    • Background: There is tremendous potential for genome sequencing to improve clinical diagnosis and care once it becomes routinely accessible, but this will require formalizing research methods into clinical best practices in the areas of sequence data generation, analysis, interpretation and reporting. The CLARITY Challenge was designed to spur convergence in methods for diagnosing genetic disease starting from clinical case history and genome sequencing data. DNA samples were obtained from three families with heritable genetic disorders and genomic sequence data were donated by sequencing platform vendors. The challenge was to analyze and interpret these data with the goals of identifying disease-causing variants and reporting the findings in a clinically useful format. Participating contestant groups were solicited broadly, and an independent panel of judges evaluated their performance. Results: A total of 30 international groups were engaged. The entries reveal a general convergence of practices on most elements of the analysis and interpretation process. However, even given this commonality of approach, only two groups identified the consensus candidate variants in all disease cases, demonstrating a need for consistent fine-tuning of the generally accepted methods. There was greater diversity of the final clinical report content and in the patient consenting process, demonstrating that these areas require additional exploration and standardization. Conclusions: The CLARITY Challenge provides a comprehensive assessment of current practices for using genome sequencing to diagnose and report genetic diseases. There is remarkable convergence in bioinformatic techniques, but medical interpretation and reporting are areas that require further development by many groups.

  • Highly efficient composite electrolyte for natural gas fed fuel cell

    • Solid oxide fuel cells (SOFCs) have the ability to operate with different variants of hydro carbon fuel such as biogas, natural gas, methane, ethane, syngas, methanol, ethanol, hydrogen and any other hydrogen rich gas. Utilization of these fuels in SOFC, especially the natural gas, would significantly reduce operating cost and would enhance the viability for commercialization of FC technology. In this paper, the performance of two indigenously manufactured nanocomposite electrolytes; barium and samarium doped ceria (BSDC-carbonate); and lanthanum and samarium doped ceria (co-precipitation method LSDC-carbonate) using natural gas as fuel is discussed. The nanocomposite electrolytes were synthesized using co-precipitation and wet chemical methods (here after referred to as nano electrolytes). The structure and morphology of the nano electrolytes were examined by X-ray diffraction (XRD) and scanning electron microscopy (SEM). The fuel cell performance (OCV) was tested at temperature (300-600 °C). The ionic conductivity of the nano electrolytes were measured by two probe DC method. The detailed composition analysis of nano electrolytes was performed with the help of Raman Spectroscopy. Electrochemical study has shown an ionic conductivity of 0.16 Scm-1 at 600 °C for BSDC-carbonate in hydrogen atmosphere, which is higher than conventional electrolytes SDC and GDC under same conditions. In this article reasonably good ionic conductivity of BSDC-carbonate, at 600 °C, has also been achieved in air atmosphere which is comparatively greater than the conventional SDC and GDC electrolytes.

  • Mortality outcomes with hydroxychloroquine and chloroquine in COVID-19 from an international collaborative meta-analysis of randomized trials

    • Substantial COVID-19 research investment has been allocated to randomized clinical trials (RCTs) on hydroxychloroquine/chloroquine, which currently face recruitment challenges or early discontinuation. We aim to estimate the effects of hydroxychloroquine and chloroquine on survival in COVID-19 from all currently available RCT evidence, published and unpublished. We present a rapid meta-analysis of ongoing, completed, or discontinued RCTs on hydroxychloroquine or chloroquine treatment for any COVID-19 patients (protocol: https://osf.io/QESV4/). We systematically identified unpublished RCTs (ClinicalTrials.gov, WHO International Clinical Trials Registry Platform, Cochrane COVID-registry up to June 11, 2020), and published RCTs (PubMed, medRxiv and bioRxiv up to October 16, 2020). All-cause mortality has been extracted (publications/preprints) or requested from investigators and combined in random-effects meta-analyses, calculating odds ratios (ORs) with 95% confidence intervals (CIs), separately for hydroxychloroquine and chloroquine. Prespecified subgroup analyses include patient setting, diagnostic confirmation, control type, and publication status. Sixty-three trials were potentially eligible. We included 14 unpublished trials (1308 patients) and 14 publications/preprints (9011 patients). Results for hydroxychloroquine are dominated by RECOVERY and WHO SOLIDARITY, two highly pragmatic trials, which employed relatively high doses and included 4716 and 1853 patients, respectively (67% of the total sample size). The combined OR on all-cause mortality for hydroxychloroquine is 1.11 (95% CI: 1.02, 1.20; I-2=0%; 26 trials; 10,012 patients) and for chloroquine 1.77 (95%CI: 0.15, 21.13, I-2=0%; 4 trials; 307 patients). We identified no subgroup effects. We found that treatment with hydroxychloroquine is associated with increased mortality in COVID-19 patients, and there is no benefit of chloroquine. Findings have unclear generalizability to outpatients, children, pregnant women, and people with comorbidities. Hydroxychloroquine and chloroquine have been investigated as a potential treatment for Covid-19 in several clinical trials. Here the authors report a meta-analysis of published and unpublished trials, and show that treatment with hydroxychloroquine for patients with Covid-19 was associated with increased mortality, and there was no benefit from chloroquine.

    • Here we present an electrospinning technique for the fabrication of cadmium titanate/polyvinyl-pyrrolidone composite nanofibers. The composite nanofibers are then annealed at 600 degrees C to obtain ilmenite rhombohedral phase cadmium titanate nanofibers. The structure, composition, thermal stability and optical properties of as synthesized and annealed cadmium titanate nanofibers are characterized by X-ray diffraction, energy dispersive X-ray spectroscopy, scanning electron microscopy, transmission electron microscopy, thermogravimetric analysis, Fourier transform infrared spectroscopy and ultraviolet-visible spectroscopy. The average diameter and length of the nanofibers are found to be similar to 150-200 nm and similar to 100 mu m, respectively.

    • Aims: The current study was aimed to evaluate the beneficial effects and bioremediation potential of a Cd-tolerant bacterial strain, Serratia sp. CP-13, on the physiological and biochemical functions of Linum usitatissimum L., under Cd stress.Methods and Results: The bacterial strain was isolated from the wastewater collection point of Chakera, Faisalabad, Pakistan, as this place contains industrial wastewater of the Faisalabad region. The Serratia sp. CP-13, identified through 16S rRNA gene sequence analysis, exhibited a significant phyto-beneficial potential in terms of in vitro inorganic phosphate solubilization, indole-3-acetic acid production and 1-aminocyclopropane-1-carboxylic acid deaminase activity. Effects of Serratia sp. CP-13 inoculation on L. usitatissimum were evaluated by growing the plants in CdCl2 (0, 5 or 10 mg kg(-1) dry soil)-spiked soil. Without inoculation of Serratia sp. CP-13, Cd stress significantly reduced the plant biomass as well as the quantity of proteins and photosynthetic pigments due to enhanced H2O2, malondialdehyde (MDA) contents and impaired nutrient homeostasis. Subsequently, Serratia sp. CP-13 increased the plant fresh and dry biomass, plant antioxidation capacity, whereas it decreased the lipid peroxidation under Cd stress. In parallel, Serratia sp. inoculation assisted the Cd-stressed plants to maintain an optimum level of nutrients (K, Ca, P, Mg, Fe and Mn).Conclusions: The isolated bacterial strain (Serratia sp. CP-13) when applied to Cd-stressed L. usitatissimum inhibited the Cd uptake, reduced Cd-induced lipid peroxidation, maintained the optimum level of nutrients and thereby, enhanced L. usitatissimum growth. The analysis of bio-concentration and translocation factor revealed that L. usitatissimum with Serratia sp. CP-13 inoculation sequestered Cd in plant rhizospheric zone.Significance and Impact of the Study: Serratia sp. CP-13 inoculation is a potential candidate for the development of low Cd-accumulating linseed and could be used for phytostabilization of Cd-contaminated rhizosphere/soil colloids.

  • Discovery of Genetic Variation on Chromosome 5q22 Associated with Mortality in Heart Failure

    • Failure of the human heart to maintain sufficient output of blood for the demands of the body, heart failure, is a common condition with high mortality even with modern therapeutic alternatives. To identify molecular determinants of mortality in patients with new-onset heart failure, we performed a meta-analysis of genome-wide association studies and follow-up genotyping in independent populations. We identified and replicated an association for a genetic variant on chromosome 5q22 with 36% increased risk of death in subjects with heart failure (rs9885413, P = 2.7x10-9). We provide evidence from reporter gene assays, computational predictions and epigenomic marks that this polymorphism increases activity of an enhancer region active in multiple human tissues. The polymorphism was further reproducibly associated with a DNA methylation signature in whole blood (P = 4.5x10-40) that also associated with allergic sensitization and expression in blood of the cytokine TSLP (P = 1.1x10-4). Knockdown of the transcription factor predicted to bind the enhancer region (NHLH1) in a human cell line (HEK293) expressing NHLH1 resulted in lower TSLP expression. In addition, we observed evidence of recent positive selection acting on the risk allele in populations of African descent. Our findings provide novel genetic leads to factors that influence mortality in patients with heart failure.

  • Pathological complete response in patients with resected pancreatic adenocarcinoma after preoperative chemotherapy

    • Importance: Preoperative chemo(radio)therapy is increasingly used in patients with localized pancreatic adenocarcinoma, leading to pathological complete response (pCR) in a small subset of patients. However, multicenter studies with in-depth data about pCR are lacking.Objective: To investigate the incidence, outcome, and risk factors of pCR after preoperative chemo(radio)therapy.Design, Setting, and Participants: This observational, international, multicenter cohort study assessed all consecutive patients with pathology-proven localized pancreatic adenocarcinoma who underwent resection after 2 or more cycles of chemotherapy (with or without radiotherapy) in 19 centers from 8 countries (January 1, 2010, to December 31, 2018). Data collection was performed from February 1, 2020, to April 30, 2022, and analyses from January 1, 2022, to December 31, 2023. Median follow-up was 19 months.Exposures: Preoperative chemotherapy (with or without radiotherapy) followed by resection.Main Outcomes and Measures: The incidence of pCR (defined as absence of vital tumor cells in the sampled pancreas specimen after resection), its association with OS from surgery, and factors associated with pCR. Factors associated with overall survival (OS) and pCR were investigated with Cox proportional hazards and logistic regression models, respectively.Results: Overall, 1758 patients (mean [SD] age, 64 [9] years; 879 [50.0%] male) were studied. The rate of pCR was 4.8% (n = 85), and pCR was associated with OS (hazard ratio, 0.46; 95% CI, 0.26-0.83). The 1-, 3-, and 5-year OS rates were 95%, 82%, and 63% in patients with pCR vs 80%, 46%, and 30% in patients without pCR, respectively (P < .001). Factors associated with pCR included preoperative multiagent chemotherapy other than (m)FOLFIRINOX ([modified] leucovorin calcium [folinic acid], fluorouracil, irinotecan hydrochloride, and oxaliplatin) (odds ratio [OR], 0.48; 95% CI, 0.26-0.87), preoperative conventional radiotherapy (OR, 2.03; 95% CI, 1.00-4.10), preoperative stereotactic body radiotherapy (OR, 8.91; 95% CI, 4.17-19.05), radiologic response (OR, 13.00; 95% CI, 7.02-24.08), and normal(ized) serum carbohydrate antigen 19-9 after preoperative therapy (OR, 3.76; 95% CI, 1.79-7.89).Conclusions and Relevance: This international, retrospective cohort study found that pCR occurred in 4.8% of patients with resected localized pancreatic adenocarcinoma after preoperative chemo(radio)therapy. Although pCR does not reflect cure, it is associated with improved OS, with a doubled 5-year OS of 63% compared with 30% in patients without pCR. Factors associated with pCR related to preoperative chemo(radio)therapy regimens and anatomical and biological disease response features may have implications for treatment strategies that require validation in prospective studies because they may not universally apply to all patients with pancreatic adenocarcinoma.

  • Responses to Dehydration in the One-Humped Camel and Effects of Blocking the Renin-Angiotensin System

    • Our objectives were to compare the levels of circulating electrolytes, hormones, and renal function during 20 days of dehydration in camels versus the level in non-dehydrated camels and to record the effect of blocking angiotensin II AT1 receptors with losartan during dehydration. Dehydration induced significant increments in serum sodium, creatinine, urea, a substantial fall in body weight, and a doubling in plasma arginine vasopressin (AVP) levels. Plasma aldosterone, however, was unaltered compared with time-matched controls. Losartan significantly enhanced the effect of dehydration to reduce body weight and increase serum levels of creatinine and urea, whilst also impairing the rise in plasma AVP and reducing aldosterone levels. We conclude that dehydration in the camel induces substantial increments in serum sodium, creatinine, urea and AVP levels; that aldosterone levels are altered little by dehydration; that blockade of angiotensin II type 1 receptors enhances the dehydration-induced fall in body weight and increase in serum creatinine and urea levels whilst reducing aldosterone and attenuating the rise in plasma AVP.

  • Availability Analysis of Three Different Types of Combined Cycle Power Plants

    • The goal of power plants with higher efficiency and lower environmental risk than conventional power plants has produced three types of combined cycle power plants. These are: 1) the conventional combined cycle (CCC) power plant, 2) the pressurised fluidised bed combustion (PFBC) power plants, and 3) the integrated gasification combined cycle (IGCC) power plants. The efficiency of the CCC power plant has been sufficiently improved so that it is higher than the conventional power plant. Due to high combustion temperature in the gas turbine, the problem of NOx arose. Different NOx reduction techniques were adopted, such as steam or water injection, and dry low-NOx combustors. Strict environmental regulations in various countries were still violated. Therefore, the selective catalytic reduction (SCR) was installed in the waste heat boiler. Due to design of the CCC power plant, there is still risk of high NOx emission with a simple-cycle operation of the gas turbine. If the simple-cycle operation is not permitted then the effectiveness (energy availability) of the plant deteriorates. The PFBC and IGCC are new rival technologies. Both have a potential for higher efficiency and lower environmental risk than pulverized coal fired (PCF) power plants. These benefits will be lost if the effectiveness of these plants does not compete with the effectiveness of PCF power plants. Backup fuel can be utilized to improve the effectiveness of an IGCC power plant. Improving the effectiveness by using backup fuel involves increased cost for the fuel. If the gas turbine is operated as a simple-cycle with a backup fuel, then the problem of high NOx emission arises, as for the CCC power plant. Differently designed combined cycle power plants have been analysed to investigate the effects of reserve capacity (active redundancy) and repair policy. The Markov process has been used for the analysis and the following results were obtained. 1. The CCC power plant with an appropriate reserve capacity and a suitable repair policy can be chosen to obtain greater effectiveness of the plant with less NOx emission. 2. The effectiveness of PFBC power plants can potentially compete with the effectiveness of PCF plants. With an appropriate reserve capacity or double repair crew policy, the effectiveness of PFBC power plants can be even higher than that of the PCF power plants. 3. With an appropriate reserve capacity and with double repair crepolicy, the effectiveness of IGCC power plants can be improved to equal the effectiveness of PCF power plants. 4. The PFBC power plants have more potential for higher effectiventhan IGCC power plants without backup fuel. 5. The use of appropriate reserve capacity and a suitable repair minimize the use of backup fuel and NOx emissions and, at the same time, improve the effectiveness of integrated gasification combined cycle power plants.

  • FL-PMI : Federated Learning-Based Person Movement Identification through Wearable Devices in Smart Healthcare Systems

    • Recent technological developments, such as the Internet of Things (IoT), artificial intelligence, edge, and cloud computing, have paved the way in transforming traditional healthcare systems into smart healthcare (SHC) systems. SHC escalates healthcare management with increased efficiency, convenience, and personalization, via use of wearable devices and connectivity, to access information with rapid responses. Wearable devices are equipped with multiple sensors to identify a person's movements. The unlabeled data acquired from these sensors are directly trained in the cloud servers, which require vast memory and high computational costs. To overcome this limitation in SHC, we propose a federated learning-based person movement identification (FL-PMI). The deep reinforcement learning (DRL) framework is leveraged in FL-PMI for auto-labeling the unlabeled data. The data are then trained using federated learning (FL), in which the edge servers allow the parameters alone to pass on the cloud, rather than passing vast amounts of sensor data. Finally, the bidirectional long short-term memory (BiLSTM) in FL-PMI classifies the data for various processes associated with the SHC. The simulation results proved the efficiency of FL-PMI, with 99.67% accuracy scores, minimized memory usage and computational costs, and reduced transmission data by 36.73%.

  • Vericiguat in Patients with Heart Failure and Reduced Ejection Fraction.

    • BACKGROUND: The effect of vericiguat, a novel oral soluble guanylate cyclase stimulator, in patients with heart failure and reduced ejection fraction who had recently been hospitalized or had received intravenous diuretic therapy is unclear.METHODS: In this phase 3, randomized, double-blind, placebo-controlled trial, we assigned 5050 patients with chronic heart failure (New York Heart Association class II, III, or IV) and an ejection fraction of less than 45% to receive vericiguat (target dose, 10 mg once daily) or placebo, in addition to guideline-based medical therapy. The primary outcome was a composite of death from cardiovascular causes or first hospitalization for heart failure.RESULTS: Over a median of 10.8 months, a primary-outcome event occurred in 897 of 2526 patients (35.5%) in the vericiguat group and in 972 of 2524 patients (38.5%) in the placebo group (hazard ratio, 0.90; 95% confidence interval [CI], 0.82 to 0.98; P = 0.02). A total of 691 patients (27.4%) in the vericiguat group and 747 patients (29.6%) in the placebo group were hospitalized for heart failure (hazard ratio, 0.90; 95% CI, 0.81 to 1.00). Death from cardiovascular causes occurred in 414 patients (16.4%) in the vericiguat group and in 441 patients (17.5%) in the placebo group (hazard ratio, 0.93; 95% CI, 0.81 to 1.06). The composite of death from any cause or hospitalization for heart failure occurred in 957 patients (37.9%) in the vericiguat group and in 1032 patients (40.9%) in the placebo group (hazard ratio, 0.90; 95% CI, 0.83 to 0.98; P = 0.02). Symptomatic hypotension occurred in 9.1% of the patients in the vericiguat group and in 7.9% of the patients in the placebo group (P = 0.12), and syncope occurred in 4.0% of the patients in the vericiguat group and in 3.5% of the patients in the placebo group (P = 0.30).CONCLUSIONS: Among patients with high-risk heart failure, the incidence of death from cardiovascular causes or hospitalization for heart failure was lower among those who received vericiguat than among those who received placebo.   • Validation of borehole heat exchanger models against multi-flow rate thermal response tests

    • A recently developed vertical borehole ground heat exchanger model that accounts for transit time effects and time-varying short-circuiting heat transfer has been validated against two multi-flow-rate thermal response tests (MFR-TRT). The MFR-TRT, when performed with a wide range of flow rates, results in significant changes in the borehole thermal resistance, the borehole internal thermal resistance, and the short-circuiting heat transfer between the two legs of a single U-tube. The model accounts for short-circuiting by an analytically computed weighting factor that is used to determine the mean fluid temperature. The weighting factor portion of the model can be readily utilized in other ground heat exchanger models that currently rely on a simple mean fluid temperature. Use of the weighting factor is shown to give significantly better estimations of entering and exiting fluid temperature than using the simple mean fluid temperature. The new model is also compared to an alternative approach using an effective borehole thermal resistance. While both the effective borehole thermal resistance model and the weighting factor give quite good results a few hours after a step change in flow rate, the weighting factor model gives much better results in the first few hours after a step change in flow rate.

    • Psychiatric practice faces many challenges in the first quarter of 21st century. Society has transformed, as have training requirements and patient expectations, underlining an urgent need to look at educational programmes. Meanwhile, awareness has grown around psychiatric disorders and there are evolving workforce trends, with more women going to medical school and specialising in psychiatry. Trainee psychiatrists carry different expectations for work-life balance and are increasingly becoming conscious of their own mental health. A tendency to see health as a commodity and the litigious nature of society has elicited additional pressures for healthcare professionals. Cartesian mind-body dualism has created further complexity and this can often be frustrating for patients and care-partners alike. In many cultures across Asia and beyond, patients can present with physical symptoms to express underlying psychological distress with increasing physical investigations. Simultaneously, in various countries, a shift from asylums to community-based interventions and then home treatments have changed psychiatric care in remarkable ways. These changes have added to pressures faced by mental healthcare professionals. However, trainees and other mental healthcare professionals continue to receive similar training as they did a generation ago. The tensions and differences in ideology/orientation between different branches of psychiatry have made responses to patient needs challenging. Recognising that it is difficult to predict the future, this World Psychiatric Association-Asian Journal of Psychiatry Commission makes recommendations that could help institutions and individuals enhance psychiatric education. This Commission draws from existing resources and recent developments to propose a training framework for future psychiatrists.

    • Heart failure is a global public health problem that affects more than 26 million people worldwide. The global burden of heart failure is growing and is expected to increase substantially with the ageing of the population. Heart failure with reduced ejection fraction accounts for approximately 50% of all cases of heart failure in the United States and is associated with substantial morbidity and reduced quality of life. Several diseases, such as myocardial infarction, certain infectious diseases and endocrine disorders, can initiate a primary pathophysiological process that can lead to reduced ventricular function and to heart failure. Initially, ventricular impairment is compensated for by the activation of the sympathetic nervous system and the renin-angiotensin-aldosterone system, but chronic activation of these pathways leads to worsening cardiac function. The symptoms of heart failure can be associated with other conditions and include dyspnoea, fatigue, limitations in exercise tolerance and fluid accumulation, which can make diagnosis difficult. Management strategies include the use of pharmacological therapies and implantable devices to regulate cardiac function. Despite these available treatments, heart failure remains incurable, and patients have a poor prognosis and high mortality rate. Consequently, the development of new therapies is imperative and requires further research.

    • This paper aims to identify global digital trends across industries and to map emerging business areas by co-word analysis. As the industrial landscape has become complex and dynamic due to the rapid pace of technological changes and digital transformation, identifying industrial trends can be critical for strategic planning and investment policy at the ¯rm and regional level. For this purpose, the paper examines industry and technology pro¯les of top startups across four industries (i.e. education, ¯nance, healthcare, manufacturing) using CrunchBase metadata for the period 2016–2018 and studies in which subsector early-stage ¯rms bring digital technologies on a global level. In particular, we apply word co-occurrence analysis to reveal which subindustry and digital technology keywords/keyphrases appear together in startup company classification. We also use network analysis to visualize industry structure and to identify digitalization trends across sectors. The results obtained from the analysis show that gamification and personalization are emerging trends in the education sector. In the finance industry, digital technologies penetrate in a wide set of services such as financial transactions, payments, insurance, venture capital, stock exchange, asset and risk management. Moreover, the data analyses indicate that health diagnostics and elderly care areas are at the forefront of the healthcare industry digitalization. In the manufacturing sector, startup companies focus on automating industrial processes and creating smart interconnected manufacturing. Finally, we discuss the implications of the study for strategic planning and management.

    • With growing urbanization and increase in energy demand for space heating and cooling in buildings, reliability, security, distribution of energy and its efficient end use is of utmost importance particularly when emissions from energy use and indoor thermal comfort of the end users isof question. Numerous solutions have been put forth in the field of effective distribution of thermal energy to meet the space heating and cooling requirements of buildings to assure indoor comfort of which fifth generation district heating and cooling (5GDHC) is regarded as a viable solution in terms of low emissions, low distribution losses and effective energy supply. The challenges however lie in connecting the buildings in the network to facilitate the energy sharing between the buildings such that otherwise waste heat from one of the buildings can be used by another building connected to the same network. This paper presents the concept and design of such advanced 5GDHC network which facilitates energy sharing between connected buildings comprising ofoffice spaces, residential complex, shopping centre and other occupying 61,901 square meters in Hyllie region of Malmö, Sweden. The system design consists of a bi-directional two pipe network with a local bore field consisting of 103 bore hole heat exchangers as the primary energy source for the network with heat pumps and dedicated direct cooling heat exchangers to alter the temperature of district heating and cooling fluid going into the buildings. The system as such is self-sufficient and requires electricity only to run the heat pumps and auxiliary control systems to meet the thermal demands of the connected buildings.

    • IMPORTANCE: Worsening chronic heart failure (HF) is a major public health problem.OBJECTIVE: To determine the optimal dose and tolerability of vericiguat, a soluble guanylate cyclase stimulator, in patients with worsening chronic HF and reduced left ventricular ejection fraction (LVEF).DESIGN, SETTING, AND PARTICIPANTS: Dose-finding phase 2 study that randomized 456 patients across Europe, North America, and Asia between November 2013 and January 2015, with follow-up ending June 2015. Patients were clinically stable with LVEF less than 45% within 4 weeks of a worsening chronic HF event, defined as worsening signs and symptoms of congestion and elevated natriuretic peptide level requiring hospitalization or outpatient intravenous diuretic.INTERVENTIONS: Placebo (n = 92) or 1 of 4 daily target doses of oral vericiguat (1.25 mg [n = 91], 2.5 mg [n = 91], 5 mg [n = 91], 10 mg [n = 91]) for 12 weeks.MAIN OUTCOMES AND MEASURES: The primary end point was change from baseline to week 12 in log-transformed level of N-terminal pro-B-type natriuretic peptide (NT-proBNP). The primary analysis specified pooled comparison of the 3 highest-dose vericiguat groups with placebo, and secondary analysis evaluated a dose-response relationship with vericiguat and the primary end point.RESULTS: Overall, 351 patients (77.0%) completed treatment with the study drug with valid 12-week NT-proBNP levels and no major protocol deviation and were eligible for primary end point evaluation. In primary analysis, change in log-transformed NT-proBNP levels from baseline to week 12 was not significantly different between the pooled vericiguat group (log-transformed: baseline, 7.969; 12 weeks, 7.567; difference, -0.402; geometric means: baseline, 2890 pg/mL; 12 weeks, 1932 pg/mL) and placebo (log-transformed: baseline, 8.283; 12 weeks, 8.002; difference, -0.280; geometric means: baseline, 3955 pg/mL; 12 weeks, 2988 pg/mL) (difference of means, -0.122; 90% CI, -0.32 to 0.07; ratio of geometric means, 0.885, 90% CI, 0.73-1.08; P = .15). The exploratory secondary analysis suggested a dose-response relationship whereby higher vericiguat doses were associated with greater reductions in NT-proBNP level (P < .02). Rates of any adverse event were 77.2% and 71.4% among the placebo and 10-mg vericiguat groups, respectively.CONCLUSIONS AND RELEVANCE: Among patients with worsening chronic HF and reduced LVEF, compared with placebo, vericiguat did not have a statistically significant effect on change in NT-proBNP level at 12 weeks but was well-tolerated.     • The development of energy storage systems paves the way towards a high integration of renewable energy sources in the electricity generation sector. Considering above, this paper introduces a new energy management strategy to efficiently coordinate a hybrid energy storage system based on pumped hydro storage (long term bulk storage) with batteries (short term, more flexible). For the purpose of this analysis, hourly time series of irradiation, wind speed, temperature and real measured load (characteristic for farmstead) covering one year were gathered for the selected site in Algeria. The optimal size of the system is determined based on a multi-objective optimization using a grey wolf optimizer implemented in MATLAB software. The results indicate that the hybrid storage system enables achieving higher reliability at lower cost in comparison to a system with single storage technology. The use of hybrid storage also reduces the curtailment of renewable generation. Further findings reveal that the cost of an optimal energy supply system with 97.5% reliability is 0.162 €/kWh, 0.207 €/kWh and 1.462 €/kWh for hybrid storage, battery and pumped storage, respectively. However, sensitivity analysis shows that the optimal hybrid storage configuration is less resilient when changes in irradiation/temperature/load are considered. This indicates that special actions (upscale of installed power) must be undertaken to avoid lower performance of hybrid storage systems. In summary, the hybrid storage system seems to be better sized for consideration in optimized solar/wind conditions, but by avoiding oversizing they are less resilient to future potential changes in renewable energy availability.

    • Natural gas is the most promising renewable energy source and its widespread availability ensured its importance for early applications in stationary fuel cells as a reliable and low cost fuel. Therefore it is very important to efficiently utilization of natural gas in low temperature fuel cells. Herein, we demonstrate the synthesis of perovskite material of Yttrium doped Sr0.92FexTi1-xO3-delta (x = 0.25, 030) (YSFT) by solid state reaction method and further investigated as a new cathode material for a low temperature solid oxide fuel cell fueled by natural gas. The YSFT is characterized by X-ray powder diffraction, Brunauer-Emmett-Teller and scanning electron microscopy. The perovskite structure is achieved at relatively low temperature (850 degrees C). The average crystalline size is found 28 nm and 36 nm for x = 0.25 and 0.30 respectively. TGA results showed the lattice oxygen loss of YSFT is about 0.206% in its original weight in the temperature range of 25-1000 degrees C. The maximum electronic conductivities of 2.3 Scm(-1) and 2.07 Scm(-1) are achieved for x = 025 and x = 0.30 at 550 degrees C in air atmosphere respectively. It is observed that the oxygen reduction is enhanced due to the perovskite crystal structure and oxygen vacancies play an important role in the redox reaction to improve the performance of fuel cell. The YSFT perovskite cathode material based fuel cell with natural gas have achieved the power density of 250 mWcm(-2) for x = 025 at 550 degrees C. The fuel cell device has demonstrated very stable results by running continuously for 5 h with domestic available natural gas.

    • Hierarchical zeolites have been identified as special catalytic materials with improved catalytic properties. In this study, hierarchical bifunctional ZSM5 based catalysts were prepared by desilication for controlled mesoporosity development and have been modified by Co doping. Their performance in the catalytic pyrolysis of oak in a lab scale reactor was evaluated. Desilicated counterparts were proven more active in deoxygenation of bio oil, while carbon deposition on the catalysts reduced compared to non-desilicated counterparts. Increased Lewis acidity favors decarboxylation reactions, while higher olefins as well as PAH content indicate easier diffusion within and from the porous network and interactions in the mesopores. The conversion of bulky lignin molecules (alkoxy phenols) is enhanced by the mesopores, while acidity is of secondary importance. Coke deposition inside the pores is more profound in the desilicated catalysts due to larger pore size. Carbon deposition on the catalysts is reduced in the following order: HZSM5 > Co/HZSM5 > Ds-HZSM5 > Co/Ds-HZSM5. GC–MS characterization of the CH2Cl2 soluble coke indicated that for the desilicated counterparts the main coke precursors are the bulky lignin molecules which are partially deoxygenated.

    • Conventional randomized controlled trials (RCTs) can be expensive, time intensive, and complex to conduct. Trial recruitment, participation, and data collection can burden participants and research personnel. In the past two decades, there have been rapid technological advances and an exponential growth in digitized healthcare data. Embedding RCTs, including cardiovascular outcome trials, into electronic health record systems or registries may streamline screening, consent, randomization, follow-up visits, and outcome adjudication. Moreover, wearable sensors (i.e. health and fitness trackers) provide an opportunity to collect data on cardiovascular health and risk factors in unprecedented detail and scale, while growing internet connectivity supports the collection of patient-reported outcomes. There is a pressing need to develop robust mechanisms that facilitate data capture from diverse databases and guidance to standardize data definitions. Importantly, the data collection infrastructure should be reusable to support multiple cardiovascular RCTs over time. Systems, processes, and policies will need to have sufficient flexibility to allow interoperability between different sources of data acquisition. Clinical research guidelines, ethics oversight, and regulatory requirements also need to evolve. This review highlights recent progress towards the use of routinely generated data to conduct RCTs and discusses potential solutions for ongoing barriers. There is a particular focus on methods to utilize routinely generated data for trials while complying with regional data protection laws. The discussion is supported with examples of cardiovascular outcome trials that have successfully leveraged the electronic health record, web-enabled devices or administrative databases to conduct randomized trials.

    • The aim of our study is to investigate the effect of spironolactone on 30-day outcomes in patients with acute heart failure (AHF) and the association between treatment and outcomes stratified by biomarkers. We conducted a secondary analysis of the biomarker substudy of the multicenter COACH (Co-ordinating Study Evaluating Outcomes of Advising and Counseling in Heart Failure) trial involving 534 AHF patients for 30-day mortality and HF rehospitalizations. Spironolactone therapy was initiated and terminated at the discretion of the treating physician; 30-day outcomes were compared between patients who were treated with spironolactone and those who were not. Outcomes with spironolactone therapy. were explored based on N-terminal pro-B-type natriuretic peptide, ST2, galectin-3, and creatinine levels. Spironolactone was prescribed to 297 (55.6%) patients at discharge (158 new and 139 continued). There were 19 deaths and 30 HF rehospitalizations among 46 patients by 30 days. Patients discharged on spironolactone had significantly less 30-day event (hazard ratio 0.538, p = 0.039) after adjustment for multiple risk factors. Initiation of spironolactone in patients who were not on spironolactone before admission was associated with a significant reduction in event rate (hazard ratio 0.362, p = 0.027). The survival benefit of spironolactone was more prominent in patient groups with elevations of creatinine, N-terminal pro B-type natriuretic peptide, ST2, or galectin-3. In conclusion, AHF patients who received spironolactone during hospitalization had significantly fewer 30-day mortality and HF rehospitalizations, especially in high-risk patients.

    • Acute heart failure is a major cause of urgent hospitalizations. These are followed by marked increases in death and rehospitalization rates, which then decline exponentially though they remain higher than in patients without a recent hospitalization. Therefore, optimal management of patients with acute heart failure before discharge and in the early post-discharge phase is critical. First, it may prevent rehospitalizations through the early detection and effective treatment of residual or recurrent congestion, the main manifestation of decompensation. Second, initiation at pre-discharge and titration to target doses in the early post-discharge period, of guideline-directed medical therapy may improve both short- and long-term outcomes. Third, in chronic heart failure, medical treatment is often left unchanged, so the acute heart failure hospitalization presents an opportunity for implementation of therapy. The aim of this scientific statement by the Heart Failure Association of the European Society of Cardiology is to summarize recent findings that have implications for clinical management both in the pre-discharge and the early post-discharge phase after a hospitalization for acute heart failure.

    • Drøbak Montessori lower secondary school is Norway’s first plus-energy school and also the first school built after the Norwegian Powerhouse-concept, www.powerhouse.no. This concept implies that the building shall produce more renewable energy during the lifetime of the building, than used for materials, production, operation, renovation and demolition. To achieve the energy ambitions for the project there has been a high focus on obtaining an effective and low-pressure ventilation system. The chosen solution is a concept with relatively tall rooms with displacement ventilation that varies between fully mechanical and hybrid ventilation depending on the time of the year. The extract air is removed from the rooms by overflow to adjacent areas. During summer the exhaust air is directly let out through an opening in the top of the centrally placed atrium. In the heating periods the exhaust air is driven mechanically through the air handling unit for heat recovery. The school has an all-air HVAC-system where all the heating and cooling demand for the building are covered by the ventilation system, no additional heating sources is installed in the building. Room heating only provided by ventilation is not something new, but is often combined with mixed-flow ventilation. At the Montessori school in Drøbak the heating with ventilation is combined with displacement ventilation. To avoid a short circuiting of the supplied fresh-air when the occupants are present, the air is supplied with a lower temperature than the surrounding air. During night time the system switches to heating mode when necessary, depending on recorded indoor and outdoor temperatures. When in heating mode, the supply air is recirculated and heated to a supply temperature higher than the surroundings. The supply air temperature depends on the outdoor temperature and the airflow is demand-controlled based on temperature and CO2. To ensure that the chosen solution provides a satisfactory thermal indoor climate and to provide input to the tuning and optimization of the control strategies, simulations with a two-zone model, advanced simulations in IDA-ICE, laboratory tests and field measurements has been performed. This paper will present the analysis, results and conclusions from the above mentioned two-zone model compared to the results from the field test. The results from the advanced simulation in IDA-ICE and the laboratory tests will not be presented in this paper. The results from the two-zone model and the field test show promising results when it comes to achieving thermal comfort by using displacement ventilation for heating. The results also shows good consistency between the 2-zone model and the measured data from the field test. This research will provide useful knowledge for future projects when it comes to minimizing the energy use, costs and complexity for HVAC-installations in buildings where displacement ventilation is suitable.

    • The incineration furnace at National Veterinary Institute (SVA) in Uppsala, Sweden, generates a large amount of waste heat. Some of this heat is partially recovered and is used for heating one of the main buildings during the furnace operation. However, around 2,000 MWh is wasted annually due to unavailability of any storage system. This work studies in detail four different solutions to utilize the available waste heat to provide heating to neighbouring buildings. The study initiates with the quantitative analysis of the available waste heat and the heating demands of the nearby buildings. The first solution considered in this work is to use water storage tanks to provide heating to the SVA main building when the incineration process is not operational. The second considered solution is to maximize the direct use of waste heat in the SVA buildings during the incinerator operating period. As a third alternative, the first two solutions, i.e. direct use of waste heat and use of storage tank, respectively, have been studied simultaneously in various combinations. The fourth solution explores the possibility of using borehole thermal energy storage for seasonal storage of heat during summer months. The results of this study underscore the complexity of storing and using waste heat. The results suggest that the direct use of heat (i.e. the second option) results in the largest savings-to-investment ratio and the shortest payback period. The borehole thermal energy storage and the ground-source heat pump system (i.e. the fourth option) turns out to be the most profitable solution after 25 years. However, this solution requires significant investments; hence payback period is much longer compared to other design options. For shorter analysis period, e.g. 15 years, the third option, i.e. the combination of direct waste heat and storage tank, shows the highest economic profitability. It provides highest monetary savings when discounted to net present values and has the second shortest payback period. Overall, this study contributes to the understanding of heat management between different buildings and underlines the importance of demands and availability analysis to determine the optimal solution.

    • In the present work, cost-effective nanocomposite electrolyte (Ba-SDC) oxide is developed for efficient low-temperature solid oxide fuel cells (LTSOFCs). Analysis has shown that dual phase conduction of O-2 (oxygen ions) and H+ (protons) plays a significant role in the development of advanced LTSOFCs. Comparatively high proton ion conductivity (0.19 s/cm) for LTSOFCs was achieved at low temperature (460°C). In this article, the ionic conduction behaviour of LTSOFCs is explained by carrying out electrochemical impedance spectroscopy measurements. Further, the phase and structure analysis are investigated by X-ray diffraction and scanning electron microscopy techniques. Finally, we achieved an ionic transport number of the composite electrolyte for LTSOFCs as high as 0.95 and energy and power density of 90% and 550 mW/cm2, respectively, after sintering the composite electrolyte at 800°C for 4 h, which is promising. Our current effort toward the development of an efficient, green, low-temperature solid oxide fuel cell with the incorporation of high proton conductivity composite electrolyte may open frontiers in the fields of energy and fuel cell technology.

    • Fuel cell technology holds the combination of benefits, which are barely offered by any other energy generating technology. Because the fuel used in this technology is found in abundance in nature and can also be renewed/sustained. Pakistan is blessed with renewable energy resources which are suitable for fuel cell technology. Therefore, fuel cell technology offers a great opportunity to meet the demand of energy and for the sustainable development of Pakistan. The energy research group at COMSATS Institute of Information Technology (CIIT), Lahore has made efforts to study the technical aspects of fuel cell technology and its commercial benefits. The research group is interested in finding ways and means of generating and storing the energy produced by using fuel cells. In this paper, the research activities on fuel cell technology in Pakistan have been reviewed and it is also discussed how this technology can resolve the current energy crises in Pakistan and can be the source of sustainable energy. It has been also reviewed that the country would greatly benefit from fuel cells and fuel cell hybrid system (environmental friendly technology), which could be the best solution for electricity production as well for automobile industry.

    • The Swedish Strategic Innovation Agenda for Geoenergy develops a strategy for highlighting the need for geoenergy research, development and innovation in relation to the business community and authorities. Sweden has a strong global position in the field. Geothermal energy is the third­largest renewable energy source in Sweden, along with wind power. Currently, approximately 18 TWh of renewable heat per year is supplied to household, industrial and commercial buildings. In addition, there is 1­2 TWh of cooling. There is a great need for a long­term strategy and financial plan for geoenergy research and innovation in Sweden, in order to maintain and further develop research groups and the country's leading position in the field. Today, Swedish research in this field comprises about 14 full­time positions at universities, institutes and companies. This is little compared to the existing use of geothermal energy, about 19 TWh, which is worth about 15 billion SEK in consumer sales. It is also little in relation to possible future contributions to sustainable development in Sweden and abroad. The agenda is divided into short independent chapters that are tailored to different target groups/ applications. Each chapter consists of an introduction to the current situation, how geoenergy can strategically contribute to a sustainable society, and what actions/needs are a priority.
