  • Evidence for a spin-aligned neutron-proton paired phase from the level structure of 92Pd

    • Shell structure and magic numbers in atomic nuclei were generally explained by pioneering work(1) that introduced a strong spin-orbit interaction to the nuclear shell model potential. However, knowledge of nuclear forces and the mechanisms governing the structure of nuclei, in particular far from stability, is still incomplete. In nuclei with equal neutron and proton numbers (N = Z), enhanced correlations arise between neutrons and protons (two distinct types of fermions) that occupy orbitals with the same quantum numbers. Such correlations have been predicted to favour an unusual type of nuclear superfluidity, termed isoscalar neutron-proton pairing(2-6), in addition to normal isovector pairing. Despite many experimental efforts, these predictions have not been confirmed. Here we report the experimental observation of excited states in the N = Z = 46 nucleus Pd-92. Gamma rays emitted following the Ni-58(Ar-36,2n)Pd-92 fusion-evaporation reaction were identified using a combination of state-of-the-art high-resolution c-ray, charged-particle and neutron detector systems. Our results reveal evidence for a spin-aligned, isoscalar neutron-proton coupling scheme, different from the previous prediction(2-6). We suggest that this coupling scheme replaces normal superfluidity (characterized by seniority coupling(7,8)) in the ground and low-lying excited states of the heaviest N = Z nuclei. Such strong, isoscalar neutron-proton correlations would have a considerable impact on the nuclear level structure and possibly influence the dynamics of rapid proton capture in stellar nucleosynthesis.

  • Thromboembolism in Muscle-Invasive Bladder Cancer : A Population-based Nationwide Study

    • BACKGROUND: Routine VTE prophylaxis within 30 days of radical cystectomy (RC) for urinary bladder cancer (UBC) is used to protect from venous thromboembolism (VTE). However, randomized studies and nationwide population-based studies are lacking.OBJECTIVE: To study VTE and risk factors for VTE in muscle-invasive UBC in a nationwide population-based series, with a focus on the association with RC with and without chemotherapy.MATERIALS AND METHODS: We studied all patients with clinical stage T2-T4 UBC diagnosed 1997 to 2014 in the Bladder Cancer Data Base Sweden (BladderBaSe). Previous VTE events and risk factors for VTE were registered from 1987. Cox regression analyses and Kaplan-Meier curves were performed to study risk factors for VTE and cumulative incidence of VTE.RESULTS: In 9720 patients (71% males) with a median age of 74 years 546 (5.6%) had VTE after diagnosis. In Cox analyses controlling for patient's and tumour characteristics, and risk factors for VTE, VTE after diagnosis and first treatment date were associated with chemotherapy with or without RC. Cumulative incidence of VTE increased during 24 months after diagnosis and first treatment date. VTE were less common in patients with previous cardiovascular disease.CONCLUSION: VTE was commonly observed after 30 days from diagnosis and from first treatment date in patients with T2-T4 UBC, particularly after chemotherapy. The findings suggest that long-term intervention studies of benefit and possible harms of VTE prophylaxis after UBC should be undertaken.

  • Dosimetric Impact of MRI Distortions : A Study on Head and Neck Cancers

    • Purpose: To evaluate the effect of magnetic resonance (MR) imaging (MRI) geometric distortions on head and neck radiation therapy treatment planning (RTP) for an MRI-only RTP. We also assessed the potential benefits of patient-specific shimming to reduce the magnitude of MR distortions for a 3-T scanner.Methods and Materials: Using an in-house Matlab algorithm, shimming within entire imaging volumes and user-defined regions of interest were simulated. We deformed 21 patient computed tomography (CT) images with MR distortion fields (gradient nonlinearity and patient-induced susceptibility effects) to create distorted CT (dCT) images using bandwidths of 122 and 488 Hz/mm at 3 T. Field parameters from volumetric modulated arc therapy plans initially optimized on dCT data sets were transferred to CT data to compute a new plan. Both plans were compared to determine the impact of distortions on dose distributions.Results: Shimming across entire patient volumes decreased the percentage of voxels with distortions of more than 2 mm from 15.4% to 2.0%. Using the user-defined region of interest (ROI) shimming strategy, (here the Planning target volume (PTV) was the chosen ROI volume) led to increased geometric for volumes outside the PTV, as such voxels within the spinal cord with geometric shifts above 2 mm increased from 11.5% to 32.3%. The worst phantom-measured residual system distortions after 3-dimensional gradient nonlinearity correction within a radial distance of 200 mm from the isocenter was 2.17 mm. For all patients, voxels with distortion shifts of more than 2 mm resulting from patient-induced susceptibility effects were 15.4% and 0.0% using bandwidths of 122 Hz/mm and 488 Hz/mm at 3 T. Dose differences between dCT and CT treatment plans in D-50 at the planning target volume were 0.4% +/- 0.6% and 0.3% +/- 0.5% at 122 and 488 Hz/mm, respectively.Conclusions: The overall effect of MRI geometric distortions on data used for RTP was minimal. Shimming over entire imaging volumes decreased distortions, but user-defined subvolume shimming introduced significant errors in nearby organs and should probably be avoided.

  • Dagfjärilar i naturbetesmarker, kraftledningsgator, på hyggen och skogsbilvägar : betydelse för miljöövervakning

    • Ett förändrat och intensifierat jordbruk har bland annat lett till att naturbetesmarker och andra gräsmarksbiotoper minskat i odlingslandskapet. Detta har fått till följd att många arter knutna till öppna gräsmarker minskat, däribland fåglar, växter och dagfjärilar. Dagfjärilar svarar snabbt på förändringar både i miljön och klimatet och kan därför vara viktiga indikatorarter för miljöövervakning. I år startar en nationell övervakning av dagfjärilar med hjälp av volontärer och de senaste fem åren har dagfjärilar inventerats i ängs- och betesmarker inom den nationella inventeringen av landskapet (NILS). För att få en rättvisande bild av hur situationen ser ut för dagfjärilar i Sverige kan det vara viktigt att systematiskt inventera olika typer av miljöer, inte enbart ängs- och betesmarker eller känt artrika platser. I den här studien jämför vi artrikedom, individantal och artsammansättning av dagfjärilar (Rophalocera) och bastardsvärmare (Zyganidaea) i naturbetesmarker och tre typer av öppna biotoper i skogslandskapet; hyggen, kraftledningsgator och skogsbilvägar. Vi delar också in fjärilarna efter val av värdväxt och flygtid på säsongen för att se om det finns några skillnader mellan biotoperna vad gäller art- och individrikedom inom dessa grupper. Vi undersöker hur mängden skog och betesmark inom det omgivande landskapet påverkar dagfjärilsfaunan på de olika platserna och hur många inventeringstillfällen (3, 5 eller 7) som krävs för att få tillförlitliga data på artrikedomen på en plats. Våra resultat visar att artsammansättningen av fjärilar skiljer sig mellan biotoperna och att alla bidrar med unika arter och således kompletterar varandra. Kraftledningsgatorhade både fler fjärilsarter och individer än de övriga biotoperna och hyggen och skogsbilvägar var lika art- och individrika som betesmarkerna. Det är därför viktigt att övervaka flera typer av miljöer och även skogsbiotoper inom framtida dagfjärilsövervakning. Eftersom alla tre skogsbiotoperna hyser en stor mångfald av fjärilar kan det vara värt att anpassa skötseln av dessa miljöer för att gynna fjärilar. I kraftledningsgator och skogsbilvägar som är relativt beständiga miljöer och som redan idag sköts med ett visst intervall är detta fullt möjligt. Betydelsen av antal inventeringstillfällen beror på vilken frågeställning man har. Vill man jämföra artrikedomen i olika miljöer eller studera förändring i artrikedom mellan år kan det kanske räcka med tre besök, men vill man ha en mer heltäckande bild av artrikedomen på en plats kan det sju eller fler besök spridda över säsongen

  • Challenges in diagnosing normal pressure hydrocephalus : evaluation of the diagnostic guidelines

    • Purpose: To evaluate the present diagnostic guidelines of idiopathic normal pressure hydrocephalus (iNPH) in a sample from the general population.Methods: A total of 168 individuals (93 females, 75 males), mean age 75 years (range 66-92) with and without symptoms of iNPH underwent a CT-scan of the brain, a neurological examination with assessment of the triad symptoms, i.e. gait disturbances, memory impairment and urgency incontinence. The participants were then diagnosed as "unlikely", "possible" and "probable" iNPH according to the American-European and the Japanese guidelines, respectively. Separately, a senior consultant in neurology diagnosed each patient based on the overall clinical picture.Results: Obtaining a diagnosis of "probable iNPH" was three times more likely according to the American-European guidelines (n = 35) compared to the Japanese guidelines (n = 11) or the neurologist (n = 11). The concordance was highest (Kappa = 0.69) between the Japanese guidelines and the neurologist.Conclusions: Considerable discrepancies were found when diagnosing iNPH according to two international guidelines and a neurologist, respectively. The Japanese guidelines, which include a minimum of two triad symptoms, were most concordant with the neurologist. As a step towards widely accepted, standardized diagnostic criteria, we suggest a revision of the current guidelines, preferably into one common diagnostic system.

  • The Role of Enterprise Architecture in Local eGovernment Adoption

    • eFörvaltning ses som ett medel för att göra kommunalt förvaltningsarbete mer effektivt och interoperabelt. Dock begränsas kommuner av oförmåga att uppnå det vilket försvårar genomförandet av deras uppdrag. En orsak till varför införandet av eFörvaltning inte ger de systematiska fördelar som efterfrågas beror på att införandet av informations och kommunikations teknologier (IKT) inte är tillräckligt. Förändringen som krävs kan inte ske över en natt utan är en gradvis förändring över tid som även kräver nya sätt att arbeta på. Kommuner behöver hantera sin eFörvaltningsutveckling på ett mer strukturerat sätt för att öka möjligheterna att förverkliga de fördelarna som sägs kunna erhållas. Enterprise Architecture (EA) har på senare tid kommit att ses som en möjlig lösning för att komma tillrätta med denna problematik. Genom att arbeta mot en EA skapas möjligheten att brygga IKT system med affärsprocesser och därigenom leda till effektivitet och interoperabilitet. Dock är EA inom offentlig sektor ifrågasatt. Forskare menar att EA saknar tydlig definierad omfattning och begreppsvärld, vilket gör användningen av EA inom förvaltningsarbete svår. De omfattande logiska EA ramverk som finns idag saknar innehåll som är specifikt relaterade till offentlig sektor. Vari det blir problematiskt att arbeta mot en EA inom offentlig sektor.Denna licentiatavhandling lägger följande forskningsfråga: ”Vilken roll har EA för införandet av eFörvaltning inom Svenska kommuner?” Samt följande underfrågor ”Vilka förutsättningar finns för att använda EA vid eFörvaltningsutveckling?” ”Vad har Örebro kommun uppnått genom att arbete mot en EA i deras eFörvaltningsutveckling?” Och ”Vilka problem har Örebro kommun upplevt av att arbeta mot en EA vid eFörvaltningsutvecklingen?”. En longitudinell fallstudie av ett eFörvaltningsutvecklingsprojekt i Örebrokommun genomfördes mellan 2007 och 2009 med syfte att utröna dessa frågor och för att skapa insikt om EAs roll för eFörvaltningsutveckling. Förutsättningar, resultat och upplevda problem vid eFörvaltningsutvecklingen studerades. I anslutning till detta empiriska arbete genomfördes även en jämförandestudie av eFörvaltningsmål såsom de anges i officiella statliga dokument inom så väl Sverige som i EU gentemot förmodade fördelarna med EA som diskuteras i samtida EA-litteratur.Studien visar att EA som fenomen ses som en förutsättning för att lyckas med eFörvaltning. 7 viktiga aspekter gällande förutsättningen för att lyckas med kommuners eFörvaltningsutveckling identifierades: Skillnaden mellan administrativa och politiska ansvaret, Politiskt mandat, Politisk timing, Resursfördelning, Samordning under NPM, Leverantörsberoende, Val av standard och bäst praxis. För att förbättra möjligheten att röna framgång i eFörvaltningsutveckling behöver dessa aspekter aktualiseras och hanteras. New Public Management (NPM) som styrmodell utgör ett strukturellt hinder för eFörvaltningsutveckling som förhindrar en mer explicit användning av EA-ramverk, vilket påverkar möjligheten att uppnå uppsatta mål negativt. Trots den strukturella problematik som föreligger, kan stöd erhållas genom att arbeta utifrån ett ”EA-tänk”. Ett ”EAtänk” kan här ses som ett medel för att påbörja förändringen mot en EA utan att för den delen explicit använda sig av ett EA-ramverk, eller -metod. Dock är det viktigt skapa en förståelse för att ”EA-tänk” som fenomen i sin tur ger upphov till strukturer och således kan leda till problem som även de måste hanteras för att eFörvaltningsutveckling skall lyckas. eFörvaltning, NPM och ett ”EA-tänk” skapar en strukturell triad där olika strukturella egenskaper i vissa fall sammanfaller med varandra vari de kan stödja kommuner i dess arbete och således bidra till positiv utveckling. Medan det i andra fall leder till negativ utveckling där administrationen upplever problem att ta till sig av de rekommendationer eFörvaltningsprojektet kan tänkas ge till organisationen som helhet. Paradoxalt nog skapar denna strukturella triad en situation där kommuner måste arbete runt sig själv för att komma framåt. Normer och befintliga strukturer i kommuner förhindrar effektiv samarbete både internt mellan olika förvaltningar och externt gentemot andra kommuner och landsting vilket leder till fragmentariska framsteg mot projektmålen och i slutänden även kommunens övergripande mål gällande eFörvaltningsutveckling. Givet de i många fall motstridiga strukturella egenskaper som finns mellan NPM och eFörvaltning innebär att det initiala arbetet mot EA i eFörvaltningsutveckling är ett ytterst komplext fenomen. För att öka möjligheten att eFörvaltningsutvecklingen blir lyckad krävs det att kommuner känner till och hantera den negativa inverkan NPM har på eFörvaltningsutveckling. Samtidigt behöver de känna till och hanterar problematiken som uppstår av att arbeta mot en EA genom ett ”EA-tänk”, Samt att som studien visat, EA ger inget stöd för mer politiseras mål, vari kommuner behöver annat sätt och andra strategier för sådant arbete. Detta är viktigt att beakta, speciellt med tanke på att mer politiserade mål i mångt och mycket är de mål som kommer premieras av politiker. Av just den enkla orsaken att politiker inte blir omvalda på grund av “effektiva IKT integrerade interna processer” utan blir omvalda utifrån förbättringar som är synliga och som gagnar medborgare positivt.Avhandlingen bidrar till forsknings genom att öka förståelse för och behovet av att hantera både positiva och negativa faktorer som påverkar möjligheten att lyckas med eFörvaltningsutveckling inom kommunalt arbete. Vilket inkluderar de 7 identifierade kritiska faktorer måste aktualiseras och hanteras för att öka möjligheten att lyckas. Den negativa effekt NPM har på eFörvaltningsutveckling och hur ett ”EA-tänk” kan leda till positiv utveckling, även om det som denna studie visat inte kan bidra till mer politiserade mål. Vad gäller denna licentiatavhandlings bidrag till praktiken så bidrar den med att belysa den problematiska situation som föreligger av att institutionaliserade strukturer i många fall verkar som ett hinder mot förändring. Studien möjliggör även till att kommuner kan skapa en förståelse för sin egna eFörvaltningsutveckling och möjligtvis kan verka för att inte uppleva samma problematik som identifierats i Örebros eFörvaltningsutveckling samt visa på hur en kommun kan arbeta för att hantera den problematik som inte kan förhindras alternativ som uppstår trots denna kunskap.

    • This paper discusses critical success factors (CSF) and best practice in relation to IT implementation in the health sector. We have studied a University Hospital’s implementation of a health information system (HIS) and particularly one clinic which implementation process was described as very successful compared to other units at this hospital. The purpose of the paper is to gain further understanding of if and how well CSFs and best practice solutions can explain this successful case. We do this in order to explore if CSFs and best practice offer any shortcuts to successful IT implementation. By understanding the reasons behind this case’s success we can identify if CSFs and best practice potentially can explain the success, or if there are other explanations in this case. Based on our findings we discuss and question the sometimes overestimated belief in CSFs and best practice as shortcuts to success performance. An important contribution from this study is that situational and contextual factors are very critical to understand and acknowledge during HIS implementations.

  • Butterfly distribution and abundance is affected by variation in the Swedish forest-farmland landscape

    • Remaining patches of semi-natural grasslands are hot spots for biodiversity in modern agricultural landscapes. In Sweden semi-natural pastures cover approximately 500,000 ha. However, power-line corridors, road verges and clear-cuts cover larger areas (in total about 2,000,000 ha), and these open, less intensively managed habitats are potentially important for species associated with taller vegetation and flower resources (e.g. pollinating insects). The aim of the present study was to evaluate the relative importance of semi-natural pastures and the other three open habitats for butterflies in 12 forest-farmland mosaic landscapes in south central Sweden. Species composition differed significantly between habitats in multivariate analyses. Power-line corridors and semi-natural pastures harbored several species that were disproportionally abundant in these habitats (13 and 8 species, respectively), and power-line corridors also harbored several species that were classified as typical in indicator species analyses. There were more butterfly species, higher abundances and a tendency for more individuals of red-listed species in power-line corridors than in the other three habitats. Effects of the surrounding landscape composition seemed to be weaker than that of the local habitat. However, species composition was significantly associated with landscape composition and species with intermediate and low mobility were more abundant in forested landscapes than in landscapes dominated by arable fields. Analyses of flying time and host plants for larvae suggest that early flying species and species associated with dwarf shrubs were more common in power-line corridors than in the other habitats. A landscape perspective, which takes several habitats into account, is needed for conservation of butterfly communities in forest-farmland landscapes. Power-line corridors and road verges offer possibilities for creating habitats that are suitable for pollinating insects through conservation-oriented management. (C) 2011 Elsevier Ltd.   • National implications of EU harmonization - The case of banks' reporting requirements in Sweden

    • The purpose of this paper is to discuss how internationally formulated policies and regulation of banks affect national supervisory practices and what consequences this may have on the form and quality of banking supervision conducted on the national level. Using the case of Swedish banks’ reporting requirements, we argue that the last decade of EU level regulatory reforms are motivated by a different “regulatory agreement” (Young 2016) than the Swedish one. In the decade since the global financial crisis 2007-2009, banking regulation reforms appear to be shaped by a view that all financial institutions are more or less immoral and cannot be trusted to self-regulate. The development can be characterized as a move from a dialogue based relationship between the supervisor and the supervised (Roberts, 2009) and being based on fairly high levels of trust (Tomkins, 2001), to a more quantitative and centralized supervision based on collection and comparison of large amounts of standardized quantitative data. We argue that Swedish financial regulation and supervision traditionally were based on the notion of dialogue and mutual trust and less on extensive reporting requirements. Thus, for Sweden to conform to the post-crisis international regulatory framework, this means abandoning what appears to be a fairly well-functioning national arrangement.

  • Soil Resources Influence Spatial Patterns of Denitrifying Communities at Scales Compatible with Land Management

    • Knowing spatial patterns of functional microbial guilds can increase our understanding of the relationships between microbial community ecology and ecosystem functions. Using geostatistical modeling to map spatial patterns, we explored the distribution of the community structure, size, and activity of one functional group in N cycling, the denitrifiers, in relation to 23 soil parameters over a 44-ha farm divided into one organic and one integrated crop production system. The denitrifiers were targeted by the nirS and nirK genes that encode the two mutually exclusive types of nitrite reductases, the cd(1) heme-type and copper reductases, respectively. The spatial pattern of the denitrification activity genes was reflected by the maps of the abundances of nir genes. For the community structure, only the maps of the nirS community were related to the activity. The activity was correlated with nitrate and dissolved organic nitrogen and carbon, whereas the gene pools for denitrification, in terms of size and composition, were influenced by the soil structure. For the nirS community, pH and soil nutrients were also important in shaping the community. The only unique parameter related to the nirK community was the soil Cu content. However, the spatial pattern of the nirK denitrifiers corresponded to the division of the farm into the two cropping systems. The different community patterns, together with the spatial distribution of the nirS/nirK abundance ratio, suggest habitat selection on the nirS-and nirK-type denitrifiers. Our findings constitute a first step in identifying niches for denitrifiers at scales relevant to land management.

    • This thesis deals with students’ views on respect as an element in their relationships with teachers and peers. The purpose of the thesis is to gain further knowledge of relationships in school by applying concepts of the theory of ethical demand in the analysis of students’ descriptions of respect in their relationships with their teachers and peers.The theoretical basis of the study is the theory of the ethical demand (Løgstrup, 1997) which is supplemented with Thomas Ziehe’s (1986/2003, 1993) theoretical concepts of proximity and distance in relationships. An important point of departure for the study is the meaning of relationships in education. According to Løgstrup, interrelationship is seen as a characteristic feature of human existence, and respect and trust as the natural basis in human relationships.The empirical material consists of 21 group interviews with 69 students aged 14 attending two Swedish schools. The interviews were conducted as semi-structured qualitative interviews aiming to understand the students’ perspective on respect in relationships in school.The results show that respect is described as a reciprocal phenomenon in the students’ relationships. The students’ starting-point was in experiences of disrespect, which indicates that it is difficult to describe respect. The students picture respectful relationships to teachers with a wish of being seen for who they are, e.g. by equal treatment, being listened to and existentially confirmed. The students’ disrespectful relationships to teachers deal with descriptions of teachers’ inability to listen, abuse of power, and teachers’ inability to teach with structure and planning. Respect in peer relations is described as allowing a person to be the way she or he is. Honesty is important in peer relations due to the students’ need to see the other person’s true self in order to get to know him or her, which is their starting point for respectful peer relations. The theoretical interpretation of these results gives an understanding of respect as an essentially human need to reciprocally affirming the life of one another.The overall conclusion is that respect is given a deeper understanding as a human phenomenon in relationships, something that goes beyond the students’ volition. Disrespectful relationships force the students to harbour mistrust and insecurity when interacting with peers and teachers. Respectful relationships in school are seen as necessary for the students’ possibilities to enjoy life.

    • Since the nineteen-seventies, electricity has become a common heating source in Swedish single-family houses. About one million smallhouses can use electricity for heating, about 600.000 have electricity as the only heating source.A liberalised European electricity market would most likely raise the Swedish electricity prices during daytime on weekdays and lower it at other times. In the long run, electrical heating of houses would be replaced by fuels, but in the shorter perspective, other strategies may be considered. This report evaluates the use of electricity for heating a dwelling, or part of it, at night when both the demand and the price are low. The stored heat is utilised in the daytime some hours later, when the electricity price is high.Essential for heat storage is the thermal time constant. The report gives a simple theoretical framework for the calculation of the time constant for a single-family house with furniture. Furthermore the “comfort” time constant, that is, the time for a house to cool down from a maximum to a minimum acceptable temperature, is derived. Two theoretical model houses are calculated, and the results are compared to data from empirical studies in three inhabited test houses.The results show that it was possible to store about 8 kWh/K in a house from the seventies and about 5 kWh/K in a house from the eighties. The time constants were 34 h and 53 h, respectively. During winter conditions with 0°C outdoor, the “comfort” time constants with maximum and minimum indoor temperatures of 23 and 20°C were 6 h and 10 h.The results indicate that the maximum load-shifting potential of an average single family house is about 1 kW during 16 daytime hours shifted into 2 kW during 8 night hours. Up-scaled to the one million Swedish single-family houses that can use electricity as a heating source, the maximum potential is 1000 MW daytime time-shifted into 2000 MW at night.

    • Purpose: The purpose of this paper is to examine the challenges that arise when introducing an electronic identification (eID) card for professional use in a health-care setting.Design/methodology/approach: This is a case study of an eID implementation project in healthcare. Data were collected through interviews with key actors in a project team and with eID end users. The authors viewed the eID card as a boundary object intersecting social worlds. For this analysis, the authors combined this with an electronic government initiative challenge framework.Findings: The findings of this paper illustrate the interpretative flexibility of eID cards and how eID cards as boundary objects intersect social worlds. The main challenges of implementing and using eID cards in healthcare are usability, user behaviour and privacy. However, the way in which these challenges are interpreted varies between different social worlds.Practical implications: One of the implications for future practice is to increase our understanding of the eID card as a socio-technical artefact, where the social and technical is intertwined, at the same time as the eID card affects the social as well as the technical. By using a socio-technical perspective, it is possible to minimise the potential problems related to the implementation and use of eID.Originality/value: Previous research has highlighted the need for more empirical research on identity management. The authors contextualise and analyse the implementation and use of eID cards within healthcare. By viewing the eID card as a boundary object, the authors have unveiled its interpretative flexibility and how it is translated across different social worlds.

    • Purpose: The purpose of the paper is to reveal how identities are constructed when electronic identification (eIDs) cards are introduced through information systems in public organisations. Design/methodology/approach: Through two case studies, the authors generate rich data on the construction of identities through use of eID within public organisations. The author’s analysis, based on actor network theory, focusses on the translation of eIDs in these two settings. Findings: ID can be viewed as an artefact where the public and private spheres meet. The authors found at least three mixed roles in employees’ use of eID: as a purely private person; as a private person in the work place; and as a professional in the work place. Research limitations/implications– There is a need for further research on how eID is translated into organisational contexts and how institutional settings define the openings for local translation processes. However, the results are based on two small cases, meaning that broad generalisations are difficult to make. Practical implications: EID is so much more than technology. The technical framing of the identification system appears to be subordinated to organisational arrangements and cultures, making it important to apply a socio-technical perspective when working with eID. Originality/value: The empirical cases have offered a unique chance to study implementation and use of eID in two very different public service organisations. The findings illustrate how eID translated into organisational contexts, and how identity management within an organisational setting is linked to the employees’ private and professional roles.

    • BACKGROUND: In recent years, there has been a considerable research effort concerning the integration of magnetic resonance imaging (MRI) into the external radiotherapy workflow motivated by the superior soft tissue contrast as compared to computed tomography. Image registration is a necessary step in many applications, e.g. in patient positioning and therapy response assessment with repeated imaging. In this study, we investigate the dependence between the registration accuracy and the size of the registration volume for a subvolume based rigid registration protocol for MR images of the prostate.METHODS: Ten patients were imaged four times each over the course of radiotherapy treatment using a T2 weighted sequence. The images were registered to each other using a mean square distance metric and a step gradient optimizer for registration volumes of different sizes. The precision of the registrations was evaluated using the center of mass distance between the manually defined prostates in the registered images. The optimal size of the registration volume was determined by minimizing the standard deviation of these distances.RESULTS: We found that prostate position was most uncertain in the anterior-posterior (AP) direction using traditional full volume registration. The improvement in standard deviation of the mean center of mass distance between the prostate volumes using a registration volume optimized to the prostate was 3.9 mm (p < 0.001) in the AP direction. The optimum registration volume size was 0 mm margin added to the prostate gland as outlined in the first image series.CONCLUSIONS: Repeated MR imaging of the prostate for therapy set-up or therapy assessment will both require high precision tissue registration. With a subvolume based registration the prostate registration uncertainty can be reduced down to the order of 1 mm (1 SD) compared to several millimeters for registration based on the whole pelvis.

    • BACKGROUND: The use of magnetic resonance imaging (MRI) as a complement to computed tomography (CT) in the target definition procedure for radiotherapy is increasing. To eliminate systematic uncertainties due to image registration, a workflow based entirely on MRI may be preferable. In the present pilot study, we investigate dose calculation accuracy for automatically generated substitute CT (s-CT) images of the head based on MRI. We also produce digitally reconstructed radiographs (DRRs) from s-CT data to evaluate the feasibility of patient positioning based on MR images. METHODS AND MATERIALS: Five patients were included in the study. The dose calculation was performed on CT, s-CT, s-CT data without inhomogeneity correction and bulk density assigned MRI images. Evaluation of the results was performed using point dose and dose volume histogram (DVH) comparisons, and gamma index evaluation. RESULTS: The results demonstrate that the s-CT images improves the dose calculation accuracy compared to the method of non-inhomogeneity corrected dose calculations (mean improvement 2.0 percentage points) and that it performs almost identically to the method of bulk density assignment. The s-CT based DRRs appear to be adequate for patient positioning of intra-cranial targets, although further investigation is needed on this subject. CONCLUSIONS: The s-CT method is very fast and yields data that can be used for treatment planning without sacrificing accuracy.

At the same time there is limited research on the usefulness of existing user participation approaches in public e-service development. In this paper we, therefore, analyze how the three user participation approaches participatory design, user-centered design, and user innovation - meet the strategic e-service goals of the EU and the US. In doing so, we identify three challenges that need to be considered when choosing among these approaches: 1) unclear user target segments can impede the fulfillment of usability and relevance goals, 2) the nature of participation can impede the fulfillment of democracy goals. and 3) lack of adequate skills can impede the fulfillment of efficiency goals.     • Background and purpose: The idiopathic normal pressure hydrocephalus (iNPH) Radscale was developed to standardize the evaluation of radiological signs in iNPH. The purpose of this study was to estimate the diagnostic accuracy of the iNPH Radscale in a sample of "true positive" and "true negative" cases.Methods: Seventy-five patients with definite iNPH, i.e. who had improved at clinical follow-up one year after ventriculoperitoneal shunt surgery, were compared with 55 asymptomatic individuals from the general population. A radiologist assessed the seven radiological features of the iNPH Radscale in computed tomography of the brain in the patients (preoperatively) and controls.Results: The iNPH Radscale score was significantly higher in the iNPH group (Median = 10, interquartile range 9–11) than in the control group (Median = 1, interquartile range 1–2) (p <0.001). Receiver operated characteristics analysis yielded an area under the curve of 99.7%, and an iNPH Radscale score ≤ 4 identified those without iNPH, with a sensitivity of 100%, specificity of 96% and overall accuracy of 98.5%.Conclusions: In this study, iNPH Radscale could accurately discriminate between patients with definite iNPH and asymptomatic individuals over 65 years old. According to the results, a diagnosis of iNPH is very likely in patients with an iNPH Radscale score above 8 and corresponding clinical symptoms. On the other hand, the diagnosis should be questioned when the iNPH Radscale score is below the cut-off level of 4. We conclude that the iNPH Radscale could work as a diagnostic screening tool to detect iNPH. Whether the scale also can be used to predict shunt outcome needs further studies.

    • Purpose: Assess the agreement for two investigators between computed tomography (CT) and magnetic resonance imaging (MRI) for seven imaging features included in the iNPH Radscale, a radiological screening tool.Methods: The study included 35 patients with idiopathic normal pressure hydrocephalus (iNPH) who were treated surgically from 2011 to 2015 at Uppsala University Hospital with preoperative CT and MRI performed with maximum 3 months between scans. Seven features were assessed: Evans’ index, temporal horn size, callosal angle, periventricular white matter changes, narrow high convexity sulci, focally enlarged sulci, and enlarged Sylvian fissures. All scans were assessed by two investigators who were blinded to each other’s results and to clinical data.Results: The agreement between CT and MRI was almost perfect for Evans’ index, temporal horns, narrow sulci, and Sylvian fissures (kappa and intraclass correlation, 0.84–0.91, p ≤ 0.001). There was substantial to almost perfect agreement for callosal angle and focally enlarged sulci. The concordance between modalities was fair for changes in periventricular white matter.Conclusion: CT and MRI are equally good for assessing radiological signs associated with iNPH except for periventricular white matter changes, as MRI has superior soft tissue contrast. The other imaging features can be evaluated consistently, and assessments are reproducible independent of modality. Therefore, the iNPH Radscale is applicable to both CT and MRI and may become an important tool for standardized evaluation in the workup in patients with suspected iNPH.

    • BACKGROUND AND PURPOSE: Despite the important role of imaging in diagnosing idiopathic normal pressure hydrocephalus (iNPH), a structured overall assessment of radiological signs is still lacking. The purpose of this study was to construct a radiological scale, composed of morphological signs of iNPH, and compare it with clinical symptoms.METHODS: In this prospective, population based study of iNPH, 168 individuals (93 females, 75 males), mean age 75 years (range 66-92 years), underwent a computed tomography (CT) of the brain and a neurological examination with assessment of clinical symptoms according to Hellström's iNPH scale. Two radiologists, blinded to clinical data, independently evaluated and measured eight radiological parameters i.e. Evans' index, callosal angle, size of temporal horns, narrow high convexity sulci, dilated Sylvian fissures, focally dilated sulci, periventricular hypodensities and bulging of the lateral ventricular roof.RESULTS: In a linear regression model, all parameters except ventricular roof bulging were significantly associated with clinical iNPH symptoms. The seven remaining parameters were summarized into a total iNPH Radscale score ranging from 0 to 12. There was a significant correlation (r = 0.55, P < 0.001) between the total score of the iNPH Radscale and clinical symptoms. The interrater agreement for the included radiological parameters was high (ICC = 0.74-0.97).CONCLUSION: The iNPH Radscale may become a valuable diagnostic screening tool, allowing a structured radiological assessment. A high iNPH Radscale score together with clinical symptoms, should raise suspicion of iNPH motivating further evaluation for shunt-surgery.

    • Background: Physical inactivity is globally recognized as a major risk factor for morbidity, particularly the incidence of non-communicable diseases, and mortality. Engaging in active transportation (AT) is a viable approach toward increasing physical activity (PA) on a daily basis. Mobile (mHealth) interventions enable promoting AT to a larger population. The Smart City Active Mobile Phone Intervention (SCAMPI) study is a randomized controlled trial designed to evaluate a smartphone application (app)’s ability to motivate participants to increase their PA by engaging in AT.Objective: This qualitative study examines the acceptance and usability of the SCAMPI app from the participants’ perspectives.Methods: Seventeen residents of Stockholm county (13 women; age range 25-61 years), who had completed the three-month, app-based behavioral change program in the SCAMPI randomized controlled trial during 2018 agreed to participate in a semi-structured telephone-based interview. These 17 participants were well representative of the whole intervention group (n = 127) in terms of baseline characteristics such as age, sex, and area of residence. The interviews were audio-recorded, transcribed verbatim, and analyzed using inductive qualitative content analysis.Results: The content analysis revealed two themes and four subcategories. The first theme “Main motivators: monitoring and messages” highlighted that monitoring AT and being able to set weekly goals using the app were the primary motivators reported among study participants. The second theme “Acceptable but modifiable” reflects that the app was well accepted and effectively encouraged many participants to use more AT. Nevertheless, there were functions in the app that require modification. For example, while the automated travel tracking feature was appreciated, participants found it time-consuming and unreliable at times.Conclusions: This study contributes novel insight into healthy adults’ experiences of using a mobile app to promote the use of AT. The results showed that the app was well-accepted and that self-monitoring and goal setting were the main motivators to engage in more AT. The automated tracking of AT was appreciated; however, it was also reported to be energy- and time-consuming when it failed to work. Thus, this feature should be improved going forward.

    • The pulp and paper industry is an energy-intensive industrial sector that faces several challenges such as increased competition and rising feedstock and energy prices. To adress this, it is crucial for the industry to improve the material and energy efficiencies to the greatest possible extent. Process integration methods like pinch analysis and mathematical programming are useful tools for evaluating possible process alternatives, i.e. applications of new technologies, changes to new equipment and/or different operating conditions. Development of industrial energy combines is an interesting approach towards an overall optimization of energy and material flows. One problem is often that there are a large number of essentially different actors and financers that are interested in studying other parameters than those that are normally investigated in process integration studies, for example national or regional economics and various social values.In this project, engineering, economic and statistical tools and methods have been applied separately as well as in combination for different types of investigations conducted at the paper and pulp mill Billerud Karlsborg AB in Kalix, Sweden. One main objective has been to develop a process integration model of the mill based on the reMIND method to be used for introductory process simulations of the existing mill configuration. Additionally, pinch analysis has been used to identify alternatives for energy savings in the mill. Another objective has been to develop a regional economic market model (ReCOM) that should be suitable for analysis and predictions of price changes on relevant feedstock markets. A more simplified model based on the reMIND method has been used for intitial studies on how the mill can be turned into a biorefinery. The main purpose of that work has been to investigate if biomass gasification can be economically interesting for the mill and if so, under what boundary conditions. A statistical technique, conjoint analysis, has been used to study and analyze the attitude of employed people at the mill to changes in the production process that may affect for example the local and global environment etc. Finally, possible interactions between the different models and tools have been investigated.The reMIND modelling of the existing mill configuration has showed several alternatives to save steam and fuel. For example, if the wood-chips supplied to the digester is pre-heated from a temperature of 0°C to say 60°C by the use of low grade residual heat, approximately 1.5 ton per hour of 10 bar steam or 5 ton per hour of biomass fuels can theoretically be saved. Furthermore, if the inlet liquor temperature to effect 4 of the evaporation plant increases from 85 to 105°C, the steam used for evaporation decreases from 77 to 66 ton per hour and as a consequence, the biomass fuel supply to the bark boiler decreases from 51 to 39 ton per hour. This, however, also leads to a slightly reduced electricity production, from 35 to 34 MW due to a reduced production of the high pressure steam.The results from the developed ReCOM model, suggest that only none to small changes in the fibrous input prices from an increase in the fuel price (affecting the forestry sector) and a small price increase as a result from a reduced supply of purchasable wood-chips and pulp wood. The small effect that increasing fuel prices has on the fibrous input prices can largely be explained by the relatively small cost share that fuels have in the forestry sector. An increase of the labour costs would most likely have a larger impact. As for the price effect from a reduction in the supply of purchasable wood-chips, there is a substitution possibility between purchased and internally produced wood-chips for the pulp mill. However, when the limit for how much internally produced wood-chips is reached its will probably results in larger price effectsThe Pinch study of the mill indicated that there is a theoretical steam-saving potential of 18.5 MW, corresponding to 12% of the current steam demand. Two different retrofit proposals were suggested for how to achieve specific steam saving levels in practice. According to a basic retrofit proposal, a steam saving of 5.8 MW could be achieved at an investment cost of 7 MSEK while a more rigorous retrofit would enable steam savings of 11 MW at an investment cost of 14.5 MSEK. An approach for using these results in a reMIND model of the mill has also been proposed.The results from the more simplified reMIND modelling have showed that if the mill starts to produce DME via biomass gasification, the necessary policy support to make it economically feasible ranges from 92-561 SEK per MWh biofuel (DME) over four different future scenarios. This could be compared to the Swedish exemption from energy tax on biofuels, which currently amounts to approximately 275 SEK per MWh. It is also concluded that biomass gasification results in a larger net CO2 reduction when integrated with the pulp and paper mill, than when the mill and the gasification plant operate separately.The conjoint analysis showed that it is possible to find groups of respondents that were unknown prior to the study. If an organisation wants to implement a change in the process, conjoint analysis can be used to identify groups of participants with similar preferences and then tailor information to suit these specific groups.Many possibilities for the different models to interact have been identified and illustrated. The interaction between the reMIND method and ReCOM is based on exchanging information on fibrous input prices and quantities and conducted through an iterative process. The results indicate that the models can interact to produce more robust and reliable conclusions regarding optimal resource utilization suggesting that the described approach is feasible and that further research efforts can be made to extend the models. Pinch analysis and reMIND modelling has in other studies shown to be able to interact iteratively. In this study, the retrofit proposals obtained from the pinch analysis could serve as inputs to future reMIND modelling. Another interaction between reMIND and pinch analysis that has been identified during the project is to use pinch analysis to evaluate the opportunity to pre-heat certain process streams. The results from a conjoint analysis are quantitative in form of regression coefficients. However, to use these numbers for example in a Pareto front analysis will be difficult as the numbers has no monetary, energy or emission unit. Nonetheless, conjoint analysis can interact in many different ways with ReCOM as well as the reMIND models. For example, to choose scenarios to be modelled in ReCOM where the factors in the conjoint analysis can be tailored to indicate how the market would respond in a hypothetical situation. Conjoint analysis can be used to weight different factors in the reMIND model. The weighting can possibly also be used in the ReCOM model.This work has illustrated how the various engineering, economic and statistical methods and tools can be used both separately and in combination to help an industry towards more energy-efficient production processes.

    • Objective The incidence of benign ureteroenteric strictures following radical cystectomy (RC) for urinary bladder cancer (UBC) is investigated mainly in single-centre studies from high-volume centres. The aim of this study was to evaluate the cumulative incidence of strictures and risk factors in a population-based cohort. Patients and methods Data was collected from Bladder Cancer Data Base Sweden (BladderBaSe). The primary endpoint was stricture with intervention. Secondary endpoint included hydronephrosis both with/without intervention. Results In total, 5,816 patients were registered as having had RC due to UBC between 1997 and 2014. After a median follow-up of 23.5 months (IQR = 9.0-63.1 months; range = 0.0-214.0 months), we found that 515 (8.9%) patients underwent intervention for stricture. Seven hundred and sixty-one (13.1%) patients were diagnosed with hydronephrosis without intervention. The cumulative incidence of strictures with intervention was 19.7% (95% CI = 16.7-23.1%) during the 17 years of follow-up. In the first year, the cumulative incidence of strictures was 5.6% (95% CI = 5.0-6.2%), and in the first 2 years 8.4% (95% CI = 7.6-9.3%). For the secondary endpoint, the cumulative incidence was 30.4% (95% CI = 26.7-33.1%) after 17 years. Only the year of RC was associated with stricture incidence in Cox regression analysis, whereas hospital cystectomy volume, patient age and patient sex were not. Conclusion Ureteroenteric strictures requiring intervention may be more common than previously reported, affecting nearly one fifth of patients who have undergone RC for UBC. The annual incidence was highest in the first 2 years after surgery but the cumulative incidence increased continuously during 17 years of follow-up.

    • Purpose– The purpose of this paper is to analyse and understand the contemporary management of electronic identification (e-ID) development to: identify and formulate challenges and reflect upon the use of a combination of perspectives. To generate knowledge on this issue, we investigate e-ID development in Sweden from: an e-government systems development lifecycle perspective and a project challenge and critical success factor (CSF) perspective. Design/methodology/approach– This is a qualitative case study covering an analysis of the three years in a larger project focusing e-ID in a public e-service setting. Empirical sources have been face-to-face interviews; official documents and different kind of forums for presentations and discussions in, for example, hearings arranged by authorities; meetings with the coordinating agency, and practitioners’ networks events. Findings– This study concludes that there are significant challenges involved in managing e-ID development because of its contextual and integrated character. Challenges involve the organization and management of the program and can be traced back to e-government, general project management literature and theory on path dependency. Based on this study, we can question, e.g. governance models, centralization and a narrow focus on the technical artefact. Our study is also an illustration of a possible way to analyse e-ID within an e-government initiative. Research limitations/implications– The present study shows that an e-ID can be considered as a back office-enabler for launching e-services, but also highlights the need for management of the artefact as an integral part of e-service development because it is intertwined with the use of e-services from a user perspective. This aspect together with the insights related to challenges and success factors including path dependency provides implications for future practice of e-ID management and development in particular and information systems artefact development in general. Originality/value– This paper addresses challenges related to the development of e-ID in a public e-service setting. Few studies have theoretically combined a lifecycle perspective on challenges and success factors related to e-ID development while also focusing different dimensions of path dependency as an example of a challenging area within a program frame. Studying e-ID as a contemporary phenomenon from a contextual perspective in line with sociomaterial thinking – with a focus on the interplay between technology and people –can also help us to understand and discuss artefact development in general.

    • This paper investigates the management of developing electronic identification (e-ID) within a public e-service context. e-ID is an important key enabler for secure identification, authentication and digital signing via the Internet and a part of e-service design. As users, and citizens, we become reliant on electronic solutions that give us a certain level of utility and trust, and use e-ID solutions to interact with local and central government in an e-service context. The management of e-ID development in a national context is the case in focus for investigation. Such development initiatives, and especially inter-organizational projects, face a number of challenges. Therefore it is a need for a more thorough understanding of e-ID development within a public e-service context. The purpose is to analyse the contemporary management of e-ID development in Sweden from: a) an e-government systems development life-cycle perspective and b) a project challenge and critical success factor perspective. This study concludes that there are significant challenges involved in managing integrated e-ID development. Challenges involve the organization and management of the program and can be traced back to e-government and general project management literature, but based on this study one can question, e.g. governance models, centralization, and a narrow focus on a technical artefact. Important implications from this paper are a call for further contextual studies of e-ID development, putting the user and organizational setting, path dependency, and governance models in focus.

    • It may be challenging to differentiate normal pressure hydrocephalus (NPH) from neurodegenerative disorders such as Parkinson's disease. In this population-based study, we wanted to describe the frequency of parkinsonian symptoms among individuals with and without NPH, and whether the motor examination part of the Unified Parkinson's Disease Rating Scale (UPDRS-m) score differs between these groups. Furthermore, we wanted to find out whether there was a relationship between UPDRS-m score, NPH symptoms, and radiological signs of NPH. A sample of 168 individuals over the age of 65 with and without self-reported symptoms of NPH underwent a computerized tomography of the brain and clinical examination, including UPDRS-m to grade parkinsonian symptoms. According to diagnostic guidelines, 38 fulfilled criteria for NPH, whereas 130 had unlikely NPH. Bradykinesia was significantly more common among those with NPH (79%) compared to those with unlikely NPH (32%) (p < 0.001). The corresponding figures for rigidity were 43 vs. 15% (p < 0.001), for postural instability 71 vs. 22% (p < 0.001), and for tremor at rest 5 vs. 6% (not significant). The total UPDRS-m score was significantly higher among individuals with NPH (median = 12) than without (median = 1) and correlated significantly with the degree of NPH symptoms (r = -0.72) and ventriculomegaly (r = 0.31). In this study, parkinsonian symptoms, except resting tremor, were frequent in individuals with NPH and correlated with the severity of NPH symptoms. Asymmetric symptoms were uncommon. We recommend a liberal use of neuroradiological imaging when investigating a patient with parkinsonian features.

    • Purpose: To develop an infrastructure for structured and automated collection of interoperable radiation therapy (RT) data into a national clinical quality registry. Materials and methods: The present study was initiated in 2012 with the participation of seven of the 15 hospital departments delivering RT in Sweden. A national RT nomenclature and a database for structured unified storage of RT data at each site (Medical Information Quality Archive, MIQA) have been developed. Aggregated data from the MIQA databases are sent to a national RT registry located on the same IT platform (INCA) as the national clinical cancer registries. Results: The suggested naming convention has to date been integrated into the clinical workflow at 12 of 15 sites, and MIQA is installed at six of these. Involvement of the remaining 3/15 RT departments is ongoing, and they are expected to be part of the infrastructure by 2016. RT data collection from ARIA (R), Mosaiq (R), Eclipse (TM), and Oncentra (R) is supported. Manual curation of RT-structure information is needed for approximately 10% of target volumes, but rarely for normal tissue structures, demonstrating a good compliance to the RT nomenclature. Aggregated dose/volume descriptors are calculated based on the information in MIQA and sent to INCA using a dedicated service (MIQA2INCA). Correct linkage of data for each patient to the clinical cancer registries on the INCA platform is assured by the unique Swedish personal identity number. Conclusions: An infrastructure for structured and automated prospective collection of syntactically inter operable RT data into a national clinical quality registry for RT data is under implementation. Future developments include adapting MIQA to other treatment modalities (e.g. proton therapy and brachytherapy) and finding strategies to harmonize structure delineations. How the RT registry should comply with domain-specific ontologies such as the Radiation Oncology Ontology (ROO) is under discussion.

    • PurposeWe describe a public dataset with MR and CT images of patients performed in the same position with both multiobserver and expert consensus delineations of relevant organs in the male pelvic region. The purpose was to provide means for training and validation of segmentation algorithms and methods to convert MR to CT like data, i.e., so called synthetic CT (sCT). Acquisition and validation methodsT1- and T2-weighted MR images as well as CT data were collected for 19 patients at three different departments. Five experts delineated nine organs for each patient based on the T2-weighted MR images. An automatic method was used to fuse the delineations. Starting from each fused delineation, a consensus delineation was agreed upon by the five experts for each organ and patient. Segmentation overlap between user delineations with respect to the consensus delineations was measured to describe the spread of the collected data. Finally, an open-source software was used to create deformation vector fields describing the relation between MR and CT images to further increase the usability of the dataset. Data format and usage notesThe dataset has been made publically available to be used for academic purposes, and can be accessed from . Potential applicationsThe dataset provides a useful source for training and validation of segmentation algorithms as well as methods to convert MR to CT-like data (sCT). To give some examples: The T2-weighted MR images with their consensus delineations can directly be used as a template in an existing atlas-based segmentation engine; the expert delineations are useful to validate the performance of a segmentation algorithm as they provide a way to measure variability among users which can be compared with the result of an automatic segmentation; and the pairwise deformably registered MR and CT images can be a source for an atlas-based sCT algorithm or for validation of sCT algorithm.     • Background: The use of magnetic resonance (MR) imaging as a part of preparation for radiotherapy is increasing. For delineation of the prostate several publications have shown decreased delineation variability using MR compared to computed tomography (CT). The purpose of the present work was to investigate the intra- and inter-physician delineation variability for prostate and seminal vesicles, and to investigate the influence of different MR sequence settings used clinically at the five centers participating in the study.Methods: MR series from five centers, each providing five patients, were used. Two physicians from each center delineated the prostate and the seminal vesicles on each of the 25 image sets. The variability between the delineations was analyzed with respect to overall, intra-and inter-physician variability, and dependence between variability and origin of the MR images, i.e. the MR sequence used to acquire the data.Results: The intra-physician variability in different directions was between 1.3 - 1.9 mm and 3 - 4 mm for the prostate and seminal vesicles respectively (1 std). The inter-physician variability for different directions were between 0.7 - 1.7 mm and approximately equal for the prostate and seminal vesicles. Large differences in variability were observed for individual patients, and also for individual imaging sequences used at the different centers. There was however no indication of decreased variability with higher field strength.Conclusion: The overall delineation variability is larger for the seminal vesicles compared to the prostate, due to a larger intra-physician variability. The imaging sequence appears to have a large influence on the variability, even for different variants of the T2-weighted spin-echo based sequences, which were used by all centers in the study.

    • Background and purpose: Health related quality of life (HRQoL) was assessed in the randomised, prospective ARTSCAN study comparing conventional radiotherapy (CF) with accelerated radiotherapy (AF) for head and neck cancer. Material and methods: 750 patients with squamous cell carcinoma (of any grade and stage) in the oral cavity, oro-, or hypopharynx or larynx (except T1-2, NO glottic carcinoma) without distant metastases were randomised to either conventional fractionation (2 Gy/day, 5 days/week in 49 days, total dose 68 Gy) or accelerated fractionation (1.1 + 2.0 Gy/day, 5 days/week in 35 days, total dose 68 Gy). HRQoL was assessed with EORTC QLQ-C30, QLQ-H&N35 and HADS at baseline, at end of radiotherapy (eRT) and at 3 and 6 months and 1, 2 and 5 years after start of treatment. Results: The AF group reported HRQoL was significantly lower at eRT and at 3 months for most symptoms, scales and functions. Few significant differences were noted between the groups at 6 months and 5 years. Scores related to functional oral intake never reached baseline. Conclusion: In comparison to CF, AF has a stronger adverse effect on HRQoL in the acute phase.

    • Background: Weight loss is a common problem in patients with Squamous Cell Carcinoma of the Head and Neck (SCCHN) treated with radiotherapy (RT). The aims of the present study were to determine if treated volume (TV), as a measure of the radiation dose burden, can predict weight loss in patients with oropharyngeal cancer and to analyze weight loss and body mass index (BMI) in the same patient group in relation to 5-year overall survival. Methods: The ARTSCAN trial is a prospective, randomized, multicenter trial in patients with SCCHN. Nutritional data from the ARTSCAN trial were analyzed retrospectively using univariate and multivariate statistical methods based on information on percentage weight loss from the start of RT up to five months after the termination of RT (study cohort 1, n = 232) and information on patients' BMI at the start of RT (study cohort 2, n = 203). TV was defined as the volume of the patient receiving at least 95% of the prescribed dose. TV64.6 (Gy) encompasses macroscopic tumor and TV43.7 (Gy) elective lymph nodes of the neck. Results: TV64.6 Gy and TV43.7 Gy were both significantly correlated with higher weight loss up to five months after the termination of RT in study cohort 1 (p < 0.001 for both). BMI at the start of RT was shown to be a prognostic factor for 5-year overall survival in study cohort 2 but weight loss was not. The hazard ratios and 95% confidence intervals were 3.78 (1.46-9.75) and 2.57 (1.43-4.62) in patients with underweight and normal weight, respectively. Conclusions: TV can predict weight loss during RT in patients with oropharyngeal cancer regardless of clinical stage. A high BMI (>25 kg/m(2)) at the start of RT is positively associated with survival in patients with oropharyngeal cancer.

    • Leaving small patches of forest intact at harvesting is now a standard procedure to mitigate negative effects on biodiversity. One purpose of the patches is to "life-boat" species over the forest regeneration phase, although the capacity of small forest fragments to do so is very uncertain. We investigated the survival of red-listed and indicator species of bryophytes and lichens in 74 retention patches in boreal Sweden. The patches were between 0.01 and 0.5 ha in size and of six different types with respect to tree species composition and location on the harvested area. Species presence and abundance were recorded shortly after harvest in transects covering the whole patches, and an identical inventory was carried out 6 years after the first. During this time, bryophytes generally decreased, most pronounced for liverworts. The largest decreases were found in buffer zones to streams and lakes and the smallest in tree groups dominated by deciduous trees. By contrast, among the lichens some species decreased while others increased, and there was no difference between retention patch types. Among the species abundant enough to be analyzed individually, the lichens Calicium parvum and Micarea globuloselia decreased less in larger patches and the bryophyte Hylocomiastrum umbratum decreased more in patches of irregular shape. The results imply that retention patches of this size might be too small to function as refugia for sensitive bryophytes and lichens until the surrounding forest regenerates, but that some lichens appear to persist or even increase. Retention harvesting is still a young management practice and further studies on its long-term conservation benefits will be valuable. (C) 2009 Elsevier Ltd. All rights reserved.

    • Background:Excessive gestational weight gain (GWG) is common and associated with negative health outcomes for both mother and child. Mobile health–delivered lifestyle interventions offer the potential to mitigate excessive GWG. The effectiveness of a smartphone app (HealthyMoms) was recently evaluated in a randomized controlled trial. To explore the users’ experiences of using the app, a qualitative study within the HealthyMoms trial was performed.Objective:This qualitative study explored participants’ engagement and satisfaction with the 6-month usage of the HealthyMoms app.Methods:A total of 19 women (mean age: 31.7, SD 4.4 years; mean BMI: 24.6, SD 3.4 kg/m2; university degree attainment: 13/19, 68%; primiparous: 11/19, 58%) who received the HealthyMoms app in a randomized controlled trial completed semistructured exit interviews. The interviews were audiorecorded and fully transcribed, coded, and analyzed using thematic analysis with an inductive approach.Results:Thematic analysis revealed a main theme and 2 subthemes. The main theme, “One could suit many: a multifunctional tool to strengthen women’s health during pregnancy,” and the 2 subthemes, “Factors within and beyond the app influence app engagement” and “Trust, knowledge, and awareness: aspects that can motivate healthy habits,” illustrated that a trustworthy and appreciated health and pregnancy app that is easy to use can inspire a healthy lifestyle during pregnancy. The first subtheme discussed how factors within the app (eg, regular updates and feedback) were perceived to motivate both healthy habits and app engagement. Additionally, factors beyond the app were described to both motivate (eg, interest, motivation, and curiosity) and limit (eg, pregnancy-related complications, lack of time) app engagement. The second subtheme reflected important aspects, such as high trustworthiness of the app, increased knowledge, and awareness from using the app, which motivated participants to improve or maintain healthy habits during pregnancy.Conclusions:The HealthyMoms app was considered a valuable and trustworthy tool to mitigate excessive GWG, with useful features and relevant information to initiate and maintain healthy habits during pregnancy.
