    • Objective: Anaemia in low-risk myelodysplastic syndromes (MDS) is associated with reduced quality of life (QoL). Response to treatment with erythropoietin +/- granulocyte colony-stimulating factor (G-CSF) is associated with improved QoL, but whether transfusion therapy with higher haemoglobin (Hb) target levels has similar effects is unknown. The objective for this prospective phase II Nordic multicentre trial was to assess QoL, response rate and physical function in elderly anaemic MDS patients treated to a target Hb level of andgt; 120 g /L. Methods: Thirty-six elderly patients with low-and intermediate-1 risk MDS received darbepoetin (DA) 300 mu g/wk, with the addition of G-CSF if no response. If the Hb target was reached at 16 wk, treatment was maintained until week 26. Remaining patients were transfused to reach the target level for at least 8 wk. Results: Twenty-seven patients completed the study. Response rate to DA +/- G-CSF was 67% in evaluable patients and 56% according to intention to treat. Eighteen patients reached the target Hb level according to protocol. QoL scores for fatigue, dyspnoea, constipation, and physical, role and social functioning improved significantly during study, with similar results for transfused and untransfused patients. Maintaining Hb andgt; 120 g /L did not confer a higher transfusion rate, once the target was reached. In two of fourteen patients, magnetic resonance imaging T2* indicated cardiac iron overload, however, without association with ferritin levels. Conclusions: In elderly anaemic MDS patients, an increment in haemoglobin is associated with improved QoL, whether induced by growth factor treatment or transfusion therapy.

  • Tissue variations of mosaic genome-wide paternal uniparental disomy and phenotype of multi-syndromal congenital hyperinsulinism

    • Mosaic genome-wide paternal uniparental disomy (GW-pUPD) is a rarely recognised disorder. The phenotypic manifestations of multilocus imprinting defects (MLIDs) remain unclear. We report of an apparently non-syndromic infant with severe congenital hyperinsulinism (CHI) and diffuse pancreatic labelling by 18F*-DOPA-PET/CT leading to near-total pancreatectomy. The histology was atypical with pronounced proliferation of endocrine cells comprising >70% of the pancreatic tissue and a small pancreatoblastoma. Routine genetic analysis for CHI was normal in the blood and resected pancreatic tissue. At two years’ age, Beckwith-Wiedemann Syndrome (BWS) stigmata emerged, and at five years a liver tumour with focal nodular hyperplasia and an adrenal tumour were resected. pUPD was detected in 11p15 and next in the entire chromosome 11 with microsatellite markers. Quantitative fluorescent PCR with amplification of chromosome-specific DNA sequences for chromosomes 13, 18, 21 and X indicated GW-pUPD. A next generation sequencing panel with 303 SNPs on 21 chromosomes showed pUPD in both blood and pancreatic tissue. The mosaic distribution of GW-pUPD ranged from 31 to 35% in blood and buccal swap to 74% in the resected pancreas, 80% in a non-tumour liver biopsy, and 100% in the liver focal nodular hyperplasia and adrenal tumour. MLID features included transient conjugated hyperbilirubinaemia and lack of macrosomia from BWS (pUPD6); and behavioural and psychomotor manifestations of Angelman Syndrome (pUPD15) on follow-up. In conclusion, atypical pancreatic histology in apparently non-syndromic severe CHI patients may be the first clue to BWS and multi-syndromal CHI from GW-pUPD. Variations in the degree of mosaicism between tissues explained the phenotype.

  • Disturbed intestinal microbiota (dysbiosis) and micro dynamics in patients treated for appendicitis and diverticulitis

    • Introduction: The human gut microbiota is a large dynamic bacterial community, which is influenced by for instance antibiotic treatment and hospitalization. In patients with inflammatory bowel disease the diversity of gut microbiota is thought to be less diverse. The role of the gut microbiota in acute appendicitis and diverticulitis is still unclear. To investigate the microbial diversity in patients suffering from appendicitis or diverticulitis, and the microbiota dynamics after antibiotic therapy and hospitalization we performed an open observation study.Methods and population: We have performed 16S rRNA sequence analysis on 42 individuals diagnosed with appendicitis and 18 individuals with diverticulitis as well as 33 healthy controls. Cultivation of the aerobic bacterial flora was performed as a complement to sequence analysis.Results: In sequencing data at genus level, there are distinctive differences when comparing healthy controls to patients diagnosed with appendicitis. Healthy controls have a flora dominated by Bacteroides, and Faecalibacterium, Ruminococcus and Prevotella while appendicitis patients show an intestinal flora with a higher abundance of Escherichia/Shigella and unclassified Enterobacteriaceae. The same pattern, however not quite as distinct could be seen for the diverticulitis patients. The microbial diversity increases after treatment with antibiotics and hospitalization.In the cultivated aerobic flora there was a significant loss of Escherichia coli and a significant gain of Citrobacter species, in the appendicitis group. In the appendicitis group as well as in the diverticulitis group there was a significant gain of Enterococcus faecium and Yeasts.Conclusion: The main findings of this study are that patients arriving at the emergency department with acute appendicitis or diverticulitis have an already significant disturbed fecal microbiota previous to antibiotic treatment and hospitalization.

    • The incidence of scoliosis and trunk asymmetry were studied in 60 patients with upper limb dysmelia of the transverse type. The evaluations were based on radiographic measurements of the spine and scoliometer readings of the angle of trunk rotation (ATR), which were correlated with the side and level of the limb deficiency and also with leg length-inequality (LLI). Nineteen patients (31%) had a scoliosis between 10 and 19 degrees, whereas the curves were between 5 and 10 degrees in another 30 patients and the remainder had no measurable curve. The scoliosis seemed at least partly to be of postural origin as LLI significantly correlated with the direction of the curves, but there was no correlation between the Cobb angles and the magnitude of LLI. The scoliometer readings did not correlate with the scoliosis or LLI. Our findings indicate that most patients with upper limb transverse amputations do not develop a significant scoliosis. A scoliometer screen has a low positive predictive rate for scoliosis and the diagnosis of scoliosis requires a radiograph of the spine.

    • Det råder brist på lärare. Det slutar fler lärare än det utbildas nya. Fler måste lockas till att bli lärare. Nya vägar måste hittas för att nå ut till blivande lärarstudenter. Unga människor tillbringar numera mer tid framför datorn än de sitter framför tv:n eller läser tidningar. Det framstår därför som logiskt att söka nya lärare, unga människor, via kampanjer på internet och via sociala medier. Det framstår kanske lika logiskt att ge en alternativ bild av skolan och läraren än den vi vanligtvis nås av i medierna. För den bilden berättar inte hela historien.

  • Framing service innovation in healthcare

    • Healthcare faces multiple challenges. Besides a raging pandemic have the number of people struggling with mental illness and chronic illnesses increased. Service innovation is a possible way of meeting these problems. However, service innovation is a scattered concept, with many different conceptualizations and schools of thought, all providing vital research aspects. Research has unraveled three elements that form different approaches for service innovation research. Healthcare also offers a complex context for conducting change and service innovation, containing prerequisites challenging previous understanding and conceptualizations of service innovation.To study service innovation in healthcare and to clarify research, this thesis aims to develop a framework for service innovation that relates and differentiates three approaches and their key characteristics and to determine how the prerequisites for healthcare relate to this framework, and finally to study, test, and illustrate the framework in the empirical context. The thesis builds upon three studies and four individual papers. The studies consist of one literature review, providing the conceptual groundwork for constructing the framework, and two empirical studies assessing and further developing the framework. The individual papers uses the empirical studies and depart from the different approaches to service innovation and healthcare prerequisites, providing key insights and clarifying the different approaches' strengths and weaknesses. The proposed framework recognizes the need for multiple service innovation research approaches since no single approach is enough to study the multifaceted service innovation phenomena in healthcare. The individual papers also contribute to the growing literature stream addressing service innovation in healthcare by describing social entrepreneurs' motivation during the service innovation process and explaining some of the effects of different service innovation types.

  • God havsmiljö 2020 : Marin strategi för Nordsjön och Östersjön Del 3: Övervakningsprogram

    • Havsmiljöförordningens övergripande mål är att upprätthålla eller uppnå en god miljöstatus i de svenska förvaltningsområdena Nordsjön och Östersjön till år 2020. En av uppgifterna i den första förvaltningsperioden är att fastställa övervakningsprogram.God miljöstatus baseras på ett ramverk av så kallade deskriptorer som anges i havsmiljödirektivet, det vill säga det EU-direktiv som i Sverige genomförs genom havsmiljöförordningen. Deskriptorerna beskriver god miljöstatus på en övergripande nivå för elva temaområden. Till varje deskriptor hör en rad kriterier som anger vad som ska ingå i en bedömning av miljöstatus. Utifrån de elva deskriptorerna har Sverige fastställt 13 övervakningsprogram. Sex program utgår ifrån olika biodiversitetsteman som berörs av en upp till tre deskriptorer, medan de övriga sju programmen utgår ifrån de deskriptorer som är mer inriktade mot belastning och miljöförändring.För varje program har ett antal underprogram föreslagits baserat på den nuvarande övervakningen och/eller planerad övervakning. Övervakning som ingår i programmen ska vara pågående och data ska vara tillgängliga. I programmen ingår nationell och regional miljöövervakning inklusive verksamhetsutövares recipientkontroll. Dessutom ingår annan typ av datainsamling som till exempel inventeringar av tumlare och uppgifter om omfattningen av mänskliga aktiviteter som orsakar belastning och miljöförändringar. Enligt havsmiljödirektivet ska övervakningen fånga upp tillstånd och miljöförändringar, belastning och omfattning av aktiviteterna som orsakar belastningen samt effekter av åtgärder. Eftersom nästa steg i havsförvaltningscykeln är att fastställa åtgärdsprogram kommer övervakning för att följa upp åtgärder att läggas till övervakningsprogrammen först under nästa förvaltningscykel.I beskrivningarna av programmen framgår hur den nuvarande övervakningen motsvarar de krav som ställs på dataunderlag genom havsmiljödirektivets bilaga III samt genom deskriptorer, kriterier, indikatorer och beslutade miljökvalitetsnormer. I dagens övervakning saknas bland annat tillräcklig övervakning för uppföljning av livsmiljöers tillstånd och utbredning. För marint avfall, buller och främmande arter saknas nationellt samordnad övervakning, men det görs regionala insatser och ett antal projekt har genomförts eller påbörjats för att öka kunskapen om hur övervakning bäst ska utformas. För de program som har pågående övervakning beskrivs utvecklingsbehoven för att förbättra underlaget för de återkommande tillståndsbedömningarna.Övervakningsprogrammet som fastställs under 2014 utgör således inte ett fast program för kunskapsinhämtning. Bristerna kommer att beaktas i det fortsatta genomförandet av havsmiljöförordningen där utveckling av indikatorer och övervakning kommer att ske kontinuerligt.

  • Diabetes in treated hypertension is common and carries a high cardiovascular risk: results from a 28-year follow-up.

    • OBJECTIVE: The objective of this study was to analyse predictive factors for development of type 2 diabetes during life-long therapy for hypertension and the alleged additional cardiovascular risk this constitutes. METHODS: The study group (n = 754) comprised the hypertensive subgroup of a randomized population sample of 7500 men, aged 47-54 years, screened for cardiovascular risk factors and followed for 25-28 years. The patients were treated with thiazide diuretics and beta-adrenergic blocking drugs with the addition of hydralazin during the first decade. Calcium antagonists were substituted for hydralazin and, if needed, angiotensin-converting enzyme inhibitors were added when these drugs became available. RESULTS: A total of 148 (20.4%) treated hypertensive patients developed diabetes during 25 years, and in multivariate Cox regression analysis body mass index, serum triglycerides and treatment with beta-blockers were positively related with this complication. New-onset diabetes implied a significantly increased risk for stroke [hazard ratio (HR): 1.67; 95% confidence interval (95% CI): 1.1-2.6; P < 0.05], myocardial infarction (OR: 1.66; 95% CI: 1.1-2.5; P < 0.05) and mortality (OR: 1.42; 95% CI: 1.1-1.9; P < 0.05). The greatest risk for stroke was new-onset diabetes, followed by smoking (OR: 1.46; 95% CI: 1-2.2; P = 0.07) and the greatest risk for myocardial infarction was new-onset diabetes, followed by smoking (HR: 1.64; 95% CI: 1.1-2.4; P < 0.01). The greatest risk for mortality was smoking (HR: 1.73; 95% CI: 1.3-2.2; P < 0.005). Achieved systolic and diastolic blood pressure were not predictive of cardiovascular complications or death. The mean observation time from onset of diabetes mellitus to a first stroke was 9.1 years and to a first myocardial infarction 9.3 years. CONCLUSION: Diabetes in treated hypertensive patients is alarmingly common and carries a high risk for cardiovascular complications and mortality.

    • The inner crust of mature neutron stars, where an elastic lattice of neutron-rich nuclei coexists with a neutron superfluid, impacts on a range of astrophysical phenomena. The presence of the superfluid is key to our understanding of pulsar glitches, and is expected to affect the thermal conductivity and hence the evolution of the surface temperature. The coupling between crust and superfluid must also be accounted for in studies of neutron star dynamics, discussions of global oscillations and associated instabilities. In this paper we develop Lagrangian perturbation theory for this problem, paying attention to key issues like superfluid entrainment, potential vortex pinning, dissipative mutual friction and the star's magnetic field. We also discuss the nature of the core-crust interface. The results provide a theoretical foundation for a range of interesting astrophysical applications.

    • Fokus för den här avhandlingen är företag som växer genom replikering, det vill säga genom att skapa och driva liknande enheter på många olika platser, så som t.ex. McDonald’s och Starbucks. Replikerande företags livscykel beskrivs normalt i termer av en initial utforskande fas, följt av en exploateringsfas där standardisering är en hörnsten i företagets strategi - vad som därefter följer är emellertid oklart. Varsamt åldrande innebär dock vanligtvis att man måste anta de utmaningar som följer i kölvattnet av stabilitet och förändring.Modeindustrin kan beskrivas som oberäknelig och föränderlig, och har varit så i århundraden. Konkurrensen är stenhård och efterfrågan är svår att förutse då mode och trender hela tiden förändras. Med tanke på det beroende av standadisering som finns i replikerande företag kan man fråga sig hur ett företag i modebranschen kan hantera en replikeringsstrategi. Den trade-off som finns mellan fördelarna av standardisering och fördelarna av lärande och anpassning ställs på sin spets i ett sådant företag.Renässansen stora konstnärer mötte en liknande utmaning om än i lite annan form. Baserat på kollektivt arbete skapade renässansens verkstäder såväl replikor som nya verk i mästarens namn. För verkstaden betydde det att de skulle replikera mästarens stil till den grad att enskilda bidrag från lärlingar inte gick att skilja från mästarens eget arbete. Nya lärlingar var tvungna att lära sig att “emulera” mästarens stil och fungerar som en förlängning av hans hand och öga.Den här avhandlingen tar därför avstamp i konstens värld, mer specifikt i Renässansen, för att utforska hur ett konstinspirerat förhållningssätt kan öka vår förståelse av replikeringsprocesser i erfarna företag som är verksamma i starkt föränderliga branscher och vars strategi bygger på replikering. Det empiriska underlaget för avhandlingen är en djupgående fallstudie av den svenska modekedjan Hennes & Mauritz, och bygger på deltagande observation såväl i etablerade butiker som under öppningen av nya.

    • An algorithm for finite element analysis of problems in elastoplasticity with continuous stress and strain approximation is presented. By a global iteration procedure, equilibrium is preserved at the nodes in a weak sense, and the local constitutive relation between stresses and strains is satisfied. A high order numerical integration is used to achieve a good quality stiffness matrix and to evaluate the boundary between elastic and plastic regions in the case of partly plastic elements.

    • The industrial aim for shorter lead times in the development process for new products has encouraged a fast development of finite-element procedures for the simulation of sheet-metal forming. These procedures can shorten the design stage for new sheet-forming tools and the try-out period. In order to reach these goals robust calculation procedures and extensive verification of results are required. This paper presents a detailed validation of one promising type of finite-element code, namely the explicit code DYNA3D. Experiments for verification were performed on a zinc-coated sheet steel with a hot-dip galvanized coating. Stretch forming and deep drawing of cylindrical cups were performed. Rectangular boxes were formed from rectangular blanks and blanks with cut corners. Dies both with and without draw beads were used. Punch forces, flange draw-in and strain distributions were measured. The pressings with cylindrical shape were used to determine coefficients of friction with a fitting procedure based on comparison of data from pressings and from the DYNA3D calculations. These tribological data and constitutive data of the steels were used in the simulation of pressings of rectangular boxes. The calculations with DYNA3D gave a good description of flange draw-in and the strain distributions in the pressings. In most of the cases studied the punch-force curves were well reproduced. It is concluded that the present code is well suited for the simulation of sheet-forming operations.

  • Incidence and root causes of cancellations for elective orthopaedic procedures : a single center experience of 17,625 consecutive cases.

    • BACKGROUND: The purpose of the Swedish public health-care system is to provide care on equal terms for all citizens. In this, as in most other systems where taxes and/or insurances pay for most of the care, normal market forces are set aside at least in part. At times, this has, for example, resulted in long waiting lists, particularly in terms of elective orthopaedic surgery, with several negative consequences, such as cancellations of planned surgery.METHODS: The main purpose of this retrospective observational single center study was to evaluate and describe the number and reasons for cancellations in elective orthopaedic surgery. Studied were all the elective patients scheduled for joint replacement, arthroscopy and foot & ankle surgery, January 1, 2007 to December 31, 2011, whose procedure was cancelled at least once.RESULTS: Of all 17,625 patients scheduled for elective surgery 6,911 (39%) received at least one, some several cancellations. The most common reason for cancelling a planned surgery was different patient-related factors 3,293 (33%). Cancellations due to treatment guarantee legislation reached 2,885 (29%) and 1,181 (12%) of the cancellations were related to incomplete pre-operative preparation of the patients. Organisational reasons were the cause of approximately 869 (9%) of the cancellations.CONCLUSIONS: In this study of patients waiting for elective orthopaedic surgery 6,911(39%) had their surgical procedure cancelled at least once, some several times. It appears that it should be possible to eliminate many of these cancellations, while others are unavoidable or caused by factors outside the responsibility of the individual clinic or even hospital. One possible way of influencing the high rate of cancellations might be to change the view of the patients and involve them in the overall planning of the care process.

    • Pharmaceuticals are among factors that might be associated with temporomandibular disorders (TMDs), but knowledge about their utilization is limited. The purpose was to systematically register the regular use of medication in general among TMD patients and matched controls to enable comparisons to be made.Three hundred consecutive patients referred for diagnosis and treatment of TMDs and fulfilling the Research Diagnostic Criteria were examined prospectively and any medication recorded. Matched controls were registered parallel in time. The pharmaceuticals used were categorized according to the Anatomical Therapeutic Chemical Classification System (ATC).Forty-four percent of the patients received a main diagnosis of "muscle disorder", 39% "disk disorder", and 17% "joint disorder". Fifty-one percent of all patients used some medication on a regular basis compared to 36% of the controls (p<0.001). The average number of ATC categories used among all patients was 0.9 and among controls 0.5 (p<0.001). Of the female patients with the diagnosis "muscle disorder", 23% used antidepressants (N06A), 6% tranquilizers (N05B), and 7% sleep medication or sedatives (N05C) significantly more frequently than controls. Of the female patients diagnosed with a "joint disorder", 26% used antidepressants (N06A) significantly more frequently than controls. All other ATC categories differed non-significantly.The results suggest that the use of pharmaceuticals differs between patients and controls. TMD patients, particularly women diagnosed with "muscle" or "joint" disorders, appear to use drugs for depression more frequently than ordinary dental patients.

    • Users of digital media leave traces that corporations and authorities can harvest, systema-tise, and analyse; on the societal level, an overall result is the emergence of a surveillance culture. In this study, we examine how people handle the dilemma of leaving digital footprints: what they say they do to protect their privacy and what could legitimise the collection and storing of their data. Through a survey of almost 1,000 students at Umeå University in Sweden, we find that most respondents know that their data are used and choose to adjust their own behaviour rather than adopting technical solutions. In order to understand contemporary forms of surveillance, we call for a humanistic approach – an approach where hermeneutic and qualitative methods are central.

  • Applying a Systems Perspective on the Notion of the Smart City

    • This paper focuses on the need for a widened definition of the notion of technology within the smart city discourse, with a particular focus on the “built environment”. The first part of the paper describes how current tendencies in urban design and architecture are inclined to prioritize high tech-solutions at the expense of low-tech functionalities and omits that information and communication technology (ICT) contrasts the art of building cities as an adaptable and habitually smart technology in itself. It continues with an elaboration on the need for expanding the limits of system boundaries for a better understanding of the energy and material telecouplings that are linked to ICT solutions and account for some perils inherent in smart technologies, such as rebound effects and the difficulty of measuring the environmental impacts of ICT solutions on a city level. The second part of the paper highlights how low-tech technologies and nature-based solutions can make cities smarter, representing a new technology portfolio in national and international policies for safeguarding biodiversity and the delivery of a range of ecosystem services, promoting the necessary climate-change adaption that cities need to prioritize to confer resilience.

The paper elaborates on three types of media for urban designers to intervene in urban systems, including urban form, institutions, and discourse, that together function as a significant enabler of urban change. The paper ends by presenting four future research frontiers with a potential to advance the field of social-ecological urbanism: (1) urban density and critical biodiversity thresholds, (2) human and non-human movement in urban space, (3) the retrofitting of urban design, and (4) reversing the trend of urban ecological illiteracy through affordance designs that connect people with nature and with each other.

  • Patient-Specific Clavicle Reconstruction Using Digital Design and Additive Manufacturing

    • There is a trend toward operative treatment for certain types of clavicle fractures and these are usually treated with plate osteosynthesis. The subcutaneous location of the clavicle makes the plate fit important, but the clavicle has a complex shape, which varies greatly between individuals and hence standard plates often have a poor fit. Using computed tomography (CT) based design, the plate contour and screw positioning can be optimized to the actual case. A method for patient-specific plating using design based on CT-data, additive manufacturing (AM), and postprocessing was initially evaluated through three case studies, and the plate fit on the reduced fracture was tested during surgery (then replaced by commercial plates). In all three cases, the plates had an adequate fit on the reduced fracture. The time span from CT scan of the fracture to final implant was two days. An approach to achieve functional design and screw-hole positioning was initiated. These initial trials of patient-specific clavicle plating using AM indicate the potential for a smoother plate with optimized screw positioning. Further, the approach facilitates the surgeon's work and operating time can be saved.

    • OBJECTIVES:During long term follow-up of a cohort of patients with essential thrombocythemia (ET) and polycythemia vera (PV) a higher than expected incidence of myelofibrosis (MF) was noted. In order to test if the explanation could be found in the diagnostic criteria a re-evaluation of diagnosis using the 2008 WHO diagnostic criteria for ET and MF was performed.METHODS: This prospective study of 60 patients with ET and PV was set up in 1998 to evaluate the long-term efficacy and tolerability of anagrelide treatment. Bone marrow trephine biopsies were requested from study start, after 2 and 7 years of follow-up. A blinded re-evaluation of the bone marrow trephines was performed. The 2008 WHO bone marrow criteria were used for diagnosis and fibrosis grading.RESULTS: Of 40 patients with an initial diagnosis of ET, 21 were confirmed as 'true ET' whereas 17 were reclassified as primary myelofibrosis (PMF) (12 PMF-0, 3 PMF-1, 2 PMF-2) and 2 as myeloproliferative neoplasms of uncertain origin. After 7 years of follow-up, 19 of 21 patients with 'true ET' were alive, none had transformed to MF, leukemia, or myelodysplastic syndrome. In contrast, 4/17 patients reclassified as PMF had died, two patients transformed to myelodysplastic syndrome and 7 patients progressed to overt MF.DISCUSSION:We conclude that a blinded re-evaluation of bone marrow trephines from study start and after 7 years of follow-up using 2008 World Health Organization criteria was able to differentiate between true ET and PMF with a marked difference in follow-up outcome.

    • Introduction and hypothesis: A previous randomized controlled trial (RCT) demonstrated that the app Tät II, for self-management of mixed urinary incontinence (MUI) and urgency urinary incontinence (UUI), yielded significant, clinically relevant improvements in symptom severity and quality of life (QoL) compared with a control group. We aimed to assess the cost-effectiveness of Tät II.Methods: A cost–utility analysis with a 1-year societal perspective was carried out, comparing Tät II with an information app. Data were collected alongside an RCT: 122 community-dwelling women aged ≥18 years with MUI or UUI ≥2 times/week were randomized to 3 months of Tät II treatment focused on pelvic floor muscle training (PFMT) and bladder training (BT; n = 60), or to an information app (n = 62). Self-assessed data from validated questionnaires were collected at baseline and at 3-month and 1-year follow-ups. Costs for assessment, treatment delivery, incontinence aids, laundry, and time for PFMT and BT were included. We calculated quality-adjusted life-years (QALYs) using the International Consultation on Incontinence Modular Questionnaire Lower Urinary Tract Symptoms Quality of Life. The incremental cost-effectiveness ratio (ICER) between the groups was our primary outcome. Sensitivity analyses were performed.Results: The mean age was 58.3 (SD = 9.6) years. Annual overall costs were €738.42 in the treatment group and €605.82 in the control group; annual QALY gains were 0.0152 and 0.0037 respectively. The base case ICER was €11,770.52; ICERs in the sensitivity analyses ranged from €−9,303.78 to €22,307.67.Conclusions: The app Tät II is a cost-effective treatment method for women with MUI and UUI.

    • Over the recent decades, the possibilities to surveil people have increased and been refined with the ongoing digital transformation of society. Surveillance can now go in any direction, and various forms of online surveillance saturate most people’s lives, which are increasingly lived in digital environments.To understand this situation and nuance the contemporary discussions about surveillance – not least in the highly digitalised context of the Nordic countries – we must adopt cultural and ethical perspectives in studying people’s attitudes, motives, and behaviours. The “culture of surveillance”, to borrow David Lyon’s term, is a culture where questions about privacy and publicness, and rights and benefits, are once again brought to the fore.This anthology takes up this challenge, with contributions from a variety of disciplinary and theoretical frameworks that discuss and shed light on the complexity of contemporary surveillance and thus problematise power relations between the many actors involved in the development and performance of surveillance culture. The contributions highlight how more and more actors and practices play a part in our increasingly digitalised society.The book is an outcome of the research project "iAccept: Soft surveillance – between acceptance and resistance", financed by the Marcus and Amalia Wallenberg Foundation. The anthology’s editors are project members, all based at Umeå University, Sweden: Lars Samuelsson, associate professor of philosophy; Coppélie Cocq, professor of Sámi studies and digital humanities; Stefan Gelfgren, associate professor of sociology of religion; and Jesper Enbom, associate professor of media studies.

  • Urea clearance: a new method to register local changes in blood flow in rat skeletal muscle based on microdialysis

    • Pgreater thanIncreasing evidence suggests that local blood flow should be monitored during microdialysis (MD) as the recovery of analytes is affected by local blood flow. At present ethanol clearance is the standard technique for this purpose, but it is not functional at very low perfusion velocities. Here, we introduce a technique for MD whereby local tissue blood flow is recorded by the use of urea clearance (changes inflow/outflow concentration), in conjunction with measurements of tissue metabolism (glucose, lactate and puruvate). MD probes were inserted into the gracilis muscle of 15 rats and perfused with a medium containing urea (20 mmol l-1). Changes in muscle blood flow were made by addition of noradrenaline (5 mu g ml-1) to the perfusion medium at two perfusion velocities (0 center dot 6 and 0 center dot 4 mu l min-1). The clearance of urea from the perfusion medium was then calculated and examined in relation to the dose of noradrenaline and to the coexisting changes in extracellular metabolites. The results showed reproducible and dose-dependent changes in blood flow that were induced by noradrenaline. These were characterized by dose-dependent changes in the urea clearance as well as blood-flow-specific changes in the MD metabolic markers (reduction in glucose and increase in lactate). The sensitivity for blood flow changes as assessed by urea clearance (MD) was increased at 0 center dot 4 compared with the 0 center dot 6 mu l min-1 perfusion speed. The results indicate that inclusion of urea to the perfusion medium may be used to monitor changes in skeletal muscle blood flow at low perfusion velocities and in parallel assess metabolic variables with a high recovery (greater than 90%).

    • The contributions in this book shed light on the complexity of surveillance in a digital age and problematise power relations between the many actors involved in the development and performance of surveillance culture. More and more actors and practices play an increasing role in our contemporary digitalised society, and the chapters show how people negotiate surveillance in their use of digital media, often knowingly leaving digital footprints, and sometimes trying to avoid surveillance. The digital transformation will continue in the foreseeable future. The coordination and analysis of data is viewed by many government agencies, corporations, and other actors as important tools for improving public administration, health, and economic growth. For this development to be legitimate, it is important that hard values, such as technical and legal developments, and soft values, such as ethical and cultural values, are taken into consideration.

    • The possibilities to surveil people have increased and been further refined with the implementation of digital communication over the last couple of decades, and with the ongoing process of digital transformation, surveillance can now go in any direction, leaving a label such as “surveillance state” somewhat outdated. Corporations and governmental organisations may surveil people, people may surveil each other, and surveillance may take place in subtle ways that are difficult for the surveilled to detect. In David Lyon’s terms, we are living in a “culture of surveillance”, a culture that surrounds and affects our everyday life. Today, it is of utmost relevance to study people’s attitudes, motives, and behaviours in relation to the fact that we live in a culture of surveillance. This includes the need for cultural and ethical perspectives to understand and nuanced contemporary discussions on surveillance, not least in the highly digitalised context of the Nordic countries. The chapters in this anthology address these issues from a variety of disciplinary and theoretical frameworks.

    • Distributed generation, DG, introduces multi-generator grids and new modes of operation. DG will likely introduce power electronic, PE, converters on a large scale at the low- and medium-voltage levels. DC injection into the AC grid is a threat from grid-connected PE converters that are badly structured. Low DC currents in AC current are difficult to measure at those low levels decided in IEEE™ 1547 and discussed in other international working groups. Transformers and PE can be co-designed to eliminate DC injection. Modern adjustable-speed drives, ASDs, especially, those with active rectifiers, form a basis to discuss and solve issues of DC injection to AC grids from DG also named distributed energy resources, DER or even DR. DC components from power electronics, embedded in solar power, wind power, energy storage, smart houses, smart-office buildings, own generators, etc. give rise to grid-aspects like AC components give frequencies around the fundamental one. FMEA, failure mode and effects analysis, is judged to be a tool for further synthesis
