interaction, is a complex interaction practice and it can be difficult to perceive specific results. This study confirms the difficulties, but also provides a developed perspective and an attempt to understand the driving force behind the obvious desire to meet the demands for local youth crime initiatives through interaction. Shared sense is made in ICPI through the willingness of stakeholders and representatives to make a difference for young people and families, but also through the organisations’ pursuit of legitimacy.In the study, the relationship between care and control, sometimes manifested in tensions, becomes important. The tension between care and control is a dynamic relationship that occurs both within and between institutions. A care-based control logic is identified as a common logic emerging in ICPI. The creation of a common logic may even lead to institutionalisation of the forms of interaction, where ICPI has a tendency to become an institution in itself, with a common language and a common logic.

    • Denna licentiatuppsats behandlar brottspreventiv samverkan på individnivå, vilket är ett relativt nytt fenomen i Sverige och Skandinavien. Ett specifikt arbetssätt, samverkan mellan skola, socialtjänst, polis och fritidssektor – SSPF – undersöks i studien och övergripande studeras hur idén om brottspreventiv samverkan i form av SSPF konstrueras och motiveras, hur den tolkas och omtolkas av samverkansparterna samt hur den omsätts i praktik.Det mer specifika syftet med studien är att undersöka brottspreventiv samverkan på individnivå som idé och praktik. Tre olika fokus eller aspekter på detta belyses: policy/struktur; organisation/profession och praktik/familj.De frågeställningar som besvaras i studien och som är kopplade till ovanstående fokusområden är följande: Hur har denna form av samverkan runt unga växt fram och hur motiveras den? Hur ser de organisatoriska förutsättningarna ut och hur upplevs SSPF av samverkansparterna? Slutligen hur interagerar företrädare för samhällets institutioner/ de professionella med familjen?Tre kommuner ingår i undersökningen. Empiriskt material har samlats in  genom dokumentstudie, intervjuer i grupp och enskilt med samverkansparterna i de tre kommunerna, genom observationer vid SSPF-möten samt genom en enkät som gick ut till 30 kommuner och stadsdelar i Sverige.Resultaten visar att en koppling finns mellan den allmänna samhällsutvecklingen i stort och brottspreventionens utveckling. Allmänt kan sägas att talet om brottsprevention idag handlar allt mindre om strukturella förklaringar och åtgärder. I stället talas alltmer om kontroll och effektivitet, vilket bland annat innebär tidiga upptäckter och snabba insatser. För att lyckas med detta behöver samverkan komma till stånd. Resultaten visar också att polisens roll i individriktad prevention har blivit viktigare och framträder som den tydligaste aktören i SSPFsamverkan.Gällande förutsättningarna för SSPF-arbetet visar studien att såväl organisatoriska förutsättningar som exempelvis stabilitet i organisationer och implementeringsmöjligheter, som möjligheter till informationsutbyte på individnivå mellan parterna spelar stor roll för arbetet. De professionella bildar allians där den gemensamma bilden av den unge skapar mening. En spänning mellan stöd och kontroll blir synlig i  SSPF-arbetet, likväl som en sorts likriktning, där poliser agerar  socialarbetare och socialtjänsten talar om mer kontroll. Allians bildas  också mellan professionella och föräldrar, vilket främst syftar till ökad  kontroll runt den unge, men också till att stärka stödet till den unges  föräldrar. De professionellas strävan efter ett nytt sätt att arbeta för att  göra det bättre för målgruppen blir trots allt tydligt i studien.

    • Inom svenska kustområden utgör fisk en viktig resurs för såväl yrkesfisket som fritidsfisket. Därför är det av stor vikt att följa fiskbeståndens tillstånd och utvecklig. Tillståndet för de arter som har störst värde för yrkesfisket behandlas årligen i en sammanställning som baserats på uppskattningar gjorda inom Internationella havsforskningsrådet (ICES). Lekbeståndets storlek och fiskeridödligheten (det vill säga den andel av beståndet som fångas årligen) utgör riktmärken vid bedömningen av beståndens överlevnadsförmåga och fiskets varaktiga hållbarhet. Fångstkvoter reglerar sedan det yrkesmässiga fisket på dessa arter. För Egentliga Östersjön och Bottniska viken är lax, sill/strömming, skarpsill och torsk kvoterade arter. För övrig arter saknas kvoter och därmed motsvarande samlad bedömning av beståndens tillstånd och utveckling. I nuläget (mars 2004) är en resursöversikt under bearbetning hos Fiskeriverket. Den redogör för bedömningar av tillståndet hos de viktigaste icke kvoterade arterna (Fiskeriverket 2004). Fiskeriverkets standardiserade provfisken och andra undersökningar tillsammans med loggboksstatistik utgör underlag för bedömningarna som baseras på data från det senaste decenniet.

  • Samordnat program för övervakning av kustfisk i Bottniska viken och Stockholms skärgård – utveckling av undersökningstyp och indikatorer

    • Här har utvärderingen skett utifrån de indikatorer som HELCOM MONAS under våren 2004 föreslagit som relevanta indikatorer baserade på kustfisk. Indikatorerna har delats in i fisksamhällsnivå, populationsnivå samt individnivå och i denna rapport presenteras ett flertal exempel på indikatorer från respektive grupp. Projektet syftar till att utarbeta en strategi för ett samordnat program för övervakning av kustfisk i Bottniska viken och Svealands kustvatten och är inne på sitt andra år. Projektet är ett samarbete mellan Länsstyrelserna i Norrbottens, Västerbottens, Västernorrlands,Gävleborgs, Uppsala och Stockholms län, Fiskeriverket, Umeå Marina Forskningscentrum och Naturvårdsverket. Med tyngdpunkt i Bottniska viken och med en geografisk spridning längs hela Sveriges ostkust redovisas resultat från provfisken i fem områden där provfiske genomförts med Kustöversiktsnät enligt HELCOM guidelines,parallellt med provfiske med djupstratifierat/randomiserat provfiske med översiktsnät av typen Norden. Ytterligare fem områden har dessutom provfiskats med enbart översiktsnät av typen Norden. Provtagningstrategin har undersökts för att analysera effekter av provfiskeinsatsens storlek på antalet fångade arter och antalet fångade individer. Resultaten visar att djupstratifierat/randomiserat provfiske med översiktsnät av typen Norden på mellan 30 till 50 stationer per område i första hand ger en kvantitativ bild av abborrens abundans ner till 10 m djup. I ett flertal provfiskeområden kan även abundansen av mört och gers övervakas kvantitativt. För andra förekommande arter blir övervakningen av mer kvalitativ natur. När avståndet mellan nätstationer inom ett provfiskeområde analyserades visade det sig att i hälften av provfiskeområdena är närbelägna stationer mer likartade med avseende på fångst av abborre och mört än stationer med större inbördes avstånd. Vid jämförelse av betydelsen om samma provfiskestationer används upprepat år efter år eller om nya stationer slumpas ut varje år, antyder resultaten att mellanårsvariationen är större än den variation som beror av att nya provfiskestationer utslumpats. Sammanfattningsvis kan sägas att med reservation för något ökad risk för förlust i representativitet kan det vara fördelaktigt att fiska samma stationer år efter år i denna typ av övervakning av kustfisk. Det förslag till typindelning av kustvatten som SMHI har tagit fram med anledning av genomförandet av Ramdirektivet för vatten föreslås i denna rapport vara en grund för dimensioneringen av ett program för övervakning av kustfisk. Den tidigare geografiska spridningen av provfiskeområden har gjort att Kustfiskprojektet har valt att under år 2004 utöka programmet med två nya provfiskeområden.Resultat från provfisken under projektperioden 2002–2004 och den samlade kompetensen och erfarenheten inom projektet kommer att ligga till grund för den slutliga programstrategi som ska tas fram under år 2004/2005.

  • MEASURES OF WAIST AND HIP MODIFY SEX-SPECIFIC ASSOCIATIONS BETWEEN BODY MASS INDEX AND PREVALENCE OF CORONARY ARTERY CALCIFICATION IN OPPOSITE DIRECTIONS

    • Background: Obesity is associated with increased risk of cardiovascular disease. However, there is still a debate whether accumulation of fat in certain depots modifies this risk. Using data from the CArdioPulmonary bioImage Study (SCAPIS), we investigated if anthropometric measurements of obesity (waist and hip) modifies the risk of coronary artery calcification. Methods: In the first 15,810 participants in SCAPIS (mean age 58 years, 52% women), data on coronary artery calcification score (CACS) and anthropometry were recorded and traditional cardiovascular risk factors were measured. Body mass index (BMI) was categorized as; <25, 25-30, 30-35 and >35 kg/m2 , quartiles of waist and hip circumferences were constructed within each BMI category and compared using the lowest quartile as reference. Results were adjusted for site, age, smoking and diabetes status. Results: Obesity (BMI >30 kg/m2 ) was found in 21.9% of men and in 20.5% of women. In both sexes the odds ratio (OR) for CACS >0 increased with increasing BMI categories: comparing <25 and >35 kg/m2 , OR = 2.1 (95% CI: 1.6-2.7) for men and OR = 1.4 (1.2-1.8) for women. In addition, increasing quartiles of waist significantly increased the prevalence of CACS >0 for men [p = 0.05; OR = 1.2 (1.0-1.4) for highest quartile] and women [p = 0.005; OR = 1.3 (1.1-1.5)] while increasing quartiles of hip significantly decreased the prevalence for men [p = 0.005; OR = 0.8 (0.6-0.9)] and women [p = 0.04; OR = 0.8 (0.7-0.9)]. Data on education level and physical activity did not affect the model. Conclusion: Increased BMI is associated with increased prevalence of coronary artery calcification and the distribution of fat modifies this risk. Our results suggest that gluteofemoral adipose tissue (hip) counteracts the negative effects associated with BMI and abdominal adipose tissue (waist).

    • Background and Objectives. The immunoglobulin VH gene mutation status is a strong prognostic indicator in B-cell chronic lymphocytic leukemia (CLL), since unmutated VH genes are correlated with short survival. However, the traditional cut-off level dividing mutated and unmutated cases, i.e. more or less than 2% mutations, has been questioned and other cut-offs have been suggested. We investigated whether an alternative cut-off should be applied and the relation of mutational status to another prognostic marker, Binet staging. Design and Methods. VH gene mutation status was assessed in 332 CLL cases by polymerase chain reaction amplification and nucleotide sequencing and was further correlated with overall survival using different VH mutation cut-offs (1-7%) and Binet stage. Results. After testing different mutation borders, the 2% cut-off remained the best discriminative level for determining prognosis. Interestingly, prognostic stratification was improved by combining the information on VH gene mutation status with that of Binet stage: unmutated cases (all stages, n=151, mutated cases with stage A (n=77), and mutated cases with stage B or C (n=37) had a median survival of 82, 179 and 74 months, respectively. Interpretation and Conclusions. CLL cases displaying mutated VH genes with Binet stage B or C had a survival similar to that of unmutated cases and significantly shorter than that of mutated stage A CLL. Our result reveals clinical heterogeneity within the VH mutated CLL group by inclusion of Binet stage data, a finding which is of importance when considering surrogate marker(s) for VH mutation status. ©2005 Ferrata Storti Foundation.
